【技術分野】本発明は、手術縫合糸のための固定具に関

【従来技術及びその課題】骨に形成された開口において
縫合糸を固定するために使用される縫合糸固定具は、関
節再構成手術と関節鏡外科技術のために重要である。そ
のような縫合糸固定具は、例えば、ひざ、肩及びひじ再
構成及び修復手術において靱帯又は腱を骨に固定するた
めに使用される。
【０００３】骨固定具の重要な属性は、挿入が容易で確
実な固定ができることである。骨固定具はまた、単純で
信頼性があるべきである。骨固定具は、骨固定具を形成
するために使用される材料により、生物吸収性又は非生
物吸収性である。いずれも、手術の形式と外科医の選択
により使用される。

する別の特許は、Ｒｏｓｅｎｂｅｒｇへの米国特許第４

【課題を解決するための手段】本発明は、骨において形
成された穴に縫合糸を固定するために、縫合糸とともに
使用されることを意図された骨固定具に関する。骨固定
具は、本体の一方の端部において尖端部を有する伸長は
め輪形状本体である。第１及び第２溝が、間隔をあけた
位置において伸長本体を通って縦に延びている。縫合糸
収容開口が、尖端部において設けられる。少なくとも一

される。縫合糸材料８０の糸の両端部は、骨固定具７０
が挿入された時、骨におけるボア穴から延びている。本
体７２の比較的鋭い後縁８６は、遠位端部７７とは反対
側の本体７２の端部に位置する。後縁８６は、ボア穴の
側部に係合する突起又は隆起を形成する。文字Ｔによっ
て指定された張力が糸８０の両端部にかけられた時、第
１及び第２壁８３と８４の後縁８６は、骨におけるボア
穴の側部にくい込み、そして互いに外側に膨張させられ
、骨固定具７０をボア穴において収容させる。
【００２５】骨固定具を形成するために使用された材料
は、生物吸収性材料又は非吸収性永久材料のいずれかか
ら作製される。好ましい吸収性材料は、ポリグリコール
酸、多乳酸又はトリメチレンカーボネートコポリマーを
含む。好ましい非吸収性材料は、アセタールホモポリマ
ー又はコポリマー、ポリエチレン、ポリプロピレン、ポ
リエステル、及びそれらのコポリマーを含む。Ｄｅｌｒ
ｉｎ　　５０ＳＡは、骨固定具のために一つの好ましい
材料である。縫合糸材料は、Ｄａｖｉｓ＆Ｇｅｃｋの商
標であるＴｉｃｒｏｎ又はＤｅｘｏｎブランド縫合糸の
如く、従来の形式の縫合糸材料である。
【００２６】発明の骨固定具１０で使用される挿入装置
１００が、図８に示される。装置１００は、ハンドル部
材１０２と伸長軸１０４を有する。軸１０４は、固定具
の内側空洞１３に嵌入するように順応されたテーパ端部
１０６を有する。
【００２７】固定具１０が軸１０４の端部１０６に位置
付けられた時、縫合糸２２の２つの端部２９、３０は、
ハンドル１０２における柱１０８と１１０の回りに確実
に締め付けられる。柱１０８と１１０の頭部の下に位置
付けられたゴム製Ｏリング１１２と１１４は、柱の回り
に巻かれた後に縫合糸を位置にしっかりと保持する。骨
固定具と挿入装置の完成した組立体が、図９に示される
。
【００２８】図１０は、固定具及び挿入組立体を示し、
深さ部材又は管１１６をその上に位置付けている。深さ
部材１１６は、伸長軸１０４の端部にわたって位置付け
られ、かつ軸に半田付けされた止め又はカラー部材１１
８において位置に保持される。深さ部材１１６は、（Ｄ
ｅｌｒｉｎグレード５５０の如く）プラスチック材料か
ら作製され、そして止め部材におけるピン１２２に食い
入る一対の溝１２０を有し、挿入装置における位置に保
持する。
【００２９】図１１と図１２に示された如く、深さ部材
１１６の端部１２４は、挿入装置１００の挿入移動を制
限し、そしてこのため、骨固定具が骨３８において位置
付けられる深さを制限する。これは、固定具が、あまり
深く挿入されないことを保証する。深さ部材はまた、駆
動装置及び固定具組立体の使用中、縫合糸を保護する。 深さ部材１１６は、種々の長さにおいて設けられ、外科
医による所望に応じて皮質表面の下に固定具の挿入深さ
を設ける。
【００３０】骨固定具１０の挿入は、直接の圧力により
達成されるが、好ましいより制御された方法は、固定具
をゆっくりと進める小槌により挿入装置１００の端部を
軽くたたくことである。いったん皮質骨を通過し、ある
形式の深さ部材又は装置がないならば、ずっと大きな圧
力により、固定具は網状の骨にずっと深く駆動される。
【００３１】いったん固定具１２が、図１２に示された
如く部位に位置付けられたならば、挿入装置１００が除
去される。それから延展装置１３０が、固定具をしっか
りとセットし位置させるために使用される。図１３に示
された如く、装置１３０は、ハンドル１３２と消息子１
３４を有する。消息子１３４は、丸くわずかにテーパし
た端部１３６を有し、そして挿入装置における伸長軸１
０４よりも直径が大きい。
【００３２】固定具１０をセットするために、延展装置
１３０は、固定具１０の中央に置かれ（図１４参照）、
そして縫合糸２２の２つの端部が同時に引かれる。これ
は壁２７、２８を開放するために役立ち、固定具を骨に
おいてしっかりと位置させる。
【００３３】固定具１０をセットする時、固定具から引
き出されない如く、縫合糸の両端部をしっかりと保持又
は締め付けることが重要である。この点における一つの
オプションは、２つの縫合糸端部を一緒に結ぶことであ
り、結び目１４０（図１５）が骨表面の下にあることを
保証する。
【００３４】いったん固定具が部位に据え付けられ位置
付けられたならば、靱帯１５０又は他の軟組織が、固定
具部位上の位置にもたらされる。それから縫合糸は、靱
帯を通ってしっかりと結ばれ、骨に隣接した部位にしっ
かりと保持する。これは、図１５に示される。このよう
にして、軟組織と骨は、時間の経過により、再付着され
る。
【００３５】発明の別の実施態様が図１６に示される。 この実施態様において、縫合糸２２を取り付けた固定具
１０’が、中空挿入装置１６０を通って挿入される。装
置１６０は、ハンドル１６２と挿入管１６４を有し、そ
して骨固定具１０’は管内に滑りながら嵌入する大きさ
である。固定具１０’は、頭部又はハンドル１７２と伸
長押し棒１７４を有する押し部材１７０によって位置に
据え付けられる。押し棒の端部１７６は、（挿入装置１
００の端部１０６に類似する）固定具１０’の中央空洞
内にはまるように先鋭にされるか、あるいは図示の如く
、平たんか又は拡大される。
【００３６】「中空管」挿入方法は、特に、約１６分の
１インチの直径と１６分の３インチの長さの次元におけ
る小固定具のために有益である。また、固定具１０’の
挿入深さを測定するために、マーキング又はスケール（
不図示）が、頭部又はハンドルに隣接して押し棒１７４
において設けられる。
【００３７】いったん骨固定具１０’が骨３８における
開口又はボア３６に挿入されたならば、延展装置（上記
の延展装置１３０に類似）が、固定具を部位にしっかり
と位置付けるために使用される。その後、固定具は、固
定具１０に関して上記と同一の方法と同一の目的で使用
される。
【００３８】骨固定具の別の実施態様が、図１７に示さ
れる。骨固定具１８０は、比較的平たん前端部１８２と
、後端部において外側に広がる隆起１８４を有する。 前端部１８２は、骨におけるドリル穴への固定具の進入
に役立つために４５°の面取り１８６を有する。後端部
は、外側に広がる環状隆起１８４をたわませ、挿入中放
射状内側に移動させる凹部１８８を有する。延展部材に
よる膨張とともに、固定具材料の「記憶」により、隆起
１８４は外側に押しやられ、固定具を位置において確実
に保持する。開口１９０と対向した溝１９２、１９４は
、縫合糸にために固定具を通って設けられる。
【００３９】本発明の好ましい実施態様の上記の説明は
、発明の３つの好ましい形式を例示することを意図して
いる。本発明の他の修正と強化が上記の説明に基づいて
明らかになることが予期される。本発明の範囲は、特許
請求の範囲によって示され、そして好ましい実施態様の
上記の説明によって制限されない。

に延びて、ボア穴の壁において収容されるように適合さ
れた縁を規定する少なくとも一つの隆起とを有する骨固
定具。
【００４２】２．該尖端部が、部分的に円すいであり、
そして丸い端部において終端する上記１に記載の骨固定
具。
【００４３】３．本体が、実質的に円筒形部分を含み、
第１及び第２溝が、正反対に対向し、そして縫合糸収容
開口が、第１溝が形成された本体の側から第２溝が形成
された本体の側に延びている上記１に記載の骨固定具。
【００４４】４．複数の隆起が設けられる上記１に記載
の骨固定具。
【００４５】５．該隆起が、一般に円形であり、そして
円筒形部分の周囲に延びており、そして第１及び第２溝
によって中断される上記１に記載の骨固定具。
【００４６】６．該隆起が、本体の遠位端部に近い方に
最小直径を有する円すい台状前面と、固定具をボア穴に
おいてロックするためにボア穴の壁にくい込む縁を前面
により形成する遠位端部から向きがそらされた放射状に

る穴への該骨固定具の挿入に役立つように位置付けられ
る中空挿入装置と組み合わせた上記１０に記載の骨固定
具と押し部材。
【００５３】１３．該本体が骨における穴に挿入された
後、該弾性壁を骨に対して外側に駆り立てるための延展
部材と組み合わせた上記７に記載の骨固定具。

  1. 【請求項１】　　細長い本体を具備する骨におけるボア
    穴に縫合糸を固定するための骨固定具であり、本体の遠
    位端部における尖端部と、本体における間隔を設けた位
    置において縦に延びている第１及び第２溝と、尖端部を
    通って延びている縫合糸収容開口と、本体の外側から外
    方に延びて、ボア穴の壁において収容されるように適合
    された縁を規定する少なくとも一つの隆起とを有する骨
    固定具。
