Description
前記免疫原性ポリペプチドをコード化する核酸は、限定されないが、例えば、野生型又は突然変異型の肺炎球菌菌体から単離される場合があるが、代替的には、ポリメラーゼ連鎖反応(PCR)法を用いることによるか、当業者に認識される代替的な標準的技術を用いることによって、適用可能なDNA遺伝子配列(例えばpcpA又はphtD)を有する肺炎球菌株のDNAから直接入手される場合もある。使用することが可能な菌株は、例えば、肺炎球菌TIGR4株及び14453株を含む。好ましい実施態様では、前記ポリペプチドは肺炎球菌14453株から組換え技術により得られる。
組成物(例えばワクチン組成物)は、アジュバントとともに、あるいは、アジュバントなしに投与される場合がある。一般にアジュバントは抗原の免疫原性を増強できる物質である。アジュバントは獲得免疫及び自然免疫(例えばtoll様受容体)の両方で役割を果たし、その全てが理解されているわけではないさまざまなやり方で機能する場合がある。
CD4+T細胞は、例えば肺炎球菌のような細胞外病原体に対しては最も重要であると考えられる。MHCクラスII分子のコンテキストで抗原提示細胞(APCs)に装荷された抗原で刺激されると、未熟なCD4+T細胞は、機能的に異なるTヘルパー(Th)のサブセットに分化する場合がある。異なるThサブセットへのコミットメントは、抗原タイプ及び装荷、共刺激分子及びサイトカインシグナル伝達を含む、許容的環境でのAPCsとの複雑な相互作用に依存する(本件文献7−9)。例えば、インターロイキン(IL)−2、インターフェロン−ガンマ(IFN−γ)及び腫瘍壊死因子−ベータ(TNF−β)産生を特徴とするTh1細胞は、細胞内病原体を除去するために最重要である。細胞外病原体を除去するのに必須のTh2細胞は、IL−4、IL−5、IL−6、IL−10、IL−13及びIL−25を発現する。最近発見されたTh17細胞はIL−17、IL−21及びIL−22を分泌する(本件文献10)。
肺炎球菌タンパク質抗原を用いて、小児での耳炎好発状態を評価するために、耳炎非好発及び耳炎好発小児のコホートの末梢血中の肺炎球菌特異的機能メモリーCD4+Th細胞サブセットが定量された。これらのコホートの小児の血清中での同一抗原に対するB細胞IgG応答も計測された。
使用される肺炎球菌タンパク質抗原は、PhtD(配列番号6)、PhtE(配列番号8)、LytB(配列番号11)、PcpA(配列番号3)及びニューモリシンの無毒化誘導体PlyD1(配列番号13)である。対照実験として、PspAも使用される。各タンパク質は肺炎球菌血清型6B株式会社からクローン化され、大腸菌で可溶性タンパク質として組換え技術で発現され、イオン交換クロマトグラフィーの組合せで精製された。SDS−PAGE法及びRP−HLPC法によりアッセイしたところ、各タンパク質は精製後90%以上の純度であった。
肺炎球菌が鼻咽喉に定着しているか、肺炎球菌で急性中耳炎感染している耳炎好発性及び耳炎非好発性小児由来の末梢血単核細胞は前記6種類の肺炎球菌抗原で刺激され、無莢膜型インフルエンザ菌が鼻咽喉に定着しているか、無莢膜型インフルエンザ菌で急性中耳炎感染している小児は3種類の無莢膜型インフルエンザ菌抗原で刺激された。刺激前に、凍結末梢血単核細胞は37℃のウオーターバスで急速解凍され、ゆっくりと完全培地(10%のFBSと、2mMのL−グルタミンと、0.1mMのピルビン酸ナトリウムと、非必須アミノ酸と、100U/mlのペニシリンと、100μg/mLのストレプトマイシンとが添加されたRPMI1640)が添加された。それから、細胞は洗浄され、完全培地の入った24穴プレート中で終夜静置された。末梢血単核細胞は過去の報告(本件文献35、36)から適応された標準的なプロトコールを用いて刺激された。簡潔には、細胞数が計測され、96穴平底培養プレートに入れられ、1μg/mLのさまざまなタンパク質抗原か、1μg/mLのブドウ球菌エンテロトキシンB(SEB)かのいずれかで刺激された。共刺激を提供し、抗原特異的応答を増強するために1μg/mL濃度の抗CD28及び抗CD49d抗体(それぞれクローンL293及びL25、BD Biosciences)が前記細胞培養に添加された。抗CD28及び抗CD49d抗体はバックグランドレベルに影響のない共刺激のために広く用いられている(本件文献18、37)。その後細胞は抗原プロセシングのために、2時間、37℃、5%CO2存在下でインキュベーションされた。2時間後、ゴルジ輸送阻害剤(BD Biosciences)がサイトカインを細胞内に保持するために添加され、インキュベーションがさらに4時間継続された。
マルチパラメーターフロー・サイトメトリー法が、本研究のコホートにおける急性中耳炎又は鼻咽喉定着後の循環血中肺炎球菌タンパク質に対する特異的CD4+T細胞応答を検出するために用いられた。細胞内サイトカイン染色アッセイ(ICCS)が抗原特異的CD4+T細胞サブセット(Th−1、Th−2及びTh−17)を評価するために用いられた。刺激後、細胞は96穴V字底プレートに移され、FACSバッファー(5%FBSが添加されたPBS)で1回洗浄され、さまざまな細胞表面マーカーに対する抗体で染色された。使用された抗体は、抗CD4APC Alexafluor 750(クローンRPA T4、eBiosciences)、PE−テキサスレッド抗CD45RA(クローンMEM56、Invitrogen)及び抗CCR7 PerCP/Cy5.5コンジュゲート(クローンTG8/CCR7、Biolegend)であった。細胞は固定及び透過化溶液(BD Biosciences)で20分間透過化され、その後1×透過化バッファー(BD Biosciences)で3回洗浄された。さまざまなサイトカイン特異的抗体のカクテルが刺激の結果細胞内に捕捉されたサイトカインを染色するのに用いられた。使用された抗体は、PE−Cy7結合抗IFN−γ(クローンB27、BD Biosciences)と、パシフィックブルー結合抗IL17A(クローンBL168、Biolegend)、Aleca fluor700抗IL−2(クローンMQ1−17H12、Biolegend)、PE結合抗IL−4(クローン8D4−8、BD Biosciences)、AF488結合TNF−α、抗CD3 Qdot605(クローンUCHT1、Invitrogen)及びPE−Cy5抗CD69(クローンFN50、BD Biosciences)であった。細胞内染色後、細胞はさらに3回1×透過化バッファーで洗浄され、FACS用試験管に再懸濁する前にFACSバッファーで1回最終洗浄された。12種類の蛍光パラメーター検出用に装備された注文生産のBD LSRIIフロー・サイトメーターが、各サンプルについて2−5×105回のイベントを収集するために使用され、データはFLO JO(Tree Star)ソフトウェアを用いて解析された。細胞の破片及びクランプを排除するために、細胞は最初に前方及び側方散乱特性に基づいてゲーティングされ、その後、CD4+T細胞、CD45RA弱陽性、そしてCD69+サイトカイン陽性細胞と順番にゲーティングされた。代替的には、細胞は確認のためにTNF−α対他のサイトカイン(TNF−α Vs other cytokines)でもゲーティングされた。低頻度の応答細胞は過剰な戻しゲーティングによって確認された。過去に報告されたとおり、抗原特異的細胞の検出を補助するために、マルチパラメーター染色とともに抗CD28/CD49d抗体が使われて無関係なバックグランドを避けるのを手助けする(本件文献37)。全アッセイは標準化され、サイトカインのプロフィール検出についてマルチプレックスビーズアレイ(CBA、BD Biosciences)と比較された。
サンプル中のIgG抗体レベルを計測するために、以前説明されたとおりELISA法が実行された(本件文献26、38)。簡潔には、96穴プレート(Nunc−Immulon)が、コーティングバッファー(重炭酸(pH9.4))中の0.25μg/mLの個々の抗原(ウェルあたり100μL)でコーティングされ、終夜4℃でインキュベーションされた。洗浄後、前記プレートは3%の脱脂粉乳(ウェルあたり200μL)で37℃1時間ブロッキングされた。5回洗浄後、血清が初回希釈1:100(3%脱脂粉乳入りのPBS)で前記ウェルに100μL添加され、2倍ずつ連続希釈された。前記混合液は室温で1時間インキュベーションされ、その後、2次抗体として、ホースラディッシュペロキシダーゼ(Bethyl Laboratories,Inc,Montgomery、テキサス州)に結合されたアフィニティー精製ヤギ抗ヒトIgG抗体が添加された。反応産物は、TMB Microwellペロキシダーゼ基質システム(KPL、Gaithersburg、メリーランド州)を用いて発色され、1.0モルリン酸の添加により反応が停止され、450nmフィルターを用いる自動ELISAリーダーによって読み取られた。抗体濃度に関する定量的結果を提供するために、未知サンプル中に存在する特異抗体レベルが内部参照血清(抗原特異的抗体レベルが高いヒト血清のプール)との比較により決定された。4種類のパラメーターのロジスティク−対数関数が、参照及びサンプルの曲線を作成するのに用いられた。このELISA法はICHガイダンスに従って完全に認証された。
耳炎好発性グループの小児は耳炎非好発性の小児と年齢は類似していた。性別、託児所利用歴、家庭内受動喫煙曝露、8才未満の兄弟姉妹の数及び母乳哺育歴の分布は2つの研究グループ間で類似していた。
本研究では、1)AOMエピソードが6ヶ月間に3回以上又は12ヶ月間に4回以上の小児を含む耳炎好発性群と、2)適切な抗生物質療法の少なくとも48時間後に細菌除去及び/又は徴候の消失を達成できなかった小児(本件文献70、71)と、1回の抗生物質治療コースを完了して14日以内に急性中耳炎の徴候がぶり返した小児とを含む急性中耳炎治療失敗(AOMTF)群と、3)急性中耳炎のエピソードが1回か2回しかなかった小児を含む耳炎非好発性群という、3群の急性中耳炎を罹患している生後6ヶ月ないし36ヶ月の小児でのPhtD、PhtE、LytB、PcpA及びPlyに対する血清IgG抗体の発生が比較された。
肺炎球菌のPhtD,LytB、PcpA、PhtE及びPlyタンパク質に対するIgG抗体力価が、耳炎好発性小児35名、急性中耳炎治療失敗(AOMTF)小児25名及び耳炎非好発性群の1回目又は2回目の急性中耳炎罹患時の小児34名の急性中耳炎急性期に測定された(図4)。
耳炎好発性小児22名、急性中耳炎治療失敗(AOMTF)小児13名及び耳炎非好発性小児20名から急性期(急性中耳炎罹患時)及び回復期(3週間後)に1対の血清サンプルが得られた。小児の3群全てで、5種類のタンパク質に対するIgG抗体レベルのうち4種類は急性期対回復期で有意な上昇は認められなかった(例外は急性中耳炎治療失敗小児でのPhtEタンパク質で、p=0.04と有意差が認められた。表1)。しかし急性期及び回復期の血清中の抗体レベルに大きな個別のばらつきが認められ、3群全ての小児の一部では1種類以上の抗原に対する抗体で2倍の上昇がみられた(表2)。
図5は、生後6−24ヶ月の予後追跡中の耳炎非好発性小児及び耳炎好発性小児が急性中耳炎を罹患していない定期診察を受けたときのPhtD、LytB、PcpA、PhtE及びPlyに対するIgG抗体レベルを示す。示されたデータは、耳炎非好発性小児150名及び耳炎好発性小児10名からのものである。耳炎非好発性小児では、LytB(p=0.075)を除く全てのタンパク質でIgG抗体レベルは経時的に有意に上昇した(p<0.001)。これに対し、耳炎好発性小児は、5種類のタンパク質のいずれについてもIgG抗体レベルの有意な経時的変化は認められなかった(PhtDタンパク質につきp=0.40、KytBについてp=0.39、PcpAについてp=0.11、PhtEについてp=0.09、Plyについてp=0.42)。
実施例1で言及された研究由来の耳炎好発性小児及び耳炎非好発性小児多数から得られた血清サンプル中の肺炎球菌抗原特異的メモリーB細胞の循環頻度が評価及び比較された。研究対象小児総数約387名中22名の小児がここでは研究された。(十分な量の末梢血単核細胞サンプルの利用可能性にもとづいて)耳炎好発性小児10名が本実施例の研究のために同定され、前記耳炎好発性小児と類似の年齢で急性中耳炎罹患回数1回又は2回の耳炎非好発性小児12名が対照の役割を果たすためにランダムに選択された。前記小児の臨床的な特徴は表3に示される。
これら2群の小児の血清中の抗原特異的IgG力価は、プレートは0.5μg/mLの抗原でコーティングされ、アフィニティー精製されたホースラディッシュペロキシダーゼ(Bethyl Laboratories,Inc.、Montgomery、テキサス州)に結合されたヤギ抗ヒトIgG、IgM又はIgA抗体が2次抗体として用いられた。

Claims (19)
- 肺炎球菌感染に起因する急性中耳炎の再発を、肺炎球菌急性中耳炎の再発を生じるリスクがある患者において予防又は治療するのに使用するための組成物であって、治療上有効量の、肺炎球菌PhtD、PhtE、PcpA、LytBおよび無毒化ニューモリシンからなる群より選択される少なくとも1つの単離および実質的に精製された免疫原性ポリペプチドまたはそれらの免疫原性断片を含み、少なくとも1回経皮的に投与される、組成物。
- 前記患者は、6ヶ月以内に3回以上、あるいは12ヶ月以内に4回以上、急性中耳炎の発症を経験している、請求項1記載の組成物。
- 前記患者は、PhtD、PhtE、PcpA、LytBおよび無毒化ニューモリシンからなる群より選択される少なくとも1つの肺炎球菌抗原またはそれらの免疫原性断片に対して免疫低応答性を示す、請求項1記載の組成物。
- 前記患者は、肺炎球菌感染に起因する急性中耳炎の発症を経験し、適切な抗生物質療法の少なくとも48時間後に細菌の除去および/又は症状の消失を達成できなかったものである、請求項1記載の組成物。
- 前記患者は、肺炎球菌感染に起因する急性中耳炎の発症を経験し、該急性中耳炎のための抗生物質治療コースを完了して14日以内に急性中耳炎の症状が再発したものである、請求項1記載の組成物。
- 前記患者は急性中耳炎に罹患している、請求項1から5いずれか1項記載の組成物。
- 組成物の投与が、抗原特異的CD4+T細胞の産生を惹起する、請求項1記載の組成物。
- 組成物の投与が、IFN−γ、IL−4、IL−2および/又はIL−17aを産生する抗原特異的CD4+T細胞の産生を惹起する、請求項7記載の組成物。
- 組成物の投与が、前記少なくとも1つの単離および精製された免疫原性ポリペプチドを欠く組成物の投与と比較して、IFN−γ、IL−4、IL−2および/又はIL−17aを産生する抗原特異的CD4+T細胞の百分率を高める、請求項7記載の組成物。
- 組成物の投与が、IFN−γ、IL−4、IL−2および/又はIL−17aサイトカインの産生を促進する、請求項1記載の組成物。
- アジュバントをさらに含む、請求項1記載の組成物。
- 肺炎球菌PhtD、PcpAおよび無毒化ニューモリシンからなる群より選択される少なくとも1つの単離および精製された免疫原性ポリペプチド、またはそれらの免疫原性断片を含む、請求項1記載の組成物。
- 前記組成物は、肺炎球菌PhtD、PcpAおよび無毒化ニューモリシン、またはそれらの免疫原性断片を含む、請求項1記載の組成物。
- 前記無毒化ニューモリシンは、野生型配列の第65番目、第293番目および第428番目の位置にアミノ酸置換を含む突然変異型ニューモリシンタンパク質である、請求項12または13記載の組成物。
- 前記3箇所のアミノ酸置換は、T65→C、G293→CおよびC428→Aを含む、請求項14記載の組成物。
