脊椎固定装置発明の背景
1.発明の分野
本発明は、ロッドおよび独自の中空のねじを使用して、隣接する椎骨を互いに
固定するための装置に関する。
2.関連技術の説明
骨材料もしくはロッドおよびプレートによる脊柱の固定(または融合)は、多
様な症状を治療するために長く実施されてきた一般的な外科的手法である。既存
の手法の多くは、体の部分、たとえば大動脈、大静脈、交感神経、腸および尿管
と接し、それらを損傷しかねない、外方へ突出する部品が関与している。また、
多くの構造は、緩みを生じ、望ましくない問題を引き起こすおそれのある部品を
含む。ダン(Dunn)装置が市場に出されていたが、結局は、その装置によって派生
する大動脈の遅延破裂が、大動脈に接し、その表面を侵食し、場合によっては致
命的な出血につながるほど大きいという問題のため、米国食品医薬品局によって
回収された。
1992年10月5日にアレン(Allen)に発行された米国特許第5,152,
303号は、椎骨にねじ込まれるカニューレ式ねじと、そのねじに取り付けられ
るロッドとを含む前外側脊椎固定システムに関する。その処置は、カニューレ式
ねじを、椎骨本体部にあけた案内孔にねじ込み、ロッドの下端および上端をカニ
ューレ式ねじによって椎骨本体に留めることを含む(3段目62〜64行、4段
目5〜8行)。
1977年11月22日にジュマシェフ(Jumashev)らに発行された米国特許第
4,059,115号は、椎骨軟骨症における前開窓脊椎固定術のための外科用
器具に関する。この器具は、切れ刃を有する中空の円筒形カッタと、取っ手とを
含む。わずかに圧を加えながら取っ手を回すことにより、隣接する椎骨の本体に
カッタを差し込む(要約、6段目56〜58行)。
1991年5月14日にミケルソン(Michelson)に発行された米国特許第5,
015,247号は、脊椎の内部的安定化を実施する方法に関する。この方法は
、
ドリルスリーブを2個の椎骨の中に取り付け、ドリルスリーブの中に設置したド
リルで椎骨に孔をあけることを含む。バグビイ(Bagby)第4,501,269号
が述べられている(6段目27〜30行、7段目68行、9段目22〜25、3
9行)。
現在の装置には、骨粗しょう症の骨に遭遇した場合に重大な欠点がある。その
ような患者の軟らかなカルシウム不足の骨は、ねじに対し、引抜き強度に乏しい
。骨ねじは、骨粗しょう症の骨では保持力をほとんど発揮せず、緩みやすく、現
在の装置の保持力および固定能力をひどく制限することが知られている。
いくつかの装置は、横断方向にあけた孔を有する中空のねじを含み、おそらく
は保持力を高め、骨をその中に成長させる設計を有している。これらの装置はす
べて、広い表面積にわたる固定を行うことができない比較的小さなねじである。
この章に記載の技術は、本明細書で引用する特許、出版物または他の情報が、
別段その旨を指定しない限り、本発明に対する「先行技術」であるということを

本発明は、2個以上の椎骨を固定するための方法および装置を提供する。処置
は、科学的に精密で簡潔であり、従来の装置に付随する問題の多くを解消する。
接合すべき各椎骨は、ホールソーなどによって部分環状のくりぬきを形成し、
好ましくは骨のコアプラグを適所に残すことによって調製する。そのようにして
形成した環状リングくぼみに中空のねじをねじ込む。椎骨の中、各ねじの間に、
各ねじの上に配されるロッドを収容するための溝を切る。各ねじにはまる固定キ
ャップがロッドをねじに固着し、それにより、望みどおりに脊椎を固定する。
本方法および装置は多くの利点を提供する。中空のねじは抜群に強く、従来の
中実のねじよりも広い保持表面積を有する。ロッドは、ねじの中で、広く離間し
た2個のスロットの間に保持される。ロッドはまた、固定キャップ上のディンプ
ルにより、第三の点によってしっかりと保持される。ロッドは、従来のシステム
よりもはるかに大きな距離にわたって、少なくとも3個の固定点によってねじに
固着される。これが、機械的安定性の点で従来技術よりも有意に優れた連結を提
供する。
中空ねじの側壁の孔が、骨を内方成長させて接続をさらに強化することに備え
る。骨プラグが除去されておらず、ねじの壁が非常に薄いため、骨は、ねじの中
を速やかに成長することができ、それにより、ねじを椎骨に確実に融合させ、椎
骨に対してよりよい固定具を提供する。さらには、骨がねじの孔の中を成長する
とき、骨は時間とともにいっそう強くなる。従来技術の装置は、材齢とともにゆ
っくりと確実さを失うねじを使用しており、ねじと椎骨との間では微細動が起こ
ることが避けられない。
ロッドの断面寸法形状および直径を変えることにより、程度の異なる剛性を付
与することができる。また、ロッドの幾何学的断面構造を変えることにより、適
切な運動面に剛性を選択的に付与することができる。たとえば、曲げ拡大安定性
の増大を望むならば、ロッドを曲げ拡大面に向け、横方向の曲げよりも曲げ拡大
により大きな剛性を提供するように伸ばすことができる。このような特徴により
、外科医は、遭遇する疾病に適合するのに必要な剛性面を定めることができる。
切った溝に入れられたロッドは、脊椎の中心から離れたところで負荷が担持さ
れる従来技術の装置の片持ち効果を回避する。したがって、ロッドは、椎骨中で
むしろ髄内ロッドのように作用する。これは、ロッドが回転軸の中心により近い
ところにあるため、従来技術のシステムにおける片持ち効果を備えていないとい
うことにおいて、はるかに好ましい。これはまた、生命維持に必要な体の部分に
当接するおそれのある突起を示さない。
処置は非常に簡単であり、各椎骨に1個のホールソーによるくりぬきをあけ、
椎骨間に溝を形成し、中空のねじを取り付け、ロッドを配置し、固定キャップで

図1および2を特に参照すると、前脊椎固定システム10が、隣接する椎骨を
接合することが見てとれる。このシステムは、細長いロッド12と、少なくとも
2個の中空の円筒形ねじ14とを含む。各ねじ14は、骨と係合する側のおねじ
16と、キャップと係合する側のめねじ28と、上縁18と、下縁20とを含む
。めねじ28は、固定キャップと同じだけの深さであればよい。ねじを挿入する
際に、残る骨コアにトルクを加えないよう、滑らかな内壁が好ましい。図2に示
すように、上縁18は、対向する2個の固定スロット22、24によって割られ
ている。これらのスロットは、図示するようにロッド12をスロット22、24
に通すことができるような大きさである。好ましくは、ねじ14は、骨を内方成
長させるための複数の開口36をその側壁に備え、骨を成長させて通すことがで
き
る。
ロッド12は固定キャップ26によってねじに対して保持される。図示するよ
うに、固定キャップ26は、ねじ30を円形の外周に有する円盤形であってもよ
い。キャップ26の頂部32は、前記キャップ26を中空のねじ14に挿入する
ための器具(図示せず)を連結可能な離間した1対の孔34を有することができ
る。キャップ26は、そのキャップのいかなる部分もねじ14から突出しないよ
うに前記ねじ14にねじ込むことができる。キャップはまた、多孔質であっても
よいし、骨の内方成長を許し、内部への血液供給を増すための孔を有していても
よい。
また、図6に示すように、キャップは、骨ねじ14の外面のねじ74と係合す
るオーバキャップ72として設計してもよい。図示するように、オーバキャップ
72は、ロッド12に当接するキャップ突起42を含む。オーバキャップの場合
、図示するようにそのオーバキャップを受け入れるために、骨をいくらか除去し
てもよい。
ロッド12は、チタンのような生物学的適合性で可鍛性の金属で作られている
ことが好ましい。チタンのロッドには、天然の骨に類似した弾性率を有する利点
がある。いずれの場合にも、外科医がロッドを曲げて、患者に望まれる正しい形
状を達成する。図面に示すように、ロッド12は、丸くても細長くてもよい複数
の離間したディンプル40を有してもよい。ディンプル40は、はめ合い突起4
2と接する。丸いディンプル40の場合、突起42の係合が、ねじ14に対する
ロッドの滑りを防ぐように働く。細長いディンプル40は、しばしば必要とされ
るある程度の滑りを許す。
図5a〜eは、ロッド12の断面を円形以外のほぼいかなる形状にしてもよい
ことを示す。円形の断面のロッドでもよいが、非円形のロッドのほうがよりよい
ねじれ抑制を提供する。ロッドの大きさは、個々の患者の大きさに依存して選択
することができる。先に述べたように、ロッドの断面形状を変えて、特定の患者
に適した運動軸において安定性を提供することができる。
装置の取り付けは簡単である。外科医は、椎骨46を前方に露出し、椎骨の中
に、図2に示すような円筒形の孔50をあける。ホールソーを使用するとコア5
2を適所に残せるため、好ましくはホールソーを使用して孔50を形成する。説
明しやすくするため、図2には骨コア52が示されていないが、骨コア52は、
図3および4には示されている。骨コアが適所に残されていなければ、開口に骨
または骨代用物を詰めることもできる。使用するドリルがセルフタッピング型で
あるならば開口50にねじを切ってもよいし、穿孔後に使用するさらなる器具に
よってタッピングしてもよいことがわかる。
好ましくは、インプラントねじ14は、ホールソーによるくりぬきの外径より
もわずかに大きな外径を有し、それにより、椎骨本体に対して高摩擦固着連結を
提供する。これはまた、骨コアの外径よりもわずかに大きな内径を有するねじを
提供して、それにより、ねじを配置する際にコアにトルクが加わる危険性を減ら
す。挿入の過程でコアにトルクを加えることは、コアへの血液供給をその後面で
破壊するおそれがある。これは、ねじの中の孔を介する骨固定能力の組込みを遅
らせるおそれがあるため、望ましくない。
また、装置の骨ねじ14を使用して、それ自体で開口を切ることも可能である
。そのような場合、ねじ14は、完全に挿入した後、そこに残しておく。
次に、隣接する各椎骨ごとに、別の骨開口50を、たとえばのみまたはルータ
器具でくりぬいて、ロッド12の直径の少なくとも半分を保持する深さの溝54
を形成する。本発明の装置および方法を使用すると、椎骨は溝なしで接続するこ
ともできるが、ロッドの少なくとも大部分が溝54に収まらないならば、その利
点の多くは失われる。
そして、骨ねじ14の雌ねじ及び雄ねじ28、16が椎骨と係合する状態で、
骨ねじ14を骨開口50にねじ込む。ねじは、ロッド固定スロット22、24が
骨溝54とまっすぐ並ぶように配置する。図8に示す器具58を使用すると、骨
ねじ14を骨開口50にねじ込むことができる 図示するように、器具58は、
取っ手62を一端に有し、ねじ係合ヘッド64を他端に有する軸60を含む。ね
じ係合ヘッド64は、スロット22、24と係合する1対のタブ66、68を含
む。ヘッド64は、ねじ14の内部に密に適合する。器具58のどの部分も、骨
ねじ14の外周よりも外に突出しない。
次に、必要な長さのロッド12を必要な形状に曲げ、ロッド固定スロット22
、
24を介して各ねじに挿入し、また、形成した骨溝54にも挿入する。必要に応
じ、ロッド12を取り外して、取り付けるシステムによって決定される脊椎の湾
曲を調節してもよい。ひとたび矯正の程度を達成したならば、固定キャップ26
を各ねじ14に固着し、それにより、ロッド12を適所に捕らえることにより、
ロッドを適所に保持する。
骨ねじ14は、約3 mmの安全区域を残しながら椎骨の中にかなり深く配置す
ることが好ましい。椎骨の大きさに依存して、ねじ直径は1.5〜3.5 cmの
範囲であることができる。好ましくは、ねじ14の直径は、より硬い外側の椎骨
に切り込むのに十分な大きさである。ねじ14は、比較的薄い円筒形の壁を有す
るが、それでもなお、大きな強度および保持力を提供することができる。
取り付けられた本発明のシステムは、多くの重要な利点を脊椎矯正に提供する
。円筒形のねじは従来の中実ねじよりもはるかに広い表面積を有するため、保持
力がはるかに大きい。取り付けられたシステムは、椎骨の範囲の中に完全に含ま
れる。隣接する骨構造に接するおそれのある、外に突出するものはない。ロッド
12は椎骨の中心により近く、従来技術の装置におけるような望ましくない片持
ち効果が大幅に減ることを意味する。
本発明のシステムは、多くの椎骨を安定化するのに使用することもできるし、
2個の椎骨だけを安定化するのに使用することもできる。このシステムは、腫瘍
、骨折、変性性疾患、奇形または感染による矯正を付与するのに使用することが
できる。大部分の場合に使用される非円形のロッドは、縦方向の回転の抑制を提
供する。体の正常な治癒過程が、ねじ、ロッドおよびキャップの回りの骨の成長
を生じさせて、システムを椎骨に対してより確実に固定する。ねじ14は、保持
力を高めることができる骨の内方成長を許すように、円筒の長手方向に延びる複
数の孔を含むことができる。
図1、7及び9は、本発明を2個以上の隣接する椎骨の間で使用することもで
きるし、切除された椎骨本体の代わりのスペーサとして機能するコーペクトミー
(corpectomy)ブロック80とともに使用することもできることを示す。コーペ
クトミーブロック80は、たとえば腫瘍や折れた骨を除くために椎骨本体の大き
な部分を切除したときや、かなりの骨を損失した場合に使用される。椎骨の大部
分を前方から除去すると、残る椎骨の間に隙間ができる。通常の従来技術の解決
方法は、ロッドを備えた多数の充填プレートまたはプレートおよび従来のねじで
固定される大きなセラミックブロックを設ける方法である。米国特許第5,19
2,327号は、ロッド12を通すことができるスロットを設けるだけで設計す
ることができる適当なコーペクトミーブロックを開示している。
図7および9には、実質的に中空であり、生体適合性材料、たとえばチタンま
たはセ ミックから形成されるコーペクトミーブロック80が示されている。ブ
ロックは、多孔質であってもよいし、骨の内方成長を許すために少なくとも端部
が粗仕上げされていてもよい。充填口82をブロック80に形成して、骨移植片
の追加を可能にしてもよい。ブロック80は、ロッド12を通すことができる長
手方向のスロット84を含む。使用するときには、切除ののち、適当な大きさの
ブロック80を残りの椎骨の間に取り付け、骨移植片で充填する。ロッド12を
スロット84の中に配置し、端部キャップ26または27により、各骨ねじ14
で締め付ける。これがコーペクトミーブロック80をしっかりと適所に保持して
、図1に示すような隣接する椎骨に対する骨の融合を可能にする。
ブロックは、少なくとも多孔質の端板86、88を有することにより、血管の
内方成長を可能にすべきである。ブロック80は、多孔質のチタンであってもよ
いし、粗仕上げした端板を有するセラミックであってもよい。
図1は、1個以上の椎骨を切除し、コーペクトミーブロックで置き換える場合
でも、本発明が機能することを示す。本発明のすべての態様において、中空のね
じ14がより大きな保持力を提供し、従来技術のペディクルねじシステム、たと
えば1994年6月28日に発行された米国特許第5,324,290号に見ら
れる片持ち式構造を排除した髄内ロッドの使用を許容するものである。本発明は
、椎骨の中心、したがって、力および動きの軸の中心により近いところから力を
向ける。
場合によっては、脊柱は、前後、側方またはその両方向に並びの矯正を要する
。図10〜17は、本発明の脊椎固定装置10を使用する際の並び矯正手段を示
す。
図10〜13には、除去した椎間板の代わりに椎骨間に挿入することができる
くさびが示されている。図10〜12のくさび90は、中実のセラミックブロッ
クであってもよいし、チタンのくさびであってもよいし、椎間板の代用にするこ
とができる他のいかなる生体移植可能な材料であってもよい。くさび90の前側
92が後縁54よりも高くなっている。ロッド12を通すためのスロット96が
形成されている。
図12のくさび100は、同様に形成された本体を含み、スロット96を含む
が、向きの定まったくさびではない。むしろ、くさび100の表面102は、丸
みのある形または他の形の非平面である。このようにして、椎骨に対する面10
2を有するくさび100は、奇形を回転方向または角度方向に矯正することがで
きる。椎間板に代えてくさび100を挿入すると、隣接する両方の椎骨が丸い面
102に当たるクラムシェル形の外見を作ることができる。
図14〜17は、本発明のくさびを使用して、脊椎の並びの欠陥を矯正する態
様を示す。図14及び15では、椎骨104、106からなる脊柱の並びが狂い
、脊椎が前に傾いている。図15では、各椎骨は、ねじ14、ロッド12及びく
さび90を含み、前側がより高いなるように並びが矯正されている。この手法は
、ロッド12を曲げずにそれと同様な結果を得るために使用することもできるし
、曲げたロッドと併せて使用することもできる。
図16及び17は、矯正くさび90を、くさび90の最も厚い部分が横を向く
ように側方から滑り込ませ、それにより湾曲を矯正する典型的な脊柱側弯を示す
。すべての場合において、くさびを適所に挿入し、椎骨をくさびと接触させ、固
定キャップ26をねじ込んでロッド12と係合させ、構造全体を望みどおりに維
持している。
本発明は、患者の要求に応じて、前方から、前側方から、及び側方から使用す
ることができる。図面は、一つの可能な位置として、前方からの骨ねじの使用を
示す。
本発明は多数の異なる形態に具現化することができるが、本発明の具体的な好
ましい実施態様を図面に示し、本明細書で詳細に説明している。本開示は、本発
明の原理を具現化したものであり、本発明を、例示された特定の実施態様に限定
しようとするものではない。
これをもって、本発明の好ましい実施態様および代替態様の説明を完了する。
当業者であれば、添付の請求の範囲によって包含される、本明細書に記載する特
定の実施態様に対する他の同等な態様を認識することができよう。

請求の範囲
1.脊椎異常を矯正するため、外科的に移植可能なシステムであって、
(a)矯正を要する脊椎部分の椎骨内に延びるように適合されたロッドと、
(b)少なくとも2個の実質的に中空の円筒形部材であって、それぞれの円筒
形部材が骨と係合する一連のねじをその外面に有し、それぞれの円筒形部材が上
縁および下縁を含み、前記上縁のそれぞれが前記ロッドを前記縁に対して横断す
るようにして受け入れ、かつ、前記縁の面よりも下方に位置する一対の対向する
ロッド固定スロットを含み、前記ロッドは各円筒形部材の各ロッド固定スロット
に受け入れられることと、
(c)前記上縁にて前記円筒部材それぞれに固着可能であり、それにより、前
記ロッドを前記円筒部材それぞれに固定する固定キャップと
を含む外科的に移植可能なシステム。
2.前記円筒形部材がねじを含み、かつ、前記固定キャップが、前記円筒形部
材のねじと係合して前記キャップを前記円筒形部材に固定するためのねじを含む
請求項1に記載の外科的に移植可能なシステム。
3.前記ロッドが、その断面が円形ではないように構成及び配設される請求項
1に記載の外科的に移植可能なシステム。
4.前記ロッドが複数の離間したディンプルを含み、前記固定キャップが、前
記ディンプルの1個とインタロックして、前記ロッドを前記円筒形部材およびキ
ャップにより確実に固定することができる突起を含む請求項3に記載の外科的に
移植可能なシステム。
5.前記ロッドが複数の離間した突起を含み、前記固定キャップが、前記突起
の1個とインタロックして、前記ロッドを前記円筒形部材およびキャップにより
確実に固定することができるディンプルを含む請求項3に記載の外科的に移植可
能なシステム。
6.前記ロッドがほぼ楕円形の断面を有する請求項1に記載の外科的に移植可
能なシステム。
7.脊椎を固定するための脊椎固定システムにおいて、
(a)椎骨の中に前方、前側方または側方からくりぬかれたくぼんだリング状
部の中に配置し得るように構成された少なくとも2個の中空のねじであって、そ
れぞれのねじが外面及び内面並びに上縁及び下縁を有し、さらに、骨と係合する
ねじを外面に含み、前記上縁から延びる対向するロッド固定スロットを含むこと
と、
(b)前記各中空ねじの間の距離にわたって延び、前記中空ねじのそれぞれを
連結するのに十分な長さを有するロッドと、そのロッドは前記ねじのロッド固定
スロットに挿入されることと、
(c)前記上縁で前記ねじそれぞれに固着され、それにより、前記ロッドをロ
ッド固定スロット内にて前記ねじそれぞれに固定する固定キャップと、
を含む脊椎固定システム。
8.脊椎固定システムを取り付ける方法において、
(a)接合すべき各椎骨に対し、各椎骨の前面、前側面または側面に環状のく
りぬきを形成するステップと、
(b)環状のくりぬきを有する前記椎骨のそれぞれの間に溝を形成するステッ
プと、
(c)各環状のくりぬきに中空のねじをねじ込むステップと、各中空ねじは上
縁を含み、前記上縁から前記ねじの中に延びる対向するロッド固定スロットを有
することと、
(d)前記ねじのロッド固定スロットおよび前記椎骨の溝に1本のロッドを配
置し、それにより、前記ロッドが前記椎骨表面よりも少なくとも実質的に下にあ
る状態で、各ねじを前記ロッドと連結するステップと、
(e)固定キャップを前記ねじの前記上縁に配置することによって前記ロッド
を前記ねじそれぞれに固定し、それにより、前記ロッドをねじに捕らえるステッ
プと、
を含む方法。
9.隣接する少なくとも一対の椎骨の間で、かつ前記ロッドの回りに、脊椎並
びの矯正くさびを配置して、脊椎の並びを矯正するステップをさらに含む請求項
8に記載の方法。
10.椎骨の大部分を切除し、切除ののち脊椎を安定化する方法において、
(a)疾病、腫瘍または骨折により、必要に応じて除去されるべき脊椎の骨の
一部を外科的に切除し、それにより、隣接する椎骨の間に隙間を残すステップと
、
(b)接合すべき前記隣接する椎骨に対し、各椎骨の前面、前側面または側面
に環状のくりぬきを形成するステップと、
(c)環状のくりぬきを有する前記椎骨のそれぞれの間に溝を形成するステッ
プと、
(d)各環状のくりぬきに中空ねじをねじ込むステップと、前記各中空ねじは
上縁を含み、前記上縁がら前 ねじの中に延びる対向するロッド固定スロットを
有することと、
(e)前記隣接する椎骨の間の隙間にコーペクトミーブロックを配置するステ
ップと、前記コーペクトミーブロックは端板を有し、この端板が、前記隣接する
椎骨の間に適合し、前記端板の中に骨を内方成長させて、前記コーペクトミーブ
ロックに対する前記椎骨の融合を容易にするように構成及び配設され、前記コー
ペクトミーブロックはさらに溝を含むことと、
(f)前記ねじの各ロッド固定スロットならびに前記椎骨の溝および前記コー
ペクトミーブロックの溝に1本のロッドを配置し、それにより、前記ロッドが前
記椎骨表面よりも少なくとも実質的に下にある状態で、各ねじを前記ロッドと接
続するステップと、
(g)固定キャップを前記ねじの前記上縁に配置することによって前記ロッド
を前記ねじそれぞれに固定し、それにより、前記ロッドをねじに捕らえるステッ
プと
を含む方法。
Claims (1)
- 【特許請求の範囲】 1.脊椎異常を矯正するため、外科的に移植可能なシステムであって、 (a)矯正を要する脊椎部分の椎骨内に延びるように適合されたロッドと、 (b)少なくとも2個の実質的に中空の円筒形部材であって、それぞれの円筒 形部材が骨と係合する一連のねじをその外面に有し、それぞれの円筒形部材が上 縁および下縁を含み、前記上縁のそれぞれが前記ロッドを前記縁に対して横断す るようにして受け入れ、かつ、前記縁の面よりも下方に位置する一対の対向する ロッド固定スロットを含むことと、 (c)前記上縁にて前記円筒部材それぞれに固着可能であり、それにより、前 記ロッドを前記円筒部材それぞれに固定する固定キャップと を含む外科的に移植可能なシステム。 2.前記円筒形部材が、前記上縁から下向きに延びるねじを含み、前記固定キ ャップが、前記円筒形部材のねじと係合して前記キャップを前記円筒形部材に固 定するためのねじを含む請求項1に記載の外科的に移植可能なシステム。 3.前記ロッドが、その断面が円形ではないように構成及び配設される請求項 1に記載の外科的に移植可能なシステム。 4.前記ロッドが複数の離間したディンプルを含み、前記固定キャップが、前 記ディンプルの1個とインタロックして、前記ロッドを前記円筒形部材およびキ ャップにより確実に固定することができる突起を含む請求項3に記載の外科的に 移植可能なシステム。 5.前記ロッドが複数の離間した突起を含み、前記固定キャップが、前記突起 の1個とインタロックして、前記ロッドを前記円筒形部材およびキャップにより 確実に固定することができるディンプルを含む請求項3に記載の外科的に移植可 能なシステム。 6.前記ロッドがほぼ楕円形の断面を有する請求項1に記載の外科的に移植可 能なシステム。 7.脊椎を固定するための脊椎固定システムにおいて、 (a)椎骨の中に前方、前側方または側方からくりぬかれたくぼんだリング状 部の中に配置し得るように構成された少なくとも2個の中空のねじであって、そ れぞれのねじが外面及び内面並びに上縁及び下縁を有し、さらに、骨と係合する ねじを外面に含み、前記上縁から延びる対向するロッド固定スロットを含むこと と、 (b)前記各中空ねじの間の距離にわたって延び、前記中空ねじのそれぞれを 連結するのに十分な長さを有するロッドと、そのロッドは前記ねじのロッド固定 スロットに挿入可能であることと、 (c)前記上縁で前記ねじそれぞれに固着可能であり、それにより、前記ロッ ドをロッド固定スロット内にて前記ねじそれぞれに固定する固定キャップと、 を含む脊椎固定システム。 8.脊椎固定システムを取り付ける方法において、 (a)接合すべき各椎骨に対し、各椎骨の前面、前側面または側面に環状のく りぬきを形成するステップと、 (b)環状のくりぬきを有する前記椎骨のそれぞれの間に溝を形成するステッ プと、 (c)各環状のくりぬきに中空のねじをねじ込むステップと、各中空ねじは上 縁を含み、前記上縁から前記ねじの中に延びる対向するロッド固定スロットを有 することと、 (d)前記ねじのロッド固定スロットおよび前記椎骨の溝に1本のロッドを配 置し、それにより、前記ロッドが前記椎骨表面よりも少なくとも実質的に下にあ る状態で、各ねじを前記ロッドと連結するステップと、 (e)固定キャップを前記ねじの前記上縁に配置することによって前記ロッド を前記ねじそれぞれに固定し、それにより、前記ロッドをねじに捕らえるステッ プと、 を含む方法。 9.隣接する少なくとも一対の椎骨の間で、かつ前記ロッドの回りに、脊椎並 びの矯正くさびを配置して、脊椎の並びを矯正するステップをさらに含む請求項 8に記載の方法。 10.椎骨の大部分を切除し、切除ののち脊椎を安定化する方法において、 (a)疾病、腫瘍または骨折により、必要に応じて除去されるべき脊椎の骨の 一部を外科的に切除し、それにより、隣接する椎骨の間に隙間を残すステップと 、 (b)接合すべき前記隣接する椎骨に対し、各椎骨の前面、前側面または側面 に環状のくりぬきを形成するステップと、 (c)環状のくりぬきを有する前記椎骨のそれぞれの間に溝を形成するステッ プと、 (d)各環状のくりぬきに中空ねじをねじ込むステップと、前記各中空ねじは 上縁を含み、前記上縁から前記ねじの中に延びる対向するロッド固定スロットを 有することと、 (e)前記隣接する椎骨の間の隙間にコーペクトミーブロックを配置するステ ップと、前記コーペクトミーブロックは端板を有し、この端板が、前記隣接する 椎骨の間に適合し、前記端板の中に骨を内方成長させて、前記コーベクトミーブ ロックに対する前記椎骨の融合を容易にするように構成及び配設され、前記コー ペクトミーブロックはさらに溝を含むことと、 (f)前記ねじの各ロッド固定スロットならびに前記椎骨の溝および前記コー ペクトミーブロックの溝に1本のロッドを配置し、それにより、前記ロッドが前 記椎骨表面よりも少なくとも実質的に下にある状態で、各ねじを前記ロッドと接 続するステップと、 (g)固定キャップを前記ねじの前記上縁に配置することによって前記ロッド を前記ねじそれぞれに固定し、それにより、前記ロッドをねじに捕らえるステッ プと を含む方法。
