Claims (19)
- 환자에게로의 가스의 전달을 제어하는 방법에 있어서,
생리학적 파라미터를 모니터링하는 단계;
지정 압력으로 가스를 전달하는 단계;
생리학적 파라미터로부터 각성의 시작을 판단하는 단계;
각성의 시작이 상기도 저항(upper airway resistance) 또는 가스 전달에 의해 야기되는지를 판단하는 단계; 그리고
각성이 상기도 저항 또는 가스 전달에 의한 것인지 아닌지에 기초하여, 전달되는 가스의 압력을 조절하는 단계
를 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
각성에 대한 임계 레벨을 결정하기 위하여 강제 진동 방법을 사용하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
호흡 노력 관련 각성(Respiratory Effort Related Arousal, RERA)의 시작을 판단하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
폐쇄성 수면 무호흡-저호흡(obstructive sleep apnea-hypopnea)의 시작을 판단하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
모니터링되는 복수의 서로 다른 생리학적 파라미터에 대한 값들의 세트에 기초하여 사건의 시작을 정의하는 복수의 성분(entry)을 갖는 테이블을 제공하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
모니터링되는 복수의 생리학적 파라미터들 중 하나 이상으로부터 유래된 지수 값(index value)에 기초하여, 전달되는 가스의 압력 레벨을 조절하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
수면 품질을 판단하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
체인-스토크스형(cheyne-stokes) 호흡을 판단하는 단계를 추가로 포함하는 단계를 포함하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
모니터링된 파라미터들로부터의 복수의 값을 사건과 연계시키는 테이블을 제공하고, 환자의 민감도(sentivity)를 수용하도록 상기 테이블을 업데이트하는 단계를 추가로 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
상기 판단하는 단계는, 각성 또는 수면 호흡 장애를 판단하기 위하여 공기 흐름 파형(airflow waveform) 또는 압력 파형의 고대역폭 모니터링을 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
상기 판단하는 단계는, 맥파 전달 시간, 혈압, 및 ECG 신호를 분석하여 각성의 시작을 판단하는 단계를 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
상기 판단하는 단계는, 주기적인 호흡 사건을 판단하기 위하여 환자의 호흡을 분석하는 단계를 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 제 1 항에 있어서,
구강 호흡의 존재를 판단하는 단계를 추가로 포함하고, 상기 조절하는 단계는 구강 호흡을 보상하는 단계를 포함하는 것을 특징으로 하는, 환자에게로의 가스의 전달을 제어하는 방법. - 가스 전달 장치에 있어서,
복수의 생리학적 파라미터를 모니터링하기 위한 복수의 센서;
조절 가능한 압력 레벨을 갖는 가스 전달 장치; 그리고
상기 센서 및 가스 전달 장치와 협력하는 제어기, 여기서 상기 제어기는, 호흡 사건 관련 각성(respiratory event related arousal, RERA) 및 치료 사건 관련 각성(treatment event related arousal, TERA)으로부터 유래하는 각성의 시작을 판단하도록 그리고 이에 따라 가스 전달 장치의 가스 레벨을 조절하도록 적응됨,
를 포함하는 것을 특징으로 하는, 가스 전달 장치. - 제 14 항에 있어서,
상기 가스 전달 장치는 지속적 비강 양압(Continuous Positive Air Pressure, CPAP) 머신인 것을 특징으로 하는, 가스 전달 장치. - 제 14 항에 있어서,
상기 가스 전달 장치는 환기 장치(ventilator)인 것을 특징으로 하는, 가스 전달 장치. - 제 14 항에 있어서,
상기 가스 전달 장치는 산소 집중기(oxygen concentrator)인 것을 특징으로 하는, 가스 전달 장치. - 제 14 항에 있어서,
상기 제어기는 또한, 각성의 시작을 나타내는 환자-특정적 생리학적 파라미터를 획득 및 저장하도록 적응되는 것을 특징으로 하는, 가스 전달 장치. - 제 14 항에 있어서,
상기 복수의 센서는, 무선 기술을 이용하여 제어기와 통신하는 것을 특징으로 하는, 가스 전달 장치.
