（関連技術の背景）
輪状吻合を実施するための種々の形式の外科手術用ステープル留め具が公知であり、手術者は、輪状吻合が発生する場所から比較的遠隔の場所において、その装置を起動させる。吻合は、別個の中空の器官区分の外科手術的接合である。通常、吻合処置は、中空の組織の病気または欠陥のある区分が除去される外科手術を含む。

（本実施形態の詳細な説明）
本明細書に開示されるエンドエフェクタの種々の実施形態が、ここで図を参考にして詳細に記載され、同様の参照数字が類似のまたは同一の要素を識別する。以下の図においておよび説明においては、伝統的技術および従来技術と同様に、「近位の」は、使用の際、手術者により近接している構成要素端を指し、「遠位の」は、手術者からより離れている構成要素端を指す。
本明細書に開示されるエンドエフェクタの種々の実施形態が、ここで図を参考にして詳細に記載され、同様の参照数字が類似のまたは同一の要素を識別する。以下の図においておよび説明においては、伝統的技術および従来技術と同様に、「近位の」は、使用の際、手術者により近接している構成要素端を指し、「遠位の」は、手術者からより離れている構成要素端を指す。
図１、２、および７を参照して、外科手術用締結装置１０００に好適なエンドエフェクタ１００が示されている。外科手術用締結装置１０００の好適な例は、米国特許第７，１６８，６０４号、米国特許第７，３０３，１０６号、および２００９年８月３１日に出願された米国特許出願第１２／５５０，４４３号に開示されている。これら特許／出願の各々の全体内容が本明細書に参考として援用される。外科手術用締結装置１０００は、ハンドルアセンブリ２００、およびそこから遠位に延びる細長いシャフト３００を含み、エンドエフェクタ１００は細長いシャフト３００の遠位端に連結されている。細長いシャフト３００の長さおよび湾曲度合いは、実施される外科手術の特定のニーズに合うように仕立てられ得る。あるいは、細長いシャフト３００は、外科手術用締結装置１０００、より具体的には、エンドエフェクタ１００を、接合される組織を含む身体内の目標エリアに向けて操縦することを容易にするようにフレキシブルであり得る。
ハンドルアセンブリ２００は、概して、固定ハンドル２４０、旋回可能なトリガ２１０、および／または回転可能なノブ２２０を含む。回転可能なノブ２２０が手動で回転させられると、アンビルアセンブリ２０は、以下に記載される態様で、開いた位置と閉じた位置との間で、シェルアセンブリ１０に対して長手方向に並進する。トリガ２１０の固定ハンドル２４０に向かう起動は環状プッシャ４０（図３）をシェルアセンブリ１０の中で遠位に前進させ、以下に記載される態様で、ファスナをシェルアセンブリ１０から放出する。トリガ２１０の起動はまた、輪状ナイフ５１を長手方向に並進させ、以下に記載される態様で、組織を切断して、形成されたファスナを保持フレームから分離する。

ファスナ保持フレーム２４のフランジ部材１６（フランジ部材１６はアンビル２２に固く取り付けられる）への取り付けは、ファスナ保持フレーム２４のアンビル２２に対するすべりを減少させ、その軸方向の並進運動の間にアンビル２２に対するその位置を保持する。一旦、身体組織の締め付けが起こると、手術者はトリガ２１０を起動させ、トリガは次いで環状プッシャ４０に連結された駆動シャフト４を遠位に駆動する。遠位に並進させられた環状プッシャ４０上の複数のプッシャ４２は、ファスナガイド部材３０のファスナスロット３４から対応する外科手術用ファスナ３２のセットを放出する。放出された外科手術用ファスナ３２のセットの各々は、身体組織を貫通し、対応する保持具２８の中に受け取られる。具体的には、ファスナ３２の爪の各々の部分は、保持具２８のポスト２９の中に形成されたそれぞれの空洞２９ａの中に受け取られる。ファスナ爪３３の先端３３およびかかり３４が、ファスナ爪３３が空洞２９ａを通して強制されるときにポスト２９の受け取り壁２９ｂを変形することに注意されたい。ファスナ３２の後退（近位への移動）を防ぐために、かかりは空洞２９ａの突起を係合し、それにより、ファスナ３２を保持具２８に対してロックする。ファスナ保持フレーム２４がフランジ部材１６にしっかりと取り付けられ、フランジ部材はアンビル２２に固く取り付けられているので、トリガ２１０の起動によって生成され、複数のファスナ３２をファスナスロット３４から身体組織に放出する発射力による、ファスナ保持フレーム２４のアンビル２２に対するすべりは減少させられる。このように、ファスナ保持フレーム２４の位置は、外科手術用ファスナ３２の発射の間、アンビル２２に対して維持される。さらに、複数の保持具２８は、一体として形成されるか、または個別に形成されて例えばリンクのような好適な構造によって連結され、それによって単体を形成する。どちらの場合にも、単体としての複数の保持具２８の構成は、アンビル２２に固く取り付けられているフランジ部材１６に固定されているファスナ保持フレーム２４と共に、外科手術用ファスナ３２のファスナ保持フレーム２４に対する半径方向のミスアライメントを減少させる。
上記で表わされたように、ファスナ保持フレーム２４およびファスナガイド部材３０は、実施される特定の手術のニーズに合うように組織上に外科手術用ファスナ３２の複数のリング（例えば、外科手術用ファスナの各リングが１６個のファスナによって画定される２つのリング）を提供するように構成されている。複数の外科手術用ファスナ３２がファスナ保持フレーム２４と嵌合すると、環状ブレード５１が、シェルアセンブリ１０を通って遠位に並進するように起動させられる。環状ブレードが、ファスナを前進させるトリガ２１０の起動によって、あるいは、別個のステップで起動される別個のアクチュエータによって並進可能であり得ることに注意されたい。環状ブレードは、ファスナ保持フレーム２４と接触するようになり、ファスナ保持フレーム２４の中に画定される複数の打ち抜き穴２６に対して押され、打ち抜き穴２６を切断または分離する。分離または切断された打ち抜き穴２６は、嵌合された外科手術用ファスナ３２およびファスナ保持フレーム２４がアンビル２２から取り外されることを可能にする。打ち抜き穴２６の切断または分離は、残りのリング、すなわち、ファスナ保持フレーム２４の第２の環状部分２４ｂの内側部分を、フランジ部材１６の環状凹部１８の突起１８ａ内に残す。いくつかの実施形態において、打ち抜き穴２６の切断または分離の間に生成され得る打ち抜き穴の切れ端を防ぐために、複数の打ち抜き穴２６の各々が、例えば、上記で述べたように、一対の対向するスリットまたは１つの刻み目部分を伴って提供され得、環状ブレードの厚みは、例えば、一対の対向するスリットの幅よりも薄いように選択される。嵌合された外科手術用ファスナ３２およびファスナ保持フレーム２４は、それらの間に身体組織を締結して、アンビル２２に制限されたり取り付けられたりすることなく、次いで自由に退去する。ナイフの起動は組織を切断する。手術者は、次にハンドルアセンブリの回転可能なノブ２２０の回転によってアンビル２２を開いた（近接していない）位置に移動し、それによって、アンビルアセンブリ２０を遠位に並進させる。切断された組織、例えば痔の組織、の除去の後に、外科手術用締結デバイスは身体から取り除かれる。
前述の種々の図から、および種々の図を参考にして、当業者はある修正が本開示の範囲から逸脱することなく本開示になされ得ることも理解するであろう。例えば、環状ブレードは、アンビルに固く固定され得、複数のファスナの発射によってファスナ保持フレーム上の複数の打ち抜き穴を破壊するように構成され得る。そのような設計は、環状ブレードの長手方向の並進に対するニーズを削除する。加えて、輪状締結装置を伴う使用に対して環状部分として開示されたが、ファスナの線形アレイを適用する装置を伴う使用に対して線形部分の使用もまた企図される。
本開示のいくつかの実施形態が図の中に示され、および／または本明細書において論じられてきたが、本開示がそれらに限定されることは意図されていないし、本開示が技術の許す限りの広い範囲におけるものであり、本明細書はそのように読まれることが意図されている。それゆえに、上記の記載は限定として解釈されるべきではなく、単に具体的な実施形態の例示として解釈されるべきである。当業者は、本明細書に付随する請求項の範囲および精神の範囲内において他の修正を想定する。

Claims (14)
- 外科手術用締結装置であって、該外科手術用締結装置は、
第１の環状部分と第２の環状部分とを含むファスナ保持フレームであって、該第１の環状部分は、該第１の環状部分の上に配置された複数の保持具を有し、該第２の環状部分は、該第２の環状部分の中に形成された複数の打ち抜き穴を有する、ファスナ保持フレームと、
複数の外科手術用ファスナを含むファスナガイド部材であって、該外科手術用ファスナは、複数の保持具と嵌合することにより、該ファスナと該保持具との間で組織を締結する、ファスナガイド部材と、
該ファスナ保持フレームの支持構造であって、該ファスナ保持フレームは、該支持構造に取り外し可能に固定されている、支持構造と、
該複数の外科手術用ファスナを該ファスナガイド部材から該複数の保持具に向けて放出するための環状プッシャと、
該複数の外科手術用ファスナが該複数の保持具と嵌合した後に、該複数の打ち抜き穴を切断するための環状ブレードと
を含み、
該支持構造は、フランジ部材を含み、該フランジ部材は、アンビルに固定的に取り付けられており、かつ、該ファスナ保持フレームに取り外し可能に固定されており、該フランジ部材は、環状の凹部と突起とを有し、該ファスナ保持フレームは、該環状の凹部の中に配置されており、かつ、該環状の凹部の中に該突起によって固定されている、外科手術用締結装置。 - 前記第１の環状部分上の前記複数の保持具は、該第１の環状部分上に円周状に配置されており、前記第２の環状部分の中の前記複数の打ち抜き穴は、該第２の環状部分の中に円周状に配置されており、該第１の環状部分および該第２の環状部分は、同心状に配置されている、請求項１に記載の外科手術用締結装置。
- 前記第２の環状部分は、前記第１の環状部分と同心状に配置されている、請求項１から２のいずれか一項に記載の外科手術用締結装置。
- 前記第１の環状部分は、前記複数の保持具に接続された複数のリンクをさらに含む、請求項１から３のいずれか一項に記載の外科手術用締結装置。
- 前記複数の保持具は一体として形成されている、請求項１から４のいずれか一項に記載の外科手術用締結装置。
- 前記第１の環状部分は、前記第２の環状部分と一体として形成されている、請求項１から５のいずれか一項に記載の外科手術用締結装置。
- 前記ファスナ保持フレームの前記第１の環状部分上に円周状に配置された前記複数の保持具は、保持具の２つの同心状のリングを画定するように配置されている、請求項１から６のいずれか一項に記載の外科手術用締結装置。
- 前記複数の保持具は、実質的に均一に離間されており、前記第２の環状部分の中に円周状に配置された前記複数の打ち抜き穴は、実質的に均一に離間されている、請求項１から７のいずれか一項に記載の外科手術用締結装置。
- 前記複数の保持具および前記複数の外科手術用ファスナは、生物分解性のポリマーから作成されており、前記ファスナ保持フレームは、生物分解性のポリマーから作成されている、請求項１から８のいずれか一項に記載の外科手術用締結装置。
- 前記ファスナ保持フレームの遠位端は、該ファスナ保持フレームの厚みに対して軸方向にテーパ付けされている、請求項１から９のいずれか一項に記載の外科手術用締結装置。
- 前記ファスナ保持フレームは、前記フランジ部材とのスナップ嵌め構成を有する、請求項１から１０のいずれか一項に記載の外科手術用締結装置。
- 前記第２の環状部分の中に画定される前記複数の打ち抜き穴の各々は、半径方向に刻み目のある部分を有するか、または、一対の対向するスリットを有する、請求項１から１１のいずれか一項に記載の外科手術用締結装置。
- 前記複数の外科手術用ファスナは、前記複数の保持具とスナップ嵌めするように構成されている、請求項１から１２のいずれか一項に記載の外科手術用締結装置。
- 前記第２の環状部分の中の前記複数の打ち抜き穴を画定する部分の厚みは、該第２の環状部分の残部の厚みよりも薄い、請求項１から１３のいずれか一項に記載の外科手術用締結装置。
