Description
i)アトルバスタチン封入PLGA
PLGA(和光純薬、PLGA7520、乳酸:グリコール酸=75:25、平均重量分子量20,000)1.2gとアトルバスタチン120mgをアセトン40mL、エタノール20mLの混液に溶解しポリマー溶液とした。これを40℃、400rpmで攪拌した0.5wt%PVA溶液120mL中に一定速度(4mL/min)で滴下しアトルバスタチン封入PLGAナノ粒子懸濁液を得た。続いて減圧下40℃、100rpmで攪拌を続けながら、有機溶媒(アセトン、エタノール)を留去した。約2時間溶媒留去後、懸濁液をフィルターろ過(目開き32μm)し、ろ液を一晩凍結乾燥し、アトルバスタチン封入PLGAナノ粒子を得た。アトルバスタチンは、ナノ粒子中6.3%(w/v)封入された。アトルバスタチンに代えて蛍光マーカー(FITC)を封入したPLGAナノ粒子を、同様に製造した。蛍光マーカーは、ナノ粒子中5%(w/v)封入された。
peg−PLGA(Absorbable Polymers International製;乳酸/グリコール酸=75/25、分子量22,900 そのうちpeg分子量6,000)1.2gとアトルバスタチン120mgをアセトン40mL、エタノール20mLの混液に溶解しポリマー溶液とした。これを40℃、400rpmで攪拌した0.04wt%キトサン(片倉チッカリン、モイスコートPX、塩化N−〔2−ヒドロキシ−3−(トリメチルアンモニオ)プロピル〕キトサン)水溶液120mL中に一定速度(4mL/min)で滴下しアトルバスタチン封入peg−PLGAナノ粒子懸濁液を得た。続いて減圧下40℃、100rpmで攪拌を続けながら、有機溶媒(アセトン、エタノール)を留去した。約2時間半溶媒留去後、懸濁液をフィルターろ過(目開き32μm)し、アトルバスタチン封入PEG/CS−PLGAナノ粒子を得た。アトルバスタチンは、ナノ粒子中8.8%(w/v)封入された。アトルバスタチンに代えて蛍光マーカー(FITC)を封入したPEG/CS−PLGAナノ粒子を、同様に製造した。蛍光マーカーは、ナノ粒子中2.4%(w/v)封入された。
研究プロトコルは、九州大学医学部、動物実験倫理委員会により検討され、承認を得、実験は、アメリカ生理学会のガイドラインに従って行われた。
i)アトルバスタチン封入ナノ粒子の評価
九州大学動物実験研究室で育てられた雄のC57BL/6J野生タイプのマウスを実験に使用した。塩酸ケタミンとキシラジン塩酸塩の腹腔内注射による麻酔の後、動物の左大腿動脈を結紮し切除して片側のみの後脚虚血を作成した。虚血誘導性新血管形成におけるスタチンの役割を調べるため、一群のAto−マウス(Atoグループ)にアトルバスタチン[0.01のmg/100μl(0.5mg/kg)]を筋内注射した。PLGA−Ato−マウス(PLGA−Atoグループ)の一群と、PEG/CS−PLGA−Ato−マウス(PEG/CS−Atoグループ)の一群に、それぞれアトルバスタチン封入PLGAナノ粒子(0.16mg/100μl)と、アトルバスタチン封入PEG/CS−PLGAナノ粒子(0.12mg/100μl、各0.01mgのアトルバスタチンを含む)を筋内注射した。対照群として、PLGA−FITC−マウス(FITCグループ)の一群にFITC封入PLGAナノ粒子(0.16mg/100μl)を筋内注射し、PEG/CS−PLGA−FITC−マウス(PEG/CS−FITCグループ)の一群に、FITC封入PLGAナノ粒子(0.12mg/100μl)を筋内注射し、そしてもう一群のマウス(NTグループ)は、処置を施さなかった。マウスは、後脚虚血誘導直後、左大腿動脈と脛骨筋肉にアトルバスタチン、またはナノ粒子を27−ゲージ針で注射された。
ナノ粒子の投与後の動態を調べるため、FITCグループおよびPEG/CS−FITCグループマウスの筋組織を、虚血作成から7日後に採取し、光学顕微鏡および蛍光顕微鏡でそれぞれ観察を行った。結果を図1に示す。いずれのグループでも虚血作成7日後に蛍光が観察されることから、筋組織内にナノ粒子が残存している。
肢血流の測定を、0日目(手術の直後)、3日目、7日目と14日目に、ドップラーレーザー局所灌流イメージング(LDPI)アナライザー(モアー・インストラメント社製)で行った。14日目の結果を図2に示す。また、LDPIインデックスを、標準の手足のものと虚血性のものとの比率として表した(図3)。Atoグループ、PLGAのみを投与したグループ、NTグループでは血流の改善が見られなかったが、PLGA−AtoグループとPEG/CS−Atoグループでは虚血側の血流改善が認められる。
壊死領域を、HE染色した筋組織を観察することによって各グループ毎に比較した。結果を図4に示す。PLGA−AtoグループおよびPEG/CS−AtoグループではNTグループやAtoグループと比較して壊死領域が小さい。
下肢虚血から14日後の血清学的評価を、下記表に示す。血清中の全コレステロール、トリグリセリド、ミオグロビン、CK、BUN、Creは各群間で差がなかった。アトルバスタチンおよびアトルバスタチン封入ナノ粒子の筋注により、横紋筋融解を示唆する所見はみられなかった。
アトルバスタチン封入ナノ粒子と同様に、マウスを麻酔し、虚血を作成した後、一群のPit−マウス(Pitグループ)にピタバスタチン[0.01のmg/100μl(0.5mg/kg)]を筋内注射した。PLGA−Pit−マウス(PLGA−Pitグループ)の一群と、PEG/CS−PLGA−Pit−マウス(PEG/CS−Pitグループ)の一群に、それぞれピタバスタチン封入PLGAナノ粒子(0.18mg/100μl)と、アトルバスタチン封入PEG/CS−PLGAナノ粒子(0.44mg/100μl、各0.01mgのアトルバスタチンを含む)を筋内注射した。対照群として、PLGA−マウス(PLGAグループ)の一群にPLGAナノ粒子(0.18mg/100μl)を筋内注射し、そしてもう一群のマウス(NTグループ)は、処置を施さなかった。

毛管密度を、抗マウス血小板内皮細胞接着分子(PECAM)−1抗体(サンタクルス)による、免疫組織化学的染色によって測定した。結果を図11および12に示す。PLGA−PitグループではNTグループやPitグループと比較して内皮細胞(CD31陽性細胞)の増加が見られる。
日本ウサギ(2.7〜3.2kg)の片足の大腿動脈を結紮除去することによって下肢虚血を作成した。作成7日後にPLGA−ピタバスタチン投与群、PLGA−FITC投与群、ピタバスタチン投与群とPBS投与群の4群にランダムに分け、それぞれの治療物質を虚血筋肉内に5ml(ピタバスタチン封入ナノ粒子濃度6mg/ml、ピタバスタチンとして0.33mg/ml)筋肉注射した。筋肉注射28日後(虚血作成35日後)に血管造影を行った。5Frのカテーテルを右内頸動脈より挿入し、透視下に左内腸骨動脈に留置した。ミリスロール0.25mgをカテーテルより動脈内注入した後、造影剤を1ml/秒の速度で5秒間注入し血管造影を行った。造影剤注入開始3秒後の静止画像を使い、血管数の評価を行った。上縁を大腿骨下縁、下縁を大腿四頭筋下縁、内縁を内腸骨動脈(本幹は計測しない。)、外縁は血管切断末梢部の範囲内で5mm方眼のグリッドを作成し血管の走行が認められた方眼数を全方眼数で割った値を血管造影スコア(angiographic score)とした。
非虚血マウス(コントロール)および虚血後14日目の無治療群とピタバスタチン封入ナノ粒子投与群より末梢血を採取した。末梢血0.5mlを溶血後、得られた白血球をFITC付抗Sca−1抗体1μgとPE付抗Flk−1抗体1μgとともに30分、4℃でインキュベーションし、フローサイトメトリーにて解析した。Sca−1およびFlk−1ともに陽性の細胞を血管内皮前駆細胞(EPC)として、末梢血白血球中の割合(%)を算出した。
結果:非虚血マウスにおける末梢血EPC0.51±0.09%に対し、虚血マウスでは無治療群1.19±0.05%、スタチン封入ナノ粒子群1.21±0.03%といずれも有意に増加していた。しかし、無治療群とスタチン封入ナノ粒子群ではほぼ同等であった(図21)。下肢虚血により骨髄から末梢血へのEPC動員が増加するものの、スタチン封入ナノ粒子によるさらなる増加はみられなかった。これにより、ナノ粒子によるスタチンの局所送達による血管新生作用は、骨髄を含む全身への作用というより、虚血局所での作用が主体であることが理解できる。
Claims (7)
- スタチンを生体適合性ナノ粒子の内部に封入してなるスタチン封入ナノ粒子を含む、創傷、壊疽、バージャー症候群又は虚血疾患の処置または予防に使用するための医薬組成物であって、前記生体適合性ナノ粒子がポリラクチドグリコライド共重合体またはそのポリエチレングリコール修飾体を含み、2.5〜1,000nmの個数平均粒子径を有する、医薬組成物。
- スタチンがプラバスタチン、シンバスタチン、フルバスタチン、アトルバスタチン、ピタバスタチンおよびロスバスタチンから選択される、請求項1に記載の医薬組成物。
- スタチンがピタバスタチンまたはアトルバスタチンである、請求項1に記載の医薬組成物。
- 1種以上のスタチンを封入した、請求項1〜3のいずれかに記載の医薬組成物。
- スタチンと共にその他の薬剤を封入した、請求項1〜4のいずれかに記載の医薬組成物。
- その他の薬剤が、新血管形成効果を有する薬剤、抗生物質、抗炎症剤、ビタミン、またはスタチンと併用することが望ましくない薬剤以外のものである、請求項5に記載の医薬組成物。
- 虚血疾患が脳血管虚血、腎虚血、肺虚血、肢虚血、虚血性心筋症、心筋虚血、筋虚血または脳虚血である、請求項1〜6のいずれかに記載の医薬組成物。
