Abstract

우마나(Umana, P.) 등의 문헌[Nature Biotechnol. 17 (1999) 176-180] 및 US 6,602,684 호에 기재되어 있는 바와 같이 단클론성 항체의 올리고사카라이드 성분을 조작함으로써 단클론성 항체의 세포-매개되는 실행기(effector) 기능을 향상시킬 수 있다. 암 면역요법에 가장 통상적으로 사용되는 항체인 IgG1 형 항체는 각 CH2 도메인의 Asn297에 보존된(conserved) N-연결된 당화 부위를 갖는 당단백질이다. 벤다무스틴[상표명 리보무스틴(Ribomustin) 및 트린다(Treanda); SDX-105로도 알려져 있음]은 만성 림프구성 백혈병(CLL)[케이쓰(Kath, R.) 등, J. Cancer Res. Clin. Oncol. 127 (2001) 48-54] 및 비-호지킨 림프종(NHL)의 치료에 사용되는 질소 머스타드이다. 이는 알킬화제로 불리는 약물의 부류에 속한다. 이는 또한 육종의 치료에 대해서도 연구되고 있다[백치(Bagchi, S.), Lancet Oncol. 8 (2007) 674].

놀랍게도, 본 발명자들은 최근 어푸코실화된 항-CD20 항체와 벤다무스틴의 조합이 어푸코실화되지 않은 CD20 항체인 리툭시맙과의 조합에 비해 상승작용적인(예를 들어, 상가작용보다 더 많음) 항증식 효과를 나타냄을 발견하였다.
본 발명은 벤다무스틴과 조합하여 암 치료용 약제를 제조하기 위한, Asn297에서 올리고사카라이드(당)의 총량의 60% 이하의 푸코즈 양을 갖는 어푸코실화된 항-CD20 항체의 용도를 포함한다.
본 발명의 한 양태는 암 치료가 필요한 환자에게, 벤다무스틴과 조합하여, Asn297에서 올리코사카라이드(당)의 총량의 60% 이하의 푸코즈 양을 갖는 어푸코실화된 항-CD20 항체를 투여함으로써, 암 환자를 치료하는 방법이다.
본 발명의 다른 양태는 벤다무스틴과 조합하여 암을 치료하기 위한, Asn297에서 올리고사카라이드(당)의 총량의 60% 이하의 푸코즈 양을 갖는 어푸코실화된 항-CD20 항체이다.

용어 "키메라 항체"는 통상적으로 재조합 DNA 기법에 의해 제조되는, 하나의 공급원 또는 종으로부터의 가변 영역, 즉 결합 영역, 및 상이한 공급원 또는 종으로부터 유래된 불변 영역의 적어도 일부를 포함하는 단클론성 항체를 말한다. 쥐의 가변 영역 및 인간의 불변 영역을 포함하는 키메라 항체가 특히 바람직하다. 이러한 쥐/인간 키메라 항체는 쥐의 면역 글로불린 가변 영역을 코딩하는 DNA 분절 및 사람의 면역 글로불린 불변 영역을 코딩하는 DNA 분절을 포함하는 발현된 면역 글로불린 유전자의 생성물이다. 본 발명에 의해 포괄되는 "키메라 항체"의 다른 형태는 등급 또는 소등급이 원래 항체로부터 변형 또는 변화된 것이다. 이러한 "키메라" 항체는 또한 "등급-변경된 항체"라고도 불린다. 키메라 항체를 생성시키는 방법은 현재 당 업계에 널리 공지되어 있는 통상적인 재조합 DNA 및 유전자 형질감염 기법을 포함한다. 예를 들어, 모리슨(Morrison, S.L.) 등의 문헌[Proc. 용어 "인간화된 항체"는 구조 형성 영역 또는 "상보성 결정 영역"(CDR)이 모 면역 글로불린과 비교하여 상이한 특이성의 면역 글로불린의 CDR을 포함하도록 변형된 항체를 말한다. 바람직한 실시양태에서는, 쥐의 CDR을 인간 항체의 구조 형성 영역으로 그라프팅하여 "인간화된 항체"를 제조한다. 예컨대, 리치만(Riechmann, L.) 등의 문헌[Nature 332 (1988) 323-327] 및 노이버거(Neuberger, M.S.) 등의 문헌[Nature 314 (1985) 268-270]을 참조한다.
본원에 사용되는 용어 "인간 항체"는 인간 생식선 면역 글로불린 서열로부터 유래되는 가변 영역 및 불변 영역을 갖는 항체를 포함하고자 한다. 인간 항체는 당 업계에 널리 공지되어 있다[반 디지크(van Dijk, M.A.) 및 반 드 윙클(van de Winkel), Curr. Opin. Pharmacol. 5 (2001) 368-374]. 이러한 기술에 기초하여, 매우 다양한 표적에 대한 인간 항체를 생성시킬 수 있다. 인간 항체의 예는 예컨대 켈러만(Kellermann, S.A.) 등의 문헌[Curr. Opin. Biotechnol. 13 (2002) 593-597]에 기재되어 있다.
본원에 사용되는 용어 "재조합 인간 항체"는 NS0 또는 CHO 세포 같은 숙주 세포로부터 또는 인간 면역 글로불린 유전자를 위해 유전자 이식된 동물(예를 들어, 마우스)로부터 단리된 항체, 또는 숙주 세포 내로 형질감염된 재조합 발현 벡터를 사용하여 발현된 항체 같은 재조합 수단에 의해 제조, 발현, 창조 또는 단리되는 모든 인간 항체를 포함하고자 한다. 이러한 재조합 인간 항체는 인간 생식선 면역 글로불린 서열로부터 유래되는 가변 영역 및 불변 영역을 재배열된 형태로 갖는다. 본 발명에 따른 재조합 인간 항체를 생체 내에서 체세포 초돌연변이시켰다. 따라서, 재조합 항체의 VH 및 VL 영역의 아미노산 서열은 인간 생식선 VH 및 VL 서열로부터 유래되고 그에 연관되기는 하지만 생체 내에서 인간 항체 생식선 목록 내에 자연적으로 존재할 수 없다.
본원에 사용되는 용어 "결합" 또는 "특이적으로 결합"은 정제된 야생형 항원을 사용하는 생체 외 검정, 바람직하게는 플라스몬 공명 검정[비아코어(BIAcore), 지이-헬쓰케어 업살라(GE-Healthcare Uppsala), 스웨덴]에서 종양 항원의 항원결정부로의 항체의 결합을 가리킨다. 결합 친화력은 용어 ka(항체/항원 복합체로부터의 항원의 결합에 대한 반응 속도 상수), kD(해리 상수) 및 KD(kD/ka)에 의해 정의된다. 결합 또는 특이적으로 결합은 10- 8몰/l 이하, 바람직하게는 10-9M 내지 10- 13몰/l의 결합 친화력(KD)을 의미한다. 따라서, 본 발명에 따른 어푸코실화된 항체는 10- 8몰/l 이하, 바람직하게는 10-9M 내지 10- 13몰/l의 결합 친화력(KD)으로 종양 항원에 특이적으로 결합한다.
본원에 사용되는 용어 "핵산 분자"는 DNA 분자 및 RNA 분자를 포함하고자 한다. 핵산 분자는 단일 가닥 또는 이중 가닥일 수 있으나, 바람직하게는 이중 가닥 DNA이다.
"불변 도메인"은 항체를 항원에 결합시키는데 직접적으로 관련되지 않고, 실행기 기능(ADCC, 보체 결합 및 CDC)에 관련된다.
본원에 사용되는 "가변 영역"(경쇄의 가변 영역(VL), 중쇄의 가변 영역(VH))은 항체를 항원에 결합시키는데 직접적으로 관련되는 각각의 경쇄 및 중쇄 쌍을 말한다. 가변 인간 경쇄 및 중쇄의 도메인은 동일한 전체적인 구조를 갖고, 각 도메인은 3개의 "초가변 영역"(또는 상보성 결정 영역, CDR)에 의해 연결된, 그의 서열이 광범위하게 보존된 4개의 구조 형성 영역(FR)을 포함한다. 구조 형성 영역은 b-시트 구조를 채택하고, CDR은 b-시트 구조를 연결하는 루프를 형성할 수 있다. 각 쇄의 CDR은 구조 형성 영역에 의해 그의 3차원 구조로 유지되며, 다른 쇄로부터의 CDR과 함께 항원 결합 부위를 형성한다.

벤다무스틴은 4-[5-[비스(2-클로로에틸)아미노]-1-메틸벤즈이미다졸-2-일]뷰탄산이다. 상표명은 리보무스틴 및 트린다이고; 벤다무스틴은 또한 SDX-105로도 알려져 있다. 벤다무스틴은 만성 림프구성 백혈병(CLL)[케이쓰 등, J. Cancer Res. Clin. Oncol. 127 (2001) 48-54] 및 비-호지킨 림프종(NHL)의 치료에 사용되는 질소 머스타드이다. 이는 알킬화제로 불리는 약물의 부류에 속한다. 이는 또한 육종의 치료에 대해서도 연구되고 있다[백치, Lancet Oncol. 8 (2007) 674].
용어 "어푸코실화된 항체"는 감소된 푸코즈 잔기 수준을 갖는 Asn297에서의 Fc 영역의 변화된 당화 패턴을 갖는 IgG1 또는 IgG3 아이소타입(바람직하게는 IgG1 아이소타입)의 항체를 가리킨다. 인간 IgG1 또는 IgG3의 당화는 2개 이하의 Gal 잔기로 종결되는 코어 푸코실화된 바이안테너리 복합 올리고사카라이드 당화로서 Asn297에서 일어난다. 이들 구조는 말단 Gal 잔기의 양에 따라 G0, G1(α1,6 또는 α1,3) 또는 G2 글라이칸 잔기로서 지칭된다[라주(Raju, T.S.), BioProcess Int. 1 (2003) 44-53]. 항체 Fc 부분의 CHO 형 당화는 예를 들어 루티어(Routier, F.H.)의 문헌[Glycoconjugate J. 14 (1997) 201-207]에 기재되어 있다. 당화 재조정되지 않은 CHO 숙주 세포에서 재조합 기법으로 발현된 항체는 통상 85% 이상의 양으로 Asn297에서 푸코실화된다. 본원에 사용되는 용어 어푸코실화된 항체는 그의 당화 패턴에 푸코즈를 갖지 않는 항체를 포함하는 것으로 이해되어야 한다. 항체에서의 전형적인 당화 잔기 위치는 EU 명명법에 따라 위치 297의 아스파라긴("Asn297")인 것으로 통상적으로 알려져 있다.
면역 글로불린 중쇄 불변 영역에서의 잔기를 칭하는 경우에 "EU 명명법" 또는 "EU 색인"을 통상적으로 이용한다[예컨대, 본원에 명백하게 인용되는 카밧 등의 문헌(Sequences of Proteins of Immunological Interest, 5th Ed. Public Health Service, 국립보건원, 미국 메릴랜드주 베데스다(1991))에 보고된 EU 색인].

그러므로, 본 발명의 한 양태는 벤다무스틴과 조합하여 암 치료용 약제를 제조하기 위한, Asn297에서 올리고사카라이드(당)의 총량의 60% 이하의 푸코즈 양을 갖는 CD20에 특이적으로 결합하는 IgG1 또는 IgG3 아이소타입(바람직하게는 IgG1 아이소타입)의 어푸코실화된 항-CD20 항체의 용도이다. 하나의 실시양태에서, 푸코즈의 양은 Asn297에서 올리고사카라이드(당)의 총량의 40% 내지 60%이다.
CD20[B-림프구 항원 CD20, B-림프구 표면 항원 B1, Leu-16, Bp35, BM5 및 LF5로도 알려져 있음; 서열은 스위스프롯(SwissProt) 데이터베이스 엔트리 P11836을 그 특징으로 함]은 전-B 및 성숙 B 림프구 상에 위치된 약 35kD의 분자량을 갖는 소수성 막통과 단백질이다[발렌타인 등, J. Biol. 용어 "CD20" 및 "CD20 항원"은 본원에서 호환성 있게 사용되며, 세포에 의해 자연적으로 발현되거나 CD20 유전자로 형질감염된 세포 상에서 발현되는 인간 CD20의 임의의 변형체, 이소폼(isoform) 및 종 호몰로그를 포함한다. 본 발명의 항체를 CD20 항원에 결합시키면 CD20을 불활성화시킴으로써 CD20을 발현하는 세포(예컨대, 종양 세포)의 죽음을 매개한다. CD20을 발현하는 세포의 죽음은 하기 기작중 하나 이상에 의해 일어날 수 있다: 세포사/세포자살 유도, ADCC 및 CDC.

본 발명에 따른 용어 "항-CD20 항체"는 CD20 항원에 특이적으로 결합하는 항체이다. 항-CD20 항체의 CD20 항원으로의 결합 특성 및 생물학적 활성에 따라, 크래그 등의 문헌[Blood 103 (2004) 2738-2743]; 및 크래그 등의 문헌[Blood 101 (2003) 1045-1051]에 따라 항-CD20 항체의 두 유형(유형 I 및 유형 II 항-CD20 항체)이 구분될 수 있다. 표 2를 참조한다.

유형 II 항-CD20 항체의 예는 예를 들어 인간화된 B-Ly1 항체 IgG1(WO 2005/044859 호에 개시된 키메라 인간화 IgG1 항체), 11B8 IgG1(WO 2004/035607 호에 개시됨) 및 AT80 IgG1을 포함한다. 전형적으로, IgG1 아이소타입의 유형 II 항-CD20 항체는 특징적인 CDC 특성을 보여준다. 유형 II 항-CD20 항체는 IgG1 아이소타입의 유형 I 항체에 비해 감소된 CDC(IgG1 아이소타입의 경우)를 갖는다.
유형 I 항-CD20 항체의 예는 예를 들어 리툭시맙, HI47 IgG3(ECACC, 하이브리도마), 2C6 IgG1(WO 2005/103081 호에 개시됨), 2F2 IgG1(WO 2004/035607 호 및 WO 2005/103081 호에 개시됨) 및 2H7 IgG1(WO 2004/056312 호에 개시됨)을 포함한다.
본 발명에 따른 어푸코실화된 항-CD20 항체는 하나의 실시양태에서는 유형 II 항-CD20 항체이고, 더욱 구체적인 실시양태에서는 유형 II 항-CD20 항체는 어푸코실화된 인간화 B-Ly1 항체이다.
본 발명에 따른 어푸코실화된 항-CD20 항체는 푸코즈가 감소되지 않은 항-CD20 항체와는 달리 증가된 항체 의존성 세포 독성(ADCC)을 갖는다.
"증가된 항체 의존성 세포 독성(ADCC)을 갖는 어푸코실화된 항-CD20 항체"는 어푸코실화된 항-CD20 항체를 의미하는데, 이 용어가 본원에서는 당 업자에게 공지되어 있는 임의의 적합한 방법에 의해 결정할 때 증가된 ADCC를 갖는 것으로 정의되기 때문이다. 하나의 인정된 생체 외 ADCC 검정은 다음과 같다:
1) 검정은 항체의 항원-결합 영역에 의해 인지되는 표적 항원을 발현하는 것으로 알려진 표적 세포를 사용한다;
2) 검정은 실행기 세포로서, 무작위적으로 선택된 건강한 공여자의 혈액으로부터 단리된 인간 말초 혈액 단핵 세포(PBMC)를 사용한다;
3) 하기 프로토콜에 따라 검정을 수행한다:
i) 표준 밀도 원심분리 절차를 이용하여 PBMC를 단리하고, RPMI 세포 배지 중에 5×106개 세포/ml로 현탁한다;
ii) 표적 세포를 표준 조직 배양 방법에 의해 생육시키고, 90%보다 높은 생존률로 대수증식기로부터 수획하며, RPMI 세포 배지 중에서 세척하고, 51Cr 100마이크로큐리(micro-Curies)로 라벨링하며, 세포 배지로 2회 세척하고, 105개 세포/ml의 밀도로 세포 배지에 재현탁시킨다;
iii) 상기 최종 표적 세포 현탁액 100㎕를 96개-웰 미소적정판의 각 웰에 옮겨넣는다;
iv) 항체를 세포 배지 중에서 4000ng/ml로부터 0.04ng/ml로 연속 희석시키고, 생성되는 항체 용액 50㎕를 96개-웰 미소적정판의 표적 세포에 첨가하여, 상기 전체 농도 범위를 포괄하는 다양한 항체 농도를 3회씩 시험한다;
v) 최대 방출(MR) 대조용으로서, 라벨링된 표적 세포를 함유하는 판의 3개의 추가적인 웰에 항체 용액(상기 iv) 대신 비-이온성 세제[노니뎃(Nonidet), 시그마(Sigma), 세인트루이스]의 2%(VN) 수용액 50㎕를 넣는다;
vi) 자발적인 방출(SR) 대조용으로서, 라벨링된 표적 세포를 함유하는 판의 3개의 추가적인 웰에 항체 용액(상기 iv) 대신 RPMI 세포 배지 50㎕를 넣는다;
vii) 이어, 96개-웰 미소적정판을 50×g에서 1분간 원심분리하고, 4℃에서 1시간동안 배양한다;
viii) PBMC 현탁액(상기 i) 50㎕를 각 웰에 첨가하여 25:1의 실행기:표적 세포 비를 수득하고, 판을 5% CO2 대기하에 37℃에서 4시간동안 배양기에 넣어둔다;
ix) 각 웰로부터 세포가 없는 상청액을 수획하고, 감마 카운터를 이용하여 실험에 의해 방출된 방사능(ER)을 정량한다;
x) 수학식 (ER-MR)/(MR-SR)×100에 따라 각 항체 농도에 대해 특이적인 세포 용해의 백분율을 계산하는데, 이 때 ER은 그 항체 농도에서 정량된 평균 방사능(상기 ix 참조)이고, MR은 MR 대조용(상기 v 참조)의 경우에 정량된 평균 방사능(상기 ix 참조)이고, SR은 SR 대조용(상기 vi 참조)의 경우에 정량된 평균 방사능(상기 ix 참조)이다;
4) "증가된 ADCC"는 상기 시험된 항체 농도 범위 내에서 관찰되는 특이적인 세포 용해의 최대 백분율의 증가 및/또는 상기 시험된 항체 농도 범위 내에서 관찰된 특이적인 세포 용해의 최대 백분율의 절반을 달성하는데 필요한 항체 농도의 감소로서 정의된다. ADCC의 증가는 상기 검정에 의해 측정되고 동일한 항체에 의해 매개되며 동일한 유형의 숙주 세포에 의해 생성되고 당 업자에게 공지되어 있는 동일한 표준 생성, 정제, 제형화 및 저장 방법을 이용하지만, GnTIII를 과발현하도록 조작된 숙주 세포에 의해 생성되지 않은 ADCC에 대한 것이다.
상기 항체의 당화 조작에 의해 상기 "증가된 ADCC"를 수득할 수 있는데, 이는 우마나 등의 문헌[Nature Biotechnol. 17 (1999) 176-180] 및 US 6,602,684 호에 기재되어 있는 그들의 올리고사카라이드 성분을 조작함으로써 단클론성 항체의 상기 천연, 세포-매개되는 실행기 기능을 향상시킴을 의미한다.
용어 "보체-의존성 세포 독성(CDC)"은 보체의 존재하에서 본 발명에 따른 항체에 의해 인간 종양 표적 세포를 용해시킴을 말한다. 바람직하게는 보체의 존재하에 본 발명에 따른 항-CD20 항체로 CD20 발현 세포의 제제를 처리함으로써 CDC를 측정한다. 항체가 4시간 후에 100nM의 농도에서 종양 세포의 20% 이상의 세포 용해(세포사)를 유도하는 경우 CDC가 발견된다. 바람직하게는 51Cr 또는 Eu 라벨링된 종양 세포 및 방출되는 51Cr 또는 Eu의 측정을 이용하여 검정을 수행한다. 대조용은 보체와 함께, 그러나 항체 없이 종양 표적 세포를 배양함을 포함한다.

약학 조성물

복합 치료의 항종양 활성
시험 약품
어푸코실화된 항-CD20 항체 B-HH6-B-KV1 GE(어푸코실화된 인간화 B-Ly1, 당화 조작된 B-HH6-B-KV1, WO 2005/044859 호 및 WO 2007/031875 호 참조)는 스위스 쉴리렌 소재의 글라이카트(GlycArt)로부터 모용액(c=9.4mg/ml)으로서 제공되었다. 항체 완충액은 히스티딘, 트레할로즈 및 폴리솔베이트 20을 포함하였다. 항체 용액을 주사하기 전에 모용액으로부터 PBS 중에 적절하게 희석하였다.
임상 등급의 리툭시맙(맙테라)은 바젤 소재의 호프만 라 로슈(Hoffmann La Roche)에서 수득하였다.
벤다무스틴(리보무스틴(등록상표))은 독일 림부그크 암 데어 란 소재의 문디파마 게엠베하(Mundipharma GmbH)로부터 구입하였다. 2.5mg/ml의 제조된 모용액으로부터 필요한 희석을 조정하였다.
세포주 및 배양 조건
8% CO2에서 수증기로 포화된 대기에서 37℃에서 10% 태아 소 혈청[피에이에이 래보러토리즈(PAA Laboratories), 오스트리아] 및 2mM L-글루타민으로 보충된 DMEM 중에서 인간 Z138 맨틀세포 림프종 세포주를 통상적으로 배양하였다. 이식에는 4세대를 이용하였다. 세포를 매트리겔(Matrigel)과 함께 동시 주사하였다.
동물
도착시 3 내지 4주령인 암컷 SCID 베이지 마우스[독일 줄즈펠트 소재의 찰스 리버(Charles River)에서 구입함]를 명확한 지침[지브이-솔라스(GV-Solas); 펠라사(Felasa); 티어쉬지(TierschG)]에 따라 12시간/12시간 명-암 1일 사이클로 특이적-병원균-비함유 조건하에 유지하였다. 지방 정부에서 실험 연구 프로토콜을 재검토하고 승인하였다. 도착 후 동물을 1주일간 동물 설비의 격리부에 유지시켜, 새로운 환경 및 관찰에 익숙하게 만들었다. 규칙적으로 지속적인 건강 모니터링을 실시하였다. 식사 음식[프로비미 클리바(Provimi Kliba) 3337] 및 물(pH 2.5 내지 3으로 산성화됨)을 무제한으로 제공하였다.
모니터링
동물들을 매일 임상적 증상 및 부작용의 검출에 대해 검사하였다. 실험 내내 모니터링하기 위하여, 동물의 체중을 매주 2회 서면으로 작성하였고, 스테이징(staging) 후 캘리퍼에 의해 종양 부피를 측정하였다.
동물의 치료
종양 세포를 접종한지 19일 후 무작위로 배정하는 날 동물 치료를 개시하였다. 인간화된 어푸코실화된 항-CD20 항체 B-HH6-B-KV1 GE 또는 리툭시맙을 1mg/kg의 표시된 투여량으로 제19연구일 및 제26연구일에 하루에 4회 단일 약제로서 복강내 투여하였다. 상응하는 비히클을 동일한 날에 투여하였다. 벤다무스틴을 3mg/kg으로 제19일, 제20일, 제21일 및 제22일에 복강내 투여하였다. 복합 요법 군에서는, 화학치료제를 제19일에 두 항체의 8시간 후에 투여하였다.
종양 성장 억제 생체 내 연구
종양 세포를 접종한 후 제33일에, 각각 리툭시맙, 항-CD20 항체 B-HH6-B-KV1 GE, 리툭시맙과 벤다무스틴의 조합, 또는 항-CD20 항체와 벤다무스틴의 조합을 투여한 동물에서는 대조군과 비교하여 표 1에 주어진 바와 같은 종양 성장 억제(TGI)가 존재하였다. 본 실험에서, 벤다무스틴을 단독으로 사용한 치료에서는 어떠한 항종양 활성도 나타내지 않았다.
항-CD20 항체 B-HH6-B-KV1 GE/벤다무스틴 조합 군을 항-CD20 항체 B-HH6-B-KV1 GE 단독으로 사용한 치료와 비교함으로써, 상가작용보다 큰 효과가 있었다.

벤다무스틴과 B-HH6-B-KV1 GE의 조합은 터키-크래머(Tukey-Kramer) 시험(p<0.001)에 의해 분석되는 단독 치료에 비해 상가작용보다 크고(상승작용) 통계학적으로 유의한 효과를 나타내었다(표 3 및 도 1 참조).

Claims (11)
- Asn297에서 올리고사카라이드(당)의 총량의 60% 이하의 푸코즈 양을 갖는 어푸코실화(afucosylating)된 항-CD20 항체 및 벤다무스틴을 포함하는, 암을 치료하기 위한 조성물.
