脳卒中

本発明の方法によれば、本明細書に記載されている、本発明のAng(1−7)ペプチドまたはアンジオテンシン(1−7)受容体アゴニストは、単独で(例えば、精製済みペプチドまたは化合物として)、または本明細書に記載されている組成物もしくは医薬の構成要素(例えば、疾患の処置のための医薬製造において)として、対象に投与することができる。本組成物は、生理学的に許容される担体または添加剤と一緒に製剤化して、医薬組成物を調製することができる。担体および組成物は、無菌とすることができる。製剤は、投与、例えば静脈内または皮下投与の形式に好適となるべきである。組成物の製剤化方法は、当分野で公知である(例えば、Remington’s Pharmaceuticals Sciences、第17版、Mack Publishing Co.(Alfonso R. 様々な実施形態は、異なる投与レジメンを含むことができる。一部の実施形態では、アンジオテンシン(1−7)ペプチドまたはアンジオテンシン(1−7)受容体アゴニストは、連続注入によって投与される。一部の実施形態では、この連続注入は、静脈内である。他の実施形態では、連続注入は、皮下である。あるいはまたは追加的に、一部の実施形態では、アンジオテンシン(1−7)ペプチドまたはアンジオテンシン(1−7)受容体アゴニストは、2か月毎、毎月、毎月2回、3週間毎、2週間毎、毎週、毎週2回、毎週3回、毎日、毎日2回、または別の臨床的に望ましい投与スケジュールで投与される。単一の対象のための投与レジメンは、一定間隔である必要はないが、対象の必要性に応じて経時的に変えることができる。

PanCyteの連続投与
その病態生理学を理解し、かつ虚血性傷害の重症度を最小化するための治療戦略を特定する目的で、いくつかの動物モデルを使用して、脳虚血の検討が行われてきた。局所虚血は、局所的な脳梗塞を引き起こし、中大脳動脈閉塞(MCAO)によって誘発され得る。MCAOのラットモデルは、ヒトにおける半球梗塞のモデルとして認められている。MCAOの後、時間的かつ空間的な進展を伴う皮質梗塞および線条体梗塞が、中大脳動脈によってもたらされる血管領域内で起こる。
動物は、ステップ試験における前肢運動喪失に関して試験した。動物を、抑制していない前足をテーブルに触れさせながら、片手でラットの両方の後肢、およびもう一方の手でモニターしない方の前肢を固定して保持した。動物をテーブル表面にそって、両方の前肢に対して順手方向および逆手方向へ、横向きに動かしながら(およそ5秒間で85cm)、ステップの調節回数を数えた。図1は、8、15、22、29および36日目における、ステップ試験の結果を示す。本試験において、4つの群、対照群(49日間、PBSのみ投与)、49日間のPanCyte50μg/kgの投与群、49日間のPanCyte500μg/kgの投与群、および、14日間のPanCyte50μg/kgの投与群を解析した。データは、PanCyteの投与を受けた各群は、対照動物と比べて、22日目までにラットの成績向上を享受したこと、およびこの効果は、統計的有意性の向上を伴って、36日目まで続いたことを示す。追加的に、49日間、用量50μg/kgのPanCyteにさらされた群は、15日目において、対照よりもかなり優れた成績を示した一方、49日間、500μg/kgのPanCyteまたは14日間、50μg/kgのPanCyteのどちらかにさらされた群は、改善する傾向を示しながらも、この特定の実験では、この時点において統計的に有意なレベルには到達しなかった。
肢の配置試験は、前肢試験と後肢試験の両方に分けた。前肢配置試験の場合、試験者が、ラットをテーブル上面近くに保持して、ひげ、視覚、触覚、または固有受容感覚刺激に応答して、テーブル上面の前肢を配置するラットの能力を得点にした。同様に、後肢配置試験の場合、試験者が、触覚および固有受容感覚刺激に応答して、テーブル上面の後肢を配置するラットの能力を評価した。感覚入力の各モードに対して、別々の部分得点を取得して加算し、総得点を得た(前肢配置試験の場合:0=正常、12=最大障害;後肢配置試験の場合:0=正常;6=最大障害)。得点は、以下の通り、0.5ポイント増分で得た。前肢配置試験:ひげによる配置(0〜2)、視覚による配置−前方向(0〜2)、横方向(0〜2);触覚による配置−背面(0〜2)、側面(0〜2);固有受容感覚による配置(0〜2);合計0〜12。図2は、8、15、22、29および36日目における、前肢配置試験の結果を示す。本試験において、4つの群、対照群(49日間、PBSのみ投与)、49日間のPanCyte50μg/kgの投与群、49日間のPanCyte500μg/kgの投与群、および、14日間のPanCyte50μg/kgの投与群を解析した。データは、PanCyteの投与を受けた各群は、対照動物と比べて、この試験について、29日目までに成績の向上を享受したこと、およびこの効果は、36日目まで続いたことを示す。さらに、14日間、PanCyte50μg/kgにさらされた群は、対照動物よりもかなり優れた成績を示すことが15日目に始まった一方、別の処置群は、同じ傾向を示したが、この実験では、29日目まで統計的な有意性には到達しなかった。

修正神経症状評価尺度(mNRS)は、投与された薬物/用量を知らない個体により実施した(盲検試験)。実施したmNRSは、可能な点数0〜18点の尺度で神経得点にすることができる。より高い得点を有する動物は、より低い得点のラットよりも重症な症状および能力障害があることを示した。図4は、1、8、15、22、29および36日目における、mNRS評価の結果を示す。本試験において、4つの群、対照群(49日間、PBSのみ投与)、49日間のPanCyte50μg/kgの投与群、49日間のPanCyte500μg/kgの投与群、および、14日間のPanCyte50μg/kgの投与群を解析した。データは、PanCyteの投与を受けた各群が、対照群と比較して、29日目までに、本試験に対して成績の改善を享受したことを示す。29および36日目に加えて、49日間のPanCyte50μg/kgの投与群は、8および15日目において統計的に成績が改善することを示しており、また49日間のPanCyte500μg/kgの投与群は、本実験では、22日目において統計的に成績が改善することを示した。
TXA127、PanCyte、または直鎖状PanCyte投与の比較
動物モデル、外科処置手順、ならびに動物の世話の手順、および条件は、特に指定しない限り、実施例1に関して上で記載した通りとした。本実施例において、合計105匹の動物を使用し、表2は、本検討の群割振りを示す。
動物は、ステップ試験(ST)における前肢運動喪失に関して試験した。動物を、抑制していない前足をテーブルに触れさせながら、片手でラットの両方の後肢、およびもう一方の手でモニターしない方の前肢を固定して保持した。動物をテーブル表面にそって、両方の前肢に対して順手方向および逆手方向へ、横向きに動かしながら(およそ5秒間で85cm)、ステップの調節回数を数える。図5は、TXA127、PanCyteまたは直鎖状PanCyteによる処置により、ビヒクル対照条件と比較して、外科処置後の21日目までに、すべての実験条件において処置したラットの成績が著しく改善することを示す。改善傾向は、外科処置後、早くも14日目で観察される。直鎖状PanCyteは50ug/kgしか投与しなかったが、その結果は、実質的に、TXA127またはPanCyteの10倍に等しいことに留意すべきである。
前肢配置試験の場合、ラットをテーブル上面近くに保持して、ひげ、視覚、触覚、または固有受容感覚刺激に応答して、テーブル上面の前肢を配置するラットの能力を得点にした(0=正常、12=最大障害)。得点は0.5ポイント増分で示した(以下を参照されたい)。通常、脳卒中後の最初の1か月間に、ゆっくりとした定常的な肢の配置行動の回復がある。図6は、処置条件のすべてにおいて、ビヒクル対照と比べて成績の有意な改善が観察され、これは14日目に始まり、検討期間を通じて継続したことを示す。他の実験群よりもかなり少ない用量の薬剤にさらされているにもかかわらず、直鎖状PanCyteの群は、特に35日目以降、最良の成績を有したように、やはり思われる。

修正神経症状評価尺度(mNRS)は、投与された薬物/用量を知らない個体により実施した(盲検試験)。実施したmNRSは、可能な点数0〜18点の尺度で神経得点にすることができる。より高い得点を有する動物は、より低い得点のラットよりも重症な症状および能力障害があることを示した。図8は、各実験群は、ビヒクル対照と比較して、14日目までに成績が著しく改善することを示したことを示す。観察された成績の向上は、本検討の期間、維持された。
PanCyteおよびAVE0991投与の比較
動物モデル、外科処置手順、ならびに動物の世話の手順、および条件は、特に指定しない限り、実施例1に関して上で記載した通りとした。本実施例において、合計75匹の動物を使用し、表3は、本検討の群割振りを示す。

前肢配置試験の場合、ラットをテーブル上面近くに保持して、ひげ、視覚、触覚、または固有受容感覚刺激に応答して、テーブル上面の前肢を配置するラットの能力を得点にした(0=正常、12=最大障害)。得点は0.5ポイント増分で示した(以下を参照されたい)。通常、脳卒中後の最初の1か月間に、ゆっくりとした定常的な肢の配置行動の回復がある。図11は、PanCyteとAVE0991による処置の両方が、ビヒクル対照と比べて、22日目までに成績の有意な改善をもたらし、29日目まで続いたことを示す。50mkgのAVE0991を受けた動物は、15日目までに成績の統計的に有意な改善を示した一方、10倍ものAVE0991(500mkg)を含めた、その他の群は、改善する傾向を示すに過ぎなかったことに留意されたい。
各ラットを、その尾の基底部からおよそ1インチのところを保持した。次に、テーブルの表面から1インチ上に持ち上げた。ラットを垂直軸に保持し、左側または右側のどちらかに対して10°以下として定義した。ラットが垂直軸から片側にその頭を動かす毎に、揺れを記録した。別の揺れを試みる前に、ラットは、次の揺れを数えるために、垂直位に戻さなければならなかった。20回の揺れ総数を数えた。正常ラットは、通常、各側への揺れは等数となる。局所虚血後、ラットは、対側(この場合、左側)に揺れる傾向がある。体揺れ得点は、揺れ総数に対する右側への百分率として表現する。図12は、PanCyteまたはAVE0991(強制経口投与による)による処置は、ビヒクル対照と比べて、22日目までに有意な改善を示したことを示す。皮下投与したAVE0991は、22日目までに改善する傾向を示した。
修正神経症状評価尺度(mNRS)は、投与された薬物/用量を知らない個体により実施した(盲検試験)。実施したmNRSは、可能な点数0〜18点の尺度で神経得点にすることができる。より高い得点を有する動物は、より低い得点のラットよりも重症な症状および能力障害があることを示した。図13は、PanCyteおよびAVE0991(強制経口投与による)の投与により、22日目までに成績が有意に改善した一方、皮下投与したAVE0991は、ビヒクル対照と比べて、改善する傾向を示したことを示す。29日目までに、処置群はすべて、ビヒクル対照と比較して、成績が顕著に改善することを示した。処置群はすべて、早くも15日目に改善する傾向を示した。
当業者は、決まった手順の実験だけを使用して、本明細書に記載されている本発明の具体的な実施形態に対する多くの均等物を認識するか、または確認することができるであろう。本発明の範囲は、上の詳細説明に限定されることを意図するものではなく、むしろ以下の特許請求の範囲において説明される。
Claims (41)
- アミノ酸配列Asp 1 −Arg 2 −Val 3 −Tyr 4 −Ile 5 −His 6 −Pro 7 (配列番号1)を含む直鎖状または環状アンジオテンシン(1−7)ペプチドを含む、脳卒中または外傷性脳損傷を処置するための組成物であって、全身投与によって前記組成物が脳卒中または外傷性脳損傷に罹患している対象に投与されることを特徴とし、前記全身投与が脳室内投与ではない、組成物。
- 前記脳卒中が、虚血性脳卒中、出血性脳卒中、またはそれらの組合せのいずれかである、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、連続注入により投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、1か月あたり3回投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、1か月あたり2回投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、1か月あたり1回投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、約1〜1,000μg/kg/日の範囲の有効用量で投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、約50〜500μg/kg/日の範囲の有効用量で投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、約1〜60μg/kg/日の範囲の有効用量で投与されることを特徴とする、請求項1に記載の組成物。
- 前記アンジオテンシン(1−7)ペプチドが、直鎖状ペプチドである、請求項1に記載の組成物。
- 前記アンジオテンシン(1−7)ペプチドが、環状ペプチドである、請求項1に記載の組成物。
- 前記環状ペプチドが、アミノ酸の間に結合を含む、請求項11に記載の組成物。
- 前記結合が、天然に存在するアンジオテンシン(1−7)中のTyr 4 位およびPro 7 位に対応する残基に位置している、請求項12に記載の組成物。
- 前記結合が、チオエーテル架橋である、請求項12に記載の組成物。
- 前記環状ペプチドが、以下の式
を有する4,7環化アンジオテンシン(1−7)である、請求項11に記載の組成物。 - 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが、プロテアーゼ抵抗性、血清安定性、および/またはバイオアベイラビリティを向上させるために、1つまたは複数の化学修飾を含む、請求項1に記載の組成物。
- 前記1つまたは複数の化学修飾が、ペグ化を含む、請求項16に記載の組成物。
- 前記全身投与が、静脈内投与、皮下投与、吸入、皮内投与、経皮投与、経粘膜投与、および/または経口投与から選択される、請求項1に記載の組成物。
- 前記全身投与が静脈内投与である、請求項18に記載の組成物。
- 前記全身投与が皮下投与である、請求項18に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが毎日投与されることを特徴とする、請求項1に記載の組成物。
- 前記直鎖状または環状アンジオテンシン(1−7)ペプチドが毎日2回投与されることを特徴とする、請求項1に記載の組成物。
- アミノ酸配列Asp 1 −Arg 2 −Val 3 −Tyr 4 −Ile 5 −His 6 −Pro 7 (配列番号1)を含む直鎖状または環状アンジオテンシン(1−7)ペプチドを含む、脳卒中または外傷性脳損傷を処置するための組成物であって、経口投与によって前記組成物が脳卒中または外傷性脳損傷に罹患している対象に投与されることを特徴とする、組成物。
- 前記脳卒中が、虚血性脳卒中、出血性脳卒中、またはそれらの組合せのいずれかである、請求項23に記載の組成物。
