Description

バイオフィルムは、通常、少量の水溶液中に沈められた又は曝露された固体基質(solid substrates)上に見出されるが、特に湿度の高い環境では、液体表面上に、そしてまた葉の表面上に移動性マット(floating mats)を形成することができる。増殖にとって十分な物資が与えられると、バイオフィルムは、急速に成長して肉眼で見えるようになる。バイオフィルムは、多くの異なるタイプの微生物、例えば細菌、古細菌、原生動物、真菌及び藻類を含むことができ;各群は、分化した代謝機能を果たしている。しかしながら、一部の生物は、ある種の条件下で単種フイルム(monospecies films)を形成する。
バイオフィルムは、消毒薬及び抗生物質、食細胞並びにヒト免疫系に対してそれ自体で防御することができると考えられる。
バイオフィルムは、EPSと称する分泌されたポリマー化合物のマトリックスと共に保持され、そして保護されている。EPSは、細胞外ポリマー物質又はエキソポリサッカライドの各々についての略語である。このマトリックスは、その中で細胞を保護し、そして生化学的シグナルを通してそれらの間でコミュニケーションを促進する。いくつかのバイオフィルムは、栄養分及びシグナル伝達分子の分配を助ける水チャネルを含むことがわかっている。このマトリックスは、ある種の条件下でバイオフィルムが固着化(fossilized)できるほど十分に強い。

・バイオフィルムは、管の内部で成熟して目詰まり及び腐食に至ることがある。床及び流し台上のバイオフィルムは、調理区域で衛生を困難にすることがある。冷却水系中のバイオフィルムは、熱伝達を低下させることが知られている(非特許文献2参照)。
・船体への細菌付着は、航洋船の生物付着の土台として作用する。細菌フィルムが形成されたら、フジツボのような他の海洋生物が付着することがより容易になる。このような付着物は、船の速度を最大20%抑制することがあり、航海がより長期化し、そしてさらなる燃料が必要になる。乾ドックにおける改装及び再塗装の時間により資産輸送の生産性が低下し、そしてまた、腐食及び船体からの海洋生物の機械的除去(削り落とす)のため船の耐用年数が減る。
・また、バイオフィルムは、構造的用途にも利用することができる。例えば、多くの下水処理場は、処理段階を含んでおり、その際、排水は、有機化合物を抽出して分解するフィルター上に成長したバイオフィルムの上を通過する。このようなバイオフィルムでは、細菌は、主に有機物質(BOD)を除去する役割を果たしているのに対して;原生動物及びワムシは、主に病原体及び他の微生物を含む浮遊物質(SS)を除去する役割を果たしている。緩速濾過器は、飲用目的で湖、泉又は川の水源から地表水を濾過するのと同じやり方でバイオフィルム成熟に依存している。精製水からはさらなる栄養を全く抽出することができないため、本発明者らがきれいな水とみなすものは、これらの微小細胞生物にとって無益な物質である。

歯垢は、歯に付着した細菌細胞(主にミュータンス連鎖球菌(Streptococcus mutans)及びサングイス連鎖球菌(Streptococcus sanguis))、唾液ポリマー及び細菌性細胞外生成物からなる物質である。歯垢は、歯の表面上のバイオフィルムである。微生物のこの蓄積により歯及び歯肉組織が高濃度の細菌代謝物にさらされ、その結果、歯科疾患が生じる。

化合物10の固有抗菌活性−最小阻止濃度(MIC)及び最小殺菌濃度(MBC)の測定
特異的な微生物に対する抗菌剤の最小阻止濃度(MIC)は、明確に目視できる生物の増殖が観察されない抗菌剤の最小濃度として定義される(最小阻止濃度のFDA定義)。MIC(MIC's)は、典型的に、従来から連続2倍希釈から得られた濃度を用いて測定した(米国臨床検査標準委員会(NCCLS)ハンドブックM7−A5:「好気的に増殖する細菌についての希釈抗菌物質感受性試験方法;認可基準−第5版」第20巻 第2号 2000年1月)(National Committee for Clinical Laboratory Standards (NCCLS) Handbook M7-A5: “Methods for Dilution Antimicrobial Susceptibility Tests for Bacteria that Grow Aerobically; Approved Standard - 5th Edition” Volume 20 Number 2. January 2000)。光がない場合の化合物10についてのMICは、NCCLS(米国臨床検査標準委員会(NCCLS)ハンドブックM7−A5、前出)によって作成されたMICプロトコールに基づくプロトコールを用いて調査した。
最小殺菌濃度(MBC)は、所定の設定条件下で一定期間(一般に24時間)インキュベーション後、生存可能な生物のほとんど(99.9%)を死滅させるために必要な薬物の最小限の濃度として定義される(米国臨床検査標準委員会(NCCLS)ハンドブックM26−A;「抗菌剤の殺菌活性の測定方法;認可されたガイドライン」第19巻 第18号、1999年9月("Methods for determining Bactericidal Activity of Antimicrobial Agents; Approved Guidelines" Volume 19 number 18, September 1999))。
黄色ブドウ球菌BAA−44、ATCCカタログから入手した多剤耐性メチシリン耐性黄色ブドウ球菌(MRSA)株を、本研究に用いた。以下の濃度の化合物10を調べた:0.764;0.382;0.191;0.0955;0.0478;0.0239、0.0119、0.00597、0.00298、0.00149、0.00075及び0.00037g/mL。蒸留水で保存液を調製し、そしてこれを系列希釈して使用直前に必要な濃度にした。
結果は、光がない場合、化合物10が低いMIC及びMBC値を有することを示している。これらのデータは、化合物10が、抗生物質として、いくつかの従来の抗生物質よりかなり強力であることを示している(表2参照):

4.次いで、培養物を新たなブイヨンで104生物/mlの最終濃度に希釈し、そして37℃で90分間振盪機にかけた。

結果を表3に示す。
結果は、化合物10がグラム陽性細菌株の範囲についてきわめて低いMIC及びMBC値を有することを示している。MIC及びMBC値は、方法の限度内ではほとんど同じであり、静菌剤とは対照的に抗菌活性の様式が殺菌性であることを示唆している。
方法
セルシステムズ・バイオテクノロジー社、独国(CellSystems Biotechnologie GmbH, Germany)から購入した正常ヒト表皮角化細胞(NHEK)及び正常ヒト皮膚線維芽細胞(NHDF)を用いて培養したヒト皮膚細胞に対する毒性について試験化合物をスクリーニングした。
NHEK及びNHDF細胞は、継代3から10の間で用いた。細胞を7.5及び/又は15×104の細胞/ウェル(マイクロタイタープレート)で播種し、そしてインキュベーター(37℃,5%CO2)中で一夜付着させた。異なる濃度の選ばれた光増感剤と共に種々の時間インキュベートした後、細胞を暗所で24時間インキュベートした。
標準のMTT−アッセイによって毒性を試験した(Mossman et al., 1983 J. Immunological Methods 65: 55-63)。MTTは、代謝活性細胞のインジケーターである。ミトコンドリア中の酵素活性に依存して呈色反応を視覚化することができ、それはELISAリーダー(540nm)で測定することができる。細胞生存性を1つに正規化し、これは、試験化合物がない場合のインキュベーション後の細胞のOD値を1に正規化したことを意味する。各実験を3回繰り返した。
角化細胞及び線維芽細胞における毒性試験の結果を図2及び3に示す。データは、化合物10が、抗菌効果を有することが知られている用量で正常なヒト表皮角化細胞又は正常なヒト皮膚の線維芽細胞に対して固有の毒性を示さないことを示している。
化合物8、10及び12との大腸菌との結合
大腸菌細胞を、種々の濃度(1〜7.5μM)の化合物8、10又は12と共に5分間インキュベートした。培養期間の終了後、細胞を遠心分離によって沈殿させて結合していない試験化合物の画分を除去し、そして細胞ペレットを2%SDS2mlに再懸濁して細胞溶解物を得た。SDSで一夜インキュベーション後、細胞に結合した試験化合物の量を細胞溶解物の蛍光分光分析によって評価した。細胞溶解物中の化合物の濃度は、発光蛍光スペクトルの最大で強度を測定し、較正プロット上でデータを補間することによって算出した。細胞に結合した試験化合物の量は、細胞タンパク質1mg当たり化合物のナノモルとして示した。タンパク質濃度はローリー法(Lowry et al., 1951, J. Biol. Chem. 193:265-275)によって決定した。
すべての実験は三つ組で行い、そして結果は標準偏差と共に3つの測定の平均値を表す。
細胞から回収されたポルフィリンの量を表4に示す。
化学安定性
本発明の典型的な化合物を分析するため、以下のHPLC方法を確立した。方法には、これらの化合物にとってきわめて特異的な波長420nmのUVによる検出が含まれる。ポルフィリン構造と関係のない(そのため420nmで吸収がない)不純物をモニターするため、幾つかの実験ではDAD(ダイオードアレイ検出器)によって200nmから700nmの間で全クロマトグラムのUVスペクトルも記録した。

化合物10及び12は、いずれも良好な安定性を示すことがわかり、そして試験プロトコールのストレス条件下でも非常に安定であった。化合物8は化合物10及び12よりも安定でなかったが、示された安定性は、実際の使用には十分であることがわかった。
種々の水性基剤製剤においてポリエチレンバイアルで8週間にわたり暗所中40℃で保存した3つの典型的な化合物(化合物8、10及び12)及び1つの参照化合物(化合物1)の安定性を、以下のように評価した:
・ラウレス硫酸ナトリウム(SLES)+水
・9:1の水:エタノール
・SLES+9:1の水:エタノール
結果から、試験したすべての化合物は、8週間の期間にわたって良好な安定性を示したことがわかった(図6参照)。
化合物8及び10については、安定性試験を17週まで延長した(図7参照)。
標準の急性皮膚毒性試験において化合物10を局所製剤中3.2mMで試験して化合物についてもしあるとすれば臨床的又は組織学的毒性が検出されうるどうかを測定した。
急性毒性プロトコールは、化学物質の試験のためのOECDガイドライン/第4節 − 健康影響試験第402号:急性皮膚毒性(OECD Guidelines for the testing of chemicals /Section 4 - Health Effects Test Number 402: Acute Dermal Toxicity)に基づく。
臨床的な肉眼、及び顕微鏡観察の後、臨床毒性は観察されなかった。いずれの主要な器官(皮膚を含む)について組織学的毒性は、観察されなかった。
結論として、化合物10は、いずれの急性毒性影響をもたらさない:実際、物質又はそのビヒクル適用に関連する有意な臨床的又は病理学的徴候は、観察されなかった。
要約
背景:バイオフィルム形成の重要な特徴は、抗生物質の活性に抵抗する細菌の能力である。バイオフィルム中の細菌のより緩慢な増殖速度は、増強された耐性における重要な因子でありうる。本発明者らは、黄色ブドウ球菌のバイオフィルム及び緩慢増殖培養物に対する、抗菌剤のまったく新しい種類のリード化合物である化合物10の活性を調べた。
方法:英国抗菌化学療法学会(British Society for Antimicrobial Chemotherapy)(BSAC)ガイドラインに従った微量液体希釈法(broth microdilution)によって浮遊培養物(planktonic cultere)についてMICを測定した。バイオフィルムMIC(bMIC)及び最小バイオフィルム形成阻止濃度(minimum biofilm eradication concentration)(MBEC)は、カルガリー(Calgary)バイオフィルムデバイスを用いて測定した。寒冷培養細胞(cold culture cells)の生存能力に対する化合物10の効果は、黄色ブドウ球菌SH1000を37℃で初期指数期(early exponential phase)まで増殖させ、そして予め冷却した培地に細胞を再懸濁し、ここで、化合物10及び対照薬剤の存在下及び非存在下でそれを維持することによって測定した。また、緊縮応答を発現する緩慢増殖細胞に対する化合物10の効果は、初期対数期(early log phase)まで培養物を増殖させ、そしてイソロイシルtRNAシンテターゼ阻害剤ムピロシンの添加により緊縮応答を誘発することによって測定した。次いで、化合物10及び対照薬剤を加え、そして生存細胞測定のためサンプルを回収した。
結果:化合物10は、シプロフロキサシン、フシジン酸、テトラサイクリン及びリファンピンについてのそれぞれ4、0.5、0.5及び0.03μg/mLのbMIC及び>256、>256、>256及び128μg/mLのMBECと比較して黄色ブドウ球菌SH1000に対する1μg/mLのbMIC及び2μg/mLのMBECという強力な抗バイオフィルム活性を有した。寒冷培養物及び緊縮応答培養物は、ホスホマイシン、バンコマイシン及びダプトマイシンで処理した培養物の生存能力の喪失がなかったのと比較して、1時間以内に生存能力における5対数降下(5 log drop)が観察され、化合物10に対して感受性のままであった。
結論:黄色ブドウ球菌バイオフィルム及び緩慢増殖黄色ブドウ球菌培養物に対する化合物10の強力な活性は、その抗菌活性が細菌の増殖状態に依存せず、そしてバイオフィルム関連の黄色ブドウ球菌感染症の治療における化合物10の有用性を示唆していることを示している。
バイオフィルムの形成は、広範囲の細菌感染症における主要な因子として益々認識されている。異物関連感染症、嚢胞性線維症患者における肺の慢性感染症及び歯牙感染は、バイオフィルムが介在する感染症のほんの2、3の例である。実際、最近では、先進国におけるヒト感染症の80%は、バイオフィルム形成の直接の結果であると報告されている1。
さらにまた、バイオフィルム培養物は、遺伝子型の耐性の発現なしに、化学療法によって根絶することに対して典型的に高度に不応性である。結果的に、治療選択肢の数は限られ、そして抗バイオフィルム活性を有する新規抗菌剤を開発する重要性が高まっている。
化合物10は、新しい種類の抗菌剤の例であり、そして抗菌療法に対する新しいアプローチを表している。化合物10は、殺菌性(MBC50 1μg/mL)であり、メチシリン感受性黄色ブドウ球菌(MSSA)、医療関連型メチシリン耐性黄色ブドウ球菌(healthcare-associated methicillin-resistant S. aureus)、及び市中感染型メチシリン耐性黄色ブドウ球菌(community-associated methicillin-resistant S. aureus)を含む一連の黄色ブドウ球菌株に対して活性(MIC50 1μg/mL)であることはこれまで示されている2。本試験の目的は、バイオフィルム及び他の緩慢増殖細菌培養物に対する化合物10の活性を示すことである。
・BSACガイドライン3に従って浮遊のMICを測定した。
・標準の方法4に従ってカルガリーデバイスにおいてbMIC及びMBECを測定した。
・培養物を4℃に保つことを除いて標準の時間−死滅プロトコール(time-kill protocols)を用いて、寒冷培養物中の黄色ブドウ球菌SH1000(MSSA)に対する化合物10の死滅速度を測定した。
・ムピロシンで誘導された黄色ブドウ球菌SH1000の緊縮培養物に対する化合物10の死滅速度をOliva et al. (2003)5の方法に従って測定した。
・化合物10は、シプロフロキサシン、フシジン酸、テトラサイクリン及びリファンピンについてのそれぞれ4、0.5、0.5及び0.03μg/mLのbMIC及び>256、>256、>256及び128μg/mLのMBECと比較して、黄色ブドウ球菌SH1000に対する1μg/mLのbMIC及び2μg/mLのMBECという抗バイオフィルム活性を有した(表5)
・ホスホマイシンで処理した培養物について生存能力の喪失がなかったのと比較して、寒冷培養物及び緊縮応答培養物では、1時間以内に生存能力における5対数降下(5 log drop)が観察され、化合物10に対して感受性のままであった(図8及び9)。

・化合物10は、他の抗菌剤の殺菌活性が有意に低下する寒冷培養物及び緊縮応答培養物に対して強力に殺菌性のままである。

SH1000細胞を37℃で初期指数期(OD600nm 0.2)まで増殖させることによって寒冷培養物を準備した。次いで、培養物を遠心分離し、そして細胞沈殿物を、4℃に予冷却したMHBに再懸濁した。5時間のサンプリング期間にわたって培養物を4℃に維持したことを除いて上節で記載したように、抗菌剤の死滅速度を試験した。
緊縮応答を発現する黄色ブドウ球菌における化合物10及び化合物12の効果
ムピロシンの添加によって増殖を妨げられる黄色ブドウ球菌SH1000に対する化合物10及び化合物12の活性を試験した。これまでの研究は、緊縮応答が黄色ブドウ球菌に対するβ−ラクタム抗生物質及びホスホマイシンの殺菌活性を完全に無効にしたことを示している(Oliva et al. 2003)。ホスホマイシンを本試験における対照として含めた。4×MICで、ホスホマイシン活性は、緊縮条件下で完全に弱められた(図12)。同様の効果は、リファンピシンについて観察された(図12)。対照的に、化合物10及び化合物12は、緊縮応答の誘導によって増殖を妨げられたSH1000の培養物に対する強力な殺菌活性を保持していた(図12)。
寒冷培養物細胞の生存能力における化合物10及び化合物12の効果を5時間の期間にわたって測定した(図13)。低い温度は、化合物10及び化合物12の活性に対して効果がなく、これらは増殖がその温度変化によって停止している黄色ブドウ球菌SH1000に対して強力な殺菌活性を保持していた(図13)。これらの条件下で、ダプトマイシン及びナイシンは、いずれも限られた殺菌活性を保持していたが、生物を死滅させる他の薬剤の能力は、消失していた(図13)。
化合物10及び化合物12は、緩慢増殖又は細胞分裂しない黄色ブドウ球菌の種々の形態に対して高度に活性を保持していた。

標準の時間−死滅方法(Oliva et al. 2003, Hobbs et al. 2008)を用いて黄色ブドウ球菌の静止期培養物に対する化合物10、化合物12及び一連の比較薬剤の抗ブドウ球菌活性を試験した。増殖曲線を構築してSH1000培養物が静止期に入る及び出るときを同定した。ミュラーヒントンブイヨン(MHB)50mLを黄色ブドウ球菌SH1000一夜培養物500μLと共に接種し、そして撹拌しながら37℃で8日間維持した。光路1cmのJenway 6300分光光度計(Jenway, Essex, UK)を用いてOD600nmでの培養物濁度を一定の間隔で測定した。
黄色ブドウ球菌SH1000培養物をそれぞれ24、48及び72時間のインキュベーションによって37℃で初期、中期及び後期静止期まで増殖させた。培養物を遠心分離し、上清を除去し、次いで細胞ペレットの一部を対応する上清中でOD600nm 0.2(108細菌/mL)に再懸濁した。次いで、これらの静止期懸濁液において時間−死滅アッセイを行なって細菌細胞生存性に対する抗菌剤の効果を試験した。未処理の培養物は、陰性対照として機能した。実験は、三つ組で行った。
黄色ブドウ球菌の静止期培養物における化合物10及び化合物12の効果
MHB中37℃で培養した黄色ブドウ球菌SH1000について延長期間にわたる生物の増殖曲線を検討することによって静止期の開始及び終了を確立した。細胞は、増殖24時間後に静止期に入り、そして96時間で出ることが明らかになり、その後、培養物濁度が低下し、細菌溶解及び死滅を示す。従って、初期静止期は、接種の24時間後、中期静止期は48時間で、そして後期静止期は72時間で開始すると考えられる。静止期培養物で達成された高い細胞密度に関連する感受性試験についての接種物効果を回避するために、24時間、48時間及び72時間の時点で生物を回収し、そしてこれらの培養物から使用済みの増殖培地中で108細菌/mLにこれを希釈した後、阻害剤の殺菌活性を測定した。化合物10及び化合物12は、静止期のすべての時点から回収された細胞に対して強力な殺菌活性を保持していた(図14〜16)。化合物10及び化合物12と対照的に、静止期培養物に対する比較薬剤の活性は劣っていた(図14〜16)。
化合物10及び化合物12は、静止期の初期、中期及び後期段階において黄色ブドウ球菌の培養物に対して高度に活性のままであった。

- 化合物が中心金属イオンを含む、請求項1〜4のいずれか1項に記載の医薬組成物。
- 微生物バイオフィルムが細菌を含む又は細菌からなる、請求項1〜5のいずれか1項に記載の医薬組成物。
- バイオフィルムが口腔、尿路、洞、耳、心臓、前立腺、骨、肺、腎臓又は皮膚にある、請求項1〜6のいずれか1項に記載の医薬組成物。
- バイオフィルムが生きている哺乳類の体内にある請求項1〜7のいずれか1項に記載の医薬組成物。
- バイオフィルムが体内の不活性支持体に付着している請求項1〜8のいずれか1項に記載の医薬組成物。
- 緩慢増殖期又は静止期の細菌の増殖を死滅、抑制又は予防するための請求項1〜5のいずれか1項に定義された医薬組成物。
- 請求項1〜5のいずれか1項に記載された医薬組成物で含浸、コーティング又は別の方法で処理された植込み型医療用デバイス。
- 血管内デバイス、カテーテル、シャント、挿管及び気管切開チューブ、眼科用デバイス、関節補綴、人工心臓弁及び乳房インプラントからなる群より選ばれる請求項11に記載の植込み型医療用デバイス。

- 化合物が5,15−ビス−[4−(3−トリメチルアンモニオ−プロピルオキシ)−フェニル]−ポルフィリン(「XF−73」)若しくは5,15−ビス−[3−(3−トリメチルアンモニオ−プロピルオキシ)−フェニル]−ポルフィリン(「XF−70」)、又はそれらの二塩化物塩である、請求項13〜15のいずれか1項に記載のインビトロの方法。
- 化合物が中心金属イオンを含む、請求項13〜16のいずれか1項に記載のインビトロの方法。
- バイオフィルムが、家庭環境、商業的又は工業的環境にある、請求項13〜17のいずれか1項に記載のインビトロの方法。
