本技術は、慢性の閉塞性肺疾患等の呼吸機能不全の病状を検出及び/又は治療するためのシステム、方法、及び装置に関する。より具体的にいえば、本技術は、このような病状の重症度における変化を検出することに関する。
Claims (16)
- 患者の肺の状態に起因する呼吸機能不全の病状を評価するために患者をモニターする装置であって、
前記患者の呼吸に関連するセンサーの信号から得られる測定値を表すデータにアクセスするプロセッサーを備え、前記測定値は、前記信号から判定される、提供された呼吸補助療法の程度を表すものであり、
前記プロセッサーが、複数の治療セッションから得られる、提供された呼吸補助療法の程度を表す前記測定値の変化を検出することに基づく重症度変化インジケータの計算を制御するように構成され、前記重症度変化インジケータが、前記患者の呼吸機能不全の病状における重症度の変化に関する情報を表し、
前記プロセッサーは、提供された呼吸補助療法の程度を表す第1の測定値及び第2の測定値から、提供された呼吸補助療法の程度を表す前記測定値の変化が検出されたときに、該変化のレベルと、前記第1の測定値と前記第2の測定値との間の時間の長さとを評価するものであり、
前記重症度変化インジケータの計算は、複数の治療セッションを通じて継続的に提供された呼吸補助療法の程度を表す前記測定値の継続的上昇の検出を含むものである、装置。 - 前記重症度変化インジケータの計算は、複数の治療セッションにおける現在の測定値と過去の測定値との連続的な比較を含むものである、請求項1に記載の装置。
- 前記重症度変化インジケータの決定が、上気道閉塞を取り除くための、吸気フローリミテーションに関連する検出データのアセスメントにも基づくものである、請求項1又は2に記載の装置。
- 前記重症度変化インジケータの決定は、少なくとも1つの呼吸パラメータの評価をさらに含むものである、請求項1〜3のいずれか一項に記載の装置。
- 少なくとも1つの前記呼吸パラメータが呼吸数である、請求項4に記載の装置。
- 前記計算は、複数の日にわたって判定される呼吸数の反復的な上昇の兆候を表す、閾値との比較をさらに含むものである、請求項5に記載の装置。
- 少なくとも1つの前記呼吸パラメータが前記信号から判定される呼吸のデューティーサイクルである、請求項4に記載の装置。
- 少なくとも1つの前記呼吸パラメータが内因性呼気終末陽圧である、請求項4に記載の装置。
- 少なくとも1つの前記呼吸パラメータが呼気フローリミテーションである、請求項4に記載の装置。
- 少なくとも1つの前記呼吸パラメータが、呼気の後半部分にて測定されるフローの大きさであり、
前記重症度変化インジケータの決定が、さらに呼吸数又は換気量における有意ではない変化のアセスメントを含むものである、請求項4に記載の装置。 - 前記計算はさらに、少なくとも1つの前記呼吸パラメータに関するより早い段階での1つ又は複数の測定値の、少なくとも1つの前記呼吸パラメータに関するその後に続く1つ又は複数の測定値と比較した変化を判定することを含んでいる、請求項4〜8のいずれか一項に記載の装置。
- 前記信号から判定される、提供された呼吸補助療法の程度を表す前記測定値が、前記信号から判定される治療圧力の測定値である、請求項1〜6のいずれか一項に記載の装置。
- 前記重症度変化インジケータの計算が、(a)閾値と、(b)前記治療圧力の測定値と、過去の治療セッション中に目標換気量を維持するために使用された、過去の治療圧力の測定値との差との比較を含み、前記閾値は前記差の評価基準である、請求項12に記載の装置。
- 前記比較が、複数の日にわたって繰り返される治療圧力の測定値における上昇の兆候を表すものである、請求項13に記載の装置。
- 患者の肺の状態に起因する呼吸機能不全の病状を評価するために患者をモニターするシステムであって、
患者インターフェイスに対する呼吸可能なガスのフローを大気圧よりも高い圧力で生成するフロー生成器と、
前記患者の呼吸に関連する信号を生成するセンサーと、
請求項1〜14のいずれか一項に記載の装置と
を備えたシステム。 - 前記プロセッサーが、前記重症度変化インジケータに基づいて、出力メッセージを作成するように構成されている、請求項15に記載のシステム。
