Description

【発明の詳細な説明】 光里公苛厭 本発明は、外科用ステープラに関し、さらに詳しくは複
数の外科用ファスナを人体組織に実質的に同時に作用さ
せるための外科用ステープラに関する。
複数の外科用ファスナを実質的に同時に作用させて外科
用ファスナの整列を作る外科用ファスナは知られている
。
典型的には、この装置は、締結すべき組織の１方の側に
配置されたファスナ保持器と、締結すべき組織の他方の
側のファスナ保持器に平行なアンビル組立体と、ファス
ナ保持器とアンビル組立体との間で組織が締めつけられ
るようにファスナ保持器とアンビル組立体とを互いに向
かい合わせて直線的に移動させるための装置と、ファス
ナの端がアンビル装置と接触するにつれてファスナの端
が組織を貫通して、締結されたファスナを形成し、それ
によって組織に締結されたファスナの整列を形成するよ
うにファスナをファスナ保持器から駆動させるための装
置とを含む。ここで「ファスナ」は、金属の外科用ステ
ーブル、２つの部品からなる樹脂製外科用ファスナのス
テーブル形の部分、およびそれらと同等なものの包括的
用語として使用する。同様に、用語「アンビル組立体」
は、金属製外科用ステープルの先を曲げ潰すのに使用さ
れるアンビル、リテーナ保持器、２つの部品からなる樹
脂製外科用ファスナのリテーナ部材、およびこれらの要
素の同等なものの包括的な用語として使用する。
一般的に使用されているのは、ファスナ保持器とアンビ
ル組立体とがカートリッジを支持し、かつ、これを操作
するための永久アクチュエータに取り外し可能に取り付
けられた使い捨てカートリッジからなるステープラであ
る。カートリッジは１回の使用後に使い捨てられる。永
久アクチュエータは、同じ外科処置では、新しいカート
リ・ノジの細装填後に再び使用することができ、別の外
科処置では、掃除、滅菌、および再装填後に再び使用す
ることができる。同様に利用できるのは、カートリッジ
とアクチュエータとが予め組立られ、使用のために準備
され、次いで１回の使用後は処分される使い捨て外科用
ステープラである。
これらのステープラを使用する際には、ファスナ保持器
とアンビル組立体とが互いに正確に位置決めされ、その
結果、組織の接合処置が正しい仕方で行われることが非
常に重要である。米国特許第４．３５４．６２８号に開
示された型式の外科用ステープラでは、締結すべき組織
がステープル保持器とアンビル組立体との間に完全に封
じ込まれる（該特許第４図に示されるように）、即ち、
すべての組織は、ステープラ保持器とアンビル組立体の
端を越えて延びないようになっている。これにより、ス
テープル保持器とアンビル組立体とは、該特許に開示さ
れ特許請求されたスペーサ部材によって互いに正確に位
置決めされる。しかしながら、ステープラの２つの脚の
中に締結すべき組織を完全に封じ込めることができない
成る外科処置がある。かかる状況では、ステープラの２
つの脚を越えて延びる組織は、スペーサ部材がアンビル
組立体に載るのを妨げ、その結果、ファスナが正確に形
成されないことがあり得る。
ステープラの脚の端を越えて延びるＭｉ織に外科用ファ
スナを適用するために、米国特許第３．４９４．５３３
号に示される型式の器具を使用することが知られている
。かかる状況では、組織が十分に柔らかいならば、整合
ピンを組織に押し通すか、または、外科医が、組織に整
合ピンを押し通す前に、手で組織を切断しなければなら
ない。米国特許第３．４９４．５３３号に開示された型
式の器具に使用されているカートす・ノジとアンビル組
立体とは、カートリッジとアンビルとが配置されている
器具の脚の平行な位置決めによって互いに平行に整合す
る。
米国特許第４．３５４．６２８号に記載されるように、
−該特許のファスナ保持器とアンビル組立体との間の正
確な整合は、主としてこれらの要素自身によって維持さ
れ、その整合を与える器具に頼らないことが望ましい。
従って、本発明の目的は、接合すべき組織がステープラ
を越えて延びるときにもファスナ保持器とアンビル組立
体とが実質的に平行であることを確実にする米国特許第
４．３５４．６２８号に示される型式の外科用ステープ
ラを提供することである。
光１ど」許 本発明の原理に従って、本発明の以上の目的および他の
目的は、ファスナ保持器のスペーサ部材組織を変位させ
る（もし変位させないものとすればスペーサ部材が正確
に当接しなくなってしまう）ことを特徴とする外科用ス
テープラを提供することによって達成される。整合ビン
は、同様に、組織を貫いて押し、かつ、ステープラの遠
い方の部分の中に押すことを確実にできるように成形さ
れている。これにより、外科医による更に別の、手動操
作を必要とすることな（、ファスナ保持器とアンビル組
立体との相対的位置決めが維持される。
かくして、患者が手術室で過ごす時間を少なくすること
ができる。同様に、ステープラは、成る外科処置に関し
て整合ビンを押す前に組織を切断する必要を避けること
によって以前に知られた技術から改良された方法で組織
を変位させることで、組織に対する外傷を減する。
本発明の更に別の特徴として、本発明の性質と種々の利
点が添付図面および本発明の以下の詳細な説明からより
明らかになるであろう。
光１■圧里呈ｌ凱 本発明の原理は他の同様な型式の外科用ステープラに適
用可能であるけれども、本発明は、上記の型式の外科用
ステープラへの適用の説明から明確に理解されるであろ
う。同様に、本発明は、永久および使い捨てステープラ
の両方に適用可能である。従って、本発明を、ファスナ
保持器とアンビル組立体とからなる使い捨てカートリッ
ジが永久器具に取り付けられた実施例について例解する
けれども、本発明を、使用に際して全部が使い捨て可能
な実施例において同様に説明することができよう。
本発明の外科用ステープラは、米国特許第４．３５４．
６２８号に示された型式であるのが良い。
好ましい実施態様はかかる外科用ステープラであり、該
特許の全開示を参考としてここに示す。該特許は、Ｍｉ
織片スペーサ部材を開示し、かつ、これについて特許請
求しているが、本発明はその改良形である。この改良形
によって、組織全部がファスナ保持器とアンビル組立体
との間の領域に取り囲まれることがない外科技術におい
てステープラを使用することが可能となる。
第１図に示すように、器具１０は、器具の近い方の５４
に隣接しているハンドル２０と、ハンドル２０に略３０
°から４５°の角度をなす長さ方向の連結構造３０と、
連結構造３０の遠い方の端の開いたＵ字形またはＶ字形
部分の支持構造４０とを含む。第３図でよりよくわかる
ように、支持構造４０は、近い方の訓４２、遠い方の脚
４４、および脚４２と脚４４の各、端を連結している基
部４６からなる。支持構造４０は、支持構造３０の長さ
方向の軸に実質的に平行な平面内にある。使用に際し、
例えば婦人外科では、組織が、一般的に脚４２と脚４４
との間にあり、かつ、支持構造４０の平面に横向きに走
るように、器具を、３■織１２に関して位置決めする。
スペーサ部材を有するごの型式の外科用ステープラの以
前に知られた使用に比べて、組織１２は、脚４２および
脚４４の開いた端を越えて延在するようになる。米国特
許第４．３５４．６２８号に示される型式の知られたス
テープラの使用は、組織全体を脚４２と脚４４間に収容
できる処置に限定されてきた。しかしながら、組織全体
の周囲にステープラを置くことは望ましくなく、かつ、
不可能な成る操作があり、本発明は、かかる処置にステ
ープラを使用することを可能にする。
第２図でより明確にわかるように、使い捨てカートリッ
ジ５０は、ファスナ保持器５２およびアンビル組立体５
２は、遠い方の脚４４の中に取り付けられ、ファスナ保
持器５２は、近い方の脚４２の中に取り付けられている
。枢軸５６があるカートリッジ５０の端は、基部４６に
挿入されている。枢軸５６によって、ファスナ保持器５
２の回転運動が起こり、枢軸５６はスロット１０５と協
働して、アンビル組立体の表面５３に垂直な保持器の運
動量を制限する。
アンビル組立体５４は、支持構造４０の脚４４の中にま
たここから外へ長さ方向に摺動するようになっている。
アンビル組立体５４の遠い方の側部は、遠い方の脚４４
のプレー１−／Ｉ　４ａとプレート４４ｂ間に嵌まった
遠い方に突出している保持用の構造５８を有している。
カートリッジ５０は、プレート４４ａと４４ｂ間に摩擦
嵌めによって解放可能に保持されており、脚４４におい
てアンビル組立体５４の相応する端が突出部４５３．４
５ｂおよ゛び切欠部４７ａおよび４７ｂに押し付けて置
くとき、アンビル組立体５４は、近い方の脚４２のプレ
ート４２ａおよびプレー＋−４２ｂ間に位置するように
なる。
例えば、第９図に見るように、アンビル組立体５４は、
ファスナ保持器５２に位置する、ファスナを含む孔６６
ａおよび６６ｂにそれぞれ整合するリテーナ格納孔６５
ａおよび６５ｂ、の２つの平行な列を有する。各ファス
ナ格納孔は、初めに１つのファスナを格納し、各リテー
ナ格納孔は、初めに１つのリテーナを格納する。これら
の孔は、リテーナに関連する列の１つのリテーナがファ
スナに関連する列の１つのファスナの反対側にあるよう
に更に整合している。ファスナの２つの尖った先端部は
、リテーナの２つの孔に整合している。
各ファスナの後方には、ブツシャ保持２Ｈ６０ａおよび
６０ｂ（第４図参照）に摺動可能に取り付けられたファ
スナブツシャ６２がある。ファスナの射出中、ファスナ
ブツシャ６２の近い方の端はずべて、ファスナ保持器５
２に同様に取り付けられたナイフファスナブツシャ部材
６３に接触する。
ファスナブツシャ６２は、ファスナブツシャ６２の突出
部６７が中に延びるブツシャ保持器６０２１および６０
ｂのスロット５９によって遠い方に向かって案内される
。ナイフファスナブツシャ部材６３への接近は、ファス
ナ保持器５２の近い方の側部の細長いスロット６８およ
びばね８１のスロノ１−８８を介して行われるが、この
ことは、以下で更に詳しく議論する。通常、ファスナ保
持器５２は、例えば第５図に示すように、板ばね５１お
よびばね７１によってアンビル組立体５４から離れる方
に付勢される。第４図でわかるように、板ばね５１の１
方の端は、アンビル組立体５４の舌状部１３６に取り付
けられている。仮ばね５１の他方の端は、ファスナ保持
器５２内の表面に載る。ばね７１は、スロット１０５の
遠い方の端に枢軸５６を付勢し、また突出部７５および
円筒状間隔７７によって所定位置に維持される。
同様に、ファスナ保持器５２は、整合ビン８０を支持す
る。カートリッジが開いているとき整合ピン８０の端が
カートリッジ５０の開いた端を部分的に遮断し、そのこ
とによって器具の中に配置されまたはそこから携り外さ
れる組織にあり得る危険が生じるのを防止するために、
整合ピン８０は往復可能にファスナ保持器５２に取り付
げられ、かつ、締結操作中には自動的にビンを延ばし、
カートリッジ５０が開かれるときには自動的にビンを引
っ込めるための装置を備えている。例えば、第４図に示
すように、ビン８０の近い方の端は、ファスナ保持器５
２の近い方の端は、ファスナ保持器５２の近い方の端に
沿って延び、かつ、ファスナ保持器５２の底に固定され
ている板ばね８１のスロットされた端８４によって係合
されている。
板ばね８１は、例えば第２図および第４図に示すように
、通常、ファスナ保持器５２の近い方の側部からビン８
０に向かう方向に傾くように配置されている。この状態
で、ビン８０の遠い方の端がファスナ保持器５２から突
出しないように、ばね８１は引っ込み位置にビン８０を
保持する。整合ビン８０の遠い方の端は、円錐形である
。これにより、締結操作中ファスナ保持器の表面５５を
越えてビンを延ばすとき整合ビン８０が組織１２を突き
刺すことが可能となる。
組織１２がファスナ保持器５２とアンビル組立体５４間
の所定位置にあるとき、枢動するクランプアクチュエー
タ７０は、第１図に示すように、枢軸７２の周りに時計
廻りに枢動される。この結果、アクチュエータ７０の遠
い方の端のカム面７４は、ファスナ保持器５２と対合す
るアンビル組立体の表面５３間でＭｉｒａＩ２を徐々に
締めつける。
次いで、第３図に示すように、アクチュエータ７０が連
結構造３０の長さ方向の軸線に実質的に平行になるよう
に十分に時計廻りに回転されるとき、組ｖｊａ１２をア
ンビル組立体の表面５３と対向するファスナ保持器の表
面５５との間にしっかりと締めつける。第５図に示すよ
うに、ファスナ保持器５２が締めつけられた組織１２の
上に来たとき、整合ビン８０は組織を突き刺し、そして
、スペーサ部材５７は、ビン８０の周りの組織を変位さ
せてアンビル組立体の表面５３に接触する。細長い孔１
０５は、第５図において矢印６４で示すように、枢軸５
６が中央に近い方に向かって直線的に僅かに移動するこ
とを可能にする。その結果、ファスナ保持器の表面５５
とアンビル組立体の表面５３は、平行になり、そして、
ファスナ１２０の射出の準備を完了させる 以前の利用可能な装置では、ステープラを越えて延びる
組織の存在は、スペーサ部材５７がアンビル組立体の表
面５３に接触するのを妨げ、それにより、ファスナの正
確な射出を干渉する虞れがあった。しかしながら、本発
明においては、ファスナ５２が、アクチュエータ７０に
よって回転されるとき、スペーサ部材５７がアンビル組
立体の表面５３に接触するのを妨げるであろう組織は、
スペーサ部材の傾斜表面７５ａおよび５７ｂによって変
位される（第４Ａ図参照）これらの表面は、スペーサ部
材５７の最も遠い突出部５７ｃがアンビル組立体の表面
５３に接することができるように作用する。ファスナ保
持器５２とアンビル組立体５４の正確な整合は、ファス
ナ保持器の表面５５をアンビル組立体の表面５３に平行
に枢動させるとき、枢軸５６の反対側でファスナ保持器
５２の側部を貫通して延び、かつ、整合ビンの孔８２の
中に延びる整合ビンによって助けられる。
ファスナ保持器を反時計廻りに回転させるとき、整合ビ
ン８０は、スペーサ部材５７を越えて延び、アンビル組
立体の整合ビンの孔８２の正面に位置する組織に接触し
かつこれを押し通すく第５図参照）。ファスナ保持器５
２が、反時計廻りに枢動し続けるとき、スペーサ部材５
７は、ｍ織に達し、全整合ピン８０を取り囲んでいる組
織を変位させ始める。第６図に示すように、ファスナ保
持器５２を十分に枢動させるとき、スペーサ部材５７は
、Ｍｉ織を変位させて、アンビル組立体の表面５３に接
し、ファスナ保持器の表面５５とアンビル組立体の表面
５３との間の平行な整合を確保する。
同様に、第３図に示すように、アクチュエータ７０を十
分に時計廻りに回転させるとき、アクチュエータ７０に
支持されているドライバ７６もまた、連結構造３０の長
さ方向の軸線に実質的に平行になる。次いで、ドライバ
７６の遠い方の端は、ファスナ保持器５２の近い方の側
部の中に延び、ファスナ保持器のナイフファスナブツシ
ャ部材６３の近い方の表面に隣接している。アクチュエ
ータレバー１）０がハンドル３０から時計廻りにされた
状態にあるのを通常維持する安全ラッチ１（１０を、第
３図に示すように、今、実線位置まで反時計廻りに枢動
することによって解放する。
ファスナ駆動機構を駆動させるために、例えばハンドル
を握る手の指でレバー１）０をハンドルの方に押すこと
によって、レバー１）０を反時計廻りに枢動させること
ができる。
以上に述べたように、レバー１）０を反時計廻りに枢動
させるとき、連結構造３０の近い方の端の内部のレバー
１）０の端は、ドライバ７６の近い方の端７８に接触し
、遠い方向にドライバ７６を駆動させる。ドライバ７６
の遠い方の端は、ナイフファスナブツシャ部材６３の近
い方の表面に接触し、それによって、部材６３を遠い方
向に駆動させ、かつ、部材−６３にファスナ保持器５２
から、組織１２を通して、アンビル組立体に保持された
りテーナ１３０の中に、ファスナ１２０を駆動させる。
ファスナ１２０の遠い方の端から僅かに近い方に位置す
るのは、ナイフファスナブツシャ部材６３のナイフの表
面６１である（第９図参照）。ファスナ１２０が組織１
２を突き刺し始めた後、ナイフの表面６１は組織を切断
し始める（第１Ｏ図参照）。レバー１）０を反時計廻り
の方向に十分押すとき、ファスナ１２０はリテーナ１３
０の中に固定され、ナイフの表面６１は組織１２を完全
に切断する（第１）図参照）。
組織１２を完全に切断するのを確保するべくナイフの表
面６１が接して作用できる面を与えるために、ナイロン
のような、幾分可撓性で、弾力のある材料のストリップ
１５０が、ナイフの表面６１に平行なアノビル組立体の
表面５３に位置決めされｔいる。ストリップ１５０は、
アンビル組立体の表面５３の凹部１５４に嵌合されてい
る。
組織の接合が完了された今、なすべき残された操作は、
器具から、締結された組織を取り外すことだけである。
これは、レバー１）０を解放することによって達成され
る。板ばね６９がナイフファスナブツシャ部材６３を・
近い方向に付勢するので、解放レバー１）０は、ナイフ
ファスナブツシャ部材６３にファスナ保持器５２への引
っ込みを起こさせる。アクチュエータ７０は、反時計廻
りの方向に回転され、ファスナ保持器５２は、板ばね５
１の圧力に呼応してアンビル組立体から離れて時計廻り
に枢動する。同様に、ばね８１は、アンビル組立体５４
から離して整合ピン８０を付勢し、かくして、ピン８０
をファスナ保持器５２の中に引っ込めさせる。今や、ｍ
織を器具から容易に引き戻すことができる。遠い方の脚
４４からアンビル組立体５４を引くことによって、今や
カートリッジ５０は器具から取り外される。使用された
カートリッジは廃棄され、外科処置中もし更に組織の締
結が必要とされる場合には、もう１つ別のカートリッジ
を器具に再装填する。外科処置が完了したとき、器具を
、別の外科処置に使用するための準備のために、掃除し
、滅菌する。
ここに図示し、かつ、説明した実施例は、本発明の原理
の例証としてのみ掲げたものであり、種々の変更例を、
本発明の範囲と精神に逸れることなく当業者が作ること
ができることが理解されるであろう。特に、本発明は、
使い捨てカートリッジおよび永久、再使用可能な器具に
関連して説明されてきた。器具のできるだけ多くの部分
は、好ましくはプラスチック等の比較的安価な材料で作
る。高い応力を受ける器具の部分だけは、好ましくは金
属で作る。
同様に、ここに示す型式のカートリッジを、特にここに
説明した以外の多くの設計を有する器具に使用すること
ができる。ねじ式アクチュエータ（例えば、米国特許第
４．３５４．６２８号の第１８図から第２１図参照）ま
たはねじとレバーとを組み合わせたアクチュエータも当
業界には知られており、これを本発明のカートリッジと
ともに使用することができる。最後に、本発明の好まし
い実施態様は、組織を密封する手段としてファスナとリ
テーナを利用してきた。初めに説明したように、本発明
は、組織を密封する手段としてアンビルに対し金属の外
科用ステーブルを射出するようになったカー１ヘリツジ
を同等に適用できる。

Claims (10)

    【特許請求の範囲】
  1. （１）アンビル組立体と、 該アンビル組立体の１方の端に隣接してアンビル組立体
    に連結されており、かつ、複数の外科用ファスナを格納
    し、すべてのファスナをファスナ保持器から実質的に同
    時に駆動させるためのファスナ駆動装置を備えたファス
    ナ保持器と、 前記アンビル組立体をＵ字形部分の第１の脚に隣接させ
    、かつ、アンビル組立体とファスナ保持器との間の枢動
    連結部をＵ字形部分の基部に隣接させ、かつ、ファスナ
    保持器がＵ字形部分の第２の脚に隣接させるべく、アク
    チュエータのフレームにカートリッジを取り外し可能に
    取り付けるための、アンビル組立体と関連する装置とを
    有し、前記ファスナ保持器をアンビル組立体から離して
    枢動させるべく、カートリッジがフレームに対して動く
    ことができ、締結すべき組織は、Ｕ字形部分の開いた部
    分を介してアンビル組立体とファスナ保持器との間に位
    置決めされ、 ファスナ保持器をアンビル組立体に向けて枢動させると
    き、アンビル組立体とファスナ保持器との間に予め小さ
    な間隔を維持するための、枢動連結部の反対側でアンビ
    ル組立体とファスナ保持器との間にカートリッジの側部
    に隣接して配置されたスペーサ装置とを更に有する、人
    体に複数の外科用ファスナを同時に形成するための外科
    用ファスナのカートリッジにおいて、ファスナ保持器を
    アンビル組立体に向けて回転した後、ファスナ保持器と
    アンビル組立体とを互いに正確に位置決めされるべく、
    ファスナ保持器とアンビル組立体との端を越えて延在す
    る組織を変位させるための、前記スペーサ装置と関連す
    る装置とを設けたことを特徴とする外科用ファスナのカ
    ートリッジ。
  2. （２）組織を変位させる前記装置が、組織を変位させる
    ためのナイフ状先端部を形成する、互いに向かい合って
    傾斜した２つの表面を備えていることを特徴とする特許
    請求の範囲第（１）項に記載のカートリッジ。
  3. （３）組織を変位させる前記装置が、ファスナ保持器の
    表面から延びているＵ字形の表面を更に備えていること
    を特徴とする特許請求の範囲第（２）項に記載のカート
    リッジ。
  4. （４）ファスナ保持器とアンビル組立体との間で締結す
    べき組織を締めつけるべく、ファスナ保持器をアンビル
    組立体に向かって回転させるときに枢軸線に平行な方向
    にアンビル組立体とファスナ保持器とを整合させるため
    に、枢動連結部の反対側でカートリッジの側部に隣接し
    て配置された整合装置を有し、該整合装置はファスナ保
    持器およびアンビル組立体のそれぞれの端と関連する表
    面部分は、実質的に垂直であり、かつ、枢軸線に平行な
    方向には実質的に動かないようになっており、前記ファ
    スナ保持器と関連する表面部分と接触することを特徴と
    する特許請求の範囲第（１）項に記載のカートリッジ。
  5. （５）前記整合装置は、組織を締めつけるべくファスナ
    保持器とアンビル組立体とを互いに向かい合わせて枢動
    させるとき、ファスナ保持器とアンビル組立体の両方の
    中に延入する、枢軸線に実質的に垂直なピンを更に備え
    ていることを特徴とする特許請求の範囲第（４）項に記
    載のカートリッジ。
  6. （６）組織を締めつけるべくファスナ保持器とアンビル
    組立体とを互いに向かい合わせて枢動させるとき、ピン
    を挿入すべき穴を遮断している組織を突き刺すことがで
    きるように、ピンの先端部を円錐形にしたことを特徴と
    する特許請求の範囲第（５）項に記載のカートリッジ。
  7. （７）締結すべき人体組織をフレームの内側領域に入れ
    るために通常開いた周辺部分を有する堅固なフレームと
    、 組織を締めつけるべくファスナ保持器をアンビル組立体
    に向かって枢動させるためのクランプアクチュエータ装
    置とを有することを特徴とする特許請求の範囲第（１）
    項または第（４）項に記載の外科用ファスナのカートリ
    ッジを利用する外科用アクチュエータ組立体。
  8. （８）ファスナ保持器とアンビル組立体との間に締めつ
    けられた組織を切断するためのナイフ装置を更に備えて
    いることを特徴とする特許請求の範囲第（１）または第
    （４）項に記載のカートリッジ。
  9. （９）ナイフ装置の遠い方の端は、ファスナ保持器に保
    持されたファスナの遠い方の端から中央に近い方に配置
    されており、ファスナとナイフ装置とをファスナ保持器
    から同時に駆動させるためにファスナ駆動装置と関連し
    ているナイフ駆動装置を更に備えていることを特徴とす
    る特許請求の範囲第（８）項に記載のカートリッジ。
  10. （１０）Ｕ字形部分を備えた堅固なフレームと、アンビ
    ル組立体がフレームに対し静止するようにＵ字形部分の
    第１の脚に取り付けられたアンビル組立体と、 Ｕ字形部分の第２の脚に隣接して配置され、かつ、アン
    ビル組立体の１方の端に隣接して枢着されているファス
    ナ保持器を有し、枢着部はＵ字形部分の基部に隣接して
    おり、ファスナ保持器は、複数の外科用ファスナを格納
    し、かつ、ファスナをファスナ保持器から同時に駆動さ
    せるための装置を備え、 ファスナ保持器をアンビル組立体に向かって枢動させ、
    かつ、ファスナをファスナ保持器から駆動させるための
    装置を駆動させるための、フレームに取り付けられたア
    クチュエータ装置と、 通常開いている部分を跨ぐようにアンビル組立体を枢動
    させるとき、アンビル組立体の反対側でアンビル組立体
    の一部に接触するための、および、ファスナ保持器の残
    余の部分とアンビル組立体との間に間隔を維持するため
    の、ファスナ保持器の枢着部から離れた位置に設けたス
    ペーサ部材とを更に有する、人体組織に複数の外科用フ
    ァスナを同時に形成するための外科用ステープラにおい
    て、 前記スペーサ部材の形状は、組織がリテーナ保持器の開
    放した周辺部分およびファスナ保持器を越えて延びてい
    るとき、ファスナ保持器とアンビル組立体との間に適正
    な最小距離を得ることを確保するために、ファスナ保持
    器がアンビル組立体に接近するときに組織を変位させる
    ような形状になっていることを特徴とする外科用ステー
    プラ。
