Description
本出願は米国予備特許出願第60/649,991号(2005年2月3日出願);および米国予備特許出願第60/735,662号(2005年11月10日出願)に関わり;両出願の内容は本明細書に参照によりその全てが組み入れられる。
本発明は癌を治療する方法に関する。より具体的には、本発明は患者の癌、例えば肺癌、多発性骨髄腫、リンパ腫、および上皮性卵巣癌を治療する方法であって、それを必要とする患者にヒストンデアセチラーゼ阻害剤(例えば、PXD-101またはその類似体)の治療上有効な量、およびある(すなわち、他の)化学治療薬(例えば、デキサメタゾンまたは5-フルオロウラシル)および/または上皮成長因子レセプター(EGFR)阻害剤(例えば、Tarceva(登録商標))の第2の量または用量を投与することを含んでなる上記方法に関する。

多発性骨髄腫は、形質細胞の播種性悪性腫であって、米国では毎年約14,600人の新患が発生している。主に中高年集団が発症するこの稀な血液疾患の病因学はほとんど判っていないが、遺伝素因と環境因子が示唆されている。発病してから、クローン増殖から生じる悪性形質細胞は骨髄に蓄積して異常に高いレベルの免疫グロブリンを産生する。多発性骨髄腫は、初期段階には症候群がないので、初期に診断するのが困難である。とりわけ肋骨または椎骨の圧迫骨折の二次的な骨痛が最も一般的な症候である。
何年もの新しい治療方法の開発研究にも関わらず、リンパ系の癌、またはリンパ腫は全く一般的なものとして残っている。例えば、米国において60,000人超が毎年リンパ腫と診断されてこれには非ホジキンリンパ腫(NHL)の55,000超の症例が含まれ、これらの数字は絶えず増加している。さらに、これらの疾患を患うものの予後は不良であることが多く、リンパ腫患者の生存率は低いままである。明らかにこれらの疾患を治療する新しい方法が必要とされている。
前立腺癌は、米国男性のなかで、死に導く最も一般的な癌である。その発生率は過去20年にわたってかなり増加している。この癌はしばしば最初はホルモン治療に応答性であるが、その後、容易に治療されないホルモン非感受性状態に進行することが多い。しかし、一旦、癌が広がるかまたは前立腺から骨髄などの他の組織に転移すると、この癌は通常、現在利用しうる治療により治療することはできない、というのはこれらの治療はあまりにも毒性が強いかまたは転移に追いつくことができないからである。転移した前立腺癌を含む前立腺癌を治療するための低毒性の新しい方法に対する差し迫った必要性がある。
卵巣癌は米国女性のなかで、癌死の第4位の原因でありかつ組み合せた全ての他の婦人科の悪性疾患を超える死の原因である。
肺癌は米国において癌死亡率の首位にある(「2003年における癌の実情と数字(Cancer Facts and Figures 2003)」, American Cancer Society)。肺癌は、特に潜行性であり、その理由は、初期段階の局所疾患の症候群が非特異的でありかつ喫煙の影響に帰することが多いからである。患者が医療を求める時点には疾患が通常進行していて、完全な外科切除が可能なのは全症例の30%未満でしかなく、かつ全5年の生存率は15%未満でしかない。例えば「肺癌:癌スクリーニングと早期検出(Cancer of the Lung: Cancer Screening and Early Detection)」, Cancer Medicine、第5版、Bastら編、B.C. Decker Inc.、Hamilton、Ontario、Canadaを参照されたい。
ヒストンはクロマチンの主要タンパク質構成要素である。クロマチン構造の調節が遺伝子発現を制御する中心機構であることが明らかになりつつある。一般的パラダイムとして、ヌクレオソームヒストンのアミノ末端尾部中のリシン残基のε-アミノ基のアセチル化は転写活性化と関係があると同時に、脱アセチル化はクロマチンの凝縮および転写抑制と関係がある。ヒストンのアセチル化および脱アセチル化は、ヒストンアセチルトランスフェラーゼ(HAT)およびヒストンデアセチラーゼ(HDAC)の酵素活性により制御される。p53およびGATA-1を含むいくつかの転写因子はまた、HDACに対する基質であることもわかっている。
本発明は、ヒストンデアセチラーゼ(HDAC)阻害剤、例えばPXD-101を化学治療薬および/または上皮成長因子レセプター(EGFR)阻害剤と併用すると、治療上有効な抗癌効果を与えることができるという発見に基づく。さらに、HDAC阻害剤と化学治療薬またはEGFR阻害剤との間の予想しなかった相乗的相互作用がもたらされ、上記併用効果は2つの治療薬をそれぞれ治療用量で投与することからもたらされる相加効果より大きい。
本発明は癌を治療する方法に関する。上記方法は、それを必要とする患者にヒストンデアセチラーゼ(HDAC)阻害剤、例えばPXD-101の第1の量または用量、および(他の)化学治療薬、例えばデキサメタゾンもしくは5-フルオロウラシル、または上皮成長因子レセプター(EGFR)阻害剤、例えばTarceva(登録商標)の第2の量または用量を投与することを含んでなる。第1と第2の量または用量は一緒に治療上有効な量を含む。
一実施形態においては、第2の量または用量はある(すなわち、他の)化学治療薬の第2の量または用量である。
ヒストンデアセチラーゼ(HDAC)はヒストンおよび非ヒストンタンパク質(p53、チューブリン、および様々な転写因子)の可逆的アセチル化に関わる。哺乳動物のHDACは公知の酵母因子とのそれらの類似性に基づいて3クラスに整理されている。クラスI HDAC(HDAC1、2、3および8)は酵母RPD3タンパク質と類似性があり、核内に位置し、転写のコリプレッサー(co-repressor)に関連する複合体中に見出される。クラスII HDAC(HDAC4、5、6、7および9)は酵母HDA1タンパク質に類似し、核と細胞質細胞下の両方に局在する。クラスIII HDACは酵母SIR2タンパク質に関係するNAD依存性酵素の構造的に遠隔なクラスを形成する。

本発明は、上に掲げた化合物の使用だけでなく、かかる化合物の立体異性体およびそれらの混合物の使用を包含することを意図する。
本明細書に開示した活性化合物は、先に記載した通り、それらの製薬上許容される塩の形態で調製することができる。製薬上許容される塩は、親化合物の所望の生物学的活性を保持しかつ望ましくない毒性学的効果を与えない塩である。製薬上許容される塩の例は、Bergeら, 1977, 「製薬上許容される塩(Pharmaceutically Acceptable Salts)」, J. Pharm. Sci., Vol. 66, pp. 1-19に考察されている。
本発明は、上に掲げた化合物の使用だけでなく、本明細書に開示した化合物(例えば、HDAC阻害剤)のプロドラッグの使用を包含することを意図する。いずれかの化合物のプロドラッグは周知の薬理学的技法を用いて作ることができる。
本発明は、上に掲げた化合物の使用だけでなく、かかる化合物の同族体および類似体の使用を包含することを意図する。本明細書の文脈において、同族体は上記化合物と実質的な構造類似性を有する分子でありかつ類似体は、構造類似性に関係なく、実質的な生物学的類似性を有する分子である。
本発明における使用に好適な化学治療薬には、1種以上の他の抗腫瘍物質、例えば、有糸分裂阻害剤、例えばビンブラスチン;アルキル化剤、例えばシスプラチン、カルボプラチン、およびシクロホスファミド;微小管アセンブリーの阻害剤、例えば、パクリタキセルまたは他のタキサンなど;抗代謝物、例えば5-フルオロウラシル、カペシタビン、シトシンアラビノシド、およびヒドロキシウレア、または、例えば、インターカレーション抗生物質、例えば、アドリアマイシンおよびブレオマイシン;免疫賦活薬、例えばトラスツズマブ;DNA合成阻害剤、例えば、ゲムシタビン;酵素、例えばアスパラギナーゼ;トポイソメラーゼ阻害剤、例えばエトポシド;生物学的応答改変因子、例えばインターフェロン;および抗ホルモン、例えば、抗エストロゲン、例えばタモキシフェンまたは、例えば、抗アンドロゲン、例えば(4'-シアノ-3-(4-フルオロフェニルスルホニル)-2-ヒドロキシ-2-メチル-3'-(トリフルオロメチル)-プロピオンアニリド、または他の治療薬、ならびに例えば、DeVita, V. T., Jr., Hellmann, S., Rosenberg, S. A.; in: Cancer: Principles & Practice of Oncology, 第5版, Lippincott-Raven Publishers (1997)に記載の原理から選択される物質が含まれる。
本明細書で使用される用語「EGFR阻害剤」は、生来の上皮成長因子レセプター(EGFR)の生物学的機能を阻害できる分子を意味する。従って、用語「阻害剤」はEGFRの生物学的役割の文脈において定義される。ここで好ましい阻害剤はEGFRと特異的に相互作用する(例えば、結合する)一方、EGFRシグナル伝達経路の他のメンバーと相互作用することによりEGFR生物学的活性を阻害する分子も明確にこの定義内に含まれる。EGFR阻害剤により阻害される好ましいEGFR生物学的活性は腫瘍の発生、増殖、または伝播と関連している。EGFR阻害剤には、限定されるものでないが、ペプチド、非ペプチド小分子、抗体、抗体フラグメント、アンチセンス分子、およびオリゴヌクレオチド・デコイ(decoy)が含まれる。
いくつかの好ましい化学治療薬およびEGFR阻害剤の例には、シスプラチン、5-フルオロウラシル、オキサリプラチン、トポテカン、ゲムシタビン、ドセタキセル、ドキソルビシン、タモキシフェン、デキサメタゾン、5-アザシチジン、クロランブシル、フルダラビン、Tarceva(登録商標)、Alimta(登録商標)、メルファランが含まれる。
本発明の治療の方法は、それを必要とする患者に、第1の手順において、ヒストンデアセチラーゼ(HDAC)阻害剤、例えばPXD-101の第1の量または用量を、そして、第2の手順において、(他の)化学治療薬、例えばデキサメタゾンもしくは5-フルオロウラシル、または上皮成長因子レセプター(EGFR)阻害剤、例えばTarceva(登録商標)の第2の量または用量を投与することを含んでなる。第1および第2の量または用量は、共に、治療上有効な量を含む。いくつかの実施形態においては、上記併用療法は相乗効果をもたらす。
本発明のヒストンデアセチラーゼ(HDAC)阻害剤は経口剤形、例えば、錠剤、カプセル(それぞれ徐放性または持続放出製剤を含む)、丸薬、粉末、顆粒、エリキシル、チンキ、懸濁剤、シロップ、および乳剤で投与することができる。
HDAC阻害剤を用いる投与体制は、型、種、年齢、体重、性別および治療する癌の型;治療する癌の重症度(すなわち、段階);投与経路;患者の腎および肝機能;ならびに使用する特定の化合物またはその塩を含む、様々な因子によって選択することができる。当技術分野の医師または獣医は、治療する、例えば、疾患の進行を予防し、阻害し(全てまたは部分的に)または停止するために必要な薬物の有効量を、容易に決定しかつ処方することができる。
ヒストンデアセチラーゼ(HDAC)阻害剤の経口投与量は、所望の癌(例えば、ヒトにおける)を治療するために用いる場合、1日当たり約2mg〜約2000mg、例えば1日当たり約20mg〜約2000mg、例えば1日当たり約200mg〜約2000mgの範囲でありうる。例えば、経口投与量は1日当たり約2、約20、約200、約400、約800、約1200、約1600または約2000mgでありうる。1日当たりの全量は、単一用量で投与してもよくまたは1日当たり2、3または4回などの多回投薬で投与してもよいと解釈される。
治療手順はいずれかの順序で逐次的に、同時に、またはそれらの組合わせで行うことができる。例えば、第1治療手順、HDAC阻害剤の投与は、第2治療手順、化学治療薬またはEGFR阻害剤の投与前に(例えば、7、14、21日まで前に)、第2治療手順後に(例えば、7、14、21日まで後に)、第2治療手順と同時に、またはそれらの組合わせで行うことができる。一実施形態においては、第1治療手順の少なくとも一部は第2治療手順の一部と同時に行われる。
本発明の一態様はキットまたはパーツのキットであって、
(a)好ましくは製薬上許容される製剤の一成分としての、そして好ましくは好適な容器に入れておよび/または好適な包装を施して提供される、ヒストンデアセチラーゼ(HDAC)阻害剤、例えばPXD-101;および
(b)好ましくは製薬上許容される製剤の一成分としての、そして好ましくは好適な容器に入れておよび/または好適な包装を施して提供される、他の化学治療薬、例えばデキサメタゾンもしくは5-フルオロウラシル、または上皮成長因子レセプター(EGFR)阻害剤、例えばTarceva(登録商標)を含み、かつ
癌を治療する方法に使用するのに好適である、上記キットまたはパーツのキットに関する。
標準化学治療薬と併用したHDAC阻害剤の効果をin vitroで試験し、併用化学療法におけるHDAC阻害剤(すなわち、PXD-101)の臨床使用の可能性を確認した。例えば、以下の添付データ表を参照されたい。
細胞を培養し、PXD-101単独でまたは化学治療薬と組合わせて曝し、そしてある時間インキュベートし、次いで生存細胞の数を、次に記載したBoehringer Mannheim製の細胞増殖試薬WST-1(Cat. No. 1 644 807)を用いて評価した。
%活性 = {(Sc - B)/(S0 - B)} x 100
[式中、Scは試験化合物の存在のもとで測定したシグナルであり、S0は試験化合物の非存在のもとで測定したシグナルであり、そしてBは培地だけを含有するブランクウエルで測定したバックグラウンドシグナルである]
を用いて計算した。IC50は50%活性を達成する濃度に対応する。IC50値は、ソフトウエアパッケージPrism(登録商標) 3.0(GraphPad Software Inc., San Diego, California, USA)を用いて計算し、最高値を100および最低値を0に設定した。
PXD-101+5-FU
PXD-101と5-FUの併用の抗増殖効果を様々な細胞株(例えば、HT-116、A2780)および様々な条件、例えば、化学治療薬5-FUの投与と同時、24時間前、または24時間後のもとで試験した。これらの実験結果を図2および添付データ表3に総括したが、これらはWST-1抗-増殖性アッセイにおけるPXD-101と5-FUの相乗作用を示し、CalcuSyn(登録商標)により計算したものである。
マウスP388モデルにおいてPXD-101とフルダラビンの相乗効果が存在する:薬物の併用により治療したマウスの生存率は、それぞれの薬物単独で治療したマウスの生存率より高い(添付データ表10を参照)。
L-フェニルアラニンマスタード(PAM、メルファラン)は、いくつかの骨髄腫細胞株においてPXD-101と相乗作用を示す(添付データ表1を参照)。さらに、相乗作用は、いくつかの腫瘍細胞株において、PXD-101と他の化学治療薬、特に、シスプラチン、5-FU、トポテカン、ゲムシタビン、ドセタキセル、ドキソルビシン、タモキシフェン、デキサメタゾン、および 5-アザシチジンとの間にみられる(添付データ表2を参照)。最後に、培養リンパ腫細胞の細胞増殖実験において、PXD-101とクロランブシルの相乗作用およびPXD-101とフルダラビンの相乗作用が見られる(添付データ表4を参照)。
化学治療薬デキサメタゾンをJJN3、LP-1、RPMI-8226、U266腫瘍細胞に加えると同時に(例えば、48時間、共インキュベーション)、加えた後24時間に、または加える前24時間に、PXD-101を加えることの抗増殖効果も上記の方法により試験した。
骨髄腫細胞株(例えば、JJN3、LP-1、RPMI-8226、U266)をPXD-101およびドキソルビシンを用いて処理した。上記のように、骨髄腫細胞株に対するこれらの化合物の併用効果を、アイソボログラムをプロットして併用指数を計算することにより確認した。
骨髄腫細胞株(例えば、JJN3、LP-1、RPMI-8226、U266)を、PXD-101およびビンクリスチンを用いて処理した。上記のように、骨髄腫細胞株に対するこれらの化合物の併用の効果を、アイソボログラムをプロットして併用指数を計算することにより決定した。この併用について、PXD-101およびビンクリスチンを共インキュベートすると、ある範囲の濃度にわたって、相加ないし相乗効果が観察された(添付データ表1を参照)。
葉酸代謝拮抗薬ペメトレキセド(Alimta(登録商標))は肺癌用に市販されている。培養細胞を用いる増殖実験において、PXD-101をAlimta(登録商標)と併用すると、明らかな相乗効果がある(添付データ表12および図6を参照)。
本実施例は、PXD-101を標準化学治療薬と併用することの効果を報じている。
クローン原性アッセイ
クローン原性アッセイを、本質的に(Jensenら、1993)に記載の通り実施して、PXD-101に関わる併用、特にPXD-101と5-FUに関わる併用の抗増殖効果を確認した。
In vivo研究
HCT116大腸癌皮下異種移植モデル(nu/nuマウス)でPXD-101の腫瘍増殖阻害効果を研究するために、2通りのPXD-101用量(60および100mg/kg)、q1d x 5/週、2週間を計画した。5-FUと併用したPXD-101の化学感受効果も研究した。5-FU用量は15mg/kgに設定した。
P388マウス白血病細胞をB6D1F1雌性マウスに腹腔内(IP)注射した。PXD-101および/または化学治療薬を第3日に、次いで毎日、所要の連続日数の間、IP投与した。Kaplan-Meierの生存率分布グラフをそれぞれの併用に対して示した。
EGFR阻害剤とHDAC阻害剤に関わる増殖阻害アッセイ
本研究の目的は、PXD-101単独療法またはTarceva(登録商標)との併用療法がA431細胞(ヒト類表皮癌)またはCalu-3細胞(ヒト非小細胞肺癌)の増殖に効果があるかどうかを確認することであった。
細胞を96-ウエルプレート中に3,000細胞/ウエルをまき、PXD-101またはTarceva(登録商標)(EGFR-キナーゼ阻害剤)を単独または組合わせて用いて、示した濃度で72時間処理し、そして増殖/生存率を上記CellTiter-Gloアッセイを介して評価した。
PXD-101+Tarceva(登録商標)の併用は、単独で使用した薬物より大きい増殖阻害を引き起こす。例えば、図5および添付データ表11を参照されたい。この効果は相乗的である。
Tarceva(登録商標)と併用したPXD-101は、非小細胞性肺癌およびおそらく他の型の癌の治療に有益でありうる。
ウェスタンブロット
本研究の目的は、PXD-101処理がEGFRタンパク質レベルに影響を与えるかどうかを確認することであった。
A431細胞(ヒト類表皮癌)を6-ウエルプレートに約80%集密度でまき、PXD-101を用いて、示した濃度で24時間処理し、次いで全細胞溶菌液を収穫し、そして抗-EGFR抗体を用いてウェスタンブロットにより分析した。ブロットを抗アクチン抗体を用いて再プローブしてレーン負荷を調節した。
PXD-101処理は明らかにA431細胞中のEGFRタンパク質のレベルを低下させる。
A431細胞は野生型EGFRを過剰発現し、EGFR経路を阻害する試薬による増殖阻害に対して感受性があることは公知である。PXD-101により誘発されるEGFRタンパク質の低下は、単独で使用したPXD-101がこれらの細胞に対して増殖阻害性である1つの機構でありうる。さらに、PXD-101をTarceva(登録商標)と併用したときに、PXD-101が介在するEGFRタンパク質の低下は、観察された相加/相乗増殖の阻害機構の理論的根拠を提供しうる。

骨髄腫におけるPXD101のin vitro併用研究の総括。括弧内の数字は、各実験で用いたスケジュールを示す(以下参照)。表の結果は「標準」併用プロトコルを用いて得た結果である。

- 癌が肺癌である、請求項1または2に記載の使用。
- 癌がヒト非小細胞肺癌である、請求項1または2に記載の使用。
- 癌が多発性骨髄腫である、請求項1または2に記載の使用。
- 癌がリンパ腫である、請求項1または2に記載の使用。
- 癌が上皮性卵巣癌である、請求項1または2に記載の使用。
- 癌が白血病、リンパ腫、皮膚T細胞リンパ腫(CTCL)、非皮膚末梢T細胞リンパ腫、ヒトT細胞リンパ向性ウイルス(HTLV)に関連するリンパ腫、成人T細胞白血病/リンパ腫(ATLL)、急性リンパ性白血病、急性非リンパ指向性白血病、慢性リンパ性白血病、慢性骨髄性白血病、ホジキン病、非ホジキンリンパ腫、または多発性骨髄腫である、請求項1または2に記載の使用。
- 癌が脳腫瘍、神経芽細胞腫、網膜芽細胞腫、ウイルムス腫瘍、骨腫瘍、または軟組織肉腫である、請求項1または2に記載の使用。
- 癌が頭部および頸部癌、口腔癌、喉頭癌、食道癌、尿生殖器癌、前立腺癌、膀胱癌、腎癌、子宮癌、卵巣癌、精巣癌、直腸癌、大腸癌、肺癌、乳癌、膵臓癌、黒色腫もしくはその他の皮膚癌、胃癌、脳癌、肝臓癌または甲状腺癌である、請求項1または2に記載の使用。
