Description
課題は、癌バイオマーカーを発見することである。分子的に標的化された作用物質を用いて、慢性骨髄性白血病、消化管間質腫瘍、肺癌、および多形性膠芽腫などのいくつかの腫瘍タイプの分子的に規定されたサブセットを標的化することに、臨床的に成功してはいるが、このような成功をさらに広範な状況において適用する能力は、患者において標的作用物質を評価するための効率的な戦略を欠いていることによって非常に限られている。問題は、主に、これらの興味深い新たな薬剤を評価するための臨床試験に、分子的に規定された癌を有する患者を選択することが不可能であることにある。解決のためには、特定の作用物質が有益である可能性が最も高い患者を確実に同定するバイオマーカーが必要である(Sawyers、2008、Nature 452:548〜552、548)。

本明細書において用いられる場合、Inlyta(登録商標)、「AG−13736」、および「アキシチニブ」は、その塩および多形を含む、以下の化学構造を有する6−[2−(メチルカルバモイル)フェニルスルファニル]−3−E−[2−(ピリジン−2−イル)エテニル]インダゾールを意味する。

Inlyta(登録商標)は、以下ではアキシチニブと呼ぶが、血管内皮増殖因子受容体(VEGFR)に作用する、経口的に投与される低分子受容体チロシンキナーゼ阻害剤である。アキシチニブは、血管新生を阻害することによって腫瘍の成長および転移を低減させ、かつ、これらの受容体を発現する細胞およびこれらの受容体に依存性の細胞に直接的に作用することによって、腫瘍の成長を低減させ、退縮を生じさせることが期待されている。アキシチニブは、サイトカインまたはスニチニブでの疾患の増悪またはそれらに対する耐性があった後の転移性腎細胞癌(mRCC)の治療について、多くの国で認可されている。
ヒト患者における腫瘍の組織試料を、RNAの供給源、タンパク質の供給源、または免疫組織化学(IHC)のための薄片の供給源として用いることができ、したがって、試料におけるCD68発現のレベルを、本開示において記載されているように決定することができる。組織試料は、従来の腫瘍生検機器および手順を用いて得ることができる。内視鏡生検、切除生検、切開生検、細針生検、パンチ生検、薄片生検、および皮膚生検が、腫瘍試料を得るために当業者によって用いられ得る、認識されている医療手順の例である。腫瘍組織試料は、マーカー遺伝子、例えばCD68発現レベルを測定するために、または、IHCによって、例えばCD68陽性細胞の発現によってマクロファージを可視化するために十分なRNA、タンパク質、または薄片を提供するために、十分な量でなくてはならない。
本開示の実施において、組織試料(例えば、腫瘍から得た)における、マクロファージ含有量のレベル(例えば、マクロファージの数、またはCD68などのマクロファージマーカーの発現、例えば、マクロファージマーカータンパク質の発現、またはCD68などのマクロファージマーカータンパク質をコードするmRNAの発現)の決定は、いくつか存在するあらゆる適切な方法によって行うことができる。例えば、マクロファージ含有量の間接的な測定は、CD68などのマクロファージマーカーとして有用であることが知られている1つまたは複数の遺伝子の発現を測定することによって行うことができる。遺伝子発現を測定するための様々な方法が、当技術分野において知られている。このような方法は、マクロファージマーカータンパク質またはマクロファージマーカータンパク質をコードするmRNAのレベルの決定において適用することができる。典型的なヒトマクロファージマーカー遺伝子は、CCR2、CD14、CD68、CD163、CSF1R、およびMSR1である。他のマクロファージマーカーも同様に用いることができる。
従来のマイクロアレイ分析および定量的ポリメラーゼ連鎖反応(QPCR)は、マクロファージマーカー遺伝子の発現のレベルをmRNAのレベルで決定するための方法の例である。本開示のいくつかの実施形態において、RNAは、標準的なプロトコルを用いて、目的の細胞、腫瘍、または組織から抽出される。他の実施形態において、RNA分析は、RNAの単離を要しない技術を用いて行われる。

CD68などのマクロファージマーカータンパク質をコードする1つまたは複数の遺伝子のmRNA発現レベルは、従来のDNAマイクロアレイ発現プロファイリング技術を用いて測定することができる。DNAマイクロアレイは、ガラス、プラスチック、またはシリコンなどの固体表面または固体基質に固定された特異的なDNAセグメントまたはプローブの集まりであり、各特異的DNAセグメントが、アレイ内の既知の位置に存在している。通常はストリンジェントなハイブリダイゼーション条件下の、標識されたRNAの試料とのハイブリダイゼーションによって、アレイ内の各プローブに対応するRNA分子の検出および定量が可能になる。非特異的に結合した試料材料を除去するためのストリンジェントな洗浄の後、マイクロアレイを、共焦点レーザー顕微鏡法または他の適切な検出方法によってスキャンする。DNAチップとして知られていることが多い、最新の市販のDNAマイクロアレイは、何万ものプローブを典型的には含み、したがって、何万もの遺伝子の発現を同時に測定することができる。このようなマイクロアレイは、本開示の実施において用いることができる。あるいは、CD68などのマクロファージマーカータンパク質をコードする1つまたは複数の遺伝子の発現を測定するために必要なだけのプローブと、例えばデータの正規化のための必要な対照または標準とを含む、カスタムチップを、本開示の実施において用いることができる。
CD68などのマクロファージマーカータンパク質をコードする個別の遺伝子を代表するmRNAのレベルを、従来の定量的逆転写酵素ポリメラーゼ連鎖反応(qRT−PCR)技術を用いて測定することができる。qRT−PCRの利点には、感度、柔軟性、定量的正確度、および緊密に関連するmRNAの間を区別する能力が含まれる。定量的PCRのための組織試料の処理に関連するガイダンスは、qRT−PCRのための商品の製造者および販売者(例えば、Qiagen(バレンシア、カリフォルニア州)およびAmbion(オースチン、テキサス州))を含む、様々な供給源から入手可能である。qRT−PCRの自動性能のための機器システムは市販されており、多くの実験室において日常的に用いられている。周知の市販のシステムの例は、Applied Biosystems 7900HT Fast Real−Time PCR System(Applied Biosystems、フォスターシティー、カリフォルニア州)である。
本開示の方法において、CD68などのマクロファージマーカー遺伝子の発現は、タンパク質レベルで検出することができる。タンパク質レベルでマクロファージマーカー遺伝子の発現のレベルを測定するための方法の例には、酵素結合免疫吸着アッセイ(ELISA)およびIHC分析が含まれる。

IHCバイオマーカー分析では、少なくとも1用量の研究治療を受けた全患者を含む、バイオマーカー分析セットを用いた。PFS、OS、および客観的奏効率(ORR)という有効性評価項目を分析した。要約統計量を、応答カテゴリー(各マーカーについて、完全奏効[CR]+部分奏効[PR]対、安定状態[SD]+増悪[PD]))によって、陽性細胞のパーセントおよび密度について提供した。ウィルコクソンの順位和検定を行って、応答カテゴリー間の差について試験した。フィッシャーの正確確率検定を用いて、中央値をカットオフ点として用いて、応答カテゴリーとバイオマーカー層との間の関連について試験した。OSおよびPFSの分布を、カプラン・マイヤー法を用いて、カットオフ点としてのバイオマーカーの中央値によって、バイオマーカー層の間で比較し、p値は、いずれの層においても、N<10である場合には提示されなかった。推定ハザード比(HR)およびその両側の95%信頼区間(CI)、ならびに中央値イベント時刻およびその両側の95%CIが報告された。腫瘍容積における最良の応答パーセントの変化を、ウィルコクソンの順位和検定を用いて、カットオフ点としての中央値を用いて、バイオマーカー層の間で比較した。
Claims (22)
- アキシチニブでの治療に対して感受性である腫瘍を同定する方法であって、(a)アキシチニブでの治療を考慮されているヒト患者から得られた腫瘍の組織試料における、CD68ポリペプチド発現レベルを測定するステップ、ならびに(b)ステップ(a)におけるCD68発現レベルを、アキシチニブで事前に治療されアキシチニブに対して耐性であることが示されているヒト患者、およびアキシチニブで事前に治療されアキシチニブに対して感受性であることが示されているヒト患者から得られた腫瘍の組織試料におけるCD68ポリペプチド発現を測定することによって決定された閾値CD68発現レベルに対して比較するステップを含み、CD68発現レベルが閾値レベルを上回る場合、腫瘍がアキシチニブでの治療に対して感受性であることが示される、方法。
- CD68ポリペプチド発現を測定するステップが、免疫組織化学によって行われる、請求項1に記載の方法。

- 対象の腫瘍におけるCD68陽性細胞の細胞密度を決定し、前記細胞密度が少なくとも0.08細胞/mm2であるときの、癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 対象の腫瘍における細胞の少なくとも2%、少なくとも3%、少なくとも4%、少なくとも4.5%、少なくとも4.6%、少なくとも4.7%、少なくとも4.8%、少なくとも4.9%、少なくとも5.0%、少なくとも5.1%、少なくとも5.2%、少なくとも5.3%、少なくとも5.4%、少なくとも5.5%、少なくとも5.6%、少なくとも5.7%、少なくとも5.8%、少なくとも5.9%、少なくとも6.0%、少なくとも6.5%、少なくとも7.0%、少なくとも8%、少なくとも9%、少なくとも10%、少なくとも15%、または少なくとも20%がCD68陽性である、癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 対象の腫瘍における細胞の少なくとも5%がCD68陽性である、請求項10に記載された癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 対象の腫瘍におけるCD68陽性細胞の細胞密度が少なくとも0.05細胞/mm2、少なくとも0.06細胞/mm2、少なくとも0.07細胞/mm2、少なくとも0.08細胞/mm2、少なくとも0.09細胞/mm2、少なくとも1.0細胞/mm2、少なくとも1.1細胞/mm2、少なくとも1.2細胞/mm2、少なくとも1.3細胞/mm2、少なくとも1.4細胞/mm2、または少なくとも1.5細胞/mm2である、癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 対象の腫瘍におけるCD68陽性細胞の細胞密度が少なくとも0.08細胞/mm2である、請求項12に記載された癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 対象の腫瘍における細胞の少なくとも2%、少なくとも3%、少なくとも4%、少なくとも4.5%、少なくとも4.6%、少なくとも4.7%、少なくとも4.8%、少なくとも4.9%、少なくとも5.0%、少なくとも5.1%、少なくとも5.2%、少なくとも5.3%、少なくとも5.4%、少なくとも5.5%、少なくとも5.6%、少なくとも5.7%、少なくとも5.8%、少なくとも5.9%、少なくとも6.0%、少なくとも6.5%、少なくとも7.0%、少なくとも8%、少なくとも9%、少なくとも10%、少なくとも15%、または少なくとも20%がCD68陽性であり、かつ、腫瘍におけるCD68陽性細胞の細胞密度が少なくとも0.05細胞/mm2、少なくとも0.06細胞/mm2、少なくとも0.07細胞/mm2、少なくとも0.08細胞/mm2、少なくとも0.09細胞/mm2、少なくとも1.0細胞/mm2、少なくとも1.1細胞/mm2、少なくとも1.2細胞/mm2、少なくとも1.3細胞/mm2、少なくとも1.4細胞/mm2、または少なくとも1.5細胞/mm2である、請求項10または12に記載された癌の治療のための薬剤の製造におけるアキシチニブの使用。

- a)対象の腫瘍におけるCD68陽性細胞のパーセンテージを決定し、およびb)腫瘍におけるCD68陽性細胞の細胞密度を決定し、前記パーセンテージが少なくとも5%であり、かつ、前記細胞密度が少なくとも0.08細胞/mm2である、請求項15に記載された癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 対象の腫瘍における細胞の少なくとも5%がCD68陽性であり、かつ、腫瘍におけるCD68陽性細胞の細胞密度が少なくとも0.08細胞/mm2である、癌の治療のための薬剤の製造におけるアキシチニブの使用。
- 前記腫瘍が、乳房腫瘍、肺腫瘍、腎臓腫瘍、結腸直腸腫瘍、および膵臓腫瘍からなる群から選択される、請求項8〜17のいずれか一項に記載の使用。
- 腫瘍が、mRCCである、請求項18に記載の使用。
- CD68陽性細胞のパーセンテージまたは細胞密度を測定するステップが免疫組織化学を用いて行われる、請求項8、9、15および16のいずれか一項に記載の使用。
- CD68陽性細胞のパーセンテージまたは細胞密度を測定するステップが、免疫組織化学およびホールスライドスキャンからの画像分析を用いて行われる、請求項20に記載の使用。
- a)定量的逆転写酵素ポリメラーゼ連鎖反応(qRT−PCR)に基づくキット[ここで定量的逆転写酵素ポリメラーゼ連鎖反応(qRT−PCR)に基づくキットは、マクロファージマーカーCD68の発現を分析するためのPCRプライマーだけでなく、緩衝液、試薬、およびPCR技術を用いてCD68発現レベルを測定するための詳細な指示を含む]
b)DNAマイクロアレイに基づくキット[ここで、DNAマイクロアレイに基づくキットが、特定の機器と共に用いるように設計されたマイクロ流体カード(アレイ)を含む]、または
c)IHCによって腫瘍のマクロファージ含有量を決定するための材料、
の構成要素を含む、請求項1〜6のいずれか一項に記載の方法を行うための診断試験キット。
