Description

【発明の詳細な説明】 本発明は、組織を一緒に接合する手段およびこのような
手段を供給するための医療器具に関する。
組織の接合は、医学および外才・Ｆの処置においては非
常（重要でである。接合される組織には、筋膜、色／／
な器官、筋肉、皮膚等の組織がある。過去においては、
組織は色々な形式の縫合器を用いて接合され、色々な特
定の組織の接合のため特定の縫合器か開発されている。
更に最近、では、金属製の固定具（ｆａｓｔｅｎｅｒ）
が開発されている。例えは、皮膚の接合のためのステー
ブルかある。また、腸および他の器官の組織を含む組織
の接合のためステープルおよび適切な器具が開発されて
きた。最近では、組織のための非金属の固定具を開発す
る試み、およびこのような固定具を吸収可能なあるいは
吸収可能でないポリマー材料から作ろうとする試みが研
究されている。色々な形式のこのような固定具について
記載した特許には、米国特許第４．００６，７４７号お
よび同第４．０１３９，０８９号がある。
米国特許第３，５４５，４４４号においては、ワイヤ縫
合包込み（ｗｉｒｅ　５ｕｔｕｒｅ　ｗｒａｐｐｉｎｇ
）具が開示されており、同特許は〜れ臼体にコイルを形
成して管状部材をクランプするためこの管状部材の周囲
に設置することかできるワイヤ縫合について記載してい
る。ワイヤは所望の緊密さを得るため所望の程氾までそ
れ１１体の周囲に巻付けられている。関連する器具が米
国特許第３，７３５，７８２号において記載されている
。
ｆｉｉＴ述の。固定具は多くの異なる目的の用途に対し
満足できるものであるが、皮膚、筋膜、筋肉、器官等の
如何を問わず実質的にどんな組織にも使用することがで
きる単一・形態の固定具は開発されていない。全ての目
的の用途に使用することができるｉ＋ｉｌ＋Ｉ、ｌｌな
固定具を得るためには、壊死を生じることなく１Ｆ＋ｍ
を行なうものでなければならない、即ち組織を一緒に保
持して組織を破壊せずに出血を１トめなければならない
。固定具は広範囲の組織、理ち種々の厚さの組織ならび
に種々の強さの組織と共に使用可能でなければならない
。固定具は、設置が筒中でありまた整合性を以てＪ装置
可能でなければならない。各固定具は、閉鎖の際他の固
定具と同じ程度の張力をケえる同じ方法で設置されなけ
ればならない。更に、固定具は製造が簡単で経済的でな
ければならない。
種々の種類の組織に使用することができる新しい形式の
固定具が発見された。この固定具は構造が簡単であり、
製造が経済的である。この固定具は設置が容易であり、
ま失各固定具はその簡単な構造により一定した方法で設
置することができる。この新しい固定具は種々の種類の
組織ならびに組織形態を一緒に保持して矢面を防ぎ、望
ましからざる外傷即ち壊死を生じることなく止血を行な
う。
本発明の固定具は単一の構造のものであり、動物または
人間の組織の接合に使用することができる。この固定具
は、１対の自由端部を有する全体的に円筒形状を有して
いる。固定具の自由端部は、割りリングを形成するよう
に横に配置される。本発明のあるズ施態様においては、
本固定具は連続的方法で自由端部が相互に側方に離間さ
れ、−緒に接合された複数の円筒形状部からなる。本発
明の他の実施態様においては、固定具の自由端部は相互
に当接するも相互に側方に配置されている。
この新しい固定具は、種々の形式の器具により添、１ｆ
（ａｐｐｌｙ）することができる。本固定具は、一端部
において非常に緻密な先端部に成形されることが望ま叫
、）比較的直線状の材料片から始まる。
この添着用器具は、湾曲成形部材と駆動装置とを有する
。湾曲状態に成形された部材の自由端部は、前記器具お
よび駆動装置に対して取付けられた他端部即ち駆動端部
から偏倚している。直線状の固定具は駆動装置により湾
曲成形部材を経て自由端部に駆動され、従って固定具の
鋭利な先端部は接合されるべき組織に直接隣接している
。この部材は湾曲されているから、部材は尖った端部が
接合されるべき組織と係合するように導かれる。
この部材は組織を経て円形の形状に駆動され続ける。部
材がそれ自体に対して逆に湾曲されてリング即ちループ
を形成すると、係合された組織が一緒になる。この操作
は、組織と係合する複数の分れたループを形成するため
何回も行なうことができる。ある実施態様においては、
器具自体は接合されるべき組織と以上述へたように形成
された固’、、’ｇを接近するため使用できる尖った端
部を有することができる。
図面を参照して説明すると、第１図には本発明の一一個
の固定具（ｆａｓｔｅｎｅｒ）　１５を形成する単一の
ループか示されている。このループの本体１６は形状が
円形であり、第２図に更に明瞭に示されるように、固定
具の自由端部１７および１８は相尾に側方に隔てられて
いる。このループの自由端部の間隔は、側方の偏倚量（
ａ）および重なり（ｂ）に関して変更することができる
。ループの全体的形状もまた変更が可能であり、即ち円
形状、長円形状、楕円形状等に変更することができる。
固定具の一端部１８は、組織内の刺通しくｐｅｎｅｔ丁
ａｔｉｏｎ）を助けるため尖端となるように尖らされて
いる。
第３図においては、本発明の固定具の他の実施態様が示
されている。この固定具２０は、連続゛しており、相互
に側方に隔てられた自由端部２２及び２３を備えた複数
のループ２１を有している。固定具の自由端部の一方は
、組織の刺通しを助けるため鋭利な尖端部２４をなすよ
うに尖らせである。この固云′具を使用するには、固定
具の尖端部２４が接合されるべき組織の一方の側を通り
挿入される。この操作は何回も継続されて、固定具が組
織を通過する昨、これが長手方向線に沿って組織を接合
させる。
本発明の固定具を添着するための器具が第４図に示され
ている。本実施態様においては、器具３゜は湾曲端部３
３を有する細長い部材３２の形状の添着ヘッド３１を設
けた皮下注射器の形ｙＥと類似した形ｍコラ有している
。この器具は、本発明の複数の未成形の固定具３４を保
持するための領域を有する。
この未成形の固定具は、好ましくは尖った端部を備えた
直線状の形態を有している。皮下注射器本体は適切なプ
ッシャー３７を含み、このプ・ンノヤーが適すノな駆動
体（図示せず）を介して直線状の固！　ＪＪ、の１つを
細長いヘンドの下方に押す。これは、第４図に４（され
た添着用器具のへ、ラドの拡大図である第５−において
更に詳細に示されている。もし皮下注射型の器具が複数
の固定具を保持することになっている場合は、この器具
は直線形Ｆ；ｆ、’Ｂである１つの固定、１ｔ４０を保
持領域から選択し、皮ド４ト射器のプランジャにより押
された駆動体４１が直線状固定具を中空管ヘッド４２が
ら下方に湾曲した開口先端部４３まで押し進める。先端
部４３の端部は、中空管ヘット４２の長手方向軸心から
側方に僅かに偏倚されている。この偏倚が、固定具が定
置される時その尖端部を偏倚即ち後端部から側方に離間
させる。第６図に更に明瞭に示されているように、湾曲
した先端部４３は、組織を接近させて、接合されるべき
組織４４に隣接して首かれる。
プッシャーは固定具を湾曲端部から組織を通るよう強制
し続けて、この部材が組織を貫通する時これを湾曲させ
る。
第７図においては器具の一部の断面図が示され、適切な
成形用ヘッド内に順々に配置される複数のＩＭ固定具提
供するための１つの手法を示している。本実施態様にお
いては、皮下注射器型器具５０が使用されており、複数
の未成形の固定具５１がばねを装填したマガジン５３を
介して中空管の添着−ット５２に提供される。この駆動
体５４は、固定具を湾曲した開口ヘッド５５に向けて中
空管の下方に押す。駆動体は、前方の固定具を定置し、
列をなす他の直線状固定具を受入れるため引かれる。
固定具を設置すると同時に、一旦固定具がある最小限度
の曲率まで湾曲されて固定具を押し続けると固定具はこ
の曲率に−・致し続け、従って使用渚は適切な器具のヘ
ッドで約ｌＯ乃至１００°固定其を湾曲するだけでよく
、固定具は所望の曲率に従い続けることが判るであろう
。曲率の精度は器具の整形ヘッドにおける曲率半径に依
存し尖端部の角度だけ側方に偏倚する。
第８図は、口径内ステープル装置６０である特定の形式
の器具の断面図であり、前記の新しい固定具がこのよう
な装置と共に使用できる方法をパしている。直線状の未
成形固定具６１は口径内ステープル装置の一端部６２に
保持され、この口径内ステーブル装置の他端部６３は湾
曲したアンビル６４を有する。接合されるべき組織は、
通常の如く２つの器具端部６２と６３の間に設置される
。プッシャー６５が、直線状固定具を接合されるべき組
織を通して円形状のアンビルに向けて押圧し、直線状の
同定共を本発明に従ってループを形成して組織を接合す
る。
明らかなように、このループの大きさは、接合したい組
織によって広い範囲にわたって変更することができる。
更に、固定具の直径もまた、接合したい組織によって広
い範囲にわたり変更することができる。
本発明の固定具は、もしプラスチック材料が円形状に形
成できるように、またこの円形形状を維持できるよう充
分に可撓性に富むならばある場合にはプラスチック材料
もまた使用することができるが、ステンレス鋼、タンタ
ル、チタン等の如き金属材料から作られることが望まし
い。生物学的に吸収可能な材料もまた本発明の固定具の
製造に使用できると考えられる。
本発明の外科用固定具は殺菌されなければならない。本
発明の金属製固定具は、熱、放射線、エチレンオキシド
等によって殺菌することができ本発明の固定具の設置に
用いられる器具は、１つの固定具を設置するか、あるい
は多数の固定具を設置することができる。この器具は再
使用可能であり、即ち固定具のカートす・ンジを収納可
能でありかつ器具自体が再使用可能であるか、あるいは
種々の形式の外科用ステープルの場合のように器具自体
が完全に使い捨て可能である。
Ｕ−Ｌ本発明および本発明が実施可能な方法について記
述したか１本発明の精神および範囲から逸脱するこ−と
なく種々の変更が可能であると理解すべきである。

Claims (1)

  1. 【特許請求の範囲】 １、動物または人間の組織を接合するための外科用の殺
    菌された弔−・の固定具において、１対の自由端部を有
    するループを有し、該ループの自由端部が相ｌＩニに側
    方に配置されていることを特徴とする単一の固定具。 ２、前記自由端部が相互に当接する特許請求の範囲第１
    項記載の中−の固定具。 ３、前記ループが全体的に円形の形状を有する特許πｉ
    ′ｉ求の範囲第１項記載の弔−の固定具。 ４、前記固定−具は相〃に結合された複数のループを有
    する特許請求の範囲第１項記載の単一の固定具。 ５、少なくとも１つの自由端部が尖った先端部を有する
    特許請求の範囲第１項または第４項に記載の単一の固定
    具。 ６、前記固定具がステンレス鋼から作られる特許請求の
    範囲第１項または第４項に記載の単一の固定具。 ７、接合すべき組織接近して、接近した組織に固定↓↓
    の一端部を挿入し、同時に前記固定具が組織を通り挿入
    されている間に略々円形の形状に前記固定具を成形し、
    更にこれと同時に前記固定具が成形されている間に、該
    固定具の前記挿入端部を４１ｊ記固定具の反対側端部か
    ら側方に配置させることを＃！？徴とする方法。 ８、前記固定具が複数の全体的に円形の形状に成形され
    る特許請求の範囲第７項記載の方法。 ９、組織を一緒に接合するため、１対の自由端部を相互
    に横に配置したループを具備する単一の固′ｒ具を供給
    する器具において、湾曲した偏倚端部を有する成形ヘッ
    ドと、直線状の固定具素材を前記成形用ヘッドに供給す
    る供給手段と、該供給手段から前記成形ヘッドを経て前
    記の湾曲した偏倚端部に前記素材を駆動して前記の直線
    状の固定具をその端部が相互に横に配置された全体的に
    円形の形状に成形する駆動手段とを具備することを特徴
    とする器具。 １０、前記湾曲偏倚端部が尖った先端部を有する特許請
    求の範囲第９項記載の器具。
