Description
人工心臓弁を運ぶためのカテーテルチューブ、人工心臓弁を圧縮状態で保持するためのカバーシース、人工心臓弁の第2の端部を保持するための保持手段、および保持手段への人工心臓弁の連結を維持するための維持手段を含み、
前記カテーテルチューブが、人工心臓弁システムの第1の端部を解放可能に保持するための第1の保持手段を有する先端部を含み、該先端部が人工心臓弁の第1の端部を解放するために遠位方向に移動可能であること、
前記カバーシースが、人工心臓弁の中間部を解放するために後退可能であること、
前記保持手段が、後退することのないよう送達システムに固定的に取り付けられ、かつ人工心臓弁の第2の端部と係合するよう設計されていること、および
前記維持手段が、人工心臓弁の第2の端部を解放するために後退可能であること
を特徴とする送達システムに関する。
本発明による拡張型人工心臓弁要素を装填した送達システムを提供する工程、
カバーシースを後退させ、それによって人工心臓弁の中間部を解放し、人工心臓弁要素の第1の端部が先端部内に固定されたままであり、かつ人工心臓弁要素の第2の端部が、保持手段への連結を維持する維持手段によって連結維持されたままであることにより人工心臓弁をバルーン様に拡張する工程、
カテーテル先端部を移動させて人工心臓弁の第1の端部を解放する工程、および
作動機構を作動させることによって維持手段を解放し、それによって人工心臓弁を完全に配置する工程
を含む方法に関する。
Claims (14)
- 人工心臓弁要素(20)を含む、患者の本来の弁を置換するための人工心臓弁システム(10;200;300)であって、
人工心臓弁要素(20)が、拡張可能な略管状のステント支持体(102)を含むこと、
ステント支持体(102)が、金属線フレームを形成し、かつ近位端(103)と、遠位端(107)と、近位端(103)から遠位端(107)に及ぶ長手軸(131)と、周囲(132)とを有すること、
ステント支持体(102)が、近位端(103)と遠位端(107)との間に、長手軸方向(131)に沿って平行に延びる、相互に接続されたダイヤモンド形のセル構造体(110)からなる隣接した複数の列を含むこと、
ステント支持体(102)が、本来の弁の弁輪のレベルに位置づけできるよう設計された、遠位端(107)と近位端(103)との間に位置する中間部(105)を有し、中間部(105)における直径が、遠位端(107)および近位端(103)における直径より小さいこと、ならびに
ステント支持体(102)が中間部(105)において、周方向に並んだ複数の金属線アンカー構造体(171)からなる列(170)を含み、金属線アンカー構造体(171)が、互いに離間して金属線フレームに取り付けられ、ステント支持体(102)の長手軸(131)から角度αで少なくとも部分的に外側に突出しており、人工心臓弁要素(20)が、外側シート(190)を含み、外側シート(190)が、複数の金属線アンカー構造体(171)からなる列(170)を含むステント支持体(102)の中間部(105)から遠位端(107)までを覆うよう延在しており、複数の金属線アンカー構造体(171)はステント支持体(102)の近位端(103)に向かっていることを特徴とする人工心臓弁システム(10;200;300)。 - 金属線アンカー構造体が外側に突出している角度αが、5〜90度であることを特徴とする、請求項1に記載の人工心臓弁システム(10;200;300)。
- 外側シート(190)が、布および生物組織から選択されるポリマー材料から作製されているか、またはこれらから選択されるポリマー材料を含むことを特徴とする、請求項1または2に記載の人工心臓弁システム(10;200;300)。
- 金属線アンカー構造体(171)が、第1の端部(182)および第2の端部(183)を有する単一の長手状金属線部(181)を意味し、第1の端部(182)が管状ステント支持体(102)に取り付けられており、第2の端部(183)が自由端として外側に突出していることを特徴とする、請求項1〜3のいずれかに記載の人工心臓弁システム(10;200;300)。
- 金属線部(181)の自由な第2の端部(183)が湾曲部(183a)を含み、該湾曲部が管状ステント支持体(102)の長手軸(131)に対して平行になるように、または管状ステント支持体(102)の長手軸(131)側を向くように曲げられていることを特徴とする、請求項4に記載の人工心臓弁システム(10;200;300)。
- 金属線アンカー構造体(171)が、V字形の金属線セル構造体(175)を表すことを特徴とする、請求項1〜3のいずれかに記載の人工心臓弁システム(10;200;300)。
- V字形の各金属線セル構造体(175)が2つの金属線セグメント(172、173)を有し、各金属線セグメントが第1の端部(172a、173a)と第2の端部(172b、173b)とを含み、V字形のセル構造体(175)が頂点(174)を外側に突出して形成されるよう、セグメントの第1の端部(172a、173a)が頂点(174)で結合し、第2の端部(172b、173b)が互いに一定の間隔を空けてステント支持体(102)の金属線フレームに取り付けられていることを特徴とする、請求項6に記載の人工心臓弁システム(10;200;300)。
- 金属線セグメント(172、173)の第2の端部(172b、173b)が、連結点(176)に取り付けられており、連結点(176)において、相互に接続されたダイヤモンド形のセル構造体(110)からなる隣接する列に含まれる2つの隣接するダイヤモンド形金属線セル構造体(110)が結合していることを特徴とする、請求項7に記載の人工心臓弁システム(10;200;300)。
- 人工心臓弁要素(20)がさらに、複数の弁葉(119)と、弁スカート(121)と、複数の弁交連点(120)とを有する弁構造体(118)を含み、弁構造体(118)が、管状ステント支持体(102)の内部表面(130)の近位部が弁構造体(118)で裏打ちされて密閉域を形成し、かつ弁構造体(118)が自体の交連点(120)を介してステント支持体(102)に固定されるように、ステント支持体(102)の内部表面(130)に取り付けられていることを特徴とする、請求項1〜8のいずれかに記載の人工心臓弁システム(10;200;300)。
- 人工心臓弁要素(20)が、その遠位端(107)の周囲において、V字形の開口部(159)を3つ含み、これらV字形またはU字形の開口部(159)が、ステント支持体(102)の遠位端(107)に向かって開いていることを特徴とする、請求項1〜9のいずれかに記載の人工心臓弁システム(10;200;300)。
- ステント支持体(102)の近位端(103)および遠位端(107)がそれぞれ、近位方向(202)および遠位方向(204)を向いた自由先端(112;134)を複数有するクラウン部を含み、ステント支持体の遠位端(107)が、そのクラウン部(135)において自由先端(112)を3つのみ有し、これらの先端がいずれも接続金属線延長構造体(114)を有することを特徴とする、請求項1〜10のいずれかに記載の人工心臓弁システム(10;200;300)。
- 人工心臓弁要素(20)に接続可能なステント要素(310)またはステントグラフト要素(310)をさらに含むことを特徴とする、請求項1〜11のいずれかに記載の人工心臓弁システム(10;200;300)。
- ステント要素(310)またはステントグラフト要素(310)が、該ステント要素(310)またはステントグラフト要素(310)に固定的に取り付けられている靱帯状の接続手段(312)を介して、人工心臓弁要素(20)に接続可能であることを特徴とする、請求項12に記載の人工心臓弁システム(10;200;300)。
- 靱帯状の接続手段(312)が、棒状に形成された柔軟性のある構造体を含むことを特徴とする、請求項13に記載の人工心臓弁システム(10;200;300)。
