Description
本出願は、どちらも、Device, a Kit and a Method for Heart Supportという名称を有し、参照により本明細書に組み込まれている、2010年3月25日に出願された米国仮出願第61/317619号及び2010年3月25日に出願されたスウェーデン国特許出願第SE1050282−1号の優先権を主張するものである。
Claims (22)
- 左心室ポンプ作用を永久的に補助することによって患者の心臓の心臓内血液循環を強化するように構成された医療デバイスであって、
冠状静脈洞(CS)、大心臓静脈(GCV)、前心室間静脈(AIV)、又はその側枝血管を含む心静脈血管のような、前記心臓の僧帽弁(MV)に近接した前記心臓の心臓血管内に移植されるように構成された少なくとも1つの第1のアンカーユニットと、
前記第1のアンカーユニットと連絡するように構成され、前記心臓の拍動サイクルに応じて前記左心室ポンプ作用を補助する力を発生させるように作製された力発生ユニットとを備え、
前記第1のアンカーユニットは、移植されると、前記心臓血管の補助された移動、したがって僧帽弁平面内での前記僧帽弁の補助された移動が前記心臓の心尖に至る方向と前記心尖から離れる方向との少なくとも一方の方向に与えられるように、前記力を受け取ることを特徴とするデバイス。 - 前記第1のアンカーユニットが前記血管内に移植され、前記補助された移動は、実質的に前記心臓の前記左心室の長軸に沿って広がり、前記僧帽弁の前記補助された移動は、前記力によって心臓収縮期に前記僧帽弁平面内で前記心臓の心尖の方へ向かうように与えられ、及び/又は前記僧帽弁の前記補助された移動は、前記力によって心臓拡張期に前記僧帽弁平面内で前記心尖から離れるように与えられ、心臓収縮期と心臓拡張期との少なくとも一方の間の少なくとも一部で前記心臓の前記ポンプ作用を補助することを特徴とする、請求項1に記載のデバイス。
- 前記補助された移動は、前記心臓の前記ポンプ作用を補助するように、心臓収縮期に前記心臓の前記心尖の方へ向かい心臓拡張期に前記心尖から離れるように往復運動する前記補助された移動において前記僧帽弁の自然移動を制御可能に補助するように与えられることを特徴とする、請求項2に記載のデバイス。
- 前記移動は、心室内中隔に対する左心室側壁の内方及び外方への自然移動を補助する、左心室の長軸を横切る短軸に沿った前記僧帽弁の制御された移動を含むことを特徴とする、請求項1乃至3の何れか1項に記載のデバイス。
- 前記力発生ユニットは、外部エネルギー源に動作可能に連結され、前記外部エネルギー源からエネルギーを受け取り、かつ前記補助された移動を前記心臓の自然拍動サイクルと共時的に制御可能に与えることを特徴とする、請求項1乃至4の何れか1項に記載のデバイス。
- 前記第1のアンカーユニットが、前記アンカーユニットを前記心臓血管内に固定する拡張可能なステント構造を有することと、
前記第1のアンカーユニットが、少なくとも1つの組織固定部材を有することと、
のうちの少なくとも一方を特徴とする、請求項1乃至5の何れか1項に記載のデバイス。 - 前記力発生ユニットは、前記力を機械的な力として供給する作動ユニットであり、前記第1のアンカーユニットと前記作動ユニットは、前記力を伝達し前記移動を与える連結ユニットを介して連絡していること、又は、
前記力発生ユニットは、前記力を磁気的に誘導される力として供給する磁気ユニットであって、前記第1のアンカーユニットは磁気を有し、前記第1のアンカーユニットと作動ユニットは磁気的に連絡しており、前記力を伝達しかつ前記移動を与えること
を特徴とする、請求項1乃至6の何れか1項に記載のデバイス。 - 前記力発生ユニットは、前記力を磁気的に誘導される力として供給する磁気ユニットであって、前記第1のアンカーユニットは磁気を有し、前記第1のアンカーユニットと作動ユニットは磁気的に連絡しており、前記力を伝達しかつ前記移動を与え、
前記第1のアンカーユニット及び前記力発生ユニットの少なくとも一方は電磁石であり、前記電磁石の少なくとも一方は、前記心臓の拍動サイクルと共時的に極性を変化させるように構成される
ことを特徴とする、請求項7に記載のデバイス。 - 前記心臓血管内に移植されるように構成された少なくとも1つの第2のアンカーユニットを有し、前記第2のアンカーユニットは、移植されるときに、前記第1のアンカーユニットよりも前記CSの入口部の近くに配置されることを特徴とする、請求項1乃至8の何れか1項に記載のデバイス。
- 前記少なくとも1つの第2のアンカーユニット(72)は、組み込まれた電気モータを有し、前記力発生ユニットが前記モータであり、前記デバイスは、前記連絡が可能なように前記モータと前記第1のアンカーとの間に連結ユニット(54)を有し、前記力は前記モータにより供給されることと、
前記少なくとも1つの第2のアンカーユニットは、連結ユニットを前記第1のアンカーから前記第2のアンカーを通して作動ユニットまで案内する案内ユニットを有することと、
のうちの少なくとも一方を特徴とする、請求項9に記載のデバイス。 - 前記第1のアンカーユニットは、移植されるときに、前記GCV又はその延長部内に位置させられ、前記第2のアンカーユニットは、移植されるときに前記CS内に位置させられ、前記デバイスは、前記第1のアンカーユニットと前記第2のアンカーユニットをループ状にかつ機械的に連絡するように連結する細長い伸長ユニットを有し、前記伸長ユニットは、近位において前記第2のアンカーを越えて、前記心臓の拍動サイクルと同期して前記伸長ユニットを回転させるように構成された機械的アクチュエータユニットまで延び、前記デバイスは、
前記ループ状の伸長ユニットが左心房の方へたわみ、前記CS及びGCV並びにそれによって前記MVが前記左心房の方へ移動する、第1の方向への前記伸長ユニットの回転時の心臓拡張期動作位置と、
前記ループ状伸長ユニットがLV心尖の方へたわみ、前記CS及びGCV並びにそれによって前記MVが前記LV心尖の方へ移動する、前記第1の方向とは逆の第2の方向への前記伸長ユニットの回転時の第2の動作位置と、
を含む、前記伸長ユニットの回転時の様々な動作位置を有することを特徴とする、請求項9に記載のデバイス。 - 前記力発生ユニットは、前記心臓内、前記心臓の左心室壁上の静脈系の側枝内部、前記心臓の前記左心室、右心室、右心房、若しくは左心房内、又は前記心臓の左心室外壁上に配置でき、あるいは作動ユニットは、心膜、横隔膜、脊椎、若しくは胸郭の近く、胸膜内、又は皮下のような、前記心臓から離れた位置に配置されることと、
前記少なくとも1つの第1のアンカーユニットは、前記心臓の前記冠状静脈洞(CS)内と前記大心臓静脈(GCV)内との少なくとも一方若しくはその側枝血管内に配置でき、及び/又は前記デバイスは、前記冠状静脈洞(CS)、前記大心臓静脈(GCV)、若しくはその側枝血管内に位置する第2のアンカーユニットを含むことと、
のうちの少なくとも一方を特徴とする、請求項1乃至11の何れか1項に記載のデバイス。 - 遠隔エネルギー源(84)と、制御ユニット(88)と、前記心臓の拍動サイクル活動に関する生理学的パラメータを測定しセンサ信号を生成するセンサとをさらに備え、前記センサ信号は、前記遠隔エネルギー源(84)からのエネルギーによって前記センサ信号に基づいて前記移動を与えるように前記力発生ユニットを制御する前記制御ユニット(88)に供給されることを特徴とする、請求項1乃至12の何れか1項に記載のデバイス。
- 前記遠隔エネルギー源(84)は、回転運動又は直線運動が発生される機構部(90)と、前記機構部から延びる伸長ユニット(54)とを有し、前記機構部が前記力発生ユニットであり、前記運動は、前記機構部の動作時に、前記伸長ユニット(54)を介した前記僧帽弁平面の前記移動のために前記第1のアンカーユニットに伝達されること、又は、
前記遠隔エネルギー源(84)は、a)前記僧帽弁に対して固定された1つ又は複数の電磁アンカーユニット、又はb)前記心臓に若しくは前記心臓内に配置された少なくとも1つの力発生ユニット、に電気エネルギーを供給して、前記僧帽弁平面の前記移動を実現するように前記制御ユニットによって制御されること
を特徴とする、請求項13に記載のデバイス。 - 前記デバイスは非動力デバイスであり、前記力発生ユニットは弾性ユニットであり、前記第1のアンカーユニットは、前記CS及びGCV又はその側枝血管内に配置された遠位アンカーユニット(73)及び近位アンカーユニット(75)を含み、前記弾性ユニットは、前記遠位アンカーユニットと前記近位アンカーユニットを連結するループ(55)であり、前記弾性ユニットは、上方MV平面位置又は下方MV平面位置の一方に、それぞれ下方MV平面位置又は上方MV平面位置の他方に対するばね荷重によって確立される弛緩位置を有し、前記LVの心筋力によって前記ループが前記ばね荷重位置に送られ、かつ前記弾性ユニットは、前記弛緩位置の方へ向かう逆方向における前記LVの前記心筋力を補助することを特徴とする、請求項1に記載のデバイス。
- 前記弾性ユニットは、前記下方MV平面位置に対するばね荷重によって確立される前記上方MV平面位置に弛緩位置を有し、前記LVの前記心筋力によって前記ループが前記下方位置に送られ、かつ前記弾性ユニットは、開いた前記MVを血流に対抗してさらにLAの方向に押し上げることによって前記LVの心臓拡張期充填を補助することによって心臓拡張期に補助するか、あるいは
前記弾性ユニットは、前記上方MV平面位置に対するばね荷重によって確立される前記下方MV平面位置に弛緩位置を有し、前記LVの心弛緩力によって前記ループが前記上方位置に送られ、かつ前記弾性ユニットは、閉じた前記MVをLV心尖の方へ押し下げることによって前記LVの心臓収縮期収縮を補助することによって心臓収縮期に補助することを特徴とする、請求項15に記載のデバイス。 - 再吸収可能な材料が少なくとも部分的に再吸収されたときに前記ばね荷重作用が最初に開始され、それによって、前記デバイスが移植された際に作動が遅延されるように、前記弾性ユニットは、PLLA、ポリビニル、又はポリラクチドのような組み込まれた生体再吸収可能な材料によってロックされていることを特徴とする、請求項15又は16に記載のデバイス。
- 前記デバイスは双安定であり、MV平面の心臓拡張期上方位置及び心臓収縮期下方位置が前記デバイスの平衡状態として与えられることを特徴とする、請求項1乃至17の何れか1項に記載のデバイス。
- 前記デバイスは、前記力発生ユニットを制御して前記補助された移動の設定されたシーケンスを与える制御ユニット(88)を備えることを特徴とする、請求項1乃至18の何れか1項に記載のデバイス。
- 前記制御ユニットは、前記設定されたシーケンスにおける前記補助された移動の頻度と速度と停止時間持続時間とのうちの少なくとも1つを設定するように構成されることを特徴とする、請求項19に記載のデバイス。
- 心臓の左心室ポンプ機能を永久的に強化又は増強するためのキットであって、
請求項1乃至20の何れか1項に記載の移植可能な心臓補助デバイスと、
前記補助デバイスを患者に挿入するのに適しており、ガイドワイヤ、案内カテーテル、及び導入カテーテルを含む導入システムとを備えることを特徴とするキット。 - コンピュータによる処理のためのコンピュータプログラムが実装されたコンピュータ可読媒体であって、前記コンピュータプログラムは、医療デバイスを制御し、左心室ポンプ作用を補助することによって患者の心臓の心臓内血液循環を永久的に強化するためのコードセグメントを備え、前記心臓の心尖に至る方向と前記心尖から離れる方向との少なくとも一方への心臓血管の補助された移動、したがって僧帽弁平面内での前記僧帽弁の補助された移動のために、心臓血管内で前記心臓の僧帽弁に近接しかつ前記僧帽弁に組織連結したインプラントに前記力を加えるために、力発生ユニットを制御して前記心臓の拍動サイクルに応じて力を発生させるためにコードセグメントが与えられることを特徴とするコンピュータ可読媒体。
