Description
Claims (15)
- 人工換気器と、
前記人工換気器を付けた患者に対して、気道圧及び気道空気流量の少なくとも一方を時間の関数として測定する少なくとも1つの気道センサと、
前記気道センサによって測定された気道圧及び気道空気流量の少なくとも一方を分析して、前記人工換気器を付けた患者によって生成される、時間の関数としての呼吸筋圧の自発呼吸異常を検出し、
前記人工換気器から受け取られた信号から、前記人工換気器のサイクルオフの時間を判定し、
前記人工換気器のサイクルオフの時間に対する、前記気道圧及び気道空気流量の少なくとも一方において検出されたピークの位置に基づいて、患者−人工換気器間の不同調の種類を特定する、
少なくとも1つのマイクロプロセッサと、
前記少なくとも1つのマイクロプロセッサによって検出された自発呼吸異常の指示を表示するディスプレイコンポーネントとを備えた、人工換気装置。 - 前記少なくとも1つの気道センサは、気道圧を時間の関数として測定する気道圧センサを含み、
前記少なくとも1つのマイクロプロセッサは、前記気道圧センサによって測定された気道圧を分析して、早いサイクルオフの事象からなる自発呼吸異常を、前記人工換気器のサイクルオフと同時の、時間の関数としての前記気道圧の二段ピークとして検出する、請求項1に記載の人工換気装置。 - 前記少なくとも1つの気道センサは、気道圧を時間の関数として測定する気道圧センサを含み、
前記少なくとも1つのマイクロプロセッサは、前記気道圧センサによって測定された気道圧を分析して、二段努力の事象からなる自発呼吸異常を、二段ピークのうち少なくとも一方のピークが前記人工換気器のサイクルオフと同時でない、時間の関数としての前記気道圧の前記二段ピークとして検出する、請求項1又は2に記載の人工換気装置。 - 前記少なくとも1つの気道センサは、気道圧を時間の関数として測定する気道圧センサを含み、
前記少なくとも1つのマイクロプロセッサは、前記気道圧センサによって測定された気道圧を分析して、努力性呼気を含む自発呼吸異常を、指数関数的減衰からの呼息流量の逸脱として検出する、請求項1乃至3のいずれか一項に記載の人工換気装置。 - 前記少なくとも1つのマイクロプロセッサは、前記人工換気器のサイクルオフを示す信号を受け取り、前記信号を使用して前記人工換気器のサイクルオフの時間を判定する、請求項2乃至4のいずれか一項に記載の人工換気装置。
- 前記少なくとも1つの気道センサは、気道圧を時間の関数として測定する気道圧センサを含み、
前記少なくとも1つのマイクロプロセッサは、前記気道圧センサによって測定された気道圧を分析して、自発呼吸異常を、1回の呼吸サイクル中の時間の関数としての前記気道圧の二段ピークとして検出する、請求項1に記載の人工換気装置。 - 前記少なくとも1つの気道センサは、気道圧を測定する気道圧センサと、気道空気流量を測定する空気流量センサとを含み、前記少なくとも1つのマイクロプロセッサはさらに、
測定された前記気道圧及び気道空気流量に基づいて、少なくとも呼吸筋圧を含む1つ又は複数の呼吸器系変数を時間の関数として計算する、請求項1乃至6のいずれか一項に記載の人工換気装置。 - 前記少なくとも1つのマイクロプロセッサは、早いサイクルオフ異常、努力性呼気異常、及び二段努力異常を少なくとも含む自発呼吸異常を検出し、
前記ディスプレイコンポーネントは、早いサイクルオフ異常、努力性呼気異常、又は二段努力異常が検出された場合、計算された前記呼吸器系変数の非信頼性を示す、請求項7に記載の人工換気装置。 - 呼吸力閉ループコントローラをさらに備え、前記呼吸力閉ループコントローラは、
早いサイクルオフ異常、努力性呼気異常、及び二段努力異常を少なくとも含む少なくとも1つの自発呼吸異常の検出に応答した少なくとも1つのフラグ値に応答して、前記人工換気器の出力を調節する、請求項1乃至8のいずれか一項に記載の人工換気装置。 - 前記ディスプレイコンポーネントは、測定された前記気道圧及び気道空気流量に基づく時間の関数としての呼吸筋圧を少なくとも含む、1つ又は複数の呼吸器系変数の値を表示する、請求項7に記載の人工換気装置。
- 自発呼吸異常を検出する方法を行うために1つ又は複数のマイクロプロセッサにより読み取り及び実行が可能な命令を記憶した非一時的な記憶媒体であって、前記方法は、
人工換気器を付けた患者に対して獲得された気道圧対時間信号のピークを検出するステップと、
1回の呼吸内で、前記気道圧対時間信号の二段ピークによって示される患者−換気器間の不同調を検出するステップと、
人工換気器から受け取られた信号から、前記人工換気器のサイクルオフの時間を判定するステップと、
前記人工換気器のサイクルオフの時間に対する前記二段ピークの位置に基づいて、患者−人工換気器間の不同調の種類を特定するステップと、を有する、非一時的な記憶媒体。 - 前記特定するステップが、
前記二段ピークのうち第2のピークの位置が、前記人工換気器のサイクルオフの時間より後であることに基づいて、早いサイクルオフである前記患者−人工換気器間の不同調を特定するステップをさらに含む、請求項11に記載の非一時的な記憶媒体。 - 前記方法が、
前記二段ピークが前記呼吸中で最も大きいピークであることに基づいて、早いサイクルオフである前記患者−人工換気器間の不同調を特定するステップをさらに有する、請求項11に記載の非一時的な記憶媒体。 - 前記方法が、
測定された気道圧及び気道空気流量に基づいて、少なくとも呼吸筋圧を含む1つ又は複数の呼吸器系変数を時間の関数として計算するステップをさらに有する、請求項11乃至13のいずれか一項に記載の非一時的な記憶媒体。 - 前記方法が、
早いサイクルオフ異常、努力性呼気異常、及び二段努力異常を少なくとも含む少なくとも1つの自発呼吸異常の検出に応答した少なくとも1つのフラグ値に応答して、前記人工換気器の出力を調節するステップをさらに有する、請求項11乃至14のいずれか一項に記載の非一時的な記憶媒体。
