Description
本願は、米国仮特許出願第62/101,666号(2015年1月9日出願、名称「Patient Remote And Associated Methods Of Use With A Nerve Stimulation System」、代理人事件番号97672−001400US)に対する優先権の利益を主張し、上記出願は、あらゆる目的のためにその全体が参照により本明細書に引用される。
本発明は、神経刺激治療システムおよび関連デバイス、ならびにそのような治療システムの治療、埋め込み、および構成の方法に関する。
本発明はさらに、例えば、以下を提供する。

本明細書に説明されるもののいずれか等の神経刺激(または以降で同義的に使用され得るような神経調節)治療システムは、急性疼痛障害、運動障害、情動障害、ならびに膀胱関連機能不全等の種々の病気および関連症状を治療するために使用されることができる。神経刺激によって治療され得る、疼痛障害の例は、脊椎手術後疼痛症候群、反射性交感神経性ジストロフィまたは複合性局所疼痛症候群、灼熱痛、クモ膜炎、および末梢神経障害を含む。運動障害は、筋麻痺、振戦、ジストニア、およびパーキンソン病を含む。情動障害は、うつ病、強迫性障害、群発頭痛、トゥレット障害、およびあるタイプの慢性疼痛を含む。膀胱関連機能不全は、OAB、切迫尿失禁、切迫頻尿、および尿閉を含むが、それらに限定されない。OABは、単独で、または組み合わせて、切迫尿失禁および切迫頻尿を含むことができる。切迫尿失禁は、突然の強い尿意(切迫性)に関連付けられる失禁である。切迫頻尿は、頻繁な、多くの場合、非常に少量の排尿をもたらす(頻尿)、頻繁で多くの場合は制御不能な尿意切迫感(切迫性)である。尿閉は、膀胱を空にすることができない状態である。神経刺激治療は、その状態もしくは関連症状に関連付けられる感覚および/または運動制御に関する標的神経組織の神経刺激を達成することによって、特定の状態に対処するように構成されることができる。
SNMは、切迫尿失禁、切迫頻尿、および非閉塞性尿閉の管理のための安全、効果的、可逆的、かつ長期継続的な治療選択肢を提供する確立された療法である。SNM療法は、下背に位置する仙骨神経を刺激するために、弱電気パルスの使用を伴う。電極は、電極導線を仙骨の対応する孔の中に挿入することによって、仙骨神経に隣接して、通常、S3レベルに配置される。電極は、皮下に挿入され、後に、埋め込み可能パルス発生器(IPG)に取り付けられる。切迫尿失禁および切迫頻尿の両方の患者のために、5年の耐久性を含む、OABの治療のためのSNMの安全性ならびに有効性が、複数の研究によって支持され、十分に証明されている。SNMは、より保守的治療に失敗した、またはその候補ではない患者における慢性便失禁を治療するためにも承認されている。
現在、SNMの資格は、試験段階を設けており、成功した場合、恒久的埋め込みが続く。試験段階は、療法が効果的であるかどうかを評価することを患者が許可される試験刺激期間である。典型的には、試験刺激を行うために利用される、2つの技法がある。第1は、経皮的神経評価(PNE)と称される通院ベースの手技であり、他方は、段階的試験である。
従来のアプローチは、膀胱関連機能不全の治療において有効性を示しているが、神経刺激導線の配置および導線の試験的位置と恒久的埋め込み位置との間の一貫性を向上させる必要性が存在する。神経刺激は、1つ以上の神経刺激電極を介して、パルス発生器から特定の神経もしくは標的領域に治療用刺激を一貫して送達することに依拠する。神経刺激電極は、患者組織内に形成されるトンネルを通して前進させられることができる埋め込み可能な導線の遠位端上に提供される。埋め込み可能な神経刺激システムは、患者に大幅な自由度および移動性を提供するが、外科的に埋め込まれる前に、そのようなシステムの神経刺激電極を調節することがより容易であり得る。医師は、IPGを埋め込む前に、患者が所望の運動および/または感覚反応を有することを確認することが望ましい。少なくともいくつかの治療(少なくともいくつかの形態の排尿および/または排便機能不全の治療を含む)に対して、適切な運動反応を実証することは、感覚反応が要求されないか、または利用不可能であり得る(例えば、患者が全身麻酔下にある)間、正確かつ客観的な導線配置のために非常に有益であり得る。
図1は、本発明の側面による、試験的神経刺激システム200および恒久的埋め込み式神経刺激システム100の両方を含む、例示的神経刺激システムを概略的に図示する。EPG80およびIPG10の各々は、臨床医用プログラム装置60および患者遠隔装置70と適合性があり、無線で通信し、臨床医用プログラム装置60および患者遠隔装置70は、試験的神経刺激システム200および/または試験成功後の恒久的埋め込み式システム100を配置すること、および/またはプログラムすることにおいて使用される。上で議論されるように、臨床医用プログラム装置は、導線配置、プログラミング、再プログラミング、刺激制御、および/またはパラメータ設定に役立つための特殊ソフトウェア、特殊ハードウェア、ならびに/もしくは両方を含むことができる。加えて、IPGおよびEPGの各々は、患者が、患者遠隔装置を用いて、刺激の少なくとも一部を制御すること(例えば、事前に設定されたプログラムを開始すること、刺激を増減させること)、および/またはバッテリステータスを監視することを可能にする。このアプローチは、試験的システムと恒久的システムとの間のほぼシームレスな遷移も可能にする。
患者が、電気パルス発生器の刺激レベルを調節することを可能にするように、患者遠隔装置(例えば、図1、要素70)が提供される。患者遠隔装置は、(例えば、試験段階中に)EPGまたは(例えば、恒久的神経刺激システムのために)IPGのいずれか一方と無線で通信し、それを制御するために使用されることができる。いくつかの実装では、異なる患者遠隔装置が、EPGおよびIPGを制御するように提供されることができる一方で、他の実装では、単一の患者遠隔装置が、EPGまたはIPGのいずれか一方を制御するようにプログラムもしくは再プログラムされることができる。特定の患者遠隔装置は、患者が他のものの刺激を変更することを回避するために、単一のEPGまたはIPGのみとリンクし、それと無線で通信するように構成されることができる。
Claims (19)
- 患者内の埋め込み可能な導線に結合されている神経刺激パルス発生器を無線で制御するように構成されている患者遠隔装置であって、前記患者遠隔装置は、
オペレータの片手によって操作可能であるように構成されている携帯用筐体と、
前記携帯用筐体内に配置されている回路であって、前記回路は、前記パルス発生器と無線で通信するように構成されている、回路と、
前記携帯用筐体の外面上に配置され、覚醒モードと休眠モードとの間で前記患者遠隔装置を再構成するために前記回路に結合されている起動ボタンと、
前記携帯用筐体の前記外面上に配置されている刺激増加ボタンであって、前記刺激増加ボタンは、前記パルス発生器の刺激レベルを無線で増加させるために前記回路に結合されている、刺激増加ボタンと
を備え、
前記患者遠隔装置が前記覚醒モードにあるとき、第1の期間にわたる前記刺激増加ボタンの作動は、前記パルス発生器に第1の命令信号を送信することを前記回路に行わせることにより、前記パルス発生器の前記刺激レベルを増加させるように構成されており、第2の期間にわたる前記刺激増加ボタンの作動は、前記パルス発生器に第2の命令信号を送信することを前記回路に行わせることにより、前記パルス発生器を最後に記憶された刺激レベルに自動的に復元させるように構成されており、前記第1の期間は、閾値期間よりも短い期間を含み、前記第2の期間は、前記閾値期間よりも長い期間を含む、
患者遠隔装置。 - 前記第2の期間にわたる前記刺激増加ボタンの作動は、前記刺激レベルを前記最後に記憶された刺激レベルまで自動的に増減させ、前記閾値期間は、3秒以上である、請求項1に記載の患者遠隔装置。
- 前記携帯用筐体の前記外面上に配置され、前記パルス発生器の前記刺激レベルを無線で減少させるために前記回路に結合されている刺激減少ボタンをさらに備え、前記患者遠隔装置が前記覚醒モードにあるとき、前記第1の期間にわたる前記刺激減少ボタンの作動は、前記刺激レベルを減少させ、前記第2の期間にわたる前記刺激減少ボタンの作動は、前記記憶された刺激レベルとしての後続の使用のために、現在の刺激レベルをメモリ要素に記憶し、前記パルス発生器による刺激をオフにする、請求項1〜2のいずれか1項に記載の患者遠隔装置。
- 前記回路は、前記刺激増加ボタンの反復作動が、公称刺激レベルを上回る4つ以下の刺激レベルに前記刺激レベルを徐々に増加させることができ、前記刺激減少ボタンの反復作動が、前記公称刺激レベルを下回る3つ以下の刺激レベルに前記刺激レベルを徐々に減少させることができるように構成されている、請求項3に記載の患者遠隔装置。
- 前記パルス発生器の各刺激レベル増加または刺激レベル減少は、公称刺激レベルまたは現在の刺激レベルの5パーセントより多くの増加または減少を含む、請求項3〜4のいずれか1項に記載の患者遠隔装置。
- 前記携帯用筐体の前記外面上に配置されている刺激レベルディスプレイをさらに備え、前記刺激レベルディスプレイおよび回路は、前記患者遠隔装置の前記起動ボタンが前記休眠モードから前記覚醒モードに切り替えられるとき、前記パルス発生器の現在の刺激レベルを示すように構成されている、請求項1〜5のいずれか1項に記載の患者遠隔装置。
- 前記刺激レベルディスプレイは、複数の発光ダイオードを備え、照らされた発光ダイオードの数は、前記パルス発生器の前記現在の刺激レベルを示す、請求項6に記載の患者遠隔装置。
- 前記刺激レベルディスプレイは、少なくとも3つまたは4つの異なるサイズの少なくとも7つの発光ダイオードを備え、公称刺激レベルは、最初の3つまたは4つの発光ダイオードの照明に対応する、請求項6〜7のいずれか1項に記載の患者遠隔装置。
- 前記携帯用筐体の前記外面上の治療残存ディスプレイをさらに備え、前記治療残存ディスプレイおよび回路は、少なくとも、前記パルス発生器のバッテリ内に残存する電荷または電圧と、前記患者による刺激使用とに基づいて、前記パルス発生器の治療残存ステータスを示すように構成されている、請求項1〜8のいずれか1項に記載の患者遠隔装置。
- 前記治療残存ディスプレイは、複数の対照的表示モードを有する発光ダイオードを備え、前記表示モードは、複数の色または点滅および非点滅照明、または両方を備え、前記表示モードは、前記パルス発生器が再充電を必要とするかどうか、充電しているかどうか、または少なくとも刺激の閾値日数のために十分な電荷を有するかどうかを示すために十分である、請求項9に記載の患者遠隔装置。
- 前記治療残存ディスプレイ発光ダイオードは、少なくとも4日の治療が残存していることを示すために非点滅緑色で照らし、2〜4日の治療が残存していることを示すために非点滅黄色で照らし、2日未満の治療が残存していることを示すために点滅黄色で照らす、請求項10に記載の患者遠隔装置。
- 前記携帯用筐体の前記外面上に配置される自動故障状態インジケータをさらに備え、前記自動故障状態インジケータは、前記パルス発生器が故障状態にある場合、アラートを提供するように構成されている、請求項1〜11のいずれか1項に記載の患者遠隔装置。
- 前記携帯用筐体に結合されている触覚インジケータをさらに備え、前記触覚インジケータは、前記患者遠隔装置からのコマンドが前記パルス発生器によって実行されたとき、振動するように構成されている、請求項1〜12のいずれか1項に記載の患者遠隔装置。
- 前記神経刺激パルス発生器は、外部または埋め込み可能パルス発生器を備え、前記埋め込み可能な導線は、仙骨神経の近傍の仙骨の孔の中に挿入するために構成されている少なくとも1つの電極を備えている、請求項1〜13のいずれか1項に記載の患者遠隔装置。
- 前記回路は、前記パルス発生器の前記刺激レベルが、前記パルス発生器の最大刺激レベル、公称刺激レベル、または現在の刺激レベルの所定の量ずつ徐々に増加もしくは減少させられるように構成されている、請求項1〜14のいずれか1項に記載の患者遠隔装置。
- 前記所定の量は、前記パルス発生器の最大刺激レベル、公称刺激レベル、または現在の刺激レベルの5〜20%の割合である、請求項15に記載の患者遠隔装置。
- 前記最大刺激レベルは、患者の快適さのレベルに従って設定される、請求項15〜16のいずれか1項に記載の患者遠隔装置。
- 前記パルス発生器の前記刺激レベルが設定された後、前記患者遠隔装置は、前記患者遠隔装置が前記パルス発生器による刺激を終了または変更するように前記オペレータによって動作させられるまで、前記刺激レベルを維持する、請求項1〜14のいずれか1項に記載の患者遠隔装置。
- 前記患者遠隔装置は、前記パルス発生器と対になり、そのパルス発生器のみと、および/またはそのパルス発生器と直接通信するように構成されている、請求項1〜14のいずれか1項に記載の患者遠隔装置。
