Description
本発明の一つの一般的な観点では、ガイド装置は、大腿骨面に接触するよう構成された遠位先端を有するガイドアームと、このガイドアームに接続され、かつガイドアームの遠位先端が大腿骨面に接触するときに、脛骨面に接触するよう構成された遠位先端を有するドリルスリーブとからなる。
ガイド装置は、1又はそれ以上の次の特徴を含むと好都合である。すなわち、ガイドアーム及びドリルスリーブを接続し、それによってガイドアームとドリルスリーブとをそれらの接続部を介して接続できるハンドルである。ガイドアームをハンドルの一方端に配置するのが好ましく、ドリルスリーブをハンドルの他方端に配置してもよい。
ガイドアームの方向をドリルスリーブに対して調整できると好都合である。ガイドアームはドリルスリーブに対するガイドアームの方向を示すマーキングを有してもよい。
ガイドアームの遠位先端は、大腿骨面、例えば大腿骨切痕の後部皮質に接触するよう構成されたフックを含むと好都合である。ガイドアームは一点からのオフセットを与えることができ、その点で遠位先端が大腿骨面(例えば、大腿骨切痕の後部皮質)をドリルスリーブの長手方向軸(すなわち、脛骨トンネルの軸)に接触させる。
ガイドアームは、大腿骨面に接触するよう構成されたフックを有する第1遠位アーム部分と、脛骨プラトーに接触するよう構成されたスパイクを有する第2遠位アーム部分とを含むと好都合である。ガイドアームが2つの遠位アーム部分を含むときは、ガイド装置を調整して種々の大腿骨オフセットと大腿骨切痕高さを補正できる。
図1は、ACLの再建において脛骨トンネルを位置決めをするためのガイド装置100を示す。簡単に言えば、ガイド装置100は、詳細は後述するが、ハンドル105と、ハンドル105の一方端115に配置されたガイドアーム110と、ハンドル105の他端125に配置されたドリルスリーブ(又は“リベット”)120とを含む。ガイドアーム110の遠位先端130は大腿骨面(例えば、大腿骨切痕の後部皮質)に接触するよう形成され、ドリルスリーブ120の遠位先端140は、ガイドアーム110の遠位先端130が大腿骨面に接触するときに、(例えば、脛骨の前面)に接触するよう形成される。ドリルスリーブ120は、ガイドワイヤを脛骨トンネルを位置決めさせる脛骨面内に向けるよう形成される。

例えば、ドリルスリーブ120に対するガイドアーム110の方向は外科医が膝蓋腱移植を使うか、半腱様腱移植を使うかによって変えてもよい。更に、ドリルスリーブ120に対するガイドアーム110の方向を調整することによって、外科医は脛骨トンネルの長さを変え、移植片の長さを補正してもよい。
の遠位先端に大腿骨皮質フック650を有する。アイソメトリ縫合糸655はスパイク645とフック650にプーリ・機構660を使いながら接続される。アイソメトリ縫合糸655は、膝を延ばしている間にスパイク645とフック650との間の距離に変化があれば示す。アイソメトリは、縫合糸の移動がないときに存在する。
ACL再建において脛骨トンネルを位置出しする新規で改良された装置及び技術を記述した。当業者が、発明の趣旨から逸脱することなしに、ここで記述された特定の実施態様の多数の利用及び変形並びに展開を行なうことができることは明らかである。
Claims (22)
- 脛骨トンネルを位置決めするガイド装置であって、そのガイド装置が、
大腿骨面に接触するよう構成された遠位先端を有し、かつ脛骨プラトーに接触するよう構成されたスパイクを備えたガイドアームと、
ガイドアームに接続され、ガイドアームの遠位先端が大腿骨面に接触するときに、脛骨面に接触するよう構成された遠位先端を有するドリルスリーブと
からなり、ガイドアームの遠位先端が大腿骨切痕の後部皮質に接触するフックからなることを特徴とするガイド装置。 - ハンドルを更に備え、このハンドルにガイドアームが接続され、かつそのハンドルにドリルスリーブが接続され、更にガイドアームとドリルスリーブとがハンドルとの接続によって接続される請求項1によるガイド装置。
- ガイドアームがハンドルの一方端に配置され、ドリルスリーブがハンドルの他方端に配置されてなる請求項2によるガイド装置。
- ガイドアームの方向をドリルスリーブに対して調整できる請求項1〜3のいずれか一つによるガイド装置。
- ガイドアームが、ドリルスリーブに対するガイドアームの方向を示すマーキングを有する請求項1〜4のいずれか一つによるガイド装置。
- ドリルスリーブが、ガイドワイヤを脛骨面内に向け、脛骨トンネルを配置するよう構成された請求項1〜5のいずれか一つによるガイド装置。
- ドリルスリーブが脛骨トンネルの長さを測定するためのマーキングを有する請求項1〜6のいずれか一つによるガイド装置。
- ドリルスリーブの遠位先端が、歯部によって取り巻かれた傾斜開口からなる請求項1〜7のいずれか一つによるガイド装置。
- ガイドアームの遠位先端が大腿骨切痕の後部皮質に接触するよう構成された請求項1〜8のいずれか一つによるガイド装置。
- ガイドアームが、遠位先端が大腿骨面に接触する点からドリルスリーブの長手方向軸へのオフセットを与えるよう構成された請求項1〜9のいずれか一つによるガイド装置。
- ガイドアームが、大腿骨面に接触すべく構成されたフックを有する第1遠位アーム部分と、脛骨プラトーに接触すべく構成されたスパイクを有する第2遠位アーム部分とを備えた請求項1〜10のいずれか一つによるガイド装置。
- 第1遠位アーム部分が第2遠位アーム部分を摺動すべく構成された請求項11によるガイド装置。
- 第1遠位アーム部分がスプリングによって第2遠位アーム部分に取り付けられた請求項11によるガイド装置。
- 第1遠位アーム部分がリンク機構によって第2遠位アーム部分に取り付けられた請求項11によるガイド装置。
- 第1遠位アーム部分が枢支点で第2遠位アーム部分に取り付けられた請求項11によるガイド装置。
- アイソメトリを示すための装置を更に備えた請求項1〜15のいずれか一つによるガイド装置。
- 装置がアイソメトリ・リンク機構を備えた請求項16によるガイド装置。
- アイソメトリ・リンク機構を延出する手段を更に備えた請求項17によるガイド装置。
- 延出手段が液圧手段からなる請求項18によるガイド装置。
- 装置がアイソメトリ機構からなる請求項16によるガイド装置。
