Description
최근의 연구에서 산화질소 (NO)는 중요한 생물학적 분자인 것으로 나타났다. NO는 숙주 방어, 심혈관 조절, 신호 형질도입, 신경전달 및 상처 치료와 같은 다양한 프로세스에서 중심적인 역할을 한다. 효소 산화질소 신타제 (NOS)는 L-아르기닌을 L-시트룰린 및 NO로 변환시키고, 상처 치료 프로세스와 연관된 수많은 세포들이 NOS 활성을 나타내고 있다. NO의 가능한 주요 역할이 초기 감염 단계중 대식세포 및 다른 식세포에 의해 방출되는 세포장애제 또는 세포증식억제제의 역할이지만, 조직 복구에 있어서 NO의 정확한 기능은 확립되지 않았다. 또한, 상처 잔류 세포로부터 방출되는 NO는, 새롭게 혈관화된 조직이 형성될 때 특유한 세포 신호화 경로 및 미소순환의 재확립에서 중요하다.
NO의 산화에 의해서, 불안정한 중간체 (예를 들어, N2O3및 N2O4) 및 후속적으로 안정한 대사산물 아질산염 (NO2) 및 질산염 (NO3)이 생성된다. 이전의 연구에서는, 뇨중 NO2는 상처입거나 감염된 쥐에서 무시할 정도이고 뇨중 NO3는 NO 생성의정확한 간접적인 척도인 것으로 나타났다.

최근에, 산화질소를 특정 친핵체와 반응시켜 형성된 착물이 새로운 종의 NO-방출 화합물로서 소개되었다. 키퍼 (Keefer)와 공동연구자들은 NONO에이트로 칭해지는 양쪽 이온성 폴리아민/NO 부가물을 합성하였다. 미국특허 제 5,250,550호에서 키퍼 등은 심혈관 약제로서 유용한 하기의 산화질소-폴리아민 착물과 그의 제약상 허용가능한 염을 제시하고 있다.

상기 NONO에이트는 수성 매질에서 정량의 NO를 방출하고 NO 발생 속도 및 양은 pH, 온도 및 친핵성 잔기의 존재에 좌우되는 것으로 보인다.
NONO에이트 화합물은 NO 기재의 혈관확장제로 사용될 수 있고, 항종양 요법과 같은 다른 임상적 적용성을 가질 수 있다. 하지만, 상기 가용성 아민 기재 NONO에이트에 의해 NO를 상처로 전달하는 것은 NO 방출후 잔류하는 독성 아민의 용해도에 의해 복잡해진다.
본 발명은 불용성이고, 비독성이며, 수용액에서 장시간의 제어된 NO 방출을 나타내어 새로운 종의 NONO에이트를 형성하는 중합체/NO 부가물을 포함한다. 중합성 NONO에이트, NO-폴리에틸렌이민 셀룰로오스 (PEIC-NO)는 수성 환경에서 장시간에 걸쳐 상당량의 NO를 방출한다. 이제는, 현 시점에서 출원인이 가장 좋은 방법으로 알고 있는 방법을 설명하기 위해 본 발명을 수행하는 가장 좋은 방법을 기재하고자 한다. 실시예는 단지 설명을 위한 것으로, 특허 청구범위의 범주 및 취지에 의해 나타나는 본 발명을 제한하지 않는다.
재료:
폴리에틸렌이민 셀룰로우스 (PEI-셀룰로우스)(미세 또는 중간 메시의 음이온교환 수지)를 시그마 케미칼 캄파니 (Sigma Chemical Company) (미조리주 세인트 루이스 소재)로부터 구입하였다. 질산칼륨 (99.999%) 및 아세토니트릴 (99.5%)을 알드리치 케미칼 캄파니 (Aldrich Chemical Company) (위스콘신주 밀오키 소재)로부터 구입하였다. 아르기닌 개질된 (2%), AIN-76 저질산염 다이어트를 ICN 바이오 케미칼즈 (Biochemicals) (오하이오주 클리브랜드 소재)로부터 구입하였다. 염화바나듐 (III) (99%)을 죤슨 마트히/알파 프로덕츠 (Johnson Matthey/Alfa Products)(매사추세츠주 워드 힐 소재)로부터 구입하였다. 고농도의 실리콘 고무 발포체는 베리실 코포레이션 (VariSeal) (오하이오주 파크맨 소재)으로부터 공급되었다. 살균 바이오클루시브 (Bioclusive) (TM) 투명 드레싱을 죤스 엔드 죤슨 메디칼 인크(Johnson & Johnson Medical Inc.) (텍사스주 알링톤 소재)로부터 구입하였다. 용액 제제에 사용되고 쥐가 먹을 물을 밀리-Q (Milli-Q) (TM) 카트리지 여과 시스템(매사추세츠주 베드포드 소재의 밀리포어 코퍼레이션 (Millipore Corporation) 제품)으로 정제하였다. 산화질소를 마터슨 프로덕츠, 인크 (Matheson Products, Inc.) (오하이오주 트윈스부르그 소재)로부터 구입하였다. 다른 시약 등급 재료를 피셔 사이엔티픽 (Fisher Scientific) (펜실바니아주 피츠버그 소재)으로부터 구입하였다.
실험방법
동물 실험을 위한 모든 과정은 아크론 대학의 동물 보호 위원회에 의해 승인되었다. 수컷 스프라그 다울리 (Sprague Dawley) 쥐 (75 ∼ 99 g)를 지빅 밀러 코퍼레이션 (펜실바니아주 젤리에노플 소재)으로부터 구입하였다. 동물 보호 설비는일정 습도 (50 ∼ 60 %), 및 온도 (21∼ 25 ℃)의 12 시간 광/암 교대 주기를 제공한다. 도착하자마자 쥐를 2주 동안 격리시키고 이어서 다른 실에 옮기어 7일 동안 새환경에 순응시켰다. 쥐에게 깨끗한 베드가 깔린 케이지, 임의량의 증류된 탈이온수가 제공되고, 2% 아르기닌을 함유하는 통상의 저질산염 (NO3) 다이어트를 먹였다. 쥐를 신진대사 케이지로 옮기고 대조군 (n=6) 또는 처리군 (n=9)으로 랜덤하게 할당하였다.
기준선 뇨 질산염 배출량 수준을 설정하기 위해, 상처를 입히기 전 9일 동안 24 시간 간격으로 소변을 수집하였다. 각 소변 수집 유리병에 3몰/ℓ HCl 5 ㎖를 부가하여 소변의 pH를 1 이하로 유지시켜서 박테리아 성장을 억제시켰다. 또한 낮은 뇨 pH는 질산염을 분석하는 동안 NO 분석기 성능을 최적으로 유지하도록 돕는다. 또한 뇨는 전체 상처 치료 기간 동안 24시간 간격으로 수집하였다. 표본을 즉시 사용하거나, 분석하기 전까지 냉동 보관한다.
상처를 가한 날, 뇨를 수집하고 쥐를 넴부탈 (Nembutal) (40 mg/kg, 복강내 투여)로 마취시켰다. 각 쥐의 등쪽 털을 깍고 이어서 살균 이소프로판올 흡수 패드로 세척하였다. 살균 장치 및 무균 기술을 사용하여, 진피 및 피부근층을 제거하여 각 쥐에게 2 cm x 2 cm 평방으로 최대 두께의 상처를 입혔다. 의학 등급의 접착제 및 4 cm x 4 cm 평방의 홀을 이면에 구비한 실리콘 고무 포말을 상처에 나란한 피부에 배치시킴으로써 치료 화합물을 유지시키고 상처가 오염되는 것을 막았다. 또한 실리콘 웰(well)은 설치류의 초기의 상처 치료 이후의 기간동안 전형적으로 나타나는 상처 모서리에서의 피부 수축을 막았다. 상처 처리 후, 실리콘 웰을 바이오 클루시브 필름으로 덮고 이어서 베트랩 (Vetrap)으로 덮었다.
처리된 쥐는 NONO에이트 (PEIC-NO) 200 mg 및 살균 IX PBS 200 μL를 수용하였다. 대조용 쥐는 PEIC 200 mg 및 살균 IX PBS 200 μL를 수용하였다. 또한, 쥐는 마취중에 겐타미신 (4.4 mg/Kg, 근육내 투여)이 주사되었다. 외과적 처치 및 상처 이미지화 후, 각 쥐를 등온 패드상에 위치시키고 마취로부터 회복될 때까지 밀착 모니터링하고, 이어서 신진대사 케이지로 복귀시켰다. 처리 및 대조용 국부 투여를 미리 코드화하여 실험 과정 전체에 걸쳐 비가시적인 연구를 제공하였다.
PEI - 셀루로오스 NONO에이트 합성
드라고 (Drago) 및 카르스테터 (Karstetter)에 의해 개발된 고압 기술을 다소 변형시켜, 70 ㎖ 아세토니트릴과 함께 PEI 셀루로우스 (7.0 g, 15.4 밀리몰)를 자기교반기가 구비된 변형된 에이스 (Ace) 트레드 (thread) 반응병에 위치시켰다. 동시에 전달될 수 있는 NO 및 N2용의 2개의 가스 주입구로 이루어진 4-식 가스 밸브 기구를 통해 10 분 동안 질소 가스로 용액을 채우고, 제3 출구를 사용하여 시스템을 개방 상태로 유지시켰다. 모든 가스 연결부는 투명한 테플론 (Teflon) 튜브 (OD : 0.25) 및 스테인레스 스틸 스웨지락 (swagelock) 부속으로 형성되었다. 이어서, 산화질소를 70 psig 압력하에서 30분 동안 투여하고 반응을 감압상태로 유지시키면서 반응병을 밀폐시켰다. NO 가스를 투여하는 상기 공정을 10일 동안 격일로 반복하고, 이후 과량의 NO를 발산시키고 N2가스를 15 분 동안 가하였다. 황색 생성물(6.82 g)을 여과에 의해 단리시키고, 아세토니트릴 및 이후에 에테르로 세척하고, 진공하에서 철야 건조시켰다. 폴리에틸렌이민 셀루로오스 중합체는 pH 7.4 완충액에서 중합체 mg 당 NO 약 67 nmol을 방출시켰다.
수득된 생성물은 하기 화학식 XI의 개략적인 형태로 나타났다.
ED-BDE NONO에이트 합성
에틸렌 디아민과 1,4-부탄디글리시딜 에테르 약 동몰량을 반응시켜 불안정한 중합체가 얻어지도록 함으로써 가교된 폴리(에틸렌 디아민-코-1,4-부탄디글리시딜에테르)를 제조하였다. 증류수 약 150 ㎖를 부가하고, 중합체를 팽윤시킨 후 여과시키고 아세톤으로 세척하였다. 중합체를 50℃에서 오븐 건조시켰다. x값은 중합화된 반응물 초기량에 좌우된다.
아세토니트릴 25 ㎖ 중의 중합체 약 0.5 g에 70 psi에서 25분 동안 NO 가스를 부가하였다. 반응 용기의 밸브를 밀폐시키고 48 시간 동안 반응시켰다. 반응기를 발산시키고 질소로 정제한 후 추가의 NO를 30 분 동안 70 psi에서 부가하였다. 반응기 밸브를 밀폐시킨 후, 24 시간 동안 더 반응시켰다. 최종 생성물을 여과시키고 에테르로 세척하였다. 최종 NONO에이트 중합체는 물에 불용성인 백색 고상 분말이었다. 하지만, 물과 접촉시 NONO에이트 중합체는 NO 가스를 방출하였고 최초 공중합체를 재생시켰다. 폴리(에틸렌디아민-코-1,4-부탄디글리시딜 에테르) 중합체는 pH 7.4 완충액에서 중합체 mg 당 NO 65 nmol을 방출하였다.
상기 반응은 하기와 같이 개략적으로 나타난다.
2개의 합성 경로는 상기와 같고, 상기의 특정 중합체가 지금까지 본 발명자가 알고 있는 가장 우수한 형태를 나타내지만, 본 발명이 상술한 특정 중합체로 한정될 필요는 없다. 덱스트란 기재를 사용하는 것과 같이 다른 중합체를 사용하는것 또한 본 발명의 범주에 속한다. 기재에 대한 논의는 본 명세서에서 완전히 인용된 공개류 출원 미국특허 제 08/065,742호에 포함된다.
비디오 화상 분석
상처를 입힌 직후 및 그 이후 3일 마다, 각 상처를 비디오 카메라(마이크로 초점 6배 파워 줌 렌즈가 달린 Nikon VN-3000) 및 VHS 테이프(FUJI A/V Master Super XG)를 사용하여 비디오 테이프에 담았다. 임의로 남아있는 각 상처를 살균 1X PBS로 주의깊게 세척한 후, 자체 접착성의 원형 라벨(직경 1.9 cm)을 상처에 인접하게 놓았다. 이는 비디오 화상을 분석하는 동안에 외부 표준으로서 작용하며, 렌즈 대 상처 거리를 변화가능하게 하였다. 카메라 렌즈는 상처 부위에 대해 수직으로 배치시키고, 상처와 외부 표준은 동일한 수평면 상에 배치시키고, 렌즈들의 초점을 맞춰 가능한 가장 큰 화상을 제공하였다.
게이트웨이 2000 386/16SX 컴퓨터(Gateway 2000, Inc. 제품, 사우쓰 다코타노쓰 시우스 씨티)에 인스톨된 스펙트럼 NTSC + 프레임 그래버 보드(Redlake Corporation 제품, 캘리포니아주 힐 모간 소재)에 카메라 출력 신호를 입력함으로써 상처 화상의 디지털 컴퓨터 분석을 달성하였다. 아큐웨어(Accuware) 화상 분석 소프트웨어(Automated Visual Inspection 제품, 캘리포니아주 산타 클라라 소재)를 사용하여 상처와 외부 표준을 나타내는 몇몇 최적 화상을 연속적으로 잡아 삼성 CSA7571 멀티스캐닝 17 인치 RGB 모니터(Samsung Information Systems America, Inc. 제품, 캘리포니아주 산 조세 소재) 상에 표시하였다. 상처와 외부 표준의 주변을 마우스로 트레이싱하고, 각 화상의 화소 면적을 계산하였다. 외부 표준에 대한 상처의 비율을 사용하여 상처의 상대 면적을 얻었고, 이는 카메라 대 상처 거리와 독립적인 측정값을 제공하였다. 각 상처의 상대 면적을 초기값과 시간에 대해서플롯트한 값의 비율로서 표현하여 상처 치료 진행을 측정하였다. 짝지어 표지시킨 2명의 스튜던트 t-테스트를 이용하여 처리된 쥐와 대조 쥐 사이의 상처 치료의 현저한 차이를 평가하였다.
질산염 분석
모든 뇨 시료를 모니터 랩스 모델 8440 질소 산화물 분석기(Lear-Siegler Corporation 제품, 콜로라도주 이글우드 소재) 및 브라만 및 헨드릭스(Braman and Hendrix)에 의해 기재된 방법의 변형을 사용하여 질산염(NO3)에 대해 분석하였다. 마춤 임핀저에 1.5 mol/L의 HCl 중 염화바나듐(III)(VCl3, 0.4 mol/L)의 환원 용액 40 내지 50 ㎖를 충전하였다. 환원 용액을 95 내지 100℃로 가열하고, 유동율을 125 ㎖/분으로 설정하여 헬륨으로 탈기시켰다. 테플론으로 라이닝된 격막을 통하여 뇨 시료를 환원 용액으로 주입하였고, VCl3은 존재하는 임의의 NO3를 NO로 환원시켰다. 헬륨의 유동은 1 mol/L의 NaOH로 충전된 제2 임핀저를 통하여 새로 발생된 NO를 운반시켜 임의의 산성 가스를 제거시켰다. 분석기 진공 펌프의 유동율(즉, 시료 주입 유동율)을 미세 계량 밸브를 사용하여 150 ㎖/분으로 설정하였다. 분석기 주입구와 NaOH 임핀저 사이의 "티(T)"형 연결부는, 정상 주입 유동율을 유지하고 분석기 주입구와 헬륨 유동율의 매칭 문제를 피하는 개방 시스템을 제공하였다. 분석기 및 (분석기에 의해 발생되는 O3와 NO 사이의) 후속 화학발광 반응으로 유입되는 NO에 의해 시료 당 NO 양을 결정하였다. 또한, 기지 농도의 KNO3를 주입 사용하여매일 표준 곡선을 결정하고, 이를 사용하여 동물 당 평균 NO3배출량(μmol/일)을 계산하였다. 출력 신호는 HP3392A 통합 레코더(Hewlett Packcard CO 제품, 펜실베니아주 아본달 소재)를 사용하여 잡았다. 모든 뇨 시료에 대해 2회 주입을 실행하고, 평균치를 각 동물에 대한 매일 NO3배출량으로서 사용하였다. 짝짓지 않고 표지시킨 2명의 스튜던트 t-테스트를 이용하여 처리된 쥐와 대조 쥐 사이의 상처 치료 전과 후의 뇨 NO3농도의 현저한 차이를 평가하였다.
NONO에이트(NONOate) 분석
LC/9540 크로마토그래피 데이터 인테그레이터(IBM, Inc, 제품, 코네티컷주 덴버리 소재)에 연결된 모니터 랩스 모델 8440 질소 산화물 분석기(Lear-Siegler Corporation 제품, 콜로라도주 이글우드 소재) 상에서 PEIC-NO의 분석을 실시하였다. 시료 채취 챔버는 챔버 내에 NO 가스를 축적가능하게 하는 두 방향 밸브로 변형시킨 가스 임핀저 병으로 이루어졌다. 시료 채취 챔버의 한쪽 말단을 NO 분석기에 연결시키고, 한편 다른 말단은 유동 계량기와 헬륨 가스 탱크에 연결시켰다. 헬륨 가스를 시스템을 통하여 10 psig에서 펌핑하고, 유동 계량기는 150 내지 200 ㎖/분으로 조정하였다. 150 ㎖의 시료 채취 챔버에 pH 7.4의 PBS 50 ㎖를 충전하고, 용액을 15분 동안 탈기시켰다. PEIC-NO 샘플 10 mg을 첨가하고, 밸브를 밀폐하고, 밸브를 개방하고 제조된 NO를 헬륨 가스에 의해 검출기로 밀어넣어 주기적으로 기록하였다.
PEIC-NO로부터 방출된 NO의 농도를 100 μmol/L KNO3표준 곡선을 사용하여계산함으로써 동력학적 측정치를 얻었다. 시간(hr) 대 생성된 NO(nmole) 방출 총량을 플롯트하여 방출 프로파일을 얻었다. 이 그래프로부터, 무한대에서의 NO 농도를 결정하였다. ln(Conct-Conct) 대 시간(hr)을 플롯트하여 1차 반응 속도를 계산함으로써 K 값과 중합체의 반감기를 계산하였다.
혈압 측정
원격측정 장치를 자발적 고혈압(SHR) 또는 위스터-교토(WKY) 쥐에 미리 이식하였다. 수축기 혈압(SBP), 확장기 혈압(DBP), 평균 동맥 혈압(MAP), 심박도수(HR) 및 운동 활성(ACT)을 데이터퀘스트 Ⅳ 데이터 획득 시스템(Data Sciences Inc. 제품, 미네소타주 세인트 폴 소재)을 사용하여 24시간 간격으로 연속적으로 측정하였다. 마취시킨 쥐의 복부 중심선을 절개하고, 라디오 송신기의 가요성 카테테르 끝을 신장 혈관 및 장골 동맥 사이의 하향 대동맥에 삽입하여 원격측정 장치를 이식하였다. 송신기를 복막강에 배치하고 복부 벽을 봉합하여 중심선 절개부를 밀폐하였다. 동물을 개별 회수 케이지에 1 주일 동안 놓아두었다. 수신기를 각 케이지 아래에 놓았고, 이 수신기는 별개의 방에서 컴퓨터에 의한 데이터 획득 시스템에 신호를 연속적으로 보냈다. 파라미터를 측정하고, 매 20초 내지 매 5분 사이에 세이브한 후, 30분 간격으로 평균하였다. 상처를 입히기 전, 상처를 입힐 때 및 NONO에이트를 국부 적용하거나 마취제를 주입한 후(복강내투여), 기준선 혈압을 기록하였다. 국소적 NONO에이트 및 마취제 소듐 브레비탈의 3 % 용액(50 mg/kg 복강내투여)을 3일 간격으로 투여하였다. 수축기 혈압을 기록하고 얻어진 평균 데이터를 시간에 대하여 플롯트하여, 국부적 NONO에이트 및 브레비탈 주입 모두에 대하여 시간 경과에 따른 혈압의 임의의 현저한 변화를 표시하였다. 짝지어 표지된 2명의 스튜던트 t-테스트를 수행하여 (NONO에이트) 국부 전달 또는 (브레비탈) 주입된 쥐의 수축기 혈압의 현저한 차이를 평가하였다.
논의
상처로부터 NO 방출을 간접적으로 측정하는 뇨 NO3프로파일이 도 1에 도시되어 있다. 0일은 상처를 입힌 날이고, 각 테이터 포인트는 각 군에 대한 평균 일일 뇨 NO3배출량을 나타낸다. 상처를 입기 전 평균 (n = 9일) 뇨 NO3 -배출량은 대조군 및 NONO에이트 군에 대해 각각 6.7 ± 0.34 S.E.M 대 7.4 ± 0.37 S.E.M. μmol/일 NO3이었다. 상처를 입기 전에 양쪽 군에 대한 뇨 NO3 -배출량에 현저한 차이는 없었다.
치료의 초기 단계(n = 3일, 0 내지 2일)에서, 평균 뇨 NO3 -배출량은 대조군 및 NONO에이트 군에 대해 각각 12.4 ± 2.4 S.E.M 및 22.5 ± 1.1 S.E.M. μmol/일 NO3 -이었다(P < 0.019). 3일째에 PEIC-NO 군의 뇨 NO3 -배출량은 상응하는 대조군보다 3.5 배 컸다. 4 내지 10일(n = 7일)의 평균 뇨 NO3 -배출량은 PEIC-NO 군 및 대조군에 대해 각각 28.1 ± 1.2 S.E.M 및 21.0 ± 0.81 S.E.M. μmol/일이었다(P <0.0004).
그러나, 상처를 입은 후 치료의 초기 단계에서(n = 11일, 0 내지 10일), 평균 뇨 NO3 -배출량은 대조군 및 NONO에이트 군에 대해 각각 17.7 ± 1.5 S.E.M 대 27.4 ± 1.4 S.E.M. μmol/일로, 극도로 현저하였다(P < 0.0002). 치료 후기 단계(n = 14일, 16 내지 29일)의 평균 뇨 NO3 -배출량은 대조군 및 NONO에이트 군에 대해 각각 10.2 ± 0.43 S.E.M 및 12.3 ± 0.61 S.E.M. μmol/일이었다(P < 0.011).
뇨 NO3 -배출량은 양쪽 군에 대해 치료 후 11일째에 점진적으로 하강하였다. 그럼에도 불구하고, 외부 상처를 21일째에 약 93 % 봉할 경우 NONO에이트 군에 대한 뇨 NO3 -배출량은 16 내지 25일(n = 9일, P < 0.0001) 사이의 기준선보다 79% 높았다.
도 2는 대조 쥐 및 PEIC-NO 처리된 쥐 모두에 대한 상처 치료 데이타를 나타낸다. (초기 상처 면적에 대한) 상처 노출 %를 기준으로 하여, PEIC-NO 군 상처의 치료는 대조군에 비해 7일, 10일 및 17일째에 현저하게 강화되었다(P < 0.05). 도 3은 PEIC-NO의 국소 도포에 기인한 전형적인 수축기 혈압 프로파일을 나타낸다. 수축기 혈압은 약 45 내지 50분 동안 60 mmHg로 하강한 후, 동물이 회복하기 시작함에 따라 상승하기 시작한다. 처리하고 약 3시간 후, 수축기 혈압은 정상 수준으로되돌아왔다. 그러나, 수축기 혈압의 초기 하강은, PEIC-NO 처리된 쥐 및 마취제만 수용한 동일한 쥐 사이의 유사한 평행관계로부터 알 수 있는 바와 같이 주로 마취제 브레비탈의 효과에 기인하였다. 이는 NONO에이트 PEIC-NO가 일시적인 저혈증 효과를 갖는다는 것을 나타낸다.
도 4는 중합성 PEIC-NO로부터의 NO 방출 프로파일을 보여준다. NO-PEIC 시료 10 mg은 16시간의 반감기를 갖는 NO를 685 nmol 방출하였고, 이는 NO-PEIC가 연장된 기간에 걸쳐 생리학적 완충액 내에서 조절된 NO 방출을 제공한다는 것을 입증한다.
상처 치료 연구에 사용된 중합성 NONO에이트는 다음의 바람직한 성질들을 갖는다. (1) 이들은 안정한 고체이다, (2) 수불용성이다, (3) 산화환원 활성화없이 NO를 제공한다, (4) 동력학적으로 단정하다(1차 차수로 NO 방출), 및 (5) 각종 물리적 구조로 제제화할 수 있다. 이 상처 연구에는 수용성이지만 중합성 장치 또는 리포좀에 봉입되는 NONO에이트를 사용할 수 있다. NONO에이트가 상처 부위에 남고 이동하지 않아서 잠재적으로 전신 부작용을 제공하지 않는다는 것이 주된 관심사이다. 또한, 가용성 NONO에이트는 이온 상호작용에 의해 중합체 지지체에 부착되어, 예를 들면 (NONO에이트는 폴리양이온성 폴리아민으로부터 형성되기 때문에) NONO에이트는 폴리음이온성 수지와 착물을 이룰 수 있었다.
이 상호작용은 상처 부위에서 중합성 NONO에이트에 의해 관찰된 것과 유사한 NO 제공자를 보유할 수 있었다.
또한, 가용성 NONO에이트는 상처 드레싱에 사용되는 통상의 재료에 봉입될 수 있었다. 예를 들면, 가용성 고체 NONO에이트를 우레탄 중합체에 혼합할 수 있었다. 이 중합체를 막 상에서 주조하거나 형성하여 드레싱을 제조할 수 있다. 상처 위에 NO를 방출하는데 필요한 것은 (우레탄의 부분적 수화에 의한) H+공급원 및 NO 이동을 위한 간단한 경로이다. 그러므로, NO 공여체를 트랩핑하고, 국소적인 NO 방출을 달성하는 것도 가능하다.
NONO에이트 이외에 다른 NO 공여체가 고려된다. 예를 들면 S-니트로소 화합물을 사용할 수 있다. 예를 들어 S-니트로소-N-아세틸페니실아민(SNAP)은 생물학적 조건하에서 NO를 방출한다. 이 물질은 중합체에 혼입되거나 제어 방출시스템에 봉입되어 전신 효과를 유발하는 이동없이 상처 부위에 NO를 방출시킬 것이다. S-니트로소알부멘과 같은 단백질을 사용하여 NO를 전달할 수 있다.
다른 NO 공여체는 NO를 형성하기 전 몇몇 형태의 생물학적 산화 또는 환원을 필요로 한다. 니트로글리세린(환원을 필요로 함)과 같은 화합물을 중합체에 부착시키거나 봉입시키고 충분히 환원시킬 경우 상처에 NO를 제공할 수 있다. SIN-1(몰시도민)과 같은 다른 것들은 NO를 방출하기 위하여 산소로부터의 산화를 필요로 한다. 다시, 상처에 NO를 전달하는데 적합한 조건하에 몰시도민 동족체의 중합체를 고려할 수 있다.
피부의 모든 발진 궤양을 포함하는 모든 장애의 치료는, 국소 도포된 NO 중합성 착물의 제어 방출을 통하여 촉진하는 능력을 나타낸다. 이는 가용성이었던 선행 기술의 NONO에이트 착물과 대조적으로, NONO에이트가 수성 환경에서 불용성이라는 사실을 통하여 성취된다. 산화질소를 중합성 매트릭스로 혼입하는 잇점은, 착물의 저장 수명이 선행 기술의 생성물보다 극적으로 증가되고 이로인해 사용하지 않을 경우 즉시 분해되는, 즉 저장 수명을 갖지 않는다는 경향이 있다는 것이다.
중합성 착물 캐리어의 다른 잇점은, 착물 중의 산화질소 치환체의 소비 또는 사용 후 남아 있는 중합체가 종래 기술에 의해 교시된 아민 착물과는 달리 생체 적합성이라는 것이다.
그러나, 중합성 착물 캐리어는 드레싱과 같은 다른 재료를 추가로 포함할 수 있다. 현재 각종 군의 드레싱이 급성 및 만성 피부 상처의 처리에 사용되고 있다. 이들 중, 히드로콜로이드 드레싱(HCD)이 임상 환경에 가장 자주 사용되고 있다. 드레싱이 제공된 폐색 및 습한 환경에 의해 결부된 상기 드레싱의 고흡수능 특성은 신속한 과립화, 재상피화 및 상처 폐쇄를 유도한다.
HCD 드레싱을 위한 임상 적용에는 화상 및 화상 제공 부위, 만성 정맥 궤양, 욕창, 나성 궤양, 표피 수포증, 부종성 경화증, 건선 및 비감염 부분 치밀 상처의처치가 포함된다.
종래 HCD 드레싱에는 통상적으로 저분자량 및 고분자량 폴리이소부틸렌으로 이루어진 접착제 혼합물 및 젤라틴, 펙틴 및 카르복시메틸 셀룰로오스, 실리카 및 면 섬유와 같은 흡수제가 혼합된다. 대표적인 HCD 드레싱은 예를 들어 미국 특허 제3,972,328호(1976년 8월 3일, Chen등), 동 제4,253,460호(1981년 3월 3일, Chen등) 및 동 제4,538,603호(1985년 9월 3일, Pawelchak 등)에 기재되어 있다.
현재, 상처 필러 및 드레싱의 제조에 각종 흡수제가 사용되고 있다. 상처 드레싱 성분으로서의 상기 흡수제의 선택에 있어서 중요한 특징은 유체 조작능인 것으로 보이며, 생분해성은 주요 관심사의 논점이 되지 않는다. 이를 고려하여, 최근의 역사적 연구가 특정 상처 드레싱의 사용이 외부적으로 치료된 조직에 광범위한 미해결 및 고질적인 만성 염증을 일으킨다는 사실을 보여준다는 것은 놀라운 일이 아니다. 이러한 염증은 생리학적 조건하에 비독성 및 비염증성 생성물로 분해되는 드레싱 성분을 사용함으로써 잠재적으로 감소시킬 수 있다. 이러한 점에 있어서, 주목할 점은 조절된 약물 전달에 통상적으로 사용되는 생분해성 미소구체(예, 폴리락티드 또는 글리콜리드) 중 어느 것도 감지할 수 있는 흡수능 또는 유체 조작능을 전혀 갖지 못한다는 것이다.
유용한 흡수제는 수 팽윤성, 수 불용성, 가수분해에 불안정한 중합체 조성물 및 제약상 허용가능한 가교 다당류(바람직하게는 덱스트란) 중합체 조성물을 비드상 또는 미립자 형태로 포함할 것이다. 미립자는 형상이 본질적으로 구형이므로 마소구체라고 말할수 있다. 건조될 때 생성물은 자유 유동 분말이다. 가교기는 선형이미도카르보네이트기, 선형 카르보네이트기 또는 이들의 혼합물이다. 생성물은 25℃에서 수불용성이고, 37 ℃ 이상의 온도에서 본질적으로 중성의 수성 매질 중에서 수용성 비가교 다당류로 분해될 수 있다. 이 생성물은 본질적으로 중성의 수성 매질 중에서 분해성이기 때문에, 이들은 가수분해에 불안정한 (또는 가수분해적으로 분해성인) 특성을 가질 수 있다. 또한, 가수분해 불안정성은 이 생성물이 생분해성이라는 것을, 즉 정상 체온(37 ℃)에서 혈액 및 림프와 같은 수성 체액의 존재하에 수용성 생성물로 분해될 수 있다는 것을 나타낸다.
미소구체는 가교가 일어나는 알칼리성 조건하에 유중수 분산액의 수성상 중에서 수용성 비가교 다당류를 시아노겐 할라이드로 가교시켜 형성된다. 바람직한 시아노겐 할라이드는 시아노겐 브로마이드이다. 가교 생성물은 다당류 쇄 및 시아노겐 할라이드와 염기의 반응에 의해 형성된 가교기를 포함한다. 형성된 바와 같은 가교기는 다당류 쇄 상의 히드록실기에 의해 상이한 다당류 쇄 (또는 동일한 쇄의 원거리 부분)에 결합되어 있는 선형 이미도카르보네이트기라고 생각되어 진다. 이러한 선형 이미도카르보네이트기는 마무리처리 과정 동안 산 중에서 선형 카르보네이트기로 부분적으로 가수분해될 수 있다.
가교 생성물은 시아노겐 할라이드와 염기의 반응에 의해 도입된 것 이외의 가교기를 본질적으로 포함하지 않는다. 특히, 가교 생성물은 가수분해 비분해성 가교기를 포함하지 않는다.
출발 다당류는 수용성이고, 분자량이 약 40,000 내지 약 1,000,000 이상일 수 있다. 바람직하게는, 출발 다당류의 (평균) 분자량은 약 100,000 내지1,000,000이고, 보다 바람직하게는 약 200,000 내지 약 600,000이다. 바람직한 출발 다당류는 덱스트란이다.
미립자는 형상이 본질적으로 구형이고, 주로 약 1 내지 약 100 미크론의 범위에 있다. 일반적으로, 미립자의 직경은 주로 약 2 내지 약 50 미크론의 범위 내에 있다. 최종 생성물 미소구체는 자유 유동 분말 형태이다.
구형 미립자를 상기 정의된 크기 범위로 얻기 위하여 활성화 반응을 유중수 분산액의 수성상 내에서 수행하는 것이 필수적이다. (오일 상이 전혀없이) 물이 반응 매질로서 사용될 경우, 겔이 초기에 형성된다. 겔은 유용한 생성물을 얻기 위하여 에탄올과 같은 탈수 용매의 존재하에 (예를 들면 블렌더로) 분쇄되어야 한다. 이러한 가공의 최종 생성물은 구형의 형태가 아니라 불규칙 형상의 응집물 형태이다.
미소구체 형태의 생성물은 응집물 형태의 생성물에 비해 몇몇 잇점을 제공한다. 먼저, 가공 및 제제화가 더 쉽다. 둘째로, 생성물이 보다 균일하다. 그 결과, 본 발명의 생성물은 응집물 형태의 생성물보다 더 균일하고, 더 예측가능한 정도의 습윤, 습윤 속도 및 수분의 존재하의 가수분해 또는 분해 속도를 나타낸다.
본 발명의 하나의 형태에서, 중합성 NONO에이트가 상기한 바와 같은 덱스트란 입자 위에 화학적으로 그라프트화되지만, 또한 선택된 합성 방법에 따라 화학적 포획(entrapment) 가능하다.
본 발명에 따른 상처 드레싱은 예를 들면 본 발명의 가교 다당류 미소구체와 소수성 접착제 중합성 매트릭스 재료의 블렌드를 포함하고, 이 블렌드를 불활성 방수 배면지의 한 측면 또는 표면에 도포한다.
때때로, 매트릭스 재료를 상기한 바와 바와 같은 흡수 재료에 혼입시킬 수 있다. 이는 체온에서 소수성, 화학 불활성, 제약상 허용성, 접착성의 고체상인 무정형 중합체(유리 전이 온도는 갖지만 융점은 없음)일 수 있다. 마지막으로, 유리 전이 온도는 정상 체온보다 약간 높을 수 있다. 예를 들면 약 45 ℃ 이상일 수 있다.
적합한 매트릭스 재료는 당업계에 알려져 있다. 매트릭스 재료는 탄력성(즉, 엘라스토머성) 및 소수성이다. 적합한 매트릭스 재료의 예로 각종 등급의 폴리이소부틸렌 스틸렌-부타디엔 고무, 및 부틸 고무(이소부틸렌과 소량의 이소프렌의 공중합체)가 있다. 저분자량 폴리이소부틸렌(평균 분자량 약 10,000 내지 약 50,000)이 전형적인 매트릭스 성분이다.

Claims (19)
- (a) 수불용성 중합체, 및 (b) 수성 환경에 치료량만큼 노출시 상기 중합체로부터 방출될 수 있는 상기 중합체에 대한 화학적 결합량의 산화질소를 포함하는(comprising), 상처 표면에 국소 도포했을 때 상처 치료를 촉진시키는 부가물.
- 제1항에 있어서, 상기 중합체가 수불용성 중합체인 부가물.
- 제2항에 있어서, 상기 중합체가 산화질소 방출 후 생체 적합성으로 되는 부가물.
- 제2항에 있어서, 중합체가 폴리에틸렌이민 셀룰로오스 및 폴리(에틸렌 디아민-코-1,4-부탄디글리시딜 에테르)로 이루어진 군으로부터 선택된 것인 부가물.
- 제3항에 있어서, 흡수 드레싱을 더 포함하는 부가물.
- 제5항에 있어서, 드레싱이 저분자량 및 고분자량 폴리이소부틸렌, 젤라틴, 펙틴, 카르복시메틸 셀루로오스, 실리카, 면섬유, 수팽윤성 및 수불용성이며 가수분해에 불안정한 중합체 조성물, 및 미립자 형태의 제약상 허용가능한 가교 다당류로 이루어진 군으로부터 선택된 것인 부가물.
- 제6항에 있어서, 상기 다당류가 선형 이미도카르보네이트기, 선형 카르보네이트 기 및 그의 혼합물로 이루어진 군으로부터 선택된 가교기에 가교된 것인 부가물.
- 제7항에 있어서, 다당류의 분자량이 40,000 내지 1,000,000인 부가물.
- 제8항에 있어서, 다당류의 분자량이 100,000 내지 1,000,000인 부가물.
- 제9항에 있어서, 다당류의 분자량이 200,000 내지 600,000인 부가물.
- 제10항에 있어서, 다당류가 덱스트란인 부가물.
- 제5항에 있어서, 매트릭스 재료를 더 포함하는 부가물.
- 제12항에 있어서, 매트릭스가 가교된 다당류 및 소수성 접착 중합성 매트릭스와 블렌드인 부가물.
- 제13항에 있어서, 매트릭스가 비정질, 소수성, 화학적 불활성, 제약상 허용가능하며, 체온에서 고체인 접착 중합체인 부가물.
- 제14항에 있어서, 매트릭스의 유리 전이 온도가 45℃ 이상인 부가물.
- 제15항에 있어서, 매트릭스가 폴리이소부틸렌 스티렌-부타디엔 고무, 부틸 고무 및 저분자량 폴리이소부틸렌으로 이루어진 군으로부터 선택된 것인 부가물.
- 제1항에 있어서, 전달될 때 생체에 대해 비독성인 부가물.
- 제17항에 있어서, 산화질소가 3주 이상 전달되는 부가물.
- 제18항에 있어서, 반감기가 960분 이상인 부가물.
