Description
本出願は、2015年10月14日付けで出願された、米国特許出願第62/241,522号に対する、米国特許法第119条(e)下の優先権を主張する。前述のものの全内容は、参照により本明細書に取り込まれる。
ドライAMDの顕著な徴候の1つは、局所組織(RPE/網膜)から、および循環からもたらされる成分である、ドルーゼンの蓄積である(Curcio et al., 2011 Br J Ophthalmol 95(12):1638-45;Wu et al., 2010 J Neurochem 114(6):1734-44)。ドルーゼノイド色素上皮剥離(PED)もまたAMDと関連付けられ、そこでは、網膜色素上皮が、1つまたは複数のドルーゼンの存在に起因して下層のブルッフ膜から離れている。
本明細書において記載される方法は、高用量スタチンの投与を含む。スタチン(またはHMG-CoA還元酵素阻害剤)は、コレステロールを低下させる薬物の一種であり、構造上HMG-CoAに類似し、以下に示される。
患者においてAMDの処置のため高用量スタチンを使用する方法が、本明細書において提供される。幾つかの実施態様において、本開示は、患者においてドルーゼン(例えば、ソフト)および/またはドルーゼノイドPEDを退縮させる方法を提供する。幾つかの実施態様において、本開示は、患者においてRPEおよび/または1つもしくは複数の光受容器の萎縮を予防する(すなわち、そのリスクを低減する)方法を提供する。幾つかの実施態様において、本開示は、患者において視力喪失を予防する(すなわち、そのリスクを低減する)、および/または視力を改善する方法を提供する。幾つかの実施態様において、本開示は、患者においてAMD進行(例えば、ドライAMDからウェットAMDへの)を予防する(すなわち、そのリスクを低減する)方法を提供する。本明細書において提供される方法は、患者に本明細書において記載される高用量スタチンを投与することを含み得る。本明細書において提供される方法はまた、患者に本明細書において記載される維持用量スタチンを連続投与することも含み得る。本明細書において提供される方法はまた、患者をAMD(例えば、ドライAMD)またはソフトドルーゼンを有すると同定すること、および場合により、患者を、彼らがAMDまたはソフトドルーゼンを有するという基準に基づき選択することも含み得る。患者においてAMDおよびドルーゼンを診断する方法は、当該技術分野において公知である。本明細書において提供される方法はまた、患者に本明細書において記載される高用量スタチンを投与することの有効性について患者をモニタリングすること(例えば、ドルーゼン退縮、RPEおよび/または1つもしくは複数の光受容器の萎縮、視力喪失および/または視力、AMD進行について患者をモニタリングすること)も含み得る。
IRB認可を有する、症例報告およびパイロット多施設前向き介入研究を行った。試験対象患者基準は、AMDの診断を有し、かつ両眼における多くの巨大なソフトドルーゼン/ドルーゼノイドPEDの存在を有する年齢50歳を超える患者であった。除外基準は、以下の通りであった:どちらかの眼における有意な地図状萎縮または脈絡膜の血管新生の存在(もしくは既往歴);視力を低減し得る他の眼疾患(軽度の白内障を除く);眼科手術(白内障摘出以外)の既往歴;アトルバスタチン40mgに均等な用量でのスタチン療法を現在または最近(2年以内)受けた患者;肝疾患、横紋筋融解症、またはスタチンに対するアレルギーの既往歴;妊娠または授乳;スタチンと相互作用することが公知の医薬(例えば、シクロスポリン、全身イトラコナゾール、クラリスロマイシン、HIVタンパク質分解酵素阻害剤)の現在の使用;およびベースライン時の上昇したアミノ基転移酵素またはクレアチニンキナーゼ。有意な後嚢混濁が伴わない限り、偽水晶体は除外の理由ではなかった。
初期事象の報告
AMDを有するその他は健常な63歳の男性は、悪化している視力を提示した。彼のベースラインの視力は、20/25であり、有意な歪みを伴い;彼は、加齢眼疾患研究(AREDS)補給を既に受けていた。眼底検査により、両側性の広範な集密的巨大ソフトドルーゼンおよび色素性変化が明らかになった(図1、上部の列)。スペクトル領域光干渉断層撮影(SD-OCT)は、有意な程度のこれらの沈着物および色素上皮剥離ならびに覆っているRPEおよび光受容器の構造上の歪みを裏付けた(図2、上部の列)。網膜下または網膜内体液は存在しなかった。標準的AREDS補給を継続した。1年後、患者はより症状を示し、VAは20/30まで有意に低下した。患者は、9ヶ月かけて、開始時の1日試験用量10mgから標的の1日用量80mgまで投薬量を段階的に計画して増やしながら、アトルバスタチンを始めた。アトルバスタチン80mgの6ヶ月後、視力は、12文字、20/20まで改善し、眼底検査およびSD-OCTでの検査は、RPEの萎縮を付随することなく(図2)、ドルーゼンの完全な消失(図1)を明らかにした。
パイロット研究において登録した26人の患者のうち、23人がこれを達成した。3人の患者が研究を降り、1人は筋痙攣のため、1人は筋痛のため、1人は、その患者が、薬物が抜け毛を誘発していると感じたためであった。
これらの結果は、高用量スタチン処置を使用して、高いリスク特性のドライAMDを処置し、萎縮または視力喪失への進行を予防することができることを示す。
実施例2:アトルバスタチンはヒト網膜色素上皮細胞において貪食を促進し、炎症促進性応答を減弱する
材料
ヒトRPE細胞株ARPE-19を、ATCC(マナッサス、バージニア州、米国)から購入した。DMEM/F-12, HEPES培地、ウシ胎児血清(FBS)、およびペニシリン-ストレプトマイシンを、ライフテクノロジーズ(グランドアイランド、ニューヨーク州、米国)から得た。組み換えヒトIL-1αを、R&Dシステムズ(ミネアポリス、ミネソタ州、米国)から得た。コレステロール結晶を、シグマアルドリッチ(セントルイス、ミズーリ州、米国)から購入した。抗IL-18抗体、抗IL-6抗体、および抗β-アクチン抗体を、アビーム(ケンブリッジ、マサチューセッツ州、米国)から得た。抗IL-1β抗体および抗IL-8抗体を、R&Dシステムズ(ミネアポリス、ミネソタ州、米国)から購入した。HRP結合2次抗体を、セル・シグナリング・テクノロジー(ダンバーズ、マサチューセッツ州、米国)から得た。
アトルバスタチン(アトルバスタチンカルシウム三水和物)、シンバスタチン、およびロバスタチン(アスペルギルス種由来のメビノリン)を、シグマアルドリッチ(セントルイス、ミズーリ州、米国)から購入した。それらを、ジメチルスルホキシド(DMSO;ATCC、マナッサス、バージニア州、米国)中にて再構成した。5mMの各スタチンのストック溶液を調製し、さらなる希釈液を培地中にて作製した。対照培養物を、スタチンと共に加えた最高濃度に対応する終濃度にてDMSOと共にインキュベーションした。
ARPE-19細胞を、10%FBS、100U/mLペニシリン、および100μg/mLストレプトマイシンを添加したDMEM/F-12, HEPES培地において維持した。細胞を、加湿5%CO2において37℃にて成長させ、80%コンフルエントに達したとき、継代した。
コレステロール結晶をグラインダーで微粉砕し、続いて、紫外線で30分間滅菌した。ARPE-19培養液をコレステロール結晶に加えて、6mg/mLストック溶液を作製した。コレステロール結晶が培養液中にて一様に懸濁されるまで、ストック溶液を超音波処理した。
他で(Hendriks et al., 1996 Biochem J 314 (Pt 2):563-8)記載された通り、CuSO4(シグマ、セントルイス、ミズーリ州、米国)を使用して、LDL(リーバイオソリューションズ、メアリーランドハイツ、ミズーリ州、米国)を酸化した。簡単に言うと、LDL(600μ1、0.25mg/ml)、CuSO4(22.5μl、1.6mM)、およびPBS(277.5μl)を混合し、37℃にてインキュベーションした。インキュベーションの24時間後、1mM EDTAを使用して、酸化反応を停止させた。酸化の直後、PD-10 disposable desalting columns(GEヘルスケア、バッキンガムシャー州、英国)を使用して、リポタンパク質を脱塩した。
Pranabにより記載された(Mukherjee et al., 2007 Proc Natl Acad Sci USA 104:13158-63)プロトコールに従い、フローサイトメトリーアッセイを使用して、細胞貪食を評価した。簡単に言うと、ARPE-19細胞を12ウェルプレート内に播種し、90%コンフルエントまで培養した。5×107/ml直径1μm Fluoresbrite(登録商標)YG Carboxylate Microspheres(ポリサイエンス、ペンシルベニア州、米国)単独、あるいはスタチン(アトルバスタチン、ロバスタチン、もしくはシンバスタチン)、コレステロール結晶またはox-LDLとの組み合わせで、細胞をインキュベーションし、使用した薬物の濃度および処置の正確な期間を、結果のセクションにおいて各実験について別々に記載する。インキュベーション後、細胞をPBSで3回洗浄して、全ての細胞外ビーズを取り除き、次に、1分間、トリプシン処理(0.25% trypsin-EDTA、ギブコ、米国)し、予め温めた培養液で中和した。細胞懸濁液を回収し、遠心分離した(241g、5分)。各試料について、細胞ペレットをPBS0.5ml中にて再懸濁した。続いて、CellQuest 3.0.1(ベクトン・デッキンソン、マウンテンビュー、カリフォルニア州、米国)およびFlowJo 10.0ソフトウェアを使用して、FACScaliburフローサイトメーターで緑色の貪食された蛍光ビーズ(励起波長441nm、および発光波長486nm)について、細胞を解析した。各群に存在する貪食細胞の割合、ならびに囲まれた粒子の平均蛍光強度を記録した。
35mmのガラス底ディッシュ(マットテック・コーポレーション、マサチューセッツ州)上、10%FBSおよび1%ペニシリン/ストレプトマイシンを添加したDMEM/F12培地中で、ARPE-19細胞を培養した。70~80%のコンフルエントな細胞を、50μΜアトルバスタチンで3時間処理した。膜流動性を測定する30分前に、FluoroBrite(商標)DMEM medium(サーモフィッシャーサイエンティフィック)中に溶解した、5μΜ4,4-ジフルオロ-5,7-ジメチル-4-ボラ-3a、4a-ジアザス-インダセン-3-ドデカン酸(BODIPY(登録商標)FL C12)(サーモフィッシャーサイエンティフィック)、炭素数12の飽和炭化水素末端と合わせた緑色フルオロフォアと共に、細胞をインキュベーションした。次に、細胞を、FluoroBrite(商標)DMEM mediumで洗浄して、取り込まれていない色素を全て取り除き、FRAP測定のため顕微鏡台に取り付けた環境室を使用して、37℃および5%CO2にて維持した。Zeiss LSM 510 Axiovert 200M共焦点レーザー走査顕微鏡の、追加の2×デジタルズーム(総拡大率=[10×][63×][2×]=1,260×)での、63×、1.4数値開口部油浸レンズを介したアルゴンレーザーからの光の短い(2秒)集中パルス(透過率100%にて同時に458、477、488、および514nmのライン)で、細胞膜の微細領域を光退色させることにより(bleach Regions of Interest(ROI)を7×7画素スクエアを使用して描いた)、FRAP測定を行った。隣接したインタクトなフルオロフォアの側方拡散に起因する、光退色した領域内の蛍光の回復を、減弱レーザービーム(透過率3%にて488nmのライン)で細胞表面(500ms毎)を反復走査することにより、評価した。
6ウェルプレートにおいて、1×105細胞/ウェルの密度にて、ARPE-19細胞を播種した。24時間後、細胞を、IL-1α(5ng/ml)で8時間刺激し、アトルバスタチン(0.1、0.5、または5μΜ)で16時間処理し、次に、アトルバスタチンを含む、もしくは含まない、2mg/mlコレステロール結晶と共に6時間、または300μg/ml oxLDLと共に18時間のいずれかでインキュベーションした。処置後、培養液を回収し、4℃にて13.3gで15分間遠心分離した。上清を回収し、-80℃にて保存した。培養液(IL-6、IL-8用)を各レーンに添加し、試料を電気泳動により泳動させた。タンパク質をPVDF膜に移し、膜を無脂肪乳でブロッキングし、IL-6に対する1次抗体およびIL-8に対する1次抗体と共にインキュベーションした。次に膜を洗浄し、2次抗体と共にインキュベーションした。膜を、増強した化学発光で現像した。タンパク質バンドの強度を、software Image Lab 4.1(バイオ・ラッド、ハーキュリーズ、カリフォルニア州、米国)を使用して測定した。
oxLDLにより誘導されたIL-8を、製造元の指示に従い、ELISAキット(IL-8、R&Dシステムズ、ミネアポリス、ミネソタ州、米国)を使用して、ARPE-19細胞の条件培地の分析により測定した。
全実験を、トリプリケートで行った。統計解析を、GraphPad Prism 5.0aを使用して行った。結果を平均±SEとして表す。2つの処置群間の統計上有意な差を、独立t検定により解析した。p<0.05値を、統計上有意と設定した。
親油性スタチンはARPE-19細胞の貪食機能を増大する
ARPE-19の貪食機能を確実に測定する方法を立証するために、細胞を、カルボキシレート微粒子と共に6時間インキュベーションし、次に、大量の50枚の画像を収集する共焦点顕微鏡を使用して画像化した。細胞は、粒子を活発に吸収することができ、それは核の近位に位置し(図3A~C)、これは、ポリスチレン微粒子がこれらの確立した条件下でARPE-19細胞により貪食され得ること、およびこの方法がARPE-19細胞の貪食機能を正しく評価することができることを示している。
高用量アトルバスタチンは、選択AMD患者においてドルーゼン沈着物の退縮および視力の改善をもたらした(実施例1;Vavvas et al., 2016 EBioMedicine 5:198-203)。ARPE-9細胞の貪食機能に対する高用量アトルバスタチンの効果をさらに調査するために、細胞を、微粒子と異なる用量のアトルバスタチンの両方と共に6時間インキュベーションし、貪食RPE細胞の割合を、既に記載した通り、フローサイトメトリーにより評価した。図4AおよびBにおいて示す通り、1、25、50、または75μΜアトルバスタチンは、貪食ARPE-19細胞の割合を、DMSO対照群のベースラインの割合(31%)と比較して、それぞれ、35%、40%、42%、および46%まで増大した(p<0.05)。加えて、1、25、50、または75μΜアトルバスタチンでの細胞の処理は、細胞の平均蛍光強度を、対照群における340から、それぞれ、368、443、453および467まで有意に増大した(p<0.05)(図4C)。これらのデータは、アトルバスタチンが、貪食細胞の割合およびARPE-19細胞の貪食活性を用量に依存した方法で増大することを明らかに示す。
RPE細胞膜流動性に対するアトルバスタチンの効果を試験するために、ARPE-19細胞を、50μΜアトルバスタチンと共に3時間、およびBODIPY(登録商標)FL C12と共に30分間インキュベーションした。細胞膜の微細領域を光退色させることにより、FRAP測定を行い、退色させた領域内の蛍光の回復を、方法において記載した通り、減弱レーザービームで細胞表面を反復走査することにより評価した。結果は、アトルバスタチン処置が、光退色後、ARPE19細胞の膜において蛍光の回復を増大したことを示した(図5A)。加えて、それは、対照群と比較して、蛍光平衡の半減時間を低下した(p<0.05)(図5B)。知見の両方が、アトルバスタチンがARPE-19細胞の膜流動性を増大することを示す。この増大は、アトルバスタチン処置後のARPE-19細胞の貪食機能の増大を少なくとも部分的に説明することができた。
RPE細胞の貪食機能に対するコレステロール結晶およびox-LDLの効果を、この機能の機能障害がAMDと結び付けられることから、研究した(Nandrot et al., 2007 Adv Exp Med Biol 801:978-l)。ARPE-19細胞を、2mg/mlコレステロール結晶で6時間、または300μg/ml oxLDLで18時間処理した。次に、貪食機能を、方法において記載した通り、フローサイトメトリーにより評価した。コレステロール結晶は、貪食細胞の割合を31%から22%に(p<0.05)(図6A)、およびARPE-19細胞の平均蛍光強度を332から280に(p<0.05)(図6B)有意に低下した。同様に、ox-LDLは、貪食細胞の割合を31%から26%に(p<0.05)(図6C)、および細胞の平均蛍光強度を325から290に(p<0.05)(図6D)低下した。
コレステロール結晶は、ARPE-19細胞において炎症性サイトカインIL-6およびIL-8の分泌を誘導する(Hu et al., 2014 Discovery Med. 18(97):7-14)。これらのサイトカインはAMDの発症および進行と関連するので、アトルバスタチンがこの効果を阻害することができるかどうかを調べた。ARPE-19細胞を、IL-1αで刺激し、異なる濃度のアトルバスタチンで処理し、次に、コレステロール結晶と共にインキュベーションした。IL-6およびIL-8のレベルを、ウェスタンブロットまたはELISAを使用して評価した。予想通り、コレステロール結晶は、対照処置と比較して、IL-6およびIL-8のARPE-19分泌を、それぞれ、3.2倍および2.5倍増大した(図7AおよびB)。しかしながら、0.1、0.5または1μΜアトルバスタチンでの細胞の事前処理は、対照と比較して、IL-6のレベルを、ぞれぞれ、1.7倍、1.-1倍、および0.8倍、IL-8のレベルを、それぞれ、1.8倍、1.6倍および1.1倍有意に低減した(p<0.05)(図7AおよびB)。加えて、本発明者らは、ARPE-19細胞においてIL-6およびIL-8に対するox-LDLの効果、および生じた結果に対するアトルバスタチンの効果を調べることを望んだ。コレステロール結晶と同様に、ox-LDLとのARPE-19のインキュベーションは、分泌されたIL-6およびIL-8において、それぞれ、2.6倍および1.3倍の増大を導いた。しかしながら、0.1、0.5または1μΜアトルバスタチンでの細胞の事前処理は、対照と比較して、IL-6のレベルを、それぞれ、1.9倍、1.3倍および1.4倍、ならびにIL-8のレベルを、それぞれ、1.2倍、1.02倍および0.99倍低減することができた(p<0.05)(図7CおよびD)。結果の両方が、アトルバスタチンが、ARPE-19細胞においてコレステロール結晶およびox-LDLにより誘導されるIL-6ならびにIL-8の分泌を、用量に依存した方法で、阻害し得ることを示す。
本発明者らの研究は、親油性スタチンが、ARPE-19細胞の貪食機能を増強すること、ならびにアトルバスタチンが、これらの細胞を、コレステロール結晶およびox-LDLにより誘導される貪食機能ならびに炎症性効果の機能障害から保護し得るという証拠を提供する。加えて、アトルバスタチンは、ARPE-19細胞の膜流動性を増大し、それは、これらの細胞の貪食機能に対するスタチンの正の効果を少なくとも部分的に説明する。
本開示は、その詳細な説明と合わせて記載される一方、前述の記載は、説明することを意図され、添付の請求の範囲により定義される、開示の範囲を制限しないことは理解されるべきである。他の態様、利点、および修飾は、以下の請求の範囲内である。
Claims (19)
- 加齢性黄斑変性症(AMD)を有する患者におけるi)ドルーゼンの退縮、ii)ドルーゼノイド色素上皮剥離(PED)の退縮、iii)AMDからウェットAMDへの進行の予防、iv)網膜色素上皮(RPE)の萎縮の予防、v)1つもしくは複数の光受容器の萎縮の予防、vi)視力喪失の予防、vii)視力の改善、viii)RPE細胞の貪食機能の増強、およびix)上記の組み合わせから選択される処置のための医薬組成物であって、当該医薬組成物は高用量スタチンを含み、当該高用量スタチンが、i)前記患者におけるドルーゼンの退縮、ii)前記患者におけるドルーゼノイド色素上皮剥離(PED)の退縮、iii)前記患者におけるAMDからウェットAMDへの進行の予防、iv)前記患者における網膜色素上皮(RPE)の萎縮の予防、v)前記患者における1つもしくは複数の光受容器の萎縮の予防、vi)前記患者における視力喪失の予防、vii)前記患者における視力の改善、viii)前記患者におけるRPE細胞の貪食機能の増強、またはix)上記の組み合わせに有効であり、当該高用量スタチンが、1日あたり少なくとも80mgのアトルバスタチン、1日あたり少なくとも0.8mgのセリバスタチン、1日あたり少なくとも240mgのフルバスタチン、1日あたり少なくとも180mgのロバスタチン、1日あたり少なくとも8mgのピタバスタチン、1日あたり少なくとも120mgのプラバスタチン、1日あたり少なくとも40mgのロスバスタチン、または1日あたり少なくとも120mgのシンバスタチンである、医薬組成物。
- 患者の加齢性黄斑変性症(AMD)の処置における使用のための医薬組成物であって、かつ、AMDを有する前記患者におけるi)ドルーゼンの退縮、ii)ドルーゼノイド色素上皮剥離(PED)の退縮、iii)AMDからウェットAMDへの進行の予防、iv)網膜色素上皮(RPE)の萎縮の予防、v)1つもしくは複数の光受容器の萎縮の予防、vi)視力喪失の予防、vii)視力の改善、viii)RPE細胞の貪食機能の増強、またはix)上記の組み合わせのための医薬組成物であって、当該医薬組成物は高用量スタチンを含み、当該処置は高用量スタチンを当該患者に投与することを含み、当該高用量スタチンが、i)前記患者におけるドルーゼンの退縮、ii)前記患者におけるドルーゼノイド色素上皮剥離(PED)の退縮、iii)前記患者におけるAMDからウェットAMDへの進行の予防、iv)前記患者における網膜色素上皮(RPE)の萎縮の予防、v)前記患者における1つもしくは複数の光受容器の萎縮の予防、vi)前記患者における視力喪失の予防、vii)前記患者における視力の改善、viii)前記患者におけるRPE細胞の貪食機能の増強、またはix)上記の組み合わせに有効であり、当該高用量スタチンが、1日あたり少なくとも80mgのアトルバスタチン、1日あたり少なくとも0.8mgのセリバスタチン、1日あたり少なくとも240mgのフルバスタチン、1日あたり少なくとも180mgのロバスタチン、1日あたり少なくとも8mgのピタバスタチン、1日あたり少なくとも120mgのプラバスタチン、1日あたり少なくとも40mgのロスバスタチン、または1日あたり少なくとも120mgのシンバスタチンである、医薬組成物。
- 前記処置が、前記患者がAMDを有すると同定すること、および前記患者を、前記患者がAMDを有するという基準に基づき選択することをさらに含む、請求項1または2に記載の医薬組成物。
- 前記処置が、前記患者がドライAMDを有すると同定すること、および前記患者を、前記患者がドライAMDを有するという基準に基づき選択することをさらに含む、請求項1または2に記載の医薬組成物。
- 前記処置が、前記高用量スタチンの有効性について前記患者をモニタリングすることをさらに含む、請求項1または2に記載の医薬組成物。
- 前記処置が、ドルーゼンの退縮、ドルーゼノイドPEDの退縮、RPEの萎縮、1つもしくは複数の光受容器の萎縮、視力喪失、またはAMD進行について前記患者をモニタリングすることをさらに含み、当該モニタリングが、前記患者におけるドルーゼンの退縮、ドルーゼノイドPEDの退縮、RPEの萎縮、1つもしくは複数の光受容器の萎縮、視力喪失、またはAMD進行の指標となるパラメーターを、前記高用量スタチンの投与に先立つ第1の時間ポイントにて測定し、前記患者における同一のパラメーターを、前記高用量スタチンの投与後である第2の時間ポイントにて測定し、および第1の時間ポイントと第2の時間ポイントにて測定されたパラメーターを比較することを含み、ここで、第1の時間ポイントから第2の時間ポイントへの前記パラメーターの低減は、ドルーゼンの退縮、ドルーゼノイドPEDの退縮、RPEの萎縮の予防、1つもしくは複数の光受容器の萎縮の予防、視力喪失の予防、またはAMD進行の予防を示す、請求項5に記載の医薬組成物。
- 前記処置が、視力について前記患者をモニタリングすることをさらに含み、当該モニタリングが、前記患者における視力の指標となるパラメーターを、前記高用量スタチンの投与に先立つ第1の時間ポイントにて測定し、前記患者における同一のパラメーターを、前記高用量スタチンの投与後である第2の時間ポイントにて測定し、および第1の時間ポイントと第2の時間ポイントにて測定されたパラメーターを比較することを含み、ここで、第1の時間ポイントから第2の時間ポイントへの前記パラメーターの増大は、視力における改善を示す、請求項5に記載の医薬組成物。
- 前記患者がヒトである、請求項1または2に記載の医薬組成物。
- 前記処置が、前記患者がAMD進行の高いリスク特性を有すると同定すること、および前記患者を、前記患者がAMD進行の高いリスク特性を有するという基準に基づき選択することをさらに含む、請求項1または2に記載の医薬組成物。
- 前記患者が、前記高用量スタチンを、少なくとも12ヶ月間、少なくとも15ヶ月間、少なくとも18ヶ月間、少なくとも21ヶ月間、少なくとも24ヶ月間、少なくとも30ヶ月間、少なくとも33ヶ月間、少なくとも36ヶ月間、少なくとも39ヶ月間、少なくとも42ヶ月間、少なくとも45ヶ月間、または少なくとも48ヶ月間投与される、請求項1または2に記載の医薬組成物。
- 前記高用量スタチンが、i)静脈内投与、ii)眼性投与、iii)筋肉内投与、iv)皮下投与、v)経口投与、vi)鼻腔内投与、vii)吸入投与、viii)経皮投与、ix)硝子体内、およびx)非経口投与からなる群から選択される経路により投与される、請求項1または2に記載の医薬組成物。
- 前記高用量スタチンが、硝子体内、局所滴下、または局所軟膏からなる群から選択される眼性経路により投与される、請求項1または2に記載の医薬組成物。
- 前記スタチンが、アトルバスタチン、セリバスタチン、フルバスタチン、ロバスタチン、ピタバスタチン、プラバスタチン、ロスバスタチン、およびシンバスタチンからなる群から選択される、請求項1または2に記載の医薬組成物。
- 前記スタチンが、親油性スタチンである、請求項1または2に記載の医薬組成物。
- 前記親油性スタチンが、アトルバスタチン、ロバスタチン、およびシンバスタチンからなる群から選択される、請求項14に記載の医薬組成物。
- 前記処置が、前記患者が前記高用量スタチンを投与された後、前記患者に維持用量スタチンを投与することをさらに含む、請求項1または2に記載の医薬組成物。
- 1日あたり投与される前記維持用量スタチンが、1日あたり投与される前記高用量スタチンより低い、請求項16に記載の医薬組成物。
- 前記高用量スタチンが、1日1回、1日2回または1日3回投与される、請求項1または2に記載の医薬組成物。
- 前記処置が、前記患者に追加の治療剤を投与することをさらに含み、追加の治療剤が、抗炎症剤、血管新生抑制剤、抗酸化剤、オメガ3脂肪酸、およびビタミン/ミネラルからなる群から選択される、請求項1または2に記載の医薬組成物。
