実施例1
種々のpHでの及び種々のシクロデキストリン誘導体濃度での溶液中における遊離塩基メルファラン(Chemwerth,Woodbridge,CT)の溶解速度を調べた。手順は以下の通りであった:遊離塩基メルファランを、シクロデキストリン誘導体(SBE6.5−β−CD、カプチゾル(登録商標))を含有する溶液へ添加し、次いで、1〜

遊離塩基メルファラン(Chemwerth,Woodbridge,CT)の結合を、pH5及びpH7でのシクロデキストリン誘導体濃度の関数として研究し、データを遊離塩基メルファラン結合の文献報告と比較した。pH5溶液は、100mM酒石酸水素ナトリウム緩衝剤を含有し、0.9%塩化ナトリウム溶液中の水酸化ナトリウムを使用してpH5へ調節した。pH7溶液は、各々50mMの一塩基性及び二塩基性リン酸塩並びに0.9%塩化ナトリウムを含有した。0、50、75及び100mM SBE6.5−β−
CD(カプチゾル(登録商標))を含有する溶液を調製し、過剰の遊離塩基メルファランを各溶液の2mLサンプルへ添加した。遊離塩基メルファランの添加後、サンプルを30秒間ボルテックス混合し、氷水浴中において20分間超音波処理し、次いで、室温で30分間エンドオーバーエンド回転によって混合した。次いで、サンプルを遠心分離し、透明な上澄みを水で希釈し、HPLCによって分析した。

均M.W.=2163g/mol)との結合を、pH7.5でのシクロデキストリン誘導体濃度の関数として測定した。温度を22℃に維持し、25mMリン酸緩衝液を各溶液へ
添加した。データを、pH7.5の25mMリン酸緩衝液中のSBE7−β−CD(平均
M.W.=2248g/mol)との遊離塩基メルファラン結合の文献報告と比較した(実施例2を参照のこと)。
離塩基メルファランを添加することによって、サンプルを調製した。サンプルを30秒間ボルテックス混合し、20〜24℃で60分間超音波処理し、次いで、22℃で60分間エンドオーバーエンド回転によって混合した。次いで、サンプルを遠心分離し、透明な上澄みを水で希釈し、HPLCによって分析した。データを図2に提供する。図2を参照して、メルファラン塩酸塩は、25mMを超える全てのシクロデキストリン誘導体濃度について、遊離塩基メルファランと比較して、顕著な溶解性促進を示した。
塩酸塩としてのメルファランを含む薬学的組成物を、図3において図で概説されるプロセスによって作製した。図3を参照して、注射用水、USPを、15〜20℃の温度でステンレススチールミキサー中に入れ、約4.6のpHが達成されるまで塩酸を添加した。得られた溶液を、ボルテックス(但し発泡又は泡立ち無し)を生じさせるために十分な速度で約15分間撹拌し、ボルテックス撹拌しながらシクロデキストリン誘導体(27.2g SBE6.5−β−CD、カプチゾル(登録商標))を徐々に添加し、得られた溶液を
約15分間撹拌し、完全な溶解を確実にした。得られた溶液は約2.5のpHを有した。ボルテックス撹拌しながら塩酸塩としてのメルファラン(516mg)を徐々に添加し、得られた溶液を約15分間撹拌し、完全な溶解を確実にした。次いで、溶液が約5.6のpHを有するまで、ボルテックス撹拌しながら、塩基(2N NaOH)を徐々に添加した。次いで、UV/vis分光光度計(260nmの検出波長)を使用して、溶液をアッセイした。溶液は、5.16mg/mLの濃度でメルファランを含み、メルファランは、シクロデキストリン誘導体に対して約1:55w/wの比で存在した。次いで、溶液を滅菌フィルター(0.22μm PVDF)に通し、10〜15℃へ冷却した。
実施例4において得られた液体薬学的組成物を凍結乾燥し、塩酸塩としてのメルファラン50mgを含む再構成可能な及び/又は希釈可能なドライパウダーを得た。ガラスバイアルに溶液(10mL)を満たし、5℃の予め冷却された棚の上のトレー中に置いた。バイアルを約30分間熱平衡化させ、次いで凍結乾燥し、各バイアル中においてドライパウダーを得た。バイアルを約400mTorrの圧力の窒素で充填し、次いで密封した。
最終溶液が10mg/mLの濃度でメルファランを含有し、メルファランがシクロデキストリン誘導体に対して約1:27w/wの比で存在したことを除いて、実施例4において説明されかつ図3において図で概説されるプロセスによって、塩酸塩としてのメルファランを含む薬学的組成物を作製した。
実施例6において得られた液体薬学的組成物を凍結乾燥し、塩酸塩としてのメルファラン200mgを含む再構成可能な及び/又は希釈可能なドライパウダーを得た。ガラスバイアルを溶液(20mL)で満たし、5℃の予め冷却された棚の上のトレー中に置いた。バイアルを約30分間熱平衡化させ、次いで凍結乾燥し、各バイアル中においてドライパウダーを得た。バイアルを約400mTorrの圧力の窒素で充填し、次いで密封し、包装し、ラベルを貼った。バイアルを、凍結乾燥、充填、密封、包装、及びラベリング手順の全ての局面の間、光への曝露から保護した。
実施例4及び6において得られた液体薬学的組成物を無菌噴霧乾燥し、無菌的に充填される自由流動性粉末を得る。自由流動性粉末は、実施例4又は6において作製された凍結ドライパウダーの溶解特性に対応するか又はこれを超える。
塩酸塩としてのメルファランを含む薬学的組成物を、図4において図で概説されるプロセスによって作製した。図4を参照して、注射用水、USPを、18〜25℃の温度でステンレススチールミキサー中に入れ、得られた溶液を、ボルテックス(但し発泡又は泡立ち無し)を生じさせるために十分な速度で撹拌した。ボルテックス撹拌しながらシクロデキストリン誘導体(SBE6.5−β−CD、カプチゾル(登録商標))を徐々に添加し、
得られた溶液を約15分間撹拌し、完全な溶解を確実にした。次いで、得られた溶液を約2〜8℃へ冷却した。ボルテックス撹拌しながら塩酸塩としてのメルファランを徐々に添加し、得られた溶液を約15分間撹拌し、完全な溶解を確実にした。溶液が約5〜6(標的pH5.5)のpHを有するまで、ボルテックス撹拌しながら、塩基(2N NaOH)を徐々に添加した。次いで、インプロセスコントロール(「IPC」)アッセイを行い、pHをモニタリングし、注射用水、USPを使用して、溶液を最終標的体積へ希釈した。次いで、UV/vis分光光度計(260nmの検出波長)を使用して溶液をアッセイし、バイオバーデンアッセイを行った。次いで、溶液を滅菌フィルター(0.22μm PVDF)に通し、15〜25℃へ冷却した。最後に、IPCアッセイを行った。
実施例8において調製した溶液を凍結乾燥し、塩酸塩としてのメルファランを含む再構成可能な及び/又は希釈可能なドライパウダーを得た。凍結乾燥のために、ガラスバイアルに溶液(10mL)を満たし、5℃の予め冷却された棚の上のトレー中に置いた。バイアルを約1時間熱平衡化させ、次いで凍結乾燥し、各バイアル中においてドライパウダーを得た。バイアルを窒素で充填し、密封し、包装し、ラベルを貼った。バイアルを、凍結乾燥、充填、密封、包装、及びラベリング手順の全ての局面の間、光への曝露から保護した。
実施例8において得られた液体薬学的組成物を無菌噴霧乾燥し、無菌的に充填される自由流動性粉末を得る。自由流動性粉末は、実施例9において作製された凍結ドライパウダーの溶解特性に対応するか又はこれを超える。
注射用水、USPでの希釈後の本発明の薬学的組成物の特性を、様々な分析法によって分析した。結果を下記の表に列挙する。組成物A〜Dを実施例8〜9に記載の方法によって作製した。希釈組成物は、それぞれ、75mM、100mM、125mM、及び125mMの濃度でSBE6.5−β−CD(カプチゾル(登録商標))を含有した。組成物の各
々は、希釈前に約1.3%〜約2.5%の含水率を有した。
本発明の薬学的組成物の希釈でのメルファラン塩酸塩の安定性を測定した。シクロデキストリン誘導体(SBE6.5−β−CD、カプチゾル(登録商標)、平均M.W.=21
63g/mol)を含有する薬学的組成物を等張生理食塩液で希釈し、それぞれ、75mM及び125mMの濃度でシクロデキストリン誘導体を含有する0.45mg/mLメルファラン溶液を得た。メルファランを時間の関数としてアッセイし、メルファランの5%又は10%減少(100%の初期メルファラン濃度に基づく)に必要な時間を測定した。データを下記の表に提供する。シクロデキストリン誘導体を含有する溶液中のその初期濃度の90%又は95%へメルファランが低下するために必要とされた時間を、参照製品(注射用アルケラン(登録商標)、GlaxoSmithKline)中のメルファランの安定性と比較した。
本発明の薬学的組成物の希釈でのメルファラン塩酸塩の安定性を、温度及び保存条件の関数として測定した。メルファラン(50mg)及びシクロデキストリン誘導体(SBE6.5−β−CD、カプチゾル(登録商標)、平均M.W.=2163g/mol、270
mg)を含有する薬学的組成物を、等張生理食塩液(8.5mL)で希釈し、濃縮溶液を得た。濃縮溶液を10倍さらに希釈し、希釈溶液を得た。濃縮及び希釈メルファラン溶液の各々を、25℃/60%相対湿度で、又は冷蔵庫(約10℃)中において保存し、メルファラン含有量を時間の関数としてモニタリングした。データを下記の表に提供する。
凍結乾燥メルファラン塩酸塩組成物の安定性を、温度及び保存条件の関数として、希釈前及び後に測定した。CyDex Pharmaceuticals,Inc.の指導の下でBioConvergence LLC,Bloomington,INによって研究は行われた。

タンパク質結合研究のために、放射標識メルファラン、非放射標識メルファラン、及びSAE6.5−β−CD(適用可能な場合)を、シンチレーションバイアルへ添加し(20
mL)、窒素流下でブロー乾燥し、各実験において存在する溶媒の量を標準化した。メタノール(50μL)をバイアルへ添加し、ガラス血清ピペットを使用して、ブランクヒト血漿(5mL)をバイアルへ添加した。次いで、混合物を短時間ブレンドした。次いで、アリコート(3x1mL)をバイアルから限外濾過装置中へ入れ、続いて遠心分離した(25℃で5分間2,000g)。バイアルへのヒト血漿の添加と遠心分離の開始との時間間隔は、0.5、1、5、10、及び30分であった。次いで、バイアル中に残っていた溶液をLSC分析のために二重で小分けした(0.1、0.05、及び0.025mLの体積で)。

スルホアルキルエーテルシクロデキストリンがメルファランのインビボ薬物動態を混乱させる可能性を調べるために、研究を行った。送達ビヒクル中においてSBE6.5−β−
CDの存在又は非存在下で雄性Sprague Dawleyラットへ静脈内投与後、メルファランについて薬物動態パラメータを測定した。

血液又は血漿対時間プロフィールにおいて観察可能な差異は有さないかったことを示している。さらに、ラットモデルにおいてメルファランの尿中排泄の見かけの差異はなかった。
プロピレングリコールフリーの希釈用ビヒクルを使用して注射によって投与されたメルファラン塩酸塩の第IIa相・多施設・非盲検・無作為化効能及び安全性試験を、自家移植のための準備において骨髄破壊的前処置を受けた3人のヒト多発性骨髄腫患者において行った。試験は継続中である。
診断時又はその後に治療を必要とする症候性多発性骨髄腫を有する患者;
移植のための適切な一次導入療法を受容した自家幹細胞移植療法に適格である多発性骨髄腫を有する患者;
移植時に70歳以下である患者(70歳を超える患者は、機関実施基準に基づく基準を患者が満たす場合、個々の場合に応じて適格であり得る);
別のバッグ中に保存される2×106 CD34+細胞/kgの予備品と共の、少なく
とも2×106 CD34+細胞/患者体重kgを含有する操作されていない、凍結保存
された、末梢血幹細胞又は骨髄幹細胞移植片として定義される、適切な自己移植片を有する患者;
並びに
以下によって測定されるような適切な臓器機能を有する患者:
− 心臓:安静時の左室駆出分画>40%;
− 肝臓:ビリルビン<2×正常上限値かつALT/AST<3×ULN;
− 腎臓:クレアチニンクリアランス>40mL/分;及び
− 肺:DLCO、FEV1、FVC>予測値の50%(Hgbについて補正)又はO2飽和>室内空気での92%。
−12個の血液サンプルを薬物動態評価のために特定の時点で採取した。血液サンプルをメルファラン用量の受容の直前及び後に集めた;
−バイタルサインを、各用量のメルファランの受容後の最初の8時間の間、1時間ごとに記録し、次いで、退院まで1日1回、次いで日+30まで毎週、繰り返した。体重を退院時及び日+30に集めた;
−10〜20秒リズムストリップと共に、12誘導心電計(ECG)を、退院まで週2回集め、次いで、12誘導ECG(リズムストリップ無し)を日+30まで毎週集めた;
−集中身体検査を退院まで毎日行い、次いで、完全身体検査を日+30まで毎週行った;
−AE/SAEについての毒性類別及び評価は、全研究期間の間、NCI−CTC AEバージョン3.0に従った;
−ディファレンシャル及び血小板カウントを伴う完全血液カウントを、好中球及び血小板生着まで毎日、次いで日+30まで毎週、行った;
−Eastern Cooperative Groupパフォーマンスステータスを、退院時に、次いで、日+30まで毎週、検査した;
−基本血清化学パネル(ナトリウム、カリウム、塩化物、グルコース、クレアチニン、炭酸水素塩、及びBUN)を、好中球生着まで毎日;
−完全血清化学パネル(ナトリウム、カリウム、塩化物、マグネシウム、炭酸水素塩、グルコース、総タンパク質、アルブミン、カルシウム、リン酸塩、尿酸、BUN、クレアチニン、CPK、総ビリルビン、アルカリホスファターゼ、LDH、SGOT、及びSGPT)を日+30まで毎週モニタリングした;
−尿検査(比重、pH、タンパク質、グルコース、ケトン、亜硝酸塩、RBC、及びWBC)を、退院まで週2回、次いで日+30まで毎週、モニタリングした;並びに
−併用薬を、全研究期間の間、記録した。
E6.5−β−CD、カプチゾル(登録商標)、125mMの濃度で)のいずれかによって
最初のメルファラン用量100mg/m2を受容する(第−3日に)ように、患者を無作
為に選択した。シクロデキストリン誘導体を含む組成物として100mg/m2の第1メ
ルファラン用量を無作為に受容した(第−3日に)患者は、次いで、プロピレングリコール希釈剤を含む組成物を使用して100mg/m2の第2メルファラン用量を受容した(
第−2日に)。逆に、プロピレングリコール希釈剤を含む組成物として100mg/m2
の第1メルファラン用量を無作為に受容した(第−3日に)患者は、次いで、シクロデキストリン誘導体を含む組成物を使用して100mg/m2の第2メルファラン用量を受容
した(第−2日に)。
4+細胞/患者体重kgの最小細胞用量で自己移植片を受容させた(第0日)。プロダクトの凍結保存及び解凍は、Foundation for the Accreditation of Cellular Therapy基準及び地方機関のプラクティスと一致した。移植片を機関のプロトコルによって注入した。日+5に開始して、絶対好中球数が500/mm3よりも大きくなるまで、G−CSFを5μg/kg/日の用量で投与
した。
− 個々の生データから誘導される、Cmax;
− 個々の生データから誘導される、Tmax;
− 見かけの最終一次排出速度定数(kel);
− 見かけの排出tス;
− 台形公式によって計算される、最後の測定可能な時点までの血漿中濃度−時間曲線下面積(AUC0-t);及び
−kelで割った最後の測定可能な時点での濃度から決定される、最後の測定可能な時点から無限大へ外挿される血漿中濃度−時間曲線下面積(AUCt-∞)。
製剤の静脈内投与後及びシクロデキストリンフリーのメルファラン製剤(注射用メルファランHCl、Bioniche Pharma USA)の静脈内投与後のヒト患者における平均血漿中メルファラン濃度の図示を提供する。図6及び上記の表中のデータを参照して、スルホアルキルエーテルシクロデキストリン誘導体と共に投与されたメルファランのインビボ分布は、メルファランの最大インビボ濃度のほぼ50%増加、及び血漿中濃度曲線下面積(即ち、AUC0-t及びAUC0-∞の両方について)のおよそ30%増加を提
供した。上記の表に示されるように、患者2及び3についてのデータは、同様の薬物動態結果を示した。ラットモデルおいてこれらのメルファラン製剤について得られた薬物動態データを考慮して、ヒト患者におけるSBE6.5−β−CD含有メルファラン製剤につい
てのCmax及びAUCの増大は、完全に予想外であった。
− 0.5×109/L未満の絶対好中球数;
− 0.1×109/L未満の絶対リンパ球数;又は
− 20,000/mm3未満の血小板数若しくは輸血を必要とする出血。
数と定義される。
− 3つの連続する毎日の評価での>20,000/mm3の非輸血血小板測定として
定義される、血小板生着の割合;
− 絶対好中球数が0.5×109/Lを超える3つの評価のうちの最初のものとして
定義される、好中球生着までの時間;
− 非輸血血小板測定が20,000/mm3を超える3つの連続する毎日の評価のう
ちの最初のものとして定義される、血小板生着までの時間;
− 自家幹細胞移植日+100までに3つの連続する毎日の評価での0.5×109/
Lを超える絶対好中球数へ到達しないこととして定義される、非生着の割合;
− 最初の100日以内の生着した後の0.5×109/L未満の絶対好中球数の発生
として定義される、後期移植不全又は後期拒絶の割合;
− 日+100でのInternational Working Group基準に従って定義される、多発性骨髄腫奏効率(sCR、CR、VGPR、PR、SD、又はPD);並びに
− 日+100での再発又は進行を伴わない死亡として定義される、治療関連死亡率。
ルファランは、幹細胞移植前の前処置として、幹細胞移植が適応とされている対象における使用について、治療的に有効かつ安全であることを実証すると予想される。
プロピレングリコールフリーのビヒクルを使用する注射によって投与されたメルファラン塩酸塩の第IIb相・多施設・非盲検・無作為化効能及び安全性試験を、症候性多発性骨髄腫を有しかつASCTに適格であるヒト多発性骨髄腫患者において行った。
に投与したことを除いて、研究のパラメータは実施例17に記載のものと同様であった。他の点では、包含基準、排除基準、安全性基準、投薬、治療、及び効能エンドポイントは、実施例17において上述したものと同様であった。

Claims (35)
- 新生物疾患を治療する医薬の製造のための組成物の使用であって、
該組成物は水性希釈剤で希釈されて、メルファラン25mg〜125mgと式I
R1、R2、R3、R4、R5、R6、R7、R8及びR9は、独立して、−H、直鎖又は分枝
鎖C1−C8−(アルキレン)−SO3 -基、又は場合により置換される直鎖又は分枝鎖C1
−C6基であり;
ここで、R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つは、直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基である]
のシクロデキストリン誘導体とを含む希釈薬学的組成物を、注射によって投与する医薬として提供するものであって、
ここで、希釈薬学的組成物は、4〜6のpHを有し;
シクロデキストリン誘導体は、メルファランに対して50:1〜55:1(w/w)の濃度で存在し;及び
希釈薬学的組成物中のメルファランは、希釈後5時間以内で25℃で2%又はそれ未満分解するものであり;及び
前記医薬は新生物疾患に罹患する対象に注射によって投与されるものである;
上記使用。 - 新生物疾患が、骨髄腫、多発性骨髄腫、黒色腫、急性骨髄性白血病、悪性黒色腫、乳が
ん、卵巣がん、精巣がん、進行性前立腺がん、神経内分泌がん、転移性黒色腫、転移性神経内分泌腫瘍、転移性腺がん、肝細胞がん、骨原性肉腫、真性多血症(polycythemia veraplasma)、形質細胞新生物、アミロイドーシス、硬化性粘液水腫、及びそれらの併発より選択される、請求項1に記載の使用。 - 新生物疾患が多発性骨髄腫であり、投与が全身性であり、多発性骨髄腫の緩和療法を提供する、請求項2に記載の使用。
- R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つが、ヒドロキシ置換C3基である、請求項1〜3のいずれか1項に記載の使用。
- R1、R2、R3、R4、R5、R6、R7、R8及びR9が、独立して、シクロデキストリン誘導体1個当たり4〜8の置換度を有する直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基であり、残りの置換基が−Hである、請求項1〜3のいずれか1項に記載の使用。
- R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つが、直鎖C4−(アルキレン)−SO3 -基で置換されている、請求項1〜3又は5のいずれか1項に記載の使用。
- 希釈薬学的組成物がアルコールを実質的に含まない、請求項1〜7のいずれか1項に記載の使用。
- 水性希釈剤が生理食塩液である、請求項1〜8のいずれか1項に記載の使用。
- 新生物疾患に罹患する対象が小児の対象である、請求項1〜9のいずれか1項に記載の使用。
- 希釈薬学的組成物中のメルファランが、希釈後10時間以内で約25℃で4%又はそれ未満分解する、請求項1〜10のいずれか1項に記載の使用。
- 希釈薬学的組成物を投与前に0.5時間〜18時間保存する、請求項1〜11のいずれか1項に記載の使用。
- 投与が、等価用量のメルファランを含有しかつシクロデキストリン誘導体を欠くメルファラン製剤によって提供されるメルファランCmaxよりも少なくとも20%又はそれ以上大きいメルファランCmaxを新生物疾患に罹患する対象に提供する、請求項1〜12のいずれか1項に記載の使用。
- 投与が、等価用量のメルファランを含有しかつシクロデキストリン誘導体を欠くメルファラン製剤によって提供されるメルファランAUC0-tよりも少なくとも20%又はそれ以上大きいメルファランAUC0-tを新生物疾患に罹患する対象に提供する、請求項1〜12のいずれか1項に記載の使用。
- 幹細胞移植が適応とされている対象を前処置する医薬の製造のための薬学的組成物の使用であって、
該薬学的組成物は、メルファランと式I
R1、R2、R3、R4、R5、R6、R7、R8及びR9は、独立して、−H、直鎖又は分枝
鎖C1−C8−(アルキレン)−SO3 -基、又は場合により置換される直鎖又は分枝鎖C1
−C6基であり;
ここで、R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つは、直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基である]
のシクロデキストリン誘導体とを含み、
ここで、薬学的組成物は、4〜6のpHを有し;
ここで、シクロデキストリン誘導体は、メルファランに対して50:1〜55:1(w/w)の比で存在し;及び
医薬を投与することにより、幹細胞移植が適応とされている対象に1日当たり50mg/m2〜300mg/m2のメルファラン用量を提供する、上記使用。 - 投与が2日又はそれより多い期間に及ぶ、請求項15に記載の使用。
- 幹細胞移植の必要がある対象が小児の対象である、請求項15または16に記載の使用。
- 投与を静脈内で行う、請求項15〜17のいずれか1項に記載の使用。
- 投与を四肢灌流を介して行う、請求項15〜18のいずれか1項に記載の使用。
- R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つがヒドロキシ置換C3基である、請求項15〜19のいずれか1項に記載の使用。
- 幹細胞移植が適応とされている対象が、骨髄腫、多発性骨髄腫、リンパ腫、非ホジキンリンパ腫、白血病、急性骨髄性白血病、ホジキン病、急性リンパ芽球性白血病、骨髄異形成症候群、骨髄増殖性疾患、慢性骨髄性白血病、神経芽腫、再生不良性貧血、慢性顆粒球性白血病、神経芽腫、鎌状赤血球症、骨原性肉腫、ユーイング肉腫、線維形成性小円形細胞腫瘍、形質細胞新生物、アミロイドーシス、硬化性粘液水腫、及びそれらの併発より選択される疾患又は障害に罹患している、請求項15〜21のいずれか1項に記載の使用。
- 濃縮メルファラン組成物を水性希釈剤で希釈して、薬学的組成物を提供することを含む、請求項15〜22のいずれか1項に記載の使用。
- 濃縮メルファラン組成物がメルファラン50mg〜125mgを含む、請求項23に記載の使用。
- 薬学的組成物がアルコールを実質的に含まない、請求項15〜24のいずれか1項に記載の使用。
- 水性希釈剤が生理食塩液である、請求項23〜25のいずれか1項に記載の使用。
- 薬学的組成物中のメルファランが、希釈後10時間以内で25℃で4%又はそれ未満分解する、請求項23〜25のいずれか1項に記載の使用。
- 薬学的組成物を投与前に0.5時間〜12時間保存する、請求項23〜27のいずれか1項に記載の使用。
- 投与が、等価用量のメルファランを含有しかつシクロデキストリン誘導体を欠くメルファラン製剤によって提供されるメルファランCmaxよりも少なくとも20%又はそれ以上大きいメルファランCmaxを、幹細胞移植が適応とされている対象に提供する、請求項15〜28のいずれか1項に記載の使用。
- 投与が、等価用量のメルファランを含有しかつシクロデキストリン誘導体を欠くメルファラン製剤によって提供されるメルファランAUC0-tよりも少なくとも20%又はそれ以上大きいメルファランAUC0-tを、幹細胞移植が適応とされている対象に提供する、請求項15〜28のいずれか1項に記載の使用。
- 塩酸塩としてのメルファラン25mg〜125mg;
緩衝剤;及び
式I
R1、R2、R3、R4、R5、R6、R7、R8及びR9は、独立して、−H、直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基、又は置換されていてもよい直鎖又は分枝鎖C1−C6基であり;
ここで、R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つは、直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基である]
のシクロデキストリン誘導体;
を含んでなる薬学的組成物であって、

水溶液での薬学的組成物の希釈は、メルファランが希釈後5時間以内で25℃で2%又はそれ未満分解する希釈薬学的組成物を提供し;
シクロデキストリン誘導体は、メルファランに対して50:1〜55:1(w/w)の比で存在する;
上記薬学的組成物。 - R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つがヒドロキシ置換C3基である、請求項31に記載の薬学的組成物。
- 塩酸塩としてのメルファラン25mg〜125mg及び水溶性ポリマーを含む第1の容
器;及び
水性希釈剤、緩衝剤、及び式I
R1、R2、R3、R4、R5、R6、R7、R8及びR9は、独立して、−H、直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基、又は置換されていてもよい直鎖又は分枝鎖C1−C6基であり;
ここで、R1、R2、R3、R4、R5、R6、R7、R8及びR9の少なくとも1つは、直鎖又は分枝鎖C1−C8−(アルキレン)−SO3 -基である]
のシクロデキストリン誘導体を含んでなる第2の容器;
を含んでなる薬学的キットであって:
ここで、シクロデキストリン誘導体は、メルファランに対して50:1〜55:1(w/w)の濃度で第2の容器中に存在し;
第1の容器及び第2の容器を合わせることが、希釈後5時間以内で25℃で2%又はそれ未満分解する4〜6のpHを有する希釈薬学的組成物を提供する;
上記薬学的キット。 - 第1の容器が、ポビドンを10mg〜30mgの量で含んでなり、第2の容器が、第1の容器及び第2の容器を合わせると4〜6のpHを与えるに十分な濃度でpH調整剤を含んでなる、請求項34に記載の薬学的キット。
