Description
本出願は、2015年8月24日付け出願の米国仮特許出願第62/209,091号および2015年8月27日付け出願の米国仮特許出願第62/210,655号(それらの開示の全体を参照により本明細書に組み入れることとする)に対する、35 USC§119(e)に基づく優先権を主張するものである。
本発明は、腸外病原性大腸菌(Escherichia coli)(ExPEC)に対する免疫応答を誘導することによりExPECに関連した疾患に対する免疫防御をもたらす組成物および方法に関する。特に、本発明の実施形態は、解毒された緑膿菌(Pseudomonas aeruginosa)外毒素A担体タンパク質にそれぞれが共有結合している大腸菌(E. coli)多糖抗原O25B、O1A、O2およびO6Aのコンジュゲートを含有する多価ワクチン、ならびにExPEC感染に対する有効な免疫防御をもたらすための該ワクチンの使用に関する。

ExPEC疾患を引き起こす大腸菌の血清型分布に関する研究は、10個の優勢なO血清型がExPEC感染の推定60〜80%を占めうる(小さな割合の型決定不能株が含まれることを想定している)ことを示している。例えば、以下の表1A〜1Cを参照されたい。UTIおよび菌血症標的集団の両方において、血清型O1、O2、O6およびO25が4つの最も優勢な大腸菌血清型として同定され、そのうち血清型O25が菌血症における最も優勢な大腸菌血清型であった。また、異なるが構造的および抗原的に関連している亜型から構成されるO抗原血清型に関しては、臨床分離体のなかでとりわけ1つの血清型がより優勢でありうることが判明した。例えば、より最近の分析臨床株または分離体のうち、O1、O6およびO25に関しては、それぞれ、O1A、O6AおよびO25B抗原がより頻繁に見られる亜型であると判定された。国際特許出願番号PCT/EP2015/053739(その開示の全体を参照により本明細書に組み入れることとする)における疫学研究に関する関連開示を参照されたい。
1つの一般的な態様において、本発明は、大腸菌臨床分離体において優勢に見出されるO抗原血清型を含有する多価ワクチンに関するものであり、これは、該ワクチン中に含有されるO抗原血清型を有するExPECにより引き起こされる疾患の予防のための能動免疫化をもたらすために使用されうる。1つの実施形態においては、本発明は、8〜48μg/mlの第1濃度の大腸菌O25B抗原と、第1濃度の10%〜100%である第2濃度の少なくとも1つの追加的大腸菌O抗原とを含む組成物に関するものであり、ここで、大腸菌O25B抗原および前記の少なくとも1つの追加的大腸菌O抗原のそれぞれは、独立して、EPA担体タンパク質に共有結合している。
もう1つの一般的な態様においては、本発明は、ExPECに対する免疫応答を、それを要する対象において誘導する方法に関する。好ましくは、該免疫応答は、ExPECに関連した疾患を、その予防または治療を要する対象において予防または治療するのに有効である。該方法は、投与当たり4〜24μgの第1有効量の大腸菌O25B抗原と、第1有効量の10%〜100%である第2有効量の少なくとも1つの追加的大腸菌O抗原とを対象に投与することを含み、ここで、大腸菌O25B抗原および前記の少なくとも1つの追加的大腸菌O抗原のそれぞれは、独立して、EPA担体タンパク質に共有結合しており、該組成物は、大腸菌O25B抗原および前記の少なくとも1つの追加的大腸菌O抗原に対する免疫応答を、それを要する対象において誘導するのに有効である。
本発明の幾つかの実施形態は、大腸菌O25B抗原および1以上の追加的大腸菌O抗原に関連した組成物および方法に関する。好ましくは、該追加的O抗原は大腸菌の臨床分離体において一般的なものである。本発明において使用されうる大腸菌抗原の例には、大腸菌O25B、O1A、O2およびO6A抗原が含まれるが、これらに限定されるものではない。
本明細書中で用いる「大腸菌O1A抗原」は、大腸菌O1A血清型に特異的なO抗原を意味する。1つの実施形態においては、大腸菌O1A抗原は式O1A:
本明細書中で用いる「大腸菌O2抗原」は、大腸菌O2血清型に特異的なO抗原を意味する。1つの実施形態においては、大腸菌O2抗原は式O2:
本発明の実施形態においては、各大腸菌O抗原はEPA担体タンパク質に共有結合している(Ihssenら, 2010, Microbial Cell Factories 9, 61)。種々の解毒EPA変異体が文献に記載されており、本明細書に記載されているコンジュゲートにおけるEPA担体タンパク質として使用されうる。
ある実施形態においては、宿主細胞は大腸菌O抗原およびEPA担体タンパク質を産生し、EPA担体タンパク質に該O抗原を共有結合させて、バイオコンジュゲートを形成しうる。例えば、Ihssenら, 2010, Microbial Cell Factories 9, 61ならびにWO 2006/119987、WO 2009/104074および国際特許出願番号PCT/ EP2015/053739(それらの開示の全体を参照により本明細書に組み入れることとする)における関連開示を参照されたい。
ある実施形態においては、本明細書に記載されている組成物は、1以上の他の療法(例えば、抗細菌または免疫調節療法)と組合せて対象に投与される。前記の1以上の他の療法はExPEC感染の治療または予防において有益でありうる、あるいはExPEC感染に関連した症状または状態を改善しうる。幾つかの実施形態においては、前記の1以上の他の療法は鎮痛薬または解熱薬の投与である。ある実施形態においては、該療法は5分未満の間隔ないし1週間未満の間隔で投与される。当業者に公知の任意の抗細菌剤(例えば、抗生物質)は、本明細書に記載されている組成物と組合せて使用されうる。
ある実施形態においては、本明細書に記載されている組成物(または方法)は無感作(ナイーブ)対象、すなわち、ExPEC感染を有さない又はExPEC感染を過去に有したことがない対象に投与(または適用)される。1つの実施形態においては、本明細書に記載されている組成物(または方法)は、ExPEC感染を受けるリスクを有する無感作対象に投与(または適用)される。
O抗原とEPA担体タンパク質とのコンジュゲートおよび/またはその組成物の投与は、臨床家に公知の種々の経路、例えば皮下、非経口、静脈内、筋肉内、局所、経口、皮内、経皮、鼻腔内経路などにより行われうる。1つの実施形態においては、投与は筋肉内注射によるものである。
本明細書に記載されているコンジュゲート/組成物が対象において免疫応答を生成する能力は、本開示を考慮して、当業者に公知の任意のアプローチを用いて評価されうる。
幾つかの実施形態においては、対象において免疫応答を生成するバイオコンジュゲートの能力は、対象(例えば、マウス)または対象の集団をバイオコンジュゲートで免疫化し、追加的対象(例えば、マウス)または対象の集団を対照(例えば、プラセボ)で免疫化することにより評価されうる。ついで対象または対象の集団はExPECでチャレンジされることが可能であり、対象または対象の集団において疾患(例えば、UTI)を引き起こすExPECの能力が決定されうる。対照で免疫化された対象または対象の集団はExPECでのチャレンジの後で疾患に罹患したが、本明細書に記載されているバイオコンジュゲートまたはその組成物で免疫化された対象または対象の集団は疾患にそれほどは罹患しなかった又は疾患に罹患しなかった場合、該バイオコンジュゲートは対象において免疫応答を生成しうる、と当業者は認識するであろう。本明細書に記載されているバイオコンジュゲートまたはその組成物が、ExPECからのO抗原と交差反応する抗血清を誘導する能力は、例えばイムノアッセイ、例えばELISAにより試験されうる。
本明細書に記載されているコンジュゲート/組成物が対象において免疫応答を生成する能力は、血清殺菌力試験(SBA)またはオプソニン貪食性死滅アッセイ(OPK)を用いて評価可能であり、これらは、複合糖質に基づくワクチンの承認を得るために用いられている確立された許容されている方法に相当する。そのようなアッセイは当技術分野でよく知られており、簡潔に説明すると、関心対象(例えば、大腸菌のO抗原、例えばO25B)に対する抗体を、そのような抗体を誘導する化合物を対象(例えば、マウス)に投与することにより産生させ、単離する工程を含む。ついで、例えば、問題の細菌(例えば、関連血清型の大腸菌)を該抗体および補体ならびにアッセイによっては好中球の存在下で培養し、例えば標準的な微生物学的アプローチを用いて、該抗体が該細菌を殺しおよび/または中和する能力をアッセイすることにより、該抗体の殺細菌能を評価することが可能である。
本明細書に記載されている組成物の成分(例えば、1以上の大腸菌O抗原、および/または本発明の実施形態に従い共有結合している大腸菌O抗原のコンジュゲート)の1以上で満たされた容器を含むパックまたはキットを本発明において提供する。所望により、医薬品または生物学的製品の製造、使用または販売を規制する政府機関により規定された形態の通知または説明がそのような容器に付随していてもよく、そのような通知は、ヒトへの投与のための製造、使用または販売の、機関による承認を示す。本発明に含まれるキットは対象の治療および免疫化の前記方法において使用されうる。
実施形態1は、大腸菌O25B抗原多糖の第1濃度と、大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖のそれぞれの第2濃度とを含む組成物であって、ここで、第1濃度:第2濃度の比は1:1〜2:1であり、大腸菌O25B、O1A、O2およびO6A抗原多糖のそれぞれは、独立して、解毒緑膿菌(Pseudomonas aeruginosa)外毒素A(EPA)担体タンパク質に共有結合しており、第1濃度は10〜36μg/mlである、組成物である。
(実施例)
EPAタンパク質担体のグリコシル化部位に共有結合している大腸菌O1A、O2、O6AおよびO25Bをそれぞれ含有するO1A-EPA、O2-EPA、O6A-EPAおよびO25B-EPAバイオコンジュゲートを、例えばIhssenら, 2010, Microbial Cell Factories 9, 61ならびにWO 2006/119987、WO 2009/104074および国際特許出願番号PCT/ EP2015/053739(それらの開示の全体を参照により本明細書に組み入れることとする)に記載されているとおりに製造し、精製し、特徴づけした。該バイオコンジュゲートは、オリゴ糖転移酵素PglBおよびタンパク質担体(EPA)の存在下、種々のO血清型の多糖合成酵素を発現する組換え大腸菌細胞を使用して合成される。このアプローチにおいては、図1および図2に図示されている生化学的プロセスにより、複合糖質ワクチンを大腸菌のペリプラズム内で発現させ、抽出し、精製することが可能である。表2は、本発明の1つの実施形態によるコンジュゲートの製造に使用される宿主株を示す。
この実施例は、本発明に有用なワクチン組成物を例示する。
第1日および第14日における2回の筋肉内(i.m.)注射(大腿四頭筋)の後のワクチン組成物の毒性および局所耐性を評価するために、14日間の回復期間を伴う優良試験所基準(GLP)毒性研究をSprague-Dawleyラットにおいて行った。14日間の回復期間の後(すなわち、第28日に)、可逆性、持続性およびいずれかの変化の遅延発生を評価した。該ラットにおける第1相可能反復投与毒性試験の設計の概要を表4に示す。
2週間間隔で行われる3回のi.m.注射(第0日、第14日および第28日)の後のExPEC4Vの毒性および局所耐性を評価するために、3週間の回復期間を伴うGLP反復投与毒性試験をNZWウサギ(TOX11163、報告書案)において行った。第28日の3回目の注射の後の3週間の無処理期間の後、すなわち第49日に、可逆性、持続性およびいずれかの変化の遅延発生を評価した。該ウサギにおける第2相可能反復投与毒性試験の設計が表5において見出されうる。
O25B、O1A、O2およびO6Aバイオコンジュゲートのワクチン誘導抗体応答の機能活性を評価するために、O25B、O1A、O2およびO6A EPAバイオコンジュゲート(それぞれ単独であるか又は組合されている)を含有する1価または4価ワクチンを接種したラットからの血清を、細菌、例えば大腸菌の死滅ならびにインビトロ補体および抗体依存性食作用を測定するオプソニン食作用死滅(OPK)アッセイを用いて分析した。OPKアッセイは、種々の大腸菌血清型のオプソニン食作用および死滅を促進する血清の能力を測定する。96ウェルプレートにおいて、一定希釈度のサンプル血清を、各ウェルにおいて、4つのワクチン特異的大腸菌血清型の1つからの細菌、一定量のHL60細胞および乳児ウサギ補体と共にインキュベートした。インキュベーション後、該混合物の一部をトリプシンダイズ寒天(TSA)上にスポットし、細菌コロニーの数を数えた。該抗体が該細菌細胞に結合し、該補体の沈着を活性化し、HL60細胞による該細菌の取り込みおよび死滅を媒介する能力をオプソニン力価として表した。オプソニン力価またはオプソニン化指数(OI)は、細菌細胞の50%を殺す血清の希釈度に対応する。免疫前血清および免疫後血清のオプソニン化指数が示されている。免疫前から免疫後までのOIの少なくとも4倍の増加が有意とみなされる。
合計194名の被験者が登録したヒト初回第1相試験においてExPEC4Vを試験した。この第1相試験はランダム化プラセボ対照多施設試験である。該試験は一重盲検法で行われ、研究者および研究スタッフは被験者のランダム化群を知っていたが、被験者は常に自分らのランダム化群に関して秘匿化されていることが要求された。
1.最初の8名の被験者は1μgの用量の各PSまたはプラセボにランダム化された(3:1)。
2.それに続く8名の被験者は4μgの用量の各PSまたはプラセボにランダム化された(3:1)。
3.残りの178名の被験者は4μgの用量の各PSまたはプラセボにランダム化された(1:1)。
現在のところ、第1相試験(これは尚も継続中である)からは以下のうちのいずれもが確認されていない:有害薬物反応、有意な臨床検査上の異常、心臓血管、肺、中枢神経系、腎臓または他の有意な副作用、過剰投与。有害事象および重篤な有害事象の発生はプラセボ群とワクチン接種群との間で同等であった。
ワクチン成分の免疫原性を評価するために、臨床試験に参加している女性から血清を得、ELISAにより分析して、4価ワクチンに含まれる4つの異なるO抗原(大腸菌O1、大腸菌O2、E大腸菌O6、および大腸菌O25B)に対するIgGを提供した。精製された血清型特異的O抗原を一次アッセイ抗原として使用して、各血清型分離体に関して最適化された適切なELISAにより、全体的な第1日(ワクチン接種前)および第30日の血清IgG抗体価を評価した。サンプルごとに半最大有効濃度(EC50)値を決定するために、4パラメーターロジスティック曲線フィットを用いて、血清型ごとの全IgG抗体価を計算した。
6名の被験者に血清型当たり1μgのPS(合計4μgのPS)のExPEC4V用量を投与した。第30日までのこれらの被験者の血清型特異的免疫応答の分析は、血清型当たりの全抗体における2倍以上の増加を示す被験者の割合が50%(血清型O1A)、83%(血清型O2)、50%(血清型O6A)、67%(血清型O25B)であることを示した。抗体価における4倍以上の増加を示す被験者の割合はより低く、すなわち、17%(血清型O6A)、33%(血清型1A)、33%(血清型2)および50%(血清型O25B)であった。
血清型当たり1μgのPS(合計4μgのPS)を含有する用量と比較して、血清型当たり4μgのPS(合計16μgのPS)のExPEC4V用量の投与はより頑強な免疫応答をもたらした。第30日までのこれらの被験者の分析は、血清型当たりの全抗体における2倍以上の増加を示す被験者の割合が81%(血清型O1A)、92%(血清型O2)、80%(血清型O6A)および82%(血清型O25B)であることを示した。抗体価における4倍以上の増加を示す被験者の割合は57%(血清型O1AおよびO6A)〜80%(血清型O2)の範囲であり、これは2倍の増加の割合より低いが、血清型当たり1μgのPSを含有する用量で観察されるものより顕著に高い。図4も参照されたい。O1A、O2、O6AおよびO25Bのそれぞれに対する頑強な免疫応答が観察され、注射前(注射の30日後)と注射前(第1日)との間のELISA力価の有意な増加はワクチン接種群においてのみ認められ(V 第30日対V 第1日)、プラセボ群においては認められなかった(P 第30日対P 第1日)。
これらの中間結果は、血清型ExPEC4V用量当たり1μgのPSが投与された健常被験者におけるワクチン特異的免疫応答を示しており、血清型ExPEC4V用量当たり4μgのより高いPSでは、30日間の観察期間にわたって、免疫応答の比較的より大きな増強を示している。この期間にプラセボ群の95名の被験者の抗体価における関連変化がないことは、ExPEC4V被投与者における抗体応答がワクチン媒介性であり、ExPEC細菌への環境暴露によるものではないことを示唆している。
臨床試験に参加している女性の機能的抗体応答を評価するために、OPKアッセイを用いた。研究参加者から血清を採取した。大腸菌を、ワクチン接種した女性からの血清の希釈液でプレオプソナイズし、補体および食細胞(分化HL60細胞)と共にインキュベートし、残りのコロニー形成単位(CFU)を決定した。ついで死滅率およびオプソニン化指数(OI:大腸菌の50%を殺す血清希釈度)の最大比率を計算した。OPK試験のために選択した大腸菌はOC24452(血清型O1A)、OC24453(血清型O2)、OC24454(血清型O6A)およびOC24176(血清型O25B)であった。
ELISA試験で観察されたとおり、プラセボ被投与者(95名の被験者)は、第1日および第30日の血清対ExPEC4V血清型に関して、類似したOPK応答を示し、ほとんどのそれぞれの被験者当たりのOPK力価値に対する変化はほとんど又は全く観察されなかった。これらの結果は、ほとんど又は全てのプラセボ被験者に関して、この期間にわたる安定した機能的抗体価を示している。
6名の被験者に血清型当たり1μのPS(合計4μgのPS)のExPEC4V用量を投与した。第30日までのこれらの被験者の血清型特異的免疫応答の分析は、血清型当たりの全抗体における2倍以上の増加を示す被験者の割合が33%(血清型O1A)、67%(血清型O2)および0%(血清型O6AおよびO25B)であることを示した。血清型O1AおよびO2に関しては、抗体価における4倍以上の増加を示す被験者の割合は、それぞれ、17%および50%に低下した。
血清型当たり4μgのPS(合計16μgのPS)のExPEC4V用量の投与は全てのExPEC4V血清型に関して機能的免疫応答をもたらした。第30日までのこれらの被験者の分析は、血清型当たりのOI値における2倍以上の増加を示す被験者の割合が63%(血清型O1A)、90%(血清型O2)、33%(血清型O6A)および55%(血清型O25B)であることを示した。OI価における4倍以上の増加を示す被験者の割合は20%(血清型O6A)〜82%(血清型O2)の範囲であり、予想どおり、これらの割合は、2倍以上の増加に関して観察されたものより、一貫して低かった。
これらの中間結果は、30日間の観察期間にわたる、血清型ExPEC4V用量当たり4μgのPSが投与された健常被験者におけるワクチン特異的免疫応答を示している。この期間にプラセボ群の95名の被験者の抗体価における有意な変化がないこと(図5A〜5Dおよび表9)はELISAの結果と一致しており、ワクチン接種に対する抗体応答が4つのExPEC血清型にわたって実証されたという結論を支持している。
第I相試験からの中間結果に基づいて、第II相試験が行われるであろう。このランダム化二重盲検プラセボ対照多施設試験は、年齢によって層化(18歳以上50歳未満(N = 275)および50歳以上(N = 560))された安定な健康状態の男性および女性における5つの異なる用量の安全性、耐容性および免疫原性を評価するように計画されている。

[5] 32μg/mlのO25B抗原多糖を含む、実施形態1〜3のいずれかに記載の組成物。
[6] 5〜18μgの第1用量の大腸菌O25B抗原多糖と、それぞれ独立して第1用量の50%〜100%である用量の大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖とを含む多価免疫組成物であって、ここで、大腸菌O25B、O1A、O2およびO6A抗原多糖のそれぞれは、独立して、解毒された緑膿菌外毒素A(EPA)担体タンパク質に共有結合している、多価免疫組成物。

[9] 大腸菌O25B抗原多糖の第1有効量と、大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖のそれぞれの第2有効量とを対象に投与することを含む、腸外病原性大腸菌(ExPEC)に対する免疫応答を、それを要する対象において誘導する方法であって、ここで、第1有効量:第2有効量の比は1:1〜2:1であり、大腸菌O25B、O1A、O2およびO6A抗原多糖のそれぞれは、独立して、解毒された緑膿菌外毒素A(EPA)担体タンパク質に共有結合しており、第1有効量は投与当たり5〜18μgである、方法。
[10] 大腸菌O25B、O1A、O2およびO6A抗原多糖を1:1:1:1の投与量比で投与する、実施形態9に記載の方法。

[16] 免疫応答が、リスク保有ヒト対象においてExPEC血清型O1A、O2およびO6AおよびO25Bにより引き起こされる侵襲性ExPEC疾患の重症度を抑制し、または該疾患を予防する、実施形態8〜15のいずれかに記載の方法。
[17] 成人対象が、尿路感染、手術部位感染、腹部または骨盤感染、肺炎、院内肺炎、骨髄炎、蜂巣炎、敗血症、菌血症、創傷感染、腎盂腎炎、髄膜炎、新生児髄膜炎、腹膜炎、胆管炎、軟部組織感染、化膿性筋炎および敗血症性関節炎からなる群から選択される侵襲性ExPEC疾患を有する、または該疾患のリスクを有する、実施形態16に記載の方法。
[18] 大腸菌O25B抗原多糖、大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖を一緒にして組成物を得ることを含む、実施形態1〜7のいずれかに記載の組成物の製造方法。
Claims (15)
- 第1濃度の大腸菌O25B抗原多糖と、第2濃度の、大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖のそれぞれとを含む組成物であって、ここで、第1濃度:第2濃度の比は2:1であり、大腸菌O25B、O1A、O2およびO6A抗原多糖のそれぞれは、独立して、解毒された緑膿菌外毒素A(EPA)担体タンパク質に共有結合しており、第1濃度は10〜36μg/mlである、組成物。
- 大腸菌O25B、O1A、O2およびO6A抗原多糖を2:1:1:1の重量比で含み、前記第1濃度が10、16、24、32又は36μg/mlである、請求項1に記載の組成物。
- 16μg/mlのO25B抗原多糖を含む、請求項1又は2に記載の組成物。
- 32μg/mlのO25B抗原多糖を含む、請求項1又は2に記載の組成物。
- 5〜18μgの第1用量の大腸菌O25B抗原多糖と、それぞれ独立して第1用量の50%である用量の大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖とを含む多価免疫組成物であって、ここで、大腸菌O25B、O1A、O2およびO6A抗原多糖のそれぞれは、独立して、解毒された緑膿菌外毒素A(EPA)担体タンパク質に共有結合している、多価免疫組成物。
- 腸外病原性大腸菌(ExPEC)に対する免疫応答を、それを要する対象において誘導するために使用される、請求項1〜6のいずれか1項に記載の組成物。
- 第1有効量の大腸菌O25B抗原多糖と、第2有効量の大腸菌O1A抗原多糖、大腸菌O2抗原多糖および大腸菌O6A抗原多糖のそれぞれとを含む、腸外病原性大腸菌(ExPEC)に対する免疫応答を、それを要する対象において誘導するために使用される組成物であって、ここで、第1有効量:第2有効量の比は2:1であり、大腸菌O25B、O1A、O2およびO6A抗原多糖のそれぞれは、独立して、解毒された緑膿菌外毒素A(EPA)担体タンパク質に共有結合しており、第1有効量は投与当たり5〜18μgである、組成物。
- 大腸菌O25B、O1A、O2およびO6A抗原多糖を2:1:1:1の投与量比で投与するための組成物であって、前記第1有効量が、投与当たり6、8、10、12、14、16または18μgである、請求項8に記載の組成物。
- 投与当たり8μgのO25B抗原多糖を投与するための、請求項7〜9のいずれか1項に記載の組成物。
- 投与当たり16μgのO25B抗原多糖を投与するための、請求項7〜9のいずれか1項に記載の組成物。
- 大腸菌O25B、O1A、O2およびO6A抗原多糖を1つの組成物において一緒に投与するための、請求項7〜11のいずれか1項に記載の組成物。
