(1) 被検癌患者由来の検体中のmiR−181cレベルを測定することを備える、脳転移を診断するための情報を提供する方法。

ここで、前記被検癌患者由来の検体中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移を有するとの情報が提供される方法。
(3) 前記被検癌患者がステージIVの癌患者である、(1)又は(2)に記載の方法。
(4) 被検癌患者由来の検体中のmiR−181cレベルを測定することを備える、脳転移を診断するための情報を提供する方法。

該miR−181c測定手段にて測定された検体中のmiR−181cレベルを決定するmiR−181cレベル決定手段と、
該miR−181cレベル決定手段にて決定されたmiR−181cレベルから前記被検癌患者における脳転移を判定する転移判定手段と、
を備え、
前記転移判定手段は、前記被検癌患者由来の検体中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移を有すると判定することとした癌患者の脳転移判定装置。
(26) 被検癌患者における脳転移が生じるリスクを判定する装置であって、
前記被検癌患者由来の検体を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手段と、

当該コンピュータプログラムは、
前記被検癌患者由来の検体を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手順と、
該miR−181c測定手順にて測定された検体中のmiR−181cレベルを決定するmiR−181cレベル決定手順と、
該miR−181cレベル決定手順にて決定されたmiR−181cレベルから前記被検癌患者における脳転移を判定する転移判定手順と、
該転移判定手順による判定結果を出力する判定結果出力手順と、を癌患者の脳転移診断装置に実行させ、
前記転移判定手順は、前記被検癌患者由来の検体中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移を有すると判断することとしたコンピュータプログラム。
(28) 被検癌患者における脳転移が生じるリスク判定する装置にインストールされるコンピュータプログラムであって、
そのコンピュータプログラムは、
当該被検癌患者由来の検体を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手順と、
該miR−181c測定手順にて測定された検体中のmiR−181cレベルを決定するmiR−181cレベル決定手順と、
該miR−181cレベル決定手順にて決定されたmiR−181cレベルから前記被検癌患者における脳転移が生じるリスクを判定するリスク判定手順と、
該リスク判定手順による判定結果を出力する判定結果出力手順と、を癌患者の脳転移診断装置に実行させ、
前記リスク判定手順は、前記被検癌患者由来の検体中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移が生じるリスクが高いと判断することとしたコンピュータプログラム。

一態様において、本発明は被検癌患者由来の検体中のmiR−181cレベルを測定することを備える、脳転移を診断する方法、又は脳転移が生じるリスクを判定する方法に関する。一態様において、本発明は、被検癌患者における脳転移を診断する(又は、脳転移が生じるリスクを判定する)方法であって、前記被検癌患者由来の検体中のmiR−181cレベルを決定するステップ、決定されたmiR−181cレベルから前記被検癌患者における脳転移の有無(又は、脳転移が生じるリスク)を判定するステップを備え、ここで、前記被検癌患者由来の検体中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合に、前記被検癌患者は脳に転移していると判定する(又は、脳転移が生じるリスクが高いと判定する)方法に関する。
(a)少なくとも1つのmiR−181c(配列番号1)の塩基配列若しくはその一部と結合する核酸コンストラクト(プローブ)と被検癌患者由来の検体を接触させるステップ;

本発明者らが見出した癌転移メカニズムによれば、癌細胞から分泌されるEVs(エキソソームを含む)に含まれるmiR−181cが脳転移に寄与していることから、別の態様において、本発明は、癌患者の脳転移を治療又は予防する方法であって、前記癌患者におけるmiR−181cの発現又は活性を阻害することを備える方法に関する。本方法において、miR−181cの発現又は活性の阻害は、miR−181cの発現抑制剤又は活性阻害剤を、それを必要とする患者に投与することにより行うことができる。よって、ある態様において、本発明は、癌患者の脳転移を治療又は予防する方法であって、前記癌患者にmiR−181cの発現抑制剤又は活性阻害剤を投与することを備える方法に関する。
本発明者らは、PDPK1の発現又は活性が低下することにより血液脳関門の透過性が向上することを見出した。よって、別の態様において、本発明は、血液脳関門の透過性を亢進させる方法であって、血管内皮細胞におけるPDPK1の発現又は活性を阻害することを備える方法に関する。更に別の態様において本発明は、所望の活性成分を血液脳関門を通過させて脳内に到達させる方法であって、前記所望の活性成分をPDPK1の発現抑制剤又は活性阻害剤と共に投与することを備える方法に関する。
(脳転移判定装置)
一態様において、本発明は、miR−181cレベルを測定することによる脳転移判定装置に関する。図21に示すように、この実施の形態に係る脳転移判定装置1は、被検癌患者における脳転移を判定する装置である。より具体的には、脳転移判定装置1は、脳転移を有する可能性のある癌患者(通常、ステージIVの患者)が脳転移に至っているかを判定する装置である。脳転移判定装置1は、後述する測定部21、レベル決定部31および転移判定部32を備える。この実施の形態では、脳転移判定装置1は、測定部21を少なくとも備える入力装置2と、レベル決定部31および転移判定部32を少なくとも備える情報処理装置3と、出力装置4とを備える。ただし、入力装置2、情報処理装置3および出力装置4は、互いに分離した装置でなくとも良く、これら装置2,3,4の2以上が1つの装置となっていても良い。
別の態様において、本発明は、miR−181cレベルを測定することによる脳転移リスク判定装置に関する。図22に示すように、この実施の形態に係る脳転移リスク判定装置1aは、被検癌患者であって未だ脳転移に至っていない患者(通常、ステージI、IIまたはIIIの患者)が脳転移に至るリスクを判定する装置である。脳転移リスク判定装置1aは、測定部21、レベル決定部31および後述のリスク判定部33を備える。この実施の形態では、脳転移リスク判定装置1aは、先に説明した脳転移判定装置1と類似の構成を有し、入力装置2と、情報処理装置3と、出力装置4とを備える。ただし、脳転移リスク判定装置1aは、脳転移判定装置1と同様、入力装置2、情報処理装置3および出力装置4に分離された形態の装置でなくとも良い。入力装置2、情報処理装置3および出力装置4の内の2以上を1つの装置としていても良い。
一態様において、本発明は、上述の脳転移判定装置又は脳転移リスク判定装置において利用されるコンピュータプログラムに関する。
この実施の形態に係るコンピュータプログラムは、被検癌患者における脳転移を診断するための情報を提供する装置、すなわち、上述の脳転移判定装置1にインストールされるコンピュータプログラムである。当該コンピュータプログラムは、被検癌患者由来の検体23を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手順と、そのmiR−181c測定手順にて測定された検体23中のmiR−181cレベルを決定するmiR−181cレベル決定手順と、そのmiR−181cレベル決定手順にて決定されたmiR−181cレベルから被検癌患者における脳転移を判定する転移判定手順と、その転移判定手順による判定結果を出力する判定結果出力手順と、を癌患者の脳転移診断装置である脳転移判定装置1に実行させ、転移判定手順により、被検癌患者由来の検体23中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合には、被検癌患者が脳転移を有すると判断される。
別の実施の形態に係るコンピュータプログラムは、被検癌患者における脳転移が生じるリスクを判定する装置、すなわち、上述の脳転移リスク判定装置1aにインストールされるコンピュータプログラムである。当該コンピュータプログラムは、被検癌患者由来の検体23を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手順と、そのmiR−181c測定手順にて測定された検体23中のmiR−181cレベルを決定するmiR−181cレベル決定手順と、そのmiR−181cレベル決定手順にて決定されたmiR−181cレベルから被検癌患者における脳転移が生じるリスクを判定するリスク判定手順と、そのリスク判定手順による判定結果を出力する判定結果出力手順と、を癌患者の脳転移診断装置である脳転移リスク判定装置1aに実行させ、リスク判定手順により、被検癌患者由来の検体23中のmiR−181cレベルが陰性比較対象由来の検体中のmiR−181cレベルと比較して高い場合には、被検癌患者は脳転移が生じるリスクが高いと判断される。
本実施例記載の実験において、MDA−MB−231−luc−D3H1細胞、MDA−MB−231−luc−D3H2LN細胞、BMD2a細胞、及びBMD2b細胞の培養は、10%熱不活化ウシ胎児血清(Invitrogen)及び抗菌−抗真菌剤含有RPMI 1640倍地中、37℃、5%CO2環境下で行った。
腫瘍形成能及び転移能が高いヒト乳癌細胞であるMDA−MB−231−luc−D3H2LN細胞(以下、「D3H2LN細胞」という)(Bos、P.D.ら、Nature、459:1005−1009(2009))を用いて、新たに脳高転移細胞株を作製した(図1a)。具体的には、2×105個のD3H2LN細胞を含む細胞懸濁液100μLを、7週令のメス免疫不全マウスであるC.B−17/Icr−scid/scidマウスの左心室に心臓内注射で投与した。脳転移による腫瘍形成は、毎週、ルシフェリンを腹腔内注射して、IVIS Spectrum(Caliper Life Science、Hopkinton、MA、米国)を用いて、生物発光in vivoイメージングを行うことにより観察した。26〜30日後、癌細胞の脳への転移が観察された(図1b)。
(1)in vitro血液脳関門モデル
BBBへの影響を簡便に測定するため、in vitroのBBB培養システムを構築した。従来のin vitro血液脳関門モデルは単層の細胞培養系を用いている(Zhou、W.ら、J.Biol.Chem、288:10849−10859)。しかし、BBBは異なる三種類の細胞から構成されており、これらの細胞が互いに協調してBBBの構造を維持している。そこで、脳毛細血管内皮細胞、脳周皮細胞、及び星状膠細胞の初代培養からなるBBBモデル系であるBBBキット(TM)(#MTB−24H、ファーマコセル株式会社、長崎、日本)を用いた(図2a)。脳毛細血管内皮細胞、脳周皮細胞、及び星状膠細胞は、Hoechst33342染色により確認した。その結果、図2bに示すとおり、構築したin vitro血液脳関門モデルにおいて、脳毛細血管内皮細胞、脳周皮細胞、及び星状膠細胞が存在することがが確認された。
密着結合はBBBの透過性低下を調節しており、内皮細胞内の特定のタンパク質(クローディン−5、オクルジン、及びZO−1など)により構成されていることが知られている。一方で、カルシウム依存性細胞間接着糖タンパク質であり、5つの細胞外カドヘリンリピートを有することにより強力な細胞間接着を媒介するN−カドヘリンは、頂端膜及び基底膜に最も発現している。密着結合タンパク質及びN−カドヘリンはアクチン細胞骨格ネットワークとの密接な結合を介して細胞の極性を制御している。そこで、in vitro BBBモデルの内皮細胞中のクローディン−5、オクルジン、ZO−1又はN−カドヘリンを免疫蛍光染色して局在を確かめることにより、密着結合(tight junction)形成、及び接着結合(adherens junction)を確認した。
脳毛細血管内皮細胞による密着結合の形成を、経内皮電気抵抗(TEER)(Gaillard、P.J.ら、Eur.J.Pharm.Sci.、12:215−222(2001);Wilhelm、I.ら、Acta.Neurobiol.Exp.(Wars)、71:113−128(2011))により測定した。抵抗値(Ω)はohmmeter(Millicell ERS−2、Millipore)により測定した。経内皮電気抵抗(TEER)は以下の計算式により単位面積当たりの抵抗として算出した。R=(A−B)×0.33cm2(式中、RはTEER(Ω・cm2)、Aは測定された抵抗値(Ω)、Bはブランクの抵抗値(Ω)を示す)。n=12とした。
NaFは分子量が低い(分子量:376.27)にもかかわらず、BBBを通過しない(Nakagawa、Sら、Neurochem.Int.54:253−263(2009))。よって、NaFの濃度を蛍光単色光分光器で測定することにより、BBBの透過性を測定した。具体的には、透過性評価は、既に報告された方法(B. Kis et al.、 Adrenomedullin regulates blood−brain barrier functions in vitro. Neuroreport 12、 4139−4142 (2001))を改変して行った。チャンバーの上面側に200μLのNaF(10μg/mL、Sigma Aldrich)を添加し、チャンバーの下面側に900μLのDPBS−H(10mM HEPES、及び4.5mg/mL D−グルコース含有Dulbecco’s PBS(Mg+、Ca+))を添加した。プレートを37℃で振盪培養した。30分後、チャンバー下面側のDPBS−Hを黒色プレート内に分注し(n=8)、multi detection monochrometer microplate reader(485/535nm、SAFIRE、Tecan)で測定した。見かけの透過性係数(Papp)は次の計算式で算出した:

脳転移癌細胞は非常に侵襲的であると考えられる。そこで、マトリゲル(商標)を用いた浸潤チャンバーアッセイを用いて、樹立した細胞株の転移能に関する病理学的な意義を確認した。BMD2a細胞、BMD2b細胞、D3H2LN細胞、及びD3H1細胞をトリプシン処理し、PKH26(Sigma Aldrich)で標識化した。2×104細胞の各癌細胞を、血清非含有RPMI1640培地中に懸濁させ、マトリゲル(登録商標)コーティング膜を有するチャンバー(24穴インサート、BD Biosciences、NJ、米国)の上面側に添加した。チャンバーの下面側には、遊走因子として10%血清を含有するRPMI1640培地を添加した。細胞を24時間培養し、膜孔を通過又は侵入しなかった細胞を綿棒で除去した。膜の下面上の細胞をDiff−Quick Staining Set(Sysmex、兵庫、日本)を用いて染色し、計数した。全てのアッセイをトリプリケートで行った。データは、製造者の説明書に従い、コントロール膜を通過した細胞に対する、マトリゲル(商標)マトリックス及び膜を通過した細胞のパーセントで表示した。
BMD2a細胞及びBMD2b細胞の脳実質細胞側への進入を確認するため、BMD2a細胞、BMD2b細胞、D3H2LN細胞、及びD3H1細胞をトリプシン処理し、PKH26(Sigma Aldrich)で標識化した。その後、各2×104個の癌細胞を無血清のHam’s F−12培地(Gibco)中に懸濁させて、実施例2で作製したin vitro BBBモデルの上側チャンバーに添加して培養した。2日(48時間)後、非浸潤細胞を綿棒で除去し、核をHoechst33342(同仁化学研究所、熊本、日本)で染色した。その後、下面側に通過した浸潤細胞のPKH26を蛍光顕微鏡を用いて計数した(図4a)。全ての細胞について、トリプリケートで実験を行った。
D3H1細胞、D3H2LN細胞、BMD2a細胞、及びBMD2b細胞を、抗菌−抗真菌剤及び2mM L−グルタミン含有Advanced RPMI培地(以下、「培地A」という)で3回洗浄した。洗浄後の細胞に新鮮な培地Aを加えて2日間培養後、培地を回収して4℃、2000×gで10分間遠心分離した。細胞破片を完全に除去するため、培養上清を0.22μm Stericup(登録商標)(Millipore、MA、米国)フィルターでろ過した。得られた調整培地を、4℃、110、000×gで70分間超遠心にかけた。得られたペレットを11mLのリン酸緩衝生理食塩水(PBS)で洗浄し、PBS中に懸濁させた。
脳転移細胞の侵襲におけるEVsの関与を調べるため、EVの分泌に関与するタンパク質である、nSMase2及びRAB27Bに対するsiRNA(Kosaka、N.ら、J.Biol.Chem.、288:10849−10859(2013);Ostrowski、M et al.、Nat. Cell Biol.、12:19−30(2010))でEVの分泌が阻害されたBMD2a細胞を用いて、in vitro BBBモデルによる浸潤能評価を行った。
RAB27B siRNA及びnSMase2 siRNAのいずれか、又は両方のsiRNA、あるいは、コントロールsiRNAの各25nMを、DharmaFECTトランスフェクション試薬(Thermo Scientific)を使用して、製造者のプロトコルに従って、PKH26標識化BMD2a細胞に導入した。形質転換から24時間後の細胞をアッセイに使用した。
QIAzol試薬及びmiRNeasy Mini Kit(共にQuiagen)を用いて、細胞から全RNAを抽出した。内部標準として、RNU6を使用して記述の方法に従って行った(Kosaka、N.et al.Net.Med.、18:883−891(2012))。各mRNA及びRNU6の発現レベルは、96穴プレート内で、Platinum Quantitative PCR SuperMix(Applied Biosystems)を用いて、7300 Real−Time PCR System(全て、Applied Biosystems、CA、米国)でqRT−PCRを行うことにより測定した。全ての反応はトリプリケートで行った。プライマー及びプローブは以下のとおり定義した、PAB27B(アッセイID:Hs01072206_m1)、nSMase2(アッセイID:Hs00920354_m1)。
siRNA導入細胞中のnSMase2のタンパク質レベルでの発現をウェスタンブロッティングにより確認した。M−PER(Thermo Scientific、MA.米国)を用いて、Mini−PROTEAN(登録商標) TGX Gel(4−12%)(Bio−Rad)に各細胞から得られたサンプルをロードしてタンパク質を分離した後、PVDF膜(Millipore)に電気泳動転写した。得られた膜は、Blocking One(Nacalai Tesque、京都、日本)中でブロッキングした後、抗nSMase2抗体(H−195、sc−67305、1/200、Santa Cruz Biotechnology Inc.)である第一抗体と共に室温で1時間インキュベートした。第二抗体であるHRP−結合抗マウスIgG抗体(NA931、GE Healthcare)又はHRP結合抗ウサギIgG抗体(NA934、GE Healthcare)を1/2000倍希釈で用いた。その後、膜をImmunoStar(登録商標) LD(Wako、大阪、日本)で発光させた。
siRNA導入細胞よるEVs放出数をNanoSightを用いて測定した。
作製した形質転換細胞のマトリゲル侵襲能及びBBB通過を、それぞれ、実施例3及び実施例4記載の方法に従って調べた。
nSMase2 siRNA及びRAB27B siRNAのいずれかを導入した細胞、及びそれらのsiRNAの両方を導入した細胞では、コントロールsiRNA導入細胞と比較して、nSMase2及びRAB27Bの発現量がmRNAレベル及びタンパク質レベル共に低下していた(非図示)。siRNA導入細胞よるEVs放出数を測定した結果、nSMase2 siRNA及びRAB27B siRNAのいずれかを導入した細胞、及びそれらのsiRNAの両方を導入した細胞では、コントロールsiRNA導入細胞と比較して、EVsの放出数が減少していた(図6a)。BBB通過能試験の結果、コントロールsiRNAで処理した細胞はBBBを通過して下面側に移動したが、EVsの産生が阻害された細胞は、下面側では確認されなかった(図6c)。一方、マトリゲルを用いた侵襲能アッセイでは、EVsの分泌阻害は細胞の浸潤能に影響を与えなかった(図6b)。このことから、溢出は細胞の浸潤能のみに依存するものではないことが示唆された。
EVsが癌細胞の脳実質細胞側への溢出に関与するか否かを更に調べるため、転移能の低いD3H1細胞が、BMD2a、BMD2b又はD3H2LN細胞由来のEVsを添加することによりBBBを通過して溢出するか否かについて調べた。
脳転移癌細胞由来のEVsがBBBの破壊に直接影響を与えるか否かを決定するため、in vitro BBBモデルにEVsを添加して、BBBを破壊させるか否かを調べた。EVsは実施例5の記載に従い調製した。in vitro BBBモデルを解凍から4日後に、D3H2LN細胞、BMD2a細胞、及びBMD2b細胞由来の精製されたEV又はPBS(陰性コントロール)を添加した。EVsの添加前、及び添加から24時間後に、実施例2に記載の方法に従い、経内皮電気抵抗(TEER)を測定して脳毛細血管内皮細胞の密着結合の強度を測定した。全てのアッセイはトリプリケートで行った。
EVsがBBB構成細胞へ取り込まれるかを確かめるため、in vitro BBBモデルにEVsを添加して、BBB構成細胞における取り込みを調べた。全てのアッセイはトリプリケートで行った。
実施例4の方法により調製した、D3H1細胞、D3H2LN細胞、BMD2a細胞、及びBMD2b細胞由来の精製されたEVsをPKH67緑色蛍光ラベリングキット(Sigma Aldrich、MO、米国)を用いて標識化した。EVを2μMのPKH67と5分間反応させ、100kDaフィルター(Microcon YM−100、Millipore)を用いて5回洗浄して、余分な染料を除去した。
実施例8に記載の方法に従って、標識化したEVs又はPBS(陰性コントロール)をin vitro BBBモデルの上面側に添加して37℃、5%CO2環境下で24時間培養した。脳毛細血管内皮細胞、脳周皮細胞、及び星状膠細胞は、Hoechst33342染色により確認した。
BBB構成細胞へのEVsの取り込みを測定した結果を図9a及び図9bに示す。全ての癌細胞由来のEVsが内皮細胞に取り込まれたが、周皮細胞及び星状膠細胞への取り込みはほとんど見られなかった。BMD由来のEVsにおいて内皮細胞内の蛍光強度が最も強かったことから、BMD細胞由来のEVsの脳血管内皮細胞への指向性が明らかとなった。
EVsによるBBB破壊のメカニズムを調べるため、in vitro BBBモデルにEVsを添加して、BBB構成細胞内における分子挙動の変化を調べた。全てのアッセイはトリプリケートで行った。
実施例4の方法により調製した、D3H1細胞、D3H2LN細胞、BMD2a細胞、及びBMD2b細胞由来の精製されたEVs又はPBS(陰性コントロール)を添加して24時間後に、in vitro BBBモデルの内皮細胞中のクローディン−5、オクルジン、ZO−1又はN−カドヘリンをアクチンフィラメントと共に免疫蛍光染色して局在が変化するか否かについて、実施例2(2)と同様の方法を用いて調べた。
各細胞からのタンパク質の単離は、M−PER(Thermo Scientific、MA.米国)を用いて、Mini−PROTEAN(登録商標) TGX Gel(4−12%)(Bio−Rad)で分離し、PVDF膜(Millipore)に電気泳動転写することにより行った。得られた膜は、Blocking One(Nacalai Tesque、京都、日本)中でブロッキングした後、抗クローディン−5抗体(Z43.JK、1/200、Invitrogen)、抗オクルジン抗体(ZMD.481、1/200、Invitrogen)、抗ZO−1抗体(H−300、sc−10804、1/100、Santa Cruz Biotechnology)、抗N−カドヘリン抗体(3B9、1/500、Invitrogen)、又は、抗GAPDH抗体(6C5、1/1000、Millipore)である第一抗体と共に室温で1時間インキュベートした。第二抗体であるHRP−結合抗マウスIgG抗体(NA931、GE Healthcare)又はHRP結合抗ウサギIgG抗体(NA934、GE Healthcare)を1/2000倍希釈で用いた。その後、膜をImmunoStar(登録商標) LD(Wako、大阪、日本)で発光させた。
in vitro BBBモデルの内皮細胞中の密着結合タンパク質の局在を確認した結果を図10a及び図10bに示す。密着結合タンパク質及びN−カドヘリンは、PBS又はD3H2LN細胞由来のEVsで処理した内皮細胞の細胞膜表面上に局在化していた。しかし、BMD2a及びBMD2b由来EVsで処理した細胞では、密着結合タンパク質及びN−カドヘリンは細胞質内に存在していた。また、脳血管内皮細胞のBMD2a及びBMD2b由来EVsによる処理は、密着結合タンパク質、N−カドヘリン及びアクチンの発現には影響しなかった(図10c)。よって、癌由来EVsは、密着結合タンパク質、N−カドヘリン、及びアクチンフィラメントの発現には影響を与えることなく局在を変化させることが明らかとなった。
in vitro BBBモデル試験の結果から、癌細胞由来EVsが内皮細胞に取り込まれてBBBの透過性を高めていることが示されたことから、in vivoにおいても血液能関門へ影響するか否かを検討するため、in vivo透過性試験を行った。D3H2LN細胞(コントロール)及びBMD2a細胞由来の精製されたEVsをXenoLight DiR蛍光染色で標識化した。標識化したEVsをマウスの尾静脈に注入して6時間後、血液脳関門の崩壊を観察するため、in vivo血管透過性試験用色素であるTracer−653をマウスに注入した。
上述の実施例で確認されたEVsによるin vivoでの脳血管透過性の増大が、実際に脳転移に影響しているか否かを調べるため、EVs処理したマウスに癌細胞を注入して脳転移の有無を調べた。D3H2LN細胞(コントロール)又はBMD2a細胞由来の精製されたEVs(各5μg/匹)、又はPBS(ネガティブコントロール)を、各群9匹のC.B−17/Icr−scid/scidマウスの尾静脈に注入した(0日目)。24時間後(1日目)、D3H2LN乳癌細胞株(2×105細胞)を各マウスの左心室に注入した。18日後(19日目)にルシフェリンを腹腔内注射して、IVIS Spectrum(Caliper Life Science、Hopkinton、MA、米国)を用いて、生物発光in vivoイメージングを行うことにより、癌細胞の脳転移を観察した。得られた画像は、脳内の光子強度を測定することで評価した。有意差の評価はMann−Whitneyの片側検定により決定した。また、ヘマトキシリン及びエオリン(HE)染色、並びに、ヒトビメンチンに対する免疫蛍光染色により、脳転移を確認した。
EVsによる血液脳関門透過性変化の分子メカニズムを解明するため、D3H2LN細胞、BMD2a細胞、及びBMD2b細胞由来のEVsに含まれるmiRNAについて分析を行った。
以上の結果から、癌細胞由来EVsによるBBB破壊には、内皮細胞に取り込まれたEVsに含まれるmiR−181cが関与していることが示唆された。そこで、miR−181cのBBBへの影響を調べるため、EVs添加後の内皮細胞におけるmiR−181cの存在量を確認した。また、miR−181cを導入した内皮細胞を含むin vitro BBBモデルを用いて透過性及び密着結合タンパク質の挙動の変化を調べた。
BBBに与えるmiR−181cの影響を評価するため、BMD2a及びBMD2bから単離されたEVsを内皮細胞に添加し、miR−181cの存在量の変化を確認した。EVsは実施例5の記載に従い調製した。in vitro BBBモデルを解凍から4日後に、D3H2LN細胞、BMD2a細胞、及びBMD2b細胞由来の精製されたEV又はPBS(陰性コントロール)を添加した。EVs添加から24時間後に、内皮細胞中のmiR−181c発現を測定した。全てのアッセイはトリプリケートで行った。
更に、miR−181cの存在量の変化が内皮細胞に与える影響を調べるため、in vitro BBBモデルを構成する内皮細胞に合成miR−181cを導入した。経内皮電気抵抗(TEER)を、実施例2(3)に記載の方法に従って測定した。また、密着結合タンパク質、N−カドヘリン、及びアクチンの局在及び発現レベルを実施例10に記載の方法に従って検出した。陰性コントロールとして、miR−181cを導入していない内皮細胞を用いた。
BMD2a及びBMD2b由来EVs添加により内皮細胞におけるmiR−181cの存在量は顕著に増加していた(図14a)。miR−181cの形質導入により、in vitro BBBモデルのTEERの値を顕著に低下させた(図14b)。密着結合タンパク質、N−カドヘリン、及びアクチンは、陰性コントロール群では膜上に局在していたのに対し、miR−181cを形質導入した細胞においては、細胞質内に局在が変化していた(図14c)。更に、脳血管内皮細胞へのmiR−181cの形質導入は、密着結合タンパク質、N−カドヘリン、及びアクチンの発現量には影響を及ぼさないことが判明した(図14d)。
実際にmiR−181cが脳転移乳癌患者の血液中EVsに存在しているか否かを確認するため、脳転移乳癌患者の血清からEVsを精製し、miR−181cの発現を確認した。56人の乳癌患者(脳転移患者10人、非脳転移患者46人)からの血清を国立がん研究センターにおいて採取した(No.2013−111)。全ての患者について、インフォームドコンセントを得ている。血清は分注され、可能な限り凍結解凍を避けて、使用するまで−80℃で保存した。Total Exosome Isolation(from sera)(Invitrogen)を用いて、血清からEVsを単離した。その後、RNeasy Mini Kit(Qiagen)を用いて得られたEVsから全RNAを単離した。miRNAの発現は、既に報告された方法を用いて、内部標準として、RNU6を使用して、qRT−PCTにより測定した(Kosaka、N.et al.Net.Med.、18:883−891(2012))。即ち、miR−181c及びRNU6の発現レベルは、96穴プレート内で、TaqMan(登録商標) MicroRNA Assays、及びTaqMan(登録商標) Universal PCR Master Mixを用いて、7300 Real−Time PCR System(全て、Applied Biosystems、CA、米国)でqRT−PCRを行うことにより測定した。全ての反応はトリプリケートで行った。
miR−181cによるBBB崩壊メカニズムを解明するため、内皮細胞内のmiR−181cの標的を探索した。陰性コントロール又はmiR−181cを形質導入した内皮細胞内の遺伝子発現、並びに、BMD2a、BMD2b又はD3H2LN細胞由来のEVsを添加後の内皮細胞内の遺伝子発現を分析した。
PDPK1が脳血管内皮細胞内に与える影響を調べるため、PDPK1 siRNAで処理した後の径内皮電気抵抗(TEER)及び密着結合タンパク質、N−カドヘリン、及びアクチンフィラメントの局在及び発現量を調べた。
更に、内皮細胞内においてmiR−181cがPDPK1の発現を直接抑制し得るか否かについて、PDPK1遺伝子の非翻訳領域(3’UTR)(Macaca:配列番号14;Human:配列番号15)を用いた、3’UTRルシフェラーゼレポーターアッセイにより確認した。

PCR産物をpGEM−T Easy Vector(Promega、WI、米国)にクローニングして組み込んだ。増幅させた3’UTRは、psiCHECK−2TM(Promega)のウミシイタケルシフェラーゼコード領域の下流にクローニングした。また、アラインメント解析により、PDPK1の3’UTR中にmiR−181cと相補的な配列が確認された(図19b)。
PDPK1は、コフィリン(cofilin)のリン酸化を制御する経路の上流に位置するタンパク質の一つであることが知られている(Lyle、A.N、et al.、Physiology(Bethesda)、21:269−280(2006);Higuchi、M.、et al.、Nat.Cell Biol.、10:1356−1364(2008))。また、コフィリンは、アクチン結合タンパク質ファミリーの一つであり、脱リン酸化により活性化されてアクチンフィラメントを分解する。PDPK1 siRNA処理内皮細胞内において、アクチンの凝縮が観察されたことから、miR−181cによるBBB破壊にコフィリンが関与しているか否かについて検討した。
Claims (18)
- 被検癌患者由来の血液中のmiR−181cレベルを測定することを備える、脳転移を診断するための情報を提供する方法。
- 被検癌患者における脳転移を診断するための情報を提供する方法であって、
前記被検癌患者由来の血液中のmiR−181cレベルを決定するステップ、
決定されたmiR−181cレベルから前記被検癌患者における脳転移を判定するステップを備え、
ここで、前記被検癌患者由来の血液中のmiR−181cレベルが陰性比較対象由来の血液中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移を有するとの情報が提供される方法。 - 前記被検癌患者がステージIVの癌患者である、請求項1又は請求項2に記載の方法。
- 被検癌患者由来の血液中のmiR−181cレベルを測定することを備える、脳転移を診断するための情報を提供する方法。
- 被検癌患者における脳転移が生じるリスクを評価するための情報を提供する方法であって、
前記被検癌患者由来の血液中のmiR−181cレベルを決定すること、
決定されたmiR−181cレベルから前記被検癌患者における脳転移が生じるリスクを判定するステップを備え、
ここで、前記被検癌患者由来の血液中のmiR−181cレベルが陰性比較対象由来の血液中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移が生じるリスクが高いとの情報が提供される方法。 - 前記被検癌患者がステージI〜IIIの癌患者である、請求項4又は請求項5に記載の方法。
- 前記被検癌患者が乳癌患者である、請求項1〜請求項6のいずれか1項に記載の方法。
- 前記陰性比較対象が、脳への転移が無い癌患者又は健常人である、請求項2又は請求項5に記載の方法。
- 血液中のmiR−181cが、血液中のEVsから抽出されたmiR−181cである、請求項1〜請求項8のいずれか1項に記載の方法。
- 請求項1〜請求項9のいずれか1項に記載の方法に用いるための、miR181cを測定することによる、脳転移の診断又はリスク評価に供する体外診断用試薬又は体外診断用測定器。
- miR−181cと特異的に結合する核酸を備える、脳転移の診断又はリスク評価のための薬剤。
- 前記miR−181cと特異的に結合する核酸が、少なくとも一部に人工的に設計された配列を有する核酸である、請求項11に記載の薬剤。
- miR−181cと特異的に結合する核酸が、標識化されていることを特徴とする、請求項11又は請求項12に記載の薬剤。
- miR−181cの発現抑制剤、又はmiR−181cの活性阻害剤を有効成分として含有する、脳転移を抑制するための医薬組成物であって、
miR−181cの発現抑制剤又はmiR−181cの活性阻害剤が、以下から選択される薬剤である、医薬組成物:
(i)pri−miR−181c又はpre−miR−181cに対するアンチセンスオリゴヌクレオチド、アプタマー、又はsiRNA;
(ii)miR−181cのアンチセンスオリゴヌクレオチド、miR−181cと特異的に結合するアプタマー、miR−181cに対するsiRNA、又はmiR−181cのmiRNA mimetics。 - 被検癌患者における脳転移を判定する装置であって、
当該被検癌患者由来の血液を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手段と、
該miR−181c測定手段にて測定された血液中のmiR−181cレベルを決定するmiR−181cレベル決定手段と、
該miR−181cレベル決定手段にて決定されたmiR−181cレベルから前記被検癌患者における脳転移を判定する転移判定手段と、
を備え、
前記転移判定手段は、前記被検癌患者由来の血液中のmiR−181cレベルが陰性比較対象由来の血液中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移を有すると判定することとした癌患者の脳転移判定装置。 - 被検癌患者における脳転移が生じるリスクを判定する装置であって、
前記被検癌患者由来の血液を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手段と、
該miR−181c測定手段にて測定された血液中のmiR−181cレベルを決定するmiR−181cレベル決定手段と、
該miR−181cレベル決定手段にて決定されたmiR−181cレベルから前記被検癌患者における脳転移が生じるリスクを判定するリスク判定手段と、
を備え、
前記リスク判定手段は、前記被検癌患者由来の血液中のmiR−181cレベルが陰性比較対象由来の血液中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移が生じるリスクが高いと判定することとした癌患者の脳転移リスク判定装置。 - 被検癌患者における脳転移を診断するための情報を提供する装置にインストールされるコンピュータプログラムであって、
当該コンピュータプログラムは、
前記被検癌患者由来の血液を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手順と、
該miR−181c測定手順にて測定された血液中のmiR−181cレベルを決定するmiR−181cレベル決定手順と、
該miR−181cレベル決定手順にて決定されたmiR−181cレベルから前記被検癌患者における脳転移を判定する転移判定手順と、
該転移判定手順による判定結果を出力する判定結果出力手順と、を癌患者の脳転移診断装置に実行させ、
前記転移判定手順は、前記被検癌患者由来の血液中のmiR−181cレベルが陰性比較対象由来の血液中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移を有すると判断することとしたコンピュータプログラム。 - 被検癌患者における脳転移が生じるリスク判定する装置にインストールされるコンピュータプログラムであって、
そのコンピュータプログラムは、
当該被検癌患者由来の血液を用いてmiR−181cの塩基配列またはその一部を有するポリヌクレオチドを測定するmiR−181c測定手順と、
該miR−181c測定手順にて測定された血液中のmiR−181cレベルを決定するmiR−181cレベル決定手順と、
該miR−181cレベル決定手順にて決定されたmiR−181cレベルから前記被検癌患者における脳転移が生じるリスクを判定するリスク判定手順と、
該リスク判定手順による判定結果を出力する判定結果出力手順と、を癌患者の脳転移診断装置に実行させ、
前記リスク判定手順は、前記被検癌患者由来の血液中のmiR−181cレベルが陰性比較対象由来の血液中のmiR−181cレベルと比較して高い場合には、前記被検癌患者は脳転移が生じるリスクが高いと判断することとしたコンピュータプログラム。
