【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は人工股関節用大腿骨
内補装具に関する。
【０００２】
【従来の技術並びに発明が解決しようとする課題】最近
の１０年間で、２つの処置方法が、痛みのかなりの知覚
や大事な機能制限を生じる壊れた大腿骨頭の処置のため
に知られている。
【０００３】第１は、患者に矯正具、即ち、外部支持装
置を付けることを可能にしている。この治療の可能性は
美学的観点で完全には受け入れられていないことに加え
て、治療技術の点からも最適ではない。
【０００４】さらにより広く受け入れられている可能性
は、患者に内補装具、即ち、股関節ステム内補装具を付
けることである。これに関連して、ステムは予め穿孔さ
れた大腿骨の骨髄領域に設置され、そして、ここに、セ
メントを使用し、もしくは使用しないで固定される。こ
のような内補装具により、人工球間接部との基端部側で
の接続が可能となる。
【０００５】後者の可能性は特許文献に正確に開示され
ている。この点で、一例としてＤＥ−Ｕ−９４１２４０
８が知られている。
【０００６】特許文献に開示されている大腿骨ステム内
補装具の１もしくは他のなかで、患者の体で長期に渡っ
て安定である幾つかの試みがある。しかし、いつかは、
例えば、１０ないし１５年の後に、摩耗や亀裂が内補装
具に生じることが見られ、この結果、埋め込んだ内補装
具を除去し新しいものに取り替える修正処置が必要にな
る。これは、骨髄領域の切除ならびに清掃により初期の
移植で自然の骨材の一部が除去されるので、全く問題が
ないことはない。一度除去された骨材は修正処置ではほ
とんど存在しない。かくして、これは、生存中に少なく
とも１回の修正処置を受けることが最初から想定できる
患者が比較的若い人の場合には特に、影響する。
【０００７】一般の股関節ステム内補装具よりも自然の
骨材の切除が少なく取り扱われる内補装具がＤＥ−２７
２４２３４Ｃ２により知られている。この開示されてい
る内補装具の部材は、骨髄領域中にまで延出するステム
を使用しないで、大腿骨の上領域中であるが、小転子の
下方に設置される。この補装具は、スラストワッシャー
内補装具として知られており、基部締め付けプレートに
より、除去された股関節頭の領域で切除される大腿骨の
皮質に基本的には達する。そして、これは締め付けプレ
ートで支えられる。この締め付けプレートは、内補装具
と大腿骨との間の全ての機械的力を大腿骨の皮質層が直
接受けるように大腿骨の横方向に延びている。この結
果、我慢できないように感じる海綿体への機械的歪みが
避けられる。そして、少量の骨セメントのみが基部領域
でスラストワッシャーを固定するために必要である。
【０００８】上述したように、この内補装具の原理の裏
付けは、大腿骨の海綿体が可能なかぎり小さい歪みを受
けるであろうという仮定に基づいている。これは、全て
の力が皮質上に加えられず、中に加えられるので、大腿
骨の皮質上に過度の歪みを犠牲にしてなされる。
【０００９】さらに、ＷＯ８９／１１８３７は、基部締
め付けプレートの機能が、特別に構成された股関節頭に
より上述した文献に従っていけるように同様に動作する
内補装具を開示している。これには、切除される大腿骨
断端が、圧力をかけられて中に設置される内部凹所が設
けられている。
【００１０】これと接続して主負荷が大腿骨の海綿体に
より受けるようになっている２つの補装具がＦＲ ２６
２６１６９並びにＦＲ ２６７４１２２Ａ１により知ら
れている。この２つの文献のうちの最初のものでは、基
部プラグ状円錐体を有し、海綿体にねじ込まれるための
ねじボルトが設けられている。これは、非常に短期間で
不安定な問題が生じる。２つの文献のうちの後者では、
プラグ状円錐体が一連の骨ねじにより締められており、
これら骨ねじは、海綿体中に達している。これからで
も、海綿体は本質的に大腿骨の皮質に比較して僅かの点
歪みのみを受けるので、重大な安定性の疑問が生じる。
【００１１】このような背景に対して、本発明の目的
は、大腿骨中への力の付与に対して自然の状態に、上述
した補装具よりも比較的良く適用可能で、自然の骨材の
切除が僅かで済む人工股関節用大腿骨内補装具を提供す
ることである。
【００１２】
【課題を解決するための手段】この目的は、以下の構成
からなっている大腿骨内補装具により達成される。
【００１３】大転子の下方の大腿骨の領域内にセメント
を使用しないで移植され得るシエル。このシエルの基端
部は、ジョイントの人工球部を収容するためにアダプタ
ーに接続可能であり、シエルの外面は少なくとも一部が
解放メッシュ状の３−デイメンション螺線格子構造体で
覆われている。
【００１４】シエルの先端部内の透孔を通ってシエルの
内部に挿入され得る引っ張りボルトが中で締め付けら、
また、シエル内に、この中に形成されたねじと螺合でき
る引っ張りプレート。
【００１５】上述したスラストワッシャー式内補装具と
は異なり、治癒されている間、骨小柱がシエルに設けら
れた解放メッシュ状の３−デイメンション螺線格子構造
体を通るように成長した後には、歪の力は、大腿骨の皮
質中にかからず、大腿骨の中の海綿体に直接かかる。か
くして、ＤＥ−２７２４２３４で欠点として見られた即
ち、歪みが海綿体にかかる、本発明の内補装具で記載さ
れた方法が正確になる。治癒の後、内補装具のシエル
は、幾つかの骨の決締組織繊維柱に自身で支えられるの
に基づいて疑似自然状態で活動的で機能する。即ち、問
題の歪に従って回避的動きを限定的に再構築することが
でき、また、補償する動きを果たすことができる。内補
装具のこの固定の形式は、外科医による最近の認識に対
応している。即ち、筒状の骨において、主な歪は皮質に
より吸収されないで、海綿体により吸収される。皮質の
圧力負荷は、本発明の内補装具で生じない。
【００１６】移植の準備として、最初に大腿骨の壊れた
頭が、大腿骨の首の軸に基本的には直交してサブキャピ
タル切除により除去される。大腿骨の首中へ、移植され
るシエルと同じような外形状を基本的には有するフライ
ス具を作用させて、大腿骨の海綿体の対応する穿孔もし
くはくり抜きが行われる。外側から大腿骨に上記引っ張
りプレート（引っ張りねじを使用してシエルの内部にね
じ込まれ得る）の移植が、シエルを横方向に締め付ける
ようになされる。このために、引っ張りねじがシエルの
内部に小柱を通って案内され、ここにねじ留めされ得る
ように、小柱に横方向に穴を開けることが必要である。
引っ張りねじを備えた引っ張りプレートは、基本的に親
しまれるテンションフランジシステムを具備する。
【００１７】シエルの基端部に接続されるアダプターは
いかなる所望の形態をとり得る。一般的に、上記ＤＥ−
Ａ−２７２４２３４から知られているようなスラストワ
ッシャーと接続されて効果が生じないことに注意する必
要がある。
【００１８】シエルは、断面において、楕円形状を有す
ることが有効である。かくして、海綿体内でのシエルの
回転に対する安定性は、予め達成される。
【００１９】さらに、基端部から先端部に向かって円錐
状に傾斜するようにシエルを構成することが好ましい。
第１に、これは、大腿骨の首の円錐形状に多少とも対応
し、解剖学上の適用を果たす。第２に、シエルが、例え
ば、円筒状に構成されているよりも、シエルが引っ張り
ねじにより支えられおり、負荷がかけられている場合に
は、力の分布は、骨全体に渡るような面圧力により治癒
中により適するようになる。

である必要はない。尾方向並びに頭方向（かくして、上
方向並びに下方向）に向かうシエルの外側で３ないし６
ｍｍのメッシュ幅の粗いメッシュで格子構造体を構成す
ることが好ましい。これは、大腿骨内の自然の海綿体床
での負荷の主な方向に並びに大腿骨内の自然移植支持の
海綿体患部の軌道に対応する。
【００２１】腹方向並びに背方向に面するシエルの外側
で１ないし２．５ｍｍのメッシュ幅の細かいメッシュで
格子構造体を構成することは、さらなる解剖学適用とし
て効果的に設けられ得る。格子構造体がシエルの外面で
２つの異なるメッシュサイズで構成される場合、上述し
たシエルの断面が楕円形となる。この場合、楕円の比較
的長い主軸は、粗いメッシュの格子構造体が配設されて
いる領域内で見られ、一方、比較的短い二次的な軸は、
小さいメッシュサイズに関連して見られる。
【００２２】球間接部のためのアダプターは基部に環状
フランジを備えた２重プラグ円錐部として構成されてお
り、基部領域で円錐プラグソケットが設けられ、１つの
プラグ円錐体に一致するように構成されていることが、
特に好ましい構成である。各位て、円錐圧縮ジョイント
が、１つのプラグ円錐体とソケット内のプラグシエルと
の間に既知の原理に従って構成され得る。一方、他方の
プラグ円錐体は人工の骨端の丸い隆起内のプラグソケッ
トと共同する。
【００２３】以下のことをこの接続で注意するべきであ
る：両円錐基部の周りにあるフランジは、一例として、
ＤＥ−２７２４２３４Ｃ２に従う基部スラストワッシャ
ーのように、切除面での設置のための機能はしない。理
想的に、移植されたシエルで円錐圧縮ジョイントを完成
した後に、約１ｍｍの幅の間隙が、切除面とこの面に面
れに面したフランジ側との間に設けられる。フランジの
外方に面した表面が解放メッシュ状の３−デイメンショ
ンの特殊な格子構造体で覆われていると、移植後の時間
の推移で、骨小柱が、これを通って成長する。この押さ
えは単にアダプターを留める機能をし、大腿骨の皮質内
に負荷の力を導かない。代わって、ねじを使用する取り
付けが、また考えられ得る。
【００２４】修正処置が必要になったときに、のこぎり
の刃が上記間隙中に挿入され、アダプターとフランジと
切除面との間の骨小柱が簡単に分離され得る。この結
果、アダプターは、取り出し工具を使用してシエルから
簡単に引き抜かれ得る。
【００２５】さらに、前記アダプターは、対応したなね
じセクションが設けられた引張りねじの端部が留めるた
めに螺合され得る、シエルに面したアダプターのプラグ
円錐体内の雌ねじがめくり穴に形成されている効果的な
やり方により、円錐圧縮で保護され得る。この場合、引
張りねじは、また、シエルの内部を通ってねじに達し、
この結果、引張りプレートは、シエルと共に、引張りね
じにねじ留めされる。即ち、引張りねじは、プラグシエ
ルの基部領域に挿入される。かくして、アダプターのプ
ラグ円錐体は、この場合、大腿骨内でシエルの円錐コレ
ット中に引張りねじを使用して引かれ、ここで動かない
ように留められる。
【００２６】アダプターとシエルとの間の接続が行われ
たときには、大腿骨の皮質の外でのシエルと引張りプレ
ートとの間の接続が完了した後には緩みが生じないよう
に、引張りねじの端部のねじセクションは、好ましく
は、シエル内のねじとは異なるピッチを有している。自
己反作用がこの手段により達成され、最初になされた接
続が緩むのを正確に防止している。
【００２７】大腿骨内補装具のさらに好ましい実施の形
態においては、大腿骨と面する引張りプレートの面もま
た、解放メッシュ状の３−デイメンション螺線格子構造
体で覆われている。この方法で、負荷のもとで微動に対
して移植物のさらなる留めが生じる。理論的には、引張
りプレートは、シエルが大腿骨の海綿領域内へのシエル
の設置を完了した後は不要となる。これは、この形式の
固定が移植物と周囲の骨との間に極めて密な接続ができ
るためである。長期の固定に関し、また不必要なリスク
を避けるための考慮は、引張りプレートにこの付加の安
全他対策をすることが望ましい。
【００２８】大腿骨の横皮質上の引張りプレートの位置
に関わる他の安全対策は以下のようである。即ち、引張
りプレートには大腿骨の方向に向いたペグの形態の少な
くとも１つの当接体が設けられている。このペグは、引
張りプレートに対して、シエルの主軸と同じように引張
りプレートに対して同じ角度αをなしている。この結
果、引張りプレートの軸として、この主軸が理解され
る。この当接体は回転を阻止するように機能刷る。ま
た、同時にいかなる曲げモーメントも負荷され得ない。
【００２９】
【発明の実施の形態】以下に、同じ部材は同じ符号が付
されている。本発明の内補装具のシエル１は、基端部８
と先端部６とを有する。この実施の形態において、シエ
ルは基端部８から先端部６に向かって細くなる円錐状に
延びており、力の分布に対して所望の動作を果たす。ま
た、断面で示されたこのシエル１は、比較的粗いメッシ
ュ幅（３ないし６ｍｍ）の解放メッシュ状の３−デイメ
ンションの特殊な格子構造体９で外面が覆われており、
また、大腿骨中に埋め込まれた後には、尾方向にかつ頭
方向に面するシエル外側が覆われる。これは、大腿骨の
首部内で自然の骨の小柱に近いサイズに解剖学的な適用
に対応している。
【００３０】前記解放メッシュ状の３−デイメンション
の特殊な格子構造体９は、尾方向にかつ頭方向に面した
シエルの外面に同様に設けられているが（図４）、これ
は、図４に概略的に示すように、基本的には比較的小さ
いメッシュ幅である。このメッシュ幅は、約１ないし
２．５ｍｍである。
【００３１】透孔５が、シエル１の先端内に形成されて
おり、この中には、シエル１の基端部８の方向に延びた
雌ねじ７が設けられている。この透孔は、基端部８の所
で円錐コレット１２で終端している円筒ボアとなってい
る。
【００３２】この内補装具のさらに重要な部材は、図２
に一例として示されている引張りプレート３である。こ
のプレートは、後で詳述するように、引張りねじを案内
するための開口２１を備えたプレート状部材を基本的に
は有する。
【００３３】この引張りプレートは、大腿骨方向に向い
た解放メッシュ状の３−デイメンションの特殊な格子構
造体を有し、これによって、骨小柱は、引張りプレート
３の、かくして、移植後のこのプレートに接続されたシ
エル１の位置固定を果たすために大腿骨の皮質から前方
に成長する。
【００３４】明らかなように、引張りプレート３は、大
腿骨の方向にペグ１８の形態の当接体を有し、これは、
引張りプレート３方向へのシエル１の主軸と同じ軸で傾
斜されている。このようにして、付与される可能性のあ
るいかなる曲げモーメントに対する特別な安定性が達成
される。
【００３５】図３は引張りねじ４の図である。この頭部
１９の前に４つのセクションを有する。全ての最初に、
隣接した円柱セクション２２と比較して短い直径の円筒
セクション１５が頭部１９に接続されている。、前記頭
部１９で開口している。この円柱セクションにねじセク
ション７´が続いており、これにはさらに別のねじセク
ション１４´が接続されている。このねじセクション７
´は、シエル１内の雌ねじ７と共同する。即ち、引張り
ねじ４が、引張りプレート３並びに大腿骨の皮質の孔を
通って、開口５からシエル１中に挿入されたときに、共
同する。小径のセクション１５は引張りプレートの開口
２１内での引張りねじ４の位置の微調整をする。
【００３６】引張りねじ４の先端ねじセクション１４´
は、例えば、図５に部分的に断面して示すように、アダ
プター２と共同する。このアダプター２は、互いに離れ
る方向に突出した２つのプラグ円錐部１０，１０´によ
り構成されている。一方のプラグ円錐部１０は、プラグ
ソケット１２内に挿入されるために設けられ、かくし
て、円錐圧縮ジョイントの作用によりソケット内に維持
されている。雌ねじ１４を有するめくら穴１３が位置を
さらに固定するためにプラグ円錐体内に設けられてい
る。この引張りねじ４のねじセクション１４´は雌ねじ
１４内に螺合される。
【００３７】人工球関節部が、円錐体１０´と球間接部
との間に生じる円錐圧縮ジョイントの作用のような通常
の方法で他のプラグ円錐体１０´に接続され得る。
【００３８】環状フランジ１１が、円錐体１０，１０´
の基部の領域に設けられている。このフランジは、大腿
骨の切除領域にアンロードを形成するための上述した機
能はしない。このフランジは、単に、回転に対する補償
を果たす。このために、幾つかの周囲開口２３がフラン
ジ１１に形成されており、これらを介して、装着のため
の適当な手段（図示せず）が、回転の補償を果たすため
に位置され得る。例えば、小ねじが大腿骨内に案内さ
れ、さらに、開口２３を通って案内され得る。これとは
異なり、もしくはこの装着の可能性に加えて、フランジ
１１の外方に面した面が解放メッシュ状の３−デイメン
ションの特殊な格子構造体１６で覆われるようにされ
る。内補装具の移植に続いて、骨小柱が、大腿骨の切除
表面からみの格子構造体中に成長し、または、これらを
通って成長し、回転に対して安定となる。
【００３９】好ましくは、内補装具の移植は、切除表面
とフランジ１１との間に、例えば、１ｍｍの隙間がある
ようになされる。この隙間は、困難がなく骨小柱により
克服され得るが、しかし、それにもかかわらず、修正処
置の間に、間隙中にのこぎり刃を挿入することにより容
易に再び分離され得る。
【００４０】内補装具の上述した部材の結合作用の様子
は図６で概略的にわかる。
【００４１】移植の間、大腿骨の横皮質（図示せず）
は、引っ張りプレート３とシエル１との間に位置され
る。そして、引っ張りねじ４が、引っ張りプレート３の
開口２１と皮質の孔とを通ってシエル１中に案内され
る。この引っ張りねじ４のねじセクション７´がシエル
１の雌ねじ７に螺合された後に、シエル１は外方（横方
向）に引かれる。この後に、アダプター２のプラグ円錐
部１０がシエル１のプラグソケット１２中に挿入される
ようにして、アダプター２はシエル１に適用され得る。
これは、図示された場合には、アダプター内にねじ込ま
れることによりなされ、引っ張りねじ４のねじセクショ
ン１４´が、プラグ円錐部１０の雌ねじ１４中にねじ込
まれる。
【００４２】シエル１の回転の安定性は、この場合に
は、上述したように異なるメッシュサイズの解放メッシ
ュ状の３−デイメンションの特殊な格子構造体でシエル
１の外面を覆うことによる楕円形の断面により最初から
達成される。
【００４３】最後に、図７に示すように、内補装具の位
置は、最初の位置で、明らかになる。さらに、人工股関
節を作るために、人工ジョイントソケットと関連して機
能する人工球ソケット部２０が図示されている。
【００４４】発明の概念からそれないで上記実施の形態
を変更することは当業者によりなされ得る。かくして、
本発明は、上記特別な実施の形態に限定されることはな
く、請求の範囲により規定された本発明の精神と範囲内
での変更を可能にしている。

Claims (10)

    【特許請求の範囲】
  1. 【請求項１】以下の構成要件（ａ），（ｂ）からなって
    いる人工股関節用大腿骨内補装具。 （ａ）大転子の下方の大腿骨の領域内にセメントを使用
    しないで移植され得るシエル（１）であり、このシエル
    の基端部（８）は、人工球部ジョイント部（２０）のを
    収容するためにアダプター（２）に接続可能であり、シ
    エルの外面は少なくとも一部が解放メッシュ状の３−デ
    イメンションの特殊な格子構造体（９）で覆われてい
    る。 （ｂ）シエルの先端部（６）内の透孔（５）を通ってシ
    エルの内部に挿入され得る引っ張りねじ（４）が中で締
    め付けられ、また、シエル内に、この中に形成されたね
    じ（７）と螺合できる引っ張りプレート（３）。
  2. 【請求項２】 前記シエル（１）は基端部（８）から先
    端部（６）に向かって円錐状に傾斜している請求項１の
    大腿骨内補装具。
  3. 【請求項３】 前記格子構造体（９）は、尾方向並びに
    頭方向に面するシエルの外側で３ないし６ｍｍのメッシ
    ュ幅の粗いメッシュである請求項１の大腿骨内補装具。
  4. 【請求項４】 前記格子構造体（９）は、腹方向並びに
    背方向に面するシエルの外側で１ないし２．５ｍｍのメ
    ッシュ幅の細かいメッシュである請求項１ないし３のい
    ずれか１の大腿骨内補装具。
  5. 【請求項５】 球間接部のためのアダプター（２）は、
    基部に環状フランジ（１１）を備えた２重プラグ円錐部
    （１０，１０´）として構成されており、対応して構成
    された円錐プラグソケット（１２）は、シエル１の基部
    領域に設けられている請求項１ないし４のいずれか１の
    大腿骨内補装具。
  6. 【請求項６】 雌ねじ（１４）を備えためくら穴（１
    ３）がシエル１に面するアダプター（２）のプラグ円錐
    部１０に設けられており、対応したねじセクション（１
    ４´）を有する引っ張りねじ（４）の端部が留めるため
    にねじ込まれる請求項５の大腿骨内補装具。
  7. 【請求項７】 前記引っ張りねじ（４）の端部のねじセ
    クション（１４´）は、シエル１の内部のねじ（７）と
    は異なるピッチを有する請求項６の大腿骨内補装具。
  8. 【請求項８】 前記アダプター（２）のフランジ（１
    １）の外方に面する面は、解放メッシュ状の３−デイメ
    ンションの特殊な格子構造体（１６）で覆われている請
    求項１ないし７のいずれか１の大腿骨内補装具。
  9. 【請求項９】 大腿骨と面する引っ張りプレート（３）
    の面は、解放メッシュ状の３−デイメンションの特殊な
    格子構造体（１７）で覆われている請求項１ないし８の
    いずれか１の大腿骨内補装具。
  10. 【請求項１０】 前記引っ張りプレート（３）は、大腿
    骨の方向に向いたペグ（１８）の形態の少なくとも１つ
    の当接部を有し、このペグはシエルの主軸と同様に引っ
    張りプレートに対して角度αだけ、引張りプレート
    （３）の主軸に対して傾斜している請求項１ないし９の
    いずれか１の大腿骨内補装具。
