Description
本出願は、米国仮特許出願第60/782,911号(2006年3月16日出願)、および米国仮特許出願第60/852,932号(2006年10月19日出願)からの優先権を主張し、該仮出願の両方は、その全体が明確に参考として援用される。
本発明は、概して、脱出症治療分野のシステムおよび方法に関する。より具体的には、本発明は、複数の構成部品を有する装置、および同装置を使用して組織壁脱出症を外科的に矯正するための方法に関する。特に、本発明の一実施形態は、予めカットされた形状のメッシュグラフトおよびグラフト送達デバイスを有するキットである。
例えば、本発明は、以下を提供する。
(項目1)
膣壁に切開口を設けるステップと、
上記切開口を通して、弾丸針と連結される少なくとも1つのウイングを有するグラフトを挿入するステップと、
グラフト設置デバイスを使用して、上記ウイングおよび弾丸針を体内組織を通して押入するステップと、
を包含する、骨盤症状を治療する方法。
(項目2)
上記グラフト設置デバイスを使用して、切開口から上記ウイングを引き出すステップをさらに包含する、項目1に記載の方法。
(項目3)
上記グラフト設置デバイスから、上記弾丸針とウイングとを取り外すステップをさらに包含する、項目1に記載の方法。
(項目4)
上記切開口を縫合して閉じるステップをさらに包含する、項目1に記載の方法。
(項目5)
上記切開口を開くステップは、筋肉および筋肉組織を後方に引くことによる膣傍組織の切開を包含する、項目1に記載の方法。

簡略的に上述された構成部品を使用して、設置方法は、次のステップを含む。膣壁に切開口を設ける。膣内および骨盤底へのアクセスを得るために切開口を開く。グラフト設置デバイスなどの縫合糸捕捉デバイスを手に取る。グラフト設置デバイスなどの縫合糸捕捉デバイスに、弾丸針を備えたメッシュウイングを取り付ける。切開口を通して膣内にウイング、針および縫合糸デバイスを挿入する。靭帯または筋肉を通してウイングおよび弾丸針を押入する。縫合糸デバイスを用いて切開口からウイングを引き出す。縫合糸デバイスから弾丸針およびウイングを取り外す。縫合糸デバイスに別のウイングとその弾丸針とを取り付け、膣内の別の位置でそのプロセスを反復する。全てのウイングが一般的に容認されている尖端および側部の支持構造のような一部の内部構造に取り付けられるまで、他のウイングおよび弾丸針を用いてプロセスを反復する。これらの支持構造は、一般的に仙棘靭帯、近位腱弓、および肛門挙筋である。このウイング固定は、患者ごとの個別の調節を可能にするが、これは従来の縫合固定では行われないことである。グラフト調節本体が膣の内部頂部壁を覆うように、次にウイングが必要に応じて締められる。過剰のメッシュウイング材料が次いで切除され、破棄される。残りのメッシュが固定された後に、膣切開口が閉口される。
Claims (39)
- 骨盤症状の処置のための装置であって、以下:
上部、下部、および該上部と該下部との間に配置された中央部分を有するグラフトであって、該グラフトが第1のウイング、第2のウイング、第3のウイングおよび第4のウイングを含み、
該第3のウイングが該中央部分から延び、該第3のウイングがメッシュ材料から形成され、該中央部分が長手軸を規定し、該第3のウイングが該長手軸に対して鋭角で該中央部分から延び;該3のウイングが定着組織に固定可能であって端部を有し;該第3のウイングが該第2のウイングと該下部との間に配置され、該第3のウイングが該第1のウイングの長さより大きい長さを有する、グラフトと;
該第3のウイングの端部に連結される弾丸針であって、該弾丸針が定着組織を通って展開されるように構成され、そして該定着組織を通って展開された後に送達デバイスのキャッチ部分内に固定される、弾丸針と、を備える装置。 - 前記グラフトが、ポリプロピレンメッシュから作製される、請求項1に記載の装置。
- 前記グラフトが、患者の膣円蓋全体を覆うサイズおよび形状である、請求項1に記載の装置。
- 前記第3のウイングが、患者の仙棘靭帯に対して固定可能なように構成される、請求項1に記載の装置。

前記第4のウイングが定着組織に固定可能であり、そして

該第5のウイングに連結された第4の弾丸針であって、該第4の弾丸針が定着組織を通って展開され、かつ患者の体内で送達デバイスのキャッチ部分内に固定されるように構成される、第4の弾丸針、を備える、装置。 - 前記中央部分および前記第3のウイングが、メッシュ材料から形成される、請求項1に記載の装置。
- 前記第3のウイングと前記弾丸針との間に延びる縫合糸をさらに備える、請求項1に記載の装置。
- 前記第3のウイングが、先細り形状の部分を含む、請求項1に記載の装置。
- さらに、前記第3のウイングと前記弾丸針との間に配置される針接続セグメントを備え、該針接続セグメントが先細り形状を有する、請求項1に記載の装置。

切開を通って患者の身体中に挿入されるように構成された請求項1に記載のグラフトと;
該患者の身体組織を通って、前記ウイングおよび該弾丸針を、該ウイングが第1の位置で該身体組織に入り、そして第2の位置で該身体組織から出るように押すためのグラフト配置デバイスと;を備え、
該弾丸針が該グラフト配置デバイスを用いて捕捉されるよう構成されており、そして
該グラフトが該患者の身体内に、該中央部分の第1の部分が該患者の膣の前壁に隣接して配置されるように位置決めされる、システム。 - 患者の膣壁に切開を作る手段をさらに備える、請求項22に記載のシステム。
- 前記グラフト配置デバイスがさらに、前記切開から前記ウイングを、該ウイングの一部分が前記身体組織を通って延びるように戻して引くように構成されており、該身体組織が該患者の腱弓および該患者の仙棘靭帯のうちの1つである、請求項22または23に記載のシステム。
