Abstract
Description
本発明は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩(以下、リボフラビン系化合物ということがある)を有効成分とし、サイトカイン抑制作用を有する医薬に関する。また、本発明は、リボフラビン系化合物を有効成分とし、感染に起因しない全身性炎症反応症候群の予防又は治療に有用な医薬に関する。
関連技術
多くの疾患において炎症反応が関与しており、いわゆる炎症性疾患だけではなく、アルツハイマー痴呆症や心臓疾患等においても炎症反応が重要な影響を持つことが分かってきている。
炎症反応が関与する疾患の中でも、とりわけ全身性の炎症兆候を伴う病態である全身性炎症反応症候群は、侵襲を受けた生体の反応を把握するサインとして重要である。また、この全身性炎症反応症候群となっている状態において、症状が進行したり合併症が起こると、成人呼吸促迫症候群(ARDS;adult respiratory distress syndrome)、播種性血管内血液凝固(DIC;disseminated intravascular coagulation)、多臓器不全(MOF;multiple organ failure)等の機能不全(MODS;multiple organ dysfunction syndrome)に陥り、意識障害、呼吸困難、血圧低下等の症状を示し、ショック状態となり死に至る場合もある。この全身性炎症反応症候群を引き起こす原因としては、感染症のほか、外傷、熱傷、膵炎、手術等の様々な侵襲がある。
従来から、全身性炎症反応症候群の病態には、蛋白質性シグナル伝達物質であるサイトカインの過剰誘導が関与していることが知られている。これに関して、全身性炎症反応症候群の進行度とサイトカインの血中濃度との間には一定の相関関係があることが報告されている(小川道雄:新・侵襲とサイトカイン.生体防御と生体破壊という諸刃の剣,メジカルセンス,東京,1999,Bone,R.C.:Toward a theory regarding the pathogenesis of the systemic inflammatory response syndrome:What we do and do not know about cytokine regulation.Crit.Care Med.24,163−172,1996)。このことから、全身性炎症反応症候群は高サイトカイン血症の一つであるということができる。
現在まで、高サイトカイン血症の予防又は治療のために、サイトカインの誘導や発現を抑制する試みがなされている。
例えば、副腎皮質ステロイド剤を投与してサイトカインの誘導を抑制する試みがなされている(Luce,J.M.et al.:Ineffectiveness of high−dose methylprednisolone in preventing parenchymal lung injury and improving mortality in patients with septic shock.Am.Rev.Respir.Dis.138,62−68,1988)。
また、抗TNF−α抗体やインターロイキン−1レセプターアンタゴニスト(IL−1RA)等を投与してサイトカインの機能発現を抑制する試みがなされている(Abraham,E.et al:Efficacy and safety of monoclonal antibody to human tumor necrosis factor α in patients with sepsis syndrome:Arandomized,controlled,double−blind,multicenter clinical trial.J.A.M.A.273,934−941,1995,Fisher,C.J.et al.:Recombinant human interleukin−1 receptor antagonist in the treatment of patients with sepsis syndrome:Results from a randomized,double−blind,placebo−controlled trial.J.A.M.A.271,1836−1844,1994,Dhainaut,J−F.A.et al.:Platelet−activating factor receptor antagonist BN 5021 in the treatment of severe sepsis:A randomized,double−blind,placebo−controlled,multicenter clinical trial.Crit.Care Med.22,1720−1728,1994,Fisher,J−C.J.et al.:Treatment of septic shock with the tumor necrosis factor receptor:Fc fusion protein.E.Engl.J.Med.334,1697−1702,1996,Bone,R.C.:Sir Issac Newton,sepsis,SIRS,and CARS.Crit.Care Med.24,1125−1178,1996)。
また、特開平11−322601号公報には、N−[2−(2−フタルイミドエトキシ)アセチル]−L−アラニル−D−グルタミン酸を有効成分とするサイトカイン産生反応の制御による抗炎症性薬剤組成物が開示されている。
また、特表平8−506322号公報には、カルボキシル化および/または硫酸化オリゴ糖を含有する活性TNF−αの生産を阻害するための医薬組成物が開示されている。
発明の開示
しかしながら、上記の高サイトカイン血症の予防又は治療剤では、病態やその進行情況によっては十分な薬効を期待できない場合があり、より高い薬効を有し予後の改善効果についても優れた医薬の提供が望まれている。
なお、特開平5−201864号公報及び特開平10−29941号公報には、リボフラビン系化合物が免疫賦活・感染防御剤やトキシンショック予防治療剤として有用である旨が開示されているが、これらにおいてはリボフラビン系化合物とサイトカインとの関係については開示されていない。
したがって、本発明の目的は、高サイトカイン血症の予防又は治療のための優れた医薬を提供することにある。
本発明者らは、鋭意研究の結果、リボフラビン系化合物が優れたサイトカイン抑制作用を有することを見出した。すなわち、リボフラビン系化合物を有効成分とする医薬を投与することにより、サイトカインの誘導や発現を抑制することができ、炎症反応が関与する様々な疾患、特に、過度の侵襲が加わった生体の高サイトカイン血症の治療において、優れた薬効を示すことが分かった。
これは、侵襲により生体内において炎症性サイトカインが誘導又は発現された際に、リボフラビン系化合物がこの炎症性サイトカインの誘導や発現を抑制して、生体の恒常性を保つ働きをすることによるものと考えられる。
このように、リボフラビン系化合物がサイトカイン抑制作用を有することを初めて見出したことにより、本発明では、より薬効に優れた高サイトカイン血症の予防又は治療剤を提供することが可能となった。
すなわち、本発明は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを有効成分とし、サイトカイン抑制作用を有する医薬を提供するものである。
換言すれば、本発明は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを有効成分とする、サイトカイン抑制剤を提供するものである。
「少なくとも一つを有効成分」とは、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩のいずれか単独を有効成分としてもよく、あるいはこれらのうち複数を有効成分としてもよい、との意味である。
本発明の医薬は、高サイトカイン血症の予防又は治療に有用である。また、本発明の高サイトカイン血症の予防又は治療方法は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを対象物に有効量投与する、ことを特徴とする。
高サイトカイン血症とは、血中のサイトカイン濃度が高くなる状態を伴う疾患であり、例えば、アルツハイマー痴呆症、パーキンソン病、キャッスルマン病(Castleman病)、関節リウマチ、多発性筋炎、全身性硬化症、混合性結合組織病、フェルテイ症候群、回帰性リウマチ、サルコイドーシス、変形性関節症、多発性硬化症、ベーチェット病、エリテマトーデス、アテローム硬化症、心臓疾患、心房粘液腫、クローン病、潰瘍性大腸炎、炎症性大腸炎、痛風、接触性皮膚炎、自己免疫性皮膚炎、乾癬症、肺繊維症、肺気腫、びまん性汎細気管支炎、胸膜炎、糸球体腎炎、急性糸球体腎炎、メサンギウム増殖腎炎、ループス腎炎、IgA腎炎、糖尿病性腎症、粥状動脈硬化症、結節性動脈硬化症、脈管炎、気管支喘息、慢性歯肉炎、歯周病、出血性ショック、外傷性ショック、アナフィラキシー等のショック、火傷、脳腫瘍、悪性腫瘍、多発性骨髄腫、骨粗鬆症、痔、アトピー性皮膚炎、アレルギー性鼻炎、アレルギー性皮膚炎等のアレルギー、敗血症、敗血症性ショック、多臓器不全、一酸化炭素中毒、薬物等の中毒、急性放射線障害、くも膜下出血、全身性炎症反応症候群、インシュリン依存性糖尿病、ブドウ膜炎、慢性炎症、血液疾患、白血病、膠原病、筋萎縮性側索硬化症、脳浮腫、てんかん、特発性血小板減少性紫斑病、重症筋無力症、神経痛、ギラン・バレー症候群、ニューロパチー、脊髄損傷、花粉症等の自己免疫疾患、脳梗塞、心筋梗塞等の虚血・再灌流障害、動脈硬化、慢性心不全、心筋炎、光過敏症、蓄膿症、強直性脊椎炎、ウエゲナー肉芽腫症、多発性筋炎・皮膚筋炎、シェーングレン症候群、側頭動脈炎、SAPHO症候群、慢性疲労症候群、全身性若年性突発性関節炎、成人ステイル病、子宮内膜症、子宮蓄膿症、中耳炎、腹膜炎、心内膜炎、急性腎不全、急性肝障害、下痢、白内障が挙げられる。
ここでいう敗血症とは、血中のサイトカイン濃度が通常値よりも高くなった状態を伴う敗血症を意味する。敗血症性ショックとは、当該敗血症が進行した結果、心、肺、肝、腎、脾、脳、脊髄等の内臓器のうち1以上の臓器の機能不全が起きた状態、または臓器の機能不全の結果、脱力感、めまい、起立困難、血圧低下、体温低下、不整脈、心室細動、呼吸困難、体温低下、痙攣、意識混濁、意識不明等の症状を示す状態を意味する。
また、本発明の医薬は、特に、全身性炎症反応症候群の予防又は治療に有用である。
高サイトカイン血症の一つである全身性炎症反応症候群は、炎症性サイトカインが血中で優位となり炎症反応が起こるSIRS(systemic inflammatory response syndrome)と、抗炎症性サイトカインが血中で優位となり免疫抑制状態となって抗炎症反応(例えば、重症感染症)が起こるCARS(compensatory anti−inflammatory response syndrome)という二つの病態があると考えられている。本発明のリボフラビン系化合物を有効成分とする医薬を投与すると、そのサイトカイン抑制作用によって、SIRSの症状を予防又は治療する効果があることが分かった。また、SIRSとCARSとは互いに関連しているため、SIRSを予防又は治療することにより、本発明の医薬が有するサイトカイン抑制作用によって、CARSについても予防又は治療をすることができる。
本発明の「全身性炎症反応症候群の予防又は治療」とは、上記のSIRSやCARSの症状が進行して引き起こされる成人呼吸促迫症候群(ARDS)、播種性血管内血液凝固(DIC)、多臓器不全(MOF)等の機能不全(MODS)の予防又は治療も含む意味であり、本発明の医薬はSIRSやCARSが進行した病態の予防又は治療にも極めて有用である。
さらに、本発明者らは鋭意研究の結果、本発明のリボフラビン系化合物を有効成分とする医薬が、感染に起因しない全身性炎症反応症候群の予防又は治療においても顕著な効果を有することを見出した。
「感染に起因しない全身性炎症反応症候群」としては、外傷、熱傷、急性又は慢性の膵炎、アルコール性肝傷害、薬物性肝傷害、肝障害、肝炎、肝硬変、虫垂炎、胃潰瘍、虚血/再灌流障害、I型糖尿病、II型糖尿病、糖尿病の悪化、腹膜透析後の後遺症の予防・治療、又は手術に起因する全身性炎症反応症候群が好適に挙げられる。
前記サイトカイン抑制作用は、IL−1β、IL−6、IL−10、INF−γ、TNF−α、GM−CSF、IL−8、又はMCP−1のうちの少なくとも一つを抑制するものであることが好ましい。
本発明にいう「サイトカイン抑制作用」は、サイトカインの一種であるケモカインの抑制作用も含む意味である。ケモカインは、分子量約1万の保存された位置にシステイン残基を保有する、50を越えるサイトカインの総称であり、白血球に対して走化活性を示す。IL−1β(interleukin−1β;インターロイキン1β)、IL−6(interleukin−6)、IL−10(interleukin−10)、INF−γ(interferon−γ;インターフェロンγ)、TNF−α(tumor necrosis factor−α;腫瘍壊死因子)、GM−CSF(granulocyte macrophage−colony stimulating factor)はサイトカイン、IL−8(interleukin−8)、MCP−1(macrophage chemoattractant protein−1)はケモカイン(サイトカインの一種)である。
また、本発明は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを有効成分とする、感染に起因しない全身性炎症反応症候群の予防又は治療に用いられる医薬を提供する。
「感染に起因しない全身性炎症反応症候群」としては、外傷、熱傷、膵炎、肝障害、虚血/再灌流障害、又は手術に起因する全身性炎症反応症候群が好適に挙げられる。
上記において、前記リボフラビン誘導体又はその薬理学的に許容される塩は、フラビンモノヌクレオチド、フラビンアデニンジヌクレオチド、リボフラビンテトラブチレイト、リン酸リボフラビンナトリウム、リン酸リボフラビンのモノジエタノールアミン塩、ロイコフラビン、モノハイドロフラビン、ロイコフラビンリン酸エステル、ロイコフラビンモノヌクレオチド、ロイコフラビンアデニンジヌクレオチド、又はこれらの薬理学的に許容される塩であることが好ましい。
前記サイトカイン抑制作用は、活性酸素の消去作用を機序の一つとするものであってもよい。換言すれば、上記のリボフラビン系化合物投与による前記サイトカイン抑制作用は、活性酸素による組織の機能障害を防御する作用、すなわち活性酸素の消去作用を機序の一つとするものであってもよい。
本発明に係る医薬を投与する対象物は、ヒトまたは動物である。本発明の医薬は特にヒトにおける予防又は治療に有用である。
本発明において動物とは産業動物、伴侶動物および実験動物を指す。産業動物とはウシ、ウマ、ブタ、ヤギ、ヒツジ等の家畜、ニワトリ、アヒル、ウズラ、七面鳥、ダチョウ等の家禽、ブリ、ハマチ、マダイ、マアジ、コイ、ニジマス、ウナギ等の魚類など産業上飼養することが必要とされている動物である。また、伴侶動物とはイヌ、ネコ、マーモセット、小鳥、ハムスター、金魚などのいわゆる愛玩動物、コンパニオン・アニマルを指し、実験動物とはラット、モルモット、ビーグル犬、ミニブタ、アカゲザル、カニクイザルなど医学、生物学、農学、薬学等の分野で研究に供用される動物を示す。
本発明に係る医薬の投与形態としては、特に限定されず病態やその進行状況、その他の条件によって異なるが、前記リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを、注射剤、錠剤、顆粒剤、散剤、細粒剤、カプセル剤、丸剤、又は経口液剤(シロップ剤を含む)の形で含有することが好ましい。サイトカイン抑制作用のさらなる向上の観点からは、注射剤により投与することが好ましい。
注射剤の形で投与する場合には、静脈内、腹腔内、筋肉内、皮下、皮内、関節内、滑液嚢内、胞膜内、骨膜内等に投与することが好ましく、特に静脈内投与又は腹腔内投与が好ましい。静脈内投与は、点滴投与、ボーラス投与(bolus)のいずれであってもよい。
投与量(有効成分量)は、特に限定されず病態やその進行状況、その他の条件(投与する対象の種類、症状、年齢、体重、性別、合併症、投与時間、投与方法、剤型、感受性差等)によって異なるが、注射剤を静脈内に点滴投与する場合には、0.001〜10mg/kg/hにて、数分〜1週間投与することが好ましい。静脈内にボーラス投与する場合には、0.1〜50mg/kg、好ましくは0.3〜20mg/kgであり、さらに好ましくは2〜20mg/kgである。腹腔内投与する場合には0.1〜50mg/kg、好ましくは0.3〜20mg/kgであり、さらに好ましくは2〜20mg/kgである。筋肉内投与する場合には0.1〜50mg/kg、好ましくは2〜20mg/kgである。経口投与する場合には1〜1000mg/kg、好ましくは10〜500mg/kg、さらに好ましくは30〜200mg/kgである。
本発明に係る医薬は、そのまま投与してもよく、また通常用いられる製剤添加剤を加えて、公知の方法により注射剤(静脈内投与用(点滴用、ボーラス投与用)、腹腔内投与用、筋肉内投与用、皮下投与用等)、経口剤(錠剤、顆粒剤、散在、細粒剤、カプセル剤、丸剤、経口液剤、シロップ剤等)、経皮吸収製剤、点眼剤、点鼻剤、坐剤、吸入剤(エアゾール剤、粉末状吸入剤、液状吸入剤等)、外用剤(軟膏剤、クリーム剤、液剤等)とすることができる。また、食品や飼料、飲水等に混合することもできる
注射剤を製造する場合には、リボフラビン系化合物に、必要に応じて、溶解補助剤、pH調整剤、緩衝剤、懸濁化剤、抗酸化剤、保存剤、等張化剤などを添加し、常法により製造することができる。溶解補助剤等を輸液として点滴剤を構成してもよく、あるいはこれらに薬理学的に許容される改変を加えたものを輸液として点滴剤を構成してもよい。これらの注射剤は静脈内、腹腔内、筋肉内、皮下等に投与することができる。あるいは、凍結乾燥して、用時溶解型の凍結乾燥製剤としてもよい。
経口用固形製剤を製造する場合には、リボフラビン系化合物に、必要に応じて、賦形剤、結合剤、崩壊剤、滑沢剤、着色剤、矯味矯臭剤、抗酸化剤、溶解補助剤、安定化剤などを添加し、常法により錠剤、被覆錠剤、顆粒剤、散剤、細粒剤、カプセル剤(硬カプセル剤、軟カプセル剤)、丸剤等にすることができる。
溶解補助剤としては、特に限定されないが、例えば、生理食塩水、リン酸緩衝生理的食塩水、乳酸化リンゲル溶液、ポリオキシエチレン硬化ヒマシ油、ポリソルベート80、ニコチン酸アミド、ポリオキシエチレンソルビタンモノラウレート、マクロゴール、ヒマシ油脂肪酸エチルエステル等が挙げられる。
pH調整剤や緩衝剤としては、特に限定されないが、例えば、有機酸又は無機酸及び/又はその塩や、水酸化ナトリウム、メグルミン等が挙げられる。
懸濁化剤としては、特に限定されないが、例えば、メチルセルロース、ポリソルベート80、ヒドロキシエチルセルロース、アラビアゴム、カルボキシメチルセルロースナトリウム、ポリオキシエチレンソルビタンモノラウレート、トラガント末等が挙げられる。
抗酸化剤としては、特に限定されないが、例えば、アスコルビン酸、α−トコフェロール、エトキシキン、ジブチルヒドロキシトルエン、ブチルヒドロキシアニソール等が挙げられる。
保存剤としては、特に限定されないが、例えば、パラオキシ安息香酸メチル、パラオキシ安息香酸エチル、ソルビン酸、フェノール、クレゾール、クロロクレゾール等が挙げられる。
等張化剤としては、特に限定されないが、例えば、塩化ナトリウム等が挙げられる。
賦形剤としては、特に限定されないが、例えば、デンプン、コーンスターチ、デキストリン、ブドウ糖、乳糖、白糖、糖アルコール(マンニトール、エリスリトール、キシリトール等)、硬化油、結晶セルロース、無水珪酸、珪酸カルシウム、第二リン酸水素カルシウム等が挙げられる。
結合剤としては、特に限定されないが、例えば、ポリビニルピロリドン、エチルセルロース、メチルセルロース、α化デンプン、アラビアゴム、ヒドロキシプロピルセルロース、ヒドロキシプロピルメチルセルロース、カルボキシメチルセルロース、カルボキシメチルセルロースナトリウム、プロピレングリコール、ポリアクリル酸ナトリウム、ポリビニルアルコール等が挙げられる。
崩壊剤としては、特に限定されないが、例えば、クロスポビドン、低置換度ヒドロキシプロピルセルロース、架橋型カルボキシメチルセルロースナトリウム、カルボキシメチルセルロース、炭酸カルシウム、カルボキシメチルセルロースカルシウム等が挙げられる。
滑沢剤としては、特に限定されないが、例えば、ステアリン酸マグネシウム、タルク、ステアリン酸カルシウム、フマル酸ステアリルナトリウム、ポリエチレングリコール6000等が挙げられる。
錠剤、顆粒剤、散剤の場合には必要に応じてヒドロキシプロピルメチルセルロース等の皮膜コーティングを施しても良い。
経口液剤を製造する場合には、リボフラビン系化合物に、必要に応じて、着色剤、矯味矯臭剤、抗酸化剤、溶解補助剤、安定化剤などを添加し、常法により製造することができる。
また、本発明は、高サイトカイン血症の予防剤又は治療剤を製造するための、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つの使用を提供するものである。換言すれば、本発明は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つの使用による高サイトカイン血症の予防剤又は治療剤の製造方法を提供するものである。
また、本発明は、高サイトカイン血症の予防剤又は治療剤としての、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つの使用を提供するものである。
また、本発明は、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを対象物(例えば、動物)に投与することを特徴とするサイトカイン抑制を行う必要のある疾患(例えば、炎症性疾患、高サイトカイン血症、全身性炎症反応症候群)の治療方法を提供するものである。
実施例
次に、実施例を示して本発明をさらに詳しく説明するが、本発明はこれらに限定されるものではない。
〔実験で用いた材料〕
5’−リボフラビンナトリウムリン酸エステル(リン酸リボフラビンナトリウム)は、日本薬局方品のリボフラビンナトリウムリン酸エステル(5’−FMN−Na)を合成し、これを生理食塩水(Otsuka Pharmaceutical Co.,Ltd.,Tokyo,Japan)に溶解(3mg/mL)したものを用いた。
LPS(Lipopolysaccharide:リポポリサッカライド)は、大腸菌(Escherichia.coli)血清型O111:B4由来リポポリサッカライドをシグマ社(Sigma Chemical Co.,St.Louis,MO,USA)から購入し、これを生理食塩水に溶解したものを用いた。
ELISA(Enzyme linked−immuno−sorbent assay)キットとしては、IL−1β、IL−6、IL−10、INF−γ、TNF−α、GM−CSF、MCP−1、の各濃度の測定にはバイオソースインターナショナル社(Biosource International Inc.,Camarillo,CA,USA)から購入したキットを用いた。MIP−2濃度の測定にはテクネ社(TECHNE Corporation,Minneapolis,MN,USA)から購入したキットを用いた。なお、MIP−2は、マウスのケモカインであり、好中球の走化因子である。ヒトではIL−8に相当する。
NO2/NO3アッセイキットは、同仁化学研究所(Dojindo Laboratories,Kumamoto,Japan)から購入したものを用いた。
SEB(黄色ブドウ球菌毒素)は、スタフィロコッカス・アウレウス・エンテロトキシン(Staphylococcus aureus enterotoxin)B,BT−202を用いた。
TNF−αは、ペプロテック社(Pepro Tech,Inc.,Rock Hill NJ,USA)からヒトTNF−α K051を購入し、これを生理食塩水(Otsuka Pharmaceutical Co.,Ltd.,Tokyo,Japan)に溶解(15μg/mL)したものを用いた。
D−ガラクトサミン(D−galactosamine)は、和光純薬工業株式会社(Wako Pure Chemical Industries,Ltd.,Osaka,Japan)から塩酸D−ガラクトサミンを購入し、これを生理食塩水(Otsuka Pharmaceutical Co.,Ltd.,Tokyo,Japan)に90mg/mLにて溶解したものを用いた。
AAPH(2,2’−アゾビス(2−アミジノプロパン)ジハイドロクロライド;2,2’−Azobis(2−amidinopropane)dihydrochloride)は、和光純薬工業株式会社から購入し、これを生理食塩水に溶解(18.75mg/mL)したものを用いた。AAPHはフリーラジカル開始剤であり、熱分解によってフリーラジカルを発生する水溶性のアゾ化合物である。
〔実施例1:エンドトキシン誘発ショックマウスモデルに対する効果〕
雄ICRマウスは、5週齢のものをJapan SLC Inc.(Shizuoka,Japan)から購入し、23℃(許容範囲:20〜26℃)、相対湿度55%(許容範囲:40〜70%)の条件下で、12時間毎の明暗サイクル(午前7時にライトオンし、午後7時にライトオフする)において収容したものを用いた。この際、マウスは、滅菌水道水や通常のえさ(MF,Oriental Yeast Co.Ltd.,Tokyo,Japan)にアクセス自由な状態で収容した。この環境に1週間順応させた後、実験に供した。
まず、LPS12mg/kgを雄ICRマウス(6週齢)に静脈内投与して、エンドトキシン誘発ショックマウスを作成した。
コントロールグループにおいては、LPSの投与時(0時間)から、1時間、3時間、6時間、9時間、12時間、15時間、18時間、21時間、及び24時間経過後にそれぞれ採血を行った。5’−FMN−Na投与グループにおいては、LPSの投与時から6時間経過後に、20mg/kgの5’−FMN−Naを静脈内に1回注入により投与して、LPSの投与時から9時間、12時間、15時間、18時間、21時間、及び24時間経過後にそれぞれ採血を行った。いずれの採血ポイントにおいても各グループ5例で構成した。実験の途中で死亡した個体はデータとして使用しなかった。採血にはEDTA(ethylene diamine tetraacetic acid)を含むプラスチック製注射器を用い、腹部の静脈から採取した。採取した各血液を遠心分離機(3000rpm、10分、4℃)にかけ血漿を分離した後、−80℃にて保存した。
次に、採取した各血漿中のサイトカイン濃度、ケモカイン濃度、及びNO(一酸化窒素)濃度を測定した。
血漿中のサイトカイン濃度及びケモカイン濃度は、上記ELISAキットを用いてELISA法により測定した。プレートリーダ(Spectra max 250,Molecular Devices Corporation,Sunnyvale,CA)により450nmにてプレートを読み取り、得られたデータをSOFT max PRO 1.1(Molecular Devices Corporation,Sunnyvale,CA)を用いて分析した。
上記測定においては、測定限界は、MIP−2については1.5pg/mL以下、IL−6については3.0pg/mL以下、IL−1βについては7.0pg/mL以下、INF−γについては1.0pg/mL以下、TNF−αについては3.0pg/mL以下、MCP−1については9.0pg/mL以下、IL−10については0.9pg/mL以下、GM−CSFについては1.0pg/mL以下、とし、測定限界以下の値については0pg/mLとした。
血漿中のNO濃度は、NO2/NO3アッセイキットを用い、NO2とNO3の合計量を検出することにより測定した。各血漿は、それぞれマイクロコンセントレータ(Amicon:Beverly,MA,USA)によって濾過した後、遠心分離機(15000rpm、15分、4℃)にかけた。遠心分離機にかけた血漿サンプルに、NO3還元酵素を添加し、NO3をNO2に転換させ、グリス試薬(Gress reagent)を用いて総NO2量を測定した。プレートリーダ(Spectra max 250)により540nmにてプレートを読み取り、得られたデータについて上記SOFT max PRO 1.1を用いて分析した。
コントロールグループと5’−FMN−Na投与グループとの差異を対応のないt検定により分析した。統計学的分析は、SAS 6.12(SAS Institute Japan Inc.,Tokyo,Japan)を用いて行い、P値が0.05(two−sided)より小さい場合に統計学的に有意であると判断した。
上記のようにして測定したサイトカイン濃度の測定結果を図1〜図6に、ケモカイン濃度の測定結果を図7及び図8に、NO濃度の測定結果を図9に示す。
なお、図1〜9においては、各プロットは平均値±標準誤差で示した。白抜きされたプロットはコントロールグループを示し、白抜きされていないプロットは5’−FMN−Na投与グループを示す。
図1に示すように、血漿中のIL−1β濃度は、LPSの投与時から9時間経過後にピークを示し、その後徐々に減少した。一方、5’−FMN−Na投与グループ(LPS投与時から6時間経過後に5’−FMN−Naを投与)においては、5’−FMN−Na投与直後からIL−1β濃度が減少し初め、LPS投与から9時間〜12時間経過の間においてIL−1β濃度がすみやかに低減した。
図2に示すように、血漿中のINF−γ濃度は、LPSの投与時から6時間経過後にピークを示し、その後徐々に減少した。一方、5’−FMN−Na投与グループにおいては、9時間〜18時間経過の間においてINF−γ濃度がすみやかに低減した。
図3に示すように、血漿中のIL−6濃度は、LPSの投与後急増し1時間経過後にピークを示した後、徐々に減少した。コントロールグループにおいては、IL−6濃度はLPS投与時から24時間経過後においても依然として高い値を示した。5’−FMN−Na投与グループにおいては、6時間経過後に5’−FMN−Naを投与したことにより、IL−6濃度がすみやかに減少し、その状態が21時間経過後まで維持された。
図4に示すように、血漿中のGM−CSF濃度は、1時間〜6時間経過の間においてピークを示し、9時間経過後までにはもとの値まで減少した。その後、LPS投与時から21時間経過後において、2回目のピークが観察された。一方、5’−FMN−Na投与グループにおいては、6時間経過後に5’−FMN−Naを投与したことにより、LPS投与時から21時間経過後においてGM−CSF濃度を減少させる傾向が認められた。
図5及び図6に示すように、血漿中のIL−10濃度とTNF−α濃度は、LPS投与時から1時間経過後と21時間経過後においてそれぞれ明確な2つのピークを示した。5’−FMN−Naを投与したことにより、2回目のピーク時においてIL−10濃度及びTNF−α濃度が減少した。
図7に示すように、血漿中のMCP−1濃度は、LPS投与後に急増し6時間経過時にピークを示した。その後、MCP−1濃度は徐々に低減した。5’−FMN−Naを投与したことにより、MCP−1濃度は減少され、この状態が21時間経過後まで維持された。
図8に示すように、血漿中のMIP−2濃度は、LPS投与後に急増し1時間〜3時間経過の間においてピークを示した後減少した。その後、コントロールグループにおいては21時間経過後において2回目のピークが観察されたが、5’−FMN−Na投与グループにおいては9時間経過後にMIP−2濃度が減少し、この状態が21時間経過後まで維持された。
図9に示すように、NO濃度はLPS投与後急増し9時間経過後にピークを示した。この高いNO値は24時間経過後まで続いた。NO値の増加はサイトカインの誘導に関係していると考えられる。5’−FMN−Naは9時間〜18時間経過の間においてNO値の抑制効果を示さなかったが、21時間〜24時間経過の間においてはNO値の抑制効果を示した。
以上のように、5’−FMN−Naはエンドトキシンによって誘発された血中サイトカイン、ケモカイン、及びNOの濃度上昇を抑制する作用があることが分かった。
〔実施例2:エキソトキシン誘発ショックマウスモデルに対する効果〕
雄BALB/cマウスは、5週齢のものを日本チャールスリバーから購入し、前記雄ICRマウスと同様の条件で1週間順応させたものを、実験に供した。
まず、SEB0.75mg/kg及びD−ガラクトサミン1.8g/kgを、雄BALB/cマウス(6週齢)に腹腔内投与して、エキソトキシン(SEB)誘発ショックマウスを作成した。
コントロールグループにおいては、SEBの投与時(0時間)から、6時間、9時間、12時間、15時間、及び18時間経過後にそれぞれ採血を行った。5’−FMN−Na投与グループにおいては、SEBの投与時から6時間経過後に、20mg/kgの5’−FMN−Naを静脈内に1回注入により投与して、SEBの投与時から9時間、12時間、15時間、及び18時間経過後にそれぞれ採血を行った。いずれの採血ポイントにおいても各グループ5例で構成した。実験の途中で死亡した個体はデータとして使用しなかった。採血にはEDTAを含むプラスチック製注射器を用い、腹部の静脈から採取した。採取した各血液を遠心分離機(3000rpm、10分、4℃)にかけ血漿を分離した後、−80℃にて保存した。
次に、採取した各血漿中のサイトカイン濃度及びケモカイン濃度を実施例1と同様にして測定・分析・プロットした。得られたサイトカイン濃度及びケモカイン濃度の測定結果を図10〜図12に示す。
図10に示すように、血漿中のINF−γ濃度は、SEBの投与時から9時間経過後にピークを示し、その後減少した。一方、6時間経過後に5’−FMN−Naを投与したところ、12時間経過付近においてINF−γ濃度の抑制効果が認められた。
図11に示すように、血漿中のMIP−2濃度は、SEBの投与時から12時間経過後にピークを示し、その後減少した。SEB投与から6時間経過後に5’−FMN−Naを投与したところ、12時間経過付近においてMIP−2の抑制効果が認められた。
図12に示すように、IL−6濃度はSEBの投与時から12時間経過後にピークを示した。SEB投与から6時間経過後に5’−FMN−Naを投与したところ、9時間〜12時間経過付近においてIL−6濃度の抑制傾向が認められた。
以上のように、5’−FMN−Naはエキソトキシンによって誘発された血中サイトカイン及びケモカインの濃度上昇を抑制する作用があることが分かった。
〔実施例3:TNF−α誘発ショックマウスモデルに対する効果〕
雌のICRマウスは、4週齢のものをJapan CRJ Inc.(Kanagawa,Japan)から購入した。雄のBALB/cマウスは、5週齢のものをJapan SLC Inc.(Shizuoka,Japan)から購入した。これらのマウスをそれぞれ、23℃(許容範囲:20〜26℃)、相対湿度55%(許容範囲:40〜70%)の条件下で、12時間毎の明暗サイクル(午前7時にライトオンし、午後7時にライトオフする)において収容した。この際、マウスは、滅菌水道水や通常のえさ(MF,Oriental Yeast Co.Ltd.,Tokyo,Japan)にアクセス自由な状態で収容した。この環境に1週間順応させた後、実験に供した。
まず、3μgのヒトTNF−αと18mgのD−ガラクトサミン(0.2mL/体重30g)を、一群各10例の雌ICRマウス(5週齢)に腹腔内注入した。TNF−α及びD−ガラクトサミンを投与した直後に、マウスに20mg/kgの5’−FMN−Na(5’−FMN−Na投与グループ)又は生理食塩水(コントロールグループ)を静脈内注入した。TNF−αを注入してから7日間経過後におけるマウスの生存率(%)を観察した。その結果を表1に示す。
なお、実施例3においても、スチールテスト(Steel teat)によって分析した。統計学的分析は、実施例1と同様のSAS 6.12を用いて行い、P値が0.05(two−sided)より小さい場合に統計学的に有意であると判断した。
表1に示すように、コントロールグループの生存率は20%であったのに対して、5’−FMN−Na投与グループの生存率は70%であった。なお、コントロールグループのマウスは8匹とも3日以内に死亡した。
次に、3μgのヒトTNF−αと18mgのD−ガラクトサミン(0.2mL/体重30g)を、一群各10例の雄BALB/cマウス(6週齢)に腹腔内注入した。TNF−α及びD−ガラクトサミンを投与した直後に、マウスに20mg/kgの5’−FMN−Na(5’−FMN−Na投与グループ)又は生理食塩水(コントロールグループ)を静脈内注入した。TNF−αを注入してから7日間経過後におけるマウスの生存率(%)を観察した。その結果を表2に示す。
表2に示すように、コントロールグループの生存率は10%であったのに対して、5’−FMN−Na投与グループの生存率は60%であった。なお、コントロールグループのマウスは9匹とも3日以内に死亡した。
以上の結果から、5’−FMN−Naを投与することによりTNF−α誘発ショックに対する生存率を著しく増加させることが分かった。すなわち、5’−FMN−Naを投与することにより、トキシン誘発ショックだけでなく、TNF−α誘発ショックに対する生存率を著しく増加させることが分かった。
次に、TNF−α誘発マウス腹腔マクロファージからのIL−6産生に対する5’−FMN−Naの添加による効果を検討した。雄のICRマウスをエーテル麻酔下にて頸動脈から放血させて死亡させ、4.5mL/マウスのハンクス液(HBSS:Gibco,Laboratories Grand Island,NY,USA)を腹腔内注入した後、腹腔液を採取した。採取した腹腔液を、冷却したHBSSとともに、遠心分離機(1000rpm、10分)にかけて、腹腔マクロファージを分離した。
これらの細胞は、15%の不活性化マウス血清を添加したイーグル培養液(Eagles minimal essential medium;Nissui Pharmaceutical Co.,Tokyo,Japan)に1×106細胞/mLで懸濁させ、37℃で5%のCO2存在下にて、Lab−Tekチャンバー(Nalgen Nunc International,Naperville,IL,USA)内で培養させた。マウス腹腔マクロファージに、1.56μg/mL又は25μg/mLの5’−FMN−Na存在下で、あるいは5’−FMN−Naの非存在下で、10ng/mLのTNF−αを反応させ、24時間培養した。
IL−6の測定には、上記ELISAキットを用いた。測定した結果を図13に示す。
図13に示すように、TNF−αが10ng/mL添加されたマウス腹腔マクロファージでは、IL−6のレベルが上がった。一方、5’−FMN−Naが1.56μg/mL又は25μg/mLがさらに添加されたマウス腹腔マクロファージでは、誘導されたIL−6が抑制されることが分かった。
〔実施例4:AAPH中毒マウスモデルに対する効果〕
実施例4においては、実施例1と同様のものを購入し、同様の条件で順応させた雄ICRマウスを用いた。
一群各10例の雄ICRマウス(6週齢)に、125mg/kgのAAPH(0.2mL/体重30g)を腹腔内注入した。AAPHの注入から24時間前、注入直後、1時間後、の各時において、マウスに20mg/kgの5’−FMN−Na(5’−FMN−Na投与グループ)又は生理食塩水(コントロールグループ)を静脈内注入した。AAPHを注入してから7日間経過後におけるマウスの生存率(%)を観察した。その結果を表3に示す。
なお、実施例4においては、5’−FMN−Na投与グループ及びコントロールグループ間の差異はスチールテスト(Steel test)によって分析した。統計学的分析は、実施例1と同様のSAS 6.12を用いて行い、P値が0.05(two−sided)より小さい場合に統計学的に有意であると判断した。
表3に示すように、コントロールグループの生存率は20%であったのに対して、5’−FMN−Na投与グループの生存率は60%であった。なお、コントロールグループのマウスは8匹とも2日以内に死亡した。
次に、AAPH注入から24時間前、注入直後、の各場合において、5’−FMN−Na又は生理食塩水を、静脈内注入に代えて腹腔内注入した以外は上記と同様にして、AAPHを注入してから7日間経過後におけるマウスの生存率(%)を観察した結果を表4に示す。
表4に示すように、コントロールグループの生存率は10%であったのに対して、5’−FMN−Na投与グループの生存率は70%であった。なお、コントロールグループのマウスは9匹とも3日以内に死亡した。
以上の結果から、5’−FMN−Naを投与することにより、AAPH中毒に対する生存率を著しく増加させることが分かった。すなわち、AAPHはフリーラジカル開始剤であることから、5’−FMN−Naを投与することにより活性酸素による組織の機能障害を防御する効果が得られることが分かった。
また、TNF−αが細胞膜受容体や細胞片の損傷部における活性酸素を増加させる作用があることが知られている(Chandel NS,Trzyna WC,McClintock DS and Schumacker PT:Role of oxidants in NF−kappa B activation and TNF−alpha gene transcription induced by hypoxia and endotoxin.J Immunol 15,1013−1021(2000))ことから、5’−FMN−Na投与によるTNF−α抑制作用は、5’−FMN−Na投与による活性酸素の消去作用を機序の一つとしていることが分かった。
〔実施例5:ブドウ糖高負荷時の内皮細胞の活性酸素産生に対する効果〕
プラスチックシャーレ上に培養したヒト血管内皮細胞(HUVEC細胞)に、5’−FMN−Na濃度がそれぞれ6.25μg/mL、1.56μg/mL、0.39μg/mL、0.1μg/mLとなるように5’−FMN−Naを添加した後、CO2インキュベータで37℃にて、3時間培養した。次に、各シャーレにおけるブドウ糖濃度が30mMになるようにブドウ糖(グルコース)を添加した後、CO2インキュベータで37℃にて、1時間培養した。培養終了後、細胞を集め、BURSTTEST(細胞内活性酸素測定キット;ORPEGEN Pharma)を用いて、細胞内活性酸素量をFACS Calibur(フローサイトメトリーシステム;日本ベクトン・ディッキンソン)にて測定した。具体的には、FACS Caliburを用いFL1の平均蛍光強度から細胞10000個の活性酸素量を測定した。その結果を図14に示す。
図14に示すように、コントロールグループの蛍光強度(平均蛍光値)は91.2であったが、30mMのブドウ糖を負荷すると活性酸素が産生し、蛍光強度は140.5に上昇した。一方、6.25μg/mLの5’−FMN−Naを添加すると蛍光強度は89.5に低下し、活性酸素の産生を抑制した。さらに、1.56μg/mLの添加では蛍光強度は111.9であり、0.39μg/mLの添加では蛍光強度は125.0であり、0.1μg/mLの添加では蛍光強度は114.2であった。
以上の結果から、5’−FMN−Naを添加することにより、高濃度のブドウ糖負荷による活性酸素産生を抑制することが示された。
さらに、シャーレ上に培養したHUVEC細胞に、5’−FMN−Na濃度がそれぞれ25μg/mL又は6.25μg/mLとなるように5’−FMN−Naを添加した後、CO2インキュベータで37℃にて、3時間培養した。次に、各シャーレにおけるブドウ糖濃度が30mMとなるようにブドウ糖を添加した後、CO2インキュベータで37℃にて、2時間培養した。培養終了後、培養液中のIL−8をELISAキットで測定した。その結果を図15に示す。
図15に示すように、コントロールグループにおいては、30mMのブドウ糖を負荷するとIL−8を産生し、培地中のIL−8濃度は1196pg/mLに上昇した。一方、25μg/mLの5’−FMN−Naを添加すると培地中のIL−8濃度は168に低下し、6.25μg/mLを添加すると培地中のIL−8濃度は297pg/mLに低下し、IL−8の産生を有意に抑制した(Dunnett検定)。
以上の結果から、5’−FMN−Naの添加により高濃度のブドウ糖負荷によるIL−8産生を抑制することが示された。実施例5の結果から、5’−FMN−Naを有効成分とする剤が、糖尿病や糖尿病による悪化病態に対する治療・予防剤として作用することが示唆された。
産業上の利用可能性
本発明の医薬は、優れたサイトカイン抑制作用を有し、高サイトカイン血症を伴う炎症性疾患の予防又は治療剤等として有用である。

Claims (15)
- リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを有効成分とし、サイトカイン抑制作用を有する医薬。
- 高サイトカイン血症の予防又は治療に用いられる請求項1記載の医薬。
- 全身性炎症反応症候群の予防又は治療に用いられる請求項1記載の医薬。
- 前記全身性炎症反応症候群は、感染に起因しないものである請求項3記載の医薬。
- 前記全身性炎症反応症候群は、外傷、熱傷、膵炎、肝障害、虚血/再灌流障害、又は手術に起因するものである請求項4記載の医薬。
- 前記サイトカイン抑制作用は、IL−1β、IL−6、IL−10、INF−γ、TNF−α、GM−CSF、IL−8、又はMCP−1のうちの少なくとも一つを抑制するものである請求項1〜5のいずれか一項に記載の医薬。
- リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを有効成分とする、感染に起因しない全身性炎症反応症候群の予防又は治療に用いられる医薬。
- 前記全身性炎症反応症候群は、外傷、熱傷、膵炎、肝障害、虚血/再灌流障害、又は手術に起因する請求項7記載の医薬。
- 前記リボフラビン誘導体又はその薬理学的に許容される塩は、フラビンモノヌクレオチド、フラビンアデニンジヌクレオチド、リボフラビンテトラブチレイト、リン酸リボフラビンナトリウム、リン酸リボフラビンのモノジエタノールアミン塩、ロイコフラビン、モノハイドロフラビン、ロイコフラビンリン酸エステル、ロイコフラビンモノヌクレオチド、ロイコフラビンアデニンジヌクレオチド、又はこれらの薬理学的に許容される塩である請求項1記載の医薬。
- 前記リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを、注射剤、錠剤、顆粒剤、散剤、細粒剤、カプセル剤、丸剤、又は経口液剤の形で含有する請求項1記載の医薬。
- 前記サイトカイン抑制作用は、活性酸素の消去作用を機序の一つとする請求項1記載の医薬。
- 高サイトカイン血症の予防剤又は治療剤を製造するための、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つの使用。
- 高サイトカイン血症の予防剤又は治療剤としての、リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つの使用。
- リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを有効成分とするサイトカイン抑制剤。
- リボフラビン、リボフラビン誘導体、又はこれらの薬理学的に許容される塩の少なくとも一つを対象物に有効量投与する、高サイトカイン血症の予防又は治療方法。
