Description
(1) 逆行性血流閉塞物フラッシングデバイスであって、
近位端及び反対の遠位端を有するカテーテル本体であって、少なくとも1つの内腔が前記カテーテル本体内で長手方向に画定されている、カテーテル本体と、
前記カテーテル本体の前記遠位端に近接して配置され、前記カテーテル本体から半径方向外側に延びる閉塞構成要素であって、折りたたみ状態から拡張状態に移行するように作動可能であり、前記カテーテル本体の縦軸から測定すると、前記折りたたみ状態の間の直径と比較して、前記拡張状態においてより拡大した直径を有する、閉塞構成要素と、
前記カテーテル本体の少なくとも1つの内腔を通して送達可能であり、前記カテーテル本体内に画定されたポートから出るフラッシング流体であって、前記ポートは前記閉塞構成要素の近位に配置されている、フラッシング流体と、
を含む、デバイス。
(2) 前記ポートが、前記カテーテル本体の側面に画定されており、前記閉塞構成要素が膨張式バルーンであり、前記カテーテル本体が、前記カテーテル本体を通して長手方向に画定された複数の内腔を有する、実施態様1に記載のデバイス。
(3) 前記複数の内腔のうちの1つが、前記フラッシング流体を送達するためのフラッシング内腔であり、前記複数の内腔のうちの別のものが、膨張流体を送達して、前記膨張式バルーンを膨張させて、前記折りたたみ状態から前記拡張状態に移行させる、実施態様2に記載のデバイス。
(4) 前記フラッシング流体は生理食塩水、対比流体、又は他の生体適合性液体であり、前記膨張流体は空気である、実施態様3に記載のデバイス。
(5) 前記複数の内腔のうちの1つは、前記カテーテル本体が上をたどるワイヤを受けて通すためのワイヤ内腔であり、前記ワイヤ内腔は、前記カテーテル本体を通して長手方向に、かつ前記閉塞構成要素を越えて遠位まで延びている、実施態様2に記載のデバイス。
(7) 前記フラップは、前記ポートから出る前記フラッシング流体により自動的に展開される、実施態様6に記載のデバイス。
(8) 折りたたみ状態と拡張状態との間で移行可能な血塊捕捉機械的構成要素を更に含み、前記拡張状態において、前記血塊捕捉機械的構成要素は、前記閉塞構成要素の近位の前記カテーテル本体の遠位セクションから半径方向外側に延びる、実施態様1に記載のデバイス。
(9) 前記血塊捕捉機械的構成要素は、少なくとも1つの突出部又はアームである、実施態様8に記載のデバイス。
(10) 標的妨害物が中に配置された血管を再疎通する間の、逆行性血流閉塞物フラッシングデバイスの使用方法であって、前記デバイスは、近位端及び反対の遠位端を有するカテーテル本体であって、少なくとも1つの内腔が前記カテーテル本体内で長手方向に画定されている、カテーテル本体、を含み、前記デバイスは、前記カテーテル本体の前記遠位端に近接して配置された閉塞構成要素であって、折りたたみ状態から拡張状態に移行するように作動可能であり、前記カテーテル本体の縦軸から測定すると、前記折りたたみ状態の間の直径と比較して、前記拡張状態においてより拡大した直径を有する、閉塞構成要素と、前記カテーテル本体の前記少なくとも1つの内腔を通して送達可能であり、前記カテーテル本体内に画定されたポートから出るフラッシング流体であって、前記ポートは前記閉塞構成要素の近位に配置されている、フラッシング流体と、を更に含み、
前記方法が、
前記閉塞構成要素が前記折りたたみ状態にある間に、前記閉塞構成要素が前記標的妨害物の遠位に配置されるまで、前記標的妨害物を血管内で遠位に横断する前記逆行性血流閉塞物フラッシングデバイスを導入する工程と、
前記標的妨害物の遠位に適切に配置されると、前記閉塞構成要素を作動させて、前記折りたたみ状態から前記拡張状態に移行させる工程であって、前記拡張状態にある間、前記拡大した直径が、前記血管の内壁との封止を形成し、順行性血流が前記閉塞構成要素を遠位に越えることを防止する、工程と、
前記拡張した閉塞構成要素により一端にて結合され、前記標的妨害物により反対端にて結合された前記血管の領域において、前記フラッシング流体を前記カテーテル本体の前記ポートから前記血管の前記領域に分注することにより、逆行性血流を確立する工程と、を含む、方法。
(12) 前記圧力は最初、圧力差が少なくとも約25%となり、前記標的妨害物を前記血管の内壁から移動させるまで、前記標的妨害物の前記遠位面にて増加され、前記標的妨害物が移動すると、その後、前記圧力差は少なくとも約5%となり、前記標的妨害物を前記血管を通して遠位に推進する、実施態様11に記載の方法。
(13) 前記増加させる工程は、前記カテーテル本体の前記ポートから、前記閉塞構成要素により前記一端にて結合され、前記標的妨害物により前記反対端にて結合された前記血管の前記領域内に、前記フラッシング流体を分注することを含む、実施態様11に記載の方法。
(14) 前記カテーテル本体は、前記カテーテル本体を通して長手方向に画定された複数の内腔を有し、前記複数の内腔のうちの1つが、前記フラッシング流体を送達するためのフラッシング内腔である、実施態様10に記載の方法。
(15) 前記ポートが、前記カテーテル本体の側面に画定され、前記閉塞構成要素が膨張式バルーンであり、前記複数の内腔のうちの別のものが、膨張流体を送達して、前記膨張式バルーンを前記折りたたみ状態から前記拡張状態に膨張させる、実施態様14に記載の方法。
(17) 前記逆行性血流を確立することにより、前記標的妨害物が前記血管から近位に逆行性方向に押される、実施態様10に記載の方法。
(18) 前記血管は、前記標的妨害物に対してすぐ遠位の分岐部を有し、前記分岐部は主な枝部及び側枝部を含み、前記逆行性血流閉塞物フラッシングデバイスの2つのうちの1つを、前記主な枝部及び前記側枝部のそれぞれにおいて、送達する、実施態様10に記載の方法。
(19) 前記閉塞構成要素が前記血管を通して近位に引き上げられる間、前記閉塞構成要素の近位に配置された血塊捕捉機械的構成要素を作動させて、折りたたみ状態から、前記カテーテル本体から半径方向外側に延びる拡張状態に移行させ、前記拡張状態の間、前記血塊捕捉機械的構成要素は、前記標的妨害物が、前記血管を通して展開された前記血塊捕捉機械的構成要素を越えて遠位まで移動することを防止する、実施態様10に記載の方法。
(20) 展開したとき、前記血塊捕捉機械的構成要素の終端した自由端が前記血管の内壁と物理的に接触している、実施態様19に記載の方法。
Claims (6)
- 逆行性血流閉塞物フラッシングデバイスであって、
近位端及び反対の遠位端を有するカテーテル本体であって、少なくとも1つの内腔が前記カテーテル本体内で長手方向に画定されている、カテーテル本体と、
前記カテーテル本体の前記遠位端に近接して配置され、前記カテーテル本体から半径方向外側に延びる閉塞構成要素であって、折りたたみ状態から拡張状態に移行するように作動可能であり、前記カテーテル本体の縦軸から測定すると、前記折りたたみ状態の間の直径と比較して、前記拡張状態においてより拡大した直径を有する、閉塞構成要素と、
前記カテーテル本体の少なくとも1つの内腔を通して送達可能であり、前記カテーテル本体内に画定されたポートから出るフラッシング流体であって、前記ポートは前記閉塞構成要素の近位に配置されている、フラッシング流体と、
折りたたみ状態と拡張状態との間で移行可能な血塊捕捉機械的構成要素と、
を含み、
前記血塊捕捉機械的構成要素の前記拡張状態において、前記血塊捕捉機械的構成要素は、前記閉塞構成要素の近位の前記カテーテル本体の遠位セクションから半径方向外側に延び、
前記血塊捕捉機械的構成要素は、少なくとも1つの突出部又はアームである、デバイス。 - 前記ポートが、前記カテーテル本体の側面に画定されており、前記閉塞構成要素が膨張式バルーンであり、前記カテーテル本体が、前記カテーテル本体を通して長手方向に画定された複数の内腔を有する、請求項1に記載のデバイス。
- 前記複数の内腔のうちの1つが、前記フラッシング流体を送達するためのフラッシング内腔であり、前記複数の内腔のうちの別のものが、膨張流体を送達して、前記膨張式バルーンを膨張させて、前記折りたたみ状態から前記拡張状態に移行させる、請求項2に記載のデバイス。
- 前記フラッシング流体は生理食塩水、又は他の生体適合性液体であり、前記膨張流体は空気である、請求項3に記載のデバイス。
- 前記複数の内腔のうちの1つは、前記カテーテル本体が上をたどるワイヤを受けて通すためのワイヤ内腔であり、前記ワイヤ内腔は、前記カテーテル本体を通して長手方向に、かつ前記閉塞構成要素を越えて遠位まで延びている、請求項2に記載のデバイス。
- 前記ポートは、前記カテーテル本体の遠位端にて画定されており、前記閉塞構成要素は、前記閉塞構成要素の前記拡張状態において前記カテーテル本体の先端から伸長した状態となるパラシュートであり、前記パラシュートは、前記伸長した状態で半径方向外側に拡張する、請求項1に記載のデバイス。
