良性前立腺肥大症(BPH)は、前立腺間質および上皮細胞の過形成を伴い、前立腺の移行領域に大きな離散結節が形成される。十分に大きければ、小結節は前立腺尿道に衝突し、膀胱からの尿の流れに対する抵抗を増加させ、患者に不快感をもたらす。尿流に対する耐性は、排尿中に膀胱がより強く働かなければならず、進行性の肥大、不安定性、および、膀胱筋の衰弱をもたらす。アルファブロッカーや5-レダクターゼ阻害薬などの薬剤、クリーン断続的自己カテーテル法、経尿道的マイクロ波温熱療法や、経尿道的ニードルアブレーションなどの手術や低侵襲治療など、BPHには様々な治療法がある。さらなる治療オプションは、前立腺尿道内にステントを埋め込むことである。そのようなステントの1つが特許文献1に記載されており、これは、2つのリングを同一平面関係に維持する3つのストラットによって接続された2つのリングを含む。使用時には、2つのリングのうち大きな方が膀胱内に配置され、小さな方のリングが前立腺尿道に配置される。前立腺ステントは、BPHの症状のための即時救済を提供することができるが、副作用が高い割合であるため好まれていない。それらは、通常、心臓血管または末梢血管疾患で使用される伝統的なステントに似た、長い円筒形のステントである。それらは、配備位置から移動して膀胱または膜状尿道(患者の12.5%まで)、尿道の封じ込めとブロック(最大27.5%)、失禁(最大3%)、および、痛みの原因となる。前立腺ステントに対する深刻な炎症反応の症例が報告されている。トータルで、これらが組み合わされた副作用は、歴史的に前立腺ステントの8~47%が除去される結果となった。さらに、これらの前立腺ステントは、前立腺尿道の独特の特徴に適応する設計を有していない。
概して、本発明は、一実施形態では、良性前立腺肥大症(BPH)の治療に適した弾性変形可能な拡張器を提供する。拡張器は、一般に、隣接する罹患した組織のために収縮した標的位置で体内管腔内に埋め込むように構成された単一の細長い波状リングの形態をとる。波状リングは、典型的には体内管腔の壁に陥入するように構成された少なくとも2つの遠位プロングおよび少なくとも2つの近位プロングを有する。拡張器は、典型的には、体内管腔を通って標的位置に経腔的に送達するのに適した収縮された配向と、標的位置で体内管腔のその場での膨張をもたらすように構成された拡張(展開)配向との間で半径方向に弾性的に変形可能である。細長い波状リング形状は、ステントと体内管腔壁との間の接触面積を最小限にし、潜在的な付着の程度を低減する。さらに、波状リングのストラットは、細長い波状のリング形状のため、実質的に長手方向に(円周方向とは反対に)延在し、それにより、流体の流れは、露出した表面積の大部分について、ストラットに沿ってストラットに対して直角とは逆になる。これはさらに、付着の発生率を減少させる。ストラットの実質的に長手方向の性質はまた、容易な除去を容易にする。さらに、拡張器の波状リング構成は、使用中に拡張器の差動屈曲を可能にし、したがって、鏡の形状を解剖学的形状にし、体内管腔の壁の形状に接着し、それと一緒に移動する。特に、本発明の拡張器は、精丘(verumontanum)および射精管を破壊せず、その3つの近位突起部は、三角形状の遠位尿道に解剖学的に座って移動を起こしにくい(除去を容易にするだけでなく)。波状のリング構成は、尿道の正しい解剖学的領域に拡張が起こることを保証する(特に、前立腺の移行領域に押し付けられる)。

典型的には、精丘を阻害することなく、膀胱頸部と外尿道括約筋の間の前立腺尿道
内に適合するように構成され、
典型的には前立腺尿道の壁に解剖学的に適合するように構成され、および、
典型的には複数の長手方向ストラットを備え、典型的には円周ストラットを備えな
い。
半径方向に収縮した向きの拡張器を、体内管腔を通して、該体内管腔によって特徴
付けられる該体内管腔内の標的位置に送達するステップ、および、
標的位置における拡張器を体内管腔の壁に対して半径方向に拡張された向きに拡張
させることを可能にするステップであって、拡張された向きの拡張器が体内管腔の狭
窄部分のその場での膨張をもたらすステップを含む。
半径方向に収縮した向きの拡張器を、尿道を通して前立腺尿道内の標的位置まで膀
胱頸部と外尿道括約筋の間で送達するステップ、および、
標的位置における拡張器が拡張して前立腺尿道の壁に対して半径方向に拡張された
向きになることを可能にするステップを含む。
半径方向に収縮した向きの拡張器を、尿道を通して、膀胱頸部と外尿道括約筋の間
の前立腺尿道内の標的位置まで送達すること、および、
標的位置における拡張器が前立腺尿道の壁に向って半径方向に拡張した向きまで拡
張することを可能にすることであって、それにより、拡張された向きの拡張器が精丘
を阻害することなく前立腺尿道のその場での膨張をもたらし、膀胱頸部の機能を維持
することを含む。
本明細書中で使用されている場合、特に明記しない限り、以下の用語は、当該技術分野
において享受するより広い(またはより狭い)意味に加えて、以下の意味を有することが
意図される。

Claims (8)
- 良性前立腺肥大症(BPH)の治療のために埋め込み型生体適合性拡張器を前立腺尿道に送達する送達装置であって、前記拡張器は、それぞれの近位頂部を有するそれぞれの近位プロングを含む近位端と、それぞれの遠位頂部を有するそれぞれの遠位プロングを含む遠位端とを有する単一の細長い起伏のあるリングを備え、前記送達装置は、
送達チューブに作動可能に接続されたハンドルであって、前記送達チューブが前記ハンドルから離れた中空の遠位端を有し、前記拡張器が尿道を通じた経管腔送達に適した半径方向に収縮した構成で前記送達チューブに受け入れられる、ハンドルと、
前記ハンドルに作動可能に接続され、前記送達チューブの前記中空の遠位端から前記拡張器を排出するように動作可能であり、これにより前記拡張器は、前記尿道内で半径方向に拡張された構成を採用できる、排出要素であって、前記拡張器の前記近位端の内腔内へ延びるように構成された頭部を備え、前記頭部は、前記拡張器の前記近位頂部の少なくとも1つと係合するように構成されたジグを備え、前記ジグは、近位側に向かって拡径された円周肩部と、前記円周肩部の遠位側に配置された少なくとも1つの突起部とを備え、前記拡張器の前記近位頂部の少なくとも1つは、前記円周肩部と前記少なくとも1つの突起部との間に係合される、排出要素と、
前記送達チューブの前記中空の遠位端の内部に配置され、前記尿道への前記拡張器の配置中に、前記尿道及び前記拡張器の前記遠位頂部を画像化するために前記拡張器の前記内腔を通して突出する撮像装置と
を備える、送達装置。 - 前記排出要素は、前記拡張器を把持し、前記拡張器の近位方向への移動を達成するように構成される、請求項1に記載の送達装置。
- 前記ジグは、前記拡張器の全ての前記近位頂部に係合するように構成される、請求項1、又は、2に記載の送達装置。
- 前記排出要素の前記頭部は、三角形の断面を有する、請求項1~3のいずれか1項に記載の送達装置。
- 前記排出要素の前記頭部は、前記送達チューブ内に取り付けられたときに、前記拡張器の内腔内に嵌合するように構成された3つの面により確定される三角形の断面を有する遠位の頭部を含み、前記頭部の前記3つの面のそれぞれが前記送達チューブ内に前記拡張器を位置決めして、係合するためのジグを一緒に形成する突起部を含み、前記突起部のそれぞれは、前記拡張器の近位プロングの前記近位頂部に係合するように構成される、請求項4に記載送達装置。
- 前記撮像装置は、前記送達チューブの内腔内に、同心円状に配置される、請求項1~5のいずれか1項に記載の送達装置。
- 前記撮像装置は、前記送達装置の端部に隣接する尿道の一部を使用時に照らすように構成された光源を含む、請求項1~6のいずれか1項に記載の送達装置。
- 前記撮像装置は膀胱鏡である、請求項1~7のいずれか1項に記載の送達装置。
