Description
本発明は、血液透析及び血液濾過の一方又は両方を行って血液の浄化処理において、体外回路内の血液に対して血液浄化器(血液透析器、血液濾過器又は血液透析濾過器)の上流側と下流側とで置換液を注入する血液浄化装置の作動方法及び血液浄化装置に関する。
“透析による腎機能の代行”(ドラッカー(Drukker)、パーソンズ(Parsons)及びマーヘル(Maher);クリューバーアカデミックパブリッシャーズ(Kluwer Academic Publishers)、1996年第4版;エイチ.ディー.ポラーシェグ(H.-D. Polaschegg)及びエヌ.ダブリュ.レビン(N. W. Levin)著“血液透析装置及びモニタ”)−その開示を本明細書が明示的に引用する−には、最も重要な血液透析手順と装置の概要が記載されている。
総括的な方式の方法から述べると、上記の目的は、治療及び血液の少なくとも何れか一方に関する1個のパラメータを制御することと、その制御を血液透析器又は血液濾過器の上流側に供給される置換液の注入量、及び下流側に供給される置換液の注入量を変更して実行することとによって達成される。
以下、本発明の実施形態を図面に基づいて詳細に説明する。

Claims (11)
- 体外血液回路(10)の血液に対して、血液浄化器(20)の上流側と下流側とにおいて置換液の注入量を制御部(100)が制御し、血液透析及び血液濾過の少なくとも何れか一方を行って血液を浄化する血液浄化装置の作動方法であって、
上記制御部(100)は、上記血液浄化器(20)の上流側と下流側とで供給される上記置換液の両注入量(QSpre,QSpost)を変更して血液濃度とヘマトクリット値(HKT)との何れか1個のみをパラメータとして制御するとともに、上記血液浄化器(20)の上流側に供給される上記置換液の注入量(QSpre)を、上記パラメータとした血液濃度及びヘマトクリット値(HKT)の何れか1つが上昇するのに従って、上記血液浄化器(20)の下流側に供給される置換液の注入量(QSpost)に対して増加させることを特徴とする血液浄化装置の作動方法。
- 請求項1において、
血液濃度とヘマトクリット値(HKT)との何れかは、持続的に検出されて制御される
ことを特徴とする血液浄化装置の作動方法。
- 請求項1又は2において、
上記血液浄化器(20)の膜の血液が流れる室に面する側に境界膜がほぼ定常に形成されるように、上記両置換液の注入量(QSpre,QSpost)を選択する
ことを特徴とする血液浄化装置の作動方法。
- 請求項1において、
治療終了後における血液の流れの中での上記両置換液の注入量(QSpre,QSpost)の
関係を変更することにより上記境界膜を分解する
ことを特徴とする血液浄化装置の作動方法。
- 浄化すべき血液を受け入れる体外血液回路(10)と、
該体外血液回路(10)と連通する血液浄化器(20)と、
上記血液浄化器(20)の上流側と下流側の上記体外血液回路(10)にそれぞれ設けられ、置換液を供給する置換液供給路(12,14)とを備え、
血液透析及び血液濾過の少なくとも何れか一方を行って血液を浄化する血液浄化装置であって、
血液濃度とヘマトクリット値(HKT)との何れか1個のみをパラメータとし、上記血液浄化器(20)の上流側と下流側の置換液の両注入量(QSpre,QSpost)を変更して上記パラメータが制御されるとともに、上記血液浄化器(20)の上流側に供給される上記置換液の注入量(QSpre)を、上記パラメータとした血液濃度及びヘマトクリット値(HKT)の何れか1つが上昇するのに従って、上記血液浄化器(20)の下流側に供給される置換液の注入量(QSpost)に対して増加させるように構成された制御部(100)が設けられていることを特徴とする血液浄化装置。
- 請求項5において、
上記制御部に接続され、血液濃度とヘマトクリット値(HKT)との何れか1個を記録する測定装置を備えている
ことを特徴とする血液浄化装置。
- 請求項6において、
上記測定装置は、上記血液浄化器(20)の上流側及び下流側で上記体外血液回路(10)及び透析液回路(30)の少なくとも何れか一方に設けられた圧力センサ(40)を備えている
ことを特徴とする血液浄化装置。
- 請求項6又は7において、
上記測定装置は、上記血液浄化器(20)の上流側及び下流側の上記体外血液回路(10)に設けられたヘマトクリット値(HKT)検出用のセンサ(50)を備えている
ことを特徴とする血液浄化装置。
- 請求項6又は7において、
上記測定装置は、上記血液浄化器(20)の上流側及び下流側の上記体外血液回路(10)に設けられた血液濃度検出用のセンサ(50)を備えている
ことを特徴とする血液浄化装置。
- 請求項5〜9の何れか1項において、
上記両注入量(QSpre,QSpost)を制御する手段は、上記置換液供給路(12,14)の
ポンプ(13,15)である
ことを特徴とする血液浄化装置。
- 請求項5〜10の何れか1項において、
上記両注入量(QSpre,QSpost)を制御する手段は、上記置換液供給路(12,14)の
弁である
ことを特徴とする血液浄化装置。
