本技術分野は広く眼科用装具に関連し、より具体的には、涙点プラグ(punctum plug)などの医療用小管挿入物に関する。
眼への薬物送達は従来、点眼剤の定期的投与、ペースト及び包袋、角膜へ適用される薬剤を含浸させたレンズ、直接注入又は眼に挿入される薬剤デポーによって達成される。例えば、白内障手術及び網膜硝子体手術の後には、数日間、数時間毎に抗生物質を投与しなければならない場合がある。加えて、非ステロイド性抗炎症薬(NSAID)などの他の薬剤を、頻繁に与える必要もある。
一般に、眼疾患又は状態を治療するための眼用薬剤は、眼表面状態、前部疾患、又は後部/眼の奥の疾患の治療に向けられる。
眼表面及び眼の前部に送達される大抵の薬剤は、点眼剤の形態で投与される。この形態の送達には幾つかの問題がある。まず、関節炎の可能性のある高齢患者にとって、液滴を眼に入れることは困難な場合がある。次に、液滴中の薬剤の95%以上が、結局眼に浸透するに至らず、無駄になると見積もられている。この浪費は、薬剤の非効率的な利用となるだけでなく、全身的な副作用につながるおそれもある(例えば、緑内障用のβ遮断薬は循環器系の問題を招き得る)。最後に、要求される治療レベルの達成を可能とするために、利用者は高濃度の薬剤を投与しなければならず、これは、灼熱感及び刺痛などの局所的問題、又は眼表面の違和感を引き起こし、服薬不順守並びに不快感を招く。しかしながら、現在まで液滴は、依然として眼用医薬品送達の主流となっている。
一実施態様は、水を吸収して加水分解的に生分解可能なヒドロゲルを形成する共有結合架橋性親水性ポリマーから形成された涙点プラグであって、これは、水に接触して吸収し、原位置で膨潤し、堅固で安定な配置のために小管を膨張させ、かつ小管に整合的に適合する。該ヒドロゲルは、眼への放出制御のための薬剤を含み、かつ高い含水量を有する。

幾つかの前駆体は、開始剤を使用して反応する。開始剤基は、連鎖成長による重合反応を開始することが可能な化学基(例えば、フリーラジカル)である。例えば、それは、別個の構成成分として、又は前駆体上の懸垂基(pendent group)として存在し得る。フリーラジカル開始剤基には、熱開始剤、光学活性化可能な開始剤(photoactivatable initiator)、及び酸化-還元(レドックス)系が含まれる。長波長のUV及び可視光に光学活性化可能な開始剤を挙げると、例えば、エチルエオシン基、2,2-ジメトキシ-2-フェニルアセトフェノン基、他のアセトフェノン誘導体、チオキサントン基、ベンゾフェノン基及びカンファーキノン基がある。熱反応性開始剤の例を挙げると、4,4'アゾビス(4-シアノペンタン酸)基、及び過酸化ベンゾイル基の類似体である。幾つかの市販されている低温ラジカル開始剤、例えば、Wako Chemicals USA社(リッチモンド、Va.)から入手可能なV-044などを使用し、体温でラジカル架橋反応を開始して、前述のモノマーのよるヒドロゲルを形成することができる。
前駆体は、患者の外部又は原位置のいずれかで、互いに反応して材料を形成する官能基を有する。一般に官能基は、重合若しくは求電子剤-求核剤反応で互いに反応するための重合可能な基を有するか、又は他の重合反応に関与するように構成される。重合反応の様々な態様が、本明細書中の前駆体の節に記載されている。
マイケル型反応に関与しない強求電子剤の例は、スクシンイミド、スクシンイミジルエステル又はNHS-エステルである。マイケル型求電子剤の例は、アクリラート、メタクリラート、メチルメタクリラート及び他の不飽和の重合可能な基である。
一般に前駆体は組み合わされて、共有結合架橋性ヒドロゲルを製造する。該ヒドロゲルは、適切な期間放出される治療薬又は作用薬を含み得る。ヒドロゲルは、事前に又は原位置で製造することができる。
原位置で製造される場合、一般に架橋反応は、生理的条件下、水性溶液中で起こる。好ましくは、該架橋反応は重合熱を発しないか、又は重合の開始若しくは誘発に外部のエネルギー供給源を必要としない。例えば、光化学的開始は一般に、眼への損害を回避するために避けられるべきものである。注入される材料の場合、その粘性は、該材料が小口径のカテーテル又は針を通って導入されるように制御され得る。ヒドロゲルが事前に製造される場合、ポリマーは、水性及び/又は有機性溶媒中で作製がなされる。
驚くべきことに、疎水性領域を有するヒドロゲルを十分な濃度でロードすることが、ヒドロゲルの膨潤性を低下させるのに効果的であることが見出された。図12はこの効果を示している。ヒドロゲルへのシクロスポリンAのロードの増加は、全体の膨潤性を低下させた。
薬剤を組み込んだヒドロゲルの製造方法は、有機溶媒又は水性溶液中でヒドロゲルを作製することを含み、該薬剤はヒドロゲルの形成時に存在しているか、又はその形成後にヒドロゲルに添加される。ヒドロゲルは、事前に製造され(事前形成デバイス(preformed device))、かつ少なくとも一部が脱水若しくは乾燥されていてもよく、又は形成場所の溶液中にて原位置で製造されてもよい。
幾つかの実施態様において、治療薬は、水性溶液の作製に先立って又は官能性ポリマーの無菌製造中に、前駆体と混合される。次いで、この混合物は前駆体と混合されて、生物学的に活性のある物質が捕捉された架橋性材料が生成される。
幾つかの実施態様において、治療薬又は作用薬は、前駆体が反応して架橋ポリマーヒドロゲルを生成する場合に、分離相に存在している。この相分離は、NHSエステルとアミン基との間の反応などの化学架橋反応における治療薬の関与を防止する。該分離相はまた、架橋材料又はゲルからの活性剤の放出動態を調節し、ここで「分離相」は、油(水中油エマルジョン)、生分解性ビヒクルなどであり得る。活性剤が存在し得る生分解性ビヒクルを挙げると、微小粒子、ミクロスフェア、マイクロビーズ、マイクロペレットなどのカプセル化ビヒクルがあり、ここで該活性剤は、生浸食性(bioerodable)又は生体吸収性(bioresorbable)ポリマー、例えば、ポリ(無水物)、ポリ(ヒドロキシ酸)、ポリ(ラクトン)、ポリ(トリメチレンカルボナート)、ポリ(グリコール酸)、ポリ(乳酸)、ポリ(グリコール酸)-コ-ポリ(グリコール酸)、ポリ(オルトカルボナート)、ポリ(カプロラクトン)のポリマー及びコポリマーなど、シクロデキストリンのような包接及び捕捉分子、モレキュラーシーブなどに捕捉されている。ポリ(ラクトン)及びポリ(ヒドロキシ酸)のポリマー及びコポリマーから作製されたミクロスフェアは、有用な生分解性カプセル化ビヒクルである。
一般にヒドロゲルは、ヒドロゲルがインビトロにおいて水分解性基の分解によって、過剰な水に溶解可能であることから測定可能であるように水分解性である。この試験は、インビボでの加水分解的駆動性(hydrolytically-driven)溶解の予測となり、進行は細胞又はプロテアーゼ駆動性分解とは対照的である。ヒドロゲルは、選択した薬剤、治療される疾患、必要な放出期間、及び選択した特定の薬剤の放出プロファイルに応じて、数日、数週間又は数ヶ月かけて吸収可能であるように選択することができる。
同様に、生分解性カプセル又は粒子を、ヒドロゲルマトリックス内部への包含のために製造することができる。該カプセル又は粒子は、ヒドロゲルマトリックスと同じであるか又は異なる分解時間を有することができる。
ヒドロゲルを涙点プラグ(涙点プラグ又は小管内プラグ)として使用することができる。涙点プラグのための様々な形状及びサイズが知られている。最も単純な形状は、その端部が平面又は円形な固体のロッドであり得る。該ロッドは、その外部形状が実質的に円筒形であり、それは隆起物を有さないが、真っすぐな側面を有する。あるいは、該ロッドは膨潤前に、ヘッド部及びシャフト部を有し得る。涙点プラグとして使用するために、乾燥又は脱水ヒドロゲルの長さは、例えば、約0.5mm〜約15mmとし得る(例えば、約2〜約4mmのように、明確に記載された範囲内の全ての範囲及び値が企図されることが、当業者には直ちに理解されるであろう)。一般に長方形のヒドロゲルについて、例えば、約0.1〜約1.0mmの幅が使用され得る。例えば、配置後に約2倍に膨潤する場合に、約0.3mmの直径がほとんどの患者に対して密接な適合を提供するであろう。本開示の解釈から明らかなことであるが、より大きな膨潤速度は、使用されるべきより小さな径のプラグを提供し、そのようなより小さなサイズが企図される。「ユニバーサルフィット」プラグとは、特定の涙点用にサイズ設定する必要がある全てのプラグを指す(ポリ(カプロラクトン-コ-ラクチド)又はシリコンプラグの場合のように)。
ヒドロゲルは、治療薬を含むことができる。該ヒドロゲルを有する涙点プラグを使用して、該治療薬を送達する。特定の眼状態の治療は、適当な用量の作用薬を眼に適当な期間に渡り送達することによって決まる。表2に状態及び治療の幾つかの実施態様を示す。
したがって、実施態様は、1以上の作用薬を組み込んだヒドロゲルを含む。作用薬は、例えば、ミクロスフェアを伴い又は伴わず、本明細書中の1以上の方法を使用して組み込むことができる。該ヒドロゲルを使用して、所定の時間に渡り有効量の作用薬を投与して、呈示された状態を治療するための医薬を製造することができる。
これらの作用薬は、ヒドロゲル中に分散される場合、好ましくは、0.05mg/mlを超える濃度で、及び好ましくは、少なくとも0.1〜約12mg/mlの濃度範囲で、及びより好ましくは、0.1〜4.0mg/mlの範囲で存在するが、最大で可視化剤の溶解度の限界までのより高い濃度が、可能性として使用され得る。これらの濃度範囲は、求電子-求核反応性前駆体の実施態様についての架橋時間(反応性前駆体種からゲル化までの時間によって測定される)を妨げることなく、ヒドロゲルに色を与えることができる。
追加的機械援用イメージング剤、例えば、蛍光化合物、X線画像装置でのイメージングのためのX線造影剤(例えば、ヨウ素化合物)、超音波造影剤又はMRI造影剤(例えば、ガドリニウム含有化合物)を使用することができる。
上記の薬物送達のための架橋組成物の使用において、架橋性ポリマー、架橋剤及びホストに導入される投与作用薬は、必然的に特定の薬剤及び治療すべき状態により決定されるであろう。投与は、シリンジ、カニューレ、外套針、カテーテルなどの任意の従来法によって行うことができる。
ヒドロゲル作製のためのキット又は系を製造することができる。該キットは医療として許容し得る条件を使用して製造され、かつ無菌性、純度、及び医薬として許容し得る調製法を有する前駆体を含む。該キットは、必要に応じてアプリケーター並びに取扱説明書を含み得る。治療薬は、予め混合したもの又は混合に利用できるものを含み得る。溶媒/溶液は、一式で若しくは別個に提供するか、又は構成成分が溶媒と予め混合されていてもよい。該キットは、混合及び/又は送達のためのシリンジ及び/又は針を含み得る。
幾つかの実施態様において、該キットは、少なくとも1の前駆体及びアプリケーターを有する。幾つかの実施態様において、生分解性ポリマー合成ヒドロゲルは、それぞれの腕の各末端にスクシンイミジルエステルを有する多腕型ポリエチレングリコール(PEG)と、トリリジン(第一級アミン求核剤を有する)とのリン酸及び他の緩衝溶液中での反応によって形成される。
原位置で涙点プラグを形成するための流動性の水性前駆体の使用は、細い針(例えば、30ゲージ)を介した投与を可能にする。またヒドロゲルは、酸性副生成物を最小化するように作製できるので、該プラグは、眼などの感受性組織に十分に許容される。
様々な実施例により、ヒドロゲル及び薬剤送達能を有するヒドロゲル涙点プラグの製造方法が説明される。概して実施例は、眼の涙液の局所的な送達を目標とした作用薬を含むポリエチレングリコールを含むように製造された浸食性又は非浸食性デバイスに関して向けられる。作用薬の例を挙げると、プロスタグランジン、抗炎症薬、免疫調節物質、抗ヒスタミン薬、NSAID、抗生物質、ステロイド薬及び麻酔薬がある。該デバイスは、事前形成されてよく、かつ乾燥した薬剤ロードデバイスとして提供されるか、又は眼に投与し、涙点の原位置で形成される液体製剤として提供されてもよい。明らかなことであるが、実施例は、限定することなく、より一般的な開示を示している。
実施例10は、図7にも見られる、高膨潤ヒドロゲルを示す。500%を超える容積変化が観察された。有機化学種又は水性化学種を使用して、様々に形成されたヒドロゲルが、実施例11〜12及び図7〜9のように、膨潤性ヒドロゲルを製造するのに有用であることが観察された。
実施例15及び16は、短期分解性ヒドロゲル及び相対的長期分解性ヒドロゲルの形成を例証している。長期材料(実施例15)は、グルタル酸スクシンイミジルで製造されるのに対し、相対的長期分解性材料(実施例16)は、アジピン酸スクシンイミジルで製造される。長期放出を示す実施例は、該長期材料を使用し(長期作用性インサートのために数週間に渡って薬剤を放出するラタノプロスト含有材料の実施など)、及び短期放出実験は、短期分解性材料を使用した(モキシフロキサシンが数日に渡って放出されるように)。
ミクロスフェアのサイズ範囲は、ヒドロゲルが粒子を含有する系において、放出動態の制御に影響を与えるように操作可能であることが発見された。図15A(実施例19A)は、プロットの比較及び小分子から大分子への放出傾向より明らかであるように、より小さな微小粒子がより迅速に薬剤を放出したことを示している図15Bは、15Aと同じデータのプロットの一部であり、より明確に該傾向を示している。ミクロスフェア中のPLGAの分子量及び分布は、別の変量であり、放出動態を制御するために操作した。より大きな分子量は、放出前に2〜3日の遅延期間を作り出したのに対し、より小さなものは、約1日目から直線的な放出を示した(図15C)。図15Dに示されるように、濃度及び分子量の両方の更なる変化によって、即効型であるが、持続性を可能とさせる製剤が作り出された。高ポリマー濃度は、放出期間を長期間に延長させるための、より密度の高いミクロスフェアを作り出すようである。
ミクロスフェア内容物のポリマー変化の更なる実験から、図16(実施例20)のように、他の薬剤(ラタノプロスト)の放出動態も操作できることが示された。図17A、17B及び17Cは、所望の放出速度の制御を得るために、実際にどのようにして競合する設計因子と配合するミクロスフェアサイズ及び組成物とのバランスを取ることができたかを詳述している。
別の薬剤(様々なステロイド)を用いた更なる試験は、更なる放出動態の制御を示した(図20)。
デバイスの成功又は失敗に影響を与えることが決定された別の因子は、ミクロスフェア密度である(図22)。簡潔に、より高密度の粒子は、より多くの薬剤を有することがわかり、デバイスの全体の性能における操作可能な因子の相互依存性を考慮して、より高密度な分子を作るために技術が開発された。
薬剤の有機溶媒への混和性に基づいた第1のプロトコルを、以下のように開発した。90mgの各腕の末端にグルタル酸スクシンイミジルを有する8腕の分子量15,000のポリエチレングリコール(8a15K PEG SG)を、6.7mgのトリリジンとともに、24.2mgの薬剤物質を含有する329.1mgのメタノール(MeOH)に溶解した。これは、MeOH溶液中20%濃度のポリマー、及び最終の乾燥涙点プラグにおける20% w/wの薬剤ロードと相関する。
該溶液を既知の直径を有するシラスティックチューブに吸入し、クリップで閉じ、架橋反応が完了するまで垂直を保つ。クリップを取り除き、ゲルが充填されたチューブを真空チャンバに配置する。ゲルを100mTorr(13.33Pa)で24時間乾燥させ、翌日取り出す。乾燥したプラグをシラスティックチューブから取り出し、最適な長さが2.0〜4.0mmの範囲のサイズに切断する。
この方法は、非水溶性薬剤のロード、又は有機相に耐性をもつカプセル化薬剤の組み込みに使用できる。
90mgの8a15K PEG SGを、6.7mgのトリリジンとともに、353.3mgのMeOHに溶解した。これは、MeOH溶液中20%濃度のポリマーと相関する。該溶液を既知の直径を有するシラスティックチューブに吸入し、クリップで閉じ、架橋反応が完了するまで垂直を保つ。クリップを取り除き、ゲルが充填されたチューブを真空チャンバに配置する。ゲルを100mTorr(13.33Pa)で24時間乾燥させ、翌日取り出す。乾燥したプラグをシラスティックチューブから取り出し、最適な長さが2.0〜4.0mmの範囲のサイズに切断する。
該事前形成デバイスを、架橋したITXヒドロゲルを膨潤させることが公知のMeOHなどの溶媒に加える。該MeOHは、溶解した高濃度の薬剤を含有する。プラグは有機薬剤溶液によって膨潤させられ、幾らかの薬剤をヒドロゲルマトリックス中に浸透させる。ゲルを取り出し、上記のように再び乾燥させるか、又はヘキサンなどの非溶媒中に置く。これはMeOHをゲルから追い出させ、薬剤をゲルマトリックス中に沈降させ、薬剤がロードされたプラグを与える。
この方法は、例えば、第一級アミンを有する薬剤などの架橋技術と不適合性の薬剤をロードするのに適している。これは薬剤ロードと架橋工程とを分離して、不適合性に関する問題を排除する。
90mgの8a15K PEG SGを、6.7mgのトリリジンとともに、24.2mgの薬剤懸濁液を含有する329.1mgの水に溶解した。これは、MeOH溶液中20%濃度のポリマー、及び最終の乾燥涙点プラグにおける20% w/wの薬剤ロードと相関する。該薬剤懸濁液は、例えば、不溶性薬剤粒子又はカプセル化製剤の懸濁液であり得る。

この方法は、例えば、他のポリマー系にカプセル化された薬剤をロードするのに適している。該水性ベースの製造は、幾つかの有機溶媒によって起こり得るカプセル化薬剤の抽出を回避するのに使用することができる。
8a15K PEG SGポリマーを、小分子量トリリジン架橋剤を含有する低pH(4.0)の希釈溶液に、20%濃度で溶解する。この溶液は、高pH(8.8)溶液との組み合わせによって活性化され、ゲル化機構が開始される。薬剤は、希釈溶液中に懸濁液として予めロードされる。該薬剤懸濁液は、例えば、不溶性薬剤粒子又はカプセル化製剤の懸濁液からなり得る。
該溶液を垂直小管に適用するか、又は27Gのカニューレを有する小型の1ccシリンジに吸入し、垂直小管に注入する。ゲルは原位置で形成される。高ポリマー濃度のために、形成物は大きく膨潤し、該小管に応じた適合を与える。
概してミクロスフェアは、薬剤物質としてラタノプロストを組み込み、溶媒蒸発によって製造される。ラタノプロストは、遊離酸形態のプロドラッグであり、プロスタノイド選択的FP受容体アゴニストである。ラタノプロストは、僅かな副作用を伴い緑内障患者の眼圧を低下させる。ラタノプロストは、比較的低い水性溶液への溶解度を有するが、典型的に、溶媒蒸発を使用するミクロスフェアの製造に有用な有機溶媒への易溶性を有する。
およそ250mgのPLGA(50:50のラクチド:グリコリド比、〜60kD分子量、Lakeshore Biomaterials社)を1.67mLの塩化メチレンに溶解した。この溶液を、およそ25mgのラタノプロストに加えて、均質になるまで混合した。次いで、該薬剤/ポリマー溶液を、およそ20秒間に渡って、25ゲージの針を介して、67mLの注射用水(WFI)中1.5%ポリビニルアルコール(31〜50kD、89%加水分解)を含有する150mLのビーカーに注入し、同時に1インチ撹拌子を使用して300rpmで撹拌した。該溶液を一晩撹拌し、およそ18時間、溶媒を蒸発させた。ミクロスフェアを真空下のメンブランフィルターで回収し、50mLのWFIで3回リンスした。次いで、洗浄したミクロスフェアを、およそ3mLの容積で20mLのシンチレーションに移し、このバイアルを凍結させ、その後、涙点プラグへの組み込みに先立ち、週末に渡って凍結乾燥した。ミクロスフェアの代表的な写真を、図6に示す。
この方法は実施例5と同様であり、加えて塩化メチレンが、1%濃度に達するまで連続相に添加される。この添加は、不連続相から連続相への溶媒の移動速度を低下させて、より密なスキン形成をもたらす。
およそ200mgのPLGA(50:50のラクチド:グリコリド比、〜60kD分子量、Lakeshore Biomaterials社)を、1mLの塩化メチレンに溶解した。この溶液を、およそ10mgのラタノプロストに加えて、20mLのシンチレーションバイアルにおいて均質になるまで混合した。次いで、該薬剤/ポリマー溶液の溶媒を、ドラフトでおよそ72時間蒸発させた。次いで、得られたフィルムを更に、真空下、周囲温度にて一晩乾燥させた。同様に、これらの方法は、溶媒としてアセトン及びクロロホルムのそれぞれを使用して、実施することができる。
実施例1又は実施例3の方法を使用して、プラグを製造した。分枝ポリエチレングリコールは、末端がグルタル酸スクシンイミジルの8腕の15,000MW(8a15KSG)であった。求核性前駆体は、トリリジンであった。ヒドロゲルを、メタノール(MeOH)又は水(WFI)中で架橋した。膨潤によるパーセント変化は、プラグの長さ又は幅などの規定寸法の、時間との相関関係による変化として評価される。ヒドロゲル形成時のヒドロゲル中の固体の量は、10%〜30%で変動した。図7に示されるように、平衡時の非抑制的な容積変化は、形成時の溶媒又は固体含有量に応じて、約500%〜約1200%の範囲であることが得られた。
実施例1又は実施例3の方法を使用して、プラグを製造した。分枝ポリエチレングリコールは、末端がグルタル酸スクシンイミジルの4腕の20,000MW(4a15KSG)であった。求核性前駆体は、トリリジンであった。ヒドロゲルを、メタノール(MeOH)又は水(WFI)中で架橋した。膨潤によるパーセント変化は、プラグの長さ又は幅などの規定寸法の、時間との相関関係による変化として評価される。ヒドロゲル形成時のヒドロゲル中の固体の量は、20%〜30%で変動した。図8に示されるように、平衡時の非抑制的な容積変化は、形成時の溶媒又は固体含有量に応じて、約500%〜約1200%の範囲であることが得られた。
実施例1又は実施例3の方法を使用して、プラグを製造した。分枝ポリエチレングリコールは、末端がグルタル酸スクシンイミジルの4腕の20,000MW(4a20KSG)であった。求核性前駆体は、トリリジンであった。ヒドロゲルを、メタノール(MeOH)又は水(WFI)中で架橋した。膨潤によるパーセント変化は、プラグの長さ又は幅などの規定寸法の、時間との相関関係による変化として評価される。ヒドロゲル形成時のヒドロゲル中の固体の量は、20%〜30%で変動した。図9に示されるように、平衡時の非抑制的な容積変化は、形成時の溶媒又は固体含有量に応じて、約1100%〜約1700%の範囲であることが得られた。
実施例3の方法を使用してヒドロゲルを製造した。該ヒドロゲルは、治療薬の非存在下の水中で製造した。グルタル酸スクシンイミジルの末端を有する8腕の15,000MWのポリエチレングリコールを、トリリジンと反応させた。該ヒドロゲルに、ラタノプロスト又は実施例5に従って製造したラタノプロスト含有ミクロスフェアをロードした。図10に示されるように、カプセル化されていないニートのラタノプロストは、約90%の送達に達するまで、実質的に0次放出プロファイルでヒドロゲルから放出された。300μmのミクロスフェアをロードしたヒドロゲルは、約6日目まで作用薬を放出せず、その後、経時的に放出され、その時間の大部分はほぼ0次放出であった。同様に、ジクロロメタンからのキャストしたフィルムは、類似であるが異なるプロファイルを有した。
実施例1の方法を使用してヒドロゲルを製造した。ヒドロゲルを製造するのに使用する有機媒体中に、シクロスポリンを予め溶解させ、カプセル化は用いなかった。本明細書中に報告される予想外の結果の1つは、薬剤ロードの増大が放出を遅らせることであり、これは薬物送達における期待に反するものである。図11に示されるように、総放出のパーセンテージとしての放出速度は意外にも、高ロードプラグよりも低ロードプラグの方が速かった。
432mgの4A20kSGを10mLシリンジに取り、1.2mLの1.7%モキシフロキサシン溶液(20mLのWFI中の400mgのモキシフロキサシン塩基を1NのHClでpH4.5に調整したもの)と混合した。余分な空気を取り除いた。第2のシリンジにおいて、2gのモキシフロキサシンミクロスフェアを、2.4mLの1.7%モキシフロキサシン溶液(20mLのWFI中の400mgのモキシフロキサシン塩基を1NのHClでpH4.5に調整したもの)と混合した。これら2つのシリンジを、シリンジ間で前後にゆっくりと材料を交換することによって合わせた。材料を1つの10mLシリンジに回収した。第3のシリンジにおいて、1.2mLのトリリジン溶液(31.2mgのトリリジンを3mLの二塩基性リン酸ナトリウムに溶解したもの)を加えた。これを先に混合したシリンジと交換し、次いで、既知の直径を有するシラスティックチューブに注入し、クリップで閉じ、架橋反応が完了するまで吊るした。クリップを取り除き、ゲル/シラスティックチューブを元の長さの2.5倍に伸ばす。これを30℃で48時間乾燥させる。貯蔵乾燥プラグを取り出し、長さ3.5〜4.5mmに切断する。この方法は、カプセル化した薬剤を他のポリマー系にロードするのに適している。
432mgの4A20kSAPを10mLシリンジに取り、1.2mLのWFI(一塩基性リン酸ナトリウムでpH4.5に調整したもの)と混合した。余分な空気を取り除いた。第2のシリンジにおいて、2gのモキシフロキサシンミクロスフェアを、2.4mLのWFI(pH4.5に調整)と混合した。これら2つのシリンジを、シリンジ間で前後にゆっくりと材料を交換することによって合わせた。材料を1つの10mLシリンジに回収した。第3のシリンジにおいて、1.2mLのトリリジン溶液(31.2mgのトリリジンを3mLの二塩基性リン酸ナトリウムに溶解したもの)を加えた。これを先に混合したシリンジと交換し、次いで、既知の直径を有するシラスティックチューブに注入し、クリップで閉じ、架橋反応が完了するまで吊るした。クリップを取り除き、ゲル/シラスティックチューブを元の長さの2.5倍に伸ばす。これを30℃で48時間乾燥させる。貯蔵乾燥プラグを取り出し、長さ3.5〜4.5mmに切断する。
ミクロスフェアは、薬剤物質としてラタノプロスト又はモキシフロキサシンを組み込み、溶媒蒸発によって製造される。ラタノプロストは、遊離酸形態のプロドラッグであり、プロスタノイド選択的FP受容体アゴニストである。ラタノプロストは、僅かな副作用を伴い緑内障患者の眼圧を低下させる。ラタノプロストは、比較的低い水性溶液への溶解度を有するが、典型的に、溶媒蒸発を使用するミクロスフェアの製造に用いられる有機溶媒への易溶性を有する。モキシフロキサシンは、VIGAMOXの有効成分であり、眼の細菌感染症の治療又は予防の使用に認可されたフルオロキノロンである。モキシフロキサシンは、容易に水性溶液に溶解する。このように、疎水性及び親水性の両方の作用薬を、ミクロスフェア形成で例証した。
およそ2.8gのモキシフロキサシ塩基を、8mLの塩化メチレンに溶解した。これに5.18gの2A 50:50 PLGAを加え、溶解するまで混合した。次いで、直ちに薬剤/ポリマー溶液を、1Lの0.5%ポリビニルアルコール、0.2%二塩基性リン酸ナトリウム、2.5%塩化ナトリウムの溶液に注入し、同時に880rpmで撹拌した。該溶液を一晩撹拌し、およそ60分間40℃で溶媒を蒸発させた。ミクロスフェアを、8LのWFIの500mLの分割量で洗浄し、次いで、複数の20mLのシンチレーションバイアルに、およそ3mLの容量で移した。これらのバイアルを凍結させ、その後、ヒドロゲル又は他の使用への組み込みに先立ち、48時間に渡って凍結乾燥して、ミクロスフェアを乾燥させた。次いで、乾燥させたミクロスフェアを、構成成分(ロード用ミクロスフェア/マクロマー/トリリジン/緩衝液)をシリンジ中で混合すること、及びヒドロゲルの架橋のためにシリコンチューブに注入し、同時にミクロスフェアが底に沈殿するのを防止するために室温で回転させることによって、涙点プラグに組み込んだ。次いで、該チューブを所望の長さに伸ばし、オーブン中、30℃で1〜2日間乾燥させる。乾燥した材料をチューブから取り出し、特定の長さのプラグに切断する。
およそ250mgのPLGA又はPLAを、1.67mLの塩化メチレンに溶解した。この溶液を、およそ25mgのラタノプロストに加えて、均質になるまで混合した。次いで、該薬剤/ポリマー溶液を、およそ20秒間に渡って、25ゲージの針を介して、67mLの注射用水(WFI)中1.5%ポリビニルアルコール(31〜50kD、89%加水分解)を含有する150mLのビーカーに注入し、同時に1インチ撹拌子を使用して300rpmで撹拌した。該溶液を一晩撹拌し、およそ18時間溶媒を蒸発させた。ミクロスフェアを真空下のメンブランフィルターで回収し、50mLの水で3回リンスした。次いで、洗浄したミクロスフェアを、およそ3mLの容量で20mLのシンチレーション管に移し、このバイアルを凍結させ、その後、ヒドロゲル又は他の使用への組み込みに先立ち、約48時間に渡って凍結乾燥して、ミクロスフェアを乾燥させた。以下のPLGA及びPLA系を、ミクロスフェアの生成に利用した:1. 50:50 PLGA 2.5A;2. 50:50 PLGA 4A;3. 75:25 PLGA 4A;4. 100 PLA 4A;5. 100 PLA 2.5E;6. 100 PLA 4.5E;7. 100 PLA 7E。これらの略語は、当業者に公知で、Lakeshore Biomaterials社によって公開される術語に従い、Aとはカルボン酸末端基を意味し、Eはエステル末端基を意味する。2.5及び4の数字は、分子量に対するIVナンバー(IV number)(特定の濃度のクロロホルムにおける溶液の固有粘度)を指す。比率は、L:G比(ラクチド:グリコリド)である。この術語は、製造業者であるLakeshore Biomaterials社(Surmodics社の事業部)に由来する。
実施例17Aと同様であり、加えて塩化メチレンが、1%濃度に達するまで連続相に添加される。これは、不連続相から連続相への溶媒の移動速度を低下させて、より密なスキン形成をもたらす。
(実施例18A)
およそ250mgのPLGA(50:50のラクチド:グリコリド比、〜60kD分子量、Lakeshore Biomaterials社)を1mLの塩化メチレンに溶解した。この溶液を、およそ10mgのラタノプロストに加えて、20mLのシンチレーションバイアルにおいて均質になるまで混合した。次いで、該薬剤/ポリマー溶液の溶媒を、ドラフトでおよそ72時間蒸発させた。次いで、得られたフィルムを更に、真空下、周囲温度にて一晩乾燥させた。

この実施例は、治療薬としてモキシフロキサシンを使用して実施した。実施例17Aのようなプロトタイプのモキシフロキサシン涙点プラグについてのインビトロでの放出プロファイルを、リン酸緩衝食塩水(PBS)中37℃で評価した。この放出を図14に示す。上記プロトタイプからの放出は、カプセル化についてLakeshore社2A 50:50 PL:GAポリマーを利用した。製造及び放出速度動態を図15Aに示す。図15Bは、放出動態が粒径の範囲を変化させることによって、どのように操作されるかを示している。図15Bに示されるように、より広い粒子範囲のCB-ITX-152-2は、より狭い粒径範囲の製剤CB-ITX-152-10と比べて、長期のヒドロゲルプラグからの薬剤放出をもたらす。
実施例19Aへの修正は、製造中使用したPLGAの平均分子量及び分布の変更による放出期間及びプロファイルの変化を含む。図15Cに示されるように、製剤CB-ITX-124-1において使用したより高い分子量のPLGAは、徐放が生じ得る前に顕著な遅延期間を示すのに対し、製剤CB-ITX-114-11において使用したより低い分子量のPLGAは、1日目から直線的な放出を示す。遅延期間は、PLGA又はPLAのミクロスフェア製剤で観察されたが、これらは、ポリマーの分子量、並びにミクロスフェアの粒径範囲及び分布を調整することによって対処することができる。
実施例19A及び19Bへの修正は、製造反応において使用したPLGAの平均分子量及び分布、並びにポリマー濃度の変更による放出期間及びプロファイルの変化を含む。図15Dに示されるように、濃度及び分子量の両方の変更によって、長期間の即時放出を可能として、製剤を作り出すことができる。より高いポリマー濃度は、放出期間を長期間に延長させ得るより高密度のミクロスフェアを作り出すようである。
遅延期間の排除は、特に抗生物質のような即時作用治療薬にとって有用である。図15Dで観察されたインビトロでの遅延期間は、図15Eのインビボでの遅延期間に直接転化した。低分子量製剤を使用した再製剤、CB-ITX-152-2製剤は、高分子量製剤、B-ITX-124-1と比較して、前臨床モデルにおいて即時放出を示した。
ラタノプロスト涙点プラグを製造するのに使用した個々のミクロスフェアタイプについてのインビトロでの放出プロファイルは、使用する分子量、組成(LとGとの比率)及び末端基(酸対エステル末端)に従い変化するであろう。個々の放出速度の例を、図16に示す。
同じ作用薬を含有する複数種のポリマーの配合の効果の例を、図17に示す。同じ活性剤を含む複数種のポリマーの配合の効果の例を、図17Aに示す。所望のミクロスフェアを異なる量で配合することによって、放出プロファイルを適用の要求に合わせることができる。より小さい粒径は、図17Bに示されるように、より大きな粒径と比べてより短い放出期間を生じさせた(速い初期放出)。図17Cは、配合がもたらす他の効果を示している。
ヒドロゲルと微小粒子との放出系の薬物動態性能を、モデルとしてモキシフロキサシンを使用して評価した。モキシフロキサシン涙点プラグを、インビボでのモキシフロキサシン放出の相関を決定するために、イヌの眼モデルにおいて評価した。2A 50:50 PLGA溶媒蒸発ミクロスフェアを含有する9% 4A20kSGのプロトタイププラグを、イヌの下涙点に埋め込み、涙試料を16日に渡ってLC-MS/MSで分析した。薬物動態データを図18に示す。薬物動態プロファイルは、ヒドロゲル涙点プラグが、インビボで所定の10日間の送達時間の間に、一定の速度で治療量をイヌの眼に送達できることを示している。
細菌学的データを含む広範な手術後の眼内炎実験から、手術後の眼内炎に関与する確認された成長分離株の94.2%が、グラム陽性病原体であり、最も一般的には、黄色ブドウ球菌(Staphylococcus aureus)及び表皮ブドウ球菌(Staphylococcus epidermidis)であることが示された(Ηan DPらの文献,「眼内炎硝子体切除術研究における微生物学的単離株のスペクトル及び感受性(Spectrum and susceptibilities of microbiological isolates in the Endophthalmitis Victrectomy Study)」, Am J Ophthalmol, 1996; 122:1-17)。図18のプロファイルは、涙液レベルが、既報の10日間のモキシフロキサシンMIC90、黄色ブドウ球菌(60ng/mL)及び表皮ブドウ球菌(130ng/mL)(Hariprasad, S.らの文献,「ヒト水様液及び硝子体液における0.5%モキシフロキサシン点眼剤の局所的投与の浸透薬物動態(Penetration Pharmacokinetics of Topically Administered 0.5% Moxifloxacin Ophthalmic Solution in Human Aqueous and Vitreous)」, Arch Ophthalmology, Jan 2005;123: 39-44)を上回ることを示している。
ヒドロゲルと微小粒子との放出系の薬物動態性能を、モデルとしてモキシフロキサシンを使用して評価した。モキシフロキサシン涙点プラグの性能を、インビボでのモキシフロキサシン放出の相関を決定するために、イヌの眼モデルにおいて評価した。PLGA/PLA溶媒蒸発ミクロスフェアの配合物を含有する20% 4A20kSAPのプロトタイププラグを、イヌの下涙点に埋め込み、涙試料を16日に渡ってLC-MS/MSで分析した。薬物動態データを図19に示す。
9% 4a20KSGヒドロゲルを、3つの異なるステロイド(フルニソリド、ブデソニド及びトリアムシノロンアセトニド)を組み込んで製造した。該ヒドロゲルにおけるステロイドの放出プロファイルとPBS中の遊離な薬剤との比較を、図20に示す。ヒドロゲル中へステロイドの閉じ込め又は捕捉は、より遅く、より持続性のある薬剤放出プロファイルをもたらすことが認められる。
特定の実施態様は、ヒドロゲルを伸張することに関する。粒径は、管のサイズとの相関関係で、乾燥中に伸張可能な長さに影響を与えることが発見された。より小さな直径の管での涙点プラグの製造及び比較的大きな粒子範囲の組み込みは、乾燥工程の間にいっそう粉砕しやすく、使用に適さないヒドロゲルプラグとなる。表4に示されるように、粒径範囲及び特定の管の直径に対する伸張係数を制御することによって、結果、ヒドロゲル/ミクロスフェアロッドは、後続の例えば、涙点プラグへの切断のための乾燥に成功し得る。より大きな粒径のミクロスフェアは、うまく伸張されるヒドロゲルの能力を低下させる。
伸張、乾燥、薬剤ロード、膨潤、分解、放出速度、合計容積、及び薬剤用量は、競合している設定パラメータを提示する。最終プラグ長及び3.5mm長のプラグあたりの推定薬剤(モデルとしてモキシフロキサシンを使用)用量に対して、総プラグ容積あたりのミクロスフェアロード、乾燥間の伸張係数、及びシリコンチューブ内径を変化させて、実施可能性実験を行った。表5に記載された結果は、1.47mm IDのチューブで製造した場合に、2.5×の伸張係数を用いて、0.50mmの最終乾燥直径で200μgを上回る標的プラグ用量が達成され得ることを示した。
実施例1への修正は、伸張乾燥は、涙点プラグ又は他の適用に最も適したサイズを標的とするための異なった乾燥直径であるが、図21Aに示されるように、類似の水和した直径を有するプラグをもたらし得ることの理解を含む。たとえそれら全てが、図21Bに示されるように、類似の乾燥時の長さを有したとしても、より短い水和した長さは、より大きな伸長係数と相関がある。したがって、長い乾燥時の長さを有するが、涙点における水和時にプラグが小管に後退するように、涙点プラグの使用に適しているであろう短い水和時の長さを有するプラグを作り出すことが可能である。
図22に示されるように、放出プロファイルを変化させるためにポリマー重量の配合を行うことが可能である。これは、広範囲のPLGA分子量をもたらし、その結果、放出プロファイルを修正する。より大きな割合の低分子量のPLGAを製剤に組み込むことによって、より速い放出又は親水性薬剤のためのより高い薬剤カプセル化効果を得ることが可能である。反対に、より大きな分子量は、より長い放出期間を与える。
高密度のミクロスフェアは、より多くの作用薬を含有することが見出された。PVA/連続相において塩化ナトリウムなしで製造されたミクロスフェアは、0.4g/mLのタップ密度を有するのに対し、塩化ナトリウムを伴うものは、1g/mLの高いバルクのタップ密度を有した。密度の調整は、様々な因子により制御されるが、幾つかの因子は、薬剤カプセル化効率を顕著に増大させ、高密度ミクロスフェアをもたらすことが観察された。高PLGA濃度と塩化ナトリウムを含有する連続相の大きな容積(有機相の100倍)との組み合わせは、高いカプセル化効率(>70%)を有する高密度ミクロスフェアを与えた。高いPLGA濃度及び連続相の大きな容積は、ミクロスフェア表面でPLGAの迅速な沈降をもたらし、擬似半透膜を形成する。連続相における塩の添加は、浸透圧を増大させ、これは、連続/水相の分散/有機相への内向き流束を防止し、ミクロスフェア表面でのチャネルの形成を低下させる。加えて、塩化ナトリウムは、連続相の極性を増大させ、それにより、塩化メチレンの極性連続相への溶解度を低下させ、これにより、ポリマーをよりゆっくりを沈降させ、比較的高密度の微小粒子を形成する。

- ヒトの眼の涙点又は小管を通る涙流を遮断し、又は減少させ、かつ薬剤を眼に送達するための医療用装具であって:
涙点を通過する寸法を有する脱水共有結合架橋合成親水性ポリマーヒドロゲルを含み、該脱水ヒドロゲルが生理的水を吸収して、少なくとも1mmの断面幅まで膨潤し、かつ小管に整合的に適合し、該ヒドロゲルが、眼への放出のためにヒドロゲル中に分散した治療薬を含み、該ヒドロゲルが、インビトロにおいて生理食塩水中で十分に水和される場合に、少なくとも50重量%又は容積%の含水量を有し、ここで、該ヒドロゲルが、追加的なカバー、スリーブ、シース、オーバーコート又はリザーバ部を伴うことなく製造され、かつ使用され、かつ多糖を含まず、

少なくとも3の第1の官能基を含む第1の合成水溶性ポリマーと、少なくとも3の第2の官能基を含む第2の合成水溶性ポリマーを、薬剤の存在下において反応させる工程であって、前記第1及び第2の官能基が互いに反応して共有結合を形成し、それにより多糖、追加のカバー、スリーブ、シース、オーバーコート又はリザーバ部を含まない合成架橋ヒドロゲルを形成し、前記薬剤が該ヒドロゲル中に分散される、前記工程、
管からヒドロゲルを押し出す工程、
該ヒドロゲルの長さを少なくとも2倍に伸張する工程であって、生じるヒドロゲルの最大断面幅が1mm未満である、前記工程、
該ヒドロゲルを乾燥させる工程、
該乾燥したヒドロゲルを不特定の長さに切断又は分割する工程、
を含み、該脱水ヒドロゲルは、少なくとも1mmの断面幅まで膨潤して小管に整合的に適合するように生理的水を吸収し、かつ該ヒドロゲルは、インビトロにおいて生理食塩水で十分に水和される場合に、少なくとも50重量%又は容積%の含水量を有し、
前記ヒドロゲルが、小管で水和される場合に、眼状態を治療するのに有効な薬剤の量を、眼の涙膜中の薬剤の濃度から測定可能なように、少なくとも7日間の期間に渡って放出する、前記製造方法。 - 前記脱水ヒドロゲルが、小管への配置の10分以内に、少なくとも幅1mmまで膨張する、請求項14記載の製造方法。
- 前記ヒドロゲルに、機械援用を伴わず、ヒトへの前記装具の可視性を提供するのに有効な濃度で可視化剤を配置させることを更に含む、請求項14記載の製造方法。
- 前記ヒドロゲルが、水中での化学加水分解によって自発的に分解する、請求項14記載の製造方法。
- 前記薬剤が、ラタノプロスト又はモキシフロキサシンである、請求項14記載の製造方法。
- 薬剤及び脱水合成ヒドロゲルを含む、眼の眼状態を治療するための医薬組成物であって、
該ヒドロゲルが眼の小管に配置され、それによって、該脱水ヒドロゲルが生理的水を吸収して、少なくとも1mmの断面幅まで膨潤し、かつ小管に整合的に適合して、小管を通る涙流を遮断又は減少させるように用いられることを特徴とし、ここで、該ヒドロゲルが、追加的なカバー、スリーブ、シース、オーバーコート又はリザーバ部を伴うことなく製造され、かつ使用され、該ヒドロゲルは、多糖を含まず、かつ眼への放出のためにヒドロゲル中に分散された治療薬を含み、かつ該ヒドロゲルは、インビトロにおいて生理食塩水で十分に水和される場合に、少なくとも50重量%又は容量%の含水量を有し、
前記ヒドロゲルが、小管で水和される場合に、眼状態を治療するのに有効な薬剤の量を、眼の涙膜中の薬剤の濃度から測定可能なように、少なくとも7日間の期間に渡って放出する、前記医薬組成物。 - 機械援用を伴わず、患者へ可視化剤の可視性を提供するのに有効な濃度で可視化剤を前記ヒドロゲル中に更に含む、請求項19記載の医薬組成物。
- 前記ヒドロゲルが、水中での化学加水分解によって自発的に分解する、請求項19記載の医薬組成物。
- 前記ヒドロゲルが、フリーラジカル開始によって重合される複数の重合可能な基を含む第1の合成ポリマーの反応生成物であるか、又は
前記ヒドロゲルが、少なくとも3の第1の官能基を含む第1の合成水溶性ポリマーと、少なくとも3の第2の官能基を含む第2の合成水溶性ポリマーとの反応生成物であり、該第1及び第2の官能基が互いに反応して共有結合を形成し、それによって合成架橋ヒドロゲルとしてヒドロゲルを形成するか、又は
前記ヒドロゲルが、ポリエチレングリコールを含む第1の合成ポリマーの反応生成物であり、第1の官能基がスクシンイミドであり、かつ第2の官能基がアミン及びチオールからなる群から選択される、請求項19記載の医薬組成物。 - 前記薬剤がラタノプロストであり、該ラタノプロストがインビボで、小管に配置されたヒドロゲルから、開放隅角緑内障又は高眼圧症に罹患している患者において眼圧亢進を低下させるのに有効な量で、少なくとも4週間の期間に渡って放出されるように用いられることを特徴とする、請求項19記載の医薬組成物。
- 前記薬剤がモキシフロキサシンであり、該モキシフロキサシンがインビボで、小管に配置されたヒドロゲルから眼の涙膜に、眼における黄色ブドウ球菌又は表皮ブドウ球菌を実質的に減少させるのに有効な量で、少なくとも7日間の期間に渡って放出されるように用いられることを特徴とする、請求項19記載の医薬組成物。
- 前記モキシフロキサシンが、塩基形態である、請求項24記載の医薬組成物。
