上述の連続グルコース監視(CGM)システムでは、2つの特有な検知技術を使用する直交冗長性により、環境の影響が明らかになることを可能にしながら、精度および信頼性の向上が得られる。具体的には、精度に関して、本明細書に記載の発明の実施形態では約13%のMARDを可能にする。この関連で、既存の血糖計(すなわち、フィンガースティック)家庭用モデルは、一般に高精度、すなわち、およそ9%のMARDを有することが期待され、ISO 15197:2003から見ると全点の95%が正確であると予測されることを理解されたい。後の方の規格によれば、ある計器は、それが、試験された試料の少なくとも95%について以下の基準に適合する場合に正確であると考えられる。すなわち、(1)75mg/dL未満の血糖レベルでは、モニタ表示値は基準の15mg/dL以内でなければならない、(2)75mg/dL以上の表示値では、モニタ表示値は基準表示値の20%以内でなければならない。
直交冗長性はまた、どちらの検知構成要素の個々の信頼性もはるかに超える総合信頼性を可能にするはずである。具体的には、以下でさらに論じるように、75%のISO精度を持つ2つの直交センサは、組み合わされたときには理論的に93.75%の時間、正確である。冗長性は両方の精度を向上させ、時間の百分率データが表示される。

光学センサと電気化学センサを組み合わせることで、個別の用量応答により、低血糖範囲でも高血糖範囲でも高い精度を持つ検知システムが得られる。図12は、光学平衡グルコースセンサおよび電気化学グルコースセンサの用量応答関数(すなわち、センサ出力とグルコース用量の間の相関関係)を示す。光学センサは、低血糖領域において急峻な勾配133を特徴として、精度が高くなり、電気化学センサは、直線勾配233を有して、結果として高血糖領域において高い精度が得られることになる。
本明細書に記載の直交冗長検知システムの実施形態ではまた、個々のセンサウォームアップ時間の最適化によって、ウォームアップ時間の低減を実現する。センサ信号が第1の較正を行うのに十分なだけ安定するまでの時間と定義される全システム起動時間は、アルゴリズムへの入力として予測可能始動挙動および起動診断を利用して適応ウォームアップを作成することによって、低減させることができる。センサ起動時間を低減させることは、患者がフィンガースティック較正をセンサの挿入直後に完了できるようにするので、システムの精度および信頼性、ならびに使用者の便宜のために重要である。
本発明の実施形態では、高度なアルゴリズムにより各センサからの信頼性情報を集約し、かつ直交冗長センサの機能を活用して遅れを低減させ、起動時間を改善し、精度を改善する。信号を比較することによって、故障を確認し、自己較正を実行し、それによって、必要な血糖計較正の回数を減らすことができる。

障害検出に関する最新技術は、センサ問題がグルコース表示値に影響を及ぼす前にそれを事前に特定するように設計される、予測診断に向かって着実に進展してきた。本発明の直交冗長システムは、障害検出の3段階手法、すなわち、電気化学電極を単独で用い、光学センサを単独で用い、次に複合信号からの情報を用いる障害検出を実施する。
感度低下分析という文脈内では、一時的感度低下事象は、回復される事象と定義でき、恒久的感度低下事象は、回復不能であるものと定義できることに留意されたい。本発明の実施形態では、感度低下分析による(電気化学)センサ障害検出は、それぞれの感度低下事象を3つの段階、すなわち、下り坂、谷、および上り坂に分けることによって実施することができ、第3の段階(上り坂)は任意選択である。
一時的感度低下、すなわちセンサ「下降」はまた、IsigとIsigの変化率(ROC)の変化との組合せを使用して検出することもできる。一般に、この方法は、Isigバッファサイズが2時間以上であるときに有利に使用することができる。したがって、1つの実施形態では、論理は、まず2時間のIsigバッファを作成することから進行する。次に、バッファの平均値およびROCの変化が計算される。そうして、平均値が第1の閾値未満になり、かつ現在のバッファと2時間前のバッファとの間のROC変化の率が第2の閾値未満になるたびごとに、下降が始まったとみなされる。下降は、Isigバッファの平均値が上方閾値まで上昇したときに終わるとみなされる。これは図29に説明的に示されており、図で「以前のセンサ」2803および「次のセンサ」2805は同じ対象上の伴センサであり、下降は、下降インジケータ2807が5(nA)に等しくなったときに始まり、下降インジケータ2807が15(nA)になったときに終わる。
光学センサについて、また図22に関連して説明したものと同様に、雑音計量計算はまた、異なる入力およびスケール変更係数を用いて、電気化学センサの生Isigに対して使用することができる。具体的には、電気化学雑音計算のために、Isig(すなわち、信号)とSG(すなわち、センサグルコース値)の両方を入力として使用することができる。どちらの場合も、Isigおよび/またはSGはスケール変更されなければならない。本発明の一実施形態では、スケール変更係数は実験的に9と決めることができる。加えて、Isigが使用されている場合、絶対二次導関数結果にもまた、スケール変更の前に現在の較正係数を掛ける必要がある。次に、パケットに雑音があるとみなすことができるかどうか判定するために、閾値を雑音値の分布に基づいて計算することができる。本発明の実施形態では、雑音検出の典型的な閾値は、たとえば、3から4の間とすることができる。閾値はまた、実験データ、使用者固有データ、およびセンサ固有データに基づいて決定することもできる。
示されたように、直交冗長システムは、較正を2〜4回/週に限定する(たとえば、1日当たり2回の較正から)ための「オンデマンド」プロトコルを使用して較正頻度の低減という結果が得られる、いくつかの特徴を含む。これらの特徴には、(1)電気化学グルコースセンサのセンサ精度/耐久性改善、(2)生理学的モデルベースの較正アルゴリズム、(3)個々の構成要素が安定状態に達した後で内部自己較正を可能にする冗長直交検知技術、および(4)タイミングベースから必要ベースの較正要求への移行を可能にする「スマート」診断、が含まれる。

直交冗長センサシステムは、少なくとも1つのセンサが信頼できる測定値を供給することを保証する冗長性、障害検出、および先進的アルゴリズムを使用することによって、着用の持続期間、およびデータの信頼性を増大させる。加えて、センサ寿命は指定の着用持続期間に限定されて、データの信頼性が保証される。
2つのセンサシステムを単一のデバイスに結合するとより多くの計器および電池の容積が必要になるが、小型化および一体化の方法を使用して、送信機デバイス10が他のCGMデバイスとサイズが確実に同様になるようにすることができる。
典型的な電気化学センサ、たとえばEnlite(登録商標)センサは、通常には室温で、また周囲大気の比較的高い湿気レベルで保存され得る。これらと同じ条件のもとで直交冗長センサ(そのような電気化学センサを含み得る)の保存を可能にするために、また同時に所望の有用性を維持するために、本発明の実施形態は光学センサ用アッセイの乾燥バージョンを含む。本明細書では「乾燥化学」という用語は、当初の湿った構成物と比較して乾燥したアッセイの形を指す。乾燥化学は、たとえば、凍結乾燥粉末の形にすること、またはポリマー中に懸濁させることができ、乾燥包装および乾燥保存を可能にするだけでなく、貯蔵寿命期間安定性もまた改善することができる。アッセイ化学は、たとえば、アッセイを凍結させること、および急速真空乾燥による液体媒体の昇華を含む、凍結乾燥ステップによって乾燥させることができる。

Claims (69)
- プログラム可能データ処理装置が使用者の体内のグルコースの濃度を決定するための、少なくとも電気化学グルコースセンサおよび光学グルコースセンサを含む直交冗長センサデバイスを較正する方法であって、
前記電気化学グルコースセンサからの第1の信号を前記プログラム可能データ処理装置が受信するステップと、
前記光学グルコースセンサからの第2の信号を前記プログラム可能データ処理装置が受信するステップと、
前記第1および第2の信号のそれぞれに対し、前記プログラム可能データ処理装置がそれぞれの完全性検査を実行するステップと、
前記第1の信号を較正することができるかどうか、また前記第2の信号を較正することができるかどうかを前記プログラム可能データ処理装置が判定するステップであり、前記判定が、前記第1の信号および前記第2の信号がそれぞれの完全性検査に合格するか、または不合格となるかに基づいて行われる、ステップと、

- 前記マイクロプロセッサが、前記第2の信号はその完全性検査に不合格と判定した場合、前記マイクロプロセッサはさらに、前記第1の信号がその完全性検査に合格するかどうかを判定し、前記第1の信号がその完全性検査に合格した場合、前記第1の信号を使用して前記第2の信号を補正する、請求項35に記載のシステム。
- 前記マイクロプロセッサが前記第1の信号を、インライン式センサマッピングによって第2の信号を補正するために使用して、補正された第2の信号を生成する、請求項36に記載のシステム。

- 前記電気化学センサおよび前記光学センサのそれぞれが遠位部分および近位部分を有し、前記光学センサおよび前記電気化学センサのそれぞれの遠位部分が、前記使用者の体内の同じ場所に位置する、請求項22に記載のシステム。
- コンピュータ可読媒体と、
前記コンピュータ可読媒体に格納された持続性コンピュータ可読プログラムコードとを備えるプログラムコード記憶デバイスであって、前記コンピュータ可読プログラムコードが命令を有し、前記命令が実行されるとマイクロプロセッサが、
使用者の体内のグルコースの濃度を決定するための直交冗長グルコースセンサデバイスの、電気化学グルコースセンサからの第1の信号および光学グルコースセンサからの第2の信号を受信し、
前記第1および第2の信号のそれぞれに対し、それぞれの完全性検査を実行し、
前記第1の信号を較正することができるかどうか、また前記第2の信号を較正することができるかどうかを判定し、ここで、前記判定が前記第1の信号および前記第2の信号がそれぞれの完全性検査に合格するか、または不合格となるかに基づいて行われ、
前記第1の信号を較正することができると判定された場合は、前記第1の信号を較正して電気化学センサグルコース(SG)値を生成し、
前記第2の信号を較正することができると判定された場合は、前記第2の信号を較正して光学センサグルコース(SG)値を生成し、かつ
前記電気化学SG値と前記光学SG値を融合して、前記直交冗長グルコースセンサデバイスの単一の融合センサグルコース値を計算する
ことになる、プログラムコード記憶デバイス。 - 前記センサデバイスが前記使用者の体に埋め込まれるか、または皮下に配置される、請求項51に記載のプログラムコード記憶デバイス。
- 前記第1の信号が電流(Isig)である、請求項51に記載のプログラムコード記憶デバイス。
- 前記光学グルコースセンサがアッセイフルオロフォアおよび参照フルオロフォアを含み、前記アッセイフルオロフォアがアッセイ蛍光信号を生成し、前記参照フルオロフォアが参照蛍光信号を生成し、前記光学グルコースセンサからの前記第2の信号が前記アッセイ蛍光信号と前記参照蛍光信号の比である、請求項51に記載のプログラムコード記憶デバイス。
- 前記各完全性検査が感度低下を検査することを含む、請求項51に記載のプログラムコード記憶デバイス。
- 前記各完全性検査が雑音を検査することを含む、請求項51に記載のプログラムコード記憶デバイス。
- 前記各完全性検査がセンサドリフトを検査することを含む、請求項51に記載のプログラムコード記憶デバイス。
- 前記電気化学センサの前記完全性検査が感度低下、雑音、およびセンサドリフトを検査することを含み、前記コンピュータ可読プログラムコードが命令を含み、前記命令が実行されると、前記感度低下、雑音、およびドリフトがそれぞれの計算された限度内に入る場合には、前記マイクロプロセッサが、前記第1の信号はその完全性検査に合格しており、較正することができると判定することになる、請求項51に記載のプログラムコード記憶デバイス。
- 前記感度低下、雑音、およびドリフトの1つまたはそれ以上がそれぞれの計算された限度から外れる場合には、前記第1の信号はその完全性検査に不合格であり、較正することができないと判定される、請求項58に記載のプログラムコード記憶デバイス。
- 前記第1の信号がその完全性検査に不合格と判定された場合、前記命令が実行されるとさらに、前記マイクロプロセッサが、前記第2の信号がその完全性検査に合格するかどうか判定し、前記第2の信号がその完全性検査に合格した場合、前記第2の信号を使用して前記第1の信号を補正することになる、請求項59に記載のプログラムコード記憶デバイス。
- 前記コンピュータ可読プログラムコードが命令を含み、前記命令が実行されると、前記マイクロプロセッサが前記第2の信号を、インライン式センサマッピングによって第1の信号を補正するために使用して、補正された第1の信号を生成することになる、請求項60に記載のプログラムコード記憶デバイス。
- 前記コンピュータ可読プログラムコードが命令を含み、前記命令が実行されると、前記マイクロプロセッサが前記補正された第1の信号を較正して、前記電気化学SG値を生成することになる、請求項61に記載のプログラムコード記憶デバイス。
- 前記コンピュータ可読プログラムコードが命令を含み、前記命令が実行されると、前記マイクロプロセッサが前記インライン式センサマッピングを、関係式first_signal_buffern=a×second_signal_buffern+bを使用することによって実行することになり、ここでaおよびbはマッピングパラメータである、請求項61に記載のプログラムコード記憶デバイス。
