Description
ここで、図面を参照すると、図中、同様の参照符号が同一又は類似の要素を示すために使用されており、図2は、患者の選択された組織中への医師による薬剤の制御された注入のための、全体が20で表わされる装置を示している。この装置は、少なくとも1つの流量で薬剤を供給するための駆動ユニット22と、医師によって保持されるためのハンドピースハンドル24とを備え、ハンドピースは、柔軟なチューブ又はホース26によって駆動ユニット22に接続されることにより、駆動ユニットに係合される既知の歯科用麻酔薬カートリッジ28から薬剤を受ける。
本発明の単歯麻酔歯根膜内即ちPDL注射技術は、主として、歯根膜の場所を見つけ且つ装置の適切な機能を確保するためにリアルタイム動的圧力検出フィードバックを使用するという点で従来技術の処置とはかなり異なっている。また、本発明の注射技術は、麻酔の送出のために正確な流量(PDL注射においては例えば0.005ml/秒)及び制御された「適度な」所望の最小絶対圧又は閾値絶対圧(例えば、PDL注射にとって好ましい250〜450psi、或いは更に200〜500psiの範囲に至るまで)を使用する。
駆動ユニット22は携帯可能であってほぼ小型ケーブルモデムのサイズを有する。ハンドピースハンドル24は1回のみ使用の使い捨て可能なハンドピースアセンブリの一部である。駆動ユニット22は標準的なAC電気接続によって給電される。例えば図示のユニットは、ユニット22の背部に差し込まれるようになっているユニットプラグ38と例えば米国で使用されるAC壁コンセントのための壁プラグ40とを備える電源コード36を有する。
a.STAモード−単一速度モード(0.005ml/秒流量)、
b.通常モード−二種速度モード(0.005ml/秒及び0.03ml/秒)、
c.ターボモード−三種速度モード(0.005ml/秒、0.03ml/秒、及び、0.06ml/秒)、を有する。
注射を行うため、被検者又は患者は仰向けの姿勢で着座させられる。医師は、所望の結果に適する麻酔液を選択すると共にハンドピースアセンブリのカートリッジホルダ34内へカートリッジ28を配置する。患者は、処置の全体にわたって自分達がシステムから可聴音を聞くことが知らされるべきである。
本発明のユニット22を通常の動作モードで使用するために選択ボタン68が1回押される。これにより、通常モードインジケータLED即ちランプ70が点灯する。
ターボモードは選択ボタンを2回押すことによって開始される。ターボモードインジケータランプ72が点灯すると共に、フットペダル42は3つのプランジャ速度及び3つの対応する流量例えば0.005ml/秒、0.03ml/秒、及び0.06ml/秒に対応する3つの押し下げ位置を有する。ターボモードは、患者のためにもはや必要とされないカートリッジ内の薬剤の残留物を急速に排出するために使用され得る。
スピーカ23からの可聴フィードバック信号の音量を増減するために、機能上端及び機能下端を有する可聴音量制御ボタン74が使用される。
Claims (4)
- 患者の歯根膜(12)内組織中への医師による歯科用麻酔薬の投与のための制御された注射のための装置(20)であって、
少なくとも1つの流量で前記歯科用麻酔薬を供給するための駆動ユニット(22)と、
前記医師により保持されるようになっているハンドピースであって、前記歯科用麻酔薬を受けるために柔軟なチューブ(26)によって前記駆動ユニット(22)に接続され、前記患者の前記歯根膜(12)内組織中へと挿入される注射針(30)を保持し、当該注射針(30)は前記患者の組織中における先端(10)の配置に応じて変化する前記注射針(30)の先端(10)における瞬間絶対圧力で前記歯根膜(12)内組織へと前記歯科用麻酔薬を送るための先端(10)を有し、200〜500psiの範囲で設定される最小閾値圧力と前記歯根膜(12)内組織へ前記歯科用麻酔薬を投与するために望まれる絶対圧力とが存在するハンドピース(24)と、
前記駆動ユニット(22)に接続されると共に、前記歯根膜(12)内組織の特性と前記注射針先端(10)の配置とに応じてリアルタイムで変化する前記注射針先端(10)における前記瞬間絶対圧力で前記歯根膜(12)内組織に前記歯科用麻酔薬を送るために前記患者の前記歯根膜(12)内組織の中へと挿入される前記注射針先端(10)を有する前記ハンドピース(24)を前記医師が保持している間は前記歯科用麻酔薬を選択された流量で前記注射針(30)に供給するよう前記駆動ユニット(22)を動作させるようにプログラムされたコンピュータ(102,104,106)と
前記コンピュータ(102,104,106)に接続され、前記瞬間絶対圧力をリアルタイムで検出するための圧力センサ又は計算するための計算ユニット(90)と、
前記コンピュータ(102,104,106)に接続され、可聴信号を供給するスピーカ(23)とを備え、
前記コンピュータ(102,104,106)は、前記瞬間絶対圧力が前記最小閾値圧力へと増大していることを示す可聴の連続信号を前記医師に供給するように前記スピーカ(23)を作動させると共に、少なくとも前記最小閾値圧力に達するまでは前記医師による前記注射針先端(10)の配置の動きを案内するようにプログラムされ、
また、前記コンピュータ(102,104,106)は、前記注射針先端(10)を前記歯根膜(12)内組織中へと前記医師が動かしたのでこの信号が供給された後は前記選択された流量で選択された量の前記歯科用麻酔薬を前記歯根膜(12)内組織に供給するために前記医師は前記注射針先端(10)を前記歯根膜(12)内組織中に留めることを意味する信号であって前記瞬間絶対圧力が前記最小閾値圧力に達したことを示す可聴の信号を前記医師に供給するように前記スピーカ(23)を作動させるようにプログラムされ、
さらに、前記コンピュータ(102,104,106)は、前記瞬間絶対圧力が前記最小閾値圧力に達する前に低下若しくは変動したときは可聴信号を生成することによって前記医師に通知するために前記スピーカ(23)を作動させるようにもプログラムされることを特徴とする装置。 - 前記コンピュータ(102,104,106)に接続された視覚ディスプレイ(32)を更に備え、前記コンピュータ(102,104,106)は、前記瞬間絶対圧力が前記最小閾値圧力に達する前の前記瞬間絶対圧力における異なるリアルタイム値に対応する複数の信号を含む視覚に訴える信号、及び、前記瞬間絶対圧力が前記最小閾値圧力に達したことを示す少なくとも1つの視覚に訴える信号を前記医師に供給するように前記視覚ディスプレイ(32)を作動させるようにプログラムされることを特徴とする請求項1に記載の装置。
- 前記コンピュータ(102,104,106)は、過圧状態が生じたことを前記医師に知らせるために前記瞬間絶対圧力が選択された過圧値を超える場合に更なる知覚可能信号を前記医師に供給するように前記スピーカ(23)を作動させるようにプログラムされることを特徴とする請求項1に記載の装置。
- 前記最小閾値圧力が200〜500psiの範囲であると共に前記選択された流量が少なくとも0.002ml/秒である請求項1に記載の装置。
