【発明の属する技術分野】
本発明は、股関節に使用する人工補綴物であって、大腿骨に埋め込まれるステムを備えるものに関する。このステムは、その上端部に一体的に形成されたネックベアリングに連結されており、長手方向に延びる溝を有している。

【従来の技術および発明が解決しようとする課題】
ステムに形成された長手方向の溝は、当該人工補綴物からそれを取り囲む骨組織への力の伝達を改善する。このことは、セメントを使用しない埋込み(implantation)の場合に特に重要である。この場合、治療過程において骨組織が溝内へと成長し、人工補綴物表面に対してある程度の接着を示す。しかしながら、セメントを使用する埋込みにおいても、人工補綴物のステムに溝を設けることが得策である。事後的な外科手術(follow-up surgery)を行う場合、溝内に存在する骨組織またはセメント材料を再度除去しなければならない。このとき、手術を行う医師は、基端側から適切なチゼル(chisel)を溝内に挿入しなければならない。この場合の前提条件は、上記溝への基端側からのアクセスが可能であるということである。

大腿骨に埋め込む股関節用の人工補綴物は、ヨーロッパ特許第93378号(EP-B 93378)明細書の図１および図２に示されている。この人工補綴物は、ステムと一体的に構成されたネックベアリングを備えている。ステムには長手方向に延びるリブが形成されており、リブ間には、深い溝が形成されている。ステム上端部において、リブおよび溝は、ネックベアリングのところまで延びており、そこで終結している。ネックベアリングが溝内の空間を連結方向から覆うので、事後的な手術を行う場合に、この空間へのアクセスが困難となる。したがって、埋め込まれる人工補綴物に対して取付部を着脱可能とすることが提案されている(上記特許明細書の図５)。しかしながら、人工補綴物への取付方法については詳しく開示されていない。同様の公知の人工補綴物(Waldemar Link社のブローシュレ(Brochure)：リンク・リブ・システム(Link rippen-system))においては、蹄鉄形状をしたネックベアリングのアームが、ステム正面および背面のガイド溝内に保持される。ネックベアリングを取り外すと、溝内の領域には、手術の邪魔となるような突出物は何もなくなる。しかしながら、ステムの長手方向を横切って延在するガイド溝は、望ましくないことが多い。その理由は、そのような溝を設けると、ステムの断面積およびその強度が減じられるからである。さらに、ネックベアリングの信頼性に関して言うと、多くの医師が、ステムと一体的に構成されたネックベアリングの方を好んでいる。

ステムの溝の無い中間の狭い側部にのみネックベアリングのような小さな突起を設けることによってこの問題を解決する股関節用の人工補綴物がフランス特許出願公開第2595241号(FR-A 2595241)明細書に開示されている。この人工補綴物においては、溝の形成されたステムの前面および背面には、ネックベアリングは設けられていない。しかしながら、ネックベアリングが骨に与える圧力を最小限に抑えるために、表面積の大きなネックベアリングを形成する試みがなされている。したがって本発明の人工補綴物においては、ステムの溝の設けられた面にもネックベアリングを設ける。

【発明の開示】
本発明により、ステムに一体的に連結されたネックベアリングを有するにも拘わらず、長手方向に延びる溝内の空間へアクセスが可能な股関節用人工補綴物が提供される。このことは、長手方向溝と接するネックベアリングの領域に切欠きを設け、該溝をネックベアリングよりも上方側で終結させることによって達成できる。言い換えると、長手方向溝は、ネックベアリングを通過して連続的に延びており、該ネックベアリングは長手方向溝によって切り欠かれている。したがって、事後的な手術を行う場合に、溝内の領域における骨の連結を離すための器具を溝内に挿入することができる。また、埋込み時には、この空間を利用して骨材料のパッキングを行うことができる。それでいて、本発明の人工補綴物は、ステムに一体的に連結されたネックベアリングを有する。ネックベアリングによって切欠きの外側に提供される表面は、多くの場合において、十分な支持部を提供する。もし、より大きな支持面が必要な場合には、本発明に従って着脱可能な取付部を使用することによりそれが提供される。この取付部は、長手方向溝の領域において切欠きを実質的に閉止する。ネックベアリングおよび(または)ステムが、ネックベアリングに沿って平行に延在するスライドガイドを構成する。埋め込まれた当該人工補綴物に対して、取付部がこのスライドガイドに沿って着脱可能である。

【発明の実施の形態】
添付の図面を参照して本発明の実施の形態を以下に説明する。

本発明の人工補綴物のステム１は、大腿骨に埋め込まれるものであって、その上方端部がネック２とされている。ネック２はコーン３を保持しており、このコーン３にジョイントヘッドが配置される。ステム１とネック２との間にはネックベアリング４が設けられている。ネックベアリング４は、ステム１から360°すべての方向において横向きに張り出している(少なくとも前面側および背面側において、横向きに張り出していればよい)。ネックベアリング４の底面は、骨の切除面上で支持されるように平坦とされている。

ステム１は、その前面および背面に、その中心線に平行に延びる２つの溝５を有している。人工補綴物の表面とそれを取り囲む骨組織との安定した接続のためにこれらの溝５が設けられていることは知られている。人工補綴物を取り囲む骨組織は、人工補綴物のステムを受け入れる空腔部(cavity)を形成した際に骨側に残された部分であって、ステムによって押しのけられるときに溝内に到達する。あるいは、この溝にパッキングを施すことによって該溝が詰められるか、骨組織が該溝内において成長する。ステム１および溝５の断面形状を、その外部輪郭として図６に示した。

埋込み時に溝５内の骨組織にパッキングを施すために、また、取外し時に骨組織と補綴物表面とを離すために、溝の横断面へのアクセスを容易にする必要がある。これを確実とするために、図４に示したように、ネックベアリング４の各溝５に接する位置には切欠き６が設けられている。このような切欠き６を設けたとしても、ネックベアリング４に相当の表面領域が残されるということが分かるであろう。状況にもよるが、この表面領域は、例えば切除面上で支持されるのには十分であろう。

ネックベアリング４の残りの表面領域では不十分な場合には、取付部７が追加される。この蹄鉄形状の取付部は、その内部８に沿って平行に延びる２つのガイドエッジ９を有している。ステム１の上端部であって、ネックベアリング４の下方の位置には、前面側および背面側に受入れ溝(スライドガイド)10が設けられている。各受入れ溝10は、各ガイドエッジ９に対応している。図１中に矢印で示したように取付部７をステム１側に向けて押圧すると、取付部７のガイドエッジ９が受入れ溝10に受け入れられて保持される。取付部７は、ネジ11を用いて所定位置に固定することができる。図５に示したように、取付部７は、前面側、背面側、およびその中間領域においてベアリング面を広くするだけでなく、切欠き６の大部分を閉止する。切欠き６は、ステムの中心付近においてまで完全に閉止する必要はない。その理由は、支持作用は、主として骨組織の皮層領域において生じるからである。

ステムと一体的に構成された部分４をネックベアリングと呼んでいるのは、この部分がそれ単独で(すなわち、取付部７を使用しない場合にも)ネックベアリングとしての機能を果たさなければならないことを意味しているのではない。その代わりに、この部分４が取付部７を固定するためだけの機能を備えていて、両者が一体となったときにのみネックベアリングとしての機能を果たすように構成してもよい。

図１に示したように、溝５は、ネックベアリング４よりも上側の位置にまで延在し、そこから徐々に消失している。この構成により、器具を取り付けることが容易になる。しかしながら、溝５がネックベアリング４から、またはそれよりも下方の位置から、始まるように構成することも考えられる。

Claims (5)

  1. 大腿骨に埋め込まれるステム(１)を備えた股関節用人工補綴物であって、ステム(１)は、その上端部にネックベアリング(４)が一体的に設けられているとともに長手方向に延びる溝(５)を有しており、
    ネックベアリング（４）は少なくとも当該人工補綴物の前面側および背面側において横向きに張り出していて、長手方向溝(５)と接するネックベアリング(４)の領域に、上記溝（５）へアクセスするための切欠き(６)が設けられていることを特徴とする、人工補綴物。
  2. 上記長手方向溝(５)がネックベアリング(４)の上方側において終結していることを特徴とする、請求項１記載の人工補綴物。
  3. 上記ネックベアリング(４)に着脱可能な取付部(７)が設けられており、該取付部(７)は、長手方向溝(５)の領域においてネックベアリング(４)の切欠き(６)を閉止していることを特徴とする、請求項１または２記載の人工補綴物。
  4. 上記ネックベアリング(４)およびステム(１)の少なくとも一方に、ネックベアリング(４)に沿って平行に延在するスライドガイド(10)を構成し、
    埋め込まれた当該人工補綴物に対して、取付部(７)がスライドガイド(10)に沿って着脱可能である、請求項３記載の人工補綴物。
  5. 上記ネックベアリング(４)および取付部(７)またはステム(１)が、協働するロック手段(11)を備えていることを特徴とする、請求項４記載の人工補綴物。
