Abstract
본 발명은 핵의학 및 분자 영상학 분야에 관한 것으로, 보다 상세하게는 이중 표적화 화합물 및 이의 제조 방법과 응용에 관한 것이다. 이중 표적화 화합물의 구조는 하기 식(I)과 같다. 본 발명은 하기 식 (I-1) 또는 식 (I-2)와 같은 구조를 갖는 방사성 핵종으로 표지될 수 있는 이중 표적화 화합물을 더 제공한다. 본 발명에 따른 이중 표적화 화합물은 FAP 표적 및 인테그린 αvβ3 표적 모두에 대해 비교적 높은 친화도를 가지며, 종양에서 FAP 표적 및 인테그린 αvβ3 표적을 상승적으로 표적화할 수 있어 높은 종양 흡수 및 종양 체류 시간을 나타낸다. 본 발명은 상기 이중 표적화 화합물에 기초한 방사성 핵종 표지의 이중 표적화 화합물, 및 이의 제조 방법과 FAP 및/또는 인테그린 αvβ3 과발현의 특징을 갖는 질병의 진단 또는 치료용 약물의 제조에서 이의 용도를 더 제공한다.
Description
본 발명은 핵의학 및 분자 영상학 분야에 관한 것으로, 보다 상세하게는 방사성 핵종 표지의 섬유아세포 활성화 단백질 FAP 및 인테그린 αvβ3 이중 표적화 화합물, 및 제조 방법과 FAP 및/또는 인테그린 αvβ3 과발현을 특징으로 하는 질병의 진단 또는 치료에서 상기 화합물의 용도에 관한 것이다.
섬유아세포 활성화 단백질(Fibroblast activation protein, FAP)은 막 세린 펩티다아제(membrane serine peptidase)로, 종양 기질에서 활성화된 섬유아세포 표면에 발현되며, 종양 발생 및 진행 과정에서 중요한 역할을 한다. 종래의 연구에서 FAP는 정상적인 인간 조직에서는 일반적으로 발현되지 않지만, 유방암, 난소암, 폐암, 대장암, 위암 및 췌장암 등을 포함한 상피 악성 종양의 90% 이상에서 간질 섬유아세포의 표면에서 선택적으로 높게 발현되는 것으로 나타났다. 인테그린 αvβ3(integrin αvβ3)는 세포 표면에 위치한 이종이량체 수용체로, 정상 혈관 내피 세포와 상피 세포에서는 매우 적게 발현되나 폐암, 골육종, 신경모세포종, 유방암, 전립선암, 방광암, 교모세포종 및 침윤성 흑색종 등 다양한 고형 종양 세포 표면에서 높은 수준으로 발현되며, 모든 종양 조직 신생 혈관 내피 세포막에서 높게 발현된다. 이는 인테그린 αvβ3이 종양 성장, 침입 및 전이에서 중요한 역할을 함을 시사한다. 아르기닌-글리신-아스파르트산(RGD) 서열을 포함하는 폴리펩티드는 인테그린 αvβ3에 특이적으로 결합할 수 있다. 종양에서 FAP 및 인테그린 αvβ3의 광범위한 발현 및 중요한 역할을 고려하여, FAP 및 인테그린 αvβ3는 종양 이미징 및 치료를 위한 중요한 표적이 되었다.
종양 진단 및 치료의 효율성을 더욱 향상시키기 위해, 종래 기술에서는 두 개의 표적에 동시에 친화도를 갖는 이중 표적 프로브가 개발되었다. 예를 들어, Anna Orlova 등은 전립선 특이적 막 항원(PSMA) 및 가스트린 방출 펩티드 수용체(GRPR)에 동시에 친화력을 갖는 이중 표적 프로브를 보고하였다. PSMA와 GRPR은 동시에 전립선에서 높은 수준으로 발현되기 때문에, 이러한 이중 표적 프로브의 단점은 전립선암의 방사선학적 진단 및 치료에만 적용되고 다른 종양에는 적용할 수 없다는 점이다.
FAP 및 인테그린 αvβ3는 주로 종양 기질 세포 및 신생 혈관에 분포하기 때문에, 다양한 종양 유형에서 동시에 높게 발현되며, "범종양" 이중 표적 프로브 개발을 위한 이상적인 표적이다. 종양의 이질성을 고려하여 종양 진단 및 치료의 효율성을 더욱 향상시키기 위해서는 FAP 및 인테그린 αvβ3의 두 가지 표적 모두에 대해 표적 역할을 할 수 있는 표적 화합물의 개발이 필요하다. 이러한 이중 표적화 화합물은 두 가지 종양에 대해 모두 높은 친화력을 가지고, 표적화 종양 중의 FAP 표적 및 인테그린 αvβ3 표적에 대한 상승적 표적을 구현하는 동시에 체내에서 우수한 약동학적 특성을 가지며, 종양 흡수 및 종양 체류 시간을 개선해야 한다. 이러한 이중 표적화 화합물에 기반한 방사성 핵종 표지의 이중 표적화 화합물은 FAP 표적 및 인테그린 αvβ3 표적을 동시에 사용하여, 종양 내 유효 수용체 수와 이용효율을 향상시켜, 양성 종양 검출 효율 및/또는 치료 효율을 향상시키는 문제를 해결할 수 있다.
상기 문제점을 해결하기 위해, 본 발명의 일차적 목적은 종양에서 FAP 표적 및 인테그린 αvβ3 표적을 상승적으로 표적화하여 종양에서 약물의 흡수 및 체류 시간을 증가시킬 수 있는 새로운 화합물 구조를 개발하는 데에 있다.
본 발명의 다른 목적은 상술한 새로운 화합물의 제조 방법을 제공하여, 입수하기 용이한 합성 경로를 통해 종양 내 FAP 표적과 인테그린 αvβ3 표적을 상승적으로 표적화할 수 있는 화합물을 합성하는 데에 있다.
본 발명의 또 다른 목적은 FAP 및/또는 인테그린 αvβ3 과발현을 특징으로 하는 질병의 진단 또는 치료에 있어서 상기 화합물의 용도를 제공하는 데에 있다.
본 발명의 상기 목적은 다음과 같은 기술적 해결방안을 통하여 달성된다.
제1 양상에 있어서, 본 발명은 FAP 및 인테그린 αvβ3을 표적화할 수 있는 이중 표적화 화합물을 제공하며, 이의 구조는 FAP 및 인테그린 αvβ3의 특이적 결합 리간드 구조를 동시에 포함하며, 상기 화합물의 구조는 하기 식 (I)로 표시된다.
제2 양상에 있어서, 본 발명은 방사성 핵종으로 표지될 수 있는 FAP 및 인테그린 αvβ3의 이중 표적화 화합물을 제공하며, 이의 구조는 FAP 및 인테그린 αvβ3의 특이적 결합 리간드 구조 및 핵종 킬레이트 구조를 모두 포함하며, 본 발명은 해당 구조를 FAPI-RGD 구조로 기재하고, 상기 화합물의 구조는 하기 식 (I-1) 또는 식 (I-2)로 표시된다.
제3 양상에 있어서, 본 발명은 방사성 핵종 표지의 FAP 및 인테그린 αvβ3의 이중 표적화 화합물을 제공하며, 이는 본 발명의 제2 양상에 따른 화합물에서 방사성 핵종을 표지하여 획득된다.
본 발명의 방안에 있어서, 상기 방사성 핵종은 α선을 방출하는 동위원소, β선을 방출하는 동위원소, γ선을 방출하는 동위원소, 오제 전자를 방출하는 동위원소 또는 X선을 방출하는 동위원소 등으로부터 선택될 수 있으며, 예를 들어 18F, 51Cr, 67Ga, 68Ga, 111In, 99mTc, 186Re, 188Re, 139La, 140La, 175Yb, 153Sm, 166Ho, 86Y, 90Y, 149Pm, 165Dy, 169Er, 177Lu, 47Sc, 142Pr, 159Gd, 212Bi, 213Bi, 72As, 72Se, 97Ru, 109Pd, 105Rh, 101mRh, 119Sb, 128Ba, 123I, 124I, 131I, 197Hg, 211At, 151Eu, 153Eu, 169Eu, 201Tl, 203Pb, 212Pb, 64Cu, 67Cu, 198Au, 225Ac, 227Th, 89Zr 또는 199Ag 중 어느 하나이며, 보다 바람직한 방사성 핵종은 18F, 64Cu, 68Ga, 89Zr, 90Y, 111In, 99mTc, 177Lu, 188Re 또는 225Ac이다.
제4 양상에 있어서, 본 발명은 제2 양상에 따른 이중 표적화 화합물 및 이의 방사성 핵종 표지 화합물(즉, 본 발명의 제3 양상에 따른 이중 표적화 화합물)의 제조 방법을 제공한다. 본 발명에서 제공하는 방법은 하기 단계를 포함한다.
단계 ①: 6-히드록시퀴놀린-4-카르복실산의 카르복실은 먼저 tert-부틸 글리시네이트의 아미노와 아미드 축합 반응을 일으킨다. 그 후 아미드 축합 생성물 히드록실 위치에서 알킬 결합을 통해 Boc로 보호된 피페라지닐을 연결한다. 산성 조건 하에서 Boc 및 tert-부틸 보호기를 제거하고, 이어서 피페라진 고리에 Boc 보호기를 도입한다. 이어서 (S)-피롤리딘-2-카르보니트릴 히드로클로라이드와 아미드 축합 반응을 일으킨다. Boc 보호기를 제거한 후 N-Boc-3-[2-(2-아미노에톡시)에톡시]프로피온산과 축합 반응을 일으킨다. 이어서 Boc 보호기를 제거하고, Fmoc-O-tert-부틸-L-글루탐산과 반응한다. tert-부틸 에스테르를 제거한 후, 활성화된 에스테르로 제조한다. 이어서 아미노-디폴리에틸렌 글리콜의 c(RGDfK)와 반응하여 이중 표적화 화합물을 수득한다. Fmoc 보호 제거 후 핵종 킬레이트제와 반응시키며, 상기 핵종 킬레이트제는 1,4,7,10-테트라아자시클로도데칸-N,N',N,N'-테트라아세트산 또는 1,4,7-트리아자시클로노난-1,4,7-트리아세트산이다. 이어서 킬레이트기 상의 tert-부틸 에스테르의 보호를 제거한 후 방사성 핵종에 의해 표지될 수 있는 이중 표적화 화합물, 즉 본 발명의 제2 양상에 따른 이중 표적화 화합물을 수득한다.
단계 ②: 단계 ①에서 수득한 방사성 핵종에 의해 표지할 수 있는 이중 표적화 화합물과 방사성 핵종 함유 화합물을 종래의 습식 표지 방법 또는 동결건조 표지 방법에 따라 반응시켜, 본 발명의 제3 양상에 따른 방사성 핵종 표지의 FAP 및 인테그린 αvβ3의 이중 표적화 화합물을 제조하여 획득할 수 있다.
제5 양상에서, 본 발명은 약학 조성물을 제공한다. 상기 약학 조성물은 본 발명의 제1 양상에 따른 FAP 및 인테그린 αvβ3을 표적화할 수 있는 이중 표적화 화합물, 본 발명의 제2 양상에 따른 방사성 핵종으로 표지 가능한 FAP 및 인테그린 αvβ3의 이중 표적화 화합물, 제3 양상에 따른 방사성 핵종 표지의 FAP 및 인테그린 αvβ3의 이중 표적화 화합물, 또는 그의 약학적으로 허용 가능한 임의 호변 이성질체, 라세미체, 수화물, 용매화물 또는 염을 포함한다.
제6 양상에 있어서, 본 발명은 본 발명의 제1 양상에 따른 FAP 및 인테그린 αvβ3을 표적화할 수 있는 이중 표적화 화합물, 제2 양상에 따른 방사성 핵종 표지 가능한 FAP 및 인테그린 αvβ3의 이중 표적화 화합물, 제3 양상에 따른 방사성 핵종 표지의 FAP 및 인테그린 αvβ3의 이중 표적화 화합물 또는 FAP 및/또는 인테그린 αvβ3 과발현을 특징으로 하는 동물 또는 인간 개체의 질병을 진단 또는 치료하기 위한 약물의 제조에서 제5 양상에 따른 약학 조성물의 용도를 더 제공한다.
본 발명에 따른 응용에 있어서, 상기 FAP 및/또는 인테그린 αvβ3의 과발현을 특징으로 하는 질병에는 암, 만성 염증, 죽상동맥경화증, 섬유증, 조직 재형성 및 흉터 질환이 포함되나 이에 제한되지 않는다. 바람직하게는 상기 암은 유방암, 췌장암, 소장암, 대장암, 직장암, 폐암, 두경부암, 난소암, 간세포암종, 식도암, 하인두암, 비인두암, 후두암, 골수종 세포, 방광암, 담관세포암, 투명세포신장암, 신경내분비 종양, 발암성 골연화증, 육종, CUP(미지의 원발성 암종), 흉선암, 교종, 신경아교종, 성상세포종, 자궁경부암 또는 전립선암으로부터 더 선택된다.
본 발명에서 제공하는 FAPI-RGD 화합물 구조는 FAP 표적 및 인테그린 αvβ3 표적 모두에 대해 비교적 높은 친화도를 가지며, 종양에서 FAP 표적 및 인테그린 αvβ3 표적을 상승적으로 표적화할 수 있어, 비교적 높은 종양 흡수 및 종양 체류 시간을 나타내므로, FAP 및/또는 인테그린 αvβ3의 과발현을 특징으로 하는 질병의 진단 또는 치료에 적용될 것으로 기대된다.
또한, 본 발명에서 제공하는 FAPI-RGD 화합물의 제조 방법은 반응 경로가 간단하고 조작이 간단하며 제조원료가 저렴하고 입수가 용이하며 생산비용이 저렴하여 산업생산에 적합하다.

실시예 1: 화합물 I-1의 제조
화합물 2의 합성:
100mL 플라스크에 화합물 1(6-히드록시퀴놀린-4-카르복실산, 1.89g, 10.0mmol), tert-부틸 글리시네이트(1.89g, 10.0mmol), HATU(3.8g, 10.0mmol) 및 N,N-디이소프로필에틸아민(2.6g, 20.0 mmol)을 30mL N,N-디메틸포름아미드에 순차적으로 각각 첨가하였다. 반응 혼합물을 밤새 교반하고, 용매를 감압 증류 제거하여 조 생성물을 수득하였다. 실리카겔 칼럼(디클로로메탄/메탄올=30:1)으로 정제하여 흰색 고체 화합물 2를 87%의 수율로 수득하였다.
화합물 3의 합성:
100mL 플라스크에 화합물 2(1.51g, 5.0mmol)), 1-브로모-3-클로로프로판(1.55g, 10.0mmol), 탄산칼륨(1.38g, 10.0mmol)을 50mL N,N-디메틸포름아미드에 순차적으로 각각 투입하였다. 계를 60℃까지 승온시키고, 계를 60℃로 유지하여 밤새 교반하였으며, 용매를 감압 증류 제거하여 조 생성물을 수득하였다. 실리카겔 칼럼(디클로로메탄/메탄올=50:1)으로 정제하여 흰색 고체 화합물 3을 63%의 수율로 수득하였다.
화합물 4의 합성:
100mL 플라스크에 화합물 3(0.76g, 2.0mmol), 1-tert-부톡시카르보닐피페라진(0.55g, 3.0mmol) 및 요오드화칼륨(0.49g, 3.0mmol)을 30mL의 아세토니트릴에 순차적으로 각각 투입하였다. 계를 60℃까지 승온시키고, 계를 60℃로 유지하여 밤새 교반하였으며, 용매를 감압 증류 제거하여 조 생성물을 수득하였다. 실리카겔 칼럼(디클로로메탄/메탄올=30:1)으로 정제하여 흰색 고체 화합물 4를 58%의 수율로 수득하였다.
화합물 5의 합성:
얼음욕 조건 하에서, 화합물 4(0.52g, 1.0mmol)를 10mL의 디클로로메탄과 트리플루오로아세트산(부피비 9:1) 혼합용액에 용해시키고, 계를 실온으로 승온시켜 2시간 동안 반응시키며, 반응 종료 후 감압 증류하여 용매를 제거하고, 10mL의 N,N-디메틸포름아미드에 용해시켜, 화합물 5을 수득하여 준비하였다.
화합물 6의 합성:
화합물 5의 N,N-디메틸포름아미드 용액에 디-tert-부틸 디카보네이트(0.22g, 1.0mmol)와 N,N-디이소프로필에틸아민(0.39g, 3.0mmol)을 각각 첨가하고, 실온에서 밤새 교반한 후, 용매를 감압 증류 제거하여 조 생성물을 수득하였다. 실리카겔 칼럼(디클로로메탄/메탄올=10:1)으로 정제하여 흰색 고체 화합물 6을 72%의 수율로 수득하였다.
화합물 7의 합성:
100mL 플라스크에 화합물 6(0.47g, 1.0mmol), (S)-피롤리딘-2-카르보니트릴 히드로클로라이드(0.13g, 1.0mmol), HATU(0.38g, 1.0mmol) 및 N,N-디이소프로필에틸아민(0.26g, 2.0mmol)을 10mL N,N-디메틸포름아미드에 순차적으로 각각 투입하였다. 반응 혼합물을 실온에서 교반하여 반응을 종료시키고, 용매를 감압 증류 제거하여 조 생성물을 수득하였다. 실리카겔 칼럼(디클로로메탄/메탄올=50:1)으로 정제하여 흰색 고체 화합물 7을 85%의 수율로 수득하였다. 도 1은 화합물 7의 H-NMR 스펙트럼이고, 도 2는 화합물 7의 C-NMR 스펙트럼이고, 도 3은 화합물 7의 질량 스펙트럼이다.
화합물 8의 합성:
화합물 7(2.50g, 4.5mmol), p-톨루엔술폰산 일수화물(2.58g, 13.6mmol), 아세토니트릴 25mL를 반응 플라스크에 넣고, 65℃에서 1시간 동안 반응시켰으며, TLC로 화합물 7의 반응 완료를 모니터링하고(메탄올:디클로로메탄=5:1), 40℃에서 감압 증발 건조시켰다. 14mL의 DMF와 DIPEA(3.05g, 23.6mmol)를 첨가하고 25℃에서 교반하여, 번호 (1)을 반응시켜, 즉 화합물 7의 피페라진을 탈보호하여 중간체를 수득하였다. 다른 반응 플라스크에 N-tert-부톡시카르보닐-디폴리에틸렌 글리콜-카르복실산(1.62g, 4.8mmol), HATU(2.60g, 6.8mmol), DMF 10mL를 첨가하고, 25℃에서 30분간 반응시켜, 번호 (2)를 반응시키고, 반응 (2)의 반응 용액계를 반응 (1)계에 점적하고 1시간 동안 반응시켰다. 40℃에서 감압 증발 건조하고, 정제수 50mL를 첨가하며, DCM으로 매회 50mL씩 2회 추출하여 DCM을 합하고, 무수 황산나트륨으로 건조하고 여과 및 증발 건조하여, 조 생성물을 수득하고, 칼럼 크로마토그래피로 정제하여 목적물 1.68g을 수득하였다. 이론 분자량은 709.3799이고, 실측 분자량은 709.38801이며, 질량 스펙트럼 결과는 목적물과 일치하였다. 도 4는 화합물 8의 질량 스펙트럼이다.
화합물 9의 합성:
화합물 8, p-톨루엔술폰산 일수화물(1.61g, 8.5mmol), 아세토니트릴 20mL를 반응 플라스크에 넣고 65℃에서 1시간 동안 반응시킨 후 감압 하에 40℃에서 증발 건조시켰다. 20mL의 DMF, DIPEA(1.83g, 14.2mmol)를 첨가하고 25℃에서 교반하고, 번호 (1)을 반응시켰다. 다른 반응 플라스크에 Fmoc-O-tert-부틸-L-글루탐산(1.43g, 3.4mmol), HATU(1.29g, 3.4mmol), DMF 20mL를 첨가하고, 25℃에서 30분간 반응시켜, 번호 (2)를 반응시키고, 반응 (2)의 반응 용액계를 반응 (1)계에 점적하고 1시간 동안 반응시켰다. 40℃에서 감압 증발 건조하여 조 생성물을 수득하였으며, 칼럼 크로마토그래피로 정제하여 목적물 1.19g을 수득하였다. 이론 분자량은 1016.5008이고, 실측 분자량은 1016.51094이며, 질량 스펙트럼 결과는 목적물과 일치하였다. 도 5는 화합물 9의 질량 스펙트럼이다.
화합물 12의 합성:
c(RGDfK)(1.00g, 1.7mmol), 아미노-tert-부틸 에스테르-디폴리에틸렌 글리콜-숙신이미드 에스테르(0.74g, 1.9mmol), DIPEA(0.44g, 3.4mmol) 및 DMF 20mL를 반응 플라스크에 첨가하여 반응시키며, 30℃에서 20시간 동안 반응시켰다. 40℃에서 감압 증발 건조하고 메탄올 10mL를 첨가하며 MTBE 60 mL를 점적하여 고체를 석출하여, 중간체 11을 수득하였으며, 흡인 여과하고 40℃에서 2시간 동안 진공 건조하였다. 고체 중간체 11을 반응 플라스크에 넣고 TFA 30mL, 정제수 1.5mL를 넣고 30℃에서 1시간 동안 반응시켰으며, 0 내지 5℃로 강온시켜 MTBE 200mL를 점적하였으며, 0 내지 5℃에서 30분간 교반하고 흡인 여과하여 MTBE로 헹구고 40℃에서 진공 건조시켜 제품을 수득하였다. 이론 분자량은 762.4024이고, 실측 분자량은 762.40768이며, 질량 스펙트럼 결과는 목적물과 일치하였다. 도 6은 화합물 12의 질량 스펙트럼이다.
화합물 13의 합성:
화합물 9, p-톨루엔술폰산 일수화물(0.34g, 1.8mmol), 아세토니트릴 20mL를 반응 플라스크에 넣고 65℃에서 4시간 동안 반응시킨 후 감압 하에 40℃에서 증발 건조시켰다. DMF 20mL, DIPEA(0.36g, 2.8mmol), DCC(0.14g, 0.7mmol), NHS(0.08g, 0.7mmol)를 첨가하고 35℃에서 15 내지 20시간 동안 반응시켜 중간체 10을 수득하였으며, 25℃로 강온시켜, 화합물 12를 첨가하고 1시간 동안 반응시킨 후, 40℃에서 감압 증발 건조하여 조 생성물을 수득하고 제조액상을 정제하여 목적물 66.5mg을 수득하였다. 이론 분자량은 1704.8300이고, 실측 분자량은 1704.84518이며, 질량 스펙트럼 결과는 목적물과 일치하였다. 도 7은 화합물 13의 질량 스펙트럼이다.
화합물 14의 합성:
화합물 13, 피페리딘 0.5mL 및 DMF 2mL를 반응 플라스크에 넣고 25℃에서 1시간 동안 반응시키고, 에틸 아세테이트 10mL를 점적하여 결정화하고, 30분 동안 교반하여 흡인 여과하였으며, 고체는 40℃ 진공에서 2시간 동안 건조시켜 제품 50.8mg을 수득하였다. Fmoc 탈보호된 화합물 13을 DMF 2mL에 용해하고, NOTA-2 tert-부틸 에스테르-NHS 활성화 에스테르, DIPEA(0.010g, 0.08mmol)를 첨가하고, 25℃에서 1시간 동안 반응시키고, 40℃에서 감압 증발 건조한 후, 2 mL의 에틸 아세테이트와 2mL의 MTBE를 첨가하여 결정화하고, 20분 동안 교반하고, 흡인 여과하고, 고체를 40℃에서 진공 건조하여 43.2mg의 제품을 수득하였다. 이론 분자량은 1880.0196, 실측 분자량은 1880.0369이며 질량 스펙트럼 결과는 목적물과 일치하였다. 도 8은 화합물 14의 질량 스펙트럼이다.
화합물 15의 합성:
화합물 14와 트리플루오로아세트산 2mL를 반응 플라스크에 넣고 25℃에서 1시간 동안 반응시킨 후 40℃에서 감압 증발 건조시켜 조 생성물을 수득하였고, 제조액상을 정제하고 동결 건조하여 제품 화합물 15를 42%의 수율로 수득하였다. 이론 분자량은 1767.8944이고, 실측 분자량은 1767.91036이며, 질량 스펙트럼 결과는 목적물과 일치하였다. 도 9는 화합물 15의 질량 스펙트럼이다.
상술한 단계의 합성 경로는 하기와 같다.
실시예 2
실시예 2의 제조 방법은 실시예 1을 참조하여, 상술한 실시예에서 NOTA-2 tert-부틸 에스테르-NHS 활성화 에스테르를 DOTA-3 tert-부틸 에스테르-NHS 활성화 에스테르로 치환하여 하기 구조를 수득하였다.
실시예 3: 방사성 Ga-68 표지 FAPI-RGD 식 (I-1) 복합체(
68
Ga-FAPI-RGD)의 제조:
습식법: 약 18.5 내지 1850MBq 68GaCl3 염산 용액(게르마늄 갈륨 발생기에서 침출)을 0.5mL의 실시에 1에서 제조된 구조가 식 (I-1)과 같은 화합물의 아세트산-아세테이트 용액(1.0g/L)이 함유된 원심분리관에 첨가하고, 37°C에서 20분 동안 반응시켰다. C18 분리 칼럼을 취하여 10mL의 무수 에탄올로 천천히 침출한 다음 물 10mL로 침출하였다. 표지액을 10mL의 물로 희석한 후, 분리 칼럼에 로딩하고, 먼저 10mL의 물로 미표지된 68Ga 이온을 제거한 다음, 0.3mL의 10mM HCl 에탄올 용액으로 침출하여 68Ga 표지된 FAPI-RGD 복합체를 수득하였다. 상기 침출액을 생리 식염수로 희석하고, 멸균 여과하여 68Ga 표지된 FAPI-RGD 복합체의 주사액을 수득하였다.
동결 건조법: 약 18.5 내지 1850MBq 68GaCl3 염산 용액(게르마늄 갈륨 발생기에서 침출)을 식 (I-1) 화합물이 함유된 동결 건조 약상자에 넣고 혼합한 후 37℃에서 20분 동안 반응시켰다. C18 분리 칼럼을 취하여 10mL의 무수 에탄올로 천천히 침출한 다음 물 10mL로 침출하였다. 표지액을 10mL의 물로 희석한 후, 분리 칼럼에 로딩하고, 먼저 10mL의 물로 미표지된 68Ga 이온을 제거한 다음, 0.3mL의 10mM HCl 에탄올 용액으로 침출하여 복합체 침출액을 수득하였다. 상기 침출액을 생리 식염수로 희석하고, 멸균 여과하여 68Ga 표지된 FAPI-RGD 복합체의 주사액을 수득하였다.
실시예 4: 방사성 Lu-177 표지 FAPI-RGD 식 (I-2) 복합체(

Lu-FAPI-RGD)의 제조:
pH=5.5의 완충용액 조제: 아세트산 57.6mg, 젠티식산 189mg 및 아세트산나트륨 삼수화물 525mg을 칭량하여 정제수 48ml로 용해시키고 수산화나트륨용액으로 pH를 5.5로 조정하였다. 200μL의 완충액(pH=5.5)으로 실시예 2에서 제조한 구조가 식 (I-2)와 같은 화합물 200μg를 충분히 용해시킨 후, 완충액(pH=5.5) 5mL와 177LuCl3 염산용액 약 150mCi를 첨가하였다. 혼합물을 진탕한 후 80℃에서 20분 동안 가열하였다. 반응 종료 후 실온으로 냉각하였다. 상기 반응액은 생리 식염수로 희석하고 멸균 여과한 후 10mCi/mL의 177Lu 표지 FAPI-RGD 복합체의 주사액을 수득하였다.
실험예 분석 및 응용 효과
1. 68Ga 표지된 식 (I-1) FAPI-RGD의 복합체의 안정성 분석
실시예 3에서 제조한 68Ga-FAPI-RGD(3.7MBq 활성도/20μL) 용액 20μL를 100μL의 생리 식염수 또는 PBS(pH=7.4)가 함유된 원심분리관에 첨가하고, 37℃에서 30분, 1시간 및 4시간 동안 배양하고 용액을 공동 배양하였다. 공동 배양 용액 20μL를 취하여 0.22μm 니들 필터를 통과시키고 HPLC로 방사화학적 순도를 분석하였다. 테스트 결과는 도 10에 도시된 바와 같이, 68Ga-FAPI-RGD는 생리식염수에서 배양된 후 현저한 분해가 없었고, 방사화학적 순도는 모두 99%를 초과하였다. 이는 본 발명에서 제조된 68Ga-FAPI-RGD의 안정성이 우수함을 설명한다.
2. 68Ga 표지된 식 (I-1) FAPI-RGD 복합체의 세포 실험 분석
HT1080-FAP 종양 세포에서 68Ga-FAPI-RGD의 세포 흡수 실험을 수행하였다. 테스트 결과는 도 11의 A 부분에 도시된 바와 같이, 68Ga-FAPI-RGD는 빠른 세포 흡수를 나타냈으며, 30분 배양 시, 흡수가 최대치에 도달하였고 유사한 흡수 수준이 최장 2시간 동안 유지되었다. 또한, 차단 실험에서 68Ga-FAPI-RGD의 세포 흡수가 C(RGDfK) 또는 FAPI-02에 의해 부분적으로 억제될 수 있고, FAPI-RGD에 의해 완전히 차단될 수 있음을 확인하였다(도 11의 A 부분 참조). HT1080-FAP 및 U87MG 종양 세포에서 세포 결합 실험을 수행하였다. 테스트 결과는 각각 도 11의 B 및 C에 도시된 바와 같다. HT1080-FAP 세포 실험에서, 68Ga-FAPI-RGD와 68Ga-FAPI-02의 IC50은 각각 11.17nM 및 4.14nM로 측정되었다. HT1080-FAP 세포 실험에서, 68Ga-FAPI-RGD 및 68Ga-C(RGDfK)의 IC50은 각각 18.93nM 및 11.49nM으로 측정되었다. 실험 결과는 FAPI-RGD가 상응하는 단량체와 비교하여, 상응하는 수용체 FAP 및 인테그린 αvβ3과 유사한 친화도를 가짐을 보여주었다.
3. 종양 보유 마우스 체내에서 68Ga 표지의 식 (I) FAPI-RGD 복합체의 MicroPET 이미징
실시예 3의 방법에 따라 68Ga-FAPI-RGD를 제조하였다. HT1080-FAP 종양 보유 마우스에서, 무작위로 그룹화한 마우스에 대해, 각각 7.4MBq의 68Ga-FAPI-RGD, 68Ga-FAPI-02 및 68Ga-C(RGDfK)를 꼬리정맥 주사한 후, 이소플루란 마취 하에서, 68Ga-FAPI-RGD군은 투여 0 내지 240분 후에 MicroPET 이미징을 수행하였고, 나머지 군은 각각 투여 0 내지 120분 후 MicroPET 이미징을 수행하였다. 그 결과는 도 12에 도시된 바와 같다. 도 12의 A, C 및 E는 각각 상기 3개 군의 마우스에 정맥 주사 후 시간별 HT1080-FAP 종양 보유 마우스(n=3)의 MicroPET 최대 밀도 투영 이미지를 도시한 것이다. B, D 및 F는 각각 상기 3개 군의 마우스의 주사 후 각 기관 또는 조직(혈액, 간, 신장, 종양 및 근육)의 시점별 흡수를 나타냈으며, 각 군에서 3개의 흡수량은 왼쪽으로 오른쪽으로 각각 주사 후 30분, 1시간 및 2시간의 시점에 해당한다. 도 12는 이미징을 획득한 시점에 종양이 명확하게 보이며 68Ga-FAPI-RGD의 종양 흡수가 단량체 68Ga-FAPI-02 및 68Ga-C(RGDfK)의 종양 흡수보다 높았음을 보여준다. 생체 내에서 인테그린 αvβ3 및 FAP에 특이적으로 결합하는 68Ga-FAPI-RGD의 성능은 차단 실험에 의해 확인되었다. 상술한 68Ga-FAPI-RGD와 C(RGDfK) 또는 FAPI-02를 함께 HT1080-FAP 종양 보유 마우스 체내에 주사하였고, 이의 MicroPET 이미징 결과도 및 장기 흡수 결과는 도 13에 도시된 바와 같다. 도 13에서, A의 4개 영상은 왼쪽에서 오른쪽으로 68Ga-FAPI-RGD 단독 주사, 68Ga-FAPI-RGD와 C(RGDfK) 공동 주사, 68Ga-FAPI-RGD와 FAPI-02 공동 주사, 68Ga-FAPI-RGD와 C(RGDfK) 및 FAPI-02 공동 주사로 획득된 영상이다. B 및 C는 각각 상기 4개 군의 상이한 주사 방식으로 주사된 마우스의 각 기관 또는 조직(혈액, 간, 신장, 종양 및 근육)의 68Ga-FAPI-RGD에 대한 흡수 및 표적/비표적 비율을 나타내며, B 및 C의 각 기관 또는 조직 중 4개의 막대 그래프는 각각 왼쪽에서 오른쪽으로 A의 4가지 주사 방식에 대응한다. 도 13에서 알 수 있듯이, 68Ga-FAPI-RGD와 함께 RGD 또는 FAPI-02를 공동 주사하면 모두 종양의 68Ga-FAPI-RGD에 대한 흡수를 줄일 수 있고, 68Ga-FAPI-RGD와 C(RGDfK)+FAPI-02를 공동 주사하면 종양의 68Ga-FAPI-RGD에 대한 흡수를 더욱 감소시킬 수 있다. 차단 실험을 통해 68Ga-FAPI-RGD가 생체 내에서 인테그린 αvβ3 및 FAP를 결합하여 종양 특이적 표적화를 구현할 수 있음을 확인하였다.
4. 종양 보유 마우스에서 177Lu 표지의 식 II FAPI-RGD의 복합체의 SEPCT 이미징
실시예 4의 방법에 따라 177Lu-FAPI-RGD를 제조하였다. HT1080-FAP 종양 보유 마우스에서 각각 37MBq의 177Lu-FAPI-RGD를 꼬리 정맥 주사한 후, 이소플루란 마취 하에서 투여 4시간 후 SPECT 이미징을 수행하였다. 결과는 도 14에 도시된 바와 같이, 투여 4시간 후 종양이 뚜렷하게 보였다.
5. 종양 보유 마우스 체내에서 68Ga 표지의 FAPI-RGD(대조 화합물) 복합체의 MicroPET 이미징
대조군으로서, 말레이미드-티올에 의해 형성된 티오숙신이미드 결합을 연결 구조로 사용하여 도 15B의 화합물을 제조하였고, 실시예 3의 방법에 따라 68Ga 표지를 수행하고, HT1080-FAP 종양 보유 마우스에서 MicroPET 이미징 연구를 수행하였으며, 그 결과를 도 15에 나타내었다. 도 15A는 이미징을 획득한 시점에서 주요 방사성 신호가 간과 신장에 집중되었으며 종양 흡수가 비교적 적음을 보여준다. 도 15C도 간 및 신장에서의 높은 흡수 및 종양에서의 낮은 흡수를 나타냈다. 따라서 티오숙신이미드 결합을 연결 구조로 사용하면, 표적기와 표적 수용체의 친화도 저하로 이어져, 표적 기관의 흡수 감소를 초래할 수 있으며, 동시에 비-표적 조직 흡수가 너무 높아 표적/비표적 비율이 낮아져, 부작용이 발생할 가능성이 높아진다. 본 발명의 특정 연결 구조는 수용체와의 높은 친화도를 보장할 수 있을 뿐만 아니라 적절한 약동학적 특성을 제공하여 비교적 높은 종양 절대 흡수 및 표적/비표적 비율을 보장할 수 있다.
6. 종양 환자에서 68Ga 표지의 식 (I-1) FAPI-RGD 복합체의 PET/CT 이미징
68Ga-FAPI-RGD의 임상시험은 샤먼대학교 제1부속병원 임상연구윤리위원회의 승인을 받았으며, 모든 피험자가 서면 동의서에 서명하였고, 여기에는 췌장암 환자 1명, 비소세포폐암 환자 1명, 소세포폐암 환자 1명 및 비인두암 환자 1명이 포함되었다. 피험자 체중에 따라 68Ga-FAPI-RGD 정맥주사 용량(1.8 내지 2.2MBq[0.05 내지 0.06mCi]/kg)을 계산하였다. 정맥주사 3시간 후, 하이브리드 PET/CT 스캐너(Discovery MI, GE Healthcare, Milwaukee, WI, USA)를 이용하여 데이터를 획득하였으며, 이미징 결과는 도 16에 도시된 바와 같다. 최대 표준화 흡수 값(SUVmax)은 축상 이미지에 제도된 관심 영역(ROI)을 사용하여 자동으로 계산되었다. 상이한 유형의 종양에서 이중 표적화의 68Ga-FAPI-RGD의 SUVmax는 FAP 단백질 단일 표적의 68Ga-FAPI-46보다 모두 높았으며, SUVmax는 약 30 내지 50% 증가하여, 이중 표적화의 설계가 종양에서 유효 수용체 수와 활용 효율을 향상시켜 종양 흡수를 높일 수 있음을 입증하였다.
요약하면, 본 발명은 FAPI-RGD 구조를 개발하였다. FAP 표적 및 인테그린 αvβ3 표적 모두에 대해 비교적 높은 친화도를 가지며, 종양에서 FAP 표적 및 인테그린 αvβ3 표적을 상승적으로 표적화할 수 있어, 우수한 약동학적 성능을 가지고 비교적 높은 종양 흡수 및 종양 체류 시간을 나타내므로, FAP 및/또는 인테그린 αvβ3의 과발현을 특징으로 하는 질병의 진단 또는 치료에 적용될 것으로 기대된다.
비록, 상기에서 일반적인 설명, 구체적인 실시방식 및 시험을 통해 본 발명을 상세하게 설명하였으나, 본 발명을 기반으로 당업자는 일부 수정 또는 개선을 용이하게 수행할 수 있다. 따라서, 본 발명의 사상에서 벗어나지 않는 범위 내에서 행해진 수정 또는 개선은 모두 본 발명의 보호 범위에 속한다.
Claims (11)
- 이중 표적화 화합물에 있어서,
이의 구조는 FAP 및 인테그린 αvβ3의 특이적 결합 리간드 구조를 동시에 포함하며, 상기 화합물의 구조는 하기 식 (I)로 표시되는 것을 특징으로 하는 이중 표적화 화합물
- 방사성 핵종으로 표지될 수 있는 이중 표적화 화합물에 있어서,
이의 구조는 FAP 및 인테그린 αvβ3의 특이적 결합 리간드 구조 및 핵종 킬레이트 구조를 모두 포함하며, 상기 화합물의 구조는 하기 식 (I-1) 또는 식 (I-2)로 표시되는 것을 특징으로 하는 방사성 핵종으로 표지될 수 있는 이중 표적화 화합물.
- 제2항에 따른 방사성 핵종으로 표지될 수 있는 이중 표적화 화합물의 제조 방법에 있어서,
6-히드록시퀴놀린-4-카르복실산의 카르복실은 먼저 tert-부틸 글리시네이트의 아미노와 아미드 축합 반응을 일으키는 단계; 그 후 아미드 축합 생성물 히드록실 위치에서 알킬 결합을 통해 Boc로 보호된 피페라지닐을 연결하는 단계; 산성 조건 하에서 Boc 및 tert-부틸 보호기를 제거하고, 이어서 피페라진 고리에 Boc 보호기를 도입하는 단계; 이어서 (S)-피롤리딘-2-카르보니트릴 히드로클로라이드와 아미드 축합 반응을 일으키는 단계; Boc 보호기를 제거한 후 N-Boc-3-[2-(2-아미노에톡시)에톡시]프로피온산과 축합 반응을 일으키는 단계; 이어서 Boc 보호기를 제거하고, Fmoc-O-tert-부틸-L-글루탐산과 반응시키는 단계; tert-부틸 에스테르를 제거한 후, 활성화된 에스테르로 제조하고, 이어서 아미노-디폴리에틸렌 글리콜의 c(RGDfK)와 반응시켜 이중 표적화 화합물을 수득하는 단계; Fmoc 보호 제거 후 핵종 킬레이트제와 반응시키는 단계 - 상기 핵종 킬레이트제는 1,4,7,10-테트라아자시클로도데칸-N,N',N,N'-테트라아세트산 또는 1,4,7-트리아자시클로노난-1,4,7-트리아세트산임 - ; 및 이어서 킬레이트기 상의 tert-부틸 에스테르의 보호를 제거한 후 방사성 핵종에 의해 표지될 수 있는 이중 표적화 화합물을 수득하는 단계를 포함하는 것을 특징으로 하는 제조 방법. - 방사성 핵종 표지의 이중 표적화 화합물에 있어서,

상기 방사성 핵종은 18F, 51Cr, 64Cu, 67Cu, 67Ga, 68Ga, 89Zr, 111In, 99mTc, 186Re, 188Re, 139La, 140La, 175Yb, 153Sm, 166Ho, 86Y, 90Y, 149Pm, 165Dy, 169Er, 177Lu, 47Sc, 142Pr, 159Gd, 212Bi, 213Bi, 72As, 72Se, 97Ru, 109Pd, 105Rh, 101mRh, 119Sb, 128Ba, 123I, 124I, 131I, 197Hg, 211At, 151Eu, 153Eu, 169Eu, 201Tl, 203Pb, 212Pb, 198Au, 225Ac, 227Th 또는 199Ag 중 어느 하나로부터 선택되는 것을 특징으로 하는 방사성 핵종 표지의 이중 표적화 화합물. - 제4항에 있어서,

제1항에 따른 이중 표적화 화합물, 제2항에 따른 방사성 핵종으로 표지될 수 있는 이중 표적화 화합물, 제4항에 따른 방사성 핵종 표지의 이중 표적화 화합물, 또는 이들의 약학적으로 허용 가능한 수화물, 용매화물 또는 염을 유효성분으로 포함하는 동물 또는 인간 개체에서 FAP 및/또는 인테그린 αvβ3 과발현의 특징을 갖는 질병인 암, 만성 염증, 죽상동맥경화증 또는 흉터 질환의 치료용 약학 조성물. - 제8항에 있어서,
