Description
1. 発明の分野
本出願は人工気道装置に関し、より具体的には、人工陽圧換気法の導入中に高い内圧を有するようにデザインされた声門上気道に関する。
一般的には、例えば急速な肺換気と、患者の喉頭部開口への医療用器具及びチューブの挿入の両方が可能な喉頭マスクなどの声門上気道が、例えば、その全開示を引用をもってここに援用することとするBrain氏の米国特許第4,509,514号及びCook氏の米国特許第6,422,239 号及び第5,937,860 号に解説されてきた。喉頭マスクは一般に、呼吸用チューブ及び膨張可能又は剛性のシールドという二つの主要な部品から成り、これらの装置は患者の喉に挿入され、適正に配置された場合には喉頭部開口を覆う。その後、マスクの前方(患者側)に向かって配置されたリング様構造が膨張することにより、喉頭部開口の周辺にシールが形成される。リングが膨張すると、喉頭の前方、側方、及び後方部分に対して圧力が印加され、装置が定位置に固定される結果、喉頭部開口がマスク顔面の凹面空洞に位置合わせされた状態で配置される。代替的には、シールド構造を、大型の半剛性プラスチック製構造物として形成し、喉の形状に沿わせると共に、それ自身の大きさ及び形状により喉に対して圧力を加えさせることで、利用可能な空間を効果的に充填することにより全体的にシールの役目をさせてもよい。
以下は、本発明のいくつかの局面の基本的な理解を提供するための、本発明の概要である。この概要は、本発明の鍵又は重要な要素を明らかにしたり、あるいは本発明の範囲を定義したりするものとしては意図されていない。この項の唯一の目的は、本発明のいくつかの概念を、後に提示するより詳細な解説への序章として、簡単な形で提供することである。
以下の詳細な説明は一例として描写するものであり、限定的なものではない。ここで解説するのは、とりわけ、陽圧換気中に患者に対して支援吸気が提供されている時に、より大きな圧力を印加するように、従ってより大きなシールが提供されるようにデザインされた、喉頭とのシール用のシールドを含む声門上気道の一実施態様である。具体的には、本声門上気道は、シーリング・リング(401)を含むシールド(201)を有する。前記リング(401)は気道路と気体(又はより一般的には流体)連絡しており、この気道路を通じて人工呼吸空気が患者に提供される。ここで解説される声門上気道は、前記気道の配置が向上するようにシールド(201)の形状及び構成にいくつかの特徴を取り入れているが、これらの特徴は必要ではなく、ここで論じるシーリング・リング(401)及び関連構造は他の形状及び形の気道でも用いることができることは認識されねばならない。
Claims (29)
- 遠位端、近位端、及びその間の長さを有する呼吸チューブであって、中空の内部を有する呼吸チューブと;
後方基部及び前記後方基部を取り囲む部分的に閉じられたシーリング・リングを含むと共に、前記シーリング・リング内の内側容積と、前記後方基部上方の空洞とを規定するシールドと;
を含む声門上気道であって;
ただしこの場合、前記シールドは、前記呼吸チューブの中空の内部が前記空洞と流体連絡するように、前記呼吸チューブの遠位端に取り付けられており;
前記中空の内部及び前記空洞は一緒になって空気路を定義し;そして
前記シーリング・リングの内側容積は前記空気路と流体連絡することができる、
声門上気道。 - 前記呼吸チューブが滑らかに湾曲している、請求項1に記載の気道。
- 前記シールドがブロー成型により形成される、請求項1に記載の気道。
- 前記シールドが射出成型により形成される、請求項1に記載の気道。
- 前記空気路が前記内側容積とポートを介して流体連絡している、請求項1に記載の気道。
- 前記ポートが前記呼吸チューブにある、請求項5に記載の気道。
- 前記ポートが単一の穴を含む、請求項6に記載の気道。
- 前記ポートが複数の穴を含む、請求項6に記載の気道。
- 前記ポートが長寸のスリットを含む、請求項6に記載の気道。
- 前記ポートが、前記空洞内に開口する前記シーリング・リングの一側面に配置される、請求項5に記載の気道。
- 前記ポートが単一の穴を含む、請求項10に記載の気道。
- 前記ポートが複数の穴を含む、請求項10に記載の気道。
- 前記ポートが長寸のスリットを含む、請求項10に記載の気道。
- 前記長寸のスリットが前記空洞を取り囲む、請求項13に記載の気道。
- 前記呼吸チューブの前記近位端に取り外し可能に取り付けられたコネクタを更に含む、請求項1に記載の気道。
- 前記気道が一回の使用後に廃棄されることが意図されている、請求項1に記載の気道。
- 前記気道が再利用を意図されている、請求項1に記載の気道。
- 遠位端、近位端、及びその間の長さを有する呼吸チューブであって、中を通る空気路を規定し、前記遠位端と前記近位端との間の前記長さの範囲内にポートを有する、呼吸チューブと;
後方基部及び前記後方基部を取り囲むシーリング・リングを含むと共に、前記シーリング・リング内の内側容積と、前記後方基部上方の空洞とを規定するシールドと;
を含む声門上気道であり;ただしこの場合、前記シールドは、前記呼吸チューブ中の空気路が前記空洞と流体連絡するように、前記呼吸チューブの遠位端に取り付けられており;そして前記空洞は前記ポートを介して前記シーリング・リングの内側容積と流体連絡する、声門上気道。 - 遠位端、近位端、及びその間の長さを有する呼吸チューブであって、中を通る空気路を規定し、前記遠位端と前記近位端との間の前記長さの範囲内にポートを有する、呼吸チューブと;
後方基部及び前記後方基部を取り囲むシーリング・リングを含むと共に、前記シーリング・リング内の内側容積を規定するシールドと;
を含む声門上気道であり;ただしこの場合、前記シールドは、前記呼吸チューブ中の空気路が前記ポートを介して前記シーリング・リング内の内側容積と流体連絡するように、前記呼吸チューブの遠位端に取り付けられている、声門上気道。 - 遠位端、近位端、及びその間の長さを有する呼吸チューブであって、中を通る中空の内部を規定する呼吸チューブと;
後方基部及び前記後方基部を取り囲むシーリング・リングを含むと共に、前記シーリング・リング内の内側容積と、前記後方基部上方の空洞とを規定するシールドと;
を含む声門上気道であって、但しこの場合、前記シールドは呼吸チューブの遠位端に取り付けられ、そして前記空洞及び中空の内部が一緒になって空気路を規定し、前記シーリング・リングが部分的に閉じられている、声門上気道、及び、
前記近位端に取り付けられた空気路中の圧力を上昇させるようにデザインされた換気装置であって、前記換気装置が前記空気路中の圧力を上昇させるとき、それは前記シーリング・リング内の内側容積内の圧力も変化させる、換気装置、
を含む、人工呼吸システム。 - 前記シーリング・リングの内側容積内の圧力変化により、前記空気路に空気が強制送入されるにつれ、前記シーリング・リングの大きさが変化する、請求項20に記載のシステム。
- 前記シーリング・リングの内側容積内の圧力変化により、前記シーリング・リングの喉に対するシール強度が増す、請求項20に記載のシステム。
- 前記シーリング・リングの内側容積内の圧力変化により、前記シーリング・リングが前記喉に印加する圧力が増す、請求項20に記載のシステム。
- 前記シーリング・リングの前記内側容積を前記空洞又は前記中空の内部の少なくとも一方に相互接続するポートを更に含み、前記ポートにより前記シーリング・リング内の前記変化が可能である、請求項20に記載のシステム。
- 前記ポートが単一の穴を含む、請求項24に記載のシステム。
- 前記ポートが複数の穴を含む、請求項24に記載のシステム。
- 前記ポートが長寸のスリットを含む、請求項24に記載のシステム。
- 前記長寸のスリットが前記空洞を取り囲む、請求項27に記載のシステム。
- 遠位端、近位端、及びその間の長さを有する呼吸チューブであって、中空の内部を有する呼吸チューブと;
後方基部及び前記後方基部を取り囲むシーリング・リングを含むと共に、前記後方基部上方の空洞を規定するシールドと;
前記シーリング・リングの内側容積と、前記呼吸チューブの中空の内部のとの間の流体連絡を可能にする手段であって、前記手段が前記遠位端と前記近位端との間の前記長さの範囲内にある、手段と;
前記中空の内部と前記空洞との間の流体連絡を可能にする分離手段と;
