Abstract
본 발명은 혈관 투과성, 특히 VPF/VEGF와 관련하여 증가된 혈관 투과성, 및 수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염/지연형 과민증으로 나타나는 피부 부종을 특히 β-동위효소 선택성 PKC 억제제, (S)-3,4-[N,N'-1,1'-((2"-에톡시) -3"'(O)-4"'-(N,N-디메틸아미노)-부탄)-비스-(3,3'-인돌릴)]-1(H)-피롤-2,5-디온 또는 그의 제약학상 허용가능한 염을 사용하여 저하 또는 억제시키는 방법을 기재하고 있다.
Description
본 출원은 참고로 본 명세서에 포함되는, 1997년 4월 30일에 출원된, 동시 계류중인 가출원 제60/044,431호의 권리를 청구한다.
수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염 및 알레르기성 피부 질환과 같은 피부 질환은 피부 부종이 나타나는 특징이 있다. 수포성 유천포창은 대개 노인에게서 나타나며, 피부 및 점막 표면의 일부 또는 전부에 영향을 미칠 수 있다. 포진성 피부염은 사지의 신근 표면, 및 두피, 둔부 및 등에 대칭적으로 분포되어 무리를 이루고 있는 소포들이 심하게 가려움증을 일으키는 특징이 있다.
다형홍반 증후군은 감염물 (허피스바이러스 호미니스, 마이코플라스마 뉴모니애(Mycoplasma pneumoniae)) 및 약물 (특히, 페니실린, 해열제, 바르비투르산염, 히단토인 및 술폰아미드)을 비롯한, 가능성 있는 여러가지 병인과 관련된 피부 및 점액막의 특징적인 반응이다. 환자 중 50%는 병인이 전혀 확인되지 않는다. 다형홍반은 혈관 주변에서 심각한 림프구조직구의 염증성 침윤이 생기는 것이 주요한 병적 변화이며, 모세혈관의 내피 세포에서의 변질성 변화 및 두드러진 유두상의 피부 부종을 포함할 수 있다. 이런 병변은 대칭 분포로 발생하는 특징이 있으며, 이 증후군 또한 심각한 독혈증 및 쇠약, 고열, 기침 및 폐의 "반점" 염증을 동반할 수 있다.
접촉성 피부염/지연형 과민증은 자극 물질(sensitizing agent)에 몇시간 내지 몇일 동안 노출되었을 때 붉은 경결이 생기는 피부 병변을 일으키는 특징이 있는 피부 질환이다. 덩굴옻나무가 이러한 피부 질환을 일으키는 흔한 자극 물질 중 하나이다.
이러한 피부 질환은 혈관 투과성항진 및 피부 부종과 관련이 있다 [Macvicar D. 외, 1963; J. Invest Dermatol, 41:289; Pierard J. 및 Whimster I., 1961; Br J. Dermatol, 73:253]. 외부 미소혈관계의 투과성항진은 접촉성 피부염/지연형 과민증의 공통적인 특징이다 [Voisin 외, Ann. Inst. Pasteur., 104:169, 1963]. 최근 연구에 의해 피부 부종의 형성 및 표피하 수포 형성에 VPF/VEGF가 관여한다는 것이 발견되었다 [Brown L. 외, 1995; J. Invest Dermatol 104:744]. 최근 조사 결과, 접촉성 피부염/지연형 과민증 반응을 일으키는 실험 모델의 단핵세포/대식세포 및 각질세포에 VPF/VEGF이 존재한다는 것이 증명되었다 [Brown 외, J. Immunol., 154:2801, 1995]. VEGF의 양은 상기에서 설명된 질환이 나타나는 피부 병변에서 상당히 증가한다. 특히, 상기 언급된 피부 병변이 있는 환자들은 이런 병변의 수포에 VEGF의 양이 매우 많았다. 이런 수포성 질환을 치료하기 위해 코르티코스테로이드, 및 술폰 또는 술파피리딘 요법과 같은 치료법이 수년간에 걸쳐 개발되었다. 그러나, 피부 병변 치료를 위한 신규한 치료제, 구체적으로 VPF/VEGF에 의해 자극된 혈관 투과성을 타켓으로 하는 치료제에 대한 개발이 이 분야에서 여전히 요구되고 있다.
<발명의 개요>
본 발명의 목적은 피부 부종의 치료 방법을 제공하는 것이다.
본 발명의 또 다른 목적은 피부 부종과 관련된 VPF/VEGF에 의해 자극된 혈관 투과성을 억제하는 방법을 제공하는 것이다.
본 발명의 상기 목적 및 다른 목적들이 하기에서 설명되는 하나 이상의 실시태양에 의해 제공된다.
본 발명의 하나의 실시태양에서는, 단백질 키나제 C 억제제의 치료 유효량을 피부 부종의 치료를 필요로하는 포유류에게 투여하는 것을 포함하는, 피부 부종의 치료 방법을 제공한다.
본 발명의 또 다른 실시태양에서는, 단백질 키나제 C 억제제의 치료 유효량을 피부 부종의 치료를 필요로하는 포유류에게 투여하는 것을 포함하는, 피부 부종과 관련된 VPF/VEGF에 의해 자극된 혈관 투과성을 저하 또는 억제하는 방법을 제공한다.
본 발명은 VPF/VEGF에 의해 유도되는 피부 부종과 관련된 혈관 투과성을 저하 또는 억제하는 것과 같은 치료 방법을 제공한다. 이 방법은 수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염/지연형 과민증 및 알레르기성 피부 질환을 치료하는데 효과적이다.
본 발명은 광범위하게는 피부 부종 (예, 경결)과 관련된 혈관 투과성, 구체적으로는 혈관 투과 인자(VPF)/혈관 내피 증식 인자(VEGF)에 의해 유도되는 혈관 투과성 증가를 억제 또는 저하시키는 방법에 관한 것이다. 본 발명은 특히 단백질 키나제 C(PKC) 억제제, 구체적으로 수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염/지연형 과민증 및 알레르기성 피부 질환 치료를 위한 특정 종류의 동위효소 선택성 PKC 억제제의 용도에 관한 것이다.
본 발명은 PKC 억제제, 특히 특정 종류의 단백질 키나제 C 억제제의 사용이 피부 부종, 특히 VPF/VEGF에 의해 유도되는 혈관 투과성 증가를 저하 또는 억제시킨다는 것을 발견하였다. 따라서, 이런 화합물을 피부 부종, 예를 들어, 수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염 및 알레르기성 피부 질환과 관련된 피부 질환을 치료하기 위해 치료 목적으로 사용할 수 있다.
본 발명의 방법은 바람직하게 β 동위효소를 효과적으로 억제하는 단백질 키나제 C 억제제를 이용한다. 적합한 하나의 화합물 군으로는 비스-인돌릴말레이미드 또는 거대 고리형 비스-인돌릴말레이미드가 당업계에 일반적으로 알려져 있다. 당업계에 잘 알려져 있는 비스-인돌릴말레이미드는 참고로 본 명세서에 포함되는 미국 특허 제5,621,098호, 동 제5,552,396호, 동 제5,545,636호, 동 제5,481,003호, 동 제5,491,242호 및 동 제5,057,614호에 기재된 화합물을 포함한다. 거대 고리형 비스-인돌릴말레이미드는 특히 화학식 I의 화합물로 나타낸다. 이들 화합물 및 그의 제조 방법은 참고로 본 명세서에 포함되는 미국 특허 제5,552,396호에 기재되어 있다. 이런 화합물들은 혈관 투과성을 저하 또는 억제하기 위해, 특히 VPF/VEGF에 의해 자극된 혈관 투과성을 저하 또는 억제하기 위해 포유류에게 치료 유효량 (VPF/VEGF 억제량)으로 투여된다. 특히, 이 화합물들은 혈관 투과성과 관련된 피부 부종, 예를 들어, 수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염/지연형 과민증 및 알레르기성 피부 질환을 치료하는데 사용할 수 있다.
본 발명의 방법에 사용되는 화합물의 바람직한 종류에는 하기 화학식 I의 화합물, 또는 그의 제약학상 허용가능한 염, 프로드럭 또는 에스테르가 있다.

본 발명에 사용하기 위한 보다 바람직한 화합물의 종류는 -X-W-Y- 잔기가 치환 또는 비치환될 수 있는 4 내지 8개의 원자를 갖는 화학식 I로 나타내어진다. -X-W-Y- 잔기가 6개의 원자를 갖는 화합물이 가장 바람직하다.
본 발명의 방법에 사용하기 위한 다른 바람직한 화합물은 R1및 R2가 수소이고, W는 치환된 알킬렌, -O-, -S-, -CONH-, -NHCO- 또는 -NR3-인 화학식 I의 화합물이다. 본 발명에서 사용하기 위한 특히 바람직한 화합물은 하기 화학식 Ia의 화합물, 또는 그의 제약학상 허용가능한 염, 프로드럭 또는 에스테르이다.
식 중, Z는 -(CH2)p- 또는 -(CH2)p-O-(CH2)p-이고; R4는 히드록시, -SH, C1-C4알킬, (CH2)m아릴, -NH(아릴), -N(CH3)(CF3), -NH(CF3) 또는 -NR5R6이고; R5는 수소 또는 C1-C4알킬이고; R6은 수소, C1-C4알킬 또는 벤질이고; p는 0, 1 또는 2이고; m은 독립적으로 2 또는 3이다. Z가 CH2이고; R4가 -NH2, -NH(CF3) 또는 -N(CH3)2인 화학식 Ia의 화합물이 가장 바람직한 화합물이다.
본 발명의 방법에서 사용하기 위한 다른 바람직한 화합물은 화학식 I에서의 W가 -O-이고, Y가 치환된 알킬렌이고, X가 알킬렌인 화합물이다. 이런 바람직한 화합물, 또는 그의 제약학상 허용가능한 염, 프로드럭 또는 에스테르는 하기 화학식 Ib에 의해 나타내어진다.
식 중, Z는 -(CH2)p-이고; R4는 -NR5R6, -NH(CF3) 또는 -N(CH3)(CF3)이고; R5및 R6은 독립적으로 H 또는 C1-C4알킬이고; p는 0, 1 또는 2이고; m은 독립적으로 2 또는 3이다. p가 1이고; R5및 R6이 메틸인 화학식 Ib의 화합물이 가장 바람직하다.
화학식 I, Ia 및 Ib의 화합물들이 염기성 잔기를 함유하고 있기 때문에, 이들은 제약학상 허용가능한 산부가염으로 존재할 수도 있다. 이런 염을 형성하는데 보통 사용되는 산은 염산, 브롬화수소산, 요오드화수소산, 황산 및 인산과 같은 무기산 뿐만 아니라 파라-톨루엔술폰산, 메탄술폰산, 옥살산, 파라-브로모페닐술폰산, 카르본산, 숙신산, 시트르산, 벤조산, 아세트산과 같은 유기산, 및 관련된 무기산 및 유기산이 포함된다. 따라서, 이러한 제약학상 허용가능한 염으로는 술페이트, 피로술페이트, 비술페이트, 술파이트, 비술파이트, 포스페이트, 일수소포스페이트, 이수소포스페이트, 메타포스페이트, 피로포스페이트, 클로라이드, 브로마이드, 요오다이드, 아세테이트, 프로피오네이트, 데카노에이트, 카프릴레이트, 아크릴레이트, 포르메이트, 이소부티레이트, 헵타노에이트, 프로피오레이트, 옥살레이트, 말로네이트, 숙시네이트, 수베레이트, 세바케이트, 푸마레이트, 말레에이트, 2-부틴-1,4-디오에이트, 3-헥신-2,5-디오에이트, 벤조에이트, 클로로벤조에이트, 히드록시벤조에이트, 메톡시벤조에이트, 프탈레이트, 크실렌술포네이트, 페닐아세테이트, 페닐프로피오네이트, 페닐부티레이트, 시트레이트, 락테이트, 히푸레이트, β-히드록시부티레이트, 글리콜레이트, 말레에이트, 타르트레이트, 메탄술포네이트, 프로판술포네이트, 나프탈렌-1-술포네이트, 나프탈렌-2-술포네이트, 만델레이트 등이 포함된다. 특히 염산 및 메실레이트 염이 사용된다. 메실레이트 염은 (참고로 본 명세서에 포함된) 미국 특허 제5,710,145에 기재되어 있다.
제약학상 허용가능한 염 이외에, 다른 염들도 존재할 수 있다. 이들은 화합물을 정제하는데, 다른 염들을 제조하는데, 또는 화합물 또는 중간체들을 확인 및 특성화하는데 중간체로서 기능할 수 있다.
화학식 I, Ia 및 Ib의 화합물의 제약학상 허용가능한 염은 물, 메탄올, 에탄올, 디메틸포름아미드, 에틸 아세테이트 등과 같은 여러 용매화물로 존재할 수도 있다. 이런 용매화물의 혼합물도 제조할 수 있다. 이런 용매화물은 원래 제조 또는 결정에서 함유된 결정화 용매이거나 그외에 첨가된 결정화 용매일 수 있다.
화학식 I, Ia 및 Ib의 화합물의 여러가지 입체 이성질체 형태가 존재할 수 있는데, 예를 들어, W가 치환된 알킬렌 잔기에서 비대칭 탄소 원자를 함유할 수 있다는 것을 알 수 있다. 화합물들은 보통 라세미체로서 제조되고, 편리하게는 라세미체로 사용할 수 있다. 별법으로, 두개의 개별 거울상 이성질체를 원한다면 통상의 기술에 의해 분리하거나 합성할 수 있다. 이러한 라세미체 및 개별 거울상 이성질체 및 그의 혼합물들은 본 발명의 방법에 사용된 화합물의 일부이다.
본 발명에서 이용할 수 있는 화합물에는 화학식 I, Ia 및 Ib의 화합물의 제약학상 허용가능한 프로드럭이 포함된다. 프로드럭은 화학적으로 개질되고, 그의 작용 부위에서 생리학상 불활성일 수 있지만, 1종 이상의 효소 또는 다른 생체내 프로세스에 의해 원래의 생활성 형태로 분해 또는 개질될 수 있는 약물이다. 이런 프로드럭은 원래의 형태에 비해 점막 상피 전체에서 보다 용이하게 흡수될 수 있고, 염 형성 또는 용해도가 보다 우수하고(거나), 전신계의 안정성이 개선된 (예를 들어, 플라스마 반감기가 증가됨) 원래의 형태와는 상이한 약물속도론적 프로파일을 가질 수 있다. 통상적으로, 이런 화학적 개질로는 다음이 포함된다:

3) 프로드럭 형태 또는 개질된 프로드럭 형태의 멤브레인 선택을 통해 활성 부위에서 축적되는 유도체; 또는 상기 1 내지 3의 임의의 조합. 적합한 프로드럭 유도체의 선택 및 제조에 대한 통상의 방법은 문헌 [H. Bundgaard, Design of Prodrugs, (1985)]에 기재되어 있다.
여러가지 비스-인돌-N-말레이미드 유도체의 합성법은 데이비스(Davis) 등의 미국 특허 제5,057,614호에 기재되어 있고, 본 발명에 사용하는데 적합한 바람직한 화합물의 합성법은 참고로 본 명세서에 포함되는 앞에서 명시한 미국 특허 제5,552,396호 및 파울(Faul) 등의 유럽 특허 제0 657 411 A1호에 기재되어 있다.
본 발명의 방법에 사용하기 위한 특히 바람직한 단백질 키나제 C 억제제는 상기 언급한 미국 특허 제5,552,396호의 실시예 5g에 기재되어 있는 화합물 ((S)-3,4-[N,N'-1,1'-((2"-에톡시)-3"'(O)-4"'-(N,N-디메틸아미노)-부탄)-비스-(3,3'-인돌릴)]-1(H)-피롤-2,5-디온 히드로클로라이드 염)이다. 이 화합물은 잠재적인 단백질 키나제 C 억제제이다. 이 화합물은 다른 키나제에 비해 단백질 키나제 C에 대해 선택적이며, 동위효소, 즉, 베타-1 및 베타-2-동위효소에 상당히 선택적이다. 본 발명의 다른 염들도 이로울 수 있고, 메실레이트염이 특히 이롭다.
하기 화학식 II의 화합물을 비활성 유기 용매, 바람직하게는 유기/물 혼합물, 가장 바람직하게는 물-아세톤 중에서 메탄술폰산과 반응시켜 바람직한 메실레이트 염을 제조할 수 있다.
메탄올, 아세톤, 에틸아세테이트 및 그의 혼합물과 같은 다른 용매들도 사용 가능하다. 용매 대 물의 비는 중요하지 않으며, 일반적으로는 시약의 용해도에 의해 결정된다. 용매 대 물의 비는 일반적으로 0.1:1 내지 100:1의 부피비가 바람직하다. 1:1 내지 20:1의 비가 바람직하고, 5:1 내지 10:1의 비가 가장 바람직하다. 최적의 비는 선택된 용매에 좌우되며 9:1의 용매:물의 비로 아세톤을 사용하는 것이 바람직하다.
반응은 보통 두가지 시약이 다른 비율인, 특히 메탄술폰산이 과량인 경우도 작용하지만 대략 등몰량으로 포함된다. 메탄술폰산의 첨가 속도는 반응에 중요하지 않으며, 6시간 이상에 걸쳐 빠르게 (5분 미만) 또는 느리게 첨가할 수 있다. 반응은 0 ℃ 내지 환류 온도의 범위에서 수행된다. 반응 혼합물은 x-선 분말 회절에 의해 측정하여 염이 완전히 형성될 때까지 교반하고, 5분 내지 12시간이 걸릴 수 있다.
본 발명의 염은 결정질 형태로 바람직하고 용이하게 제조된다. 염의 삼수화 형태는 건조시 또는 20 내지 60%의 상대 습도에 노출시 일수화물로 쉽게 전환될 수 있다. 염은 실질적으로 정해진 융점, 복굴절 및 x-선 회절 패턴을 나타내는 결정질이다. 일반적으로, 결정은 10% 미만, 바람직하게는 5% 미만, 가장 바람직하게는 1% 미만의 비결정질 고체를 갖는다.
메실레이트 염은 당업계에 알려져 있는 여과 또는 다른 분리 기술에 의해 반응 혼합물로부터 직접 분리되어 50 내지 100% 범위의 수율로 얻어진다. 원하다면 당업계에 알려져 있는 재결정 및 다른 정제 기술을 사용하여 염을 더 정제시킬 수 있다.
출원인들은 VPF/VEGF이 PKC, 특히 PKC-β의 활성화를 통해 피부 부종과 관련된 혈관 투과성 증가를 유발한다고 생각한다. PKC-β는 또한 자극 물질에 응답하여 단핵세포에 의한 항원제시에 필요하다. 따라서, PKC 억제제, 특히 PKC-β 억제제를 피부 부종, 특히 VPF/VEGF에 의해 자극되거나 또는 유도된 세포 및(또는) 혈관 투과성의 증가와 관련된 피부 부종을 저하 또는 억제하는데 사용할 수 있다.
본 발명에 의해 확인된 화합물들을 피부 부종과 관련된 여러가지 피부 질환의 임상적인 증상을 치료하는데 사용할 수 있다. 피부 부종은 체액이 혈관 공간으로부터 추출되어 수포를 형성할 수 있는 피부의 간질성 공간에 축적되는 것이 특징이다. 소포 형성은 일반적으로 체액이 담긴 국소적으로 부풀어 오른 소포 및 수포의 2가지 주요 형태로 나타난다.
피부 부종은 특정 박테리아성 및 바이러스성 감염, 알레르기성 피부 질환 (예, 혈관신경성 부종 및 아토피성 피부염), 기계적, 열적 또는 화학적 원인으로 인한 피부염, 그리고 특히 중요한 천포창과 같이 원인이 알려지지 않은 수포성 질환과 같은 여러가지 피부 질환의 주요한 특징이다. 특히, 피부 부종은 여러가지 자극 물질에 의해 유도되는 수포성 유천포창, 다형홍반, 포진성 피부염, 접촉성 피부염/지연형 과민증 및 알레르기성 피부 질환에서 발견되는 주된 징후이다. 상기에서 설명된 모든 피부 질환은 본 발명의 화합물로 치료할 수 있다.
당업계의 숙련자들은 본 발명의 단백질 키나제 C 억제제의 치료 유효량 (VPF/VEGF 억제량)이 혈관 투과성, 혈관 투과성과 관련된 VPF/VEGF 활성, 또는 피부 부종을 억제하는데 충분한 양이고, 이 양은 특히 피부 병변의 크기 및 유형, 치료 제제 중의 화합물의 농도 및 환자의 상태에 따라 달라진다는 것을 알 것이다. 생체내 시험이나 생체외 시험 모두를 사용하여 혈관 투과성 및 피부 부종을 억제하는데 필요한 화합물의 양을 결정할 수 있다. 예를 들어, 본 발명에 의해 제공된 화합물을 투여할 때, 형광물질, 혈관계로부터 피부로 누출되는 트리판 블루, 또는 당업계에 공지되어 있는 임의의 방법에 의해 혈관 투과성을 측정할 수 있고, 피부 부종은 숙련된 의사의 능력 범위 내에서 임상 시험에 의해 쉽게 알 수 있다. 본 발명의 화합물의 유효성을 평가하기 위해 접촉성 피부염에 걸린 설치류 모델 [Brown 외, J. Immunol, 154:2801, 1995]을 사용할 수 있다. 본 발명의 화합물의 유효성을 시험하기 위해 사람에게 덩굴옻나무의 추출물인 우루쉬올을 사용하여 피부 부종의 크기 (예, 병변의 경결)를 측정할 수도 있다. 일반적으로, 피부 부종과 관련된 피부 병변을 치료하기 위한 치료제로서 투여되는 단백질 키나제 C 억제제의 양은 의사에 의해 사례별로 결정될 것이다. 적정 투여량을 정할 때, 병변 정도, 환자의 체중 및 연령을 기준으로 고려한다.
일반적으로, 적정 투여량은 치료 부위에서 단백질 키나제 C 억제제의 농도가 0.5 nM 내지 200 μM, 보다 일반적으로 20 nM 내지 80 nM의 범위가 되는 양이다. 20 nM 내지 80 nM의 혈청 농도가 대부분의 경우에 충분할 것으로 기대된다.
이러한 치료 농도를 얻기 위해, 치료를 필요로하는 환자에게 하루에 체중 1 kg 당 약 0.1 mg 내지 1.5 mg를 투여한다. 일반적으로, 하루에 체중 1 kg 당 약 1.0 mg 이하의 단백질 키나제 C 억제제가 필요할 것이다. 상기에서 언급한 대로, 상기의 양은 사례별로 달라질 수 있다.
본 발명은 또한 피부 질환의 심화를 예방하거나 억제하기 위해 본 명세서에 기재되어 있는 화합물의 예방적인 용도를 고려한다. 예방을 위한 투여량은 통상적으로 활동성 질환을 치료하는데 필요한 범위의 최소량이 적합하다.
화학식 I의 화합물, 및 화학식 Ia 및 Ib의 바람직한 화합물은 투여 전에 배합하는 것이 바람직하다. 적당한 제약 제제는 쉽게 이용할 수 있고 공지되어 있는 성분을 사용하여 공지된 방법에 의해 제조된다. 본 발명의 방법에 사용하기에 적합한 조성물의 제조에서, 활성 성분은 보통 담체와 혼합하거나, 담체에 의해 희석하거나, 캡슐, 사젯, 종이 또는 다른 용기 형태일 수 있는 담체 내에 봉해진다. 담체가 희석제로 기능할 경우, 이는 활성 성분에 대해 비히클, 부형제 또는 매질로 작용하는 고상, 반고상 또는 액상 물질일 수 있다. 따라서, 조성물들은 경구용 또는 국소 도포용에 있어서 정제, 환제, 입제, 로젠지, 사젯, 카젯, 엘릭시르제, 현탁제, 유제, 액제, 시럽제, (고상으로 또는 액상 매질 중의) 에어로졸, 연질 및 경질 젤라틴 캡슐제, 좌약제, 무균 주사용 용액제 및 무균 포장된 분말제의 형태일 수 있다.
적당한 담체, 부형제 및 희석제의 몇몇 예로는 락토오스, 덱스트로스, 수크로스 소르비톨, 만니톨, 전분, 아카시아 검, 인산 칼슘, 알기네이트, 트라가칸트, 젤라틴, 칼슘 실리케이트, 미세결정질 셀룰로오스, 폴리비닐피롤리돈, 셀룰로오스, 물 시럽, 메틸 셀룰로오스, 메틸 및 프로필히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 및 광유가 포함된다. 제제는 추가적으로 윤활제, 습윤제, 유화제, 침강방지제, 보존제, 감미제 또는 향미제를 포함할 수 있다. 본 발명의 조성물은 환자에게 투여된 후 활성 성분이 빠르게, 지속적으로 또는 지연되어 방출되도록 제형할 수 있다. 조성물은 활성 성분을 약 0.05 mg 내지 약 3 g, 보다 통상적으로 약 64 mg을 함유하는 단위 투여 형태로 제형되는 것이 바람직하다. 그러나, 투여되는 치료상 투여량은 치료될 질환의 심각도, 투여될 화합물의 선택 및 투여의 선택 경로를 비롯하여 관련 상황에 비추어 의사에 의해 결정될 것이라는 것은 당연하다. 따라서, 상기 투여 범위는 어떤 식으로도 본 발명의 범위를 제한하려는 것은 아니다. "단위 투여 형태"라는 말은 원하는 치료 효과를 주기 위해 계산된 활성 물질의 소정의 양을 적합한 제약 담체와 함유하는, 사람 대상자 및 다른 포유류에게 있어서 단위 투여 형태로 적당한 물리적으로 분리된 단위를 말한다.
상기 제형 이외에도, 본 발명의 방법에 사용된 화합물들 대부분은 경구로 투여될 수 있고, 가장 흔하게는 국소적으로 도포된다. 국소 제형은 연고제, 크림제 및 겔제를 포함한다.
연고제는 일반적으로 (1) 유성 염기, 즉, 백색 페트로라툼 또는 광유와 같은 지방유 또는 탄화수소로 구성된 염기, 또는 (2) 흡수성 염기, 즉, 물을 흡수할 수 있는 물질, 예를 들어 무수 라놀린으로 구성된 염기를 사용하여 제조된다. 통상적으로, 유성 또는 흡수성 염기를 형성한 후, 활성 성분을 원하는 농도가 되는 양으로 첨가한다.
크림제는 오일/물 유제이다. 이들은 통상적으로 지방유, 탄화수소 등을 포함하는, 왁스, 페트로라툼, 광유 등과 같은 오일층 (내부층)과 물 및 임의의 수용성 물질을 포함하는, 부가염과 같은 수성층 (연속층)으로 이루어진다. 두 층은 유화제, 예를 들면 나트륨 라우릴 술페이트와 같은 계면활성제, 아카시아 콜로이드성 클레이, 비검(veegum) 등과 같은 친수성 콜로이드를 사용함으로써 안정화된다. 유제 형성시, 활성 성분 (화합물)은 통상적으로 원하는 농도를 달성하는 양으로 첨가한다.
겔은 유성 염기, 물, 또는 유제-현탁제 염기 중에서 선택된 염기를 포함한다. 이 염기에, 점도를 증가시키면서 염기 중에서 매트릭스를 형성하는 겔화제를 첨가한다. 겔화제의 예로는 히드록시프로필 셀룰로오스, 아크릴산 중합체 등이 있다. 통상적으로, 활성 성분 (화합물)을 원하는 농도로 겔화제를 첨가하는 이전의 시점에서 배합물에 첨가한다.
국소 제형에 혼입되는 화합물의 양은 중요하지 않지만, 농도는 원하는 치료 부위에 원하는 양의 화합물이 전달되는 양으로 질환을 일으킨 조직 면적에 배합물이 용이하게 도포되기에 충분한 범위이어야 한다.
질환을 일으킨 조직에 도포될 국소 제제의 통상적인 양은 질환을 일으킨 조직 크기 및 제제 중에서의 화합물의 농도에 좌우된다. 일반적으로, 제제는 질환을 일으킨 조직의 cm2당 약 1 내지 약 500 ㎍을 얻을 수 있는 양으로 도포된다. 바람직하게, 화합물의 도포량은 약 30 내지 약 300 ㎍/cm2, 보다 바람직하게는 약 50 내지 약 200 ㎍/cm2, 가장 바람직하게는 약 60 내지 약 100 ㎍/cm2의 범위이다.
하기의 제형예는 본 발명을 설명하려는 것이지 본 발명의 범주를 어떤 식으로도 제한하려는 것은 아니다.
<제형 1>
경질 젤라틴 캡슐은 하기의 성분들을 사용하여 제조된다:

총량 460 mg
상기 성분들을 혼합하여 460 mg 양의 경질 젤라틴 캡슐에 충전하였다.
<제형 2>
정제는 하기의 성분들을 사용하여 제조된다:

미세결정질 셀룰로오스 400
발연 이산화 실리콘 10
스테아르산 5
총량 665 mg
성분들을 혼합하고, 압축하여 665 mg 중량의 정제를 형성하였다.
<제형 3>
활성 성분 60 mg을 각각 함유하는 정제를 하기와 같이 제조한다:

총량 150 mg
활성 성분, 전분 및 셀룰로오스를 No. 45 메쉬 U.S. 시브로 통과시켜 완전히 혼합했다. 이 결과 생긴 분말을 폴리비닐피롤리돈 용액과 혼합하고, No 14 메쉬 U.S. 시브로 통과시켰다. 이렇게 제조된 과립을 50 ℃에서 건조하고, No. 18 메쉬 U.S. 시브로 통과시켰다. No. 60 메쉬 U.S.을 미리 통과시킨 나트륨 카르복시메틸 전분, 마그네슘 스테아레이트 및 탈크를 과립에 첨가하고, 혼합한 후 정제기에서 압축하여 150 mg의 정제를 얻었다.
본 발명의 원리, 바람직한 실시태양 및 작업 형태는 상기 명세서에서 설명하였다. 그러나, 본원에서 보호 받고자하는 발명은 기재된 특정 형태로 제한되지 않는데, 이는 특정 형태들이 제한을 위한 것이 아니라 설명을 위한 것으로 해석되어야 하기 때문이다. 본 발명의 범주를 벗어나지 않고 당업계의 숙련자들에 의해 여러 변형 및 변화가 만들어 질 수 있다.
Claims (14)
- 단백질 키나제 C 억제제의 치료 유효량을, 피부 부종을 치료할 필요가 있는 포유류에게 투여하는 것을 포함하는, VPF/VEGF에 의해 자극된 혈관 투과성을 저하시킴으로써 피부 부종을 치료하는 방법.
- 제1항에 있어서, 단백질 키나제 C 억제제가 동위효소 선택성인 것인 방법.
- 제2항에 있어서, 단백질 키나제 C 억제제가 단백질 키나제 C의 β 동위효소를 선택적으로 억제하는 것인 방법.
- 제3항에 있어서, 단백질 키나제 C 억제제가 하기 화학식 I의 화합물 또는 그의 산염인 방법.<화학식 I>식 중, W는 -O-, -S-, -SO-, -SO2-, -CO-, C2-C6알킬렌, 치환된 알킬렌, C2-C6알케닐렌, -아릴-, -아릴(CH2)mO-, -헤테로사이클-, -헤테로사이클-(CH2)mO-, -융합된 비시클릭-, -융합된 비시클릭-(CH2)mO-, -NR3-, -NOR3-, -CONH- 또는 -NHCO-이고;X 및 Y는 독립적으로 C1-C4알킬렌, 치환된 알킬렌이거나, 또는 X, Y 및 W가 함께 -(CH2)n-AA-를 형성하고;R1은 수소이거나, 또는 할로, C1-C4알킬, 히드록시, C1-C4알콕시, 할로알킬, 니트로, NR4R5또는 -NHCO(C1-C4알킬) 중에서 독립적으로 선택된 4개 이하의 임의의 치환체이고;R2는 수소, CH3CO-, NH2또는 히드록시이고;R3은 수소, (CH2)m아릴, C1-C4알킬, -COO(C1-C4알킬), -CONR4R5, -(C=NH)NH2, -SO(C1-C4알킬), -SO2(NR4R5) 또는 -SO2(C1-C4알킬)이고;R4및 R5는 독립적으로 수소, C1-C4알킬, 페닐, 벤질이거나, 또는 이들이 결합된 질소 원자와 함께 포화 또는 불포화 5원 또는 6원 고리를 형성하고;AA는 아미노산 잔기이고;m은 독립적으로 0, 1, 2 또는 3이고;n은 독립적으로 2, 3, 4 또는 5이다.

- 단백질 키나제 C 억제제의 치료 유효량을, 수포 형성을 치료할 필요가 있는 포유류에게 투여하는 단계를 포함하는, VPF/VEGF에 의해 자극된 혈관 투과성을 저하시킴으로써 수포 형성을 치료하는 방법.
