Description

腫瘍モデルにおける併用治療
2つのマウス腫瘍モデル、CT26腫瘍モデルおよび4T1腫瘍モデルを使用して、C.novyi-NTの投与ありまたはなしでの腫瘍に対するCTLA-4およびPD-1抗体の効果を決定した。
Clostridium novyi-NTは抗腫瘍応答を誘導する
要約
Clostridium菌の種は、低酸素環境で成長する腫瘍細胞を溶解するその能力で知られる。ここで、Clostridium novyi(C.novyi-NT)の弱毒化された菌株は、腫瘍内注射後ラット同所性脳腫瘍モデルにおいて顕微鏡的に明確な腫瘍局在化応答を誘導することが示される。しかし、実験モデルが治療剤に対するヒト患者の応答を確実に予想しないことが多いことは周知である。したがって、天然に存在するイヌ腫瘍はヒト治験への橋渡しとして使用された。イヌ腫瘍は、それが不均一な遺伝的背景を有する動物で発生するのでヒトの腫瘍により類似しており、宿主起源であり、操作された突然変異ではなく自然発生的な突然変異に起因する。C.novyi-NT芽胞の腫瘍内注射は自然発生的な固形腫瘍を有するコンパニオン動物のイヌでは十分に許容され、最もよくある毒性は細菌感染に関連する予想される症状であることが見出された。客観的な応答が16頭のイヌのうち6頭(37.5%)で観察され、3頭は完全奏功、3頭は部分奏功であった。これらの有望な結果に基づいて、進行性平滑筋肉腫を有するヒト患者はC.novyi-NT芽胞の腫瘍内注射で処置された。この処置により、劇的な応答が生じ、骨内および周辺において腫瘍を著しく減少させた。まとめると、これらの結果により、C.novyi-NTは新生物組織を正確に根絶する制御された道具として作用することが可能であることが明らかにされ、選択された患者におけるこの薬剤のさらなる臨床試験が正当化されることが示唆される。

USA 98巻:15155~15160頁)。移植された同系腫瘍を有するマウスおよびウサギ内への静脈内に注射されるC.novyi-NT芽胞の単回用量により、局所化された腫瘍壊死、強い炎症応答、および処置を受けた動物の25~30%における完全奏功がもたらされた(Agrawalら、(2004年)Proc. Natl. Acad. Sci. 試験設計:腫瘍病変に特異的におよび正確に局在化しているC.novyi-NT誘導感染を実証するために、ラット同所性F98神経膠腫モデルを使用して前臨床概念証明試験を行った。ルシフェラーゼ活性およびカプランマイヤー生存曲線を使用して治療効果を評価した。自然発生的固形腫瘍を有するコンパニオン動物のイヌにおける比較試験を使用して、前臨床とヒト試験の間の橋渡しを行った。実験単位は1試験イヌであり、それぞれのイヌが4サイクルまでの処置を受けた。プラセボ対照、盲検、または無作為化は本試験では使用しなかった。正式な演繹的な統計的仮説は本比較試験では計画しなかった。記述要約統計および分析は事後に提供された。ヒト臨床試験は、進行中の非盲検、非無作為化、多施設第I相試験であり、標準「3+3」用量漸増を用いた。試験は、(i)単回腫瘍内注射として投与した場合の、処置抵抗性固形腫瘍悪性病変を有するヒトでのC.novyi-NT芽胞の安全性プロファイル、用量制限毒性、および最大耐用量を決定する、(ii)注射された腫瘍と応答全体の両方の予備的抗腫瘍活性を記述する、(iii)循環しているC.novyi-NT芽胞の処分を試験する、ならびに(iv)C.novyi-NT処置に関連する宿主免疫および炎症応答を測定するように設計した。
Pharmaceutical、Burlingame、CA)、およびエタノール(14.25%)の腹腔内注射によって麻酔をかけた。レンチウイルスを経てルシフェラーゼ構築物をトランスフェクトさせたF98神経膠腫細胞(2×104)は、以前記載された通りに(Baiら、(2011年)Neuro-oncology 13巻:974~982頁)、穿頭孔を通じてブレグマの3mm外側で2mm前側に位置する右前頭葉中に定位的に植え込んだ。腫瘍サイズは、腫瘍細胞の植込み後12日目に8mg/ラット D-ルシフェリンカリウム塩を腹腔内注射し、Xenogen器具によって評価した。引き続いて、以前記載された通りに(Dangら、(2004年)Proc. Natl. Acad. Sci. U.S.A. 98巻(26号):15155~15160頁;Bettegowdaら、(2006年)Nat. Biotechnol.24巻:1573~1580頁)生成された3百万のC.novyi-NT芽胞を上記と同じ座標を使用して頭蓋内腫瘍中に定位的に注射した。ラットは最初の2日間は10mg/kg/日の腹腔内デキサメタゾンで処置して術後浮腫のリスクを最小化し、これは脳腫瘍手術および生検後のヒト患者において使用される標準臨床プロトコールを厳密に模倣している。対照ラットは同じ体積のPBSを定位的に注射され、最初の2日間は10mg/kg/日の腹腔内デキサメタゾンで処置された。動物は、ジョンズホプキンズ動物実験ガイドラインに従って、悪化、嗜眠、神経毒性、または疼痛のいかなる徴候についても毎日観察した。窮迫の症状がある場合は、水分補給とドキシサイクリンを用いた支持的治療(腹腔内に15mg/kgの負荷量とそれに続いて維持として12時間ごとに10mg/kg)を開始し、7日間続けた。症状が持続するおよび/または衰弱した場合、瀕死の動物は安楽死させた。腫瘍内に注射されたC.novyi-NT芽胞の有効性はカプランマイヤー生存曲線、ならびに脳部分の残っている腫瘍組織量(tumor burden)により評価した。後者では、脳はさらなる病態試験のために死後に収集され、ホルムアルデヒドに入れ、パラフィンに包埋した。グラム染色スライド、サフラニンでの対比染色、およびH&Eスライドを標準手順ガイドラインに従って入手した。
Lake City、UT)に従ってダイズカゼイン消化培地およびチオグリコレート培地において産物を培養することにより実施した。発芽効率アッセイは、5%ウマ血液を含むブルセラ寒天上、嫌気条件下で実施して、芽胞が予め設定した生存能力基準を確実に満たすようにした。芽胞は、体積1000μLおよび濃度1×109芽胞/mLでOリング密封ねじ口付の無菌1.8mLクリオバイアル(Simport、Beloeil、Canada)に詰めた。C.novyi-NTクリオバイアルは2~8℃で保存した。投薬のため、0.4mLアリコートの保存芽胞溶液を0.5mLクリオバイアルに詰めた。投薬後、クリオバイアルおよび未使用のC.novyi-NT芽胞は、バイオセーフティーレベル2材料の処分のために適用可能な規制に従って破棄した。

suspected adverse reactions in animals and humans to veterinary medicinal products)の用語を用いて、獣医がん臨床研究グループ(Veterinary Co-operative Oncology Group)有害事象共通用語規準(VCOG-CTCAE)v1.0(Veterinary co-operative oncology group(2004年)Vet. Comp. Oncol. 2巻:195~213頁)を使用して有害事象を評価した。C.novyi-NT発芽に関連する有害事象(標的病変反応)についての用語は表3に定義している。適切なVeDDRAも標的病変反応用語もない臨床所見は符号化されていない徴候として別々に分類した(表4)。C.novyi-NT療法との関係は報告する研究者により決定された。
腫瘍内注射されたC.novyi-NT芽胞は腫瘍組織を特異的に標的にし、ラットの生存を伸ばす:高悪性度神経膠腫は、広範囲な領域の低酸素および壊死のある著しい病理組織学的変動性を示す。この腫瘍型は一般には転移しないが、その複雑さは中枢神経系内での保護された位置と共にこのがんを最も処置するのが困難ながんの1つにしてきた。完全な外科的切除は、解剖学的制約および無情にも腫瘍再発をもたらす浸潤性成長パターンのためほぼ常に不可能である。したがって、神経膠腫は、C.novyi-NT芽胞の局所注射が治療的に有用である可能性のある腫瘍型を表すと思われた。この可能性を評価するため、ルシフェラーゼを発現するように操作されたF98ラット神経膠腫細胞を6週齢のF344フィシャーラットに同所的に移植すると、急速に致死的になる局所的浸潤性腫瘍を生じた(図3A)。これらのラットの腫瘍中へのC.novyi-NT芽胞の定位的腫瘍内注射によって、24時間以内に発芽し、48時間以内には腫瘍組織量の指標であるルシフェラーゼ活性が急速に低下した(図3Bおよび3C)。C.novyi-NT発芽は増殖性形態の細菌の出現により示された。際立ったことに、C.novyi-NTは腫瘍に正確に局在化しており、隣接する正常細胞をわずか数ミクロン離れて温存していた(図4Aおよび4B)。さらに、これらの増殖性細菌は、正常な脳実質内に埋もれた微小浸潤性腫瘍細胞のアイランド内で特異的に成長し、同時にアイランドを破壊するように見えることがあった(図4Cおよび4D)。この細菌処置により、この極めて侵襲性のラットモデルにおいて著しい生存上の利点が生じた(図3A、P値<0.0001)。C.novyi-NT発芽の結果としての脳浮腫は一般的であり、医学的に管理された。脳内の膿瘍形成は抗生物質を適切に使用した同系ラットモデルでははっきり観察はされなかった。しかし、膿瘍形成は本治療の潜在的副作用であり、ヒト患者において発症するおそれがあり、慣例的な臨床手順である、神経外科的膿瘍切除およびドレナージを必要とすると考えられる。にもかかわらず、高悪性度神経膠腫の陰鬱な予後を考慮すると、C.novyi-NT処置の利益は関連する潜在的リスクよりまさる可能性がある。
大半の従来の抗がん治療は腫瘍の十分に血管形成されている成分を標的にする。しかし、この疾患を治癒するためには、全ての新生物細胞を破壊しなければならず、残っているがん細胞はいずれも腫瘍を再生することが可能である。この原則は最近の研究で、標的化された抗がん剤を用いて劇的に例証された。顕著な寛解を誘導することは可能であるが、治療に先立って抵抗性突然変異を有するわずかな割合(<0.0001%)の細胞のため腫瘍が数か月以内にほぼ常に再発する(Sharmaら、(2007年)Nat. Rev. Cancer7巻:169~181頁;Chapmanら、(2011年)New
Engl. J. Med 364巻:2507~2516頁;Kwakら、(2010年)New Engl. J. Med 363巻:1693~1703頁)。

U.S.A 101巻(42号):15172~15177頁)。ヒト患者でもコンパニオン動物のイヌでも後天的抗腫瘍免疫応答を実証する明白な証拠はなかったが、C.novyi-NT芽胞の腫瘍内注射により誘導された著しい炎症応答は先天的免疫応答の明確な証拠を提供している。C.novyi-NTは酸素に対して鋭敏に感受性であり、腫瘍の正常酸素圧領域で発芽することは示されていないので、永続的な完全奏功が得られるイヌにおいて免疫(先天的なものでも後天的なものでも)が役割を果たしていたことはもっともらしい。さらに、腫瘍内注射C.novyi-NT芽胞での最初のヒト経験は、迅速で強固な局所抗腫瘍応答を生じた。この場合、下にある骨が近位にあったことが、最終的に手術を必要とした病的骨折の一因となった可能性がある。しかし、患者選択は将来における類似の合併症のリスクを最小化することができる。この結果はわずか10,000芽胞、イヌまたはラットを処置するのに使用した用量のわずかな割合により生じたことを指摘することは重要である。第I相試験が進行するに従って、さらに高い用量が直接局所部位から循環内に放出される芽胞の拡散を通じてまたは宿主媒介免疫を通じて、遠隔転移に影響するかどうかを見ることは興味深いことになる。
