Description
本出願は、米国特許法第119条(e)項に基づき、2013年2月15日付けで出願された米国仮特許出願第61/765,654号、及び2013年4月24日付けで出願された米国仮特許出願第61/815,636号(どちらも引用することにより本明細書の一部をなすものとする)の優先権の利益を主張するものである。
本発明は、一般的に、例えば、X連鎖性網膜分離症(XLRS)、網膜剥離(疾患関連性、傷害誘導性、及び自然発生的)、加齢性黄斑変性症、嚢胞、嚢胞様黄斑浮腫、網膜色素変性症、及び老人性スキーシスを含む眼の障害を治療する改善された方法、またかかる治療方法において有用なベクターに関する。より具体的には、本発明は、最小の免疫応答の誘発を伴う、眼においてコードされるタンパク質の高レベルの発現をもたらすことができる改良された発現ベクターに関する。これらの特性により、かかるベクターは、眼において特定のタンパク質を産生することができないこと、又は眼における非機能性タンパク質の産生のいずれかに起因する眼疾患を治療するのに特に有用である。
かかる参照を確立した場合には、高レベルの発現は、その参照プロモーターを使用して観察される発現のレベルよりも顕著に高いレベルでのORFの発現をもたらすプロモーターの能力を指す場合がある。参照プロモーターの例は、Colosi et al. (Gene Therapy, 16, 2000, 916-926)に記載される。一実施形態では、本発明のプロモーターは、参照プロモーターよりも少なくとも5倍、10倍、20倍、50倍、100倍、500倍、又は少なくとも1000倍高いレベルでORFの転写をもたらす場合がある。例えば、各発現ベクターによって産生されたORF特異的mRNAのレベルと比較することにより、発現のレベルを比較することができる。かかる比較を行う方法は、当業者に知られている。
1)疎水性:Met、Ala、Val、Leu、Ile;
2)中性親水性:Cys、Ser、Thr;
3)酸性:Asp、Glu;
4)塩基性:Asn、Gln、His、Lys、Arg;
5)鎖配向に影響する残基:Gly、Pro;及び、
6)芳香族:Trp、Tyr、Phe。
提案される臨床用アデノ随伴ウイルス(AAV)レチノスキシンベクターであるAAV8 scRS/IRBP hRSの網膜の機能及び構造を保護する能力、並びにレチノスキシン欠損Rs1KOマウスに硝子体内投与した場合にレチノスキシンタンパク質発現を媒介する能力を評価するため研究を行った。1.0×106vg/眼、1.0×107vg/眼、5.0×107vg/眼、1.0×108vg/眼、5.0×108vg/眼、及び2.5×109vg/眼の用量のAAV8 scRS/IRBP hRSベクター、又はビヒクルを、18日齢〜34日齢のRs1KOマウスに硝子体内注射により投与した。反対側の眼には注射しなかった。角膜網膜電図(ERG)a波及びb波の振幅を注射後(PI)11週間〜15週間、及び6ヶ月〜9ヶ月に測定し、網膜腔の形成を注射後12週間〜16週間に光コヒーレンストモグラフィ(OCT)により測定し、また、レチノスキシンタンパク質発現を注射後12週間〜18週間、及び6ヶ月〜9ヶ月に免疫組織化学により測定した。ERG及びOCTは、それぞれ網膜の機能及び構造の指標として臨床上使用される。注射後11週間〜15週間において、5×107vg/眼、1×108vg/眼、及び2.5×109vg/眼の用量を受ける眼は、ERG a波振幅の統計学的に有意な改善を示し、5×107vg/眼、1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼の用量は注射していない眼と比較してERG b波振幅の統計学的に有意な改善を示した。6ヶ月〜9ヶ月の時点で1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼の3つのベクター用量を試験したところ、全てが注射していない眼と比較してERG a波及びb波の振幅の統計学的に有意な改善をもたらした。注射後11週間〜15週間にOCTによって測定される網膜腔もまた、未処理の眼と比較して5×107vg/眼、1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼の用量で顕著に減少した。網膜免疫組織化学は、注射後11週間〜15週間の未処理の眼と比較して、1×107vg/眼、1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼の用量で顕著なレチノスキシンタンパク質レベルをもたらすことを示した。1×108、5×108、及び2.5×109の用量は、野生型マウスの25%以上の発現をもたらした。注射後6ヶ月〜9ヶ月において、2.5×109vg/眼の用量のみを試験したところ、野生型レチノスキシンレベルの65%を生じ、これは11週間〜15週間を上回る顕著な増加である。これらのデータは、AAV8 scRS/IRBP hRSが、これまでに与えられたベクターAAV8 hRSp4よりも10倍〜100倍低い用量で有効性を示すことを実証した。マウス網膜分離症モデルにおける有効性、及びウサギにおける毒性に関するいくつかのレチノスキシンベクターの調査により、AAV8 scRS/IRBP hRSを開発した。
本実施例で使用されるベクターであるAAV8 scRS/IRBP hRSは、ヒトレチノスキシンcDNAの発現を駆動する修飾ヒトレチノスキシンプロモーターで構成される自己相補型ベクターゲノムを送達する8型アデノ随伴ウイルスベクターである。また、このベクターは、プロモーター活性を増大するための光受容体間レチノイド結合タンパク質(IRBP)エンハンサー、切断型レチノスキシン第1イントロン、並びにヒトベータ−グロビン3’非翻訳領域及びポリアデニル化部位を採用するものである。AAV8 scRS/IRBP hRSの構造を図1に示し、このベクターの完全な配列を配列番号16として提供する。
pAAV scRS/IRBP hRSと呼ばれるAAV8 scRS/IRBP hRSベクター用の産生プラスミドは、pBluescriptプラスミド(カリフォルニア州サンディエゴのStratagene Inc.)へとクローニングされた、AAV2逆位末端反復配列により結合された上述のヒトレチノスキシン発現カセットで構成される。
レチノスキシン発現カセット:上記発現カセットのタンパク質コーディング部分は、319bpの切断型レチノスキシン第1イントロンを保持するヒトレチノスキシンcDNAで構成される。切断型イントロンは、レチノスキシン転写開始部位に対して+95〜+355、及び+14396〜+14445の塩基対からなる。これらの配列は、それぞれ、スプライス供与要素、及びラリアット/スプライス受容要素をコードする。ベクター構築を容易にするため、8塩基対のAsiSI制限酵素部位を上記イントロンの2つの部分の間に導入した。上記発現カセットの転写は、開始コドンより前の塩基である、転写開始部位に対して−739位から+42位まで伸びるヒトレチノスキシンゲノム配列によって駆動される。このプロモーター配列は、−496位〜−188位に308bpのAlu反復配列を含み、これは、欠失されて、SalI部位に隣接したヒトIRBP遺伝子に由来する261bpのエンハンサーで置き換えられている。IRBPエンハンサーは、IRBP転写開始部位に対して−1374位〜−1635位に位置する。ポリアデニル化は全ヒトベータ−グロビン3’非翻訳領域及びポリアデニル化部位をコードする218bpのフラグメントによって方向づけられる。この領域は、ヒトベーターグロビン停止コドンに直接続く218bpのゲノム配列に対応する。構築を容易にするため、XhoI部位及びBglII部位をコードする合成DNAを、レチノスキシン停止コドンと、3’非翻訳部位及びポリアデニル化部位をコードするベータグロビン配列との間に導入した。5’及び3’AAV2逆位末端反復要素に連結するため、NotI部位及びAscI部位を上記発現カセットの5’末端と3’末端にそれぞれ付加した。
AAV8 scRS/IRBP hRSベクターを以前に記載されるように調製した(Grimm D et al. 2003)。簡潔には、10%ウシ胎児血清(HyClone SH30070.03)を含有し、ペニシリン、ストレプトマイシン及びグルタミンを添加したDME(高グルコース)培地中、850cm2のローラーボトルにおいて培養した293細胞に、ヘルパープラスミドpLadeno5(アデノウイルス2型E2A、E4、及びVAのRNAをコードする)及びpHLP19−8(AAV2 rep及びAAV8 capをコードする)、並びにpAAV scRS/IRBP hRSベクタープラスミドを、リン酸カルシウム法を使用して一過性に形質移入した。形質移入の後、培地を交換し、血清を含まない同じ培地で置き換えた。60時間後、遠心分離により細胞を採取し、−80℃で保存した。ウイルスベクターを精製するため、細胞ペレットを溶解し、50mM Tris−HCl、150mM NaCl、2mM MgCl2、pH8.0に懸濁し、3回の顕微溶液化により破壊した。細胞片を遠心分離により除去し、上清を25mM CaCl2に調整し、得られたペレットも遠心分離により除去した。ベンゾナーゼヌクレアーゼを1ml当たり100単位の最終濃度まで上清に添加し、混合物を37℃で1時間インキュベートした。その後、40%ポリエチレングリコール8000(PEG)/2.5M NaClを添加して、最終濃度8%PEG及び0.650M NaClとし、ベクター画分を沈殿させて、遠心分離により採取した。ベクター画分を50mM HEPES、150mM NaCl、20mM EDTA、1%ラウロイルサルコシンナトリウム、10μg/ml RNアーゼ A、pH8.0に可溶化した。この溶液をCsClステップグラジエントに適用し、超遠心分離によりバルクタンパク質及び核酸からベクターを分離した。ベクター画分を採取し、直線CsCl勾配に適用し、再度精製した。精製したベクター画分を採取し、10mM Tris−Cl、180mM NaCl pH7.4に対して透析し、310mM Tris−Cl、180mM NaCl、0.001%プルロニックF−68 pH7.4中に配合し、濾過滅菌して−80℃で保存した。
精製したベクター粒子をQ−PCRタンパク質アッセイ(BCA)、SDS PAGE、エンドトキシンアッセイ(タイプ)及び動的光散乱により分析した。レチノスキシンのエクソン3及びエクソン4にそれぞれ位置する上流及び下流のプライマー、並びにエクソン3/4接合部に及ぶプローブを用い、Taqman Real−Time PCRアッセイを使用して、Q−PRCアッセイを行った。存在するタンパク質のレベルをウシガンマ−グロブリン(カリフォルニア州リッチモンドのRio-Rad)を標準として使用し、Bradford法により特定した。Q−PCRの結果に基づき、理論上のタンパク質濃度を1ml当たりのAAVキャプシドタンパク質の質量として算出した。7.5%SDSゲルを使用して1.2×1010vg(調製1)及び2×1010vg(調製2)についてSDS PAGE分析を行った。分離したタンパク質の可視化をクーマシーR250又は銀染色でゲルを染色することにより行った。カイネティック比色法カブトガニの血球抽出成分エンドトキシンアッセイキット(Clonegen Laboratories)を使用してカイネティックLALエンドトキシンアッセイを行った。それぞれ、AAV8 hRSp4ベクター及びAAV8 hRS/IRBPベクターに対して2.25×1012vg/ml及び5×1012vg/mlのベクター濃度で、Viscotec 802 DLS機器を使用して、動的光散乱アッセイを行った。この分析の結果を図2に示し、以下に表にまとめる。
解剖顕微鏡のもと、1マイクロリットル(μl)のベクター又はビヒクルを、10μlのNanofilシリンジ(フロリダ州サラソタのWorld Precision Instruments, Inc.)及び取り外し可能な35ゲージ針を使用して硝子体内注射により右眼又は左眼に投与した。注射物質は0.22μlフィルタの通過により滅菌され、無菌条件下でシリンジに充填した。マウスをIPケタミン80mg/kg、及びキシラジン4mg/kgで麻酔し、1滴の0.5%テトラカインを角膜に局所適用した。各マウスの一つの眼の角膜輪部からおよそ1mm後ろの上鼻側四分円の毛様体扁平部を通して、1マイクロリットルのベクター又はビヒクルの溶液を注射した。注入量は、硝子体全体積のおよそ五分の一である。低速でベクターを送達する前に硝子体の中心に針先を配置するように注射を行った。その後、眼から針を注意深く抜き、3種類の抗生物質眼軟膏(ネオマイシン、ポリミキシンB及びバシトラシン)を注射部位に塗布した。マウスが麻酔から回復するまで保温プレート(35℃〜37℃)に置き、その後、ケージに戻した。
レチノスキシンノックアウト(Rs1KO)マウスモデルを2003年に作製した。2003年の11月より、これらのマウスをアメリカ国立アレルギー感染症研究所(NIAID)によって維持される共有動物施設においてNIHで飼育し、C57BL/6J系統(メーン州バーハーバーのJackson Laboratory)に18世代を超えて戻し交配した。Rs1KOマウスは注射の時に18日齢〜34日齢であり、ERGの時に14週齢〜37週齢であり、網膜におけるレチノスキシン発現が欠如し、a波振幅と比較したb波振幅の減少、並びに外網状層(OPL)及び内顆粒層(INL)の内部の割裂又は分離を含む「スキーシス」腔の存在を含むXLRS患者に類似した網膜の構造及び機能の表現型を有することが確認された。
本研究で使用した全てのマウス、マウスの親、マウスが受けた試験材料、及びマウスの出生日の一覧、注射、ERG、OCT及び組織学的調査/屠殺を収集し、記録した。
生後(p)18日〜25日(21±2日、平均±1SD)に1マイクロリットルのAAV8 RS/IRBP hRSベクター溶液を、無菌条件下でRs1/KOマウスに硝子体内注射により一側性に投与した。対照Rs1KOマウスは、生後18日〜34日(24±5日)に1μlのビヒクルの一側性の硝子体内注射を受けた。1マイクロリットルのAAV8 hRS1/IRBP又はビヒクルを229匹のRs1KOマウス、すなわち、2.5×109vg/眼を28匹のマウス、5.0×108vg/眼を41匹のマウス、1.0×108vg/眼を39匹のマウス、5.0×107vg/眼を26匹のマウス、1.0×107vg/眼を26匹のマウス、1.0×106vg/眼を26匹のマウス、ビヒクルを43匹のマウスに投与した。角膜混濁等の眼の変化、及び注射部位からの逆流量を注射中又は注射直後に各動物について記録した。ベクターを注射した186匹の動物の注射は全て成功したが、2匹のマウス(1.1%)が注射後、まだ麻酔されている間に死亡した。
のため屠殺した1匹のマウスとは無関係と考えた。全体的には、ベクターを注射した群の25匹の動物がERG又はOCTのための麻酔中又は麻酔後に死亡したが、死亡数はビヒクルを注射した対照群と比べて統計学的に大きくはなく(P=0.18、フィッシャーの直接確率検定)、用量による有意な傾向を示さなかった(P=0.72、傾向に関するカイ二乗検定)。
ERG記録手順:Rs1KOマウスの網膜機能の保護におけるレチノスキシンベクターAAV8 scRS/IRBP hRSの有効性を評価するため、ベクターの硝子体内注射後11週間〜15週間(「短期」)及び/又はベクターの硝子体内注射後6ヶ月〜9ヶ月(「長期」)に両眼において同時に、暗順応網膜電図(ERG)を記録した。ビヒクル対照マウスを硝子体内注射後の14週間〜18週間に記録した。
ベクターを受け、短期ERGを生存し、変化していない透光体を有した全てのマウスの両眼の網膜を、ERG後2日〜21日にOCTによりin vivoで画像化した。各用量群において画像化したマウスの数には、2.5×109vg/眼で25匹の画像化したRs1KOマウス、5.0×108vg/眼で25匹の画像化したRs1KOマウス、1.0×108vg/眼で20匹の画像化したRs1KOマウス、5.0×107vg/眼で19匹の画像化したRs1KOマウス、1.0×107vg/眼で25匹の画像化したRs1KOマウス、1.0×106vg/眼で16匹の画像化したRs1KOマウスが含まれる。
1.網膜領域の中心三分の一を網膜腔についてOCTによりスキャンした。スキャンされる網膜領域の決定は以下の検討に基づいた。
a.以前に公開された研究において、本発明者らは、腔の形態における網膜病態が1ヶ月齢〜4ヶ月例の間で数及び程度が最大であり、視神経から周辺まで分布したことを見出した。
b.本研究における11週間〜15週間の注射後時間は、Rs1KOマウスの未処理の眼が最大の腔数及び分布を有する場合に動物をOCTによって分析することを意味した。
c.ベクターを硝子体の中心に注射し、全ての方向に均等に分布すると仮定した。
d.Rs1KOマウスにおける単一の矩形スキャンによりOCT画像化された網膜の中心三分の一は、残りの網膜を代表する。
2.位置A(+0.6mm)、位置B(0mm又は視神経)及び位置C(−0.6mm)で線形スキャンを行った(図3、Rs1KO、右側画像)。
3回のスキャンを行った。それぞれ最上部及び最下部のスキャンA及びCを最大の腔高さについて採点し、中心網膜を通るスキャンCを正中線の両側の最大の腔高さについて採点し、各網膜に対する合計四つの値は以下の通りであった(図3における星印)。
a.腔無し=1
b.高さ30μm未満の腔=2
c.高さ30μm〜49μmの腔=3
d.高さ50μm〜69μmの腔=4
e.高さ70μm〜99μmの腔=5
f.高さ100μm以上の腔=6
1又は2のERG記録から生存した全ての動物の網膜を、網膜レチノスキシン免疫染色のため採取してレチノスキシン発現を定量した。
1切片当たりのスコア=(切片における最大染色グレード[0〜7]+切片における最小染色グレード)×(全ての染色を含む全長の割合)
一つの眼当たりのスコア=(切片に対するスコア1+2+3+4+5)/5
ERG、Oct及びレチノスキシン発現の分析
本研究では、ビヒクル、又は1.0×106vg/眼、1.0×107vg/眼、5.0×107vg/眼、1.0×108vg/眼、5.0×108vg/眼、及び2.5×109vg/眼の用量のAAV8 scRS/IRBP hRSベクターを18日齢〜34日齢のRs1KOマウスに硝子体内注射によって投与した。その後、上記マウスを注射後11週間〜15週間、及び6ヶ月〜9ヶ月に網膜機能についてERGにより、その後、網膜構造についてOCTにより、また、レチノスキシン発現について免疫組織化学により評価した。このベクターがRs1KOマウスにおいて網膜の機能及び構造を顕著に保護し、タンパク質の顕著な網膜発現を達成する用量範囲を決定するため、これらの実験を行った。図5〜図9は、ERG、OCT(網膜腔)、及びレチノスキシン発現のデータを示す。
本研究では、18日齢〜34日齢のRs1KOマウスに硝子体内注射によって、ビヒクル、又は1.0×106vg/眼、1.0×107vg/眼、5.0×107vg/眼、1.0×108vg/眼、5.0×108vg/眼、及び2.5×109vg/眼の用量のAAV8 scRS/IRBP hRSベクターを投与した。その後、注射後11週間〜15週間、及び6ヶ月〜9ヶ月に網膜機能についてERGにより、続いて、網膜構造についてOCTにより、レチノスキシン発現について免疫組織化学によりマウスを評価した。このベクターがRs1KOマウスにおいて網膜の機能及び構造を顕著に保護し、タンパク質の顕著な網膜発現を達成する用量範囲を決定するため、上記実験を計画した。これらの実験より、以下を結論付けることができる。
1.注射後11週間〜15週間に記録された場合(短期の時点)、注射していない眼と比較して、5×107vg/眼、1×108vg/眼、及び2.5×109vg/眼のAAV8 scRS/IRBP hRSベクター用量はERG a波振幅の統計学的に有意な改善を示し、5×107vg/眼、1×108vg/眼、5×108vg/眼、又は2.5×109vg/眼の用量はERG b波振幅の統計学的に有意な改善を示した。注射ビヒクルは効果がなかった。
2.注射後6ヶ月〜9ヶ月に記録された場合(長期の時点)、未処理の眼と比較して、1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼のベクター用量はERG a波及びb波の振幅の統計学的に有意な改善をもたらした。
3.注射後11週間〜15週間に続いて評価した場合(短期の時点)、未処理の眼と比較して、5×107vg/眼、1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼のベクター用量はスキーシス腔スコアリングの統計学的に有意な改善をもたらす。
4.注射後11週間〜15週間に続いて評価した(短期の時点)1×107vg/眼、1×108vg/眼、5×108vg/眼、及び2.5×109vg/眼の用量によるベクター処理の後、Rs1/KOマウスの眼においてレチノスキシンタンパク質発現は有意に上昇される。1×108vg/眼以上のベクター用量において、レチノスキシンタンパク質発現は野生型レベルの25%以上である。2.5×109vg/眼の用量を受けたRs1/KOの眼を注射後6ヶ月〜9ヶ月に評価したところ、野生型レチノスキシンレベルの65%を示した。これは、長期の時点で評価した唯一の用量であった。
ビヒクル単独の対照注射と比較した、ウサギの眼における本発明の発現ベクターの耐容性を判定するため研究を行った。簡潔には、39匹のニュージーランド白ウサギ(注射時に6ヶ月齢〜7ヶ月齢;体重2.4kg〜3.8kg)を本研究で使用した。全ての生命操作(life procedures)は眼及び視力研究における動物の使用に関するARVO声明に準じて行われ、アメリカ国立眼学研究所の実験動物委員会により承認された。
Claims (13)
- 眼特異的レチノスキシン遺伝子プロモーター及びレチノスキシン遺伝子プロモーターからの発現を増強するように配置された光受容体間レチノイド結合遺伝子エンハンサー要素を含み、前記眼特異的レチノスキシン遺伝子プロモーター及び光受容体間レチノイド結合遺伝子エンハンサー要素はレチノスキシンタンパク質をコードする核酸配列に操作可能に連結された発現カセットを含む発現ベクターであって、
前記レチノスキシンタンパク質をコードする核酸配列はレチノスキシン遺伝子の第1イントロンのレチノスキシン転写開始部位に対して+95〜+355及び+14396〜+14445の塩基対からなる319bp部分を少なくとも含み、
該発現ベクターを個体の眼に投与した場合、該発現ベクターはレチノスキシンタンパク質を眼において発現する、発現ベクター。 - 前記発現カセットが、アデノ随伴ウイルス逆位末端リピート(ITR)配列に隣接する、請求項1に記載の発現ベクター。
- ウイルスに由来するキャプシドタンパク質及び請求項1に記載の発現ベクターを含む、ウイルスベクター。
- 前記キャプシドタンパク質が、アデノ随伴ウイルス(AAV)に由来する、請求項3に記載のウイルスベクター。
- 前記キャプシドタンパク質が、AAV1、AAV2、AAV5、AAV6、AAV7、AAV8及びAAV9のキャプシドタンパク質、または、その部分若しくは変異体であって、その部分若しくは変異体がウイルス様粒子を形成可能なものである、請求項3または4に記載のウイルスベクター。
- 個体におけるX連鎖性網膜分離症を治療するための医薬を製造するための、請求項1または2に記載の発現ベクターまたは請求項3〜5のいずれか1項に記載のウイルスベクターの使用。
- 5’ITRが130bpの長さであり、野生型ITR配列中、パリンドロームのA領域におけるITR配列の15塩基対の欠失を有する、請求項2に記載の発現ベクター。
- 5’ITRが、野生型ITR配列のrepニッキング部位を含むD領域の除去により修飾された、請求項7に記載の発現ベクター。
- 3’ITRが全長の野生型ITR配列である、請求項2、7または8のいずれか1項に記載の発現ベクター。
- 自己相補型AAVベクターゲノムである、請求項1、2または7〜9のいずれか1項に記載の発現ベクター。
- ヒトベータ−グロビン3’非翻訳領域及びポリアデニル化部位をさらに含む、請求項1、2、または7〜10のいずれか1項に記載の発現ベクター。
- 前記光受容体間レチノイド結合遺伝子エンハンサー要素が、前記レチノスキシン遺伝子プロモーターにおけるAlu反復配列を置き換えるように配置されている、請求項1、2、または7〜11のいずれか1項に記載の発現ベクター。
- 前記レチノスキシンタンパク質をコードする核酸配列が、配列番号2と少なくとも90%の配列同一性を有するタンパク質をコードし、野生型レチノスキシンタンパク質の少なくとも一つの機能を有するタンパク質をコードする、請求項1、2、または7〜12のいずれか1項に記載の発現ベクター。
