골수 이식 집단 중 약 40% 에서 침입성 진균 감염이 발병하였다 (Khan S. A., Wingard J. R., Natl. Cancer Inst. Monogr. 2001; 29, 31-36). 칸디다(Candida) 및 아스페르길루스(Aspergillus)종은 혈액암 및 골수 이식 환자에서 병원내 표면적이고 침입성인 진균증을 유발하는 가장 흔한 병원체이다. 상기 환자에서, 전신 칸디다증과 연관된 사망률은 매우 높다(50-90%).
실질 장기 이식과 관련하여, 감염성 합병증은 폐-이식 환자에서 더욱 종종 일어난다. 면역억제 섭생에 더해, 감수성 증가는 주로 외부 환경에 일정한 노출로 인한 것이다. 면역억제 치료 강도와 함께, 침입성 진균 감염은 외과 수술 후 첫째날 동안 일어날 수 있으며, 이의 빈도는 첫번째 2개월에 가장 높고 6개월 후에 감소되나, 또한 이식 수년 후에도 일어날 수 있다(Hamacher J. et al, Eur . Respir . J., 1999; 13, 180-186).

1. 화학적 합성

)퀴놀린-7-
카르복사미드
THF(5 mL) 중의 8-히드록시퀴놀린-7-카르복실산(100 mg, 0.53 mmol) 및 디(1H-이미다졸-1-일)메타논(86 mg, 0.53 mmol)의 혼합물을 60℃로 3h동안 질소 하에서 가열하였다. 반응 혼합물을 RT로 냉각시키고 피리딘-2-일메탄아민(46 mg, 0.424 mmol)을 첨가하였다. 산출된 혼합물을 40℃ 에서 2h 동안 가열한 후 RT 에서 교반하였다. 반응 혼합물을 H2O 및 탄산수소나트륨 포화 수용액으로 켄칭하고, DCM 으로 2회 농축하였다. 분리된 유기물을 Na2SO4 로 건조시키고, 여과하고 감압하에서 농축하였다. 잔류물을 SPE-SI 카트리지(2 g, DCM → DCM:MeOH 99:1)에 의해 정제하여, 표제 화합물(73 mg, 0.26 mmol)을 베이지색 고체로서 산출하였다.

2. 활성 시험: 방법 및 결과
항진균 활성을 시험하는데 사용된 유기체
트리코파이톤 루브룸(Tricophyton Rubrum)(ATCC 28188, PBI International);트리코파이톤 멘타그로파이테스(Tricophyton Mentagrophytes)(ATCC 9533, PBI International); 아스페르길루스 니게르(Aspergillus Niger)(ATCC 16404, PBI International); 스코풀라리옵시스 브레비카울리스(Scopulariopsis Brevicaulis)(ATCC 36840, DSMZ); 칸디다 알비칸스(Candida Albicans)(ATCC 90028, PBI International); 칸디다 글라브라타(Candida Glabrata)(ATCC 90030, DSMZ).
제조 및 전환율
균주를 동결-건조 앰플 또는 동결-건조 펠렛으로부터 제조하였다.
현탁액 단리물을 감자 덱스트로스 아가(Potato Dextrose Agar: PDA) 상에서 제조하여 균주 순도를 시험하였다. 이후 균주의 덩어리 성장물로 PDA 플레이트 상에 스트리킹 미생물 현탁액을 만들었다.
30℃ 에서 48-72 시간 동안(칸디다 효모) 및 7-10 일 동안(균사 진균) 인큐베이션을 하였다.
효모 콜로니 및 균사 진균 분생자를 3-5 mL 의 RPMI 1640 + 50% 글리세롤로 수확하고 분취액을 -80℃ 에서 동결시켰다.
항진균 감수성 시험
화합물의 최소 억제 농도(MIC)를 [National Committee for Clinical Laboratory Standards (NCCLS)]에서 동의를 받아 개발된 방법을 사용하는 브로스 마이크로-희석 감수성 시험을 통해 측정하였다(National Committee for Clinical Laboratory Standards. Reference Method for Broth Dilution Antifungal Susceptibility Testing of Yeasts; Approved standard-Second Edition M27-A2. 2002; Vol. 22, No. 15) (National Committee for Clinical Laboratory Standards. Reference Method for Broth Dilution Antifungal Susceptibility Testing of Filamentous Fungi; Approved standard M38-A. 2002; Vol. 22, No. 16).
0.165M 3-(N-모르폴리노)프로판술폰산(MOPS) 및 10M NaOH 로 pH 7로 완충되고 18 g 글루코스/리터가 보충된 L-글루타민 배지가 있는 RPMI 1640에서 어세이를 실시하였다. 96 웰 멸균 플레이트(접종물 크기: 1 x 105 CFU/mL)를 사용하여 시험을 수행하였다. 화합물 저장액을 100% DMSO 중 12.8 mg/mL 로 제조하였다. 연속 2 배 희석액을 RPMI 1640를 사용하는 플레이트에서 제조하였다. 최종 농도는 1% DMSO 에서 0.125 내지 128 ㎍/mL 에 달했다.
MIC는 임의의 가시적인 성장을 방지하는 항진균제의 최저 농도로서 정의되고, 효모의 경우 48h의 인큐베이션(35℃) 및 균사 진균의 경우 5일의 인큐베이션 (35℃) 후에 측정하였다.
결과
2개의 단일 실험에서 수득된 값의 기하 평균으로서 계산된 가장 바람직한 화합물에 대한 MIC 값이 표 3에 보고된다.

게다가, EP1669348A1호에서 E8로서 분류된 화합물은 EP1669348A1호에 기재된 화합물이나 본 발명에 포함되지 않는, 본 출원에 기재된 화합물의 계열과 EP1669348A1호에 기재된 화합물 사이의 연결로서 사용될 수 있는 신규 화합물(NiK-29298로서 분류됨)과 함께 합성되었다(표 4).

인지될 수 있듯이, 표 3에 나열된 모든 화합물은 효모, 피부사상균 및 곰팡이를 비롯한 시험된 모든 6개의 균주에 대해 활성이다. 본 발명의 화합물의 상기 넓은 스펙트럼은 대부분 피부사상균에 의해 유발되는 피부, 두피 및 손톱 감염; 대부분 효모에 의해 유발되는 질, 구강 및 장내 감염; 대부분 곰팡이에 의해 유발되는 귀, 폐, 눈 및 기타 전신 감염을 비롯하여 인간에서 또는 동물에서 모든 종류의 진균 감염에 대해 예상된 효능이 충분하였다.
반대로, EP1669348A1호에 기재된 화합물 E8, 및 EP1669348A1호에 기재된 동일한 퀴놀린 스캐폴드를 특징으로 하는 화합물 NiK-29298은, 오로지 효모에 대해서만 활성이었고, 피부사상균 및 곰팡이를 비롯한 다른 균주에 대해서는 임의의 뚜렷한 활성을 나타내지 않는다.
작용
메카니즘
가장 강력하고 넓은 스펙트럼의 항진균제 중 하나인 시클로피록스가 Fe3 + 을 킬레이팅 함으로써, 즉 진균 세포로부터 철 이온을 빼냄으로써 진균 세포를 치사시키며, 이의 시험관 내 활성은 적절한 양의 Fe3+ 이온을 매질에 첨가함으로써만 억제된다는 것이 당업계에 알려져 있다. 시클로피록스는 또한 기이한 작용 메카니즘으로 인해 진균 균주에서 내성을 유도하지 않는 유일한 항진균제인 것으로 당업계에 알려져 있다.

화합물의 작용 매카니즘이 철 이온의 킬레이션인 것인지를 입증하기 위해, 칸디다 글라브라타(Candida glabrata)(ATCC 90030) 균주로의 MIC측정을 시험 배지에 과량의 철 이온(100 μM FeCl3)을 첨가함으로써 수행하였다. 약물에 노출된 세포의 생존력을 금속 이온 Fe3 + 의 유무에 따라, 540 nm 에서의 OD 측정에 의해 평가하였다.
실시예 3, 4, 11 12 에 기재된 화합물, E8 및 NiK-29298의 효능을 100 μM (100 마이크로몰) Fe3 +의 존재 및 부재하에 칸디다 글라브라타(Candida glabrata)에서 평가하였다.
결과를 하기 도 1, 2 및 3 에 보고한다.
모든 도면에서, 선 및 점은 상이한 농도의 항진균제(가로 좌표)를 첨가함에 따른 진균 성장의 % 억제(세로 좌표)를 나타낸다. 청색 선 및 점은 이온 보충 없이 수행된 실험인 반면, 적색 선 및 점은 100 μM Fe3 +의 존재하에 수행된 실험 결과를 나타낸다. 당업계에 공지된 바와 같이, 시클로피록스 효과는 Fe3 + 의 존재에 의해 완전히 억제되고, 칸디다 글라브라타(Candida glabrata)는 정상적으로 성장할 수 있다(도 1). 반대로, Fe3 + 는 시클로피록스와 상이한 작용 메카니즘을 갖는다고 당업계에 공지된 항진균제인 암포테리신에 대해서는 효과가 없다.
본 발명의 모든 화합물은 시클로피록스와 유사한 거동을 갖는다, 즉 이들의 항진균 활성은 Fe3 + 의 존재에 의해 완전히 억제된다(도 2).
반대로, EP1669348A1호에 기재된 화합물 E8, 및 시클로피록스와 달리 그리고 본 발명의 화합물과 달리, EP1669348A1호에 기재된 퀴놀론 스캐폴드를 갖는 화합물 NiK-29298은 배지 배양물 내 Fe3 + 이온의 존재에 의해 억제되지 않는다.
요약하면, EP1669348A1호에 기재된 화합물은 효모에 제한되는 좁은 작용 스펙트럼을 갖는 반면, 이들은 피부사상균 또는 곰팡이에 대항해 항진균 활성을 나타내지 않는다. 게다가, 이들의 작용 메카니즘은 철 킬레이션에 대해 의존적이다.
