Abstract

본 발명은 일반적으로 과증식성 전립선 질환을 치료하는 방법 및 특히, 이들 질환의 과증식성 세포 활성을 억제하고 세포 분화를 촉진시키기 위한 활성 형태의 비타민 D의 용도에 관한 것이다.
전립선은 유일하게 수컷 포유동물에서 발견되고 특정 과증식성 질환에 잘 걸린다. 전립선의 기저 세포 및 간질 세포의 증식은 일반적인 전립선 질환으로서 양성 전립선 과형성증을 일으킨다. 또 다른 일반적인 전립선 질환은 전립선 암, 특히 전립선 선암이다. 전립선의 선암은 가장 일반적인 치명적인 병리학적 전립선 암이고, 전형적으로 전립선의 말초 영역내 상피 세포의 악성 형질전환을 초래한다. 전립선 과형성증 및 전립선 암 모두는 노화된 사람 남성 집단에서 고비율로 발병한다. 대략적으로 55세 이상의 4명의 남성중 한 사람은 임의의 형태 또는 또 다른 형태의 전립선 질환을 앓는다.

발명의 간단한 설명

본 발명의 전술한 장점 및 기타 장점은 이의 한 양태에서, 분자의 측쇄상의 C-24 위치에서 치환되는 탄화수소 잔기를 갖는 1α-하이드록시비타민 D 화합물의 유효량으로 세포를 치료함을 포함하여, 사람 종양성 세포 또는 과형성성 세포의 과증식 활성을 억제하는 방법에서 실현된다. 치료 단계는 전립선 세포에서 증식의 억제 및 분화의 유도 및 향상을 포함한다.
1α-하이드록시비타민 D 화합물은 활성 비타민 D이고 적합하게는 하기에 기술되는 화학식 I로서 나타낸다. 화학식 I의 화합물중에서 바람직한 화합물은 1α,24-디하이드록시비타민 D2, 1α,24-디하이드록시비타민 D4, 1α,25-디하이드록시비타민 D4, 1α,25-디하이드록시비타민 D2, 1α-하이드록시비타민 D2및 1α-하이드록시비타민 D4이다.
본 발명에 따라 필요한 환자에게 투여될 수 있는 1α-하이드록시비타민 D 화합물의 치료학적 유효량은 1일 기준으로 체중 kg당 0.01㎍/㎏/일 내지 2.0㎍/㎏일이다.

본 발명은 신생물성 및 과형성성 질환을 치료하는 효과적인 방법을 제공한다. 특히, 본 발명은 전립선, 예를 들면 전립선 암 및 전립선 과형성증의 과증식성 세포의 활성을 억제하고, 개선시키거나 완화시키고, 질환에 걸린 세포내의 세포 분화를 유발하고 증진시키거나 촉진시키는 치료적 방법에 관한 것이다. 본 발명은 분자의 측쇄의 C-24 위치에서 치환된 탄화수소 잔기를 갖는 활성 비타민 D 동족체를 사용하여 전립선 암 또는 전립선 과형성증과 같은 과증식성 질환으로 고통받는 환자의 신규한 치료법을 제공하는 것이다. 바람직하게는, 활성 비타민 D 동족체는 1α-하이드록시비타민 D 화합물이고, 적합하게는 하기 기술된 바와 같은 화학식 Ⅰ의 화합물로 나타낸다. 활성 비타민 D 동족체는 용량 제한 과칼슘혈증 및 과칼슘뇨증을 일으키지 않고, 즉 혈중 및 뇨중 각각의 칼슘 수준이 비생리학적으로 높고 해롭지 않게 환자에게 제공된다. 이러한 속성은 기술된 화학식 Ⅰ의 화합물의 특정 화학적 특성을 통해 달성된다.
본 발명에 따라서, 유효량의 화학식 Ⅰ의 동족체를 전립선 암 또는 전립선 과형성증에 걸린 환자에게 투여하는 경우, 동량의 활성화 비타민 D3이 이미 공지된 제형으로 투여된 후 보다 현저하게 낮은 과칼슘혈증 및 과칼슘뇨증을 나타내면서, 비정상적인 전립선 세포의 증식 활성은 억제되거나 완화되고, 세포 분화가 유도되고 촉진되거나 증진된다. 따라서, 화학식 Ⅰ의 화합물은 활성 형태의 비타민 D3동족체에 비해 개선된 치료 지수를 갖는다.
비타민 D3은 활성화되기 전에, 즉, 생물학적 반응을 나타내기 전에 C-1 및 C-25 위치에서 하이드록실화되어야만 한다는 것은 공지되어 있다. 유사한 대사는 기타 형태의 비타민 D, 예를 들면, 비타민 D2및 비타민 D4를 활성화시키는데 필요한 것으로 나타난다. 그러므로, 본원에서 사용된 바와 같이 "활성화 비타민 D"또는 "활성 비타민 D"란 용어는 적어도 분자의 A 환의 C-1 위치에서 하이드록실화되어 있고, 프로드럭, 예를 들면 1α-하이드록실비타민 D2의 경우에 화합물 자체 또는 이의 대사산물도 비타민 D 수용체(VDR)와 결합하는 비타민 D 화합물 또는 동족체를 의미한다. 본원에서 사용된 바와 같이, "탄화수소 잔기"란 용어는 저급 알킬, 저급 알케닐, 저급 아실 그룹 또는 저급 사이클로알킬, 즉 직쇄 또는 측쇄, 포화 또는 불포화 C1-C4탄화수소 라디칼을 의미한다.
본 발명에 따른 화합물은, 이러한 화합물이 C-24 위치에서 탄화수소 잔기, 예를 들면, C-24 위치에서의 저급 알킬, 알케닐 또는 아실 그룹을 갖는 경우, 활성 비타민 D 화합물이다. 추가로, 본 발명에 따른 활성 비타민 D는, 예를 들면 적합하게는 C-22 및 C-23 사이, C-25 및 C-26 사이 또는 C-26 및 C-27 사이에서 이중 결합을 갖는 불포화 측쇄를 가질 수 있다.

본 발명의 화학식 Ⅰ의 1α-하이드록시비타민 D는 특히, 전립선 질환, 예를 들면, 전립선 암 및 전립선 과형성증의 세포와 관련해서, 효과적인 증식억제 및 세포 분화 활성(즉, 악성 형질전환의 반전)을 갖지만, 과칼슘혈증 및/또는 과칼슘뇨증의 바람직하지 않은 부작용을 일으키는 경향성이 낮거나 일으킬 수 없는 화합물이다. 환언하면, 화학식 Ⅰ의 화합물은 칼슘 대사에서 현저한 변화없이 악성 또는 기타 과증식성 세포에 노출되는 경우, 증식 억제제 및 세포 분화제로서 작용하도록 하는 복용량으로 투여될 수 있다. 작용에 대한 이러한 선택성 및 특이성으로 인해 화학식 Ⅰ의 1α-하이드록시비타민 D 화합물은 과형성을 안전하게 억제하고 악성 또는 과형성 세포 분화를 촉진하기 위한 유용하고 바람직한 제제이다. 따라서, 본 발명의 1α-하이드록시비타민 D 화합물은 상기 기술한 공지된 활성 비타민 D3화합물의 단점을 극복하고 양성 전립선 과형성증 뿐만 아니라 전립선 암과 같은 악성 질환을 조절하고 치료하는 바람직한 제제로 간주될 수 있다.
화학식 1의 활성 비타민 D 화합물중 바람직한 것은 1α,24-디하이드록시비타민 D2, 1α,24-디하이드록시비타민 D4, 1α,25-디하이드록시비타민 D2, 1α,25-디하이드록시비타민 D4, 1α-하이드록시비타민 D2, 및 1α-하이드록시비타민 D4이다. 측쇄, 예를 들면 C-24에서 키랄 중심을 갖는 화학식 Ⅰ의 화합물 중에서, 두 에피머(예: R 및 S) 및 라세미체 혼합물은 본 발명의 범주내에 포함된다.
따라서, 본 발명은 유효량의 화학식 Ⅰ의 화합물을 사용하여 기타 과증식성 전립선 세포 뿐만 아니라 악성 전립선 세포를 치료하는 방법(즉, 이들의 과증식성 활성을 억제하고/하거나 이들의 분화를 유도하고 증진시키는 것)을 제공한다. 유효 투여량은 1일 기준으로 환자 체중 kg 당 약 0.01㎍/㎏/일 내지 약 2.0㎍/㎏/일 범위이다.
화학식 Ⅰ의 화합물은 전립선 암 및 전립선 과형성증을 겪고 있는 환자에게 있어서 전립선 암 및 전립선 과형성증의 치료에 유용하다. 특히, 본 발명은 전립선 암 및 전립선 과형성증의 과증식 세포 효과로부터 고통받는 환자에게 치료학적으로 유효량의 화학식 Ⅰ의 화합물, 바람직하게는 1α,24-디하이드록시비타민 D2, 1α,24-디하이드록시비타민 D4, 1α,25-디하이드록시비타민 D2, 1α,25-디하이드록시비타민 D4, 1α-하이드록시비타민 D2, 및 1α-하이드록시비타민 D4를 투여함으로써 이러한 환자를 치료하는 방법에 관한 것이다.
화학식 Ⅰ의 화합물은 예를 들면, 본원에서 참조로 인용된 문헌[미국 특허 제5,448,120호(Knutson et al.), 미국 특허 제4,554,106호(DeLuca et al.) 및 Strugnell et al., 310 Biochem. J. (1995) pp. 233-241]에 기술된 바와 같이 제조할 수 있다.
화학식 Ⅰ의 화합물의 생잠능은 비타민 D의 활성 호르몬 형태이며 측정되는 모든 비타민 D 화합물 및 동족체에 대해 표준 형태인, 1α,25-디하이드록시비타민 D3의 생효능과 비교 연구되었다. 예를 들면, 화학식 Ⅰ의 화합물 또는 이의 활성 대사산물의 비타민 D 수용체(VDR) 결합 친화도는 실질적으로 1α,25-디하이드록시비타민 D3의 친화도와 동일함(즉, 동일하거나 3배 미만 약하다)이 밝혀졌다. 이러한 수용체 결합 친화도는 잠재적인 생물학적 활성의 지표이다.
동시에, 화학식 Ⅰ의 화합물은 이들의 상응하는 비타민 D3동족체 보다 현저하게 독성이 낮다고 밝혀졌다. 예를 들면, 본원에서 참조로 인용된 동시 계류중인 특허원 제08/265,438호에서 1α-하이드록시비타민 D4에 대한 LD50은 남성에 대해 1.0㎎/㎏, 여성에서 3.0㎎/㎏, 즉 실질적으로 1α-하이드록시비타민 D3(LD500.2㎎/㎏)보다 독성이 낮다고 밝혀졌다. 또한, 본원에서 참조로 인용한 모출원인 미국 특허 제5,403,831호 및 모출원의 모출원인 미국 특허 제5,104,864호 둘 모두에서는, 1α-하이드록시비타민 D2가 1α-하이드록시비타민 D3및 1α,25-디하이드록시비타민 D3과 동일한 생잠능을 갖지만 훨씬 독성이 낮다는 것을 나타낸다. 폐경기 후의 골다공증을 가진 여성에게 1α-하이드록시비타민 D210㎍/일 이하의 복용량으로도 가벼운 과칼슘뇨증(U.Ca 〉 300㎎/24시간)만을 일으키고, 1α-하이드록시비타민 D2만으로 인한 현저한 과칼슘혈증(S. Ca 〉 11.0㎎/dL)은 전혀 나타나지 않는다. 부가적으로, 화합물은 크레아티닌 제거 및 BUN에 의해 측정된 바와 같이, 신장 기능에 역 효과를 주지 않고, 하이드록시프롤린의 뇨중 배출을 증가시키지 않으며, 이는 뼈 재흡수에 대한 어떠한 자극 효과도 없음을 나타낸다. 건강한 성인 남성에게 1α-하이드록시비타민 D2를 8㎍/일 이하의 복용량으로 투여하면 임상적으로 어떠한 현저한 과칼슘혈증 또는 기타 부작용도 전혀 나타내지 않는다.
화학식 Ⅰ의 화합물은 비타민 D3의 활성 형태의 공지된 동족체에 비해 부작용이 감소되고 독성이 낮은 약제학적 조성물내의 활성 화합물로서 유용하다.
본 발명의 약제학적 활성 화합물은 환자, 예를 들면 사람을 포함한 포유류에게 투여하기 위한 의약 제제를 제조하는 약학의 통상적인 방법에 따라서 가공될 수 있다. 예를 들면, 화학식 Ⅰ의 화합물은 통상의 부형제, 예를 들면, 활성 물질과 해롭게 반응하지 않는 장(예: 경구) 또는 비경구 적용에 적합한 약제학적으로 허용되는 담체 물질과의 혼합물로 사용될 수 있다.
적합한 약제학적으로 허용되는 담체는 물, 염 용액, 알콜, 아라비아 검, 식물성 오일(예: 옥수수 오일, 면화씨 오일, 낙화생유, 올리브 오일, 코코넛 오일), 어류 간 오일, 오일성 에스테르(예: 폴리솔베이트 80), 폴리에틸렌 글리콜, 젤라틴, 탄수화물(예: 락토오스, 아밀로스 또는 전분), 마그네슘 스테아레이트, 활석, 규산, 점성 파라핀, 지방산 모노글라세라이드 및 디글리세라이드, 펜타에리스리톨 지방산 에스테르, 하이드록시 메틸셀룰로스, 폴리비닐 피롤리돈 등을 포함하나 이제 제한되지 않는다.
약제학적 제제는 살균될 수 있고, 경우에 따라, 보조제, 예를 들면, 윤활제, 보존제, 안정화제, 습윤제, 유화제, 삼투압에 영향을 주는 염, 완충제, 착색제, 향미제 및/또는 하나 이상의 기타 활성제와의 혼합할 수 있다.
비경구 적용을 위해서는, 주사가능한 멸균 용액, 바람직하게는 오일성 또는 수성 용액, 및 현탁액, 유액 또는 좌제를 포함하는 이식체가 특히 적합하다.
장내 적용을 위해서는, 정제, 당의정제, 액제, 점적제, 함당정제, 산제 또는 캅셀제가 특히 적합하다. 시럽제, 엘릭서제 등은 감미 비히클이 요구되는 경우 사용될 수 있다.
직장 투여를 위해서는, 화합물이 카카오 오일 또는 기타 트리글리세라이드 등의 좌제 기재를 함유하는 약제학적 조성물로 제형화된다. 저장 수명을 연장시키기 위해, 조성물은 유리하게는 아스코브산, 부틸화 하이드록시아니솔 또는 하이드로퀴논 등의 항산화제를 포함한다.
본 발명의 약제학적 조성물은 경구 투여가 바람직하다. 본 발명에 따라 전립선 암 또는 과형성증의 치료를 위한 화합물의 복용량은 일반적으로 약 0.01 내지 약 2.0㎍/㎏/일, 바람직하게는 약 0.01 내지 약 1.0㎍/㎏/일이다. 일반적으로, 본 발명의 화합물은 약제학적으로 허용되는 담체내에 단위 복용형으로 분배된다.
전립선 암 치료에 있어서, 화학식 Ⅰ의 화합물의 비경구 복용량은 약 0.01㎍/㎏/일 내지 약 1.0㎍/㎏/일이다.
특정 경우에 활성 화합물의 실질적인 바람직한 양은 사용되는 특정 화합물, 특히 제형화된 조성물의 효능, 적용 양식, 및 특히 치료하고자 하는 위치 및 기관에 따라 다양할 것이다. 예를 들면, 특정 환자에 대한 특정 용량은 연령, 체중, 일반적 건강 상태, 식이, 투여 시간 및 형태, 분비 속도 및 함께 사용되는 약물 및 치료가 적용되는 특정 질환의 중증도에 따라 좌우된다. 숙주에 대한 복용량은 통상의 방법을 사용하여, 예를 들면 적합한 통상의 약리학적 프로토콜에 의해, 주요 화합물 및 공지된 물질의 상이한 활성을 통상적으로 비교하여 측정할 수 있다.
에스트로겐(예: 디에틸스틸베스트롤), LHRH 동족체, 5α-리덕타제 효소 억제제(예: 피나스테라이드), 항에스트로겐제(예: 타목시펜(TamoxifenTM)) 및 항안드로겐제(예: 플루타미드)와 같은 공지된 안드로겐 조절 또는 제거 또는 테스토스테론 농도 강하제와 함께 화학식 I의 화합물의 공투여도 또한 본 발명의 범위에 속한다. 예를 들어, 본원에 참고로 인용된 문헌 참조[미국 특허 제5,372,996호]. 공생(symbiotic) 효과는 상기한 각종 배합물로 수득할 수 있고, 증가된 치료학적 효과를 제공할 것으로 예상된다. 또한, 상기한 제제 중 일부로 인한 부작용, 예를 들어 에스트로겐의 유해한 심장혈관 효과가, 상기한 제제를 다량의 투여량으로 단독으로 사용하는 경우에 비하여 상당히 감소되는 치료법을 제공하는 잠재력이 있다. 상기한 공투여되는 안드로겐 조절 또는 테스토스테론 농도 강하제의 가능한 투여 범위는 약 0.01 내지 0.20㎍/㎏/일이다.
또한, 특히 호르몬 치료 이후, 재발하는 전이성 전립선 암에 2차 항암제(예: 세포독성제)와 함께 화학식 I의 활성 비타민 D의 공투여도 또한 본 발명의 범위내에 포함된다. 이러한 제제는 적합하게는 에스트라뮤스틴 포스페이트, 프레드니뮤스틴, 시스플라틴, 5-플루오로-우라실, 멜팔란, 하이드록시우레아, 미토마이신, 이다루비신, 메토트렉세이트, 아드리아마이신 및 다우노마이신을 포함할 수 있다. 각종 항암성 약물과 함께 배합하여 사용되는 화학식 I의 활성 비타민 D가 종양성 세포에 상당히 증가된 세포독성 효과를 일으켜, 치료학적 효과를 증가시킬 수 있다는 것이 예상된다. 구체적으로, 상당히 증가된 성장 억제 효과가, 약물이 단독으로 사용되는 치료 요법에 비하여 저농도의 항암제를 사용하는 상기한 배합물로 수득되기 때문에, 항암제 약물에 관련된 부작용이 일반적으로 다량의 투여량으로 항암제 약물을 단독으로 사용하는 경우 관찰되는 것보다 상당히 감소되는 치료법을 제공하는 잠재력이 있다. 이러한 공투여되는 2차 항암제의 가능한 투여 범위는 약 0.1 내지 1㎍/㎏/일이다.

VDR 결합 분석
실시예 1: 1α,24-디하이드록시비타민 D2[1α,24-(OH)2D2]
포유동물 비타민 D 수용체(VDR)에 대한 1α,24-(OH)2D2의 친화도를, 미국 미네소타주 스틸워터 소재의 인크스타(Incstar)사가 시판중인 소 흉선 VDR 키트와 표준 1,25-(OH)2D3용액을 사용하여 검정한다. 화학적으로 합성된 1α,24-(OH)2D2의 반-최대 결합은 약 150pg/㎖인 반면, 1α,25-(OH)2D3의 반-최대 결합은 80pg/㎖이다. 따라서, 1α,24-(OH)2D2는 1α,25-(OH)2D3과 소 흉선 VDR에 대해 매우 유사한 친화도를 가지며, 이는 1α,24-(OH)2D2가 잠재적인 생물학적 활성을 갖는다는 것을 나타낸다.
실시예 2: 1α,24-디하이드록시비타민 D4[1α,24-(OH)2D4]
1α,24-(OH)2D4의 VDR 친화도 결합을 조사한다. 1α,24-(OH)2D4를 비타민 D 수용체와 방사선 표지된 트레이서 1α,25-(OH)2D3과 함께 항온 처리한다. 항온 처리 후, 수용체와 결합된 방사능의 양을 측정하고 표지되지 않은 1α,25-(OH)2D3와 표지된 1α,25-(OH)2D3의 공-항온 처리 후 결합된 양과 비교한다. 1α,24-(OH)2D450pg/튜브가 1α,25-(OH)2D3약 20pg/튜브와 동등한 것으로 밝혀졌다.
이러한 결과는 1α,24-(OH)2D4와 비타민 D 수용체와의 결합이 1α,25-(OH)2D3와 비타민 D 수용체와의 결합보다 덜 견고함을 나타낸다. 이러한 데이터는 1α,24-(OH)2D4가 1α,25-(OH)2D3과 유사하게, VDR에 대한 높은 친화도를 갖고 유의한 생물학적 활성을 갖는다는 것을 의미한다. 이러한 데이터는 1α,24-(OH)2D4를 사용한 유전자 발현 연구(후술함)와 일치하며 이는 1α,24-(OH)2D4가 1α,25-(OH)2D3보다 약간 덜 활성임을 나타낸다.
이러한 결과는 선행 분야로 비추어 볼 때 놀라운 것이고 예상치 못한 것이다. 이는 비타민 D4화합물의 생물학적 활성이 매우 낮다는 비타민 D에 대한 일반적인 지식과는 상반된 것이다.
실시예 3: 1α,24-디하이드록시비타민 D2[1α,24-(OH)2D2]
전립선 세포에 의한 비타민 D 화합물의 VDR 결합은 본원 명세서에서 참조문헌으로 인용되는 문헌[136 Endocrinology (1995) 20-26]에 기술된 스코브론스키 (Skowronski)의 방법을 사용하여 설명할 수 있다. 전립선 유도된 세포주를 거의 컨플루언스(confluence) 때까지 배양하여 세척하고 스크레이핑하여 수거한다. 세포들을 원심분리하여 세척하고, 세포 펠렛을 프로테아제 억제제를 함유하는 완충 염 용액 속에 재현탁시킨다. 세포들을 얼음 위에서 냉각시키면서 초음파로 파쇄시킨다. 파쇄된 세포들을 4℃에서 207,000 × g에서 35분 동안 원심분리하여 수득한 상등액을 결합에 대해 분석한다. 가용성 추출물(1 내지 2 mg 단백질/ ml 상등액) 200㎕를 4℃에서 16 내지 20시간 동안 1nM3H-1α,25-(OH)2D3및 1α,24-(OH)2D2의 증가된 농도(0.01 내지 100nM)와 함께 항온 처리한다. 표준 공정을 사용하여 하이드록실아파타이트를 사용하여 결합된 호르몬과 유리된 호르몬을 분리한다. 측정된 총 결합으로부터 250배 과량의 비방사성 1α,25-(OH)2D3의 존재하에 수득된 비특이적인 결합을 빼서 특이적인 결합을 산출한다. 이의 결과로부터 1α,24-(OH)2D2가 전립선 VDR에 대해 강한 친화도를 갖는 것을 알 수 있으며, 이는 1α,24-(OH)2D2가 전립선 세포에 대하여 잠재적인 생물학적 활성을 가짐을 의미하는 것이다.
실시예 4: 1α,24-디하이드록시 비타민 D4[1α,24-(OH)2D4]

실시예 7: 1α,24(S)-디하이드록시비타민 D2및 1α,24(R)-디하이드록시비타민 D2[1α,24(S)-(OH)2D2및 1α,24(R)-(OH)2D2]
실시예 6에서 언급한 유전자 발현 연구를 수행하여 화학적으로 합성된 1α,24(S)-(OH)2D2및 1α,24(R)-(OH)2D2의 시험관내 생물학적 활성을 1α,25-(OH)2D3및 25-OH-D3과 비교하여 수행된다. 비타민 D 의존성 전사 활성화 모델 시스템이 사용되며, 이때, 플라스미드 pSG-hVDR1/3 및 p(CT4)4TKGH가 녹색 원숭이 신장, COS-1 세포내로 공-형질감염된다.
형질감염된 세포는 비타민 D 대사산물과 함께 배양하며, 성장 호르몬 생성을 측정한다. 표 3에 나타낸 바와 같이, 1α,24(S)-(OH)2D2및 이의 에피머인 1α,24(R)-(OH)2D2모두는 이 시스템에서 25-OH-D3보다 현저하게 활성이 높으며, 1α,24(S)-(OH)2D2는 1α,25-(OH)2D3와 활성도가 거의 동일하다.

전립선 세포 증식의 억제
실시예 8: 1α,24-디하이드록시비타민 D2[1α,24-(OH)2D2]
세포 증식의 억제는 본 명세서에 참고로 인용되어 있는 문헌[참조: Skowronski et al., 132 Endocrinology (1993) 1962-1960 and 136 Endocrinology (1995) 20-26]의 기술을 사용하여 입증된다. 사람 전립선 선암으로부터 유도된 세포주, LNCaP 및 PC-3은 약 50,000 세포/플레이트의 밀도하에 6-웰 조직 배양 플레이트(six-well tissue culture plates)에서 시딩한다. 세포를 부착시키고 안정화시킨 후, 즉 약 2 내지 3일 후, 배지를 10-11M 내지 10-7M의 농도로 비히클 또는 활성 비타민 D 동족체 1α,24-(OH)2D2를 함유하는 배지로 교체한다. 시험 동족체 또는 비히클 함유 배지를 3일 마다 교체한다. 6 내지 7일 후, 배지를 제거하고 세포를 세정한 다음, 차게 한 5% 트리클로로아세트산으로 침전시키고, 차게 한 에탄올로 세척한다. 세포를 0.2N 수산화나트륨으로 용해시키고, DNA의 양을 표준 방법으로 측정한다. 결과를 살펴보면, 본 발명에 따른 1α,24-(OH)2D2로 배양한 배양물은 대조군 배양물에 비해 훨씬 적은 수의 세포를 갖는다.

전립선 세포 분화의 자극
실시예 11: 1α,24-디하이드록시비타민 D2[1α,24-(OH)2D2]
본원에 참고로 인용되어 있는 두 문헌[Skowronski et al., 132 Endocrinology(1993) 1952-1960 및 136 Endocrinology(1995) 20-26]의 기술을 사용하여, 사람 전이성 전립선 선암으로부터 유도되고 PSA를 발현하는 것으로 공지되어 있는 세포주 LNCaP의 세포를 6-웰 조직 배양 플레이트에 약 50,000세포/플레이트의 농도로 시딩한다.
세포를 부착시키고 안정화시킨 후, 약 2 내지 3일 후, 배지를 비히클 또는 활성 비타민 D 동족체인 1α,24-(OH)2D2를 10-11내지 10-7M의 농도로 함유하는 배지로 교체한다. 6 내지 7일 후, 배지를 제거하고 전립선의 특이적 항원(PSA) 분석을 위해서 -20℃에서 저장한다.
대응하는 배양액으로부터의 세포를 세정 및 침전시키고, 표준 방법으로 DNA 양을 측정한다. PSA는 공지된 표준 방법으로 측정한다. 1α,24-(OH)2D2로 배양한 배양액은 PSA/세포의 양으로 표현하면, 대조군 배양액보다 상당히 더 많은 PSA를 포함한다.
실시예 12: 1α,24-디하이드록시비타민 D4[1α,24-(OH)2D4]

실시예 13: 1α,25-디하이드록시비타민 D4[1α,24-(OH)2D4]

임상 연구
실시예 14: 1α,24-디하이드록시비타민 D2[1α,24-(OH)2D2]
안드로겐-비의존성 전립선 암이 발전된 환자들이 1α,24-(OH)2D2의 공개 라벨 연구(open-labelled study)에 참여한다. 자격을 갖춘 환자들은 40세 이상이고, 전립선 선암의 조직학적 증거를 나타내며 이미 호르몬 간섭(들)에 반응한 진전된 질환을 갖는다. 연구에 참여시, 환자들은 26주 동안 지속적으로 1α,24-(OH)2D2을 사용하는 경구 치료 과정을 시작하면서, 이전에 이용한 칼슘 보충제, 비타민 D 보충제 및 비타민 D 호르몬 대체 치료를 중단한다. 치료하는 동안, 환자는 (1) 과칼슘혈증, 과인산염혈증, 과칼슘뇨증, 과인산염뇨증 및 기타 독성; (2) 전이 질환의 진행에 대한 변화 증거 및 (3) 규정된 시험 약물 투여에 대한 순응성에 대해 일정한 간격으로 모니터링된다.
두가지 단계로 조사한다. 제1 단계 동안, 1α,24-(OH)2D2의 허용되는 최대 1일 경구 투여량(MTD)을, 연속되는 환자 그룹에게 점차적으로 많은 용량을 투여함으로써 결정한다. 모든 투여량은 아침 식사전에 투여한다. 제1 환자 그룹은 1α,24-(OH)2D225.0μg을 투여한다. 그 다음 환자 그룹은 50.0, 75.0 및 100.0μg/일을 투여한다. 혈청 내 칼슘이 11.6mg/dl을 초과하지 않거나 3 또는 4 등급의 다른 독성이 관찰되지 않는다면, 조사하는 동안 지속적으로 투여하는데, 3 또는 4 등급의 다른 독성이 관찰되는 경우, 관찰되는 독성 효과가 해결될 때까지 중지한 다음, 10.0μg으로 감소된 수준에서 투여를 재개한다.
제1 조사 단계로부터의 결과는 1α,24-(OH)2D2에 대한 MTD가 20.0μg/일을 초과하며, 즉 1α,25-(OH)2D2를 사용하여 달성되는 것보다 10배 내지 40배 높은 수준을 나타낸다. 참여한 환자들로부터 일정한 간격으로 채취한 혈액 샘플을 분석해 보면 1α,24-(OH)2D2의 순환량은 투여량에 비례적으로 증가하여, 최대 투여량에서 100pg/㎖의 최대량으로 증가하고 1α,25-(OH)2D2의 순환량은 종종 감지되지 않는 양으로 억제됨을 나타낸다. 혈청 및 뇨 칼슘은 투여량 반응 방식으로 증가한다. 6개월 이상 동안 1α,24-(OH)2D2의 MTD로 치료된 환자는 전이 질환과 관련된 골 통증이 상당히 감소되었음을 나타낸다.
제2 단계 동안, 환자는 24개월 동안 MTD의 0.5 내지 10배의 1α,24-(OH)2D2로 치료된다. 치료한 지 1 내지 2년 후, 전이 질환의 진행을 검사하기 위한 CAT 스캔, X-레이 및 골 스캔에 의하면, 적은 용량으로 치료된 다수의 환자가 안정한 질환 또는 부분 완쾌를 보이며, 높은 용량으로 치료된 다수의 환자가 안정한 질환 및 부분 완쾌 또는 완전한 완쾌를 보인다.
실시예 15: 1α-하이드록시비타민 D2[1α-OH-D2]
실시예 14의 조사를 활성 비타민 D 화합물, 즉 1α-OH-D2에 대해서 반복한다. 제1 단계 조사 결과, 6개월 이상 동안 1α-OH-D2의 MTD로 치료된 환자는 전이 질환과 관련된 골 통증이 상당히 경감되었음을 나타낸다. 제2 단계 조사 결과, 2년 후 전이 질환의 진행을 검사하기 위해 사용되는 CAT 스캔, X-레이 및 골 스캔에 의하면, 적은 용량으로 치료된 환자는 안정한 질환 또는 부분 완쾌를 나타내고, 높은 용량으로 치료된 환자는 안정한 질환 및 부분 완쾌 또는 완전한 완쾌를 나타낸다.
본 발명은 몇가지에 대해 구체적으로 기술되고 예시되었지만, 당해 기술 분야의 숙련자에게는 기술되어 있는 것에 대한 다양한 수정, 첨가 및 삭제를 포함하여 각종 변형이 있을 수 있음이 자명할 것이다. 따라서, 이러한 변형은 또한 본 발명에 포함되는 것이며 본 발명의 범위는 첨부된 청구의 범위에 대한 가장 넓은 법적 해석에 의해서만 제한된다.
Claims (18)
- C-24 위치에 치환된 탄화수소 잔기를 갖는 1α-하이드록시비타민 D 화합물 유효량을 사용하여 세포를 치료함을 포함하는, 사람 전립선 신생물성 또는 과형성성 세포의 과증식 활성을 억제하는 방법.
- 제1항에 있어서, 1α-하이드록시비타민 D 화합물이 화학식 I의 화합물인 방법.화학식 I상기식에서,B 및 C는 각각 수소 또는 탄소-탄소 결합으로서, 결과적으로 C-22 및 C-23 사이에 이중결합을 형성하고;R1및 R2는 동일하거나 상이하고, 수소, 하이드록실, 저급 알킬, 저급 플루오로알킬, O-저급 알킬, 저급 알케닐, 저급 플루오로알케닐, O-저급 알케닐, O-저급 아실, O-방향족 아실, 저급 사이클로알킬, 또는 이들이 결합된 탄소와 함께 C3-8사이클로탄소 환을 형성하고, 단 R1및 R2는 둘 모두가 알케닐 그룹일 수는 없으며,R3은 저급 알킬, 저급 알케닐, 저급 플루오로알킬, 저급 플루오로알케닐, O-저급 알킬, O-저급 알케닐, O-저급 아실, O-방향족 아실 또는 저급 사이클로알킬이고;X1은 수소 또는 하이드록실이거나, R3이 알케닐 그룹일 경우 R3과 함께 결합을 형성하며;X2는 수소 또는 하이드록실이거나, R1또는 R2와 함께 이중결합을 형성한다.
- 제2항에 있어서, 화학식 I의 화합물이 1α,24-디하이드록시비타민 D2, 1α,24-디하이드록시비타민 D4, 1α,25-디하이드록시비타민 D2, 1α,25-디하이드록시비타민 D4, 1α-하이드록시비타민 D2또는 1α-하이드록시비타민 D4인 방법.
- 제1항에 있어서, 치료 단계가 전립선 세포의 증식을 억제하고, 분화를 유도하고 증진시킴을 포함하는 방법.

- 전립선 암에 걸린 남성에게, 당해 화합물 또는 이의 생체내 대사산물의 VDR 결합 친화도가 1α,25-디하이드록시비타민 D3의 결합 친화도와 실질적으로 동일하며, 과칼슘혈증 위험이 1α,25-디하이드록시비타민 D3보다 실질적으로 낮은, 암의 세포성 이상 증식 활성을 감소시키거나 안정화시키기 위한 활성 비타민 D 화합물인, 유효량의 1차 항암제를 투여함을 포함하는, 사람 전립선 암의 치료 방법.
- 제7항에 있어서, 활성 비타민 D가 에스트라뮤스틴 포스페이트, 프레드니뮤스틴, 시스플라틴, 5-플루오로-우라실, 멜팔란, 하이드록시우레아, 미토마이신, 이다루비신, 메토트렉세이트, 아드리아마이신 및 다우노마이신으로 이루어진 그룹으로부터 선택된 2차 항암제를 포함하는 혼합물로 투여되는 방법.
- (a) 활성 비타민 D 화합물인 1차 항암제, 및 (b)(i) 2차 항암제, (ii) 골 제제, (iii) 안드로겐 조절제 및 (iv) 5α-리덕타제 억제제 및 이의 배합물로 이루어진 그룹으로부터 선택된 제제를 포함하는 약제학적 조성물.

- 제9항에 있어서, 2차 항암제가 에스트라뮤스틴 포스페이트, 프레드니뮤스틴, 시스플라틴, 5-플루오로-우라실, 멜팔란, 하이드록시우레아, 미토마이신, 이다루비신, 메토트렉세이트, 아드리아마이신 및 다우노마이신으로 이루어진 그룹으로부터 선택되는 약제학적 조성물.
- 제9항에 있어서, 활성 비타민 D가 약 0.01㎍/kg/일 내지 약 2.0㎍/kg/일의 투여량 범위로 존재하는 약제학적 조성물.
- 제10항에 있어서, 약제학적으로 허용되는 담체를 추가로 포함하는 약제학적 조성물.
- 제9항에 있어서, 안드로겐 조절제가 에스트로겐, LHRH 동족체, 항에스트로겐제 및 항안드로겐제로 이루어진 그룹으로부터 선택되는 약제학적 조성물.
- 제9항에 있어서, 5α-리덕타제 효소 억제제가 피나스테라이드인 약제학적 조성물.
- 제9항에 있어서, 골 제제가 복합 에스트로겐, 항에스트로겐제, 칼시토닌, 불소화나트륨, 비스포스포네이트, 칼슘 보충물, 코발라민, 백일해(pertussis) 독소 및 붕소로 이루어진 그룹으로부터 선택되는 약제학적 조성물.
- C-24에서 치환된 탄화수소 잔기를 갖는 활성 비타민 D 화합물의 치료학적으로 유효량을 이를 필요로 하는 사람에게 투여함을 포함하는, 전립선 암 또는 과형성증의 과증식성 세포 활성을 경감시키기 위한 사람의 치료 방법.
