Description
用語「GK活性化因子」は、グルコキナーゼ(GK)センサー系を感作する(sensitize)化合物を指す。GKは、ヘキソキナーゼ・ファミリーに属する酵素であり、これは、グルコース代謝の第一段階、すなわちグルコースからグルコース−6リン酸への変換を触媒する。GKは、膵臓および肝臓において、グルコースセンサーとして機能する。1つの態様において、GK活性化因子は、グルコースの存在下で、膵臓によるインスリン分泌を増加させない、肝臓選択的活性化因子である。例示的なGK活性化因子には、FRI−1またはWO05/066145に開示されるものが含まれる。1つの態様において、GK活性化因子はFRI−1である。
1つの態様において、本発明は、抗糖尿病薬剤と組み合わせてGK活性化因子、および少なくとも1つの薬学的に許容されうるキャリアー、希釈剤または賦形剤を含む、薬学的組成物を提供する。1つの態様において、薬学的に許容されうるキャリアーは、2つの別個の投薬型で存在し、一方の投薬型はGK活性化因子を含み、そして他方の投薬型は抗糖尿病薬剤を含む。別の態様において、組み合わせの活性成分は、単一の投薬型に組み合わせられる。さらに別の態様において、投薬型は経口使用のために意図される。別の態様において、薬学的組成物は、GK活性化因子および抗糖尿病薬剤からなる。
本発明の薬学的組成物の投薬型は、症状、望ましい治療、被験体の年齢および体重、治療しようとする障害の性質および重症度、活性成分の投与経路および薬物動態に応じて多様であろう。示す用量の頻度もまた、望ましい治療および示す障害に応じて多様であろう。
1つの態様において、本発明は、治療が必要な被験体に、以下の方法のために、抗糖尿病薬剤と組み合わされてGK活性化因子を含む薬学的組成物を投与する工程を含む、被験体を治療する方法を提供する:
(a)I型糖尿病および/またはII型糖尿病を治療する;
(b)血糖レベルを正常化するかまたは低下させる;
(c)耐糖能を改善する;
(d)血糖コントロールを改善する;
(e)空腹時血漿グルコースを減少させる;
(f)食後血漿グルコースを減少させる;
(g)グリコシル化ヘモグロビンHbA1cを減少させる;
(h)糖尿病合併症、例えば糖尿病性腎障害、網膜症、神経障害または心臓血管疾患の進行を緩慢にするか、こうした疾患を遅延させるかまたは治療する;
(i)体重を減少させるかまたは体重増加を防止するかまたは体重減少を促進する;
(j)膵臓ベータ細胞の変性を治療する;
(k)膵臓ベータ細胞の機能性を改善し、そして/または回復させる;
(l)膵臓インスリン分泌の機能性を刺激し、そして/または回復させる;
(m)グルコースのリン酸化を増進させる;あるいは
(n)インスリン感受性を維持し、そして/または改善する;そして/または高インスリン血症および/またはインスリン耐性を治療する。
メトホルミン、シタグリプチンまたはエキセナチドと組み合わせたGK活性化因子の有効性を、II型糖尿病に関する遺伝的機構に相当するin vivoモデル、例えばob/obマウスにおける、異なる機能的終点、例えば体重または食物摂取または耐糖能または血漿空腹時もしくは摂食時グルコースまたは血漿インスリンもしくはインスリン感受性指数に関して調べた。
ベースライン期の最初の5日間、動物に1%カルボキシメチルセルロース(Sigma C4888;ロット120M0216V)ビヒクルを投与した。ベースライン期の最後の2日間、動物に、蒸留水中の20% gelucire(Gattefosse;ロット103201)のビヒクルを投与した。FRI−1を、到着に際して、そして使用まで、デシケーター中で冷蔵保存した。メトホルミンをSigmaから購入した(カタログ番号D150959;ロットBCBF1484V)。エキセナチドをAmerican Peptideから購入した(カタログ番号46−3−12B;ロットY10049A1)。リン酸シタグリプチンをTocris Cookson、英国から購入した(ロットTCS9133B)。適切な量のビヒクル(蒸留水中20% gelucire)を、薬剤物質の重量測定した量を含有するバイアルに直接添加した。化合物を10秒間ボルテックスし、そして室温で投薬した。薬剤混合物(すなわち群EおよびF)を単一投薬ボーラスとして投与した。適切な量のビヒクル(蒸留水中20% gelucire)を、FRI−1およびメトホルミン両方の重量測定した量を含有するバイアルに直接添加した。生じた配合物を10秒間ボルテックスし、そして室温で投薬した。薬剤を毎日、投薬直前に配合し、そして2.5mL/kgの用量体積を用いて投与した。D−グルコースを体積2.5mL/kgで投与した。
第1日の体重を共変数とした共分散分析によって、体重、ならびに全体のおよび毎週の体重増加(g)を決定した。ベースライン期(第−6日〜第0日)中の平均1日食物摂取を共変数とした共分散分析によって、食物および累積食物摂取(g)を決定した。同じ方法を用いて、平均食物摂取を決定した。M概算およびHuber加重法を用い、デフォルトパラメータc=1.345を用いた、ロバスト回帰モデルによって、水摂取(g)を決定した。モデルは、因子として治療を有し、そして共変数として、ベースライン期(第−6日〜第0日)中の平均1日水摂取を有した。同じ方法を用いて、平均水摂取を分析した。ビヒクル群に対する多数の比較は、FRI−1に関してはWilliam検定、メトホルミンに関しては多重t検定、そしてメトホルミンとFRI−1の組み合わせに関してはDunnett検定によった。FRI−1およびメトホルミンの組み合わせを、William検定によってメトホルミン単独に比較し、そして多重t検定によってFRI−1単独に比較した。因子として治療を、ならびに共変数として出血順、ベースライン体重、血漿グルコースおよびインスリンを伴うロバスト回帰によって、血漿グルコースおよびインスリンデータを分析した後、適切な比較(両側)を行って、対照群からの有意な相違を決定した。統計法は、データが通常、群において、等しい分散で分布していることを仮定する。残差の正規性に関する最初のShapiro−Wilk検定は、対数(グルコース)および対数(インスリン)が、グルコースおよびインスリンよりも正規分布していることを示し、したがって、対数変換を用いた。しかし、いくつかのShapiro−Wilk検定は有意であり、したがって、ロバスト回帰を用いて、これらの変数の分析において、いかなるありうる異常値も加重を下げた(downweight)。

B2ベースライン上のAUC(AUCB2)をAUC120分−2t0時間およびAUC60分−t0時間として計算した。上記等式において、tは添え字によって示される特定の時間でのグルコースまたはインスリン濃度を示す。負である可能性もあるため変換しなかったAUCB以外は、対数変換を用いた。OGTTの分析は、M概算、Huber加重を用い、デフォルトパラメータc=1.345を用いたロバスト回帰モデルによった。モデルには、因子として治療およびアッセイ日が含まれ、そして共変数として、出血順、第1日の体重および第−5日(空腹時)のベースライン血漿対数(グルコース)または対数(インスリン)が含まれた。AUCBに関しては、対数(グルコース)または対数(インスリン)の代わりに、共変数として非変換第−5日のグルコースまたはインスリンを用いた。第7日の空腹時血漿グルコースおよびインスリンデータを対数変換し、そして因子として治療を、そして共変数として、出血順、第1日の体重および第−5日(空腹時)のベースライン血漿対数(グルコース)または対数(インスリン)を伴うロバスト回帰によって分析した。第6日および第13日の非空腹時血漿グルコースおよびインスリンデータを対数変換し、そして因子として治療を、そして共変数として、出血順、第1日の体重および第−6日(非空腹時)のベースライン血漿対数(グルコース)または対数(インスリン)を伴うロバスト回帰によって分析した。ビヒクルに対する比較は、FRI−1に関してはWilliam検定、メトホルミンに関しては多重t検定、そして組み合わせに関してはDunnett検定によった。組み合わせ治療を、William検定によってメトホルミンに比較し、そして多重t検定によってFRI−1の同じ用量に比較した。p<0.05が統計的相違として許容されるレベルであった。偽陽性の確率は各時点の各化合物に関して5%である。両側検定として、検定を行った。
FRI−1およびメトホルミンの長期投与は、図1に示すように、オスob/obマウスにおいて、体重コントロールに対して相乗効果を示した。FRI−1(75mg/kg po)およびメトホルミン(100mg/kg po)の組み合わせでの1日1回の治療は、ビヒクル治療対照に比較して、研究第9日、第10日、第12日、第13日、および第14日に、有意に体重を減少させた(p>0.05)。研究の投薬期中、他の薬剤治療はいずれも、体重に対して統計的に有意な効果を持たなかった。
自由摂食ob/obマウスから、血漿インスリンを決定した。FRI−1およびメトホルミンの長期投与は、図2および3に示すように、インスリン感受性の増加に対して相乗効果を示した。ビヒクル処理動物において、血漿インスリンは、およそ110ng/mLのベースラインレベルから、第13日の180ng/mLまで増加した。高用量FRI−1およびメトホルミンの組み合わせは、血漿インスリンに対して有意な効果を有した。具体的には、この組み合わせ治療は、血漿インスリンを第6日(p>0.05)および第13日(p<0.001)に有意に減少させた。インスリンのこの減少は、ビヒクル処理対照のレベルに対してだけでなく、75mg/kg FRI−1(p<0.01)またはメトホルミン(p<0.001)単独のいずれかで治療された動物の第13日のインスリンレベルに対しても有意に異なった。
FRI−1およびシタグリプチンの長期投与は、図4に示すように、メス食餌誘導性肥満ラットにおいて、体重コントロールに対して相乗効果を示した。
ob/obマウスにおけるFRI−1およびエキセナチドの長期投与は、図5および6に示すように、インスリン感受性の増加に対して、相乗効果を示した。ビヒクルに比較した際、第7日に血漿グルコースを減少させる、エキセナチド、およびエキセナチドにFRI−1(25mg/kg po)を加えた組み合わせの小さいが有意な効果が観察された。エキセナチド、およびエキセナチドに両方の用量のFRI−1(25mg/kg po)を加えた組み合わせには、ビヒクルに比較して、第7日に血漿インスリンを減少させる、有意な効果があった。追加の統計比較によって、どちらの組み合わせの効果も第14日に血漿インスリンを、FRI−1単独のいずれかの用量の効果よりも有意に大きく減少させることが示された。
Claims (10)
- インスリン感受性を改善し、糖尿病を治療し、または糖尿病を有する被験体における体重を減少させるのに使用するための医薬組成物であって、
i){2−[3−シクロヘキシル−3−(トランス−4−プロポキシ−シクロヘキシル)−ウレイド]−チアゾル−5−イル−スルファニル}−酢酸または薬学的に許容されうるその塩、
ii)GLP−1類似体または薬学的に許容されうるその塩;および
iii)少なくとも1つの薬学的に許容されうるキャリアー、賦形剤、希釈剤またはその混合物
を含む組成物。 - 請求項1の医薬組成物であって、GLP−1類似体がエキセナチドまたは薬学的に許容されうるその塩である組成物。
- II型糖尿病を治療する方法に使用するための医薬組成物であって、{2−[3−シクロヘキシル−3−(トランス−4−プロポキシ−シクロヘキシル)−ウレイド]−チアゾル−5−イル−スルファニル}−酢酸または薬学的に許容されうるその塩、ならびにGLP−1類似体または薬学的に許容されうるその塩を含む組成物。
- 請求項3の医薬組成物であって、GLP−1類似体がエキセナチドまたは薬学的に許容されうるその塩である組成物。
- 血糖コントロールを改善する方法に使用するための医薬組成物であって、{2−[3−シクロヘキシル−3−(トランス−4−プロポキシ−シクロヘキシル)−ウレイド]−チアゾル−5−イル−スルファニル}−酢酸または薬学的に許容されうるその塩、ならびにGLP−1類似体または薬学的に許容されうるその塩を含む組成物。
- 請求項5の医薬組成物であって、GLP−1類似体がエキセナチドまたは薬学的に許容されうるその塩である組成物。
- 被験体における状態を治療する方法に使用するための医薬組成物であって、{2−[3−シクロヘキシル−3−(トランス−4−プロポキシ−シクロヘキシル)−ウレイド]−チアゾル−5−イル−スルファニル}−酢酸または薬学的に許容されうるその塩、ならびにGLP−1類似体または薬学的に許容されうるその塩を含み、該状態が、糖尿病に関連しており、そして、グルコース不耐性、糖尿病前症、インスリン耐性、高血糖症、耐糖能障害(IGT)、シンドロームXおよび空腹時血糖異常(IFG)からなる群より選択される組成物。
