本発明は、メッシュと、該メッシュの剛性(スティフネス)をその当初の剛性の少なくとも1.1倍に一時的に高める1層または2層以上の被膜(コーティング)とから構成される医療用補綴具に関する。かかるメッシュをコーティングする被膜は、メッシュの集結性を変化させず、従ってメッシュは多孔性のままである。一般に、この被膜はメッシュの気孔率を実質的に変化させない。より具体的には、本発明の医療用補綴具は、1層または2層以上の被膜で被覆されたメッシュからなり、少なくとも1層の被膜は該メッシュを構成するフィラメントまたは繊維の接触点を一時的に不動化(固定)するように、該フィラメントまたは繊維をコーティングする補剛剤を含んでいる。やはり、かかるメッシュをコーティングする被膜は、基材であるメッシュの集結性強度を変化させず、従ってメッシュは被覆後も多孔質のままとされる。メッシュは使用条件下でその当初の剛性に実質的に復帰することができる。
従って、本発明は生分解性ポリマーを用いて一定の決まった持続時間の間、制御された速度で薬剤を送給する方法も提供する。
メッシュの表面上に付着させた生分解性被膜は、未被覆のメッシュに比べて優れた取扱い特性をメッシュに付与し、メッシュに剛性を付与するため外科的な挿入を容易にし、それによりその取扱いを改善する。しかし、時間が経過すると、被膜が再吸収され、即ち、補剛剤が分解または軟化して、柔軟なメッシュが残り、強度を低下させずに患者により大きな快適さを付与する。
本発明の好ましい生分解性ポリマーは、チロシン誘導ジフェノール系モノマー単位を適当な化学部位(モノマー成分)と共重合させて形成されたポリアリレート、ポリカーボネート、ポリイミノカーボネート、ポリホスホネートまたは他のポリマーである。
本書で使用したDTEはジフェノールモノマーデスアミノチロシル−チロシンエチルエステルであり;DTBnはジフェノールモノマーデスアミノチロシル−チロシンベンジルエステルであり;DTは対応する遊離酸形態、即ち、デスアミノチロシル−チロシンである。BTEは、ジフェノールモノマー4−ヒドロキシ安息香酸−チロシルエチルエステルであり;BTは対応する遊離酸形態、即ち、4−ヒドロキシ安息香酸−チロシンである。

本発明の別の側面はメッシュのフィラメントまたは繊維の接触点を一時的に不動化(固定)するためにメッシュのフィラメントまたは繊維をコーティングする補剛剤によるメッシュのコーティング(塗布)方法に関する。この方法は、(a) 溶媒と前記補剛剤とを含む塗布液を調製し;(b) 前記メッシュ上に一定量の前記塗布液が付着するようにメッシュに1回以上の噴霧を行って、前記メッシュのフィラメントまたは繊維の接触点を一時的に不動化するのに十分な厚みと位置を有する、フィラメントまたは繊維を被覆した被膜(コーティング)を形成し;そして(c) 前記メッシュを乾燥して前記被膜を形成することを含む。ポリマー被膜に対するコーティング(被膜)厚みの比は図7の走査式電子顕微鏡写真に示されている。薬剤(または薬剤混合物)を併用する場合には、その薬剤は所望濃度で塗布液中に含有させる。
A.噴霧塗布によるメッシュの調製
9:1テトラヒドロフラン/メタノール中にリファンピン:ミノサイクリン:ポリマーを1:1:8の比率で含有する1%溶液を外科用メッシュに噴霧塗布した。塗布は、メッシュの両面に、各面が少なくとも10mgの抗菌剤含有ポリマーで被覆されるまで繰り返し噴霧ノズルを通して噴霧することによって行った。サンプルは、使用前に真空乾燥器で少なくとも72時間乾燥した。
抗生物質被覆メッシュのZOIをキルビー・バウアー(Kirby-Bauer)法に従って求めた。表皮ブドウ球菌(Staphylococcus epidermidis)または黄色ブドウ球菌(Staphylococcus aureus)を保存培養株からトリプリケート・ソイ・ブロス(TSB, Triplicate Soy Broth)に接種し、濁度がマクファーランド(McFarland)0.5番規格に達するまで(1〜2時間)37℃でインキュベーションした。この細菌を、ミューラー・ヒントン(Mueller-Hinton) II寒天(MHA)上に3回画線することによりプレートを調製した。各回において、プレートの画線動作(スワビング)は左から右方向にプレート全体をカバーするように行い、スワビング間に画線の方向を変化させるようにプレートを回転させた。
A.メッシュの調製
図2に示した実験に対して、THFメタノールの混合溶媒中1%P22−27.5ポリアリレートでの4%溶液としてブピバカインHClを540mg含有する第1のデポー被膜をメッシュ上に噴霧塗布した。同じポリアリレート425mgのみからなる第2の層を第1層の上に付着させた。
予め秤量したメッシュ断片を37℃のPBS中に入れ、HPLCによるブピバカインの定量のためにサンプルを定期的に取り出した。図2は多層ポリアリレート被覆からPBS中へのブピバカインの累積放出を時間の関数として示す。ブピバカインの80%近くが25時間のインキュベーション後に放出されていた。
A.概論
薬力学試験用の頸静脈カニューレをつけたラットに、7.5mg/cm2のブピバカインを含有する1×2cmのP22−27.5ポリアリレート被覆メッシュを外科移植した。手術前に、予定した外科切開部位で痛覚に対する基底 (ベースライン) ピン痛覚反応を測定すると共に、基底の血液検体を取得した。ヘルニアは、肋骨下を経た開腹中に腹腔中への切開により作製し、ブピバカイン含浸ポリアリレート被覆メッシュを用いて、リヒテンシュタイン無張力(Lichtenstein non-tension)修復を行った。血液検体は、移植から3、6、24、48、72、96および120時間後に採取した。血液を採取する前にラットにピン痛覚試験を受けさせてブピバカイン放出からの皮膚麻酔を評価した。この行動試験結果は、3〜120時間の間に中程度の皮膚麻酔が現れ、6時間および48時間の量が基底より有意に高かった (p<0.05) ことを示す。薬力学的解析は、血漿ブピバカイン濃度が0〜24時間の一次吸収で1コンパートモデルに適合することを示す。
ポリプロピレンメッシュを実施例2の最初の段落に記載したようにして噴霧塗布した。個々のメッシュを1×2cmに裁断し、個別包装し、ガンマ線照射により滅菌した。このメッシュには、1×2cm当たりの充填量で合計15mgのブピバカインとなるように7.5mg/cm2のブピバカインHClが充填されていた。
59〜63日齢で体重250〜275gの8匹の雄性ラットをTaconic Laboratory社 (ニューヨーク州ジャーマンタウン)から外頸カニューレ (SU007) と共に入手した。各ラットを、足趾ピンチと結膜反射への反応欠如により定められるように外科麻酔面にイソフルランで麻酔し、手術中は2%のイソフルランを維持した。肋骨下部位を剃毛し、10%プロビドンヨウ素で洗浄し、70%エタノールですすいだ。滅菌ドレープ(かけ布)を用いて無菌手術部位を保持し、滅菌した器具はラット間において熱ビーズ滅菌装置を用いて再滅菌した。最終肋骨に対して0.5cm尾側で該肋骨に平行に2.5cmの皮膚切開を行った。下側の皮下空間(切開部の両側1cm)を、メッシュを収容するために緩めた。皮膚切開と同じ平面に沿って、筋肉層を貫通して腹腔に達する2cmの切開を行い、腹膜を6−0 Prolene縫合糸を用いて連続縫合パターンで閉じた。古典的な「張力閉鎖」を用いた内外斜筋の縫合ではなく、修復材料としてメッシュを使用したリヒテンシュタインの「無張力」修復を実施した。上記Aで調製したメッシュを切開によるヘルニア上に配置し、6−0 Prolene縫合糸を用いて内外斜筋中に縫合した。その後、皮下組織を6〜8本の6−0 Prolene縫合糸を用いた連続パターンで縫合して、ラットがメッシュにさわらないようにした後、6〜8の皮膚縫合を行った。合計外科処置時間は、麻酔誘導と準備に10分間、手術に20分間であった。
侵害受容ピン痛覚試験を用いて皮膚麻酔を評価した(Morrow and Casey, 1983; Kramer et al., 1996; Haynes et al., 2000; Khodorova and Strichartz, 2000)。ラットを片手でつかみ、反対側の手を用いてピンを当てた。痛覚はラットからの皮膚の縮みまたは侵害防衛(即ち、驚愕もしくは逃避試行)反応により示された。メッシュの存在により皮膚縮み反応は妨げられたが、侵害防衛反応は完全にそのまま残った。
A.実施例1に記載したようにして調製したメッシュを、ASTM4032−94に基づいたTyRxファーマ社メッシュ剛性試験手順ATM0410の方法に従って剛性試験に付した。メッシュはガンマ線照射を用いた滅菌前にフォイル袋内に密封した。「ガンマN2」と表示されている場合、袋は密封および照射前に窒素気流中で処理された。メッシュは3回試験した。結果を表5に示すが、エージングが被覆メッシュの柔軟性(可撓性)に影響しないことを示している。
抗生物質を含有しない、即ち、ポリマー被覆だけのチロシンポリアリレート被覆メッシュを、噴霧塗布液への抗生物質の添加を省略した以外は実施例1に記載したようにして調製した。得られた被覆メッシュの光学画像を、メッシュの織られた性質とフィラメントの接触点を明らかに示す倍率で図7の左上のパネルに示す。接触点のクローズアップ(拡大写真)を図7の左下のパネルに示すが、これは被膜がメッシュフィラメントの接触点を不動化していることを実証している。図7の右側のパネルは被覆されたフィラメントの走査式電子顕微鏡写真である。
Claims (28)
- 移植可能な電子装置を封入する構造に形成された、多孔質外科用製織メッシュ又は編成メッシュを含む外科用メッシュ補綴具であって、
前記外科用メッシュは、ポリプロピレン、ポリエステルまたはポリアミドを含む生体適合性材料を含有し、生分解性ポリマー被膜で被覆され、
前記被膜は、(i)前記メッシュにその未被覆の剛性の少なくとも1.1倍の剛性を一時的に付与し、(ii)前記メッシュの気孔率を実質的に変化させずに前記メッシュ上に配置され、(iii)患者の体内で生分解して前記メッシュが実質的にその未被覆の剛性に戻ることができ、(iv)前記被膜が乾燥、硬化または固化した後に前記メッシュを立体形状に保持するのに十分な強度を有し、及び(v)10,000〜200,000ダルトンの分子量を有し、
前記被膜が、前記ポリマー被膜に混和できる抗菌剤、麻酔薬、鎮痛剤、抗炎症剤、抗線維症薬およびロイコトリエン阻害剤よりなる群から選ばれる少なくとも1種の薬剤をさらに含有する、外科用メッシュ補綴具。 - 前記多孔質メッシュは約0.4mm及び約3mmの孔を含む、請求項1に記載の外科用メッシュ補綴具。
- 前記メッシュは再吸収することができる、請求項1に記載の外科用メッシュ補綴具。
- 前記メッシュは再吸収することができない、請求項1に記載の外科用メッシュ補綴具。
- 前記非再吸収性メッシュが編成ポリプロピレンである、請求項4に記載の外科用メッシュ補綴具。
- 前記被膜が前記メッシュの片面または両面に配置されている、請求項1に記載の外科用メッシュ補綴具。
- 前記被膜が実質的に全ての前記メッシュに配置されている、請求項1に記載の外科用メッシュ補綴具。
- 前記被覆がテンプレートで形成されたパターンをなして前記メッシュ上に配置されている、請求項1に記載の外科用メッシュ補綴具。
- 前記生分解性ポリマーが、ポリ乳酸、ポリグリコール酸、ポリ(L−ラクチド)、ポリ(D,L−ラクチド)、ポリグリコール酸、ポリ(L−ラクチド−コ−D,L−ラクチド)、ポリ(L−ラクチド−コ−グリコリド)、ポリ(D,L−ラクチド−コ−グリコリド)、ポリ(グリコリド−コ−トリメチレンカーボネート)、ポリ(D,L−ラクチド−コ−カプロラクトン)、ポリ(グリコリド−コ−カプロラクトン)、ポリオキサエステル、ポリジオキサノン、ポリプロピレンフマレート、ポリ(エチルグルタメート−コ−グルタミン酸)、ポリ(tert−ブチルオキシ−カルボニルメチルグルタメート)、ポリカプロラクトン、ポリカプロラクトン−コ−ブチルアクリレート、ポリヒドロキシブチレート、ポリ(ホスファゼン)、ポリ(リン酸エステル)、ポリ(アミノ酸)、ポリデプシペプチド、無水マレイン酸コポリマー、ポリイミノカーボネート、ポリ[(97.5%ジメチルトリメチレンカーボネート)−コ−(2.5%トリメチレンカーボネート)]、ポリ(オルトエステル)、チロシン誘導ポリアリレート、チロシン誘導ポリカーボネート、チロシン誘導ポリイミノカーボネート、チロシン誘導ポリホスホネート、タンパク質、ならびに上記のいずれかのブレンドよりなる群から選ばれる、請求項1に記載の外科用メッシュ補綴具。
- 前記生分解性ポリマーがチロシン誘導ジフェノールモノマー単位を含む、請求項1に記載の外科用メッシュ補綴具。
- 前記生分解性ポリマーがポリアリレート、ポリカーボネート、ポリイミノカーボネート、またはポリホスホネートである、請求項10に記載の外科用メッシュ補綴具。
- 前記生分解性ポリマーがポリアリレートである、請求項10に記載の外科用メッシュ補綴具。
- 前記ポリアリレートが約1%〜約30%のデスアミノチロシル−チロシン(DT)を含有する、デスアミノチロシル−チロシン(DT)とデスアミノチロシル−チロシルエステル(DTE)スクシネートとのコポリマーである、請求項12に記載の外科用メッシュ補綴具。
- 前記ポリアリレートがデスアミノチロシル−チロシン(DT)とデスアミノチロシル−チロシルエステル(DTエステル)とのランダムコポリマーであって、このコポリマーが約0.001%〜約80%のDTを含有し、前記エステル部分が炭素数18までの分岐または非分岐アルキル、アルキルアリール、またはアルキレンエーテル基でよく、その任意の基が場合によりポリアルキレンオキシドを内部に有していてもよい、請求項12に記載の外科用メッシュ補綴具。
- 前記ポリアリレートが4−ヒドロキシベンゾイル−チロシン(BT)と4−ヒドロキシベンゾイル−チロシルエステル(BTエステル)とのランダムコポリマーであって、このコポリマーが約0.001%〜約80%のBTを含有し、前記エステル部分が炭素数18までの分岐または非分岐アルキル、アルキルアリール、またはアルキレンエーテル基でよく、その任意の基が場合によりポリアルキレンオキシドを内部に有していてもよい、請求項12に記載の外科用メッシュ補綴具。
- 前記生分解性ポリマーが再吸収性ポリマーである、請求項1に記載の外科用メッシュ補綴具。
- 前記メッシュが製織ポリプロピレンを含む、請求項1に記載の外科用メッシュ補綴具。
- 前記メッシュが発泡ポリテトラフルオロエチレン(ePTFE)の裏面層を有する、請求項17に記載の外科用メッシュ補綴具。
- 前記メッシュが移植後1週間の間は患者の体内でその剛性を保持する、請求項1に記載の外科用メッシュ補綴具。
- 前記薬剤が抗菌剤である、請求項1に記載の外科用メッシュ補綴具。
- 前記抗菌剤がリファンピン、ミノサイクリン、銀/クロルヘキシジン、バンコマイシン、セファロスポリン類、ゲンタマイシン、トリクロサンおよびそれらの混合物よりなる群から選ばれる、請求項20に記載の外科用メッシュ補綴具。
- 前記少なくとも1種の薬剤がリファンピンを別の抗菌剤と組合わせて含む、請求項1に記載の外科用メッシュ補綴具。
- 前記別の抗菌剤がミノサイクリンHClである、請求項22に記載の外科用メッシュ補綴具。
- 編成又は製織多孔質外科用メッシュと被膜とを含む、被覆された外科用メッシュ補綴具であって、
前記編成又は製織多孔質外科用メッシュは、移植可能な電子装置を封入する構造に形成され、ポリプロピレンフィラメントから形成されて、前記フィラメントで囲まれた前記多孔質メッシュの孔と前記フィラメントにより形成された交接点を画成し、前記編成外科用メッシュが剛性を有し、
前記被膜が前記外科用メッシュの前記フィラメントと前記交接点に配置され、
前記被膜が、
(i)チロシン誘導ポリアリレートを含み、
(ii)抗菌剤を含み、
(iii)前記メッシュにその未被覆の剛性の少なくとも1.1倍優れた剛性を付与し、
(iv)生理学的溶液中で生分解して前記メッシュが実質的にその未被覆の剛性に戻ることができ、
前記孔が前記被覆された外科用メッシュにおいて約0.4mm〜約3mmのサイズを有する、外科用メッシュ補綴具。 - 前記被膜が生分解して前記メッシュが約3〜4ヶ月内に実質的にその未被覆の剛性に戻ることができる、請求項24に記載の外科用メッシュ補綴具。
- 前記被膜が少なくとも約1週間の間は生理学的溶液中で前記未被覆の剛性の少なくとも1.1倍優れた剛性を保持する、請求項24に記載の外科用メッシュ補綴具。
- 前記抗菌剤は細菌増殖を阻害するための有効量で存在する、請求項24に記載の外科用メッシュ補綴具。
- 前記抗菌剤が前記メッシュのコロニー形成を防止するのに十分な薬剤放出速度を与える量で存在する、請求項24に記載の外科用メッシュ補綴具。
