Description
図1および図2に示したように、体外式自動除細動器(AED)10は取り外し可能なカバー12および装置収納箱14を含む。図2にカバー12を取り外した除細動器10を示す。電極アセンブリ16(または一対の分離電極)はケーブル18により装置収納箱14に接続されている。除細動器が使用されない時、電極アセンブリ16はカバー12の下に格納される。
カバー12は、ロゴ31および一連の図32、34、および36を含むカバー転写図30(図1および4)を含む。ロゴ31は、装置の製造者に関する情報および装置が除細動器であることを示すことができる(例えば、図1に示すように、「ZOLL AED」、は、その装置がZoll Medicalから入手できる体外式半自動除細動器であることを示している)。図32、34、および36はAHA(米国心臓協会)による、緊急医療職員向けの審議中着信物である緊急心臓ケアに対するAED治療アルゴリズムの中に概説されているように、心臓蘇生の手順の中の初期ステップを通して介護者をリードする。(心肺蘇生術および心血管治療のガイドライン2000年版、回覧用補足資料、102巻、ナンバー8、2000年8月22日、1−67頁を参照。)従って、介護者と患者を示す図32は、介護者が例えば、患者を静かに揺すり大丈夫かどうかを尋ねることによって、患者の応答性を第一に確認すべきであることを表示している。続いて、電話と救急車を示す図34は、介護者は蘇生術を施すに先立って救急支援を求めるべきであることを表示している。最後に図36は、これらのステップが実行された後、介護者は除細動器のカバー12を取り外し、蓋の下に格納されている電極アセンブリ16を取り出し、ボタン15を押して電源を入れるべきであることを表示している。その図は、第一ステップを上部左に置いた時計回りの順番で配置されている。この順番で介護者が最も直感的に従うはずであるからである。しかしこの場合では、介護者がそのステップ実行する順番は判断的なものではない、従って簡略化のためにステップの順番に関するその他の表示は無い。
患者の舌や咽頭蓋による気道障害を避けるために(例えば図10A)、患者は頭を傾けて押し下げた状態に首を持ち上げ支持する位置関係に置かれることが望ましい。患者をこのような様式で位置決めすることは、「頭傾け顎持ち上げ法」として、米国心臓協会の心肺蘇生術および心血管治療のガイドラインの中で言及されている。頭傾け顎持ち上げの位置決めは口および気管を通して肺に至る比較的直線的な開放気道を提供する。しかし、緊急治療期間中この位置で患者を保持することは困難であるかもしれない。
例えば、所望によりすべての図が半透明化できるし、バックライト点灯もできる。代わりに図は転写図よりむしろLED画像の形で提供されうる。
ある実施態様においては、図で例示されたステップの全てが、例えば、前述の転写図で例示されたように一括して同時に示される。この配置により介護者は次に実行されるステップを見たり、予測したりでき、可能であればステップを早く開始できる。しかしその代わり、図は、例えば、1個の図ずつを表示するスクリーンあるいはバックライトが点灯しないと見られないバックライト式図を使用する1個ずつ表示にできうる。場合によっては、この配置は初心者や一般救助者の混乱を避けることができる。なぜならばこの方式では介護者に同時に多すぎる情報の全てを提供しないからである。
AEDのカバー12は、その下側に例えば、図11に示した転写図200のような転写図を含めることができる。転写図200は、患者の気道を蘇生期間中開放して維持するために気道確保装置としてそのカバーを使用することを例示している。
図12に示されているものに匹敵する可視ディスプレイは、救助者にETチューブが適正に置かれたかどうかについて可視的なフィードバックを提供するために、電極アセンブリ上に置くことができる。チューブが適正に置かれたと判断されると、処理手段は電極アセンブリ上の緑色のLEDを起動する。以前の換気試行がETチューブを不適正に置いたと判断した結果になった場合、処理手段は可視ディスプレイで赤色のLEDを起動する。可視ディスプレイは、各々の連続した換気の一回換気量を示すために二重カラー棒グラフとして構成され、チューブが適正に置かれたどうかを表示する着色LEDバー(緑色で適正な位置を、赤色で不適正な位置を示す)を持った一連のLED構成をも含むことができる。一方、適正なETチューブ位置および一回換気量をそれぞれ示すために、個別に示す明かりが気道および呼吸に対して提供されうる。
Claims (9)
- 患者に除細動ショックを与えるとともに患者に心臓蘇生を行う介護者を支援する体外式自動除細動装置であって、
少なくとも一個の除細動電極を支持し、患者の胸に接着して適用されるように構成された電極パッドと、
介護者による患者への換気の遂行に関連する情報を検出するように構成された少なくとも一個の圧力センサであって、圧力センサの少なくとも一部が前記電極パッド上に搭載されており、前記圧力センサ及び前記装置は、前記センサが患者の気道内の空気圧を計測するように構成されていることと、
圧力センサの出力を処理して、換気の進捗を示すパラメータについて判断するように構成されたプロセッサと

- 前記電極パッド上の圧力センサから、患者の口付近に位置するように構成されたアダプタまで少なくとも一本のチューブが延びており、前記チューブと前記アダプタは、換気に伴う圧力が前記チューブを通して電極パッドアセンブリの前記圧力センサに伝わるように構成されている請求項1に記載の装置。
- 前記圧力センサから前記アダプタまで延びる第二のチューブをさらに備え、前記アダプタと前記チューブは、換気に伴う差圧を測定するように構成されている請求項5に記載の装置。
- 換気についての情報を伝えるために電極パッドアセンブリ上に設けられた少なくとも一個の表示灯をさらに備える請求項1に記載の装置。
- 前記情報は、換気回数が許容可能な換気回数の所定範囲内にあるか否かを含む請求項7に記載の装置。
- 前記情報は、一回換気量の表示を含む請求項7に記載の装置。
