(57)【要約】 【課題】 ステムに一体的に連結されたネックベアリン
グを有するにも拘わらず、長手方向に延びる溝内の空間
へアクセスが可能な股関節用人工補綴物を提供する。 【解決手段】 大腿骨に埋め込まれるステム１を備えた
股関節用人工補綴物である。ステム１は、その上端部に
ネックベアリング４が一体的に設けられているとともに
長手方向に延びる溝５を有している。長手方向溝５と接
するネックベアリング４の領域に切欠きを設ける。

Description

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、股関節に使用する
人工補綴物であって、大腿骨に埋め込まれるステムを備
えるものに関する。このステムは、その上端部に一体的
に形成されたネックベアリングに連結されており、長手
方向に延びる溝を有している。
【０００２】
【従来の技術および発明が解決しようとする課題】ステ
ムに形成された長手方向の溝は、当該人工補綴物からそ
れを取り囲む骨組織への力の伝達を改善する。このこと
は、セメントを使用しない埋込み(implantation)の場合
に特に重要である。この場合、治療過程において骨組織
が溝内へと成長し、人工補綴物表面に対してある程度の
接着を示す。しかしながら、セメントを使用する埋込み
においても、人工補綴物のステムに溝を設けることが得
策である。事後的な外科手術(follow-up surgery)を行
う場合、溝内に存在する骨組織またはセメント材料を再
度除去しなければならない。このとき、手術を行う医師
は、基端側から適切なチゼル(chisel)を溝内に挿入しな
ければならない。この場合の前提条件は、上記溝への基
端側からのアクセスが可能であるということである。
【０００３】大腿骨に埋め込む股関節用の人工補綴物
は、ヨーロッパ特許第93378号(EP-B 93378)明細書の図
１および図２に示されている。この人工補綴物は、ステ
ムと一体的に構成されたネックベアリングを備えてい
る。ステムには長手方向に延びるリブが形成されてお
り、リブ間には、深い溝が形成されている。ステム上端
部において、リブおよび溝は、ネックベアリングのとこ
ろまで延びており、そこで終結している。ネックベアリ
ングが溝内の空間を連結方向から覆うので、事後的な手
術を行う場合に、この空間へのアクセスが困難となる。
したがって、埋め込まれる人工補綴物に対して取付部を
着脱可能とすることが提案されている(上記特許明細書
の図５)。しかしながら、人工補綴物への取付方法につ
いては詳しく開示されていない。同様の公知の人工補綴
物(Waldemar Link社のブローシュレ(Brochure)：リンク
・リブ・システム(Link rippen-system))においては、蹄
鉄形状をしたネックベアリングのアームが、ステム正面
および背面のガイド溝内に保持される。ネックベアリン
グを取り外すと、溝内の領域には、手術の邪魔となるよ
うな突出物は何もなくなる。しかしながら、ステムの長
手方向を横切って延在するガイド溝は、望ましくないこ
とが多い。その理由は、そのような溝を設けると、ステ
ムの断面積およびその強度が減じられるからである。さ
らに、ネックベアリングの信頼性に関して言うと、多く
の医師が、ステムと一体的に構成されたネックベアリン
グの方を好んでいる。
【０００４】ステムの溝の無い中間の狭い側部にのみネ
ックベアリングのような小さな突起を設けることによっ
てこの問題を解決する股関節用の人工補綴物がフランス
特許出願公開第2595241号(FR-A 2595241)明細書に開示
されている。この人工補綴物においては、溝の形成され
たステムの前面および背面には、ネックベアリングは設
けられていない。しかしながら、ネックベアリングが骨
に与える圧力を最小限に抑えるために、表面積の大きな
ネックベアリングを形成する試みがなされている。した
がって本発明の人工補綴物においては、ステムの溝の設
けられた面にもネックベアリングを設ける。
【０００５】
【発明の開示】本発明により、ステムに一体的に連結さ
れたネックベアリングを有するにも拘わらず、長手方向
に延びる溝内の空間へアクセスが可能な股関節用人工補
綴物が提供される。このことは、長手方向溝と接するネ
ックベアリングの領域に切欠きを設け、該溝をネックベ
アリングよりも上方側で終結させることによって達成で
きる。言い換えると、長手方向溝は、ネックベアリング
を通過して連続的に延びており、該ネックベアリングは
長手方向溝によって切り欠かれている。したがって、事
後的な手術を行う場合に、溝内の領域における骨の連結
を離すための器具を溝内に挿入することができる。ま
た、埋込み時には、この空間を利用して骨材料のパッキ
ングを行うことができる。それでいて、本発明の人工補
綴物は、ステムに一体的に連結されたネックベアリング
を有する。ネックベアリングによって切欠きの外側に提
供される表面は、多くの場合において、十分な支持部を
提供する。もし、より大きな支持面が必要な場合には、
本発明に従って着脱可能な取付部を使用することにより
それが提供される。この取付部は、長手方向溝の領域に
おいて切欠きを実質的に閉止する。ネックベアリングお
よび(または)ステムが、ネックベアリングに沿って平行
に延在するスライドガイドを構成する。埋め込まれた当
該人工補綴物に対して、取付部がこのスライドガイドに
沿って着脱可能である。
【０００６】
【発明の実施の形態】添付の図面を参照して本発明の実
施の形態を以下に説明する。
【０００７】本発明の人工補綴物のステム１は、大腿骨
に埋め込まれるものであって、その上方端部がネック２
とされている。ネック２はコーン３を保持しており、こ
のコーン３にジョイントヘッドが配置される。ステム１
とネック２との間にはネックベアリング４が設けられて
いる。ネックベアリング４は、ステム１から360°すべ
ての方向において横向きに張り出している(少なくとも
前面側および背面側において、横向きに張り出していれ
ばよい)。ネックベアリング４の底面は、骨の切除面上
で支持されるように平坦とされている。
【０００８】ステム１は、その前面および背面に、その
中心線に平行に延びる２つの溝５を有している。人工補
綴物の表面とそれを取り囲む骨組織との安定した接続の
ためにこれらの溝５が設けられていることは知られてい
る。人工補綴物を取り囲む骨組織は、人工補綴物のステ
ムを受け入れる空腔部(cavity)を形成した際に骨側に残
された部分であって、ステムによって押しのけられると
きに溝内に到達する。あるいは、この溝にパッキングを
施すことによって該溝が詰められるか、骨組織が該溝内
において成長する。ステム１および溝５の断面形状を、
その外部輪郭として図６に示した。
【０００９】埋込み時に溝５内の骨組織にパッキングを
施すために、また、取外し時に骨組織と補綴物表面とを
離すために、溝の横断面へのアクセスを容易にする必要
がある。これを確実とするために、図４に示したよう
に、ネックベアリング４の各溝５に接する位置には切欠
き６が設けられている。このような切欠き６を設けたと
しても、ネックベアリング４に相当の表面領域が残され
るということが分かるであろう。状況にもよるが、この
表面領域は、例えば切除面上で支持されるのには十分で
あろう。
【００１０】ネックベアリング４の残りの表面領域では
不十分な場合には、取付部７が追加される。この蹄鉄形
状の取付部は、その内部８に沿って平行に延びる２つの
ガイドエッジ９を有している。ステム１の上端部であっ
て、ネックベアリング４の下方の位置には、前面側およ
び背面側に受入れ溝(スライドガイド)10が設けられてい
る。各受入れ溝10は、各ガイドエッジ９に対応してい
る。図１中に矢印で示したように取付部７をステム１側
に向けて押圧すると、取付部７のガイドエッジ９が受入
れ溝10に受け入れられて保持される。取付部７は、ネジ
11を用いて所定位置に固定することができる。図５に示
したように、取付部７は、前面側、背面側、およびその
中間領域においてベアリング面を広くするだけでなく、
切欠き６の大部分を閉止する。切欠き６は、ステムの中
心付近においてまで完全に閉止する必要はない。その理
由は、支持作用は、主として骨組織の皮層領域において
生じるからである。
【００１１】ステムと一体的に構成された部分４をネッ
クベアリングと呼んでいるのは、この部分がそれ単独で
(すなわち、取付部７を使用しない場合にも)ネックベア
リングとしての機能を果たさなければならないことを意
味しているのではない。その代わりに、この部分４が取
付部７を固定するためだけの機能を備えていて、両者が
一体となったときにのみネックベアリングとしての機能
を果たすように構成してもよい。
【００１２】図１に示したように、溝５は、ネックベア
リング４よりも上側の位置にまで延在し、そこから徐々
に消失している。この構成により、器具を取り付けるこ
とが容易になる。しかしながら、溝５がネックベアリン
グ４から、またはそれよりも下方の位置から、始まるよ
うに構成することも考えられる。

    【特許請求の範囲】
  1. 【請求項１】 大腿骨に埋め込まれるステム(１)を備え
    た股関節用人工補綴物であって、ステム(１)は、その上
    端部にネックベアリング(４)が一体的に設けられている
    とともに長手方向に延びる溝(５)を有しており、 長手方向溝(５)と接するネックベアリング(４)の領域に
    切欠き(６)が設けられていることを特徴とする、人工補
    綴物。
  2. 【請求項２】 上記長手方向溝(５)がネックベアリング
    (４)の上方側において終結していることを特徴とする、
    請求項１記載の人工補綴物。
  3. 【請求項３】 上記ネックベアリング(４)に着脱可能な
    取付部(７)が設けられており、該取付部(７)は、長手方
    向溝(５)の領域においてネックベアリング(４)の切欠き
    (６)を実質的に閉止していることを特徴とする、請求項
    １または２記載の人工補綴物。
  4. 【請求項４】 上記ネックベアリング(４)および(また
    は)ステム(１)が、ネックベアリング(４)に沿って平行
    に延在するスライドガイド(10)を構成し、 埋め込まれた当該人工補綴物に対して、取付部(７)がス
    ライドガイド(10)に沿って着脱可能である、請求項３記
    載の人工補綴物。
  5. 【請求項５】 上記ネックベアリング(４)および取付部
    (７)またはステム(１)が、協働するロック手段(11)を備
    えていることを特徴とする、請求項４記載の人工補綴
    物。
