Claims (1)
- 【特許請求の範囲】 1.患者の胸部空洞で心臓を拘束する方法において、 患者の胸部の経皮肋間刺孔に位置決めされたスコープを通して患者の胸部空洞 の内部分を検視し、 患者の胸部における経皮肋間刺通を通して患者の胸部空洞にクランプを導入 し、 スコープによる透視下でクランプを冠状動脈と腕頭部動脈との間で大動脈( この大動脈は血液が心臓から流通する大動脈内腔を有している)のまわりに位置 決めし、 大動脈内腔を通る血液の流れを遮断するように患者の胸部空洞の外側の位置 からクランプを作動し、 クランプの上流で心臓麻痺流体を供給カニューレの供給内腔を通して大動脈 内腔に供給し、供給カニューレは、遠位端部がクランプの上流で大動脈内腔内に 配置され、近位端部が胸部空洞の外側に配置されることを特徴とする患者の胸部 空洞で心臓を拘束する方法。 2.供給カニューレは患者の胸部における経皮刺孔を通って延びることを特徴と する請求項1に記載の方法。 3.供給カニューレの遠位端部で大動脈の壁部に刺さることにより供給カニュー レを大動脈内腔に導入することを特徴とする請求項2に記載の方法。 4.クランプに対して所定位置で大動脈の壁部に刺さるように供給カニューレを 案内することを更に含むことを特徴とする請求項3に記載の方法。 5.クランプに連結されたガイド手段により供給カニューレを案内することを特 徴とする請求項4に記載の方法。 6.供給カニューレを患者の末梢動脈に導入し、上行大動脈の大動脈内腔の内腔 通過的に位置決めすることを特徴とする請求項1に記載の方法。 7.供給カニューレを大腿動脈に導入することを特徴とする請求項6に記載の方 法。 8.クランプを作動して大動脈内腔を遮断する前に供給カニューレを上行大動脈 に位置決めすることを特徴とする請求項6に記載の方法。 9.クランプを患者の胸部における肋間通孔を通して位置決め可能な細長いシャ フトの遠位端部に固定することを特徴とする請求項1に記載の方法。 10.クランプをシャフトの近位端部に連結されたアクチュエータによって作動す ることを特徴とする請求項9に記載の方法。 11.心臓麻痺流体を供給した後、大動脈の壁部の刺孔を閉じることを更に含むこ とを特徴とする請求項3に記載の方法。 12.閉じ工程はプラグまたはパッチを刺孔に付けることよりなることを特徴とす る請求項11に記載の方法。 13.閉じ工程は閉じられた刺孔を縫い合わせることよりなることを特徴とする請 求項11に記載の方法。 14.閉じ工程はステープルを大動脈壁部に付けることよりなることを特徴とする 請求項11に記載の方法。 15.刺孔を閉じる手段がクランプまたは供給カニューレに取付けられており、閉 じ手段を胸部空洞の外側から作動することを特徴とする請求項11に記載の方法 。 16.患者の心臓を拘束するための胸腔鏡装置において、 冠状動脈と腕頭部動脈との間で患者の上行大動脈のまわりに位置決め可能な クランプを備え、該クランプは患者の胸部における経皮肋間通孔を通して患者の 胸部空洞に導入するように構成されており、 クランプに連結された遠位端部、および遠位端部と反対側の近位端部を有し 、クランプを患者の胸部空洞の外側の位置から上行大動脈のまわりに位置決めす るための細長いハンドル手段と、 ハンドル手段の近位端部に連結され、上行大動脈の大動脈内腔を通る血液の 流れを遮断するようにクランプを作動するための手段と、 患者の胸部空洞の外側の位置からクランプの上流で大動脈内腔に心臓歌碑流 体を供給する手段とを備えたことを特徴とする胸腔鏡装置。 17.心臓歌碑流体を供給する手段は上行大動脈の下流の動脈から大動脈内腔内に 内腔通過的に位置決め可能な血管内供給カテーテルよりなることを特徴とする請 求項16に記載の装置。 18.供給カテーテルは大腿動脈から大動脈内腔の中に内腔通過的に位置決め可能 であることを特徴とする請求項17に記載の装置。 19.クランプは供給カテーテルを位置決めした状態で大動脈内腔を通る血液の流 れを遮断するように構成されていることを特徴とする請求項17に記載の装置。 20.心臓麻痺流体を供給する手段は患者の胸部における経皮通孔を通して位置決 め可能な供給カニューレよりなり、該供給カニューレはポートを備えた遠位端部 と、近位端部と、ポートと流体連通した供給内腔と、遠位端部に設けられ、大動 脈の壁部に刺さるための手段と、近位端部に設けられ、心臓麻痺流体を供給内腔 に供給するための手段とよりなることを特徴とする請求項16に記載の装置。 21.供給カニューレはクランプに対する所定位置で大動脈の刺通を容易にするた めにハンドル手段および/またはクランプに連結されていることを特徴とする請 求項20に記載の装置。 22.供給カニューレはクランプに対して長さ方向に摺動可能であることを特徴と する請求項21に記載の装置。 23.供給カニューレはハンドル手段を介して内側内腔内に摺動可能に位置決めさ れることを特徴とする請求項22に記載の装置。 24.供給カニューレは大動脈に刺さる位置を選択するためにクランプに対して位 置調整し得ることを特徴とする請求項22に記載の装置。 25.心臓麻痺流体を供給する手段は近位端部から遠位端部まで延びるハンドル内 の供給内腔と、ハンドルの遠位端部に連結され、大動脈の壁部に刺さるための手 段とよりなり、刺通手段はハンドルの供給内腔と流体連通している内側内腔を有 していることを特徴とする請求項16に記載の装置。 26.クランプは大動脈の第1側に合わさるための第1接触面を有する第1ジョー と、大動脈の第2側に合わさるための第2接触面を有する第2ジョーととよりな り、第1および第2ジョーは開位置から締めつけ位置まで互いに向けて移動可能 であり、大動脈内腔を通る血液の流れを遮断するように構成されていることを特 徴とする請求項16に記載の装置。 27.第1ジョーは第2ジョーに枢着されていることを特徴とする請求項26に記 載の装置。 28.第1および第2接触面は締めつけ位置で概ね平行であることを特徴とする請 求項26に記載の装置。 29.第1および第2接触面は開位置と締めつけ位置との間の運動範囲にわたって 概ね平行であることを特徴とする請求項28に記載の装置。 30.クランプは大動脈のまわりに位置決め可能である可撓性ケーブルまたはスト ラップと、ケーブルまたはストラップを張る手段とよりなることを特徴とする請 求項16に記載の装置。 31.ハンドル手段の遠位端部に連結され、供給カニューレの刺通手段により大動 脈に形成された刺孔を閉じるための手段を更に備えていることを特徴とする請求 項20に記載の装置。 32.閉じ手段は大動脈の壁部にステープルを付けるための手段よりなることを特 徴とする請求項31に記載の装置。 33.ステープルは第1脚部と、第2脚部と、これらの脚部の間の連結セグメント とを有しており、第1および第2脚部は各々、近位端部と、大動脈の壁部の一部 に刺さるための遠位先端とを有していることを特徴とする請求項32に記載の装 置。 34.第1および第2脚部は、非応力付与位置で先端がその近位端部より近いよう に構成されていることを特徴とする請求項33に記載の装置。 35.連結セグメントは第1および第2脚部の先端を開位置へ広げ易くするために 弾性的に曲げ可能であることを特徴とする請求項34に記載の装置。 36.ステープル付け手段は第1および第2脚部が開位置にある状態でステープル を付けるように構成されていることを特徴とする請求項35に記載の装置。 37.ステープル付け手段は近位端部と、遠位端部と、これらの端部間の通路と、 通路の遠位端部に設けられ、開位置でステープルを保持するための手段とを有す るスリーブよりなることを特徴とする請求項35に記載の装置。 38.ステープルを通路に放出するための手段を更にスリーブの通路に備えている ことを特徴とする請求項37に記載の装置。 39.供給カニューレはスリーブの通路に摺動可能に配置されていることを特徴と する請求項38に記載の装置。 40.供給カニューレは、これを通路内で遠位方向に前進させるとステープルに係 合してこれを放出するように構成されていることを特徴とする請求項39に記載 の装置。 41.閉じ手段はハンドルの近位端部に閉じ作動手段を有していることを特徴とす る請求項31に記載の装置。 42.大動脈への供給カニューレの選択的刺通を容易にするために供給カニューレ の連結された供給アクチュエータをハンドルの近位端部に備えていることを特徴 とする請求項41に記載の装置。 43.閉じ作動手段は、大動脈から供給カニューレを取り出すと、閉じ手段が作動 されるように、供給アクチュエータに連結されていることを特徴とする請求項4 2に記載の装置。 44.患者の身体旧道における中空または管状の身体構造を治療する方法において 、 クランプを経皮通孔を通して身体空洞に導入し、 内側内腔を有する身体構造のまわりにクランプを位置決めし、 身体構造の内側内腔を遮断するようにクランプを患者の身体空洞の外側の位 置から作動し、 クランプに連結された供給カニューレの供給内腔を通して身体構造の内側内 腔に流体を供給し、供給カニューレは遠位端部が内側内腔内荷配置され、近位端 部が身体構造の外側に配置されることを特徴とする方法。 45.中空または管状の身体構造を締めつけ、そこに流体を注入するための内視鏡 装置において、 身体構造のまわりに位置決め可能なクランプを備え、該クランプは経皮通孔 を通して患者の身体空洞に導入するように構成されており、クランプに連結され た遠位端部と、該遠位端部と反対側の近位端部とを有し、身体空洞の外側の位置 からクランプを身体構造のまわりに位置決めするための細長いハンドル手段と、 ハンドル手段の近位端部に連結され、身体構造における内側内腔を遮断する ようにクランプを作動するための手段と、 クランプに連結され、患者の身体空洞の外側の位置から流体を内側内腔に供 給するための手段とを備えたことを特徴とする内視鏡装置。
