ヒトHCC発がんメカニズムを獲得した実験モデルは、HCCの治療を評価し、患者の応答性を予測するバイオマーカーを発見するのに重要である。患者由来の異種移植片(PDX)モデルは、患者の組織病理学的及び遺伝的プロファイルを反映する(2−7)。本発明者らは、最近、免疫不全マウスに、未治療の患者の腫瘍組織片を移植することにより、HCC−PDXモデル(HCC−HuPrime(登録商標)または患者のアバターと命名)の大規模コレクションを確立した。本発明者らは、臨床試験のような試験において、HCC−HuPrimeの無作為化コホートでソラフェニブを試験した。試験は、「応答者及び非応答者」の同定をもたらした。本発明者らは、次に、マイクロアレイGeneChip技術を使用して、これらのモデルを発現プロファイリングした。統計分析を適用することにより、本発明者らは、応答に関連付けられ、またはHuSignature(登録商標)と呼ばれる特定の分子シグネチャを同定した。少数の遺伝子だけからなる遺伝子発現シグネチャはHCC患者のソラフェニブに対する応答性を予測することができ、診療所における患者の層別化に使用するためのコンパニオン診断薬(companion diagnostics)を開発するのにも受け入れられる。このシグネチャは、おそらく応答者でないHCC患者の治療を回避しながら、おそらく応答者であるHCC患者を治療するためのガイドとして使用することができ、それ故、個別化を最小としつつ治療の利点は最大となる。
HCCは、多様な種類の疾患である。患者の腫瘍試料の網羅的遺伝子発現プロファイリングは、HCCが3つの主要なサブタイプに分類できることを明らかにした。最近、トランスクリプトーム解析により、別個の臨床パラメータ、並びに細胞分化を用いて、HCCが3つの主要なカテゴリーに分類された1。それらは、S1(WNT経路及びTGF−βの活性化を伴う幹様グループ)、S2(MYC及びAKTの活性化)、及びS3(肝細胞分化)である。HCC−PDXは、患者の元の組織病理学及び遺伝的プロファイルを維持することにより、予測的実験モデルであると考えられる2,3。本発明の試験は、HCC−アバターが分類及び生物学的特性における患者の腫瘍と同様のゲノムプロファイルを有しているかどうかを示すことにより、この仮説を試験することを試みるものであった。
線形回帰に基づく統計的手法を、上述したような%ΔT/ΔCによるように、HCC−PDXにおける、網羅的遺伝子発現レベル、及び測定されたソラフェニブの治療効果を使用する、予測的バイオマーカー(すなわち、遺伝子シグネチャ)を同定するために使用した。0.0001未満のp値、4倍を超える、試験を行った全体のHCC−PDXの遺伝子発現の範囲、線形回帰分析において異常値のないことを含む、前もって調整した統計的基準により、本発明者らは、良好な予測性を実証した8種の遺伝子からなるシグネチャを引き出すことができる(例えば、図1及び表3)。より多くの遺伝子を有するシグネチャは良好な関連性及び小さいp値を生成するが、臨床適用においては実用的価値を有していないことに注目すべきである。あらかじめ調整した基準の厳密さは、シグネチャ遺伝子の数を決定するであろう。
免疫不全マウスにおける、患者の腫瘍試料及び移植
新鮮かつ外科的に切除した腫瘍組織は、河北医科大学第四病院の治験審査会の承認及び患者からのインフォームドコンセントを得て、北京Keluoenトランスレーショナル医学研究所及び河北医科大学第四病院との共同研究とを通じてHCCと診断された患者から得た。患者の腫瘍断片の免疫不全マウス皮下への移植は、他者により広く記載されている。手短に言うと、腫瘍を3×3×3mm3の断片にスライスし、マウス(Balb/c nude,6〜8週齢、メス,Beijing HFK Bioscience Co. Ltd.,Beijing,China)の側腹部に皮下接種した。腫瘍の成長は、ノギスを用いることにより、週に2回モニターした。継代0またはP0と呼ばれる、これらの患者試料由来の確立された腫瘍モデルを、腫瘍を維持するために連続的に再移植し、それらの引き継ぐ継代をP1、2、3・・・(10未満)と呼ぶ。腫瘍の大きさが500〜700mm3(長さの1/2×幅2)に達すると、移植の次のラウンドのために腫瘍を集め、腫瘍を継代し、薬理学、組織病理学、免疫組織学、細胞及び分子的分析の試験を実施した。全ての手順は、Crown Bioscience SPF設備で、無菌条件下に実施した。実験動物が関与する全ての試験は、国立衛生研究所の実験動物のケア及び使用のためのガイドの推奨に従い、厳重に実施した。プロトコルは、Crown Bioscience,Inc.の動物実験の倫理に関する委員会(Crown Bioscience IACUC Committee)によって承認された。
腫瘍体積が100〜150mm3に達すると、マウスを、同様の平均腫瘍体積を有する5匹のマウスの2つの群に無作為に分類した。対照群は、分類後すぐに、媒体対照(PBS、週1回、腹腔注射、2週間)で処理し;処置群は、以下のうちの1つで処置した:セツキシマブ(週1回、腹腔注射、2週間、1mg/マウス、Merck KGaA)、エルロチニブ(1日1回、経口投与、50mg/kg、Nanjing Angel Pharmaceutical Co.)、クリゾチニブ(1日1回、経口投与、50mg/kg、Selleckchem.com)。腫瘍の成長を週に2回モニターし、処置に対する腫瘍応答性を評価するために、%ΔT/ΔC値を計算した(ΔT=処置群の腫瘍体積の変化、ΔC=対照群の腫瘍体積の変化)。
HuPrimeモデル由来の腫瘍組織を分析するために、標準的な免疫組織化学的検査を用いた。手短に言うと、組織を、10%の中性緩衝化したホルマリンに固定し、標準的な組織学的手順に従ってパラフィンに包埋した。脱パラフィン及び再水和後、3μm厚の組織切片を、0.01Mクエン酸ナトリウム溶液、pH6.0中、95℃で30分間、前処理し、ウサギ抗−ヒトモノクローナルpERKまたはpEGFR抗体(Cell Signaling,Boston,USA)で染色した。Ultra Vision LP large Volume Detection System HRP Polymer (Ready−To−Use) Kit (Lab Vision,Fremont,CA)を用いて、陽性染色を検出した。発色基質としてDABを使用し、切片を、ギルのヘマトキシリン(Fisher Scientific,Fair Lawn,USA)で対比染色した。次いで、試験片を、以下の基準に従って、盲検様式で、3名の研究者により独立に点数をつけた:0、染色なし;1+、最小の染色;2+中程度の染色;3+、強い染色。低倍率(×100)で腫瘍切片をスキャンすることにより、最も強度の強い領域を特定した後、Olympus BX51顕微鏡システムを用いて、DP71デジタルカメラ(Olympus,Melville,NY)により、高倍率(×400)で画像を撮影した。
新鮮なHCC HuPrime(商標)腫瘍組織を腫瘍を有するマウスから収集し、急速凍結し、遺伝的及びゲノム分析に使用するまで−80℃に保存した。遺伝子プロファイリング分析のために、製造業者の説明書に従い、Trizol(Invitrogen,Carlsbad,CA)を用いて凍結組織から全RNAを分離し、RNeasyミニカラム(Qiagen)を用いて精製した。RNAの質は、Bioanalyzer(Agilent)により評価した。高い質(RINが8超)のRNA試料のみを、標準プロトコル(GeneChip(登録商標)3'IVT Expression Kit,User Manual,Affymetrix,P/N 702646 Rev.8)に従って、Affymetrix HG−U219アレイプレートによる発現プロファイリングアッセイに使用した。全ての試料の生のCELデータセットを、RMAアルゴリズムにより標準化した。プローブセットの強度は、Log(2)交換値として表した。Affymetrix(登録商標)SNP6.0チップを用いたSNP/CNVアッセイのために、製造業者の説明書に従い、Genomic DNA Tissue and Blood Isolation Kit(Qiagen)を用いてゲノムDNAを単離し、精製した。標準的なAffymetrixプロトコル(Affymetrix(登録商標)Genome−Wide Human SNP Nsp/Sty 6.0 User Guide,P/N 702504,Rev.4)に従い、DNAプロセシング及びチップハイブリダイゼーションを実施した。生のCELデータはQC−edであり、低いコールレートの試料を除くために選別し、遺伝子コピー数分析を、PICNIC及び/またはPennCNV法により実施した。いくつかの試料については、相対的な遺伝子コピー数をqPCRにより測定した。手短に言うと、同じゲノムDNAを、定量PCRに基づくSYBR Greenにより、MET特異的プライマー
を用いて増幅に供した。基準として、哺乳類のLINE−1レトロトランスポゾン遺伝子を用いた。生データを生成するために、Opticon Monitor 3ソフトウェアを用い、クロモ4システムによりq−PCRデータを分析した。次いで、デルタCT相対的定量法を用いて生データを加工した。ΔCT=(標的遺伝子のCT値)−(基準遺伝子のCT値)。次いで、デルタCT値を強度値に変換した(POWER(ΔCT,−2))。相対的METコピー数を得るために、公知のMETコピー数を用いて、全てのデータを、試料のデータに標準化した。
肝細胞がん(HCC)は異種のがんである。承認されている唯一の標的療法は、単独療法として、または化学療法と併用した、ソラフェニブである。しかし、今までのところ、治療には、総合的な利益において限定的であった。しかし、実際に治療から利益を得ることができる可能性の高い応答者と、不必要な毒性を回避することができる可能性が高い非応答者を識別することができる患者の層別化が欠如しているので、マルチキナーゼ阻害剤のように、これは、高い毒性及び全体的な低い有効性によるところが大きい。従って、個々に応じた治療計画を作成することで、全体的な治療の利益は向上するであろう。残念なことに、今までのソラフェニブの臨床診療は、個々の治療をサポートするために効果的なバイオマーカーを明らかにはしていなかった。
ソラフェニブ、スニチニブ、アキシチニブ、バンデタニブ、パゾパニブ、及びカボザンチニブの効果は、それぞれが媒体を投与する両方の対照群で、複数のマウス、少なくとも3匹を有する、HCC PDXのパネルで測定される。治療群は、ソラフェニブ、スニチニブ、アキシチニブ、バンデタニブ、パゾパニブ、またはカボザンチニブが投与される。
Claims (9)
- 対象における疾患を治療するためのマルチキナーゼ阻害剤に対する対象の応答性判定のためのキットの製造における遺伝子マーカーのパネルの使用であって、
前記遺伝子マーカーのパネルはSEC14L2、H6PD、TMEM140、SLC2A5、ACTA1、IRF8、STAT2、及びUGT2A1からなる群から選択される少なくとも1種以上のマーカーを含み、
前記パネルの前記活性プロファイルが測定され、かつ疾患および治療に対する応答性を有する対象の集団における活性プロファイルと比較され、
前記対象における1種以上のマーカーが、前記疾患および治療への応答性を有する対象の集団における活性と同様の活性を有することが、対象がマルチキナーゼ阻害剤に対して応答性であることを示し、かつ
前記マルチキナーゼ阻害剤が、血管内皮細胞増殖因子(VEGF)が介在する血管形成を標的とし、かつRAF/細胞外シグナル調節キナーゼ(ERK)キナーゼ(MEK)/ERKカスケードを阻害する、前記使用。 - 前記遺伝子マーカーのパネルの活性プロファイルが、遺伝子発現レベル、RNA活性レベル、またはタンパク質活性レベルを含む、請求項1に記載の使用。
- 前記パネルが、少なくとも2種、3種、または4種の遺伝子マーカーを含む、請求項1に記載の使用。
- 前記活性プロファイルが遺伝子発現レベルである、請求項1に記載の使用。
- 前記マルチキナーゼ阻害剤が、ソラフェニブ、スニチニブ、アキシチニブ、バンデタニブ、パゾパニブ、カボザンチニブ、またはそれらの混合物である、請求項1に記載の使用。
- 前記対象が、肝細胞がんを有するヒトである、請求項1に記載の使用。
- 必要な対象を治療するための医薬の製造におけるマルチキナーゼ阻害剤の使用であって、
SEC14L2、H6PD、TMEM140、SLC2A5、ACTA1、IRF8、STAT2、及びUGT2A1からなる群から選択される少なくとも1種以上のマーカーを含む遺伝子マーカーのパネルの活性プロファイルについて対象を試験する段階、
前記パネルの前記活性プロファイルを、疾患および治療に対する応答性を有する患者の集団の活性プロファイルと比較する段階 を含み、
前記対象における1種以上のマーカーが、前記疾患および治療への応答性を有する対象の集団における活性と同様の活性を有する場合に、前記対象に前記マルチキナーゼ阻害剤が投与され、かつ
前記マルチキナーゼ阻害剤が、血管内皮細胞増殖因子(VEGF)が介在する血管形成を標的とし、かつRAF/細胞外シグナル調節キナーゼ(ERK)キナーゼ(MEK)/ERKカスケードを阻害する、前記使用。 - 遺伝子マーカーSEC14L2、H6PD、TMEM140、SLC2A5、ACTA1、IRF8、STAT2、及びUGT2A1の検出のためのプローブからなる、対象における疾患を治療するためのマルチキナーゼ阻害剤に対する対象の応答性判定のためのアレイ。
- 前記対象における1種以上のマーカーが、疾患および治療への応答性を有する対象の集団における活性と同様ではない活性を有する場合に、前記対象が前記マルチキナーゼ阻害剤治療に応答性でないと判定される、請求項1に記載の使用。
