Abstract

본 발명은 DMD 엑손 2의 중복(duplication)으로 유발되는 듀켄씨 근이영양증(Duchenne Muscular Dystrophy)을 치료하기 위한, 폴리뉴클레오티드의 재조합형 아데노 부속 바이러스(rAAV) 전달에 관한 것이다. 본 발명은 rAAV 생성물 및 듀켄씨근이영양증의 치료에서의 rAAV의 사용 방법을 제공한다.

근이영양증(MD)은 유전병들의 집합체이다. 상기 집합체는 움직임을 제어하는 골격근의 진행성 약화 및 퇴화를 특징으로 한다. 일부 형태의 MD는 유아기 또는 아동기에 발병하는 반면, 다른 형태의 경우는 중년, 또는 그 이후에 나타날 수도 있다. 상기 장애는 근 약화의 분포 및 정도(일부 형태의 MD는 심근에도 영향을 미침), 발병 연령, 진행 속도 및 유전 패턴의 측면에서 달라진다.
MD의 한 형태가 듀켄씨 근이영양증(DMD)이다. 이 질병은 신생남아 5000명 중 1명에게 영향을 미치는 가장 일반적인 중증 소아 근이양증이다. 결실 돌연변이와는 달리, DMD 엑손 중복은 디스트로핀병증 환자의 비편협적(unbiased) 샘플에서 질병-유발 돌연변이의 약 5%를 차지하는데 [Dent et al., Am. J. Med. Genet., 134(3): 295-298 (2005)], 돌연변이의 일부 카탈로그의 경우, 중복의 수가 훨씬 많다[United Dystrophinopathy Project in Flanigan et al., Hum. Mutat., 30(12): 1657-1666 (2009)에서 출판된 것을 포함(여기서는 11%)].

AAV8-유사 AAV는 다양한 단백질을 인코딩하는 DNA들을 전달하기 위한 rh.74로 불린다. Xu et al., Neuromuscular Disorders, 17: 209-220 (2007) 및 Martin et al., Am. J. Physiol. Cell. Physiol., 296: 476-488 (2009)은 듀켄씨 근이영양증에 대한 세포독성 T 세포 GalNAc 전이효소의 rh.74 발현에 관한 것이다. Rodino-Klapac et al., Mol. 상기 근이영양증은 개인, 가족 및 지역사회에 중대한 영향을 미치는, 확인된 치료법이 없는 질병군이다. 비용은 계산이 불가능할 정도이다. 개인은 정서적 스트레스 및 자긍심의 손실과 관련하여 삶의 질의 저하로 고통을 받는다. 사지 기능의 손실에 의한 극심한 신체적 문제는 일상적인 활동을 어렵게 만든다. 가족 역동은 금전적 손실 및 가족구성원간 관계의 문제를 통해 고통을 받는다. 환자의 형제자매들은 소원해지고, 배우자간 갈등은, 특히 근 이영양증의 책임이 부모인 두 배우자 중 한 명의 발 아래 놓일 경우, 종종 이혼으로 이어진다. 치료법을 찾기 위한 노력의 부담은 종종 삶의 모든 측면을 어지럽히고 힘들게 하는 평생의 집요한 노력이 된다. 가족을 넘어,지역사회는 특수 교육 및 특수 운송수단으로 근 이양증 환자의 장애에 부합하는 부가적 시설들의 필요로 인한 재정적 부담을 떠안고, 재발되는 호흡기 감염 및 심장관련 합병증을 치료하기 위한 되풀이되는 입원의 비용을 떠안는다. 재정적 책임은 주정부 및 연방 정부의 관련 기관들이 나눠 가지며, 이와 같은 책임은 납세의무가 있는 지역 사회로 확장된다.
따라서, DMD를 비롯한 근이영양증을 위한 치료에 대한 필요가 당해 기술에 존재한다.
본 발명은 DMD 유전자의 엑손 2 의 중복을 수반하는 DMD를 예방하고, 이것의 진행을 지연 및/또는 이것을 치료하기 위한 방법 및 생성물을 제공한다. 상기 방법은 U7 소핵 RNA 및 엑손 2 표적 안티센스 서열을 인코딩하는 폴리뉴클레오티드 작제물 , 즉 “엑손 2-표적 U7snRNA 폴리뉴클레오티드 작제물”를 위한 전달 벡터로서 AAV 를 사용하는 것을 수반한다. 예를 들어, 상기 폴리뉴클레오티드 작제물이 rAAV rh.74의 게놈, rAAV6의 게놈 또는 rAAV9의 게놈에 삽입된다. 실시예
본 발명의 측면 및 구현예들이 하기 실시예들에 의해 설명된다.
실시예 1
AAV rh.74의 단리
선형 롤링 원형 증폭(Linear Rolling Circle Amplification)이라는 새로운 기법을 사용하여 붉은털원숭이 림프절에서 고유한 AAV 혈청형을 단리하였다. 상기 LRCA 공정을 사용하여, 이중가닥 원형 AAV 게놈을 여러 붉은털원숭이에서 증폭하였다. 상기 방법은 phi29 파지 DNA 중합체 분해효소 및 AAV 특이적 프라이머를 사용하는 등온대 롤링 원형 증폭에 의해 원형 AAV 게놈을 증폭시키는 능력에 기초한다. LRCA 생성물은 전장 AAV Rep-Cap 분자형 클론이 단리된 원형 AAV 게놈의 인접한 머리-꼬리 어레이(head-to-tail arrays)이다. 4개의 단리물을 순서대로 나열하고, Rep 및 Cap ORF에 대해 예측된 아미노산 서열을 배열하여 이전에 공개된 혈청형(표)과 비교하였다. VP1 단백질 서열을 분석하여 상기 NHP AAV clades D, E 및 AAV 4-유사 바이러스 단리물에 상동성을 드러냈다. Rep78(표의 최상부) ORF의 분석은 AAV 1과 강력한 상동성(98~99%)을 드러내었다.

실시예 2
DMD 모형
DMD 엑손 2 중복의 모형의 예에는 다음과 같이 체내 및 체외 모형이 포함된다.
mdx dup2 마우스 모형
Dmd 부위 내에 엑손 2 의 중복을 갖는 마우스를 개발하였다. 엑손 2 중복 변이는 가장 일반적인 인간 중복 변이로, 비교적 심각한 DMD를 초래한다.
우선, White et al., Hum. Mutat., 27(9): 938-945 (2006)에서 유래된, 11개의 상이한 인간 엑손 2 중복의 최대 범위를 MLPA 및 장거리 PCR로 살펴보았다. 결과를 도 10에 나타내었다. 도 10에서, 각각의 수직바는 MLPA 프로브의 대략적 위치를 표시한다. 음영표시된 기둥은 확인된 2개의 빈발(hotspot) 영역을 표시하고; 이들은 엑손 2 카세트의 상동성에 의한 마우스 삽입의 위치를 판단하기 위해 사용하였다.
상기 삽입 벡터의 맵을 도 11에 나타내었다. 상기 맵에서, 숫자는 복제 부위, 엑손 및 제한 부위의 상대적 위치를 표시한다. 네오 카세트는 상기 유전자와 동일한 방향에 존재하고, 상기 삽입 지점은 정확하게 인트론2의 32207/32208 bp 지점이다. 최소한 150bp 엑스트라 인트론 서열이 삽입된 엑손 2의 각 측면에 유지되고, E2 영역은 1775-2195bp이다. 엑손 2 및 인트론 2의 크기는 각각 62bp 및 209572bp이다.
수컷 C57BL/6 ES 세포에 상기 엑손2 작제물을 가진 벡터를 형질주입하고, 이어서 삽입을 PCR로 확인하였다. 하나의 양호한 클론을 발견하여 증폭시키고, 수십개의 알비노 BL/6 배반포에 주입하였다. 주입된 배반포를 수혜 마우스에 이식하였다. 수컷 키메라에서 유래된 상기 디스트로핀 유전자를 PCR로, 그리고 이어서 RT-PCR로 확인하였다. 콜로니를 확장시켜서 동형접합성으로 사육된 일부 암컷 마우스를 포함시켰다.

불멸 및 조건에 따라 유도가능한(conditionally inducible) 섬유 MyoD 세포주
포유류 섬유아세포에서의 MyoD 유전자의 발현은 세포의 근원성 계통로의 이행분화를 초래한다. 이와 같은 세포는 추가로 근관으로 분화될 수 있고, 이들은 DMD 유전자를 비롯하여 근육 유전자를 발현한다 .
테트라사이클린-유도가능 프로모터의 통제 하에 조건적으로 MyoD를 발현하는 불멸 세포주를 생성하였다. 이것은 tet-유도가능 MyoD인 렌티바이러스의 1차 섬유아세포주의 안정적인 형질주입 및 인간 텔로머라아제 유전자(TER)의 함유에 의해 달성하였다. 수득된 안정적 세포주는 독시사이클린 처리에 의해 MyoD 발현이 개시되도록 하였다. 이와 같은 세포주를 엑손 2의 중복을 갖는 DMD를 앓는 환자에게서 생성하였다.
상기 세포주를 사용하여, Dr. Steve Wilton (Perth, Australia)에 의해 제공된 2'O-메틸 안티센스 올리고머(AONs)를 사용한 중복 스키핑을 입증하였다. 다수의 세포주를 검사하였다. 예시적 세포주에서 수득한 결과들을 도 3에 나타내었다.
일시적으로 MyoD-형질주입된 1차 세포주
아데노바이러스-MyoD이 일시적으로 형질주입된 1차 섬유아세포주를 사용하는 원리확인 실험(Proof-of-principle experiment)을 수행하였다. 상기 아데노바이러스 작제물을 세포 게놈에 통합하지 않았으나, MyoD가 일시적으로 발현되었다.(비록 안정적으로 형질주입된 세포주에서 재현가능성이 뛰어났지만) 수득된 DMD 발현은 엑손-스키핑 실험을 수행하기에 충분하였다.
실시예 3
엑손 2 중복 변이체에서의 U7 snRNA-중재 스키핑의 효과성
중복된 엑손의 바이러스성-매개 엑손-스키핑을 위한 생성물 및 방법을 개발하였다. Goyenvalle et al., Science, 306(5702): 1796-1799 (2004) 또는 Goyenvalle et al., Mol. Ther., 20(6): 179601799 (2004)에 기술된 U7snRNA 시스템과 비교하여, 상기 생성물 및 방법을 변형하였다.
U7snRNA을 변형하여, 주어진 표적 엑손에서의 스플라이싱을 방해하는 표적 안티센스 서열을 포함하게 하였다(도 4). 구체적으로, 실시예 2에 기술된 AON 연구 결과에 기초하여, 4개의 새로운 엑손 2 표적 서열을 설계하였다.
상기 엑손 2 표적 서열을 포함하는 U7 snRNA 작제물을 생성하였다. 각각의 U7 snRNA 작제물은 표적 서열들 중 하나를 포함하였다. 다른 엑손을 선택하기 위해 표적이 된 U7 snRNA 작제물 또한 생성하였다(상기의 MyoD-이행분화된 세포주 연구를 기반으로). 이어서, 하나 이상의 U7 snRNA 작제물을 포함하는 게놈을 갖는 자가보완적(SC) AAV 벡터를 생산하였다.
세포 배양에서의 실험을 위해, 그리고 Dup2 마우스에서의 근육내 주사를 위해, rAAV1 벡터를 활용하였다. HEK293 세포에서의 아데노바이러스 없는, 트리플 플라스미드 DNA 형질주입(CaPO4 침전) 방식[Rabinowitz et al., J. Virol., 76:791-801 (2002)]에 의해, 원하는 벡터 게놈을 포함하는 플라스미드를 사용하는 변형된 교차-포장 접근법에 의해 원하는 AAV 혈청형의 재조합형 SC AAV 벡터를 생산하였다. 앞서 기술된 방식[Wang et al., Gene. Ther., 10:1528-1534 (2003)]과 유사한 방식으로, AAV 헬퍼 플라스미드 및 아데노바이러스 헬퍼 플라스미드로의 공동 형질주입에 의해 벡터를 생산하였다. 아데노바이러스 헬퍼 플라스미드(pAd헬퍼)는 아데노바이러스 유형 5 E2A, E4ORF6 및, 고역가 rAAV 생산에 필요한 VA I/II RNA 유전자를 발현한다.
[Clark et al., Hum. Gene Ther, 10:1031-1039 (1999)]에서 앞서 기술된 바와 같이, 순차적 아이오디사놀 구배 정제법 및, 선형 NaCl 염 구배를 사용하는 음이온-교환 컬럼 크로마토그래피에 의해, 정화된 293 세포 용해물에서 벡터를 정제하였다. 앞서 기술된 바(Clark et al., supra)와 같이, Prism 7500 Taqman 검출기(PE Applied Biosystems)를 활용하는 특이적 프라이머/프로브 세트로 QPCR-기반 검출을 수행하여, 벡터 게놈(vg) 역가를 측정하였다. 벡터 보관 역가의 범위는 1 내지 10 x 1012 vg/mL이다.
Dup2 불멸 인간 섬유아세포에 형질도입되는 SC rAAV 벡터를 사용하는 RT-PCR로 초기 엑손-스키핑 분석을 수행하였다. 독시사이클린의 통제 하에 근 계통 세포로 이행분화할 수 있는 Dup 2 불멸 인간 섬유아세포를, 텔로머라아제-발현 및 tet-유도가능-MyoD 발현 벡터의 형질도입을 통해, 생산하였다. 이어서, 전환된 인간 섬유아세포(FM)에 엑손 2 안티센스 서열를 포함하는 상이한 U7 작제물들을 갖는 SC rAAV 를 형질도입하였다.
세 가지 상이한 안티센스 서열을 갖는 SC rAAV.1-U7 작제물에 대한 RT-PCR 결과를 도 5 에 나타내었다. 도 5에서, "(4C)”는 상기 U7 작제물의 4개 카피가 벡터 게놈에 포함되었음을 표시하고, “+”는 고용량을 표시하고, “U7_ACCA A=Along”(도 8의 플라스미드 맵에 나타내었고, 이것의 서열(서열번호: 2)이 도 9에 명시됨)는 서열에 하기의 엑손 2-표적 U7 snRNA 폴리뉴클레오티드 작제물 4개를 포함하는 벡터 게놈을 표시한다: 제1 U7Along 작제물, 제1 U7C 작제물, 제2 U7C 작제물 및 제2 U7Along 작제물. 보다시피, 상기 U7_ACCA A-Along SC rAAV(본원의 다른 곳에서는 줄여서U7_ACCA SC rAAV1라고 명명함)는 다른 기타 벡터 작제물과 비교하여 더 높은 비율의 엑손 2 스키핑을 달성하였다.
추후 실험에서, 체내 엑손-스키핑 효율을 분석하였다. Dup2 마우스 근육내 주입을 위해, 가장 효율적인 AAV-U7 벡터인 U7_ACCA SC rAAV1를 선택하였다. 결과를 아래 도 6(a-d)에 나타내었는데, (a)는 상기 단백질 발현이 복원되고, 여러 근 섬유 중 막에 적절하게 자리 잡은 디스트로핀 염색을 나타내고; (b)는 웨스턴 블롯에 의해 단백질 복원을 확인하였음을 나타낸다. RT-PCR는 (c) Dup2 마우스에서 용량-의존적 단일 또는 이중 스키핑 및 (d) 야생형 마우스에서 효율적인 스키핑을 나타낸다.
따라서, 엑손 2를 스키핑하도록, 고효율의 AAV-매개 U7snRNA를 설계하여, 하부근초 디스트로핀 복원을 허용하였다. 심장 기능; EDL 및 횡격박 힘 평가; 및 러닝머신 및 악력 검사가 미처리된 마우스와 처리된 마우스 사이에서 비교된다. 주입된 근육 내에서 검출가능한 디스트로핀 발현 정도에 기초하여, 첫 번째 무리에 1E11 vg/kg을 정맥내로 전달하고, 이어서 두 번째 무리에 1 log 더 높게 투여하기 위한 실험용으로 U7_ACCA SC rAAV를 선택하였다. 주입은 4주차에 수행하고, 앞서 기술된 바와 같이, 10주 및 24주 차에 생리학적 평가 및 조직병리학으로 동물들(n= 8마리(한 무리당))을 평가하였다.
실시예 4
AAV1에의한 U7-ACCA 의 근육내 전달이 Dup2 마우스에서의 유의미한 N-절삭 디스트로핀 발현을 초래한다
실시예 3에 기술된 방법들로 도 9의 게놈 삽입물을 포함하는 rAAV1을 생산하였다. 이어서, 근육내 주사를 통해, 상기의 AAV.1U7-ACCA를 Dup2 마우스에 투여하였다.
5e11vg AAV.1U7-ACCA의 TA 근육내 주사 후 4주가 지나, DMD mRNA에 RT-PCR을 수행한 결과는 Dup2 동물에서 엑손 2 의 양쪽 카피의 거의 완벽한 스키핑을 보여주었다[도 12(a)].
감염 1개월 후, C-말단 항체(PA1-21011, ThermoScientific)를 사용하는 면역블롯을 수행한 결과, Dup2 및 대조군 Bl6 마우스 양쪽 모두에서 N-절삭 동형단백질(별표)의 유의미한 발현이 나타났다[도 12(b)]. U7-ACCA를 주사한 Bl6 수컷에서 유도된 단백질은 Dup2 처리된 동물에서 발현된 것과 크기가 동일하였고, 이로써 본 단백질과 전장(full-length) 동형단백질 사이의 크기 차이를 확인하였다.
디스트로핀, β-디스트로글리칸 및 뉴런 산화질소 합성효소의 면역형광 염색은 디스트로핀 관련 복합체의 구성성분들의 복원을 입증하였다[도 12(c)].
미처리된 Dup2 동물에서의 강축(tetanic contraction) 후 정규화된 비력(specific force)은 유의미하게도 Bl6 마우스에서보다 작었다. AAV1.U7-ACCA 단독으로, 또는 프레드니손과 병용한 근육내 주사는 Bl6 마우스에서 나타난 것과 크게 다르지 않은 수준으로 힘을 증가시켰다. 미처리된 Dup2 마우스와 (Dup2+PDN)과 병용하여 프레드니손으로 처리된 마우스 사이에 유의미한 차이는 관측되지 않았다[도 12(d)]. 이와 같은 검사의 경우, 공개된 프로토콜[Hakim et al., Journal of Applied Physiology, 110: 1656-1663 (2011)]을 사용하여, 정규화된 비력을 평가하였다.
처리는 공개된 프로토콜(Hakim et al., supra)에 의해 평가된 바와 같이, 반복적인 비정상적 수축 후 비력 손실로부터 Dup2 근육을 유의미하게 보호하였다. AAV1.U7-ACCA 단독으로의 Dup2 마우스 처리는, 미처리된 Dup2 마우스와 비교하여, 통계적으로 유의미한 향상을 초래하였다. AAV1.U7-ACCA 및 프레드니손의 조합은 3회 내지 10회 수축 후, 힘 유지에 있어서, 대조군 Bl6 마우스와 비교하여, 유의미한 차이를 나타내지 않았다[도 12(e)].
실시예 5
Dup2 마우스 모형의 AAV9-U7_ACCA의 정맥내 주사는 N-절삭 동형단백질의 유의미한 발현 및 근력 부족의 수정을 초래한다
주입된 근육 내에서 검출가능한 디스트로핀 발현의 정도를 기초로, 추가 실험을 위해 U7_ACCA SC rAAV를 정맥내로 전달하기로 결정하고, 알려진 조직 분배 특성을 기초로 혈청형 rAAV9를 선택하였다.
실시예 3에 기술된 방법으로, 도 9의 게놈 삽입물을 포함하는 rAAV9을 생산하였다. 이어서, 상기 AAV.9U7-ACCA를 Dup2 마우스에 투여하였다. 첫 번째 무리에는 꼬리 정맥을 통해 3.3E112 vg/kg를 주사하였다. 4주령에 주사를 투여하였다.
AAV9.U7-ACCA의 꼬리 정맥 주사(3.3E12 vg/kg) 후 1개월이 지나서, 5개의 상이한 Dup2 마우스 근육에 RT-PCR를 수행하였다[도 13(a)]. 다중 전사(Dup2, wt, 및 Del2로 표지됨)의 존재에 의해 입증된 바와 같이, U7-ACCA 처리는 모든 검사 대상 근육에서 엑손 2의 카피 1개 또는 2개 모두의 스키핑을 강제할 수 있었다(TA: 전경골근; Gas: 비복근; ♥: 심장; Tri: 삼두근; dia: 횡격막).
주사 후 1개월이 지나 5개의 상이한 근육에 C-말단 항체(PA1-21011, ThermoScientific)를 사용하는 웨스턴 블롯을 수행한 결과, 모든 검사대상 근육에서 디스트로핀의 존재가 입증되었다[도 13(b)].
동일 샘플 상에서 C-말단 항체(PA1-21011, ThermoScientific)를 사용하는 디스트로핀의 면역염색은 근초에서 디스트로핀 발현 및 이것의 적절한 자리잡기를 확인시켜주었다[도 13(c)].
앞다리 및 뒷다리 모두의 악력(grip strength) 평가는 AAV9.U7-ACCA으로 처리된 Dup2 동물에서 악력의 완전한 수정을 입증하였다[도 13(d)]. 공개된 프로토콜[Spurney, et al., Muscle & Nerve, 39, 591-602 (2009)]을 사용하여 이와 같은 검사를 수행하였다.
공개된 프로토콜[Hakim et al., supra)을 사용하여, 미처리된 Dup2 동물[도 13(e)]과 비교하자, 정규화된 비력 및 강축 후 총 힘은 근력의 향상을 나타내었다.
심장 꼭지근(papillary muscles)이, 공개된 프로토콜[Janssen et al., Am J Physiol Heart Circ Physiol., 289(6):H2373-2378 (2005)]을 사용하여, 처리된 동물에서 길이-의존적 힘 생성의 향상을 입증하였다[도 13(f)].
본 발명이 특정 구현예의 측면에서 기술되었으나, 변경 및 변형이 발생하리라는 점이 당해기술의 숙련가에게 이해된다. 따라서, 청구항에서 나타난 바와 같은 제한만이 본 발명에 적용될 것이다.
