(57)【要約】 【課題】 長期間の固定振舞いが好ましく、磨耗が進行した後に、標準部品を別なものに容易に交換することも可能にする外部補綴用標準部品のアダプタの開発。 【解決手段】 長骨断端内に基端側の幹部分が位置するように取り付けることができるように作られ、幹部分が、少なくとも部分的に開放メッシュの三次元的網目構造体にて覆われ、その他端である末端には外部補綴用標準部品の接続装置が設けられたアダプタの提供。

【発明の詳細な説明】
【０００１】 本発明は、外部補綴用標準部品のアダプタに関する。
【０００２】 肢を切断した患者の外部補綴処置として色々な方法が公知である。
【０００３】 例えば、腿の断端領域内において、腿の断端の寸法及び形状に適合し得るよう
にされた外部補綴軸、例えば補綴軸を専門に取り扱うことが考えられる。次に、
所定の場合、人工外部補綴膝関節、ふくらはぎ、及び足をこの補綴軸と接続する
。
【０００４】 しかしながら、例えば更に走る場合に、より自然の知覚機能を患者に与えるこ
とができるように残存する長骨の断端の端部に固定される装置が存在する。これ
は骨知覚（ｏｓｔｅｏｐｅｒｃｅｐｔｉｏｎ）という名にて公知である。例えば
、長骨と固定するためにねじを有するアダプタ端部を残存する長骨にねじ込む。
アダプタは、パイプ片に似ており、該アダプタは、最終的に、断端の端部の組織
から外に出る。その後、例えば、人工膝関節のような外部補綴用標準部品をこの
アダプタに接続する。
【０００５】 上述した型の公知のアダプタの場合、標準部品を接続する場合と丁度同じよう
に、長骨の断端内にアダプタを固定するとき安定性が問題となる。
【０００６】 この背景に対して、本発明の課題は、長期間の固定振舞いが好ましく、また、
例えば、磨耗が進行した後に標準部品を別なものに容易に交換することも可能に
する外部補綴用標準部品のアダプタを開発することである。
【０００７】 この目的は、請求項１の特徴を有するアダプタによって達成される。
【０００８】 従って、長骨断端内に基端側の幹部分が位置するように取り付けることができ
るように作られるアダプタであって、幹部分が、少なくとも部分的に、開放メッ
シュの三次元的網目構造体にて覆われ、その他端である末端には外部補綴用標準
部品の接続装置が設けられたアダプタが提案される。
【０００９】 相互接続とも称される開放メッシュの三次元的網目構造体は、治癒する段階の
間、自然の骨材料が幹の内部へ幹部分を貫通し、また、幹部分の周りに成長する
ため、それぞれの場合において比較的短時間の後、幹部分が長骨内の基質の流れ
に関して同化し、極めて安定的な二次的固定を確実にすることを可能にする。開
放三次元的網目構造体中への骨材料の成長は、純粋な体内補綴法及びそれ自体、
公知である。例えば、ここで、ドイツ国特許第４１ ０６ ９７１号を参考とす
る必要がある。しかしながらこの場合、この種類の固定の可能性は、外部補綴的
治療を行なう場合に初めて使用される。勿論、末端側部に設けられた接続装置は
、上述した網目構造体を基礎として、顕著な二次的固定に関連して専ら使用され
る。すなわち、長骨構造体内のアダプタの有効寿命は非常に長く、このため、外
的磨耗または部品が駄目になる現象が現われ、その結果接続点における外部補綴
用標準部品の正確な取り付け状態が最早、保証されなくなる。
【００１０】 更なる好ましい改良によれば、幹部分には長骨断端の前縁の上に取り付けるた
めに取り巻くフランジが設けられる。このようにして、幹部分を長骨断端内に埋
め込んだ後で、十分な骨粒子が幹部分の開放メッシュの三次元的網目構造体内に
成長する前に、アダプタに直接的に負荷を加えることができる。このため、フラ
ンジは治癒段階の間、作用する力による負荷の主要な逃がし部分となり且つ長骨
の断端の皮質内に作用する力を案内する働きをする。
【００１１】 好ましくは、その後、接続装置が身体の外に位置するようになる迄、長骨断端
の端部から更に身体組織を通って伸びる中間の部分を末端の接続装置の方向に向
けて幹部分と接続する。その後、都合良く密封部分が中間の部分が患者の身体か
ら現れる点にてその中間の部分の上に位置する。この密封部分は、肢断端におけ
る破断点を保護し且つ破断点をより容易に無菌状態に保つことに役立つ。そのよ
うな障壁は、肢断端内に進行する可能性のあるひどい炎症を防止するために形成
される。
【００１２】 上述したアダプタは更に都合良く発展させて、接続装置が円錐体と円錐形のク
ランプ止め座部にてその円錐体の上に位置し、標準部分をクランプ止めすること
のできるアダプタスリーブとから成るようにすることができる。時間の経過に伴
って、標準部分の接続部の磨耗が増すか、または部品が駄目になったとき、この
アダプタスリーブは交換可能であるという利点を有する。このように、アダプタ
スリーブはその全体を交換することが可能である。
【００１３】 また、アダプタスリーブが接続装置の円錐体上に位置する状態は、アダプタス
リーブを通して締め付けねじを円錐体のねじ穴にねじ止めすることにより更に確
実にすることができる。
【００１４】 １つの実施の形態によれば、アダプタの中間の部品はそれ自身の長さを調節す
ることができる。このことは、本発明に従ったアダプタを１つまたは幾つかの寸
法のものだけを在庫しておけばよいという利点がある。このことは、コストを著
しく削減することになる。
【００１５】 これと代替的に、中間の部分を安定性を増す中実な金属から成るようにしても
よい。勿論、この場合は長さを調節することは出来ないので、コストは増すこと
になろう。
【００１６】 本発明を単一の図面に従って実施形態により詳細に説明する。
【００１７】 該図面は、大腿部断端３を有する腿断端１４の概略図的な断面図である。アダ
プタ１の幹部分４は管状の大腿部断端３に導入されている。幹部分４の表面は、
この場合、開放メッシュの三次元的網目構造体５によって占められおり、この三
次元的な網目構造体を通じて骨小柱が成長することができ、このため、幹部分４
は、一定の治癒段階の後、基質流を含む自然の骨の擬似的構成要素となる。
【００１８】 幹部分４は、長骨断端３の前縁１６に締結された取り巻くフランジ７により末
端側部に位置している。このことは、埋め込んだ直後のアダプタ１の主要な負荷
の支持能力を保証する一方で、皮質内に直接作用する負荷を少なくとも部分的に
案内することにつながる。
【００１９】 この実施の形態において、中実な金属片で出来ている中間の部分８は、幹部分
４と接続され、または場合によっては、フランジ７と接続されている。この中間
の部分８は、末端側にて肢の断端１４から出ることができるように十分に大きく
形成されている。この実施の形態において、この出る箇所は、中間の部分８に取
り巻く密封部分９にて密封される。
【００２０】 中間の部分８は、接続装置６の円錐体１０にて終結する。その円錐形のクラン
プ止め座部が締め付けねじ１２により円錐体１０に固着されたアダプタスリーブ
１１は、円錐形のクランプ止め配置状態にて円錐体１０上に位置し、この締め付
けねじは、円錐体１０のねじ付き穴１３内にアダプタスリーブ１１を通じてねじ
込まれている。
【００２１】 該補綴用標準部分２が締め付けねじ１５により接続される接続部分１６は、ア
ダプタスリーブ１１の上で上下逆さにされる。
【００２２】 磨耗する時間の経過に伴い、磨耗現象が生じるか、またはスリーブの部分が駄
目にれば、締め付けねじ１２をねじ付き穴１３からねじを外し、アダプタスリー
ブ１１を円錐体１０から除去し、そのアダプタスリーブを新たなアダプタスリー
ブ１１と交換することによりアダプタスリーブ１１を容易に交換することができ
る。次に、外部補綴用標準部品を再度取り付けることができる。

【０００１】 本発明は、外部補綴用標準部品のアダプタに関する。
【０００２】 肢を切断した患者の外部補綴処置として色々な方法が公知である。
【０００３】 例えば、腿の断端領域内において、腿の断端の寸法及び形状に適合し得るよう
にされた外部補綴軸、例えば補綴軸を専門に取り扱うことが考えられる。次に、
所定の場合、人工外部補綴膝関節、ふくらはぎ、及び足をこの補綴軸と接続する
。
【０００４】 しかしながら、例えば更に走る場合に、より自然の知覚機能を患者に与えるこ
とができるように残存する長骨の断端の端部に固定される装置が存在する。これ
は骨知覚（ｏｓｔｅｏｐｅｒｃｅｐｔｉｏｎ）という名にて公知である。例えば
、長骨と固定するためにねじを有するアダプタ端部を残存する長骨にねじ込む。
アダプタは、パイプ片に似ており、該アダプタは、最終的に、断端の端部の組織
から外に出る。その後、例えば、人工膝関節のような外部補綴用標準部品をこの
アダプタに接続する。
【０００５】 前述の型の公知のアダプタの場合、長骨断端部内にアダプタを固定する際の安
定性、即ち標準部品の接続が問題となる。
【０００６】 米国特許第Ａ−３，９４７，８９７号では、中間骨髄空隙に挿入され、端部に
外部補綴標準部品のための接続装置を具備するアダプタが記述されている。大腿
骨の中間骨髄空隙内の取付けシートは骨接着剤、例えばメチルメタクリル酸エス
テルを用いて固定される。標準部品と端部との接続はある種のロック装置で行わ
れる。
【０００７】 骨接着剤による接合およびロック装置による接合はいずれも問題である。骨接
着剤はある時間経過後、接着力が弱くなる。また、ロック装置は外部補綴用標準
部品の恒久取付けシートに対して耐久性がない。
【０００８】 この背景を踏まえ、望ましい長期固定挙動を有し、また、例えば摩耗が進展し
た場合、外部補綴用標準部品を別の部品と交換することを容易に可能にするアダ
プタを提供することが、本発明の目的である。
【０００９】 この目的は請求項１の特徴をもつアダプタによって解決される。従って、アダ
プタは次のように製作されることが推奨される。即ち長骨断端部にアダプタの基
端側幹部分が挿入され、挿入幹部分は少なくとも部分的に開放メッシュ三次元ス
ペース網目構造体によって覆われ、そしてもう一方の端部には外部補綴用標準部
品のための接続装置が具備される。なお、接続装置は円錐体とその外側にあるア
ダプタスリーブとから成る。アダプタスリーブの外側には円錐状の固定用シート
があり、外部補綴用標準部品はこの固定用シートによって取付けられる。
【００１０】 相互接続部とも呼ばれる開放メッシュ三次元スペース網目構造体は、幹部分が
基質の流れに関して比較的短時間に長骨と一体化し、きわめて安定した二次固定
をもたらすように、最終治癒過程において幹部分の中、貫通部、背後、周りなど
に天然の骨材料が成長することを可能にする。開放メッシュ三次元スペース網目
構造体中への骨材料の成長は、純粋な体内補綴法により公知となった。例として
、ここで、ドイツ国特許第４１ ０６ ９７１号に言及するが、（ドイツ国特許
第４１ ０６ ９７１号において）最初は、外部補綴治療の分野での固定の可能
性が紹介されている。続いて、既述のスペース網目構造のお陰で強化された二次
固定部に自由に接続された末端側に取付けられた接続装置を見ることが出来る。
特に、長骨断端部におけるアダプタの寿命は非常に長く、このため外部補綴用標
準部品の正確な取付け用シートを確保できなくなった外的摩耗や破損現象が出現
する可能性がある。
【００１１】 アダプタスリーブは、例えば標準部品の接続部が時間の経過によって摩耗が進
んだり、破損したりした場合、取り替え可能であるという利点を有する。アダプ
タスリーブ自体も丸ごと取り替えることが可能である。また、接続装置の円錐体
の外側にあるアダプタスリーブのシートは、テンション用ネジを、アダプタスリ
ーブを通して円錐体にあるネジ穴にねじ込むことによって固定できる。
【００１２】 更なる好ましい改良によれば、長骨断端部の前縁に、取付けのためのフランジ
が設けられている。この方法によって、アダプタは長骨断端部内にに幹部分を埋
め込んだ直後、特に十分な骨材料（＊）が開放メッシュ三次元スペース網目構造
内に成長する前に、力を加えて取付けられる。これにより、フランジは最終治癒
段階中に発生する力の主要荷重を受け止め、発生した力を長骨断端部の皮質の方
向に向けるのに役に立つ。 ［（＊）最善の推定である。ドイツ語のＫｎｏｃｈｅｎｔｒａｐｅｋｅｌは意味
不明であるが、Ｋｎｏｃｈｅｎは“骨”の意味である。］
【００１３】 好ましくは、幹部分は、末端接続装置の方向に向けて、中間部分に接続される
。中間部分は更に、長骨断端部の終端から人体組織を貫通して、接続装置の部分
が身体の外側に位置するまで延びる。中間部分が患者の身体の外に出る個所には
、都合よくその個所にシール部分がある。このシール部分は肢断端における貫通
部分を保護し、そしてその部分を無菌状態に保ちやすくする。このため、障壁と
なっている部分は、肢断端中に深く浸透する可能性のある過度の炎症を抑えるよ
うな構造になっている。
【００１４】 上述したアダプタは更に都合良く発展させて、接続装置が円錐体と円錐形のク
ランプ止め座部にてその円錐体の上に位置し、標準部分をクランプ止めすること
のできるアダプタスリーブとから成るようにすることができる。時間の経過に伴
って、標準部分の接続部の磨耗が増すか、または部品が駄目になったとき、この
アダプタスリーブは交換可能であるという利点を有する。このように、アダプタ
スリーブはその全体を交換することが可能である。
【００１５】 また、アダプタスリーブが接続装置の円錐体上に位置する状態は、アダプタス
リーブを通して締め付けねじを円錐体のねじ穴にねじ止めすることにより更に確
実にすることができる。
【００１６】 本発明の態様によれば、アダプタの中間部分の長さは調整可能である。このこ
とは、本発明により製造もしくは在庫品として保管貯蔵されるアダプタは、僅か
１種類か、あるいは数種類の大きさのものを必要とするだけである。これは、大
幅なコスト削減につながる。
【００１７】 別の態様例では、中間部分は１本の金属棒から成り立っており、安定性は増し
ているが、長さの調節は不可能であり、そのためコストの上昇は考慮しておかな
ければならない。
【００１８】 本発明を単一図面に従って、実施の形態の手段により具体例により説明する。
【００１９】 該図は大腿骨断端部３を内蔵した腿断端部１４の概略図的な断面図である。ア
ダプタ１の幹部分４は長骨大腿骨断端部３中に挿入される。幹部分４の表面は、
開放メッシュ三次元スペース網目構造５で覆われている。骨材料はスペース網目
構造を通して成長し、基質流に関する限り、ある治癒段階後に幹部分４はほとん
ど自然骨の１部になる。
【００２０】 幹部分４はフランジ７をもった末端部で閉じられている。フランジ７は長骨断
端部３の前面端１６に対して押し付けられている。一方では、このことは、アダ
プタ１は埋め込み直後に力を受けることがあり得ることを示している。
【００２１】 この実施の形態において、中実な金属片で出来ている中間の部分８は、幹部分
４と接続され、または場合によっては、フランジ７と接続されている。この中間
の部分８は、末端側にて肢の断端１４から出ることができるように十分に大きく
形成されている。この実施の形態において、この出る箇所は、中間の部分８に取
り巻く密封部分９にて密封される。
【００２２】 中間の部分８は、接続装置６の円錐体１０にて終結する。その円錐形のクラン
プ止め座部が締め付けねじ１２により円錐体１０に固着されたアダプタスリーブ
１１は、円錐形のクランプ止め配置状態にて円錐体１０上に位置し、この締め付
けねじは、円錐体１０のねじ付き穴１３内にアダプタスリーブ１１を通じてねじ
込まれている。
【００２３】 該補綴用標準部分２が締め付けねじ１５により接続される接続部分１６は、ア
ダプタスリーブ１１の上で上下逆さにされる。
【００２４】 磨耗する時間の経過に伴い、磨耗現象が生じるか、またはスリーブの部分が駄
目にれば、締め付けねじ１２をねじ付き穴１３からねじを外し、アダプタスリー
ブ１１を円錐体１０から除去し、そのアダプタスリーブを新たなアダプタスリー
ブ１１と交換することによりアダプタスリーブ１１を容易に交換することができ
る。次に、外部補綴用標準部品を再度取り付けることができる。

  1. 【請求項１】 基端側の幹部分（４）と共に、長骨の断端（３）内に取り付
    けることのできる、外部補綴用標準部品（２）のアダプタ（１）において、幹部
    分（４）が、少なくとも部分的に、開放メッシュの三次元的網目構造体（５）に
    より覆われ、その末端に、外部補綴用標準部品（２）の接続装置（６）が設けら
    れることを特徴とするアダプタ。
  2. 【請求項２】 幹部分には、長骨断端（３）の前端縁（１６）の上に取り付
    けるための取り巻くフランジ（７）が設けられることを特徴とする請求項１記載
    のアダプタ。
  3. 【請求項３】 中間の部分（８）が、接続装置（６）の方向に向けて幹部分
    （２）に接続されることを特徴とする請求項１または２記載のアダプタ。
  4. 【請求項４】 密封部分（９）が、中間の部分（８）が身体から貫入する箇
    所にて中間の部分（８）上に位置することを特徴とする請求項１乃至３のいずれ
    か１項記載のアダプタ。
  5. 【請求項５】 接続装置（６）が、円錐体（１０）と、円錐形のクランプ止
    め座部にて該円錐体上に位置するアダプタスリーブ（１１）とから成り、該アダ
    プタスリーブ上には、該補綴用標準部品（２）を締め付けることのできる接続部
    分（１６）が位置することを特徴とする請求項１乃至４のいずれか１項記載のア
    ダプタ。
  6. 【請求項６】 アダプタスリーブ（１１）の円錐形のクランプ止め座部が、
    アダプタスリーブ（１１）を通じて円錐体（１０）のねじ付き穴（１３）内にね
    じ込まれた締め付けねじ（１２）により更に固着されることを特徴とする請求項
    ５記載のアダプタ。
  7. 【請求項７】 中間の部分（８）の長さが、調節可能であることを特徴とす
    る請求項１乃至６のいずれか１項記載のアダプタ。
  8. 【請求項８】 中間の部分（８）が、中実な金属から成ることを特徴とする
    請求項１乃至７のいずれか１項記載のアダプタ。
