Description

多発性骨髄腫
多発性骨髄腫(骨髄腫)は、形質細胞の骨髄悪性腫瘍である。異常な形質細胞の集団が骨髄に凝集し、そこでそれらは正常な血液細胞の産生を妨げる。骨髄腫は、米国において2番目に一般的な血液悪性腫瘍であり(非ホジキンリンパ腫の次)、血液悪性腫瘍の13%を、および全てのがんの1%に相当する。この疾患は、死の前に病的骨折、感染への感受性、腎臓そして骨髄の機能不全を引き起こすことから、苦痛と医療出費の点で負担となる。
BCMA(TNFRSF17としても公知)は、専らB系列造血細胞または樹状細胞において発現する、形質細胞特異的表面抗原である。これはTNF受容体ファミリーの一員である。BCMAはナイーブB細胞では発現されないが、形質細胞芽へのB細胞分化の間にはアップレギュレートされ、記憶B細胞、形質細胞芽、および骨髄形質細胞では明瞭に発現される。BCMAは、初代骨髄腫細胞の大部分においても発現される。樹状細胞において検出される低レベルのmRNAを除き、BCMA発現は、他の組織において消失しているようであり、これは多発性骨髄腫に対する新規の治療のための標的としての可能性を示す。

本発明は、本発明のバリアントAPRILを含む、形質細胞を検出するための診断薬剤もまた提供する。
初代骨髄腫細胞を、明らかに疾患を有することが知られている多発性骨髄腫患者由来の、新鮮な骨髄サンプルでのCD138免疫磁気的選択を行うことによって単離した。これら細胞は、PEと結合体化されたBCMA特異的J6MO mAb(GSK)により染色した。同時に、既知の数の結合部位を有するビーズの標準が、PE Quantibrite bead kit(Becton Dickenson)を使用して、製造者の指示通りに生成した。骨髄腫細胞でのBCMAコピー数は、骨髄腫細胞からの平均蛍光強度と、ビーズより得られた標準曲線とを相関させることによって得ることができた。骨髄腫細胞表面でのBCMAコピー数の範囲は低く:細胞あたり348.7〜4268.4 BCMAコピーであり、平均値が1181、中間値が1084.9(図2)であることが見出された。これは、CARへの古典的な標的である、例えばCD19およびGD2などより相当低い。初代骨髄腫細胞でのBCMA発現の存在は、Vicky−1抗体(Abcam Ab17323)によっても確認され、この例は図18に示される。
天然形態におけるAPRILは、分泌型II型タンパク質である。CARのためのBCMA結合ドメインとしてのAPRILの使用、およびこのタンパク質を安定に、かつこの形態でのBCMAへの結合を保持するためには、このII型分泌タンパク質のI型膜結合タンパク質への転換を必要とする。候補分子を生成するために、APRILの先端のアミノ末端を欠失させて、プロテオグリカンへの結合を除去した。次に、新生タンパク質を小胞体へ、そしてそこから細胞表面へと指向させるために、シグナルペプチドを付加した。また、使用したスペーサーの性質が、CARの機能を変化させ得るため、3つの異なるスペーサードメインを試験した:(i)Fc結合モチーフを除去するために変化させたヒトIgG1スペーサー;(ii)CD8ストーク;および(iii)IgG1ヒンジ単独、を含むAPRILに基づくCARが生成された(図4に漫画および図5にアミノ酸配列、ならびに図19にも異なるシグナルペプチドおよびV5エピトープタグを有する点で、図5に示される配列とは異なるアミノ酸配列)。これらCARは、マーカータンパク質(短縮型CD34)が、便利なマーカー遺伝子として共発現できるように、バイシストロニックなレトロウイルスベクター(図6A)に発現させた。
本研究の目的は、構築されたAPRILに基づくCARが、細胞表面に発現するか否か、およびAPRILが本来のタンパク質を形成するようにフォールディングされるか否かを試験することであった。T細胞にこれらの異なるCARコンストラクトを形質導入し、そして市販の抗APRIL mAbを使用した染色と同時にマーカー遺伝子についての染色を行い、そしてフローサイトメトリーによって解析した。この実験の結果は、図6Bに示され、ここでAPRIL結合は、マーカー遺伝子の蛍光に対してプロットしている。これらデータは、この形式においてAPRILに基づくCARは細胞表面に発現しており、かつAPRILは、抗APRIL mAbにより認識されるのに十分にフォールディングされることを示す。
正常ドナー由来T細胞に、異なるAPRIL CARを形質導入し、そして野生型、またはBCMAおよびTACIを発現するように改変した、いずれかのSupT1細胞に対して試験した。機能を決定するために、数種の異なる分析を使用した。古典的なクロム放出分析を行った。ここで、標的細胞(SupT1細胞)を、51Crを用いて標識し、そしてエフェクター(形質導入されたT細胞)と、異なる比率で混合した。標的細胞の溶解は、共培養上清中の51Crを計数することにより、決定した(図6Aは累積のデータを示し、異なるエフェクター:標的比での単独の分析からのデータ例は、図16に示される)。
上記データは、原理的にはAPRILに基づくCARを作製することができることを実証しているため、励まされる。しかしながら、ほとんどの原発性骨髄腫細胞は、その表面において少数のBCMA分子を発現するため、そのようなAPRILに基づくCARが、特に低密度発現の症例において、原発性骨髄腫細胞の殺傷を引き起こすか否かについて調べた(investiagated)。図2において記載されたBCMA発現の範囲に相当する3症例を選択した:第一は不明瞭な発現を有する(平均より低い);第二の症例は中間的な発現を有する(ほぼ平均の発現)、そして第三は明瞭な発現を有した(平均の発現よりも上)。図8は、BCMA発現を説明するために、左に3症例全てについてのアイソタイプ対照に対するBCMA染色のヒストグラムを示す。異なるスペーサーを有するAPRILに基づくCARを比較したときに、CD8ストークのスペーサーおよびIgG1ヒンジスペーサーを有するCARが、Fc−pvaaで間隔を開けられたCARよりも良好に挙動することが決定されたため、この分析においてはCD8ストークおよびヒンジのAPRIL CARのみを試験した。左に、骨髄腫細胞とCAR T細胞の1:1共培養後3日および6日での、開始数と比較した骨髄腫細胞の生存を示す。6日までに、不明瞭なBCMA発現を有する骨髄腫細胞を含む、95%超の骨髄腫細胞が排除された。不明瞭にBCMAを発現する骨髄腫細胞は、高度に発現するものよりも殺傷の速度は遅いが、APRIL CARによって標的とされ得る。
本発明者らは、図24Aに示されるように、OKT3由来のscFvをAPRILの細胞外ドメインへと連結させる一連の二重特異性誘導体を構築した。数種の設計の検討を、これらの分子の構築の間に行った:(a)プロテオグリカンに結合するAPRILのアミノ末端を、非特異的結合を防ぐために短縮化した、(b)コンストラクト4、5および6において、シグナルペプチドをAPRILの成熟エクトドメインに結合させた、(c)OKT3を、重鎖可変領域と軽鎖可変領域とを連結させるリンカーを含むscFvとして再構成した、(d)多様な異なるスペーサーを、scFvとAPRILとの間で試行した。

コンストラクト(3)および(6)は、CD8スペーサーにおけるジスルフィド結合を通じてホモダイマーを形成する。コンストラクト(1)、(3)および(6)についてのアミノ酸配列は、図30に示される。
293 T細胞に、上記に列挙されるAPRILiTEコンストラクトをコードする発現プラスミドをトランスフェクションした。293T細胞由来の上清をアクリルアミドゲル上で流し、タンパク質を膜に転写した。その後、膜を、APRILを認識する抗体を用いて染色した。この結果は、図25に示される。タンパク質1、3および6が、予想される分子量において検出された。タンパク質2、4および5は検出されず、これは、これらの構造が不安定であることを示す。
その後、これらのタンパク質が、一方の末端でT細胞受容体(TCR)と、および他方の末端でBCMAとのいずれにも結合できるか否かを検証した。トランスフェクションした293T細胞由来の上清を、Jurkat T細胞およびTCRαβノックアウトを有するJurkat T細胞クローンを染色するために使用した。これは、APRILiTEが、TCRに結合することを実証する(図26b)。その後、BCMAを発現するように改変したSupT1細胞およびTACIを発現するように改変したSupT1細胞を、二次抗APRILビオチン次いでストレプトアビジンPEを使用して、上記の上清で染色した。この結果は、図26aに示される。APRILiTE1、3および6はBCMAに結合し、そしてTACIにはより少ない程度で結合したことが見出された。
正常ドナーT細胞を、異なるSupT1と1:1で培養した。使用したSupT1は、形質導入なし、BCMAを発現するように改変したものまたはTACIを発現するように改変したもののいずれかであった。この結果は、図27に示される。T細胞は、いずれかのAPRILiTEの存在下で、BCMAまたはTACIで改変したSupT1細胞を用いて曝露した場合にのみIFNγを放出することが見出された。BCMAに対する応答は、TACIに対する応答よりも大きかった。
T細胞を、APRILiTE1、3および6の非存在下または存在下で、野生型SupT1細胞と、BCMAを発現するSupT1細胞とおよびTACIを発現するSupT1細胞と1:1で培養した。この結果は、図28に示される。残存T細胞は、APRILiTEの添加なしの条件で存在するSupT1細胞の比率として示される。
4種の異なる骨髄腫サンプルを、ラット抗ヒトBCMA mAb Vicky1を用いて染色した。この結果は、図29に示される。臨床的および形態的に典型的な骨髄腫(パネル2〜4)において、中間的または不明瞭な染色が見られる。
既知の多発性BCMA+骨髄腫を有する2人の患者由来の診断用骨髄吸引物由来の残りの材料を使用した。CD138磁気ビーズ選択を、吸引物から骨髄腫細胞を精製するために行った。これらの細胞を、完全培養培地において48時間静置し、BCMAについての染色を、実際にBCMA陽性であることを確認するために行った。骨髄腫細胞はBCMAを発現するが、低レベルであることが見出された(図31)。
huSCIDモデルを使用した:NSG(nod−scidガンマ、NOD−scid IL2Rガンマnull)マウスに典型的なレベルのBCMAを発現する骨髄腫細胞株を異種移植する。これらの系統を、生物発光イメージングによって疾患を測定するために、ホタルルシフェラーゼを発現するように改変する。正常ドナーPBMCを、付随するAPRILiTEの腹腔内投与の間に尾静脈を介して投与する。以下を順に測定する(1)APRILiTEの血清レベル、(2)ヒトインターフェロンガンマの血清レベル、(3)フローサイトメトリーによる末梢血T細胞の増加量、生着および活性化、(4)腫瘍の生物発光測定。次に、以下を測定する:(1)骨髄の組織学による腫瘍の負荷、(2)骨髄、脾臓、血液およびリンパ節のフローサイトメトリーによるT細胞の増殖および生着および(3)残りの組織を、いずれかの毒性について肉眼および免疫組織学的に調べる。
目的は、CARのためにより適切であり得る結合を有するAPRIL変異体を生成することであった。Hymowitz et al,2004,The Journal of biological chemistry:Volume 280;Issue 8;Pages 7218−27によって記載された結晶データならびにRCSBデポジット1XU1および1XU2からの結晶データを使用して、BCMAへの結合を変化させ得るか、またはTACIよりもBCMAに対する特異性を上昇させ得る数種の残基を選択した。
CAR形式(すなわち、膜貫通ドメインに融合し、そして細胞膜に固定した)における短縮型APRILが、BCMAおよびTACIに結合できるか否か調べるために、基本的なCARを、自己切断する口蹄疫2Aペプチドを用いて、便利なマーカー遺伝子としての短縮型CD34と共に、インフレームで改変した。このコンストラクトを発現する安定なSUPT1細胞株を樹立した。ヒト(および他種、示されない)Ig Fcドメインに融合した分泌型の短縮型BCMAおよびTACIもまた生成し、そしてリコンビナントタンパク質を産生した。BCMA−FcとTACI−Fcとの両方が、改変されたSUPT1細胞株に結合することが示された。CD34マーカー遺伝子を発現する細胞のみが、BCMA−FcおよびTACI−Fcに結合することが見出された(図9)。
CARスペーサードメインは、感度および特異性を変化させ得る。3つのスペーサードメインを有する、3つの型のAPRILに基づくCARが生成された:(i)Fc結合モチーフを除去するために改変したヒトIgG1スペーサー;(ii)CD8ストーク;および(iii)IgG1ヒンジ単独(図14B)。初代ヒトT細胞にこれらの異なるCARを形質導入し、そして市販の抗APRIL mAbを使用して染色した(図15)。
通常ドナー由来T細胞に、異なるAPRIL CARを形質導入し、そして野生型、またはBCMAおよびTACIを発現するように改変したSupT1細胞に対して試験した。機能を決定するために、数種の異なる分析を使用した。古典的なクロム放出分析を行った。ここで、標的細胞(SupT1細胞)は51Crを用いて標識し、そして異なる比率でエフェクター(形質導入されたT細胞)と混合した。標的細胞の溶解は、共培養上清中の51Crを計数することによって決定した(図16)。
APRIL変異体を、特定のコドンを標的とする縮重プライマーを使用して生成した。これらのコドンを、APRIL−BCMA結合およびAPRIL−TACI結合のインシリコ解析を通じて同定した。この解析から、TACI結合に関係するようであるが、BCMA結合には関係しないようである残基を標的とした。
インビボでのAPRIL CAR T細胞機能を実証するために、ヒト/マウスキメラモデルにおいてAPRIL CAR T細胞を試験した。MM1.s(ATCC CRL−2974)は、中間的なレベルのBCMAを発現するヒト骨髄腫細胞株である。本発明者らは、細胞株MM1.s.FLucを得るために、この細胞株をホタルルシフェラーゼを発現するように改変した。
球を移植することのできる、重度免疫不全マウスである。3ヶ月齢メスNSGマウスに、いずれの予備治療もなしに、尾静脈注射を介して1×107のMM1.s.FLuc細胞を与えた。移植は、一連の生物発光イメージングによって決定した(図23)。全てのマウスにおいて、強固であり、かつ増加する髄内移植が観察された。13日目に、5×106のAPRIL−HNG−CD28OXZ CAR T細胞を、尾静脈注射により投与した。一連の生物発光を行い、これは全ての処置されたマウスにおいて、MM1.s負荷量における急速な減少から完全な寛解を示した(図23)。このCAR治療への応答は、フローサイトメトリーおよび免疫組織化学によって確認した。
4種の正常ドナーのPBMCを、WT APRILまたは多様な変異体のいずれかに基づく異なるBiTESの存在下で、SupT1細胞と、BCMAを発現するように改変したSupT1細胞と、TACIを発現するように改変したSupT1細胞と、または単独でインキュベートした。インターフェロンガンマレベルを24時間後に測定した。この結果は、図35に示される。変異体M200Gは、野生型よりも顕著に改善したBCMA対TACI特異性を示す。
Claims (18)
- 野生型APRILよりも高いBCMAに対する結合アフィニティーを有し、かつ/または野生型APRILと比較して変化した結合動態を有し、かつ/または野生型APRILよりも高いBCMA:TACI(膜貫通型活性化因子およびカルシウム調節因子およびシクロフィリンリガンド相互作用因子)結合比率を有し、かつ以下の単一変異:M200C、M200L、M200G、M200S、M200A、M200Nの1つを含む、バリアント増殖誘導リガンド(APRIL)。
- 以下の変異の組み合わせ:
V174TおよびM200G、またはT175AおよびM200G、またはV174T、T175AおよびM200G、またはM200G、D205SおよびR206H
の1つを含む、請求項1に記載のバリアントAPRIL。 - 変異M200Gを含む、請求項1に記載のバリアント増殖誘導リガンド(APRIL)。
- 抗原結合ドメイン、膜貫通ドメインおよびエンドドメインを含むキメラ抗原受容体(CAR)であって、該抗原結合ドメインが、請求項1〜3のいずれかに記載のバリアントAPRILを含む、キメラ抗原受容体。
- 抗原結合ドメインおよびT細胞活性化ドメインを含む二重特異性T細胞誘導体(BiTE)であって、該抗原結合ドメインが、請求項1〜3のいずれかに記載のバリアントAPRILを含む、二重特異性T細胞誘導体。
- 請求項1〜3のいずれかに記載のバリアントAPRIL、請求項4に記載のキメラ抗原受容体または請求項5に記載の二重特異性T細胞誘導体をコードする核酸。
- 請求項6に記載の核酸を含む、ベクター。
- 請求項4に記載のキメラ抗原受容体を含む、細胞。
- 請求項8に記載の細胞を作製するための組成物であって、キメラ抗原受容体をコードする核酸を含む、請求項7に記載のベクターを含み、該細胞は、該ベクターで形質転換またはトランスフェクションされる、組成物。
- 形質細胞障害を処置するための組成物であって、請求項8に記載の細胞または請求項5に記載の二重特異性T細胞誘導体を含む、組成物。
- 形質細胞障害を処置するための医薬品の製造における、請求項8に記載の細胞または請求項5に記載の二重特異性T細胞誘導体の使用。
- 形質細胞を検出するための組成物であって、請求項1〜3のいずれかに記載のバリアントAPRILを含む診断薬剤を含む、組成物。
- 形質細胞障害を診断するための請求項12に記載の組成物。
- インビボで被験体における形質細胞障害を診断するための組成物であって、請求項1〜3のいずれかに記載のバリアントAPRILを含む、組成物。
- 被験体における形質細胞障害を診断するための組成物であって、請求項1〜3のいずれかに記載のバリアントAPRILを含み、インビトロで該被験体由来のサンプルに添加されることを特徴とする、組成物。
- 前記サンプルが、血液サンプルであるか、または血液サンプルに由来する、請求項15に記載の組成物。
- 前記形質細胞障害が、形質細胞腫、形質細胞性白血病、多発性骨髄腫、マクログロブリン血症、アミロイドーシス、ワルデンストレーム型マクログロブリン血症、孤立性骨形質細胞腫、髄外性形質細胞腫、骨硬化性ミエローマ、H鎖病、意義不明の単クローン性免疫グロブリン血症およびくすぶり型多発性骨髄腫より選択される、請求項10または14〜16のいずれかに記載の組成物。
- 前記形質細胞障害が、多発性骨髄腫である、請求項17に記載の組成物。
